Abstract:American Journal of Men's Health, Volume 18, Issue 1, January-February 2024. Exploring the post-injury lives of those who have survived gunshot wounds is essential to understanding the entire scope of firearm violence. The lives of Black male firearm violence survivors are transformed in various ways due to their injuries both visible and invisible. This study explored how Black men who suffer from disabilities via a firearm negotiated their masculine identities. Semi-structured, qualitative interviews were conducted with 10 violently injured Black men participating in a hospital-based violence intervention program. Survivors expressed their thoughts on how their injuries impacted their manhood and masculinities. Three themes emerged: (1) perceptions of manhood, (2) loss of independence and burden on others, and (3) and mobility. These themes highlighted and described how their lives were impacted post-injury and characterized their psychological and physical experience of recovery. The research findings suggest the need for more qualitative studies to further explore the relationship between firearm injury, Black masculinity, and perceptions of manhood. While Black men are understudied in health research and invisible in disability research, they continue to be hyper-invisible when discussing violently acquired disabilities.

“That’s My Number One Fear in Life. It’s the Police”: Examining Young Black Men’s Exposures to Trauma and Loss Resulting From Police Violence and Police Killings

Abstract:Black males are disproportionately the victims of police killings in the United States, yet few studies have examined their personal narratives of trauma and bereavement resulting from police violence. Informed by critical race theory and stress and coping theory, we used a modified grounded theory approach to conduct and analyze in-depth, semistructured life history interviews with 40 young Black men (aged 18-24 years) in Baltimore, Maryland. Study participants were recruited from a GED and job readiness center serving Baltimore youth. Study results offer a nuanced understanding of the patterning and mental health consequences of police violence for young Black men. Participant disclosures of witnessing and experiencing police violence began in childhood and spanned through emerging adulthood, met Diagnostic and Statistical Manual of Mental Disorders–Fifth Edition criteria for trauma exposure, and embodied theoretical conceptualizations of racial trauma. Exposures to police violence fostered distrust of police and informed participants’ appraisals of their vulnerability to police violence across the life course. Six study participants disclosed losing loved ones to police killings. Injustice and hypervigilance accompanied grief. Implications for research, policy, and practice are discussed.

Abstract:Self-appraisal after a life-altering event is a critical process for individuals, often comprised by assigned labels that may not align with an individuals' perceptions of themselves or of their situation. Existing research within this victim-survivor dichotomy largely rests in the interpersonal violence space, with a victim assuming legal recourse and wrongdoing, and a survivor associating with positive personal characteristics like grit and resilience. Much existing literature on self-appraisal after interpersonal injury is heavily concentrated within the sexual violence literature, and this study applies these concepts to a sample of Black men injured by firearms. Ten Black men enrolled in a hospital-based violence intervention program (HVIP) were interviewed to understand how they label their experience of firearm injury, and if their perceptions aligned with common labels seen among other populations and/or in other areas of study (e.g., cancer, domestic violence). Each participant assigned themselves their own label, with three labels emerging: survivor, victim and survivor, and neither victim nor survivor. The results illustrate the nuance of experiences beyond the victim-survivor dichotomy, and how labels and personal identities may shift following injury into new terms and considerations of resilience and trauma processing. More research is warranted to understand the factors that shape self-labeling within this population, including influences of masculine norms, racialized stereotypes, community context, and availability of services. Findings support public awareness campaigns to reframe surviving violence as a strength, and for community partners and practitioners to increase access to culturally competent and trauma-informed mental healthcare.

Psychological Trauma Theory Research Practice and Policy

Abstract:Racism and gender-based prejudice produce a synergistic and toxic effect that necessitates analysis. There is a need to conduct more research with Black men as their experiences with race-based trauma may differ, given their concurrent disproportionate exposure to other forms of violence. Objective: The study's purpose was to develop items for and evaluate the factor structure and internal consistency of the Cultural Trauma Scale (CuTS) in an exclusive sample of Black men. Method: Using the Yale Center for Clinical Investigation, Community Alliance for Research Engagement principles, the study was conducted in a context of community-engaged research. Data were collected from individual interviews and focus groups, subject matter experts, and a sample of 150 adult Black men. Principal axis factoring (PAF) was used to determine the factor structure of the scale items. Results: Via PAF the final factor structure included five constructs addressing: American and its justice system: cognitive–emotional responses (Cronbach's α = .88), cognitive–behavioral coping (Cronbach's α = .77), willingness to seek cultural trauma treatment (Cronbach's α = .88), tripartite police fear (Cronbach's α = .81), and resilience (Cronbach's α = .61). Conclusions: The CuTS represents psychometric advancement in the measurement of Black male social justice and healing from cultural trauma. Abstract:The current article is part of a unique comparative study of the experiences and adjustment of 190 incarcerated young men in both Scotland and Canada. In collecting data on the participants' lives, the authors learned of the multiple traumas and losses many of them had suffered. Many participants however seemed to adhere to a prison masculinity that may constrain help-seeking behavior. Ultimately, this article analyses the levels of trauma that exists in a population of incarcerated young men within the context and alongside of the masculine ideals they appeared to adhere to. This article advocates for gender-responsive trauma-informed care for incarcerated young men that incorporates an exploration and understanding of masculine identity and how it interacts with help-seeking and trauma recovery.

Abstract:Research in clinical psychology and social psychiatry has highlighted the importance of social factors for outcomes following trauma. In this review, we speak to this issue in two ways. First, we highlight the value of a social identity framework for understanding the experience and impact of psychological trauma. Second, we draw on the Social Identity Model of Identity Change (SIMIC) to understand reactions to trauma. Specifically, we show (a) that negative responses to trauma are more apparent where trauma serves to undermine valued social identities; (b) that people prove more resilient in the face of trauma when valued social identities can be maintained or new social identities developed; and (c) that where old or new positive identities are reinvigorated or extend the self, this can be a basis for post-traumatic growth. We conclude by discussing the implications of this model for practice, and emphasise the importance of social identity management in the aftermath of trauma.

Abstract:Research shows that gender influences men's health-related beliefs and behaviours - including those within the context of traumatic brain injury (TBI) - making it a factor that should be considered when designing and implementing interventions for this population. To incorporate an understanding of such gendered influences in future educational materials for men with TBI, as well as their caregivers and clinicians, this qualitative study was informed by social constructionism, and aimed to explore how gender is related to men's post-TBI perceptions and behaviours in rehabilitation and recovery. Semi-structured interviews were conducted with 22 men with mild and moderate-severe TBI at the acute (≤ 3 months post-TBI) and chronic (> 3 months post-TBI) phases of injury. A reflexive thematic analysis approach was applied to interview data, guided by the concept of hegemonic masculinity as described by Connell, R.W. (2005. Masculinities [2nd ed.]. Polity). Three key themes were identified: (1) "I'm a man, I'm a rock": Undermining treatment, (2) "I'm going to face that challenge": Facilitation of recovery, and (3) "I don't feel as useful as a guy as I was before": Perceptions on return to work. These findings may be translated into gender-informed therapy strategies and materials.

Abstract:Criminal Justice and Behavior, Ahead of Print. In the United States, low-income Black men endure substantial stressors shaped by their social ecology (e.g., impoverished neighborhoods) and societal systems (e.g., criminal legal system) that produce racialized harm over the course of their lives. Although the stress literature has demonstrated a strong relationship between stressors and adverse health consequences, much remains unknown as to how low-income Black men experience trauma and adversity. This study draws on semistructured interviews with 20 men to understand how they experience and cope with trauma. Findings reveal that Black men experience multiple and enduring stressors—childhood trauma, interpersonal violence, loss of loved ones to violence, and police violence, throughout their lives. Participants' chronic exposure to trauma resulted in adverse psychological outcomes including desensitization, hypervigilance, and fatalism. Finally, how low-income Black men responded to trauma, sometimes resulted in additional adversity and further involvement in the criminal legal system.

Abstract:Trauma, Violence, &Abuse, Ahead of Print. Male experiences of domestic and relational violence have been only marginally explored in the literature. In connection to this, attitudes in the community and among service providers and criminal justice system entities can vary dramatically. This variance in attitudes creates an instability which has a differential impact on the help-seeking behaviors of victims. Additionally, help-seeking behaviors are often influenced by internalized shame and confusion on the part of the survivors themselves when their social constructions of masculinity do not align with lived experience. More is needed to understand the nature of male survivorship in situations of relational violence. A systematic review was conducted to begin organizing the data on the topic. This review started with 15,547 peer-reviewed articles. Those were systematically narrowed to a total of 16 of the most recent pieces of empirical science on the topic. The final articles were thematically analyzed. Findings suggest (a) cultural stigma around constructions of masculinity, (b) fear of disclosure, and (c) negative experiences with criminal justice and support system responses, among the highest drivers for the disparate experience and hesitation to seek help.

Abstract:Black boys and young men are disproportionately burdened with navigating contexts of community violence resulting from race-based structural inequities and concentrated disadvantage. Despite this chronic adversity, many Black boys and young men thrive; however, resilience research has traditionally focused on identifying individual- and family-level factors that support resilience. Research has yet to fully examine community-level resources that facilitate processes of resilience for Black boys and young men in the contexts of trauma, violence, and poverty. Guided by ecological frameworks and using the community-based participatory method of action-oriented community diagnosis, our qualitative study examines the perspectives of diverse community stakeholders (N = 29) whose roles and influence span systems levels and shape contexts of violence and healing for Black boys and young men in Greensboro, North Carolina. Findings point toward relationship (mentoring), community (safe spaces to heal), and societal (interventions to dismantle racism) level opportunities and barriers ("terroristic territorialism") to promote resilience in Black boys and young men. Implications for research and praxis that broadens the scope of resilience research from successful adaptation to conditions of community violence to community-level intervention to promote resilience and transformation are discussed. Abstract:This autoethnographic study, "Crossroads of Care: A Black Social Worker's Insights into the Lives of Black Men from Adolescence to Adulthood," explores the intersections of race, education, and mental health throughout the life course of Black men. Drawing on varied professional and personal experiences - from providing telehealth services for college students to being a resource within a predominantly Black high school - the research illuminates systemic barriers and profound effects of marginalization and isolation. Emphasizing evidence-based social work interventions and culturally relevant care, the study underscores the importance of narrative therapy and self-disclosure in addressing Black men's multifaceted needs. The findings contribute to discussions on racial equity in mental health and education, offering actionable recommendations for practitioners, policymakers, and educators to strengthen support systems. Abstract:Young Black gay, bisexual, and other sexually minoritized men (SMM) face high levels of police brutality and other negative, unwarranted encounters with the police. Such interactions have known health consequences. The purpose of this study was to understand the health, mental health, and social consequences of police brutality experienced by young Black SMM. We conducted in-depth interviews with 31 Black, cisgender men, ages of 16-30 and analyzed the data using thematic analysis. Our primary results are summarized in four themes: 1) Police brutality is built into the system and diminishes trust; 2) Videos and social media make visible violence that has long existed; 3) Police brutality contributes to anxiety and other psychosocial effects; and 4) Violence reduces feelings of safety and contributes to avoidance of police. Our results highlight the direct and vicarious police brutality participants are subjected to and sheds light on the effects of such violence on trust, perceived safety, anxiety, and trauma symptoms. Results from this study contribute to the needed public health conversation around police brutality against Black men, specifically shedding light on the experiences of Black SMM.

Abstract:In the United States, the experience of race—as a social construction—is an important cultural context to be considered in understanding resilience among black children and youth. Their normative developmental changes and challenges are complicated by their experiences as members of a socially constructed racial group and the associated racial disparities in social and economic outcomes (e.g., education, employment, wealth, standards of beauty and attractiveness, etc.). These disparities are the result of longstanding, collective racialized socio-historical experiences of blacks in the United States. Collectively, their individual, family and community-level ways of “making meaning” of, and responding to, these experiences reflect various manifestations of adaptive culture—consistent, patterned and collective responses to these group-level race-based challenges. As a critical developmental and protective mechanism, intergenerational racial-ethnic socialization can influence the behaviors and identity orientations of children and youth. Using Spencer’s Phenomenological Variant of Ecological Systems Theory, the authors explore existing literature to determine how one of Ungar et al.’s Seven Tensions—identity—is shaped by American race-based societal constructions and the racial-ethnic socialization of black children and youth in the United States.

Black adults in the United States seek mental healthcare at a lesser rate than White adults, attributed in part to the stigma of seeking psychological help. Black men in need of mental health care face a double barrier associated with the intersection of their race and gender, as men report more negative attitudes toward help-seeking than do women. To understand help-seeking attitudes among Black men, this study examined the direct effects of traditional masculinity, traditional/Black masculinity, and Black masculinity on public stigma of help-seeking as well as the indirect effects on self-stigma of help-seeking and psychological help-seeking attitudes among a sample of Black men in the United States (N = 160). Results demonstrated that Black masculinity and traditional/Black masculinity predicted greater public stigma, which predicts greater self-stigma and in turn less positive psychological help-seeking attitudes. This study also found an unexpected negative relationship between traditional masculinity and public stigma and an indirect positive effect on psychological help-seeking attitudes. Results support tailored stigma-reduction interventions for the Black community.

Abstract:Background Adverse childhood experiences have been associated with future outcomes; however, Felitti's 1998 ACEs questionnaire fails to capture the experiences of Black populations living in disinvested neighborhoods making it necessary to expand the ACEs questionnaire to examine the life experiences of violently injured Black men. Objective The aim of the study was to advance the understanding of ACEs among Black male firearm violence survivors using the ACEs questionnaire and semi-structured interviews. Participants and setting Ten Black male firearm violence survivors were recruited from an urban HVIP. Case managers conducted recruitment using the HVIP's REDcap database; active and previous HVIP participants were eligible for the study. Methods A qualitative study design was used to understand the childhood experiences of Black male firearm violence survivors using Felitti's ACEs questionnaire and a semi-structured interview examining perceptions of their childhood experiences. Due to the COVID-19 pandemic, recruitment and interviews were conducted over the phone. Results All participants experienced at least one ACE. Three themes arose from the interviews: youth incarceration, family separation and loss, and housing transition. Men at risk for violent injury experience ACEs beyond those measured in the current instrument.

Demystifying Male Emotional Development: Exploring the Implications of Forced Gender Roles on Black American Males

Abstract:Journal of Black Psychology, Ahead of Print. Statements like "boys do not cry" and other teaching modalities of emotional blandness can negatively affect the Black American male population with lower emotional development in adulthood. The focus of this study was to understand the extent of self-described societal-forced gender roles in childhood on the emotional development of Black American adult men. A total of 35 participants were interviewed using a qualitative study to further explore these experiences. After analysis, this study determined gender roles and emotional maturity affected later development. Key themes included feeling inept, stigma, lack of understanding, and loneliness. The participants discussed how frustrating it could be to be considered less of a man by society. These frustrations can lead to increased medical issues such as cardiovascular disease and increased behavioral health issues such as substance use, anxiety, depression, and issues within intimate relationships, as women may be expecting more than they can give. Professionals can utilize the findings of the study to further develop processes to teach emotional development in Black American children and assist adult males with their journey toward emotional maturity.

Abstract:Adverse childhood experiences (ACE) are important predictors of mental health outcomes in adulthood. However, commonly used ACE measures such as the Behavioural Risk Factor Surveillance System (BRFSS) have not been validated among Black sexually minoritized men (SMM) nor transgender women (TW), whom are known to have higher rates of ACE and poorer mental health outcomes. Assessing the psychometric properties of the measure is important for health equity research, as measurements that are not valid for some populations will render uninterpretable results.

Abstract:The purpose of this study was to explore potential similarities and differences in the ways boys and girls appraise and interpret their traumatic experiences, and better understand how gender roles, performance, and socialization processes may impact trauma experiences, appraisals, and narratives within the context of trauma-focused treatment. We used thematic analysis to analyze the trauma narratives of youth (N = 16) ages 8-16 who had experienced multiple types (M = 5.38) of child maltreatment and who were receiving Trauma-focused Cognitive Behavioral Therapy to address clinically elevated posttraumatic stress symptoms. Four themes emerged: variations in the content of negative cognitions, differences in relational emotion, adoption of socially prescribed gender roles, and symptom differences. Although many similarities existed in youth's trauma narratives, differences emerged that point to the importance of social context and the ways gender role expectations and socialization processes influence youth's appraisal of and responses to traumatic events. Findings indicate the importance of considering distress tolerance, relational emotion, gender identity development, and role socialization within the treatment milieu.

Abstract:One hundred twenty-three patients with lower chest and abdominal stab wounds were evaluated. If chest wounds were located between the two anterior axillary lines and abdominal examination was negative, peritoneal lavage was performed. If abdominal wounds were located between the two anterior axillary lines and physical examination was negative, local exploration was performed followed by lavage if local exploration was positive. Patients with negative lavage were observed and those with positive lavage were operated upon. Of the patients in this selected series 69.9% were spared an operative procedure. The incidence of negative laparotomy was 4.1%. It is concluded that the combination of local exploration and peritoneal lavage will increase diagnostic accuracy, eliminate unnecessary hospitalization, and reduce the number of negative laparotomies.
