Large Pedunculated Lipoma of the Esophagus: Report of a Case and Review of Literature

Abstract:Large pedunculated esophageal lipoma is uncommon. The presenting symptoms of esophageal lipoma are dysphagia, regurgitated mass, and persistent sensation of a lump in the throat. The most frequent location of the tumor pedicle is the upper esophageal sphincter. Here, we present the case of a 52-year-old man who had the symptoms of pharyngeal unwell and dysphagia. Panendoscopy showed a pedunculated tumor mass within the esophageal lumen with its peduncle arising from the cervical esophagus. Computed tomography (CT) and magnetic resonance imaging (MRI) revealed that he might have anesophageal submucosal or intraluminal and pedunculated tumor mass. The tumor mass measured 25 × 16 × 45 mm in size. Cervical approachvia the right neck was performed for confirmation. After removal of the intraluminal mass, the patient became symptom free. Pathology showed a lipoma arising from the submucosa of the esophagus.
Multiple Gastric Angiolipomas: A Case Report

Abstract:BACKGROUND Angiolipoma is a benign tumor and is generally found in subcutaneous tissues. Angiolipomas are rare in the gastrointestinal tract, including the stomach. Preoperative diagnosis of the tumor is difficult, although there are several radiological examinations such as computed tomography and endoscopic ultrasound. CASE SUMMARY We report a 24-year-old Chinese man with multiple gastric angiolipomas, with a positive stool occult blood examination. Endoscopic biopsy only showed nonspecific inflammation. Histological examination of the specimen by endoscopic snare resection showed that the tumor consisted of adipose tissues and blood vessels. We also performed a literature review. After the use of proton pump inhibitor, the fecal occult blood test was negative. Due to the difficulty of resecting multiple lesions in the stomach completely and the benign characteristics of angiolipoma, we chose to have regular upper gastrointestinal endoscopy evaluation of the lesion. No evidence of significant change in lesion size was detected after 3-years follow-up. CONCLUSION Gastric angiolipoma is rare, and benign neoplasm should be considered when lesions occur submucosally in the gastrointestinal tract.
A huge pancreatic lipoma mimicking a well-differentiated liposarcoma: A case report and systematic literature review

Abstract:BACKGROUND Pancreatic lipomas are thought to be very rare. Lipomas are usually easy to identify on imaging, particularly via computed tomography (CT). But sometimes it's quite difficult to distinguish a lipoma from a well-liposarcoma without histologic result. CASE SUMMARY Here, we present a case of pancreatic lipoma in a 59-year-old female. She was asymptomatic and had no medical history of note. CT and magnetic resonance imaging revealed a mass like well-differentiated liposarcoma in the pancreatic head, positron emission tomography/CT showed a low fluorodeoxyglucose uptake and laboratory tests revealed elevated transaminase and carbohydrate antigen-199 levels. Finally, the patient underwent a pancreaticoduodenectomy. Histologically, mature adipocytes were noted in the bulk of the tumor. Accordingly, the pathologic diagnosis of the pancreatic neoplasm was lipoma. To our knowledge, this case is the first example of a suspected well-differentiated liposarcoma that was actually a pancreatic lipoma. We also highlight the radiological features distinguishing a pancreatic lipoma from a pancreatic liposarcoma and briefly review the literature. CONCLUSION Pancreatic lipomas show no obvious gender bias and most commonly occur in the head of the pancreas, of which the maximum diameters are often less than 5 cm, and small, asymptomatic non-compressed lipomas require follow-up only. Surgical excision should be considered when the tumor has compressed important tissues or is difficult to distinguish from a liposarcoma, the choice of surgery depends on the intraoperative presentation.

Abstract:Lipoma is a benign soft tissue tumor composed of mature adipocytes. [ 1 ] It is common on the surface of the body and can also be seen inside the body. Most lipomas have a soft texture, good mobility, and are generally painless. [ 2 ] Colorectal lipoma is relatively rare in clinic. It is not easy to find in the early stage. When the tumor is large, abdominal pain, diarrhea and bloody stool may appear. Because of the low growth position, rectal or sigmoid colon lipoma may appear sagging and prolapse out of the anus. The tumor mucosa may have congestion and edema, focal erosion, or even ulceration leading to bloody stool. The clinical manifestations of this disease lack specificity, and some physicians lack understanding and experience, making it prone to misdiagnosis. [ 3 ] This article reports a case of large rectal lipoma based on existing literature materials, in order to provide reference for the diagnosis and treatment of colorectal lipoma in clinical practice.

Abstract:Objective To summarize our experiences in the diagnosis and treatment of giant colonic lipoma,and enhance awareness of the disease and avoid misdiagnosis.Methods We retrospectively analyzed the clinical data of 5 cases of giant colonic lipoma that were mistaken for colon cancer,and evaluate the possible reasons of misdiagnosis.Results Colonic lipoma is an uncommon benign tumor.Most cases with larger tumor size were misdiagnosed as colon cancer when they suffered from the symptoms of abdominal pain,bloody stool,abdominal mass,even bowel obstruction and intussusception.Barium enema and colonoscopy are important diagnostic means.Contrast enhanced CT scan can provide definitive topographical information identifying a colonic lipoma.Surgical resection was performed in all the 5 cases.Conclusion Giant colonic lipoma with intussusception can mimic a colonic carcinoma leading to misdiagnosis.

Abstract:Intermuscular lipomas are rare soft tissue tumors of mesenchymal origin. They are closely correlated with other benign and malign mesenchymal tumors in clinical presentation and imaging, causing problems in diagnosis and treatment. This study examined a rare giant intermuscular lipoma of the hip and its management, allowing clinicians to become familiar with this lesion. Furthermore, the clinical data of 13 patients with pathologically confirmed giant intermuscular lipomas (1 case treated by the authors, the remaining 12 cases reported in the Chinese literature during the period from January, 1992 to May, 2012) were also reviewed and analyzed. Based on the review, intermuscular lipomas were found to be more common in males and usually presented with an asymptomatic mass, with an average patient age of 39.7 years (range, 11-70), while symptom duration was between 1 month and 10 years. The most common tumor origin sites were the thigh and forearm and the option of treatment was surgical excision. The present study showed that it is crucial for the surgeon to consider infiltrating lipomas when dealing with deep soft tissue masses, while complete removal of the lesion was vital to avoid recurrence.
Giant Cervical Occipital Lipoma: A Case Report

Abstract:Lipomas are common, well-circumscribed neoplasms of mesodermal origin, characterized by being slow, painless growths that are mostly subcutaneous, not invasive, and not recurring after surgery. Lipomas are the most prevalent kind of mesenchymal tumor, yet giant lipomas are rare in the cervical region and the occipital area. We report a 46-year-old female with diabetes insipidus was referred with a giant occipital cervical tumor, which she had noticed for 17 years and which had rarely given her any complaints of compressive symptoms. The clinical assessment indicated a firm, painless, and mobile swelling, which demonstrated features of venous ectasia, and there was no external ulceration. Ultrasonography and MRI of the neck revealed a large, subcutaneous fatty tumor with distinctive echographic features on both modalities, including hyperintense signals on T1 and T2 and no ring enhancement after Gadolinium injection. Due to the mass being smooth, round, and not attached to any structure, the patient underwent surgical enucleation under general anesthesia, resulting in full recovery without complications. Pathology revealed a benign adipose tissue tumor without liposarcoma, and there were no difficulties observed during follow-up for two years.
Intestinal “Piggybacking Lipoma”, A Unique Lipoma Composed of Lipoma and Overlying Epithelial Lesions: A Case-control Study and Review of Literature

Abstract:BACKGROUND/AIM: Lipomas are rare but the most common benign mesenchymal lesions of the gastrointestinal (GI) tract, composed of mature adipose cells. The "piggybacking lipoma" is formed by lipomas with overlying polypoid epithelial lesions, such as sessile serrated lesion, tubular adenoma, or hyperplastic polyp, and the literature on these lesions is limited. In this study, we systematically investigated the clinical, endoscopic, and pathologic characteristics of these unique lipomas.PATIENTS AND METHODS: This is a single-institution retrospective study of gastrointestinal tract lipomas diagnosed from 2016-2021. Those with concurrent polypoid epithelial or mesenchymal lesions during the same endoscopic episode were included and reviewed in this study, and the lipomas were classified as "piggybacking lipoma" or "non-piggybacking lipoma" depending on whether the concurrent lesion was overlying the lipoma or was at a different location in the intestine. Demographic, clinical, and endoscopic data were obtained from electronic medical records.RESULTS: A total of 100 lipomas with concurrent epithelial or mesenchymal lesions were included in this study. Among them, 21 cases were classified as "piggybacking lipoma" and 79 were classified as "non-piggybacking lipoma". Patients with piggybacking lipomas showed a female predilection, and were more likely to be symptomatic and less likely to exhibit classic endoscopic features of lipoma. Histologically, the piggybacking polyps showed overlying sessile serrated lesions (SSL) (76.2%) and tubular adenoma (TA) (19%), whereas the non-piggybacking group had differing characteristic lesions with TA (57.5%) and SSL (6.0%).CONCLUSION: Piggybacking lipomas are rare lipomas with overlying polypoid epithelial lesions, most commonly SSL. They present different clinical, endoscopic, and pathologic features compared to non-piggybacking lipomas.

Abstract:To summarize the clinical features of the duodenal lipomas in order to explore effective diagnostic methods and appropriate treatment preoperatively.The clinical features, laboratory results, endoscopic appearance, radiological and pathological data of 8 cases of duodenal lipoma were retrospectively analyzed.Four patients suffered with repeated melena, 3 cases with epigastric discomfort, sour regurgitation or hiccup, while 1 patient without any symptoms. Liver functions, serum lipids and tumor markers were normal in all patients. Six patients had been detected lesions by gastroscopy (2 cases missed diagnosed in the first examination), these lesions were appeared in duodenal bulb (2 cases) or descendant duodenum (4 cases). Abdominal CT examination revealed partial duodenal wall thickening (6 cases), partial enteric cavity narrowing (4 cases), or low-density lesions in enteric cavity (3 cases) with CT value of -85 HU and evenly intensified when enhanced. EUS showed intensive hyperechoic lesions from submucosa, with homogeneous echo and clear margin. Biopsy under endoscopy in all patients showed chronic inflammation of mucosa, while the pathologic diagnosis was lipoma after surgical excision or endoscopic resection. Pancreaticoduodenectomy performed in 1 patient, duodenal tumorectomy in 3 patients and endoscopic resection with snare in 4 patients.Common site of duodenal lipoma is descending part, and the clinical manifestations are non-specific. Imaging and endoscopic examination are the mainly methods to detect the lesion, while EUS is significantly valuable in diagnosing and differential diagnosing. It can be treated by partial tumorectomy or endoscopic trap resection.
Lipoleiomyosarcoma Presenting as Massive Retroperitoneal Mass: An Unusual Soft Tissue Tumor of Abdomen and its Review of Literature

Abstract:Lipoleiomyosarcoma is a malignant tumor of both mesenchymal and smooth muscle origin. Lipoleiomyosarcoma is a very rare type of retroperitoneal sarcoma, which by itself is a rare type of soft tissue sarcoma. Lipoleiomyosarcoma is a very rare subtype of liposarcoma, which is the most common type of retroperitoneal sarcoma. To our best review of available literature, this was the first case of lipoleiomyosarcoma presenting as a massive retroperitoneal mass of size 50 × 50 × 30 cm, managed with multi-visceral R0 resection. The computed tomography of the abdomen showed a massive fat density lesion with internal soft density lesions and internal calcifications. Multivisceral resection was planned and executed and achieved R0 resection. We suggest that lipoleiomyosarcoma should be included in differential diagnosis of retroperitoneal tumors, and multi-visceral R0 resection should be considered as an option for lipoleiomyosarcoma, as these tumors have very good prognosis with R0 resection.

Abstract:We present primers and amplification conditions for 15 microsatellite loci developed for the Cope's giant salamander (Dicamptodon copei), 14 of which are tetranucleotide repeats. Cross‐species amplification revealed 10 of these loci to also be polymorphic in the Pacific giant salamander (Dicamptodon tenebrosus). Several loci produced nonoverlapping allelic ranges between the two species and may be useful in species identification. These polymorphic microsatellite loci are potentially useful for future studies of population genetics in dicamptodontid salamanders.

Abstract:A lipoma is a tumor of adipose tissue cells that can develop anywhere in the body, usually at the subcutaneous level without invading adjacent structures. Its most common location is on the back and extremities. According to the literature, it is considered common in both sexes; however, it is mentioned that it occurs more frequently in females. Clinically, it presents as a slow-growing, painless mass. Although the diagnosis is clinical, an imaging study is usually performed for confirmation and to obtain information about its location, relationship with adjacent structures, and surgical planning. The definitive treatment involves surgical resection with histopathological analysis, providing a definitive diagnosis to rule out malignancy. In the present case report, we will present a 45-year-old female patient, with no significant medical history, who comes for evaluation due to a tumor in her right shoulder approximately 5 cm in size with gradual growth to 20 cm over 12 years, seeking evaluation due to clinical manifestations. A surgical protocol was initiated, with magnetic resonance imaging (MRI) considered the imaging study of choice according to the literature, where a superficial location was observed. A surgical plan was made that included complete resection and histopathological analysis, with post-surgical evaluations at two, four, and six months post-surgery showing no signs of recurrence, remission of symptoms of nerve compression, and appropriate wound healing.

Abstract:Rectosigmoid intussusception is a rare cause of bowel obstruction, accounting for only approximately 1%-2% of all bowel obstruction cases. While intussusception in adults typically occurs intra-abdominally and presents with signs and symptoms of intestinal obstruction, in rare cases, it can mimic a rectal prolapse if the intussusceptum protrudes through the anal canal. We herein report a case where an octogenarian woman presented with rectosigmoid intussusception through the anal canal, due to a sigmoid colon submucosal lipoma, who eventually required an open Hartmann's procedure. Patients with rectal prolapse symptoms should be carefully examined to rule out intussuscepting masses as a differential, as it would necessitate earlier surgical intervention.

Abstract:Giant pedunculated hepatocellular adenomas are extremely rare tumors and often detected incidentally on cross-sectional imaging studies. We report the case of a 34-year-old woman who underwent cross-sectional imaging for staging evaluation of a uterine tumor. A large left subdiaphragmatic mass, without clear connection to the liver, was seen prompting diagnostic laparoscopy; during which a large pedunculated mass attached to the left lobe of the liver was found and resected. This case report highlights the challenges and pitfalls in the imaging diagnosis of pedunculated hepatocellular adenomas, such as difficulty in characterizing the mass or inability to identify the vascular attachment to the liver. Image-guided biopsy and diagnostic laparoscopy are valuable tools to establish diagnosis; most of these lesions are amenable to laparoscopic resection.
A Giant Childhood Mesenteric Lipoblastoma With Extensive Maturation

Abstract:Abdominal lipoblastomas are uncommon soft tissue tumors in children and rarely arise from the mesentery. Due to intraabdominal location and slow growth, these masses can go unnoticed for long periods of time and often found on surgical exploration. We present a case of a 12-year-old male with years of abdominal distension accompanied by new onset early satiety that was found to have an intra-abdominal mass. He underwent an exploratory laparotomy revealing a large 33 x 27 x 15 cm rubbery mesenteric mass displacing the entire intra-abdominal contents, connected by a single vascular pedicle and encasing a loop of small intestine. The mass was resected and the patient did well without signs of recurrence. Histology confirmed the presence of mature adipocytes but on further cytogenetic analysis, a translocation between chromosomes 2 and 8 at the 12q arm was detected, which is often associated with lipoblastomas. This case represents the one of the largest mesenteric lipoblastomas that matured extensively to lipoma-like histology at the time of surgical resection.

Abstract:Bauer first described colonic lipomas (CLs) in 1757. [ 1 ] The prevalence of this lesion ranges from 0.2% to 4.4%, representing 1.8% of all benign colonic lesions, [ 2 ] after hyperplastic and adenomatous polyps. Many authors have described a slight predominance of this lesion in females in their 5th or 6th decade of life, [ 3 ] primarily localized to the right colon. [ 4 ] The lesion is usually segmental, although there have been cases of diffuse lesions reported in the literature, which are typically small and asymptomatic. [ 5 ] Therefore, in most cases, they are discovered incidentally. Multiple lipomas have been shown in 10% to 20% of patients, notably if a lipoma was found in the cecum. The challenge for every physician is to differentiate this lesion from malignant colonic lesions, at the outset. Various diagnostic methods are available, including ultrasound, computed tomography (CT), magnetic resonance imaging (MRI), and endoscopic ultrasonography (EUS). In this review, we provide the necessary elements to preoperatively suspect this lesion. Both surgical and endoscopic resections (ERs) have good outcomes with no known recurrence after complete removal. Endoscopy is an effective diagnostic and therapeutic tool for treating small lesions. Surgery is the only method available for large lesions or those involving the muscular or serosal layers. Colotomy excision or segmental colon resection is indicated when the diagnosis is definite. In contrast, segmental resection, hemicolectomy, or subtotal colectomy are recommended when the diagnosis is questionable or when a complication occurs. Indeed, only a histopathological analysis of the tumor can provide a definitive diagnosis. However, this lesion sometimes presents as an acute abdominal condition owing to the occurrence of intussusception with intestinal obstruction. In such cases, time is limited and not all means are available to direct the diagnosis towards the suspicion of a lipoma; therefore, in these cases, it is advisable to consider this tumor as malignant.

Abstract:Giant mesenteric lipoblastoma is a rare benign tumor arising from the adipocytes. It can mimic malignant tumors, and its diagnosis is difficult before surgery. Imaging studies could lead the diagnosis but not confirm it. Those tumors arising in the abdomen are usually larger and can cause symptoms of compression. Surgical excision is the treatment of choice, and a long-term follow-up is necessary to detect local recurrences. Only a few cases of lipoblastomas arising from the mesentery are reported in literature. We present a case of a rare giant lipoblastoma arising from the mesentery of a 6-year-old girl, with a history of postprandial abdominal pain.
Pedunculated colonic lipoma causing adult colo-colic intussusception: A case report and literature review

Abstract:Introduction: Colo-colonic intussusception (CI) in adults is rare, usually caused by malignant conditions. Nonmalignant tumors, like colonic lipomas (CLs), can also be an underlying cause. Case report: We report an unusual case of a 62-year-old man admitted to the emergency department with acute abdominal symptoms. The CT scan confirmed the colonic obstruction, causing significant distention in the transverse and right colon. It also revealed an intraluminal pedunculated colonic mass with fatty density. Peroperatively, a descending colon intussusception was noted. We performed a left colon resection with a double colostomy on the left flank. The postoperative follow-up was uneventful. Pathologic examination of the surgical specimen revealed two lipomas. One of them was pedunculated and protruded into the colonic lumen causing the intussusception. Discussion: We conducted a literature review of adult CLs complicated by CI, covering the period from January 1900 to June 2024, including 203 cases. We excluded lipomas exclusive to the small intestine and ileocecal valvula. Our analysis focused on the clinical and pathological characteristics of these cases, as well as the available management options. Conclusion: Colonic intussusception due to lipomas are uncommon with a challenging preoperative diagnosis despite the evolution of imaging procedures. We aimed by our case to highlight such pathology and to study its features and the possibilities of its management.
Atypical endoscopic finding of colocolic intussusception: case report and literature review

Abstract:Background: Adult-onset colonic intussusception is a rarely encountered condition that leads to large intestinal obstruction with time. Patients often present with a variety of symptoms that are non-specific making it challenging to arrive at a definitive diagnosis. This is worrying as diagnostic delay could lead to a significant increase in morbidity and mortality. We wish to present and describe a case of an atypical endoscopic finding of colocolic intussusception secondary to ascending colon cancer. Case description: Sixty-seven-year-old lady was referred for 1 month's duration of passing melenic stools with mucus followed by a week's complain of hematochezia. Clinical examination and other relevant blood results were unremarkable except for iron deficiency anemia. Initial colonoscopy revealed a large mass within the splenic flexure with inconclusive biopsies. A more detailed colonoscopy repeated the following day revealed a massive, black-to-yellowish lesion within the splenic flexure with no viable mucosa seen. Colonic bezoar was initially suspected, however various endoscopic retrieval methods proved futile. Switching to a slimmer diagnostic gastroscope, the colon was carefully negotiated until a large ulcer was found within the ascending colon, adjacent to the mass' origin. An emergency CT abdomen and subsequently extended right hemicolectomy performed revealed a colocolic intussusception with sealed perforation secondary to an ascending colonic mass acting as an intussusceptum. Histopathology evaluation confirmed an ascending colon adenocarcinoma (pT2N0M0) amidst a background of extensive ischemic changes. Conclusions: Endoscopic descriptions of colonic intussusception are unusual given their rarity. Furthermore, these lesions can mimic a colonic bezoar as a result of fecal accretion and this can ultimately lead to false diagnostic and therapeutic decisions. In such instances, clarification with a CT scan before management decision can potentially avert unnecessary endoscopic intervention and complications.
