Abstract:We present an unusual case of a patient with bilateral-lung transplantation due to severe coronavirus disease 2019 (COVID-19), who subsequently suffered complications with acute myocardial infarction and underwent primary percutaneous coronary intervention (PCI). A 70-year-old man presented with fever, cough, and dyspnea after returning from Hubei Province at the end of Jan. 2020. The diagnosis of COVID-19 had been confirmed by the nucleic acid test at a local hospital on Feb. 2, 2020. He had a history of mild hypertension and Type 2 diabetes without regular medication. He was a current smoker but denied the history of ischemic heart disease or hyperlipidemia. Despite initial treatment with methylprednisolone, umifenovir, and darunavir, the patient’s condition worsened with continued dyspnea and persisting fever. On Feb. 9 (Day 7 after symptoms onset), the patient was transferred to the intensive care unit (ICU) of our hospital. On arrival, the patient’s vital signs were stable except for tachypnoea at 27 min. Key laboratory results were: white blood cell count, 6.6×10 L; neutrophil, 89.1%; hemoglobin, 129 g/L; platelet count, 180×10 L; creatinine, 64 μmol/L; high sensitive C reaction protein (hs-CRP), 45.51 mg/L. chest computed tomography (CT) scan showed bilateral diffuse infiltration. After admission, he was treated with high-flow nasal oxygen, methylprednisolone, umifenovir, piperacillin, and tazobactam. On Feb. 14 (Day 12), he was intubated due to severe hypoxia. On Feb. 22 (Day 20), repeated nucleic acid tests for 2019 novel coronavirus (2019-nCoV) in sputum and bronchoalveolar lavage fluid were all negative; however, his condition continued to deteriorate due to pulmonary consolidation complicated by Stenotrophomonas maltophilia infection. We began venovenous extracorporeal membrane oxygenation (ECMO) treatment on Feb. 26 (Day 24). A CT scan on Mar. 2 (Day 29) revealed significant progression of pulmonary consolidation (Fig. 1). A bilateral-lung transplantation was performed on Mar. 8 (Day 34) after multidisciplinary discussion. Postoperatively, the ECMO was withdrawn since the oxygenation had been improved and the patient’s general condition was more stable. A continuous renal replacement therapy (CRRT) was started on Mar. 10 (Day 36/Day 2 postoperative) to support renal function. On Mar. 11 (Day 3 postoperative), laboratory results were: hemoglobin, 80 g/L; platelet count, 36×10 L; creatinine, 103 μmol/L; international normalized ratio (INR), 1.77; activated partial thromboplastin time (APTT), 45.9 s; prothrombin time (PT), 20.8 s; D-dimer, 2927.00 μg/L. On Mar. 12 at 9:00 a.m. (Day 4 postoperative), the patient developed a sudden episode of ventricular fibrillation. An Efficient Combination Immunotherapy for Antitumor Immunity Without Accelerating Cardiac Allograft Rejection.

Abstract:Immunotherapy with immune checkpoint inhibitors (ICIs), including antibodies against programmed cell death protein-1 (PD-1) and its receptor programmed cell death ligand-1 (PD-L1), represents a promising systematic treatment for advanced human malignancies. Transplantation remains the ultimate therapy for end-stage organ diseases. However, the efficacy of ICI treatment in solid organ transplant (SOT) recipients remains controversial. We established a transgenic primary liver cancer mouse model and performed allogeneic heterotopic heart transplantation. Different treatments were performed and survival curves were calculated. Graft samples were collected, and immune cells and the cell surface expression of PD-L1 were analysed by flow cytometry. Inflammatory cytokine levels in the serum were measured by an inflammatory array. The specificity of the histochemical techniques was tested by staining sections. A combination immunotherapy comprising a BET protein inhibitor (JQ1) and an immune checkpoint inhibitor (anti-PD-L1 antibody) was administered to primary liver cancer model mice bearing cardiac allografts. Interestingly, the combination immunotherapy effectively suppressed the progression of primary liver cancer but did not accelerate allograft rejection. In accordance with our previous findings, BET protein inhibition enhances the expression of a putative membrane transporter (Rab8A), which upregulates the expression of PD-L1 on the plasma membrane in a transgenic primary liver cancer mouse model. This may be a crucial mechanism of tumour progression arrest. Our data showed that heart transplantation upregulated the expression of the proinflammatory factor IFN-gamma and suggested that BET protein inhibition (with JQ1) decreased PD-L1 expression in heart tissues after cardiac transplantation. This phenomenon was accompanied by enhanced infiltration of inflammatory IFN-gamma. Our study provides a novel and efficient therapeutic strategy for SOT recipients.

In this case, when adjust the immune suppression program, acute rejectionhappened, the treatment is focusedon the pulmonary infection after kidneytransplantation; Pharmaceutical care was performed in the process of anti infection drugs and immune inhibitors, as well as the treatment of immune rejection, attention matters and drug monitoring.Results andConclusion: Clinical pharmacists should participate actively in the treatment of patients with the disease and medication management of pulmonary infection after renal transplantation, grasp the kidney transplantation patients with pulmonary infection after anti infection treatment principle, pay attention to infection continuous monitoring of laboratory parameters, to help evaluate the effect of anti infection; At the same time, according to the infection condition adjust the immune suppression program, closely monitor the CNIs blood concentration and immune suppression intensity. Furthermore, to close monitoring of renal function damage, pay attention to the differential is adverse drug reaction, infection increase or new rejection; impairment of renal function, according to antimicrobial drug pharmacokinetic characteristics of antibacterial drug doses adjusted, so as to prevent any possible adverse drug reaction influence disease progression. In addition, according to the corresponding drug characteristics, more attentionshouled be paid to adverse drug reactions and interaction.
The characters of infections after heart transplantation: prevention and management

Abstract:Objective To explore the incidence and etiology of infection after heart transplantation and discuss the prophylaxis and management. Methods Retrospective study was conducted on the 140 survival heart transplant recipients (HTs) with detail follow-up information. All patients received preventive therapy against bacterial infections postoperatively, of which 87. 8% patients used antiviral drugs to prevent cytomegalovirus (CMV) infection. The immuosuppressive regiment was as follows: tacrolimus (Tac) or cyclosporine A (CsA), mycophenolate mofetil (MMF),glucocorticoid. Postoperatively all patients had throat swabs, sputum smear Gram stain, sputum culture, blood culture, urine culture and fecal culture for microbiological monitoring. Schedule tables were made to record and analyze the demography of the patients and the timetable of infections. Results The incidence of infections was 42.9 episodes per 100 HTs. Sixty-four cases (76. 2% ) of the infections occurred in the first month after transplantation and all recovered, 20 cases (23.8%) took place after 1 months, and 4 patients died. Conclusion Perioperative infections are mostly caused by bacteria and the prognosis is good. Infections 2 months after operation are commonly caused by virus or fungi with poor prognosis.

Abstract:Immunosuppressive therapy is improving the graft survival of kidney transplant recipients and increasing the potential risk of infection. Pulmonary mucormycosis is a rare post-operative infection complication characterized with rapid deterioration and high mortality. In this case, a 33-year-old patient underwent a kidney transplantation with regular immunosuppressive therapy. Soon, 38 days post-transplant, pulmonary patchy shadows can be seen in the radiological examination and rounded into a large cavity formation with splenic rupture 25 days later. The diagnosis of mucormycosis was confirmed by lung biopsy and spleen histopathology. This case is a reminder that early diagnosis is imperative, meanwhile, rational antifungal therapy, timely elimination of immunosuppressants, and alternatively, abandoning the graft should be prudently assessed in the treatment of mucormycosis.

Abstract:Objectives Immunotherapy has come to play an increasingly important role in cancer treatment. Accordingly, immune-related adverse events (irAEs) have drawn considerable attention. In this case, a young female patient developed immune-related toxic epidermal necrolysis (TEN). The same irAEs have been rarely reported in previous studies. In this study, we describe the treatment and care methods used in this case in detail in order to provide a reference for clinical practice. Methods After being diagnosed with TEN, the patient accepted systemic glucocorticoid therapy, timely care of skin and mucous membranes, nutrition support, antiacid therapy, anti-inflammatory, analgesics and other supportive measures. Results The patient's skin recovered completely, and no serious adverse outcomes, such as secondary infection or multiple organ failure, occurred during treatment. Conclusion Medical staff should be able to identify the performance of rare irAEs such as TENs and actively explore comprehensive treatments to ensure patient safety and avoid adverse outcomes.

Abstract:Background: Acute graft-versus-host disease (aGVHD) is a severe and fatal complication after orthotopic liver transplantation (OLT). Clinical manifestations of severe aGVHD can resemble drug-induced Stevens–Johnson syndrome (SJS)/toxic epidermal necrolysis (TEN), and there are also various medications, such as antibiotics and immunosuppressants, used after transplantation, causing a diagnostic dilemma. Furthermore, there have been no standardized diagnostic and therapeutic strategies for OLT-aGVHD due to its rarity. Case summary: A 52-year-old man presented with generalized maculopapular eruptions, fever, and pancytopenia 1 month after OLT and 4 days after taking sulfamethoxazole/trimethoprim. After assessment of the scoring criteria for drug causality of drug allergy, histopathological findings of skin biopsy, lymphocyte activation test of the potential offending drug, and microchimerism study, the diagnosis was in favor of aGVHD mimicking SJS/TEN. Considering severe sepsis, the anti-tumor necrosis factor alpha (TNF-α) agent, etanercept, was used to replace tacrolimus and corticosteroid. Skin lesions resolved gradually after anti-TNF-α biologics rescue; tacrolimus and corticosteroid therapy were re-administrated after controlling sepsis. Pancytopenia recovered and the patient was discharged in a stable condition. Conclusion: We demonstrated a diagnostic strategy for OLT-aGVHD. Targeting therapy with anti-TNF-α blockade and a temporary withdrawal of traditional immunosuppressants may be among effective and safe therapeutic options of OLT-aGVHD for those with severe sepsis.

Abstract:Infection after renal transplantation remains a major cause of morbidity and death, especially infection from the extensively drug-resistant bacteria, A. baumannii. A total of fourteen A. baumannii isolates were isolated from the donors' preserved fluid from DCD (donation after cardiac death) renal transplantation and four isolates in the recipients' draining liquid at the Kidney Disease Center, The First Affiliated Hospital, College of Medicine, Zhejiang University, from March 2013 to November 2014. An outbreak of A. baumannii emerging after DCD renal transplantation was tracked to understand the transmission of the pathogen. PFGE displayed similar DNA patterns between isolates from the same hospital. Antimicrobial susceptibility tests against thirteen antimicrobial agents were determined using the K-B diffusion method and eTest. Whole-genome sequencing was applied to investigate the genetic relationship of the isolates. With the clinical data and research results, we concluded that the A. baumannii isolates 3R1 and 3R2 was probably transmitted from the donor who acquired the bacteria during his stay in the ICU, while isolate 4R1 was transmitted from 3R1 and 3R2 via medical manipulation. This study demonstrated the value of integration of clinical profiles with molecular methods in outbreak investigation and their importance in controlling infection and preventing serious complications after DCD transplantation.

Abstract:Objective: To investigate the opportunistic infection onset, organism and treatment after perioperation in heart transplantation (HTx). Methods: A retrospective analysis was carried out on the clinical data of 16 cases suffered from after opportunistic perioperation infection among 284 cases of HTx during May 2000 to Dec., 2012. Results: The opportunistic infection generally occurred frequently in the lung after HTx 2 to 6 months, and the organisms included virus and fungal. Sixteen patients cured and 3 died after effective antiorganism treatment, prophylaxis against nosocomial infection, modified the immunosuppressive drugs strategy and enhanced the respairation support. Conclusions: Mortality was high after HTx opportunistic infection. Early diagnosis, rational and effective treatment plays an important role in improving the cure rate.

Abstract:Objective To conclude the experiences of the diagnosis and treatment of toxic epidermal necrolysis(TEN) after allogenic peripheral blood stem cell transplantation.Methods 3 cases developing acute graft versus host disease after hemopoietic stem cell transplantation were analysed ,whose clinical manifestation was toxic epidermal necrolysis.Results Toxic epidermal necrolysis appeared after peripheral blood stem cell transplantation and showed blister and epidemidolysis,combined with hyperpyrexia,enteritis or hepatic damage.Skin destruction recovered after the usage of effective immunosuppressive agent,associated with nutritional support and skin attendance.Conclusion TEN is a serious type of graft versus host disease.Immunosuppressive methods aimed at acute graft versus host disease is the key of the comprehensive therapy.

Abstract:In 1967, the first human allogeneic orthotopic heart transplantation (HT) was successfully performed in Cape Town, South Africa. However, the long-term survival rate was very low due to problems which included immunological rejection and infection after transplantation. [ 1 ] With the enhancement of surgical techniques, donor preservation, and immunosuppression programs in recent decades, the prognosis of HT and its clinical application has improved. At present, HT is the preferred treatment for end-stage heart disease. [ 2 ] However, there is still a series of serious complications that threaten HT recipients' quality of life after surgery.

Care for the organ transplant recipient on the intensive care unit

Abstract:All transplant recipients receive tacrolimus, mycophenolate and glucocorticoids and these drugs have many side-effects and drug-drug interactions. Common complications include surgical complications, infections, rejection and acute kidney injury. Infections as CMV and PJP can be prevented with prophylactic treatment. Given the complexity of organ transplant recipients a multi-disciplinary team of intensivists, surgeons, pharmacists and transplant specialists is essential. After heart transplantation a temporary pacemaker is required until the conduction system recovers. Stiffening of the heart and increased cardiac markers indicate rejection. An endomyocardial biopsy is performed via the right jugular vein, necessitating its preservation. For lung transplant patients, early intervention for aspiration is warranted to prevent chronic rejection. Risk of any infection is high, requiring active surveillance and intensive treatment, mainly of fungal infections. The liver is immunotolerant requiring lower immunosuppression. Transplantation surgery is often accompanied by massive blood loss and coagulopathy. Other complications include portal vein or hepatic artery thrombosis and biliary leakage or stenosis. Kidney transplant recipients have a high risk of cardiovascular disease and posttransplant anemia should be treated liberally. After postmortal transplantation, delayed graft function is common and dialysis is continued. Ureteral anastomosis complications can be diagnosed with ultrasound.

Abstract:Journal of International Medical Research, Volume 52, Issue 1, January 2024. Toxic epidermal necrolysis (TEN) is a rare severe cutaneous adverse reaction that involves more than 30% of the body surface area. TEN can be accompanied by a series of systemic symptoms and has a high risk of death. Tumor necrosis factor (TNF)-α inhibitors such as adalimumab and etanercept have been shown to be safe and effective for the treatment of TEN in some cases. However, clinical data on the use of TNF-α inhibitors to treat TEN with severe systemic infection are scarce. In the present study, three adult patients who developed TEN with serious active infection were successfully treated with etanercept. One of the three patients had active open pulmonary tuberculosis, and the other two had septicemia and/or fungal sepsis. All patients' skin lesions significantly improved after several days, and none of the patients developed emerging or re-emerging infectious diseases, adverse reactions, or a similar rash during follow-up. TNF-α inhibitors may be an effective treatment choice for TEN with severe systemic infection. However, further studies with large samples are still required for validation because clinical experience is limited.

Sepsis Resulting from Enterobacter Aerogenes Resistant to Carbapenems after Liver Transplantation.

Abstract:BACKGROUND: Sepsis due to Enterobacter aerogenes (E. aerogenes) is rare after liver transplantation but is also a serious infection that may cause liver abscess. The purpose of this case report is to relate an unusual presentation of liver transplantation to show how successive treatment can be an appropriate option in septic patients after liver transplantation.METHOD: We report on a patient with liver transplantation who developed sepsis due to extended spectrum beta-lactamases and AmpC-producing E. aerogenes.RESULTS: A 39-year-old man had a biliary fistula and then was found to have multiple liver abscesses through abdominal ultrasound and an abdominal computed tomography scan, and carbapenem-sensitive E. aerogenes infection was confirmed. The patient was not successfully treated with conservative treatment consisting of intravenous carbapenems, percutaneous transhepatic cholangial drainage, and biliary stent placement by endoscopic retrograde cholangiopancreatography, so a second liver transplantation followed. Carbapenem-resistant E. aerogenes was detected in bile and blood after a five-week course of carbapenem therapy. The patient developed septic shock and multiple organ dysfunction syndrome.CONCLUSIONS: We first report an unusual case of sepsis caused by E. aerogenes after liver transplantation in China. Carbapenem-resistant E. aerogenes finally leads to uncontrolled sepsis with current antibiotics. We hypothesize that the infection developed as a result of biliary fistula and predisposing immunosuppressive agent therapy. Further research is progressing on the aspect of immunomodulation therapy. Abstract:Toxic epidermal necrolysis (TEN) and Stevens–Johnson syndrome (SJS) are acute inflammatory skin adverse reactions characterized by epidermal exfoliation and multi-site mucositis and are considered medical emergencies. The risk factors for SJS/TEN include immune disorders, malignancy, and genetic susceptibility. In most cases, medication is considered to be the leading cause of TEN. In addition, several studies suggest that infections, such as the herpes simplex virus, human immunodeficiency virus (HIV), Mycoplasma pneumoniae , streptococcus, and meningococcus infections, can trigger the occurrence of SJS/TEN. In this rare case, we share our experience managing TEN in a hepatitis A virus infection with an acute-on-chronic liver failure patient. A 38-year-old man was infected with hepatitis A virus on the basis of liver cirrhosis and progressed to acute-on-chronic liver failure. As the infection progressed, the target-like skin lesions accompanied by mucosal involvement worsened. The condition of the patient progressively worsened with a severe generalized rash, bullae, and epidermal detachment accompanied by severe erosive mucosal lesions. His skin detachment area gradually involved 30% of the body surface area (BSA), and the disease progressed to TEN. The intravenous infusion of corticosteroids alleviated the patient's hypersensitivity, and the patient obtained lasting remission without severe adverse reactions and complications.

We present a clinical case where an oXiris hemofilter was used for continuous renal replacement therapy (CRRT) treatment in a patient with septic shock after liver transplantation. A 35-year-old man with a 20-year history of hepatitis B presented with jaundice, loss of appetite, and decreased urine output. He was diagnosed with decompensated cirrhosis with acute-on-chronic liver failure, and liver transplantation was indicated. The day after surgery, he developed hyperthermia, hypotension, anuria, and a progressive increase in blood inflammatory markers and creatinine. Combined with the donor source and blood culture results, septic shock after transplantation was considered. The patient was immediately treated with endotoxin and cytokine adsorption CRRT (oXiris hemofilter) with tigecycline, caspofungin, and ganciclovir as anti-infectives. After 48 hours on CRRT, his blood pressure gradually stabilized, the CLIF Consortium Acute-on-Chronic Liver Failure score decreased from 63 to 43. Procalcitonin, endotoxin, and the inflammatory factors interleukin (IL)-6 and IL-10 also decreased gradually. The patient's liver and kidney functions were completely restored. Our experience suggests that oXiris CRRT combined with antibacterial therapy is an effective treatment for septic shock after liver transplantation.

Abstract:Acute respiratory distress syndrome (ARDS) caused by pneumonia after renal transplantation was usually associated with overimmunosuppression and high mortality rate. We evaluated the efficacy of an aggressive systemic protocol including strategies improving body's immune function. Twenty-one recipients were enrolled in this study. Patients were subjected to a protocol including (i) withdrawal of most immunosuppressants, (ii) early use of immunoenhancers and continuous renal replacement therapy (CRRT), (iii) reasonable administration of antibiotic regimen, (iv) prompt mechanical ventilating strategy, and (v) adequate nutrition. Immunosuppressants were adjusted according to the value of CD4+, CD8+T lymphocytes in peripheral blood. CRRT was conducted at once when patients were admitted to the intensive care unit (ICU), regardless the graft function. Thirteen (62%) survived and eight died finally. This is a high survival rate for this kind of patients. Eighteen patients had received thymosin treatment. All patients who survived experienced renal allograft dysfunction during CRRT, but when CRRT stopped, the function of all grafts gradually recovered. No acute rejection episodes were documented during the treatment. The aggressive systemic protocol including strategies improving the body's immune function and CRRT can improve the outcome of patients with ARDS after renal transplantation. The count of CD4+, CD8+T lymphocytes of peripheral blood is useful in the adjustment of immunosuppressants and the prediction of patient outcome.
