Abstract:Anterior cruciate ligament (ACL) injuries are a common clinical entity among people involved in contact sports activities. With the number of primary ACL reconstructions increasing, there has been a proportional increase in the revision of failed ACL reconstruction surgeries. As our understanding of knee kinematics improves over time, there has been evidence that alignment of the lower limb weight-bearing axis also plays an important part in ACL functioning. Medial opening wedge high tibial osteotomy (MOWHTO) is one such procedure that has been used extensively worldwide to correct the varus lower limb alignment. This procedure is usually reserved for young active patients with varus lower limb weight-bearing alignment. The technical dilemma for the surgeon arises when there is a need to revise a failed ACL reconstruction while at the same time correcting the axis malalignment. The general dictum says that alignment correction is done first followed by ligament reconstruction in a dual-stage procedure. However, single-stage surgery is possible in certain indications. In this case report, we present the case of a 31-year-old male involved in recreational sports who sustained a repeat ACL tear five years post the index surgery. He also had a significant varus alignment of the lower limb weight-bearing axis which was considered to be one of the causes of index surgery failure. In this report, we would like to highlight the problems we encountered in a single-stage procedure and certain surgical facets of a single-stage alignment surgery with arthroscopic revision ACL reconstruction.
Hybrid Bone-Grafting Technique for Staged Revision Anterior Cruciate Ligament Reconstruction

Abstract:Although most patients who undergo anterior cruciate ligament (ACL) reconstruction achieve long-term functional stability and symptom relief, graft rupture rates range from 2% to 10%1,2. A small subset of these patients require a 2-stage revision ACL reconstruction because of tunnel osteolysis or tunnel malposition that will interfere with the planned revision tunnel placement3. In the present article, we describe the hybrid use of arthroscopically delivered injectable allograft matrix in the femur and pre-shaped bone dowels in the tibia for the treatment of lower-extremity bone deficiencies. Description: After induction of anesthesia, approximately 60 cc of bone marrow aspirate is harvested from the anterior iliac crest with use of sterile techniques and is processed to obtain bone marrow aspirate concentrate. Routine diagnostic knee arthroscopy is performed via the standard anterolateral and anteromedial portals. Any additional intra-articular pathology is addressed, followed by excision of the remnant graft material, removal of existing femoral hardware as needed, and exposure of the existing bone tunnels. The femoral tunnel is debrided arthroscopically, removing all soft-tissue remnants. The existing tibial tunnel is exposed via the previous anteromedial tibial incision when possible. Again, any existing tibial hardware is removed. The tibial tunnel is then prepared with use of a combination of sequential reaming and dilation. A shaver and curets are utilized to debride the sclerotic walls of the tunnel and remove the remnant graft material. A cannulated allograft bone dowel is then impacted into place over a guidewire, ensuring that the graft is not proud within the joint space. An injectable bone allograft matrix composite is prepared by manually mixing 5 mL of StimuBlast demineralized bone matrix (Arthrex) and 5 mL of FlexiGraft cortical fibers (Arthrex), along with the previously obtained bone marrow aspirate concentrate. Under dry arthroscopy, this bone graft is delivered into the femoral tunnel via a cannula with use of the anteromedial portal. Finally, a Freer elevator is used to contour the graft at the aperture of the tunnel. Graft osteointegration is mandatory prior to proceeding with the second stage of the procedure. Typically, a minimum 3-month follow-up is necessary to confirm adequate graft incorporation on computed tomography. Alternatives: As an alternative to the 2-stage procedure, previous studies have suggested the use of a single-stage revision utilizing cylindrical allografts or multiple "stacked screws."4-6 In addition, a number of bone allograft and autograft options have been described. Autologous bone graft can be harvested from the ipsilateral iliac crest or proximal aspect of the tibia with use of a variety of techniques7-10. Allograft bone options include cancellous bone chips and commercially available bone matrices or dowels11-14. Finally, another viable option is calcium phosphate bone graft substitutes15. There is a paucity of high-quality studies comparing available bone graft materials for revision ACL reconstruction; thus, no consensus exists regarding the optimal choice16. Rationale: A 2-stage approach is typically indicated for cases that demonstrate tunnel enlargement (>12 mm) that would compromise graft fixation or non-anatomic tunnel placement that will interfere with placement of the revision tibial tunnel3. The aim of the first stage is to re-establish adequate bone stock to optimize future tunnel placement and healing of the ACL graft during the second stage. We believe that this 2-stage approach is a reliable and safe method of treating enlarged, irregularly shaped bone tunnel defects while minimizing the risk of complications. Furthermore, the use of allograft material avoids the donor-site morbidity and volume limitations associated with the use of autograft bone. In the case of the femoral tunnel, the injectable bone graft composite has the advantage of being easily delivered arthroscopically while completely filling irregularly shaped tunnels. The use of bone marrow aspirate concentrate may improve the rate of graft healing as well as a hydrating substance to reduce viscosity and facilitate the flow of the bone graft material through the cannula16,17. For the tibia, especially in cases of lengthy tibial bone deficiencies, allograft bone dowels are commercially available off-the-shelf in a variety of different lengths and diameters to allow for adequate fill of bone defects. Expected outcomes: It is well known that outcomes following revision ACL reconstruction are inferior to those following primary ACL reconstruction, with a number of variables, beyond those associated with the surgical technique, influencing clinical outcomes18. Few studies have reported on the results of 2-stage revision ACL reconstruction with use of allograft bone; however, a high rate of allograft bone integration and improved bone quality at the time of revision ACL reconstruction have been reported13. Moreover, Mitchell et al. reported no differences in either subjective outcomes or failure rates between the 1-stage and 2-stage revision ACL reconstruction groups11. Important tips: Utilize computed tomography for preoperative assessment and measurement of the extent of osteolysis.If possible, obtain the operative report for the index ACL procedure in order to identify any preexisting hardware and to obtain any instrumentation that may be needed to facilitate hardware removal.Multiple bone dowel sizes are available off the shelf.A 70° arthroscope can aid in visualization of the entire tibial and femoral tunnel.Although the bone graft matrix can be injected while the joint is filled with irrigation fluid, we find it easier to administer the graft under dry arthroscopic conditions.Place the scope inside the tibial tunnel to confirm appropriate removal of soft tissue and hardware. Circumferential native cancellous bone should be visualized.It is acceptable to retain previous hardware if it does not interfere with the new tunnel placement.Utilize prior incisions to access the tibial tunnel.Do not underestimate the amount of bone graft needed for each tunnel.Avoid excessive force during impaction of the dowels. ObjectivesThe reconstruction of the anterior cruciate ligament (ACL) is the most performed knee surgery in Brazil; consequently, the number of failures in this reconstruction has increased. Each year, the exponential rise in ACL revision surgeries is linked to undiagnosed and untreated injuries. Possible causes of failures should be assessed, including the size and positioning of femoral and tibial tunnels, neglected ligament and meniscal injuries, rotational instabilities, and axis deviations.This article aims to describe a surgical technique for revising ACL reconstruction, associated with the posterolateral corner and anterolateral ligament, using the same femoral tunnel with autograft. Therefore, the article emphasizes the importance of preoperative diagnosis and the treatment of meniscal and ligament injuries associated with ACL injury. This new technique proves to be reliable, easily reproducible, providing adequate stability, reducing the number of femoral tunnels, and minimizing the chance of tunnel convergence, utilizing autologous grafts.Methodsdescription of Surgical Technique for Revision of Anterior Cruciate Ligament Reconstruction, Combined with Posterolateral Corner and Anterolateral Ligament Reconstruction Using the Same Femoral Tunnel for Reconstruction of These Structures.ResultsPatients who underwent ACL revision with the technique described presented adequate anterolateral and rotational stability, compared to other reconstruction techniques of the posterolateral corner and anterolateral ligament.ConclusionThe surgical technique described above generated stability in the revision of the ACL, as it was associated with simple peripheral ligament reconstruction using autologous tendons. This new technique appears to be reliable, easy to reproduce, generating adequate stability, thus reducing the number of femoral tunnels and the chance of confluence of these tunnels.

New approach to elaborate exfoliated starch-based nanobiocomposites.

Abstract:The present paper reports the successful elaboration of exfoliated plasticized starch-based nanobiocomposites. This was made possible by using cationic starch as a new clay organomodifier to better match the polarity of the matrix and thus to facilitate the clay exfoliation process. To demonstrate the efficiency of this new approach, either natural (MMT-Na) or organomodified (OMMT-CS) montmorillonite were incorporated into the starch nanobiocomposites by a melt blending process. The morphological analyses (SAXD and TEM) showed that MMT-Na leads to the formation of intercalated nanobiocomposites. On the contrary, OMMT-CS allowed the elaboration of well-exfoliated nanobiocomposites. Tensile tests performed on the obtained nanobiocomposites showed that exfoliated nanobiocomposites display enhanced mechanical properties compared to those of the intercalated nanobiocomposites and neat matrix. These results clearly highlight the great interest in using OMMT-CS to obtain starch-based nanobiocomposites with improved properties.
The application of proximal tibial anterior closing wedge osteotomy in anterior cruciate ligament reconstruction

Abstract:Introduction: Posterior tibial slope (PTS) plays a vital role in knee stability. PTS of more than 12° may be considered with increased strain on the native anterior cruciate ligament (ACL) fibers. To handle the instability caused by changes in PTS degree, Proximal Tibial Anterior Closing Wedge Osteotomy (PT-ACWO) is adopted by surgeons. Methods: Between October 2015 and October 2019, our department conducted a retrospective analysis of patients who experienced anterior cruciate ligament reconstruction (ACLR) graft failures, with a particular focus on pathological PTS. The time from initial ACLR to revision ranged from 1 to 10 years, with a mean of 2.5 years. Radiological assessments of PTS were conducted, and outcomes were quantified using the International Knee Documentation Committee (IKDC) score, Lysholm score, and Tegner activity scale. The surgical technique and the use of 3D Patient-Specific Instrumentation (PSI) were outlined. Preoperative imaging included the use of Kirschner wires to establish tibial plateau reference points, and 3D-PSI was employed to guide the location and depth of the tibial osteotomy, which was performed obliquely. In a notable instance, a novel tibial tunnel was mapped out, and ortho-bridge system (OBS) fixation was utilized to ensure adequate space for the new tunnel. Results: In a cohort of seven patients with a mean follow-up of 28.1 months, a significant reduction in PTS was noted postoperatively (median [interquartile range, IQR], from 15.27° [13.46°, 16.60°] to 6.25° [5.89°, 6.78°]; P = 0.002). IKDC score improved to 85.10 (80.25, 88.10), P < 0.001; the Lysholm score to 88.00 (73.00, 90.50), P < 0.001; and the Tegner score to 8.00 (7.20, 8.05), P = 0.025 at final follow-up. Skin incision healing delays occurred in two instances, yet achieved closure by six weeks. Radiographs at three months demonstrated faster bone healing in oblique osteotomies than transverse ones. Knee joint stability was maintained, with no additional complications or evidence of instability noted. Magnetic resonance imaging (MRI) confirmed graft integrity in all patients, without signs of enlargement or mispositioning at last observation. Conclusion: An augmented PTS angle exceeding 12 may constitute a potential etiology for the failure of ACLR grafts. In such patients, the implementation of ACLR combined with PT-ACWO could mitigate the risk of surgical failure during initial ACLR or subsequent revision procedures.

Background:While increased posterior tibial slope (PTS) is an important risk factor for failure after anterior cruciate ligament (ACL) reconstruction, controversy exists regarding indications and outcomes of proximal tibia anterior closing-wedge osteotomy (ACWO) with concomitant ACL reconstruction in patients with ACL tears.Purpose:To assess clinical outcomes after combined ACL reconstruction and proximal tibia ACWO.Study Design:Systematic review; Level of evidence, 4.Methods:In accordance with PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-analyses) guidelines, we performed a systematic review of the existing literature on ACWO and ACL reconstruction using PubMed (MEDLINE), Cochrane Library, Scopus, and Embase. The search phrases included "anterior closing wedge osteotomy,""anterior closing wedge tibial osteotomy,""anterior closing wedge proximal tibial osteotomy,""anterior cruciate ligament," and "revision anterior cruciate ligament." Non-English publications and single-patient case reports were excluded. Extracted data included study details, patient demographics, patient-reported outcomes (PROs), clinical outcomes, radiographic outcomes, complications, and return-to-sport (RTS) rates.Results:A total of 6 studies with 110 patients (110 knees) were included. Two-stage ACWO and ACL reconstruction was reported in 2 studies of 78 patients (71%), while a single-stage technique was reported in 4 studies of 32 patients (29%). ACWO was performed in the setting of primary ACL tear in 23 patients (21%) and in recurrent ACL tear in 87 patients (79%). Patients demonstrated postoperative improvements in Lysholm, pivot-shift test, and side-to-side difference in anterior tibial translation. After ACWO, all studies reported mean postoperative PTS of <10° (range, 4.4°-9.2°). Of patients with available RTS data (n = 43), the same-level RTS rate ranged from 65% to 100%. A two-stage procedure reported in 1 study had a lower RTS rate (n = 13 of 20 [65%]) than that of 2 studies with single-stage procedure (n = 4 of 5 [80%] and n = 18 of 18 [100%]). The overall complication rate was 0.9% to 1.3%, and there were no reported ACL retears.Conclusion:The current evidence, which is constrained by the quantity and quality of studies, showed that ACWO with single- or two-stage ACL reconstruction in patients with ACL insufficiency and increased PTS was associated with significant improvements in PROs and high RTS rates.

Total Knee Arthroplasty in Ranawat II Valgus Deformity with Enlarged Femoral Valgus Cut Angle: A New Technique to Achieve Balanced Gap.

Abstract:BACKGROUND Nearly 10% of patients undergoing primary total knee arthroplasty (TKA) have valgus deformity (VD) of the knee. For severe VD of the knee, a more lateral structural release is needed to achieve balance between medial and lateral space and neutral femorotibial mechanical axis (FTMA), which is challenging and technical. AIM To introduce a new surgical technique of resection, soft tissue release, and FTMA for Ranawat type-II VD with a 5-year follow-up. METHODS A retrospective study was conducted on patients who underwent TKA from December 2011 to December 2014. Hip-knee-ankle (HKA), range of motion (ROM), Oxford knee score (OKS), and knee society score (KSS) were used to assess the joint activity of patients in the new theory TKA group (NT-TKA) and were compared with those of the conventional TKA group (C-TKA). RESULTS A total of 103 people (103 knees) were included in this study, including 42 patients with an average follow-up period of 83 mo in the C-TKA group and 61 patients with an average follow-up period of 76 mo in the NT-TKA group. Six patients had constrained prosthesis, one had common peroneal nerve injury, and two had joint instability in the C-TKA group, but none of these occurred in the NT-TKA group. There were significant statistical differences in constrained prosthesis usage and complications between the groups (P = 0.002 and P = 0.034, respectively). The KSS at 1 mo post-operation for the C-TKA and NT-TKA groups were 11.2 3.8 and 13.3 +/- 2.9, respectively, with a significant difference (P = 0.007). However, the data of HKA, ROM, OKS KSS, and prosthesis survival rate were insignificant (P > 0.05) in both the preoperative and follow-up periods. CONCLUSION Adopting 5 degrees-7 degrees valgus cut angle for VD and sacrificing 2 degrees neutral FTMA for severe VD which cannot be completely corrected during TKA can reduce the need for soft tissue release, maintain early joint stability, reduce the use of constrained prostheses, and minimize postoperative complications.
Clinical outcomes of transtibial versus anteromedial drilling techniques to prepare the femoral tunnel during anterior cruciate ligament reconstruction

Abstract:Purpose The clinical outcomes of transtibial (TT) and anteromedial (AM) drilling techniques for anterior cruciate ligament reconstruction in preparing the femoral tunnel were directly compared by using a systematic literature review. Methods PubMed, EMBASE, the Cochrane Library, and the ISI Web of Science were searched until 10 May 2014, using the following Boolean operators: transtibial AND (anteromedial OR transportal OR independent OR three portal OR accessory portal) AND anterior cruciate ligament. All prospective and retrospective controlled trials directly comparing physical examination and scoring system results between TT and AM techniques were retrieved. No language or publication year limitations were used in our analysis. Results Of 504 studies retrieved, nine studies involving 769 patients were included. Results suggested that the AM was superior to the TT technique for preparing the femoral tunnel independent of the International Knee Documentation Committee (IKDC) Score (n.s.). A higher proportion of negative Lachman ( p = 0.002) and pivot-shift test ( p = 0.01) results, lower manual maximum displacement by KT-1000 ( p = 0.004), higher Lysholm scores ( p = 0.034), a higher incidence of IKDC grade A/B ( p = 0.04), and higher visual analogue scale scores ( p = 0.00) were observed with the AM compared with the TT technique. Conclusion Although the increases in these scores were below the minimal clinically important difference, this systematic review indicated that the AM was superior to the TT drilling technique based on physical examination and scoring system results. Abstract:Evidence-based health care with its emphasis on the need for searching, retrieving, summarizing and utilizing the best available evidence in decision-making has become essential in setting standards. Accordingly, maternity care standards should be based on best available evidence identified through systematic reviews of the literature. Promotion of standards relies on access to information, active strategies to facilitate professional behaviour change and efforts to sustain the change. Access to information is essential but insufficient to improve standards on its own. Changing professional behaviour is not accomplished easily. Active strategies based on the nature of the health care problem and an evaluation of the barriers that are likely to operate against change are required to influence professional behaviour. Once implemented, the standards should be regularly monitored and revised as new evidence becomes available.

Revision Strategy for Malunited Tibial Plateau Fracture Caused by Failure of Initial Treatment

Abstract:Objective: The aim of this study was to evaluate our treatment algorithm and results in revision surgery of malunited tibial plateau fracture after failure of initial treatment. Methods: Our revision strategy was as follows: First, we determined the presence of any infection. Second, we determined whether the patient required total knee arthroplasty (TIKA). Third, based on the characteristics of the tibial plateau fracture malunion, patients underwent one of the following surgical methods to achieve reduction: original fracture line osteotomy, tibial tubercle + original fracture line osteotomy, fibula head + original fracture line osteotomy, and metaphyseal open window reduction rod technique. The results was assessed with healing time, Rasmussen score, knee range of motion and complication rates. Results: A total of 25 patients 16 men and 9 women; Mean age: 47.4 years (range: 35-63 years) underwent tibial plateau fracture revision operation. The time interval between the two surgeries was 2-24 months. The follow-up time was 12-30 months, and the operation time was 120-300 min. All patients received bone union at the last follow-up. The healing time was 3-6 months. The postoperative Rasmussen score was 19-29 (mean 23.8) compared with 14.4 points before the operation (p < 0.05). The postoperative knee joint activity was 60-110 degrees (mean 95.0 degrees), compared with 57.8 degrees before the operation (p < 0.05). Six patients still had a 2-mm collapse on the articular surface, and 4 patients still had slight valgus (<5 degrees). Except for 2 TIKA cases, fracture reduction was excellent in 15 cases and good in 8 cases, with a good rate of 100%. Superficial wound infections occurred in 3 patients. Conclusion: Because revision of tibial plateau fracture malunion caused by failure of initial treatment is difficult, it is necessary to create a detailed surgical plan before the operation. Satisfactory clinical effects can be obtained if the correct revision strategy is used. The key to success is adopting a proper revision strategy according to the unique characteristics of the patient's tibial plateau fracture malunion. ObjectivesThe constant study evolution of the structures and biomechanical functioning of the posterolateral complex (PLC) of the knee show that this structure plays an extremely important role in joint stability, especially when associated with injury to the anterior cruciate ligament (ACL). Failure to detect the PLC lesion causes posterolateral rotational instability in the knee, generating an overload on the reconstructed ACL, increasing its potential for failure. Surgical treatment is the gold standard for patients with combined injuries of the PLC and ACL. However, despite the numerous reconstruction techniques described, the best technique is still controversial. This study presents a less invasive technique with fewer bone tunnels, lower cost and less surgical complexity, using four bone tunnels with a single tunnel for the ACL and lateral collateral ligament (LCL) in the lateral femoral condyle and only 04 interference screws for the anatomical reconstruction of the ACL / PLC and restoration of stability of the injured knee.Surgical techniquePatient in supine position. A graft is prepared with 2 semitendinosus tendons (1 from each knee) and 1 gracilis tendon (from the knee with ligament injury). The tendons must be at least 24 cm long. The graft is prepared in quadruple for the ACL (gracilis tendon duplicated and 2 simple semitendinosus tendons) and double for the PLC (continuation of the 2 simple semitendinosus tendons).An incision is made on the lateral aspect of the knee over the lateral epicondyle, towards the head of the fibula, approximately 12cm long. The femoral insertion site of the lateral collateral ligament (LCL) is identified slightly proximal and posterior to the femoral epicondyle. The unique femoral tunnel for the ACL and LCL is performed using the outside-in technique. The guidewire must be inserted in the origin of the LCL anatomical point at the external lateral cortex towards the origin of the ACL anatomical on the medial wall of the lateral fêmur condyle. The fibular head tunnel is performed, positioned from anterolateral to posteromedial, approximately 0.5 cm below the tip of the fibular head. The fourth and last tunnel is performed l at the popliteal tendon femoral insertion. It should be directed anteriorly and proximally to avoid confluence with the ACL tunnel. The graft is passed through the tibial tunnel towards the femoral tunnel. Both semitendinosus legs exit the LCL insertion at the lateral aspect of the femur. Graft fixation at the femoral tunnel lateral cortex is performed with an interference screw. The graft is tensioned at its tibial exit with knee flexed at 20° and fixed with an interference screw, reconstructing the ACL. For PLC reconstruction, the two semitendinosus legs are passed under the iliotibial tract through the fibular head tunnel from anterolateral to posteromedial. The graft is tensioned with knee at 30° of flexion and valgus and fixed with an interference screw at the fibular tunnel, reconstructing the LCL. The remaining portion of the graft is passed below the iliotibial tract and LCL, being inserted into the femoral tunnel corresponding to the insertion of the popliteal tendon. The graft is then tensioned with knee flexed at 60° and valgus and fixed with an interference screw.ConclusionWe believe that the described technique for ACL reconstruction associated with PLC injury is able to restore knee stability through a minimally invasive, reproducible and less technically challenging approach, with a lower learning curve than those currently available.

Abstract:Orthopaedic Journal of Sports Medicine, Volume 12, Issue 11_suppl4, November 2024. Objectives:Present a combined anterior cruciate ligament (ACL) and anterior oblique ligament (AOL) reconstruction technique using a rectus femoris tendon graft harvested in a minimally invasive approach.Methods:Examination under anesthesia should confirm ACL insufficiency and associated anteromedial rotational instability. The graft is harvested from the lateral portion of the rectus femoris tendon using a closed tendon stripper with the knee at 20 degrees of flexion. The distal portion is folded to form a double graft for the ACL, while the proximal remainder is used for the AOL without folding. The femoral tunnel is made using an outside-in technique, and the tibial tunnel is created within the native ACL tibial footprint. ACL graft fixation is performed with an interference screw in both the femur and tibia, with the knee at 20° of flexion. The medial epicondyle is identified by palpation (or fluoroscopy) for AOL graft fixation. The remaining graft is tunneled subcutaneously and fixed with an interference screw at the femoral tunnel in neutral rotation and full extension.Results:The technique improved knee stability and controlled anteromedial rotational instability. It is reproducible and presents few complications.Conclusion:The described technique using the rectus femoris graft harvested via a minimally invasive approach is a viable alternative for combined ACL reconstruction and anteromedial reinforcement in both primary and revision surgeries.

Abstract:Background: Injuries to the fibular collateral ligament (FCL) seldom occur in isolation and may present with a concomitant injury to the biceps femoris tendon and anterior cruciate ligament (ACL). Injuries to structures of the posterolateral corner (PLC) lead to varus and rotational instability of the knee, subjecting the cruciate ligaments to increased forces that may result in graft failure. Therefore, reconstruction of these structures should be performed concurrently with the ACL. Indications: Grade III FCL injuries heal poorly without operative treatment and often result in residual varus instability of the knee that increases medial knee compartment forces, and forces on both the native ACL and the graft status post ACL reconstruction. Therefore, preservation of biomechanical stability and long-term health of the knee are reliant on addressing injuries to the PLC surgically. Technique Description: A key concept of this surgical technique is a meticulous peroneal nerve neurolysis in the setting of altered biceps femoris anatomy, and the proper order of the surgical steps for tunnel creation, graft passage, and fixation and suture anchor insertion to achieve optimal patient outcomes. The described technique involves a lateral surgical approach, peroneal neurolysis, and preparation of fibular and femoral FCL tunnels, followed by a Bone-patellar tendon-bone graft (BTB) graft harvest. Attention is then turned to intra-articular work including the diagnostic arthroscopy, femoral and tibial tunnel preparation, passage of the ACL graft, and fixation of the grafts in femoral tunnels. Last, fixation is achieved in the following order: FCL graft on fibula, ACL graft on tibia, and biceps femoris tendon to fibular head. Results: Compared with the preoperative state, Moulton et al reported significant improvements in the average Lysholm and Western Ontario scores at 2.7 years postoperatively following anatomic FCL reconstrution. Furthermore, Thompson et al reported on primary suture anchor repair of distal biceps femoris in 22 elite athletes and reported that all patients had returned to their preinjury level of sporting activity at 2-year follow-up. Discussion: Anatomic reconstructions of the FCL and ACL, such as the one described in our technique, effectively restore near native knee biomechanics and offer superior clinical outcomes compared with nonanatomic-based FCL reconstructions. Abstract:Anterior open-wedge high tibial osteotomy of the proximal tibia is a reliable surgical procedure to treat genu recurvatum secondary to decreased posterior tibial slope. It is also useful in cases of posterior cruciate ligament (PCL) deficiency, especially after a failed PCL reconstruction procedure as reversal of posterior tibial slope is a common risk factor for failure of PCL reconstruction. In some knee joints, reversed tibial slope may be associated with varus or valgus deformity. We describe correction of reverse posterior slope along with varus deformity, which may result from a poorly performed high tibial osteotomy leading to failure of reconstructed PCL. In our technique, both of the above deformities are corrected simultaneously, thereby addressing the sagittal and coronal malalignments of the knee with one osteotomy.

Abstract:This article aims to provide a new surgical technique for rotational instability in the setting of anterior cruciate ligament rupture. Two main groups of surgical procedures can be identified in the treatment of anterolateral knee instability: lateral extra-articular tenodesis and anterolateral ligament reconstruction. Although the importance of anterior cruciate ligament reconstruction in anterolateral complex injuries is well known, the superiority of lateral extra-articular tenodesis over anterolateral ligament reconstruction or vice versa has not yet been shown. Both techniques show improved outcomes and reduced graft failure rates. The presented procedure can be considered a modification of the technique first described by Lemaire. Better tensioning can be achieved through cortical suspension by identifying the anisometric point on the lateral femur and performing a medial pullout on the femoral side. The advantages of this technique are better fine-tuning and tensioning, less invasiveness, and adjustable cortical fixation, which allows for a precise, incremental tensioning of the graft, ensuring circumferential healing of the graft within the socket and reducing the risk of graft laceration, which may happen with interference screws. Internal bracing provides excellent contact pressure between the femoral button and femoral cortex, ensuring that adequate tensioning is applied to the graft.

Abstract:It has become increasingly recognized that a steep posterior tibial slope is a modifiable risk factor for the failure of an anterior cruciate ligament reconstruction. An anterior closing wedge osteotomy (ACWO) may be performed to flatten the slope, and this osteotomy may be performed above, through, or below the tibial tuberosity. A supra-tuberosity technique may increase the patellar height. However, this increase may be modest, may not affect the clinical outcomes, and is only one risk that I would consider when deciding whether to perform an ACWO above, through, or below the tibial tuberosity. The short proximal fragment of supra-tuberosity ACWO mandates a short ACLR tibial tunnel and may compromise fixation of the osteotomy. A trans-tuberosity ACWO usually requires a tibial tuberosity osteotomy, which requires slower postoperative mobilization and risks nonunion or fracture of the tibial tubercle. And, an infra-tuberosity ACWO has lesser healing potential than a more proximal osteotomy.

Abstract:BACKGROUND It has been confirmed that the increased posterior tibial slope over 12 degrees is a risk factor for anterior cruciate ligament injury, and varus deformity can aggravate the progression of medial osteoarthritis. AIM To evaluate the efficacy of modified high tibial osteotomy (HTO) and anterior cruciate ligament reconstruction (ACLR) in the treatment of anterior cruciate ligament (ACL) injuries with varus deformities and increased posterior tibial slope (PTS) based on clinical and imaging data. METHODS The patient data in this retrospective study were collected from 2019 to 2021. A total of 6 patients were diagnosed with ACL injury combined with varus deformities and increased PTS. All patients underwent modified open wedge HTO and ACLR. The degree of correction of varus deformity and the PTS was evaluated by radiography and magnetic resonance imaging. RESULTS All 6 patients (6 knee joints) were followed up for an average of 20.8 ± 3.7 months. The average age at surgery was 29.5 ± 3.8 years. At the last follow-up, all patients resumed competitive sports. The International Knee Documentation Committee score increased from 50.3 ± 3.1 to 87.0 ± 2.8, the Lysholm score increased from 43.8 ± 4.9 to 86 ± 3.1, and the Tegner activity level increased from 2.2 ± 0.7 to 7.0 ± 0.6. The average movement distance of the tibia anterior translation was 4.8 ± 1.1 mm, medial proximal tibial angle (MPTA) was 88.9 ± 1.3° at the last follow-up, and the PTS was 8.4 ± 1.4°, both of which were significantly higher than those before surgery (P < 0.05). CONCLUSION Modified open wedge HTO combined with ACLR can effectively treat patients with ACL ruptures with an associated increased PTS and varus deformity. The short-term effect is significant, but the long-term effect requires further follow-up.

Abstract:Conventional anatomical single bundle anterior cruciate ligament (ACL) reconstruction technique with round tunnels could not simulate morphology of native insertion, while studies about ACL reconstruction technique with modified tunnels based on morphology of anatomical insertion are rare. The purpose of this study was to demonstrate an ACL reconstruction technique with rounded rectangle tibial tunnel and oval femoral tunnel and compare clinical outcomes with conventional technique. A prospective comparative study was performed in 80 consecutive subjects who underwent ACL reconstruction with the conventional round tunnels (RT-Group, n=40) or modified tunnels (MT-Group, n=40). For the modified surgery, the tunnel was modified with a bone file based on the anatomical direction and area of the remnant insertion fibers. Graft maturity were evaluated by MR images at 12 months post-operatively and patients were examined for functional scores, physical examinations at 2-year follow-up. The primary variable was the pivot-shift test. No serious complications were experienced in either group. Seventy patients (87.5%) were examined at 2-year follow-up, significant improvements were seen in both groups compared with the preoperative values in terms of all clinical assessments. Tegner scores, pivot-shift test results and SNQ value in the MT-Group were significantly better than RT-Group (P=0.04, P=0.03 and P=0.001, respectively). There were no significant differences in Lysholm scores, IKDC scores, KT-2000 measurements and Lachman tests. We successfully developed the ACL reconstruction technique with rounded rectangle tibial tunnel and oval femoral tunnel, which was superior to conventional technique in terms of postoperative Tegner scores, pivot-shift tests and early graft maturity.
