Abstract:Purpose: Amyloidosis is underdiagnosed in light-chain amyloidosis (AL) and hereditary transthyretin amyloidosis (hATTR), as well as plasma cell dyscrasias (PCD). We aimed to investigate the utility of [18F]florbetapir (FBP) and [18F]fluorodeoxyglucose (FDG) positron emission tomography (PET) for the early detection and evaluation of organ involvement in systemic amyloidosis. Methods: We retrospectively included 83 participants, including 38 AL patients, 8 hATTR polyneuropathy patients, 28 PCD patients and 9 healthy controls. Whole-body PET/CT using [18F]FBP was performed in all participants, [18F]FDG PET was performed in 37 patients with AL and PCD, and the results were analyzed by visual and quantitative assessment. Biochemical, serum, and urine assays and histological analysis of tissue biopsies were performed. Results: [18F]FBP SUV and TBR analysis showed comparable uptake in AL and hATTR-PN patients (p.A117S, p.V50M, p.K55N, p.T69AM, or p.H76R mutation carriers) and greater uptake than in PCD patients and control patients. Different regional [18F]FBP and [18F]FDG distributions were observed among the PCD, AL, and hATTR-PN groups. Both [18F]FBP and [18F]FDG enabled the detection of amyloidosis in patients with PCD before clinical detection of AL. [18F]FBP SUV and visual analysis provide comparable measures of organ involvement and were comparable to [18F]FDG and clinical assessment. Conclusions: [18F]FBP PET detected organ amyloidosis in PCD, AL and hATTR-PN patients with high sensitivity and specificity and was more sensitive than [18F]FDG. Visual analysis and SUV analysis of [18F]FBP PET data provide comparable methods for evaluating organ involvement and are useful for noninvasively assisting in the early and accurate detection of systemic amyloidosis.

Abstract:BACKGROUND Systemic amyloid light chain (AL) amyloidosis is the most common type of amyloidosis, leading to cardiomyocyte necrosis and interstitial fibrosis. Gallium-68-labeled fibroblast activation protein inhibitor 04 (Ga-68-FAPI-04) has recently been introduced for imaging fibroblast activation in cardiac diseases. To date, cardiac fibroblast and cardiac amyloidosis (CA) phenotype activities have not been mapped. OBJECTIVES The aim of this study was to evaluate the feasibility of Ga-68-FAPI-04 positron emission tomography (PET)/computed tomography (CT) in assessing AL CA. METHODS Thirty consecutive patients (mean age: 59.1 +/- 7.7 years; 20 men, 10 women) with biopsy-proven AL amyloidosis were enrolled prospectively (including 27 with AL CA and 3 without AL CA). All patients underwent Ga-68-FAPI-04 PET/CT (107.4 +/- 26.5 MBq). Global standardized uptake values and left ventricular (LV) molecular volume were calculated in correlation to echocardiography (n = 30), cardiac magnetic resonance (n = 18), and clinical biomarkers. Subsequently, the patients were categorized as having patchy (PET-patchy), extensive (PET-extensive), and negative (PET-negative) patterns. RESULTS Of all patients, 80% (24 of 30) showed increased LV uptake (PET-patchy [n = 4] vs PET-extensive [n = 20]), whereas 6 patients did not show visible myocardial uptake. Standardized uptake value ratio and LV molecular volume were significantly higher in the PET-extensive than the PET-patchy group (2.79 mL +/- 1.22 mL vs 1.53 mL +/- 0.66 mL [P = 0.045] and 198.3 mL +/- 59.97 mL vs 127.8 mL +/- 25.82 [P = 0.005], respectively). Additionally, Ga-68-FAPI-04 uptake was significantly correlated with clinical biomarkers (Mayo stage and N-terminal pro-brain natriuretic peptide), interventricular septal thickness, left ventricular ejection fraction (LVEF), LV end-systolic volume, extracellular volume, and LV global strain (P < 0.05). CONCLUSIONS Ga-68-FAPI-04 PET/CT is feasible in detecting myocardial fibroblast activation in patients with AL CA in correlation with myocardial remodeling. It might provide complementary information on cardiac molecular characterization and staging of disease. Abstract:ABSTRACT: We report the imaging findings of 2 cases of cardiac amyloidosis, one light chain (AL) type and another transthyretin (ATTR) type. The images showed 11C-Pittsburgh compound B (11C-PIB) uptake in the heart with AL amyloidosis, whereas the ATTR amyloidosis showed 11C-PIB-negative and 99mTc-PYP-positive uptake in the heart. It demonstrated that using 99mTc-PYP scintigraphy and 11C-PIB PET/CT might be able to differentiate these 2 major types of cardiac amyloidosis noninvasively.

Abstract:49 Objectives: Patients with cardiac amyloidosis (CA), especially those with amyloid light-chain type (AL-CA), have a poor prognosis. The diagnosis of CA is often in advanced disease stage due to the non-specific clinical symptoms. Unfortunately, the treatment options are limited by then1. Therefore, early detection and accurate diagnosis plays an important role on the AL-CA patient management. Dynamic analysis of PET has attracted wide attention in recent years. By tracking the specific binding process of drugs and target, more accurate and specific pathological manifestations can be obtained. The aim of this study is to investigate the diagnosis performance of dynamic 11C-Pittsburgh B (11C-PIB) PET/CT on AL-CA. Methods: Twenty-one patients (age, 59.4±7.9 years; M/F, 12/9) with biopsy-proven diagnoses of AL-CA were enrolled. Seven subjects (age, 55.7±8.7 years; M/F, 4/3) who had neither history nor symptoms of cardiovascular diseases were recruited as healthy control. All subjects underwent a 30-min 11C-PIB PET/CT dynamic scan (SinoUnion PoleStar m660, China) with a dose of 411.1±89.7 MBq. Static images as early, intermediate and late scan at 5, 15, 25 min after radiotracer injection were reconstructed retrospectively. The left atrium blood pool volume of interest (VOI) was manually delineated by an experienced physician on MIM workstation (MIM Software Inc, USA). The left ventricular myocardium VOI and the left ventricle blood pool VOI were auto-segmented and manually calibrated to calculate kinetic parameters K1-k4 and metabolic rate constant Ki of the two-compartment model using Carimas software (Turku PET Centre, Finland, version 2.10)2. The retention index (RI) derived from the time-activity curve (TAC) was defined as the mean standardized uptake value (SUV) of the myocardial VOI from 10 to 20 min divided by the integral of the mean SUV of the left atrium blood pool VOI from 0 to 15 min. Semiquantitative analysis was performed on the static images at different time points. The standardized uptake value ratio (SUVR) was defined as the mean SUV of the myocardial VOI divided by the mean SUV of the left atrial blood pool VOI. Statistical analysis was performed between AL-CA and control groups. P < 0.05 was considered as significant. Results: The representative 11C-PIB PET/CT images at different static periods were shown in Figure 1. Patient case demonstrated higher SUV and slower wash-out process comparing with the control case, which can be observed by 11C-PIB PET/CT images as well as the corresponding TACs in Figure 2. SUVR in patients were significantly higher than that in controls over the whole acquisition period (early SUVR: 5.64±2.56 vs. 2.40±0.63; intermediate SUVR: 4.96±2.76 vs. 1.43±0.23; late SUVR: 4.56±2.67 vs. 1.34±0.22; with P < 0.001). Dynamic analysis (Figure 3) showed that K1, k2 and RI were significant different between AL-CA group and controls (4.96±1.08 vs. 5.82±0.56, 2.36±0.33 vs. 2.69±0.18, 0.04±0.01 vs. 0.14±0.08, with P=0.023, P=0.002, P<0.0001, respectively). Receiver operating characteristic curve (ROC) of RI on differentiating AL-CA group from controls yielded the sensitivity and specificity both 100% with the cutoff value of 0.515. ROCs of SUVR from intermediate and late static images on differentiating AL-CA group from controls presented the same sensitivity (95.2%) and specificity (100%) when the cutoff values were 1.835 and 1.695 (Table 1). Initial results showed that the kinetic parameters were effective to differentiate AL-CA patients from controls. The significant difference can also be observed by RI, which indicated that RI is an important clinical indicator for the diagnosis of AL-CA. Higher sensitivity of SUVR in the intermediate and late period of dynamic imaging indicated that dynamic imaging is valuable. Conclusions: 11C-PIB PET/CT dynamic imaging may represent a promising noninvasive tool for the early diagnosis of AL-CA. Big cohort study was needed for further investigation.

Abstract:Often differential diagnosis between AL and ATTR amyloidosis is difficult. Prognostic Value of Left Ventricular 18F-Florbetapir Uptake in Systemic Light-Chain Amyloidosis

Abstract:Background: Positron emission tomography/computed tomography (PET/CT) with 18F-florbetapir, a novel amyloid-targeting radiotracer, can quantify left ventricular (LV) amyloid burden in systemic light-chain (AL) amyloidosis. However, its prognostic value is not known. Objectives: The authors' aim was to evaluate the prognostic value of LV amyloid burden quantified by 18F-florbetapir PET/CT, and to identify mechanistic pathways mediating its association with outcomes. Methods: A total of 81 participants with newly diagnosed AL amyloidosis underwent 18F-florbetapir PET/CT imaging. Amyloid burden was quantified using 18F-florbetapir LV uptake as percent injected dose. The Mayo stage for AL amyloidosis was determined using troponin T, N-terminal pro-B-type natriuretic peptide (NT-proBNP), and free light chain levels. Major adverse cardiac events (MACE) were defined as all-cause death, heart failure hospitalization, or cardiac transplantation within 12 months. Results: Among participants (median age, 61 years; 57% males), 36% experienced MACE, increasing from 7% to 63% across tertiles of LV amyloid burden (P < 0.001). LV amyloid burden was associated with MACE (HR: 1.46; 95% CI: 1.16-1.83; P = 0.001). However, this association became nonsignificant when adjusted for Mayo stage. In mediation analysis, the association between LV amyloid burden and MACE was mediated by NT-proBNP (P < 0.001), a marker of cardiomyocyte stretch and heart failure, and a component of Mayo stage. Conclusions: In this first study to link cardiac 18F-florbetapir uptake to subsequent outcomes, LV amyloid burden estimated by percent injected dose predicted MACE in AL amyloidosis. This effect was not independent of Mayo stage and was mediated primarily through NT-proBNP. These findings provide novel insights into the mechanism linking myocardial amyloid deposits to MACE.
New quantitative indices of cardiac amyloidosis with 99mTc-pyrophosphate scintigraphy

Abstract:Purpose: Amyloid light chain (AL) and transthyretin (ATTR) are the major subtypes of cardiac amyloidosis (CA). 99mTc-pyrophosphate (PYP) scintigraphy is used to differentiate ATTR from other CA subtypes. We adapted the standardized uptake value (SUV) for 99mTc-PYP and proposed two quantitative indices, amyloid deposition volume (AmyDV) and total amyloid uptake (TAU). This study aimed to evaluate the utility of these quantitative indices in differentiating ATTR from non-ATTRs. Materials and methods: Before the SUV measurement, the Becquerel calibration factor (BCF) of 99mTc was obtained by a phantom experiment. Thirty-two patients who had undergone hybrid SPECT/CT imaging 3 h after injection of 99mTc-PYP (370 MBq) were studied. CT attenuation correction for image reconstruction was applied in all. We calculated SUV, AmyDV, and TAU using a quantitative analysis software program for bone SPECT (GI-BONE) and analyzed AmyDV using two methods: threshold method (set 40%); and constant value method (average SUVmax of ribs). We assessed the diagnostic ability of heart-to-contralateral lung (H/CL) ratio, SUV, AmyDV, and TAU to differentiate ATTR from non-ATTR using receiver operating characteristic (ROC) analysis. Results: Statistically significant differences in all quantitative indices were observed between ATTR and non-ATTR. The area under the curve of each quantitative index for discriminating between ATTR and non-ATTR were as follows: H/CL, 0.997; SUVmax, 0.953; SUVmean (M1), 0.964; SUVmean (M2), 0.969; AmyDV (M1), 0.875; AmyDV (M2), 0.974; and TAU, 0.974. The AmyDV (M2) had higher diagnostic ability than AmyDV (M1). Thus, TAU was calculated as AmyDV (M2) × SUVmean (M2). In the ROC curve, SUV, AmyDV, and TAU had almost the same diagnostic ability as H/CL in distinguishing ATTR from non-ATTRs. Conclusions: We propose two novel 3D-based quantitative parameters (AmyDV and TAU) that have almost equal ability to discriminate ATTR from non-ATTR.
Clincial Value of 18F-FDG PET/CT in Amyloid Light Chain Amyloidosis: Whether It’s Useful or Not?

Abstract:1427 Background: Amyloid light chain (AL) amyloidosis is a disease caused by the deposit of insoluble polymeric protein fibrils formed from misfolding of monoclonal immunoglobulin light chain that ultimately lead to regional or systematic disruption of tissues and organs. It is mainly associated with abnormal proliferation of clonal plasma cells, and a few with lymphocyte proliferation. Although some previous case reports described the 18F-FDG PET / CT imaging characteristics of amyloidosis involving a certain tissue or organ, a series of studies still be rare. Objectives: To analyze the clinical and 18F-FDG PET / CT imaging data of patients with AL amyloidosis and explore the value of 18F-FDG PET / CT in AL amyloidosis. MATERIALS AND METHODS: From May 2016 to November 2017, 21 consecutive patients with clinically confirmed or suspected AL amyloidosis and 18F-FDG PET/CT imaging were retrospectively analyzed, including 15 males and 6 females with an average age of 62.0 ± 10.0 years old (rang from 40-84 years old). 18F-FDG PET/CT scans were taken in the recommended standard condition with a dedicated PET/CT scanner (Discovery VCT, GE, American). Three experienced nuclear medical physicians read the images, and diagnosis was made by consensus. Positive PET/CT was defined as presence of morphological change in CT part and/or focal increased FDG uptakes in PET part which is higher than the background and can be excluded from physiological uptake. The final diagnoses and involved tissue or organ was established according to patient’s clinical, imaging and pathological examinations. Compared with the final diagnoses, we analyzed the detection rate of 18F-FDG PET/CT to detect the associated malignancy and involved tissues or organs in AL amyloidosis. Results: Of all 21 cases, 19 were diagnosed as systemic AL amyloidosis and 3 localized AL amyloidosis. 23.8% (5/21) had malignancy, including 3 cases of systemic AL amyloidosis complicated with multiple myeloma, 1 case of localized AL amyloidosis with lymphoma and 1 case of localized amyloidosis with colon cancer. According to the final diagnosis, a total of 50 involved tissues or organs were established, with kidney (24%, 12/50) involvement is most common, followed by heart (18%, 9/50), soft tissue (18%, 9/50), lung (10%, 5/50), lymph node (10%, 5/50), peripheral nerve (8%, 4/50), gastrointestinal tract (6%, 3/50), ureter (2%,1/50), bladder (2%, 1/50) and liver (2%, 1/50). In all this group, the overall positive rate of PET/CT was 61.9% (13/21). In 5 cases with malignancy, the positive rate was 80% (4/5), of which 2 cases manifested both malignant tumor and amyloid-related organ involvement, 1 only malignant tumor and 1 only amyloid-related organ involvement. Among 16 cases without malignancy, the positive rate was 56.3% (9/16), all of which indicated the involvement of amyloidosis. Of the 50 clinically confirmed amyloid-related tissues or organs, 18 were identified by PET/CT, with an overall detection rate of 38%. When lymph node, lung, soft tissue and heart were involved, the detection rate of PET/CT was 100% (5/5), 80% (4/5), 66.7% (6/9), and 44.4% (4/9), respectively. When the urinary and digestive systems were involved, PET / CT imaging showed no positive. Conclusions: 18F-FDG PET/CT is useful in AL amyloidosis. It can help find the associated malignancy and detect 1/3 amyloid-related tissues or organs. In organs with no physiological distribution of 18F-FDG, the detection rate is especially high.
Prognostic Value of Left Ventricular 18 F-Florbetapir Uptake in Systemic Light-Chain Amyloidosis

Abstract:Background: Myocardial immunoglobulin light-chain (AL) amyloid deposits trigger heart failure, cardiomyocyte stretch and myocardial injury, leading to adverse cardiac outcomes. Positron emission tomography/computed tomography (PET/CT) with 18 F-florbetapir, a novel amyloid-targeting radiotracer, can quantify left ventricular (LV) amyloid burden, but its prognostic value is not known. Therefore, we aimed to evaluate the prognostic value of LV amyloid burden quantified by 18 F-florbetapir PET/CT and to identify mechanistic pathways mediating its association with outcomes. Methods: Eighty-one participants with newly-diagnosed systemic AL amyloidosis were prospectively enrolled and underwent 18 F-florbetapir PET/CT. LV amyloid burden was quantified using 18 F-florbetapir LV percent injected dose (%ID). Mayo AL stage was determined using troponin T, N-terminal pro-B-type natriuretic peptide (NT-proBNP), and difference between involved and uninvolved free light chain levels. Major adverse cardiac events (MACE) were defined as all-cause death, heart failure hospitalization, or cardiac transplantation within 12 months. Results: Among participants (median age 61 years, 57% males), 36% experienced MACE. Incidence of MACE increased across tertiles of LV amyloid burden from 7% to 63% (p<0.001). LV amyloid burden was significantly associated with MACE in univariable analysis (hazard ratio 1.45, 95% confidence interval 1.15-1.82, p=0.002). However, this association became non-significant in multivariable analyses adjusted for Mayo AL stage. Mediation analysis showed that the association between 18 F-florbetapir LV %ID and MACE was primarily mediated by NT-proBNP (p<0.001), a marker of cardiomyocyte stretch and component of Mayo AL stage. Conclusion: In this first study to link cardiac 18 F-florbetapir uptake to subsequent outcomes, LV amyloid burden estimated by LV %ID predicted MACE in AL amyloidosis. But this effect was not independent of Mayo AL stage. LV amyloid burden was associated with MACE primarily via NT-pro-BNP, a marker of cardiomyocyte stretch and component of Mayo AL stage. Clinical perspective: In systemic light-chain (AL) amyloidosis, cardiac involvement is the key determinant of adverse outcomes. Usually, prognosis is based on the Mayo AL stage, determined by troponin T, N-terminal pro-B-type natriuretic peptide (NT-proBNP), and the difference between involved and uninvolved immunoglobulin free light chain levels (dFLC). Cardiac amyloid burden is not considered in this staging. In the present study, we used the amyloid-specific radiotracer 18 F-florbetapir to quantify left ventricular (LV) amyloid burden in 81 participants with newly-diagnosed AL amyloidosis and evaluated its prognostic value on major adverse outcomes (MACE: all-cause death, heart failure hospitalization, or cardiac transplantation within 12 months). We found that higher LV amyloid burden by 18 F-florbetapir positron emission tomography/computed tomography (PET/CT) was strongly associated with MACE. However, this association became non-significant after adjustment for the Mayo AL stage. Mediation analysis offered novel pathophysiological insights, implying that LV amyloid burden leads to MACE predominantly through cardiomyocyte stretch and light chain toxicity (by NT-proBNP), rather than through myocardial injury (by troponin T), also considering the severity of plasma cell dyscrasia (by dFLC). This mediation by NT-proBNP may explain why the association with outcomes was non-significant with adjustment for Mayo AL stage. Together, these results establish quantitative 18 F-florbetapir PET/CT as a valid method to predict adverse outcomes in AL amyloidosis. These results support the use of 18 F-florbetapir PET/CT to measure the effects of novel fibril-depleting therapies, in addition to plasma cell therapy, to improve outcomes in systemic AL amyloidosis.

Abstract:We present a 77-year-old woman with wild-type ATTR cardiac amyloidosis (ATTR-CA) who presented with dyspnea, arrhythmia, and elevated NT-pro BNP. Initial imaging including cardiac MRI, PYP scintigraphy, PiB PET/CT and NaF PET/CT revealed cardiac abnormalities. Tafamidis treatment was initiated. After 14 months, symptomatic improvement and reduced NT-pro BNP were observed. Cardiac MRI and PYP scintigraphy showed no significant change and increased NaF accumulation, while PiB PET/CT showed decreased amyloid deposition, suggesting that it may be superior to NaF PET/CT in assessing the therapeutic effect of tafamidis in ATTR-CA.

Abstract:We present a 77-year-old woman with wild-type ATTR cardiac amyloidosis (ATTR-CA) who presented with dyspnea, arrhythmia, and elevated NT-pro BNP. Initial imaging including cardiac MRI, PYP scintigraphy, PiB PET/CT and NaF PET/CT revealed cardiac abnormalities. Tafamidis treatment was initiated. After 14 months, symptomatic improvement and reduced NT-pro BNP were observed. Cardiac MRI and PYP scintigraphy showed no significant change and increased NaF accumulation, while PiB PET/CT showed decreased amyloid deposition, suggesting that it may be superior to NaF PET/CT in assessing the therapeutic effect of tafamidis in ATTR-CA.

Abstract:Objective: We aimed to investigate the distribution of [68Ga]Ga-p14-032, a novel PET ligand that binds to vascular amyloid, in patients diagnosed clinically with probable cerebral amyloid angiopathy (CAA) compared with patients with Alzheimer's disease (AD) and normal controls (NC). Methods: This longitudinal cohort study was composed of 10 subjects (three probable CAA patients, two AD patients, five NC subjects), recruited from a clinic in China. CAA patients had a history of lobar intracerebral hemorrhage (ICH) and met modified Boston criteria for probable CAA. All participants were aged at least 55 years and underwent [68Ga] Ga-p14-032 PET/CT or/and PET/MRI, and the Montreal Cognitive Assessment on initial assessment. Demographics were measured at baseline (diabetes, hypertension, hypercholesterolemia, ischemic stroke, and ICH). Two PET imaging experts reviewed the PET images with cortical standardized uptake value ratio (SUVr) displayed on a color scale and visually classified the images as positive or negative. The mean of SUVr was calculated using the pons as reference. Results: In CAA patients, PET scans were positive in regions with higher numbers of CMBs. No significant signal was seen in AD subjects or controls. The relative [68Ga]Ga-p14-032 retention in the cortex was stronger in patients with CAA than AD and NC (median SUVr 2.68 ± 1.53 vs. 1.77 ± 0.08 and 0.83 ± 0.24). Conclusions: Our results provide early evidence that the [68Ga] Ga-p14-032 PET probe binds preferentially to vascular amyloid and may be a useful tracer for diagnosing CAA.

Abstract:Cardiac amyloidosis (CA) is an infiltrative restrictive cardiomyopathy caused by the deposition of amyloid fibrils in the myocardium, presenting primarily as transthyretin cardiac amyloidosis (ATTR) and immunoglobulin light chain cardiac amyloidosis (AL). ATTR is further classified into wild-type (ATTRwt) and hereditary (ATTRv) based on transthyretin gene mutation. The disease is increasingly recognized as a significant cause of heart failure. Advances in diagnostic modalities, including electrocardiography, echocardiography, cardiac magnetic resonance imaging, and technetium pyrophosphate scintigraphy, have revolutionized the non-invasive diagnosis of CA. While ATTR can often be diagnosed with scintigraphy, AL typically requires histological confirmation. This review explores these diagnostic tools, emphasizing their role in early detection and quantification of disease burden, which are crucial for timely treatment and prognostication. This comprehensive overview aims to aid clinicians in efficiently diagnosing CA, ultimately improving patient outcomes.

Abstract:BackgroundDevelopment of noninvasive imaging modalities to quantify amyloid burden over time is an unmet clinical need. Technetium pyrophosphate (99mTc-PYP) scintigraphy is a simple and widely available radiotracer useful to differentiate transthyretin from light-chain amyloidosis in patients with advanced cardiac amyloidosis. We examined the utility of serial 99mTc-PYP scanning to quantify amyloid burden over time in TTR cardiac amyloidosis (ATTR-CA).Methods and ResultsTwenty subjects with ATTR-CA (10 wild type, 10 mutant) underwent serial 99mTc-PYP planar cardiac imaging. Cardiac retention was assessed both semiquantitatively (visual score 0, no uptake to 3, uptake greater than bone) and quantitatively (region of interest drawn over the heart, copied, and mirrored over the contralateral chest) to calculate a heart-to-contralateral (H/CL) ratio. Index scan mean visual score and H/CL were 3.0 ± 0.2 and 1.79 ± 0.2, respectively, and after an average 1.5 ± 0.5 years follow-up, did not differ, 3.0 ± 0.2, P = .33 and 1.76 ± 0.2, P = .44. H/CL change was minimal, 0.03 ± 0.17, did not correlate with time between scans, r = 0.19, P = .43, and was observed despite obvious clinical progression (increase in troponin ≥ 0.1 ng/mL, BNP ≥ 400 pg/mL, NYHA class, and/or death).ConclusionsSerial 99mTc-PYP scanning in subjects with advanced ATTR-CA does not show significant changes over an average 1.5 years of follow-up despite obvious clinical progression.

Abstract:Objective:To investigate the application of different imaging methods of 99Tc m-pyrophosphate (PYP) in the diagnosis and pathological classification of cardiac amyloidosis (CA). Methods:A total of 31 patients (22 males, 9 females, age 21-81(57.2±13.4) years) with suspected CA who underwent 99Tc m-PYP dual-phase scintigraphy (early-phase: 1 h, delay-phase: 2-3 h) and SPECT/CT (1 h) between December 2018 and December 2019 in Peking Union Medical College Hospital were retrospectively included. Taking clinical diagnosis as the standard, the results of visual score (≥2, positive) and semi-quantitative values (heart to contralateral lung (H/CL)≥1.5, positive) of 99Tc m-PYP uptake in dual-phase scintigraphy and SPECT/CT imaging were analyzed. One-way analysis of variance and Bonferroni test were used to analyze the data. Results:Among 31 patients with suspected CA, 15 were clinically diagnosed as CA (5 patients with transthyretin-related CA (ATTR-CA) and 10 patients with light chain CA (AL-CA)) and 16 were diagnosed as non-CA. All 5 patients with ATTR-CA had positive dual-phase scintigraphy and SPECT/CT imaging results. Three out of 10 patients with AL-CA had positive early-phase scintigraphy whereas negative delay-phase scintigraphy and SPECT/CT imaging results. Sixteen patients who were clinically diagnosed as non-CA had negative dual-phase scintigraphy and SPECT/CT imaging results. The sensitivity (5/5), specificity (10/10), positive predictive value (5/5), negative predictive value (10/10) and accuracy (15/15) of delay-phase scintigraphy and SPECT/CT imaging were the same. Among 31 patients, 16 patients carried transthyretin-related (TTR) gene mutation, and 4 of them who clinically diagnosed as variant ATTR (ATTRv) had positive image findings while 12 of them who not clinically diagnosed as CA had negative image findings. There were significant differences in H/CL between ATTR-CA group and AL-CA group in early-phase (2.11±0.24 vs 1.31±0.07) and delay-phase (2.02±0.19 vs 1.30±0.05; F values: 75.41 and 87.15, Bonferroni test, both P<0.01). Conclusions:99Tc m-PYP delay-phase scintigraphy and SPECT/CT have high diagnostic efficiencies in ATTR-CA, helping to determine the pathological classification of CA; while early-phase scintigraphy has false positive results. Moreover, 99Tc m-PYP imaging is helpful to detect CA in patients with TTR gene mutation.
False positive technetium-99m pyrophosphate scintigraphy in a patient with cardiac amyloidosis light chain: Case report

Abstract:Introduction: Patients with cardiac amyloidosis light chain (AL) present with negative Tc-99m pyrophosphate (PYP) scintigraphy (absent or mild heart uptake). On the contrary, patients with cardiac amyloidosis transthyretin (ATTR) present with positive Tc-99m PYP scanning (intensive heart uptake). We present a false positive Tc-99m PYP scintigraphy (grade 2, the heart-to-contralateral ratio is 1.65) in a patient with AL. Patient concerns: A 42-year-old Chinese man complained of effort intolerance, chest discomfort, and short of breath progressively over 1 year. New York Heart Association Class III. Physical examination showed legs swelling. Laboratory revealed elevated brain natriuretic peptide of 23,031 ng/mL (0-88) and Troponin-T of 273.4 ng/mL (0-14). Diagnosis: Cardiac amyloidosis light chain. Evidences: free light chains (FLCs): decreased serum free kappa/lambda ratio of 0.043 (0.31-1.56). Immunofixation electrophoresis: a positive lambda light chain monoclonal protein. Cardiac biopsy: HE: Ambiguity Congo red strain. Myocardial immunofluorescence: positive lambda light chain. Myocardial immunohistochemistry: positive lambda light chain, negative kappa light chain, and TTR. Interventions: Furosemide 40 mg qd, torasemide 20 mg qd, spirolactone 20 mg qd, potassium chloride 10 mL per 500 mL urine, atorvastatin calcium tablet 20 mg qd, aspirin enteric-coated tablets 100 mg qd during the 2-weeks in-hospital. Outcomes: The patient died 2 months later after discharge. Conclusion: False positive Tc-99m PYP scintigraphy may rarely presented in patients with cardiac amyloidosis light chain. So, the clonal plasma cell process based on the FLCs and immunofixation is a base to rule out AL cardiac amyloidosis when we interpret a positive Tc-99m PYP scintigraphy.

Abstract:Amyloid imaging using positron emission tomography (PET) has an emerging role in the management of Alzheimer’s disease (AD). The basis of this imaging is grounded on the fact that the hallmark of AD is the histological detection of beta amyloid plaques (Aβ) at post mortem autopsy. Currently, there are three FDA approved amyloid radiotracers used in clinical practice. This review aims to take the readers through the array of various indications for performing amyloid PET imaging in the management of AD, particularly using 18F-labelled radiopharmaceuticals. We elaborate on PET amyloid scan interpretation techniques, their limitations and potential improved specificity provided by interpretation done in tandem with genetic data such as apolipiprotein E (APO) 4 carrier status in sporadic cases and molecular information (e.g., cerebral spinal fluid (CSF) amyloid levels). We also describe the quantification methods such as the standard uptake value ratio (SUVr) method that utilizes various cutoff points for improved accuracy of diagnosing AD, such as a threshold of 1.122 (area under the curve 0.894), which has a sensitivity of 92.3% and specificity of 90.5%, whereas the cutoff points may be higher in APOE ε4 carriers (1.489) compared to non-carriers (1.313). Additionally, recommendations for future developments in this field are also provided.

Abstract:Amyloid deposition is a cause of restrictive cardiomyopathy. Patients who present with cardiac disease can be evaluated for transthyretin (TTR)-associated cardiac amyloidosis using nuclear imaging with 99m Tc-labeled pyrophosphate (PYP); however, light chain-associated (AL) cardiac amyloid is generally not detected using this tracer. As an alternative, the amyloid-binding peptide p5+14 radiolabeled with iodine-124 has been shown to be an effective pan-amyloid radiotracer for PET/CT imaging. Here, a 99m Tc-labeled form of p5+14 peptide has been prepared to facilitate SPECT/CT imaging of cardiac amyloidosis. A synthesis method suitable for clinical applications has been used to prepare 99m Tc-labeled p5+14 and tested for peptide purity, product bioactivity, radiochemical purity and stability. The product was compared with 99m Tc-PYP for cardiac SPECT/CT imaging in a mouse model of AA amyloidosis and for reactivity with human tissue sections from AL and TTR patients. The 99m Tc p5+14 tracer was produced with >95% yields in radiopurity and bioactivity with no purification steps required and retained over 95% peptide purity and >90% bioactivity for >3 h. In mice, the tracer detected hepatosplenic AA amyloid as well as heart deposits with uptake ~5 fold higher than 99m Tc-PYP. 99m Tc p5+14 effectively bound human amyloid deposits in the liver, kidney and both AL- and ATTR cardiac amyloid in tissue sections in which 99m Tc-PYP binding was not detectable. 99m Tc-p5+14 was prepared in minutes in >20 mCi doses with good performance in preclinical studies making it suitable for clinical SPECT/CT imaging of cardiac amyloidosis.

Abstract:Introduction Amyloid light chain (AL) and transthyretin amyloidosis (ATTR) account for >95% of all cardiac amyloidosis (CA) cases. The different CA subtypes may have overlapping clinical presentation necessitating further testing and investigation. Determination of CA subtype is critical, as management varies depending on the nature and source of the amyloidogenic protein. We present a very rare case of concomitant clinical and histology proven AL and ATTR amyloidosis. Case A 51-year-old Caucasian male with history of carpal tunnel syndrome, spinal stenosis and bicep tendon rupture initially presented to our clinic with shortness of breath. Initial evaluation included a transthoracic echocardiogram that demonstrated normal left ventricular systolic function and severe concentric left ventricular hypertrophy (interventricular septal wall diameter end diastole 1.92cm, posterior wall thickness end diastole 1.9cm) raising suspicion for amyloidosis. Technetium99m pyrophosphate (99mTc-PYP) scan showed a semi-quantitative visual score of grade 0 and no uptake on SPECT/CT. Cardiac MRI exhibited diffuse patchy mid wall late gadolinium enhancement and elevated extracellular volume (62%). Laboratory work-up included Hs-TNT 21ng/L, NT proBNP 1711pg/mL, kappa light chain 18.8mg/L, lambda light chain 219.2mg/L, FLC difference 199.8mg/L, K/L ratio 0.09 and immunofixation revealed a poorly defined region of restricted mobility in the lambda lane. Bone marrow biopsy (BMx) showed a lambda restricted plasma cell neoplasm representing 10% of marrow cellularity but no evident amyloid deposition. Fluorescence in situ hybridization results were normal. Right ventricular endomyocardial biopsy (EMBx) was performed for further assessment which demonstrated amorphous material strongly positive with thioflavin-S staining and immunohistochemistry observed amyloid deposits positive for lambda light chains. Laser capture microdissection coupled to mass spectrometry (LCM-MS) showed relatively equal abundance of lambda immunoglobulin light chains and transthyretin deposition. Genetic testing was negative, indicating wild type ATTR. AL amyloid treatment with daratumumab, cyclophosphamide, bortezomib, and dexamethasone was initiated with plan to eventually start tafamidis in the future. Discussion This case highlights a rare but clinically relevant concurrent presence of two different types of amyloidosis in the same patient. There can be overlap in the clinical presentation of these amyloid subtypes. Proceeding with simultaneous 99mTc-PYP and hematologic work-up is highly recommended in all patients when amyloid is suspected to adequately investigate this disease. If initial work-up is suggestive of both AL and ATTR amyloidosis, histopathological investigation with EMBx and BMx is warranted. Utilization of LCM-MS provides more accurate amyloid tissue typing with high sensitivity and specificity. With an increasing number of treatment options for patients with CA, providers should perform a thorough and complete diagnostic work-up to ensure appropriate management is provided.

Abstract:Cardiac amyloidosis, characterized by amyloid fibril deposition in the myocardium, leads to restrictive cardiomyopathy and heart failure. This review explores recent advancements in imaging techniques for diagnosing and managing cardiac amyloidosis, highlighting their clinical applications, strengths, and limitations. Echocardiography remains a primary, non-invasive imaging modality but lacks specificity. Cardiac MRI (CMR), with Late Gadolinium Enhancement (LGE) and T1 mapping, offers superior tissue characterization, though at higher costs and limited availability. Scintigraphy with Tc-99m-PYP reliably diagnoses transthyretin (TTR) amyloidosis but is less effective for light chain (AL) amyloidosis, necessitating complementary diagnostics. Amyloid-specific PET tracers, such as florbetapir and flutemetamol, provide precise imaging and quantitative assessment for both TTR and AL amyloidosis. Challenges include differentiating between TTR and AL amyloidosis, early disease detection, and standardizing imaging protocols. Future research should focus on developing novel tracers, integrating multimodality imaging, and leveraging AI to enhance diagnostic accuracy and personalized treatment. Advancements in imaging have improved cardiac amyloidosis management. A multimodal approach, incorporating echocardiography, CMR, scintigraphy, and PET tracers, offers comprehensive assessment. Continued innovation in tracers and AI applications promises further enhancements in diagnosis, early detection, and patient outcomes.
