Abstract:Complete mesocolic excision with central vascular ligation is considered to contribute to superior oncological outcomes after colon cancer surgery [1]. For advanced right-sided colon cancer, this surgery sometimes requires lymph node (LN) dissection along the superior mesenteric vein (SMV), with division of the middle colic vessels, or their right branches, at origin [2]. Here, we present cranially approached radical LN dissection along the surgical trunk during laparoscopic right hemicolectomy. The omental bursa is first opened wide, and the gastrocolic trunk of Henle is exposed, using the right gastroepiploic vessels and the accessory right colic vein (ARCV) as landmarks. After division of ARCV, SMV and middle colic vein (MCV) are identified. After dividing MCV at its root, LN dissection along SMV is conducted in a cranial-to-caudal manner. Concurrently, the middle colic artery, or its right branch, is exposed and divided at origin. The transverse colon is then raised ventrally, and LN dissection along SMV using a cranial-to-caudal approach is again performed. The ileocolic and right colic vessels are divided at origin. The ascending and transverse mesocolon, including the pedicles, are then separated from the retroperitoneal tissues, pancreatic head, and duodenum, using a medial approach. The key characteristics in this procedure consist of easy access to pancreas, early division of ARCV and middle colic vessels at origin, and easy dissection along SMV. We performed a laparoscopic colectomy using this approach for 18 patients with right-sided colon cancer. The mean operative time and blood loss were 288 min and 83 ml, respectively. The mean number of harvested LNs was 24. There were 6 cases with positive LN metastasis. There were no recurrent cases at a median follow-up period of 24 months. We consider this approach to be safe and useful for radical LN dissection along SMV for right-sided colon cancers.

Abstract:Complete mesocolic excision (CME) has the potential to become the standard procedure for colon cancer surgery as a result of its effectiveness in reducing local recurrence rate and improving prognosis. There are two strategies to perform CME: medial and lateral. Researchers have demonstrated that laparoscopic CME via medial access is comparable to a traditional open surgery. Technically, we proposed two approaches for medial access: hybrid medial approach (HMA) and completely medial approach (CMA). HMA involves the entrance to the intermesenteric space (IMS) by an incision of the gastrocolonic ligament followed by the dissection of the middle colic vessels and the Henle trunk in a top-to-bottom fashion. The approach is capped by the dissection of the inferior edge of the pancreas, requiring the blending of both top-to-bottom and bottom-to-top approaches. CMA, on the other hand, involves a bottom-to-top approach in every step, including the entrance of IMS through transverse retrocolic space (TRCS); dissection of the middle colic vessels and the Henle trunk; and dissection of the inferior edge of the pancreas. According to the previous research of our medical center, laparoscopic CME via the total medial approach is safe and technically feasible after the precise identification of the surgical planes and spaces for the right hemicolon. Meanwhile, this procedure reduces operation time and has fewer vessel-related complications, which is a better choice for right hemicolectomy.
Clinical Analysis of Laparoscopic Resection of Colorectal Cancer

Abstract:Objective To study the feasibility of laparoscopic resection of colorectal cancer.Methods A(retrospectively) analysis of the clinical data of 110 patients with colorectal cancer treated by laparoscopic(resection) during last three years was made.Of them,24 cases were converted to open operation.In 86 (cases),laparoscopic resection of colorectal cancer was completed.Among them,5 cases underwent right(hemicolectomy),2 cases left hemicolectomy,10 cases sigmoidectomy,22 cases Dixon′s operation,46 cases of Miles operation,and 1 case total colorectomy.Results No patient died within 30 days postoperatively in this series.The average operation time was 225(120-360) min with average 135(20-400) mL blood loss.The average number of lymph nodes excised was 8.7(1-30).The average number of positive lymph nodes was 2.2(0-24).Six cases had intraoperative complications,including 1 case of ureteral injury and 5 cases of intra-abdominal bleeding.These patients were immediately converted to open surgery.There were 6 cases with postoperative complications,among them,2 cases of urinary leakage,2 cases of massive bleeding and 2 cases of intestinal obstruction.All of them recovered after reoperation.The time of bowel funtion(recovering) was 12-72h after operation.The hospital stay after operation was 8.6(7-15)days.The median follow-up was 14.3 months(range 1-33) for 100(90.9%) patients.There was no port-site tumor(metastasis) and no tumor recurrence at the small abdominal incision.Six to fifteen months after opteration,3 cases had diffase peritoneal metastases.1 case of Miles resection had perineal metastasis 3 months after(surgery).Conclusions Laparoscopic resection of colorectal cancer is technically feasible,and have(advantages),such as less surgical trauma,less bleeding,less gastrointestinal interference and quicker(recovery).Laparoscopic radical operation for colorectal cancer can meet the requirements of safety and radical operation.
Laparoscopic right hemicolectomy via root of small mesentery approach for colon cancer: An analysis of 20 cases

Abstract:Objective To evaluate the safety and curative effects of laparoscopic-assisted right hemicolectomy for right colon cancer via the approach of small mesentery root.Methods Technique of laparoscopic right hemicolectomy was performed in 20 patients with right colon cancer in our hospital from January 2014 to January 2016.Of the 20 cases,there were 15 males and 5 females with age ranging from 32-70 years (average age:52 years).Results The operations to 20 cases were completely finished without conversion to open surgery with a median operating time of 60 min (range:55-70 min).The total number of resected lymph nodes was 12-25 (mean number of 18),and the volume of intraoperative blood loss was 15-50 ml (mean:30 ml).The auxiliary incision was 5 cm long on average,and the median hospital stay was 7 days.No complications were found in this series.The classification in TNM stage after the operation included 5 cases of Ⅱ A,10 cases of Ⅱ B,and 5 cases of Ⅲ A stage.The postoperative pathological results were 6 cases with poorly differentiated adenocarcinoma,12 cases with medium differentiated adenocarcinoma and 2 with mucinous adenocarcinoma.Conclusion The combination of open and laparoscopic surgery via the approach of the small mesentery root makes laparoscopic right hemicolectomy more easily to be mastered.
Up-regulation of human cervical cancer proto-oncogene contributes to hepatitis B virus-induced malignant transformation of hepatocyte by down-regulating E-cadherin

Abstract:Hepatocellular carcinoma (HCC) is one of the most fatal human malignancies, Human cervical cancer proto-oncogene (HCCR) aberrantly expressed in a number of malignant tumors, including HCC. HCC is associated with Hepatitis B virus (HBV) infection in a large percentage of cases. To explore the regulation and function of HCCR expression in the development of HCC, we detected HCCR expression in HBV expressing hepatocytes. Results showed that the expression of HCCR was higher in HBV-expressing hepatocytes than that in control cells. Examining different components of HBV revealed that the HBx promotes HCCR expression in hepatocytes via the T-cell factor (TCF)/β-catenin pathway. HCCR expression in HBx transgenic mice increased with as the mice aged and developed tumors. We also found that overexpression of HCCR in hepatocytes promoted cell proliferation, migration, and invasion and reduced cell adhesion. Suppressing HCCR expression abolished the effect of HBx-induced hepatocyte growth. In addition, HCCR represses the expression of E-cadherin by inhibition its promoter activity, which might correlate with the effects of HCCR in hepatocytes. Taken together, these results demonstrate that HBx-HCCR-E-cadherin regulation pathway might play an important role in HBV-induced hepatocarcinogenesis. They also imply that HCCR is a potential risk marker for HCC and/or a potential therapeutic target.

Abstract:Abstract Objective The objective of this study is to examine the safety and feasibility of laparoscopic total colonic mesenteric resection for radical right hemicolectomy, utilizing a caudal-to-dorsal priority access approach in combination with a cephalic approach.Methods This study aims to conduct a retrospective analysis of the clinical data of 56 patients diagnosed with right hemi-colon cancer, who underwent radical right hemi-colon resection through laparoscopic total colonic mesenteric resection with caudal-to-dorsal priority, combined with cephalic approach, between January 2021 and June 2022.Results All 56 cases were completed without any instances of open surgery. The average operative time was (153.8 ± 42.5) minutes, with an average intraoperative bleeding of (53.3 ± 21.3) mL. The average time to first gas after surgery was (58.3 ± 13.6) hours, while the average time to return to a liquid diet was (68.5 ± 19.6) hours. The average postoperative hospital stay was (6.1 ± 3.0) days. Following surgery, two cases of lymphatic fistula and one case of anastomotic bleeding were reported. Following conservative treatment, all cases were cured, and no perioperative deaths were reported. Post-operative pathological results indicated that all cases were diagnosed with adenocarcinoma, with an average of 20.1 ± 5.9 lymph nodes cleared. Tumor staging revealed 8 cases in stage I, 19 cases in stage IIA, 11 cases in stage IIB, 12 cases in stage IIIA, and 6 cases in stage IIIB. During the 12-month postoperative follow-up, no instances of recurrence, metastasis, or death were observed.Conclusions The utilization of a caudal-to-dorsal priority in conjunction with a cephalic approach during laparoscopic radical resection of right hemicolectomy is a secure and viable method that aligns with the fundamental principles of radical oncology. This technique can aid in reducing the learning curve for surgeons performing laparoscopic right hemicolectomy and in guaranteeing surgical safety.

Abstract:A 67-year-old man who presented with a bloody stool was diagnosed with ascending colon cancer. He had previously experienced thoracic and abdominal aortic dissections, which were treated with thoracic and abdominal aortic grafts and superior mesenteric artery revascularization. We performed a laparoscopic right hemicolectomy with a D3 lymph node dissection. During the laparotomy, we identified the superior mesenteric artery and an enlarged anterior superior pancreaticoduodenal artery. Injury to the latter artery could lead to severe ischemia in multiple organs; therefore, it was crucial to identify the primary feeding artery and vascular anatomy before and during surgery. We chose the laparoscopic right hemicolectomy to avoid injuring the anterior superior pancreaticoduodenal artery and the intra-abdominal abscess. This case study was the first to describe a laparoscopic hemicolectomy after thoracic and abdominal aortic grafts and superior mesenteric artery revascularization.

Abstract:Since the early1990s, laparoscopic right colon resections have been the most performed advanced laparoscopic procedures just after laparoscopic left colectomies and sigmoid resections. Indications for laparoscopic right colectomies are either benign or malignant diseases. Despite its many indications, a laparoscopic right or extended right colectomy is mostly performed for cancer of the caecum, the ascending colon, the hepatic flexure or the proximal transverse colon. Worldwide, colorectal cancer is the third most diagnosed cancer: an estimated 1,880,725 people were diagnosed with colorectal cancer in 2020, out of which 1,148,515 were colon cancer cases and 40% were located in the right colon. These figures make an oncologic sound surgery for right colon cancer of the utmost relevance. More recently, complete mesocolic excision has been advocated as the optimal choice in term of radicality, especially in node-positive patients with right colon cancer. Laparoscopic standard right colectomy and extended right colectomy with or without CME should be performed according to defined principles based on a close knowledge of key anatomical landmarks. This knowledge will allow to trace anatomical structures and drive instruments along the correct surgical planes and has its foundations in teachings from surgeons and scientists of past and present time.

Abstract:Complete mesocolic excision (CME) is a high-quality colon radical surgery, which follows fine surgical dissection and precise surgical concept. Laparoscopic CME combines the above advantages theoretically, while it is still in the early stages of application for the higher quality requirements and longer learning curve of CME. The current literatures suggested that laparoscopic CME could be achieved the same effect with the open for colon cancer. However in order to maximize patient benefits, laparoscopic CME needs more strict indications. Compared with the left colon, transverse colon, right colon resection specimens are of higher quality. So the right colon cancer is more suitable for laparoscopic CME. This article will describe the procedure of laparoscopic CME of the right colon step by step.

Abstract:BackgroundWith the introduction of complete mesocolic excision (CME) and the application of laparoscopic technique, surgery for colon cancer has become more standardized and the curative effect has improved [1]. The key points in laparoscopic right hemicolectomy are high ligation of main vessels and root dissection of lymph nodes. The wide range of variations in vascular architecture and intraoperative bleeding are common causes of prolonged surgical time, wound hemorrhage, and even transfer to the opening operation.MethodsThe superior mesenteric vein (SMV) is the most important anatomical landmark in CME for the right colon, and guides all the steps of lymph node dissection. The SMV appears as a pale blue bulge on laparoscopy, which enables accurate positioning. The ileocolic vessel pedicle is relatively constant and facilitates accurate positioning. The intersection of the ileocolic vessel pedicle and the SMV is the optimal starting point in laparoscopic right hemicolectomy using a medial-to-lateral approach. A sheath with an avascular plane can be reached after opening the SMV vascular sheath, which results in less bleeding and enables vascular root and thorough lymph node dissection. The first step is to manage the ileocolic vessels. The ileocolic artery (ICA) is located anterior to the ileocolic vein (ICV) for about one-third of the incidence. The ileocolic vessels are relatively long and are easy to work with. In the vast majority of cases, the ICV drains into the SMV, and into the gastrocolic trunk (GCT) in about 2.5% of cases. The reported incidence of a right colic artery (RCA) is controversial; the RCA is absent in about 50% of cases and often crosses the SMV. The right colic vein (RCV) usually drains into the GCT, but sometimes drains directly into the SMV. The middle colic vessels have great variability and a close anatomical relationship with the pancreas, duodenum, and GCT. Moreover, the transverse colon and mesentery are long, and root positioning and processing of the middle colic vein (MCV) are relatively difficult. With the SMV and pancreas as anatomic landmarks, it is more feasible to locate the blood vessels in the neck of the pancreas. The middle colic artery (MCA) originates from the superior mesenteric artery (SMA), and the distance from the inferior border of the pancreas differs slightly in the literature, but is at the most 5 cm. Identification of the MCA trunk and branches, as well as the common origin of the MCA and RCA, is of great importance for the maintaining the blood supply during surgery for primary colon cancer. The MCV mainly drains into the SMV and GCT; however, if branching variation drains into the jejunal vein, inferior mesenteric vein, or splenic vein, the effect is serious when a vessel is torn. Isolation of the GCT is the step at which bleeding will likely occur in standard right resection and is a difficult stage of the surgery. The GCT has five origins including the right gastroepiploic vein (RGV), right colic vein (RCV), accessory right colic vein (ARCV), pancreaticduodenal vein (PDV), and MCV, which can have 2, 3, or 4 branches; therefore, familiarity with variants may be helpful to avoid bleeding. Approximately 5–10% of colon cancers at the hepatic flexure have No. 6 group lymph node metastasis, and laparoscopic radical extended right hemicolectomy requires thorough dissection of No. 6 group lymph nodes and the omental arcade 10 cm from the pylorus. The inferior arteriovenous vessels are a common source of bleeding, and the RGV can serve as a clue to finding the artery.ConclusionsThe core area of laparoscopic radical extended right hemicolectomy includes the pancreatic neck, duodenum, and right gastroepiploic vessels. The difficulty lies with the standard treatment of the GCT. A medial-to-lateral approach is more in line with the principle of no-touch in tumor surgery and is applied from lower to upper, inside to outside, and left to right, for both the vessels and plane of dissection. Familiarity with vascular variation and the management of vessels in key areas are essential for successful surgery.

Abstract:After the patient was anesthetized，pneumoperitoneum was established by the 5-hole method，and the surgeon was left sided. No metastases were found in the abdominal cavity，then the laparoscopic-assisted central approach radical right hemicolectomy was performed. Surgical steps：after fully expose the right mesentery and identify the ileocolonic vessels，incise the root of the right mesentery along the projection of the superior mesenteric vein，entering the Toldt’s space. The ileocolic vessels were isolated and the lymph nodes were dissected. The superior mesenteric vein was exposed and the lymph nodes at the root of the vessel were dissected. The right colon vessel and the middle colon vessel were exposed，and the lymph nodes were dissected. The ileocolonic vessels，the right colonic vessels and the right branch of the middle colonic vessels were ligated and transected. The Toldt’s space was expanded from the lateral side and connected to the medial side to complete the dissociation of the right colon. The ileum vascular arch was transected 10 cm from the ileocecal region. The ileum-transverse colon end-to-side anastomosis was completed by a 5cm assisted incision and at last，the specimen was removed.
Clinical Study of Laparoscopic Vs Open Right Hemicolectomy for Right Colon Cancer

Abstract:Objective To compare the results of laparoscopic and open right hemicolectomy in the management of colon carcinoma Methods We compared operation safety,recovery,complications,expenditure, oncological clearance and short term outcome for the two groups of patients undergoing either laparoscopic or open right hemicolectomy for colon cancer Results Between September 2000 and January 2003, 30 patients underwent a laparoscopic right hemicolectomy; 34 patients were treated via an open approach 2 patients in the laparoscopic group were converted to open surgery The mean operative time,blood loss, complications and expenditure were comparable in the two groups ( P 0 05); Flatus, time to resume early activity and hospital stay in laparoscopic group were 2 24±0 56、 3 94±1 64、 13 94±6 5 days respectively, which was significantly shorter than those in open group ( P 0 05) No significant difference between the two groups was observed for the oncological clearance, in terms of the number of lymph nodes removed and length of specimen ( P 0 05) The mean follow up time of the two groups were 23 15±7 95 and 22 19±7 46 months, respectively Local recurrence rate,metachronous metastases rate and cumulative survival probability at 36 months were similar for the two groups Conclusion These results suggest that laparoscopic right hemicolectomy for colon cancer can be performed safely, effectively and economically with the advantages of minimal invasiveness

Abstract:Complete mesocolic excision(CME)for colonic cancer offers a surgical specimen of higher quality,with higher number of lymph nodes compared to conventional colectomy.Laparoscopic CME in treating right colon cancer has advantages of minimal invasion and better prognosis.Laparoscopic surgery of right colon cancer is still a challenge because of complex anatomy,adjacent organs and variation of blood vessels.High quality Laparoscopic CME for right colon cancer requires the extensive laparoscopic experience and skills,solid knowledge of anatomy and tacit cooperation.Currently,the surgical approach of laparoscopic CME in right colon cancer is not uniform.This article introduced the contents of laparoscopic CME in right colon cancer and precautions,including the concept of CME,the indications and contraindications,surgical procedure,operation plane and the boundaries of central lymph node dissection.

Abstract:BackgroundWhether extended lymphadenectomy for right colon cancer leads to increased perioperative complications or improves survival is still controversial. This trial aimed to compare the efficacy and safety of complete mesocolic excision (CME) versus D2 dissection in laparoscopic right hemicolectomy for patients with right colon cancer. This article reports the early safety results from the trial.MethodsThis randomised, controlled, phase 3, superiority, trial was done at 17 hospitals in nine provinces of China. Eligible patients were aged 18–75 years with histologically confirmed primary adenocarcinoma located between the caecum and the right third of the transverse colon, without evidence of distant metastases. Central randomisation was done by means of the Clinical Information Management-Central Randomisation System via block randomisation (block size of four). Patients were randomly assigned (1:1) to CME or D2 dissection during laparoscopic right colectomy. Central lymph nodes were dissected in the CME but not in the D2 procedure. Neither investigators nor patients were masked to their group assignment but the quality control committee were masked to group assignment. The primary endpoint was 3-year disease-free survival, but the data for this endpoint are not yet mature; thus, only the secondary outcomes—intraoperative surgical complications and postoperative complications within 30 days of surgery, graded according to the Clavien-Dindo classification, mortality (death from any cause within 30 days of surgery), and central lymph node metastasis rate in the CME group only—are reported in this Article. This early analysis of safety was preplanned. The outcomes were analysed according to a modified intention-to-treat principle (excluding patients who no longer met inclusion criteria after surgery or who did not have surgery). This study is registered with ClinicalTrials.gov, NCT02619942. Study recruitment is complete, and follow-up is ongoing.FindingsBetween Jan 11, 2016, and Dec 26, 2019, 1072 patients were enrolled and randomly assigned. After exclusion of 77 patients, 995 patients were included in the modified intention-to-treat population (495 in the CME group and 500 in the D2 dissection group). The postoperative surgical complication rate was 20% (97 of 495 patients) in the CME group versus 22% (109 of 500 patients) in the D2 group (difference, −2·2% [95% CI −7·2 to 2·8]; p=0·39); the frequency of Clavien-Dindo grade I–II complications were similar between groups (91 [18%] vs 92 [18%], difference, −0·0% [95% CI −4·8 to 4·8]; p=1·0) but Clavien-Dindo grade III−IV complications were significantly less frequent in the CME group than in the D2 group (six [1%] vs 17 [3%], −2·2% [−4·1 to −0·3]; p=0·022); no deaths occurred in either group. Of the intraoperative complications, vascular injury was significantly more common in the CME group than in the D2 group (15 [3%] vs six [1%], difference, 1·8 [95% CI 0·04 to 3·6]; p=0·045). Metastases in the central lymph nodes were detected in 13 (3%) of 394 patients who underwent central lymph node biopsy in the CME group; no patient had isolated metastases to central lymph nodes.InterpretationAlthough the CME procedure might increase the risk of intraoperative vascular injury, it generally seems to be safe and feasible for experienced surgeons.FundingThe Capital Characteristic Clinical Project of Beijing and the Chinese Academy of Medical Sciences.
Combined Right Hemicolectomy and Pancreaticoduodenectomy for Locally Advanced Right Hemicolon Cancer

Abstract:Extracolonic invasion of the duodenum and/or pancreatic head rarely occurs in patients with right hemicolon cancer. However, when necessary, combined radical operation is a challenge to the surgeon. We reported 7 patients with locally advanced right hemicolon cancer who underwent combined right hemicolectomy (RH) and pancreaticoduodenectomy (PD) due to direct involvement of the duodenum or pancreatic head. This study included four males and three females with a mean age of 66.9 +/- 5.9 years. Computed tomography (CT) scans revealed right hemicolon cancer with duodenal invasion (5 patients) and pancreatic invasion (2). The mean operation time was 410 64 minutes and the estimated blood loss was 514 +/- 157 mL. After the operation, the mean postoperative hospital stay was 22.1 +/- 7.2 days. Five patients had postoperative complications. The mean follow-up time was 16.4 +/- 5.9 months. During this period, three patients died from tumor recurrence, one from postoperative complications, one from pulmonary disease, and two survived until the last scheduled follow-up. Five patients survived more than one year. Combined RH and PD for locally advanced right hemicolon cancer can be performed safely, thus providing a long-term survival rate in selected patients in a high-volume center.
Extent of Lymphadenectomy for Surgical Management of Right-Sided Colon Cancer: Multicenter, Open Label, Randomized, Controlled Trial.

Abstract:3575 Background: Complete mesocolic excision (CME) is being increasingly used for the treatment of right-sided colon cancer although there is still no strong evidence that CME provides better long-term oncological outcomes than D2 dissection. The controversy is mainly regarding the survival benefit from extended lymph node dissection emphasized by CME. Methods: This randomized, controlled, unblinded, phase 3, superiority trial involved 17 hospitals in China. Totally 1,072 patients diagnosed with stage T2-T4aNanyM0 or TanyN+M0 right-sided colon cancer were enrolled from January 11, 2016, through December 26, 2019. Participants were randomly assigned (1:1 ratio) to undergo either CME (n=536) or D2 dissection (n=536) during laparoscopic right colectomy. The primary outcome was the 3-year disease-free survival (DFS), and the main secondary outcome was the 3-year overall survival (OS). Results: Among 1072 patients, 77 were excluded after randomization. The remaining 995 patients (median age 61years, 59% male) were included in the primary analysis (CME [n=495] vs. D2 dissection [n=500]). The 3-year primary outcome assessment was completed for 97% patients. No significant differences were found between the groups in 3-year DFS (86.1% in CME group vs. 81.9% in D2 group; HR = 0.74; 95% confidence interval [CI]: 0.54-1.02; P = 0.06) or in 3-year OS (94.7% in CME group vs. 92.6% in D2 group; HR = 0.70; 95% CI: 0.43-1.16; P = 0.17). Conclusions: For primary surgical excision of right-sided colon cancer, complete mesocolic excision did not improve disease free survival, compared to standard D2 lymph node dissection. Standard D2 dissection should be the routine procedure for these patients. CME should only be considered in patients with obvious mesocolic lymph node involvement. Abstract:D3 lymph node dissection around the surgical trunk of the superior mesenteric vein is the key operative procedure of laparoscopic-assisted radical right hemicolectomy .Medial approach is adopted , from inside to outside and from downside to upside .Firstly, it is started with the anatomic projection of ileocolic vessels, dissecting the distal superior mesenteric vein from ileocolic vessels and accessing into the vascular sheath plane , along with which lymph nodes are dissected up to the inferior margin of the pancreatic neck, then dividing and disposing the right colic artery and middle colic vessels .After that, the further step is accessing into the natural surgical plane-right Toldt ’ s space , to expose the duodenum , pancreatic head and the right side of the superior mesenteric vein .Simultaneously , disposing the probable branches of the superior mesenteric vein and the gastrocolic trunk along with the right side of the superior mesenteric vein . Finally, it is necessary to separate the right colon completely from the outer laterally .By the medial approach, it could ensure a clear judge the variation of vessels and a complete D 3 dissection of lymph nodes , which could achieve a radical resection and significantly improve the surgical safety .
Modified cranial approach to right-sided colon cancer in a patient with intestinal nonrotation: A case report

Abstract:Managing colon cancer with intestinal nonrotation, a type of congenital intestinal malrotation, is challenging due to the presence of anatomical abnormalities and severe adhesions. When patients have nonrotation, it is markedly more difficult to determine which vessels correspond to the colic vessels and ileal vessels until all vascular branching patterns become evident. The optimal approach for right-sided colon cancer with intestinal nonrotation has yet to be established. In the present case of ascending colon cancer with intestinal nonrotation, we performed laparoscopic right hemicolectomy with D3 dissection using a modified cranial approach. This approach involves tracing, without resecting, branches from the superior mesenteric vein and superior mesenteric artery in a cranial-to-caudal manner until the ileocolic artery and ileocolic vein, which course toward the cecum, are identified, followed by the dissection of the colic vessels and lymph nodes in a caudal-to-cranial fashion.

Abstract:PURPOSE:To demonstrate the feasibility, safety, and technical strategies of hand-assisted laparoscopic complete mesocolic excision (HAL-CME) and to compare oncological outcomes between HAL-CME and the open approach (O-CME) for right colon cancers.METHODS:Patients who were scheduled to undergo a right hemicolectomy were divided into HAL-CME and O-CME groups. Measured outcomes included demographic variables, perioperative parameters, and follow-up data. Demographic variables included age, sex distribution, body mass index (BMI), American Society of Anesthesiologists (ASA) physical status classification, previous abdominal surgery, tumor localization, and potential comorbidities. Perioperative parameters included incision length, operative time, blood loss, conversion rate, postoperative pain score, postoperative first passage of flatus, duration of hospital stay, total cost, number of lymph nodes retrieved, TNM classification, and postoperative complications. Follow-up data included follow-up time, use of chemotherapy, local recurrence rate, distant metastasis rate, and short-term survival rate.RESULTS:In total, 150 patients (HAL-CME, 78; O-CME, 72) were included. The groups were similar in age, sex distribution, BMI, ASA classification, history of previous abdominal surgeries, tumor localization, and potential comorbidities. Patients in the HAL-CME group had shorter incision lengths, longer operative times, less operative blood loss, lower pain scores, earlier first passage of flatus, shorter hospital stay, higher total costs, similar numbers of lymph nodes retrieved, similar TNM classifications, and a comparable incidence of postoperative complications. The 2 groups were also similar in local recurrence rate, distant metastasis rate, and short-term survival rate.CONCLUSION:The results demonstrate that the HAL-CME procedure is a safe, valid, and feasible surgical method for right hemicolon cancers.

Abstract:Background: Whether extended lymphadenectomy for right colon cancer leads to increased perioperative complications or improves survival is still controversial. This multicentre randomised controlled trial (RCT) aimed to compare the efficacy and safety of complete mesocolic excision (CME) versus D2 dissection in laparoscopic right hemicolectomy for patients with right colon cancer. Methods: This superiority phase III trial was undertaken at 17 hospitals in 9 provinces of China. Patients with adenocarcinoma located between the cecum and the right third of the transverse colon, without evidence of distant metastases, were randomly assigned (in a 1:1 ratio) to receive either CME or D2 dissection during laparoscopic right colectomy. Morbidity and mortality in the first 30 days after surgery were compared between the two groups using a modified intention-to-treat (mITT) analysis. Findings: A total of 1072 patients were enrolled between January 11, 2016, and December 26, 2019. Of these patients, 995 (495 CME patients, 500 D2 patients) were included in the mITT analysis. The 30-day postoperative morbidity rate was 19·6% in the CME group vs. 21·8% in the D2 group (difference, -2·2% [95%CI, -0·07, - 0·03]; P = 0·34); no deaths occurred in either group. The CME procedures harvested significantly more lymph nodes (26·0 vs 23·0, difference, 3·4 [95%CI, 2·0, 4·9]; P<0·001), and larger mesentery areas (116·4 cm2 vs 107·8 cm2; difference, 8·4 [95%CI, 3·3, 13·5]; P = 0·001), but took longer time (163·0 minutes vs 150·5 minutes, difference 11·6 [95%CI, 5·7, 17·5]; P = 0·0002), and slight more blood loss (50 mL vs 50 mL, difference 5·4 [95%CI, -2·6, 13·4]; P = 0·049) than D2 dissections. Interpretation: When performed by experienced surgeons, CME during laparoscopic right hemicolectomy does not increase risk for intra/postoperative complications.
