Abstract:The accurate segmentation of retinal images is a basic step in screening for retinopathy and glaucoma. Most existing retinal image segmentation methods have insufficient feature information extraction. They are susceptible to the impact of the lesion area and poor image quality, resulting in the poor recovery of contextual information. This also causes the segmentation results of the model to be noisy and low in accuracy. Therefore, this paper proposes a multi-scale and multi-branch convolutional neural network model (multi-scale and multi-branch network (MSMB-Net)) for retinal image segmentation. The model uses atrous convolution with different expansion rates and skip connections to reduce the loss of feature information. Receiving domains of different sizes captures global context information. The model fully integrates shallow and deep semantic information and retains rich spatial information. The network embeds an improved attention mechanism to obtain more detailed information, which can improve the accuracy of segmentation. Finally, the method of this paper was validated on the fundus vascular datasets, DRIVE, STARE and CHASE datasets, with accuracies/F1 of 0.9708/0.8320, 0.9753/0.8469 and 0.9767/0.8190, respectively. The effectiveness of the method in this paper was further validated on the optic disc visual cup DRISHTI-GS1 dataset with an accuracy/F1 of 0.9985/0.9770. Experimental results show that, compared with existing retinal image segmentation methods, our proposed method has good segmentation performance in all four benchmark tests.

Abstract:Accurate segmentation of the optic disc (OD) depicted on color fundus images plays an important role in the early detection and quantitative diagnosis of retinal diseases, such as glaucoma and optic atrophy. In this study, we proposed a coarse-to-fine deep learning framework on the basis of a classical convolutional neural network (CNN), known as the Unet model, for extracting the optic disc from fundus images. This network was trained separately on fundus images and their vessel density maps, leading to two coarse segmentation results from the entire images. We combined the results using an overlap strategy to identify a local image patch (disc candidate region), which was then fed into the U-net model for further segmentation. Our experiments demonstrated that the developed framework achieved an average intersection over union (IoU) and a dice similarity coefficient (DSC) of 89.1% and 93.9%, respectively, based on a total of 2,978 test images from our collected dataset and six public datasets, as compared to 87.4% and 92.5% obtained by only using the sole U-net model. This suggests that the proposed method can provide better segmentation performances and have the potential for population based disease screening.

Abstract:Accurate segmentation of the optic disc (OD) regions from color fundus images is a critical procedure for computer-aided diagnosis of glaucoma. We present a novel deep learning network to automatically identify the OD regions. On the basis of the classical U-Net framework, we define a unique sub-network and a decoding convolutional block. The sub-network is used to preserve important textures and facilitate their detections, while the decoding block is used to improve the contrast of the regions-of-interest with their background. We integrate these two components into the classical U-Net framework to improve the accuracy and reliability of segmenting the OD regions depicted on color fundus images. We train and evaluate the developed network using three publicly available datasets (i.e., MESSIDOR, ORIGA, and REFUGE). The results on an independent testing set (n=1,970 images) show a segmentation performance with an average Dice similarity coefficient (DSC), intersection over union (IOU), and Matthew's correlation coefficient (MCC) of 0.9377, 0.8854, and 0.9383 when trained on the global field-of-view images, respectively, and 0.9735, 0.9494, and 0.9594 when trained on the local disc region images. When compared with the other three classical networks (i.e., the U-Net, M-Net, and Deeplabv3) on the same testing datasets, the developed network demonstrates a relatively higher performance.

Abstract:This study presents a groundbreaking deep learning model named MA‐ResUNet, which is built upon the U‐Net architecture but enhanced by integrating ResNeXt blocks inspired by aggregated residual transformations. Attention gates have been strategically incorporated to strengthen prominent features conveyed through the encoder's skip connection, simultaneously mitigating the influence of irrelevant information. Furthermore, a convolutional block attention module (CBAM) is seamlessly integrated into the decoder segment post‐concatenation, facilitating the fusion of salient features from the feature map with comprehensive global semantic information. Besides, a fundus image processing procedure is proposed to improve the model's accuracy. Glaucoma poses a significant threat to vision, capable of causing irreversible damage and, in severe instances, leading to permanent blindness. Accurate optic cup (OC) and optic disc (OD) segmentation are essential in glaucoma screening. In this study, a novel OC and OD segmentation approach is proposed. Based on U‐Net, it is optimized by introducing cardinality dimensions. Moreover, attention gates are implemented to reinforce salient features while suppressing irrelevant information. Additionally, a convolutional block attention module (CBAM) is integrated into the decoder segment. This fusion hones in on effective information in both channel and spatial dimensions. Meanwhile, an image processing procedure is proposed for image normalization and enhancement. All of these increase the accuracy of the model. This model is evaluated on the ORIGA and REFUGE datasets, demonstrating the model's superiority in OC and OD segmentation compared to the state‐of‐the‐art methods. Additionally, after the proposed image processing, cup‐to‐disc ratio (CDR) prediction on a batch of 155 in‐house fundus images yields an absolute CDR error of 0.099, which is reduced by 0.04 compared to the case where only conventional processing was performed.

Abstract:Glaucoma is a common and severe ocular disease that often leads to vision loss. The information on the optic disc (OD) and blood vessels in fundus images can significantly aid in glaucoma detection. In addition, the use of deep learning models for glaucoma detection is a highly effective approach. We propose a multi-task deep learning model called Multi-GlaucNet that can simultaneously segment the OD and blood vessels, thereby assisting doctors in diagnosing glaucoma. Multi-GlaucNet consists of three modules: the OD segmentation module, blood vessel segmentation module and glaucoma detection module. The OD segmentation module and blood vessel segmentation module both adopt an encoder–decoder structure to segment OD and blood vessels, respectively. The segmentation module is constructed using bottleneck layers in the encoding process and uses Pixel Shuffle and channel attention mechanisms in the decoding process. The detection module uses the ResNet50 network to perform glaucoma detection based on the features extracted from the segmentation modules. Multi-GlaucNet demonstrates outstanding performance in three areas. It achieves a Dice coefficient of 96.7% for OD segmentation on the ORIGA dataset, accuracy of 0.9798 and F1 score of 0.8562 for blood vessel segmentation on a mixed dataset. For glaucoma detection on the REFUGE dataset, it attains the highest accuracy of 0.967 and an area under the curve (AUC) of 0.950. These results validate the effectiveness of Multi-GlaucNet for glaucoma detection. The model's ability to perform multiple tasks with high accuracy and efficiency demonstrates that it is a valuable aiding tool for glaucoma diagnosis.

Abstract:Identification of retinal diseases in automated screening methods, such as those used in clinical settings or computer-aided diagnosis, usually depends on the localization and segmentation of the Optic Disc (OD) and fovea. However, this task is difficult since these anatomical features have irregular spatial, texture, and shape characteristics, limited sample sizes, and domain shifts due to different data distributions across datasets. This study proposes a novel Multiresolution Cascaded Attention U-Net (MCAU-Net) model that addresses these problems by optimally balancing receptive field size and computational efficiency. The MCAU-Net utilizes two skip connections to accurately localize and segment the OD and fovea in fundus images. We incorporated a Multiresolution Wavelet Pooling Module (MWPM) into the CNN at each stage of U-Net input to compensate for spatial information loss. Additionally, we integrated a cascaded connection of the spatial and channel attentions as a skip connection in MCAU-Net to concentrate precisely on the target object and improve model convergence for segmenting and localizing OD and fovea centers. The proposed model has a low parameter count of 0.8 million, improving computational efficiency and reducing the risk of overfitting. For OD segmentation, the MCAU-Net achieves high IoU values of 0.9771, 0.945, and 0.946 for the DRISHTI-GS, DRIONS-DB, and IDRiD datasets, respectively, outperforming previous results for all three datasets. For the IDRiD dataset, the MCAU-Net locates the OD center with an Euclidean Distance (ED) of 16.90 pixels and the fovea center with an ED of 33.45 pixels, demonstrating its effectiveness in overcoming the common limitations of state-of-the-art methods.

Abstract:Glaucoma is a chronic eye disease that leads to irreversible vision loss. The cup to disc ratio (CDR) plays an important role in the screening and diagnosis of glaucoma. Thus, the accurate and automatic segmentation of optic disc (OD) and optic cup (OC) from fundus images is a fundamental task. Most existing methods segment them separately, and rely on hand-crafted visual feature from fundus images. In this paper, we propose a deep learning architecture, named M-Net, which solves the OD and OC segmentation jointly in a one-stage multi-label system. The proposed M-Net mainly consists of multi-scale input layer, U-shape convolutional network, side-output layer, and multi-label loss function. The multi-scale input layer constructs an image pyramid to achieve multiple level receptive field sizes. The U-shape convolutional network is employed as the main body network structure to learn the rich hierarchical representation, while the side-output layer acts as an early classifier that produces a companion local prediction map for different scale layers. Finally, a multi-label loss function is proposed to generate the final segmentation map. For improving the segmentation performance further, we also introduce the polar transformation, which provides the representation of the original image in the polar coordinate system. The experiments show that our M-Net system achieves state-of-the-art OD and OC segmentation result on ORIGA data set. Simultaneously, the proposed method also obtains the satisfactory glaucoma screening performances with calculated CDR value on both ORIGA and SCES datasets.

Abstract:Glaucoma is a common eye disease that can cause blindness. Accurate detection of the optic disc and cup disc is crucial for glaucoma diagnosis. Algorithm models based on artificial intelligence can assist doctors in improving detection performance. In this article, U-Net is used as the backbone network, and the attention and residual modules are integrated to construct an end-to-end convolutional neural network model for optic disc and cup disc segmentation. The U-Net backbone is used to infer the basic position information of optic disc and cup disc, the attention module enhances the model’s ability to represent and extract features of optic disc and cup disc, and the residual module alleviates gradient disappearance or explosion that may occur during feature representation of the neural network. The proposed model is trained and tested on the DRISHTI-GS1 dataset. Results show that compared with the original U-Net method, our model can more effectively separate optic disc and cup disc in terms of overlap error, sensitivity, and specificity.

Abstract:Glaucoma is a major global cause of blindness. As the symptoms of glaucoma appear, when the disease reaches an advanced stage, proper screening of glaucoma in the early stages is challenging. Therefore, regular glaucoma screening is essential and recommended. However, eye screening is currently subjective, time-consuming and labor-intensive and there are insufficient eye specialists available. We present an automatic two-stage glaucoma screening system to reduce the workload of ophthalmologists. The system first segmented the optic disc region using a DeepLabv3+ architecture but substituted the encoder module with multiple deep convolutional neural networks. For the classification stage, we used pretrained deep convolutional neural networks for three proposals (1) transfer learning and (2) learning the feature descriptors using support vector machine and (3) building ensemble of methods in (1) and (2). We evaluated our methods on five available datasets containing 2787 retinal images and found that the best option for optic disc segmentation is a combination of DeepLabv3+ and MobileNet. For glaucoma classification, an ensemble of methods performed better than the conventional methods for RIM-ONE, ORIGA, DRISHTI-GS1 and ACRIMA datasets with the accuracy of 97.37%, 90.00%, 86.84% and 99.53% and Area Under Curve (AUC) of 100%, 92.06%, 91.67% and 99.98%, respectively, and performed comparably with CUHKMED, the top team in REFUGE challenge, using REFUGE dataset with an accuracy of 95.59% and AUC of 95.10%.

Abstract:Glaucoma is an irreversible neurodegenerative disease, where intraocular hypertension is developed due to the increased aqueous humor and blockage of the drainage system between the iris and cornea. As a result, the optic nerve head, which sends visual stimulus from our eyes to the brain, is damaged, causing visual field loss and ultimately blindness. Glaucoma is considered as the sneak thief of vision because it is difficult to diagnose early, and its regular screening is highly recommended to distinguish the neurological disorder. The detection of glaucoma is costly and time-consuming and not only there always remains a good possibility of human error but also this detection method is dependent upon the availability of the resources (experienced ophthalmologists and expensive instruments). In this work, an automatic glaucoma classification technique has been developed by utilizing multiple deep learning approaches. First, a new private dataset of 634 color fundus images has been collected and annotated by two eye specialists, a pediatric ophthalmologist and a glaucoma and refractive surgeon, from Bangladesh Eye Hospital, Bangladesh. Next, various deep learning models (EfficientNet, MobileNet, DenseNet, and GoogLeNet) have been used to detect glaucoma from fundus images. The model with EfficientNet-b3 architecture achieved the best results with test accuracy, F1-score, and ROC AUC of 0.9652, 0.9512, and 0.9574, respectively, for the cropped optic cup and disc fundus photographs. We also constructed a new dataset by segmenting the blood vessels from retinal fundus images employing a U-net model trained on High-Resolution Fundus Image Database. The MobileNet v3 model trained on this dataset achieved a satisfactory test accuracy of 0.8348 and an F1-score of 0.7957. This impressive result suggests that blood vessel segmentation of fundus images can be utilized as an alternative to detect glaucoma automatically.

Abstract:Diabetic retinopathy (DR) is a prevalent vision-threatening disease worldwide. Laser marks are the scars left after panretinal photocoagulation, a treatment to prevent patients with severe DR from losing vision. In this study, we develop a deep learning algorithm based on the lightweight U-Net to segment laser marks from the color fundus photos, which could help indicate a stage or providing valuable auxiliary information for the care of DR patients. We prepared our training and testing data, manually annotated by trained and experienced graders from Image Reading Center, Zhongshan Ophthalmic Center, publicly available to fill the vacancy of public image datasets dedicated to the segmentation of laser marks. The lightweight U-Net, along with two postprocessing procedures, achieved an AUC of 0.9824, an optimal sensitivity of 94.16%, and an optimal specificity of 92.82% on the segmentation of laser marks in fundus photographs. With accurate segmentation and high numeric metrics, the lightweight U-Net method showed its reliable performance in automatically segmenting laser marks in fundus photographs, which could help the AI assist the diagnosis of DR in the severe stage.

Robust Segmentation of Optic Disc and Cup from Fundus Images Using Deep Neural Networks

Abstract:Optic disc (OD) and optic cup (OC) are regions of prominent clinical interest in a retinal fundus image. They are the primary indicators of a glaucomatous condition. With the advent and success of deep learning for healthcare research, several approaches have been proposed for the segmentation of important features in retinal fundus images. We propose a novel approach for the simultaneous segmentation of the OD and OC using a residual encoder-decoder network (REDNet) based regional convolutional neural network (RCNN). The RED-RCNN is motivated by the Mask RCNN (MRCNN). Performance comparisons with the state-of-the-art techniques and extensive validations on standard publicly available fundus image datasets show that RED-RCNN has superior performance compared with MRCNN. RED-RCNN results in Sensitivity, Specificity, Accuracy, Precision, Dice and Jaccard indices of 95.64%, 99.9%, 99.82%, 95.68%, 95.64%, 91.65%, respectively, for OD segmentation, and 91.44%, 99.87%, 99.83%, 85.67%, 87.48%, 78.09%, respectively, for OC segmentation. Further, we perform two-stage glaucoma severity grading using the cup-to-disc ratio (CDR) computed based on the obtained OD/OC segmentation. The superior segmentation performance of RED-RCNN over MRCNN translates to higher accuracy in glaucoma severity grading.

Abstract:Glaucoma is the second leading cause of blindness all over the world, with approximately 60 million cases reported worldwide in 2010. If undiagnosed in time, glaucoma causes irreversible damage to the optic nerve leading to blindness. The optic nerve head examination, which involves measurement of cup-todisc ratio, is considered one of the most valuable methods of structural diagnosis of the disease. Estimation of cup-to-disc ratio requires segmentation of optic disc and optic cup on eye fundus images and can be performed by modern computer vision algorithms. This work presents universal approach for automatic optic disc and cup segmentation, which is based on deep learning, namely, modification of U-Net convolutional neural network. Our experiments include comparison with the best known methods on publicly available databases DRIONS-DB, RIM-ONE v.3, DRISHTI-GS. For both optic disc and cup segmentation, our method achieves quality comparable to current state-of-the-art methods, outperforming them in terms of the prediction time.

Abstract:Glaucoma is a serious ocular disorder for which the screening and diagnosis are carried out by the examination of the optic nerve head (ONH). The color fundus image (CFI) is the most common modality used for ocular screening. In CFI, the central region which is the optic disc and the optic cup region within the disc are examined to determine one of the important cues for glaucoma diagnosis called the optic cup-to-disc ratio (CDR). CDR calculation requires accurate segmentation of optic disc and cup. Another important cue for glaucoma progression is the variation of depth in ONH region. In this paper, we first propose a deep learning framework to estimate depth from a single fundus image. For the case of monocular retinal depth estimation, we are also plagued by the labeled data insufficiency. To overcome this problem we adopt the technique of pretraining the deep network where, instead of using a denoising autoencoder, we propose a new pretraining scheme called pseudo-depth reconstruction, which serves as a proxy task for retinal depth estimation. Empirically, we show pseudo-depth reconstruction to be a better proxy task than denoising. Our results outperform the existing techniques for depth estimation on the INSPIRE dataset. To extend the use of depth map for optic disc and cup segmentation, we propose a novel fully convolutional guided network, where, along with the color fundus image the network uses the depth map as a guide. We propose a convolutional block called multimodal feature extraction block to extract and fuse the features of the color image and the guide image. We extensively evaluate the proposed segmentation scheme on three datasets- ORIGA, RIMONEr3, and DRISHTI-GS. The performance of the method is comparable and in many cases, outperforms the most recent state of the art.

To diagnose certain disorders as early as possible, automatic segmentation of retinal arteries is used to help the doctors. Also, it is a challenge for the medical community to analyze the image with the right procedure to diagnose the disorders with high accuracy. Furthermore, this will help the doctor to make the right decision on effective treatment. Hence, the authors have implemented an enhanced architecture called U-Net to segment retinal vessels in this paper. The proposed conventional U-Net permits using all the accessible spatial setting information by adding the multiscale input layer and a thick square to the conventional U-Net in terms of improving the accuracy level of image segmentation. It achieved 95.6% accuracy with a comparatively traditional U-Net model. Moreover, the segmentation results have proved that the proposed approach outperformed in detecting most complex low-contrast blood vessels even when they are very thin. The task of segmenting vessels in retinal images is known as retinal vessel segmentation. Blood vessel density can be assessed using dense pixel values. Data augmentation and analytics play a major role in building the true value of eye blood vessels for medical diagnosis. The proposed method is very promising in the automatic segmentation of retinal arteries.

Abstract:Glaucoma is one of the most common causes of blindness in the world. Screening glaucoma from retinal fundus images based on deep learning is a common method at present. In the diagnosis of glaucoma based on deep learning, the blood vessels within the optic disc interfere with the diagnosis, and there is also some pathological information outside the optic disc in fundus images. Therefore, integrating the original fundus image with the vessel-removed optic disc image can improve diagnostic efficiency. In this paper, we propose a novel multi-step framework named MSGC-CNN that can better diagnose glaucoma. In the framework, (1) we combine glaucoma pathological knowledge with deep learning model, fuse the features of original fundus image and optic disc region in which the interference of blood vessel is specifically removed by U-Net, and make glaucoma diagnosis based on the fused features. (2) Aiming at the characteristics of glaucoma fundus images, such as small amount of data, high resolution, and rich feature information, we design a new feature extraction network RA-ResNet and combined it with transfer learning. In order to verify our method, we conduct binary classification experiments on three public datasets, Drishti-GS, RIM-ONE-R3, and ACRIMA, with accuracy of 92.01%, 93.75%, and 97.87%. The results demonstrate a significant improvement over earlier results.

Abstract:Glaucoma is a chronic eye disease in which the optic nerve is progressively damaged. As it cannot be cured, the best way to prevent visual damage is early detection and subsequent treatment. The optic nerve head examination, which involves measurement of cup-to-disc ratio, is considered one of the most valuable methods of structural diagnosis of the disease. Segmentation of optic cup on retinal fundus images can be used to estimate cup-to-disc ratio. This paper presents a novel approach for automatic optic cup segmentation, which is based on deep learning, namely, modified VGG-16 network and transfer learning technique. The modified network combines the residual, squeeze-and-excitation and multiscale feature. Our proposed method is tested on publicly available databases GlaucomaRepo and Drishti-GS. The evaluation of proposed method contains comparison with the original VGG-16 network and other state-of-the-art methods on above two fundus datasets which are captured from different devices. Experimental results show that our method outperforms the existing methods in robustness and accuracy.
Automated Segmentation of the Optic Disk and Cup using Dual-Stage Fully Convolutional Networks

Abstract:Automated segmentation of the optic cup and disk on retinal fundus images is fundamental for the automated detection / analysis of glaucoma. Traditional segmentation approaches depend heavily upon hand-crafted features and a priori knowledge of the user. As such, these methods are difficult to be adapt to the clinical environment. Recently, deep learning methods based on fully convolutional networks (FCNs) have been successful in resolving segmentation problems. However, the reliance on large annotated training data is problematic when dealing with medical images. If a sufficient amount of annotated training data to cover all possible variations is not available, FCNs do not provide accurate segmentation. In addition, FCNs have a large receptive field in the convolutional layers, and hence produce coarse outputs of boundaries. Hence, we propose a new fully automated method that we refer to as a dual-stage fully convolutional networks (DSFCN). Our approach leverages deep residual architectures and FCNs and learns and infers the location of the optic cup and disk in a step-wise manner with fine-grained details. During training, our approach learns from the training data and the estimated results derived from the previous iteration. The ability to learn from the previous iteration optimizes the learning of the optic cup and the disk boundaries. During testing (prediction), DSFCN uses test (input) images and the estimated probability map derived from previous iterations to gradually improve the segmentation accuracy. Our method achieved an average Dice co-efficient of 0.8488 and 0.9441 for optic cup and disk segmentation and an area under curve (AUC) of 0.9513 for glaucoma detection.

Abstract:Segmentation of retinal structures, namely optic disc, vessel, demarcation line, and ridge, is essential for describing the characteristics of Retinopathy of Prematurity (ROP). Computerized systems are being developed for automatic segmentation in fundus images to assist the medical experts and bring consistency in the diagnosis. There are multiple challenges in the segmentation task of premature infants’ fundus images. The annotation and ground truth preparation required for the segmentation is complex, challenging, and expensive. Further, ROP datasets are not available publicly, and hence carrying out such a task needs a primary dataset and significant assistance from the domain expert. To address this gap, two primary datasets named HVDROPDB-BV and HVDROPDB-RIDGE were developed. The datasets consist of images captured by two different imaging systems, having different sizes, resolutions, and illumination. This made the trained models generic and robust to data variability and heterogeneity. We propose the modified U-Net architectures by incorporating squeeze and excitation (SE) blocks and attention gates (AG) to segment the demarcation line/ridge and vessel from these datasets. These modifications were tested and validated by ROP experts. The performance of all the three networks (U-Net, AG U-Net, and SE U-Net) was promising, with a variation of 1 to 6% in the dice coefficient for the HVDROPDB datasets. The area under the curve (AUC) obtained for all three networks was above 0.94, indicating them as excellent models. AG U-Net outperformed the other two networks, with a sensitivity of 96% and specificity of 89% for stage detection via new test images.
