Abstract:Pushed by international trends to develop intelligent transport systems, railways operators rely more and more on new information and communication technologies to optimize exploitation and maintenance costs but also to enhance the friendliness, comfort and security feeling of public transport by offering new services to passengers while traveling. There is not a lot of study dealing with this problem of high data rate services both in uplink and downlink directions in the case of high-speed trains. In our work we consider a double links architecture (terrestrial and satellite) with possible switch between the links versus availability. The paper focuses on satellite channel modelling. Several models have been developed to characterize the mobile satellite channel. Some of them are based on different states of signal reception. In this paper, the characterization with a 3D ray-tracing tool, of an existing mobile satellite communication channel model, the Karasawa model, based on a three states Markov chain, will be presented

Inequalities in the utilisation of maternal health Care in Rural India: Evidences from National Family Health Survey III & IV

Abstract:Abstract Background Since the implementation of National Rural Health Mission (NRHM) in 2005, Maternal Mortality Ratio has significantly declined in India through a noticeable improvement in maternal health care services. However, India did not succeed to achieve the target of millennium development goal to reduced maternal mortality ratio by 2015. Also, there is substantial inequality exist at the regional, geographic, economic, and social level, and various socioeconomic factors contribute to the significantly large share in inequality in utilisation of maternal health care in India. Methods Using data from the National Family Health Survey (2005 and 2015), this study examined the degree of inequality exist in maternal health care namely full antenatal care (full ANC), skilled attendants at birth (SBA), and postnatal care (PNC) in rural India. Descriptive statistics, concentration index (CI), and Wagstaff decomposition method have been performed to understand the pattern of maternal health care utilisation, and to explain the extent of inequality in maternal health care utilisation. Results The study revealed that a substantial gap across socioeconomic groups exist in utilisation of maternal health care has significantly reduced in rural India during 2005–16. The results found a noticeable improvement in maternal health care utilisation, especially in utilisation of skilled attendants at birth (SBA). During this decade, the concentration index for SBA showed a significant decline from 0.28 in 2005–06 to 0.09 in 2015–16, while that of full ANC declined from 0.47 to 0.32 over the same period, and reduction of inequality in full ANC was least. Further, the results of decomposition analysis suggested that secondary and higher education, mass media exposure, and scheduled tribe contributed a significant share to the inequality. Conclusion The exposure to mass media is the most significant contributor to inequality, and hence, there is a need for broad dissemination of awareness regarding maternal health care schemes in rural parts of country. Based on findings of study, it is suggested that health scheme related to maternal and child health care under NRHM be continued and focused for lower socioeconomic groups and marginalized mothers to reduce maternal health services inequality, particularly in the component of full ANC.

Abstract:Today android devices face many resource challenges like battery life, storage, bandwidth etc. cloud computing offers advantages to users by allowing them to use infrastructures, platforms and software by cloud providers elastically in an on-demand fashion at low cost. Android Cloud Computing (ACC) provides android users with data storage and processing services in cloud, obviating the need to have a powerful device configuration(e.g. CPU speed, Memory, Capacity etc) and all resource-intensive computing can be performed in cloud. Nowadays more and more commercial applications are shifting to android and these days security becomes a big issue. With increasing use of mobile android devices, the requirement of cloud computing in android arises. In this paper, a brief review for how ACC is emerging in real world and further important issues towards security are discussed. Here security mechanism based on onion routing is proposed, which will secure the data on Android Cloud Computing.
Non-utilization of public health care facilities: examining the reasons through a national study of women in India.

Abstract:INTRODUCTION This article examines women's opinions about their reasons for the non-utilization of appropriate public health care facilities, according to categories of their healthcare seeking in India. METHODS This cross-sectional article uses nationally representative samples from the Indian National Family Health Surveys NFHS-3 (2005-2006), which were generated from randomly selected households. Women of reproductive age (15-49 years) from the 29 states of India participated (n = 124 385 women). The respondents were asked why they did not utilize public health care facilities when members of their households were ill, identifying their reasons with a yes/no choice. The following five reasons were of primary interest: (1) 'there is no nearby facility'; (2) 'facility timing is not convenient'; (3) 'health personnel are often absent'; (4) 'waiting time is too long'; and (5) 'poor quality of care'. RESULTS Results from logistic regression analyses indicate that respondents' education, economic status and standard of living are significant predictors for non-utilization of public health care facilities. Women who sought the services of care delivery and health check-ups indicated that health personnel were absent. Service seekers for self and child's medical treatments indicated that there were no nearby health facilities, service times were inconvenient, there were long waiting times and poor quality health care. CONCLUSIONS This study concludes that improving public health care facilities with user-friendly opening times, the regular presence of staff, reduced waiting times and improved quality of care are necessary steps to reducing maternal mortality and poverty.

Abstract:Maternal health is a major public health concern in India. MMR of India has declined significantly but maternal health status has not much improved. The prevalence of anemia and low Body Mass Index (BMI) is more severe among the women belonging to the Scheduled Caste (SC) and Scheduled Tribe (ST) categories. In this paper, attempts have been made to examine the maternal health status and to identify the factors responsible for poor health status among SC and ST women. The study is purely based on secondary data taken from latest 3 rounds of NFHS (2005-6, 2015-16 & 2019-21). Multivariate analysis have been carried out using panel regression model to understand the impact of determinants on maternal healthcare. The study found that the SC and ST women are more prone to anemia than others (GENERAL category women). Maternal health status is mostly controlled by per capita health expenditure and health infrastructure variables like no. of hospitals and nurses, irrespective of social class. Thus poor maternal health status in India, especially among socially disadvantaged groups of society, have been major concern. The government should be more focused on existing policies related to maternal healthcare.
Education and screening for obesity, hypertension, and diabetes (including gestational diabetes) "at the doorstep" of women from nine underprivileged urban areas in Delhi National Capital Region

Abstract:Background and aims: Women in underprivileged urban areas have poor access to health and education, making them vulnerable to non-communicable diseases (NCDs), specifically diabetes. The current project has sought to focus on this group, to educate and screen this population to prevent and manage obesity and diabetes. Methods: The project was conducted in 9 underprivileged areas of Delhi NCR using a mobile van unit. Women from these areas were invited for health education (this included talks, lectures and distribution of Information, Education & Communication material), screening {anthropometric, blood glucose and blood pressure (BP) measurements} diet counselling sessions and referral of subjects with uncontrolled blood glucose. Results: Baseline survey on knowledge of diabetes and nutrition showed low awareness. Health education sessions (n, 46) included face-to-face discussion and problem solving and distribution of simple pictorial leaflets (n, 3000). The health education sessions were well accepted. In total, 4% women (n,3175/91000- total population, 3056 non-pregnant, 119 pregnant) participated in the research trial. Among non-pregnant women, an average weight gain of 11.8 kg between the ages of 20-40 years was observed. The average BMI and waist circumference (WC) was 26.8 ± 5.3 kg/m2 and 91.1 ± 13.2 cm, respectively in non-pregnant women. Further, 75.3.0% (2207/2928) and 96.7% (2875/2971) of the non-pregnant women had overweight/obesity and abdominal obesity, respectively. In the non-pregnant women, 21.7% had known diabetes. Further, 7.4% non-pregnant women and 2.5% pregnant women were identified as having hyperglycaemic state. Hypertension was observed in 11.9% pregnant women and 49.9% of non-pregnant women. Conclusion: Extremely high prevalence of obesity, along with diabetes and hypertension in underprivileged urban women requires intensive individualised and group health education, screening, and counselling "at the doorsteps", as has been shown in our model.

Abstract:IntroductionMaternal delivery at home without skilled care at birth is a major public health issue. The current study aimed to assess the various contributing and eliminating factors of maternal delivery at home in India. The reasons for not delivering at healthcare facilities were also explored.MethodsThe study used the National Family Health Surveys (NFHS)-4 (2015–2016) data from states and union territories of India for analysis. A national representative sample of 699,686 women of reproductive age group (15–49 years) was used. Cross-tabulation and multivariate logistic regression analyses were performed.ResultsThe prevalence of home delivery in India was 22%, among which 34% of women believed that institutional delivery was not a necessity. Financial constraints, lack of proper transportation facilities, non-accessibility of healthcare institutions and not getting permission from family members were the main reasons cited by the women for delivering at home. The proportion of home deliveries was much higher among women from more disadvantaged socioeconomic areas than women from less disadvantaged socioeconomic areas. Domestic violence and partner control were essential factors contributing to the prevalence of home delivery. However, the women who owned mobile phones and used a short message service (SMS) facility delivered at home less often.ConclusionPolicymakers should focus more on the women living in disadvantaged socioeconomic areas and other marginalised populations with less education and low economic levels to provide them with optimum delivery care utilisation. Strengthening of public healthcare facilities and more effective use of skilled birth attendents and their networking are essential steps. Electronic and economic empowerment of women should be emphasised to bring about a significant reduction in the proportion of home deliveries in India.

Abstract:Background: Maternal health was one of the most important millennium development goals (MDGs), India didn’t achieve by the year 2015. Since, India is a multicultural, social and multiregional country, where some of the regions have good social and demographic achievement while some are poor. Haryana is one of them, which has 146 maternal mortality ratio. The level of receiving antenatal care (ANC) in Haryana is quite low as compared to other states of India. Objective of present study was to Understand the extent of use of maternal health care services in Haryana as well as examining the role of antenatal care and other socio-economic factors on the utilization of maternal health services.Methods: Bivariate analysis, chi-square test, and binary logistic regression have been used based on district level household and facility survey-4 data.Results: The utilization of ANC (any and full), institutional delivery and post-delivery treatment seeking varies among women by literacy, age at first marriage, age at women and place of residence. Literate women are two times more likely to access ANC [odds ratio (OR)=1.97 (any ANC), 1.95 (full ANC), p<0.01] and 1.52 times more likely to prefer institutional delivery [OR=1.52, p<0.01].Conclusions: Empowering women through the encouragement of mother’s education should be one of the most fundamental strategies to promote maternal health care services and reduce inequalities.

Abstract:Background: Antenatal care (ANC) and postnatal care (PNC) care services intended to secure and improve maternal health are utilized only to a limited extent. Therefore, this study was conducted to assess various barriers and association of various sociodemographic factors like age, education, economic status and religion with the utilization of services. This revealed the reasons which avert the ladies from utilising the services adequately.Methods: 150 women residents of Uruli Kanchan, a village in Pune district of Maharashtra, having at least 1 child below the age of five years were selected by simple random sampling technique. The data was collected by house-to-house survey and interview technique.Results: In the present study, better utilization of ANC services was observed (99.234%) as compared to PNC services (65.33%). Education, good socioeconomic conditions and religion were the factors which greatly influenced the utilization of PNC services. Lack of desire and awareness of PNC visits (in 55% women) was an important factor for underutilization of PNC services. Lack of trust towards government services (by 34.02%) women was observed.70% of women showed satisfaction towards the maternal and child health care (MCH) services provided by the government.Conclusions: Women showed high level of utilization of ANC services as compared to PNC services which still need a lot of attention and awareness. Women show lack of trust in the government facility.
Maternal’ Awareness of Antenatal Care on Impact of Mothers’ and Newborn Health in Bangladesh

Abstract:Background: Globally, maternal and infant morbidity and mortality is a serious public health problem. Maternal mortality rate in Bangladesh is 176 per lack live births which remain high as Government set target to be achieved by 2016. Antenatal care is extremely important health care service for the mothers’ and unborn fetus during pregnancy. It is well recognized that good antenatal care improves maternal, perinatal and neonatal outcomes. Objective: To identify the maternal awareness of antenatal care on impact of mothers’ and newborn health in Bangladesh. Methods: The study was conducted by using descriptive and cross-sectional design. This study was approved by the appropriate authority and informed consent forms were obtained from the participants. Two hundred forty-one of rural and two hundred one urban mothers completed three sets of questionnaires: 1) Socio-demographic, 2) Maternal Awareness and 3) Impact of Awareness. Content validity was done by the experts and internal consistency of these instruments was satisfactory with Cronbach’s alpha over 0.74. Result: The average awareness scores of rural and urban mothers were 18.54 ± 2.77 and 19.69 ± 2.16 respectively. The mean scores of impact on rural and urban mothers were 9.41 ± 2.13 and 9.39 ± 2.25 respectively. Maternal’ awareness of antenatal care; impact of mothers’ and newborn health were found statistically significant (t = 68.54, p < 0.001) and (t = 59.11, p < 0.001) different among rural and urban mother’s. It was observed that maternal’ awareness of antenatal care and impact of mothers’ and newborn health were significantly difference between rural and urban mothers’ (x2 = 211.869, p 0.004 and (x2 = 157.772, p 0.002 respectively). Conclusions: The findings indicated that maternal’ awareness of antenatal care on impact of mothers’ and newborn health was statistically significant different between rural and urban mothers’. Further intervention study is needed to evaluate the effect of intervention on maternal and child health outcomes that represent the whole population.

The nexus between poverty and maternal healthcare utilization with a focus on antenatal care visits and choice of place of birth in Somaliland

Abstract:Introduction: Poverty poses a significant barrier to accessing healthcare globally, particularly in relation to antenatal care (ANC) visits and the use of childbirth facilities, both of which are crucial for women's health and fetal well-being. In Somaliland, only 47% of pregnant women attend healthcare facilities for ANC, with a mere 33% receiving care from skilled birth attendants. Despite this, no previous studies have examined the relationship between poverty and maternal healthcare utilization in Somaliland. This study aims to investigate the effect of poverty on maternal healthcare utilization with focus on ANC visits and the choice of place of birth in Somaliland. Method: Utilizing data from the 2020 Somaliland Demographic Health Survey, a cross-sectional study design was employed, analyzing a nationally representative sample. The sample size used in this study was 3,183 women of reproductive age. Descriptive analysis, negative binomial regression, and multinomial logistic regression were conducted using Stata version 18.0. Diagnostic tests, including Chi-square - 2log likelihood statistic, Akaike Information Criterion, and Bayesian Information Criterion, were employed to evaluate model fit. Results: Poverty, as indicated by wealth quintile, was associated with reduced ANC visits (adjusted incidence rate ratio (aIRR) = 0.884, 95% CI: 0.791-0.987) among women in poorer households compared to those in richer households. Women in Togdheer, Sool, and Sanaag regions had lower ANC visit rates compared to Maroodi Jeex region (aIRR = 0.803, 95% CI: 0.687-0.939; aIRR = 0.710, 95% CI: 0.601-0.839; aIRR = 0.654, 95% CI: 0.558-0.768, respectively). Women from poorer households had lower probabilities of opting for public health facilities (adjusted relative risk ratio (aRRR) = 0.457, 95% CI: 0.352-0.593) and private health facilities (aRRR = 0.195, 95% CI: 0.111-0.341) over home births compared to women in richer households. Women in Togdheer, Sool, and Sanaag regions had lower probabilities of choosing public (aRRR range: 0.331-0.175) and private (aRRR range: 0.350-0.084) health facilities for delivery over home births compared to women in Maroodi Jeex region. Conclusion: Poverty significantly impedes maternal healthcare utilization, contributing to lower ANC attendance and preference for home births over public or private health facility births. Addressing these disparities requires initiatives to eliminate financial barriers, such as user fees, and enhance equitable access through community-based health insurance and improved healthcare infrastructure.

Abstract:The use of maternal health services in India is influenced by various socioeconomic factors, with higher maternal morbidity during reproductive ages 15 to 49. The Indian government has launched programs like Maternity Safety Scheme (Janani Suraksha Yojana) and Maternity and Child Protection Programme (Janani Shishu Suraksha Karyakaram) to provide free health services to women. However, the burden of these services is disproportionately high among the poorest women, leading to high mother and newborn death rates. In the study, secondary data from the National Family Health Survey (NFHS-3) found that place of residence, exposure to mass media, education, caste, religion, and wealth were all significant predictors of maternal health care utilization. The study also found that location of residence; age, birth order, education, caste, religion, and wealth were substantially and significantly associated with the use of antenatal care, skilled attendants at birth, and postnatal care in India. Women aged 25-34 were 54.8% more likely to receive antenatal care (ANC) services than those aged 15-24. Women aged 35-49 experienced a 52.7% increase in ANC services. Rural women were 23.8% less likely to use ANC than urban women. Muslims were 43.8% less likely to seek ANC assistance compared to Hindus. Women with the highest education levels were 5 times more likely to seek ANC than those who were not educated. Schedule Tribes were 1.5 times more likely to participate in ANC than Schedule Castes. Women aged 35-49 had an 86.8% higher likelihood of seeking postnatal care than those aged 15-24 years. Media exposure had a 1.3 times higher chance of receiving this care than those who had none. Muslim women were 17.8% less likely to choose PNC services than Hindu women.
Impact of roles of women on health in India

Abstract:India's population has more than doubled since 1961. Although India has been a leader in developing health and population policies, there have been major implementation problems due to poverty, gender discrimination, and illiteracy. Yet, three-quarters of the food produced annually in India is because of women. In 1991, only 39.3% of Indian women were literate. The literacy level of women can affect reproductive behavior, use of contraceptives, health and upbringing of children, proper hygienic practises, access to jobs and the overall status of women in the society. Early marriage and childbirth was a major determinant of women's health and was also responsible for the prevailing socioeconomic underdevelopment in India. The overall maternal mortality for India is 572.3 per 100,000 births, ranging from 14.9% in Bihar to 1.3% in Kerala. Anemia is an indirect factor in 64.4% of the maternal deaths. Trained birth attendants currently assist in about 60-80% of all births in women at the time of delivery. Socioeconomic factors are responsible for maternal deaths to a large extent - money in 18.3%, transport in 13.7%. When the mother dies it doubles the chances of death of her surviving sons and quadruples that of her daughters. Among the avoidable factors in maternal deaths, lack of antenatal care is the most important. Women, if educated and aware, can improve the health of their children by simple measures like good hygiene, exercise and dietary habits. Because of poverty, many of the young children, especially girls living on streets are easy prey for criminal prostitution rings, drug trafficking and consequences of HIV infection, and severe emotional and mental disturbances. Women are responsible for 70-80% of all the healthcare provided in India. Female healthcare providers can play an important role in educating society to recognize their health and nutrition needs. Women professionals and empowerment of women at all levels are required for improvement of the health and nutrition structure in India.

Methods: The analysis was conducted on 5107 women who had at least one birth in the last five years preceding the survey. Data were obtained from the 2023 Senegal Demographic and Health Survey (DHS). ANC utilization, defined as receiving four or more ANC visits during pregnancy, was the outcome variable. Independent variables included age, region, education, wealth index, total children ever born, pregnancy intention, employment status, media exposure, births in last five years and women autonomy. Bivariate analyses and multiple logistic regression were conducted to identify the significant variables. Results: The final sample included 4,785 women after excluding those with missing data. Higher education level (Adjusted Odds Ratio (AOR) = 3.649), rich wealth index (AOR=1.581), low (AOR=1.792) and high (AOR=1.692) media exposure were significantly associated with higher ANC in urban residences. On the contrary, in rural residences, primary (AOR=1.60), secondary (AOR=2.01) and higher (AOR=5.79) education level, intended pregnancy (AOR=1.33), low (AOR=1.48) and high (OR=1.35) media exposure and women autonomy (AOR=1.29) were significantly associated with higher ANC. Conclusions: This study underscores the influence of different socio-economic and demographic factors on ANC utilization in urban and rural residences in Senegal. Policy interventions should prioritize enhancing education, economic opportunities, media awareness and women’s autonomy, particularly in rural areas, to improve ANC coverage.

Maternal healthcare seeking and determinants of adequate antenatal care and institutional childbirth among Indian tribes: A cross-sectional study from nine districts

Abstract:OBJECTIVE: To report the utilisation of maternal healthcare services and factors associated with adequate antenatal care and institutional childbirths among mothers in the tribal communities from nine districts in India.METHODS: Cross-sectional data were collected from 2636 tribal women who had a childbirth experience in the past 12 months. Socio-demographic, maternal healthcare services and health system-related details were collected. Multiple logistic regression analyses were done to identify factors associated with adequate antenatal care (receiving at least four antenatal care visits, the first visit being in the first trimester and receiving a minimum of 100 iron-folic acid tablets) and institutional childbirth (mother giving birth in a health facility).RESULTS: Only 23% of the mothers received adequate antenatal care. 82% were institutional childbirths. The logistic regression revealed that particularly vulnerable tribal groups (PVTGs), those lacking all-weather roads, and women of advanced age were at risk of inadequate antenatal care. Mother's education, health worker's home visits during pregnancy and reception of advice on antenatal care were significantly associated with the reception of adequate antenatal care. Having all-weather roads, and education of the mother and head of the household were positively associated with institutional childbirths, whereas PVTGs, children of birth order three or above, and working mothers were more likely to give childbirth at home.CONCLUSION: PVTGs are at risk of foregoing adequate antenatal care and are more likely to give childbirth at home. Having all-weather roads is a strong correlate of adequate maternal care. Outreach activities by the health workers are to be strengthened as they are positively and significantly associated with the reception of adequate antenatal care. Investing in education and other social determinants and addressing certain socio-cultural practices is important to improve maternal health.

Abstract:Numerous studies have identified factors that are associated with increased access to reproductive health services in lower-middle-income countries (LMICs). However, limited studies examined the influence of access to internet or a mobile phone, media exposure and domestic migration on reproductive health services use in LMICs like Bangladesh. This study investigated the role of such factors on the use of contraceptives, antenatal care (ANC) and postnatal care (PNC) by married adolescents and young women in Bangladesh and whether it was varied by area. Secondary data for 1665 married women aged 15-24 years, sourced from the 2019 Multiple Indicator Cluster Surveys, were included in both bivariate analyses and logistic regression modelling to examine the role of access to internet and/or mobile phone, media exposure and domestic migration on the outcome variables (contraceptive, ANC and PNC). All regression models were controlled for age, wealth, education and number of existing children. Among all participants, 69.8% were aged 20-24 years and 85.6% lived in rural areas. Of the total sample, 67.5% used contraceptives, 75.7% utilised ANC and 48.7% accessed PNC. Domestic migration significantly increased contraceptive use, with women who had moved locally within the last five years 1.84 times more likely to use contraception than those who had never moved (95% CI: 1.41-2.41, p<0.001). Women with internet or mobile phone access were more likely to receive ANC (aOR: 1.57, 95% CI: 1.22-2.00, p<0.001) compared to those without internet/mobile phone access. Media exposure was found to increase the likelihood of receiving ANC in urban areas. No significant influence was found on the use of PNC. Internet/mobile-based platforms are promising avenues for public health messaging regarding ANC in Bangladeshi married adolescents and young women. Further research is required into determinants of PNC service use in low-resource settings.
