Abstract:Background: Hematopoietic stem cell transplantation (HSCT) is the only cure for acute myeloid leukemia (AML), but relapse is the most important cause of transplant failure. Patients with remission but measurable residual disease (MRD) positive have a higher probability of hematologic relapse. The MRD-directed preemptive treatments can spare some patients in remission from further therapy. The conventional first-line preemptive treatment has been ineffective in some patients, so it's necessary to find a new preemptive treatment to prevent relapse and improve the prognosis. Our study aimed to evaluate the efficacy and safety of selinexor-containing regimens in the preemptive treatment of MRD positive in post-transplant patients with AML or myelodysplastic syndrome (MDS). Methods: The study was conducted in Tongji Hospital from March 2022 to May 2023. The inclusion criteria of post-transplant AML/MDS patients included the followings: (a) Persistent MRD positive. (b) MRD turned negative after first-line preemptive treatment but was positive again. (c) Patients at high risk for hematologic relapse after transplantation; (d) AML-M4/5. The end of the follow-up was in June 2023. Bone marrow aspiration was assessed for MRD using multiparameter flow cytometry (MFC) combined with real-time quantitative polymerase chain reaction (RT-qPCR) as follows: 1) MRD assessed by MFC: MRD negativity was defined as <0.1%. 2) MRD assessed by RT-qPCR: Abnormal genes associated with prognosis were used for molecular MRD detection. 3) WT1 was used for molecular MRD assessment if no molecular marker was available at diagnosis. MRD negativity was defined as <0.6%. Results: A total of 11 patients were included in the study. There were 10 patients with AML and one patient with MDS, with a median age of 43 years old. Two patients had received autologous HSCT, nine patients had received allogeneic HSCT, of which two matched sibling donor, one matched unrelated donor, and six were haploidentical donor. All patients were WT1 positive and two of the cases were both WT1 and MFC positive. Seven patients with post-transplant MRD positive were treated with selinexor-containing regimens after first-line preemptive therapy (azacytidine, azacytidine combined with venetoclax, decitabine combined with venetoclax, etc.) failed or MRD positive again. The remaining four patients were treated directly with selinexor-containing regimens for personal reasons or with a diagnosis of AML-M4/5 may be resistant to venetoclax. The selinexor-containing regimens used in this study were either selinexor (35mg/m 2 biw) alone or in combination with other drugs (decitabine, azacytidine, venetoclax, low-dose cytarabine, and CAG (cytarabine, aclarubicin, G-CSF) regimen). MRD was evaluated after two weeks of the selinexor-containing regimens. And selinexor 20 to 40 mg per week was used as maintenance therapy after MRD converted to negative. Eight patients (72.7%) successfully cleared the MRD. Of the three patients who didn't clear the MRD, two patients successfully converted to negative after continuing the selinexor-containing regimens. It is worth mentioning that 7 of the 11 patients were treated with the selinexor-containing regimens after first-linepreemptive treatment failure, and six of them had successfully cleared the MRD (71.4%). At the end of follow-up, 10 (90.9%) patients survived and one (9.1%) died due to disease progression. In terms of hematological adverse events (AEs), grade 3-4 neutropenia occurred in nine (81.8%) patients, the median duration was 2 days (IQR 1,4). Grade 3-4 thrombocytopenia occurred in three (27.3%) patients, and the median duration was 5 days (range 4-7). Although the number of people with grade 3-4 neutropenia was large, the duration was short after using granulocyte colony-stimulating factor, and no patient developed a severe infection. Most of the non-hematological AEs were mild, including nausea (100.0%), fatigue (63.6%), hyponatremia (58.3%), and vomiting (45.5%). The AEs can be alleviated after symptomatic treatment and no patient discontinued treatment. Conclusion: Selinexor-containing regimens have been shown to be effective and well tolerated in post-transplant AML/MDS patients with MRD-positive, including those who have failed first-line preemptive treatment.

Abstract:Topic: 4. Acute myeloid leukemia - Clinical Background: Acute myeloid leukemia（AML）is primarily a disease of the elderly with a median age of 67 years, and usually manifests at onset with complicated comorbidities or severe infections. Such patients cannot tolerate intensive chemotherapy, even the standard dosing of azacytidine plus venetoclax, and are usually excluded from clinical trials. Selinexor has proved its tolerability and clinical activity in AML in a few phase 1/2 trials. Aims: To report the efficacy and safety of eight cases of AML patients ineligible for intensive therapy who were treated with triple regimen including selinexor, venetoclax and hypomethylating agents in our center. Methods: Data of eight patients with AML who received a triple regimen of selinexor, venetoclax and hypomethylating agents are retrospectively collected from December, 2021. Selinexor was given at 60mg/day at day 3, 10, 17 for 28 days a cycle. The dose of venetoclax was 100mg day 1, 200mg day 2, and 400mg from day 3 to day 14, the dosing duration adjusted by the physicians from 5 days to 14 days. Hypomethylating agents, either decitabine (0.1-0.2mg/kg) or azacytidine (75mg/m2) were administered with intermittent dosing schedule day 1-3, day 8-9(10), and/or day 15-16(17). The triple regimen was approved by the ethics committees of Shanghai Tongren Hospital, and all patients provided written informed consent. Results: The median age of 8 patients was 67 years with a range of 51-76 years. One patient previously received one cycle of idarubicin and cytarabine (IA) without response and other 7 patients were newly diagnosed AML patients. Five patients had an Eastern Cooperative Oncology Group (ECOG) performance-status score of 4 with pulmonary infections requiring intravenous intervention. Among them, co-morbidities also included diabetes with ketoacidosis, multiple subacute cerebral infarctions, COVID-19 pneumonia and advanced colon cancer. Seven patients were classified as adverse-risk group and one classified into intermediate group based on the 2022 ELN risk stratification. The complete remission plus complete remission with incomplete hematologic recovery rate (CR+CRi) was 100% (8/8). The median follow-up time was 5 months (range: 1-18), with no responders experiencing relapses and all 8 patients alive. Patient 5 didn’t respond after cycle 1, and avapritinib was added on in cycle 2 because of the c-Kit mutation detected, and then achieved CRi. Patient 1 achieved CR after two cycles of the triple regimen and one cycle of high dose cytarabine consolidation and then underwent allogeneic hematopoietic stem cell transplantation (allo-HSCT). Two patients had myelosuppression grades 3/4, and two patients developed left ventricular failure and acute pulmonary embolism, respectively, which were considered due to their underlying conditions and recovered from supportive care. The common non-hematological adverse events were mainly grade 1 or 2 including fatigue, nausea, and vomiting. Summary/Conclusion: These results imply that the triple regimen of selinexor, venetoclax and hypomethylating agents are potentially highly effective in AML patients, even for those with serious infections or other comorbidities. Venetoclax Plus Cytarabine and Azacitidine in Relapsed/Refractory AML: an Open-Label, Single-arm, Phase 2 Study.

Abstract:BackgroundThe outcome of relapsed/refractory (R/R) acute myeloid leukemia (AML) remains extremely poor. Venetoclax (VEN)-based regimens have shown promise in treating R/R AML.ObjectiveThis phase 2 study aimed to systematically evaluate the efficacy and safety of the VAA regimen (VEN plus Cytarabine and Azacitidine) in R/R AML patients.MethodsThirty R/R AML patients were enrolled. The study adopted a stepwise ramp-up of VEN dosing, starting with 100 mg on day 1, escalating to 200 mg on day 2, and reaching 400 mg from day 3 to day 9. Cytarabine (10 mg/m2, q12h) was administered intravenously twice daily from days 1 to 10, and Azacitidine (75 mg/m2) was administered via subcutaneous injection once daily from days 1-7. The primary efficacy endpoint was the composite complete remission rate (CRc), including complete response (CR) and complete response with incomplete blood count recovery (CRi). Secondary endpoints included overall survival (OS), duration of response (DOR), and safety analysis.ResultsThe CRc rate was 63.3% (19/30), with CR in 36.7% of patients and CRi in 26.7%. Notably, 14 (73.7%) of 19 patients achieving CRc showed undetectable measurable residual disease by flow cytometry. With a median follow-up of 10.7 months, the median OS had not been reached, and the median DOR was 18.3 months. The most common grade 3-4 adverse events (AEs) were neutropenia (100%), anemia (96.7%), thrombocytopenia (90.0%), and leukopenia (90.0%). Infections, with pneumonia being the most prevalent (43.3%), were observed, including one fatal case of Pseudomonas aeruginosa septicemia. There were no treatment-related deaths.ConclusionThe VAA regimen is an effective and safe option for patients with R/R AML, demonstrating a high CRc rate and manageable safety profile.
15-Days Duration of Venetoclax Combined with Azacitidine in the Treatment of Relapsed/Refractory High-Risk Myelodysplastic Syndromes: A Retrospective Single-Center Study

Abstract:The treatment of patients with refractory and/or relapsed (R/R) high-risk myelodysplastic syndrome (HR-MDS) remains a daunting clinical challenge. Venetoclax is a selective BCL-2 inhibitor, which combined with hypomethylating agents (HMAs), increased responses and prolonged survival in unfit and previously untreated acute myeloid leukemia. We performed a retrospective study of patients with R/R HR-MDS receiving combination azacytidine (AZA) plus 15-days duration of venetoclax (VEN-15d) in order to determine their efficacy and toxicity in this context. We showed that the overall response rate was 57.2% (20/35) and the median over survival was 14 months in R/R MDS. The most common treatment-emergent adverse events were peripheral blood cytopenias and infectious complications. Our retrospective study showed that the real-world experience of treating R/R MDS with AZA plus VEN-15d highlights an encouraging response rate with myelosuppression being the major toxicity. Of note, VEN-15d with AZA may salvage patients failing to respond optimally to HMAs and reduce the disease-burden for subsequent allogeneic stem cell transplantation in our analysis. These data of combination AZA plus VEN-15d in R/R MDS warrant further prospective evaluation in clinical trials.

Abstract:Introduction: Treatment of relapsed or refractory acute myeloid leukemia (R/R AML) and myeloid sarcoma (MS) has presented challenges for decades. Studies on selinexor in combination with various standard or intensive chemotherapy regimens for the treatment of R/R AML have demonstrated promising results. This study aimed to evaluate the efficacy and safety of chemotherapy-free or low-dose chemotherapy regimens with selinexor for R/R AML and MS patients. Methods: Ten patients with R/R AML or MS who received chemotherapy-free or low-dose chemotherapy regimens in combination with selinexor at Tongji Hospital from October 2021 to August 2022 were included in this study. The primary endpoint was overall response rate (ORR) and secondary endpoints included complete remission (CR), CR with incomplete hematological recovery (CRi), partial remission (PR), transplantation rate, and safety. Results: All patients were evaluable for response, achieving CR in four (40.0%) patients and CRi in two (20.0%) patients for a total CR/CRi of 60.0%. The ORR was 80.0% when patients with PR were included. Five (50.0%) patients underwent allogeneic hematopoietic stem cell transplantation (allo-HSCT) after treatment with selinexor-containing regimens. At the end of the follow-up, seven (70.0%) patients were alive, and three patients died of transplant-related complications or disease progression. The most frequently reported nonhematologic adverse events (AEs) in patients were grade 1 or 2 asymptomatic hyponatremia. Conclusion: The chemotherapy-free or low-dose chemotherapy regimens in combination with selinexor for R/R AML are feasible and tolerable and provide an opportunity for patients to receive transplantation.

Combining Selinexor with R-CHOP in the Treatment of Newly Diagnosed High-Risk (IPI 3-5) GCB Subtype DLBCL Patients: A Multicenter, Open Label Phase 2 Study

Abstract:Background Diffuse large B-cell lymphoma (DLBCL) is an aggressive and highly heterogeneous lymphoma, and for high-risk group who has an International Prognostic Index (IPI) score of 3-5 it is more difficult to treat (Teras LR,2016; Ziepert M,2010). Under the standard R-CHOP-like immunochemotherapy, the 3-year overall survival (OS) rate of patients with IPI score of 0-1, 2, 3, 4-5 were 91%, 81%, 65% and 59%, respectively (Ziepert M,2010). The efficacy for high-risk population (IPI 3-5) was limited even after intensive chemotherapy, with an ORR of 74.5% and CR of 47.3% under R-CODOX-M/R-IVAC (McMillan AK,2020). Nonetheless, adding new agents to R-CHOP shows better efficacy in patients with IPI 3-5(Tilly H,2022; Nowakowski GS,2021). GCB subtype has better prognosis, but 20% of patients still relapsed after R-CHOP therapy (Nowakowski GS,2015), and no more benefit was achieved by modifying regimen with new agents like polatuzumab-R-CHP (Tilly H,2022), adding on lenalidomide (Nowakowski GS,2015) or ibrutinib (Wilson WH,2021). Selinexor is the only approved oral selective nuclear export inhibitor which demonstrated a 34% ORR in GCB subgroup as a monotherapy in relapsed/refractory DLBCL (Kalakonda N,2020). Selinexor combined with R-CHOP showed a CR rate of 58.8% and ORR of 94.1% in 17 high risk GCB DLBCL patients in a real-world setting (Zhiming Li,2023). This phase 2 study (NCT05422066) further explored the benefit and safety of Selinexor plus R-CHOP in the treatment of newly diagnosed (ND) GCB DLBCL patients with IPI 3-5. Methods This is a multicenter, open label, single arm phase 2 study enrolled 18~75 years old ND GCB DLBCL patients with IPI 3-5. Patients were administered selinexor 60mg QW in combination with standard R-CHOP therapy for 6 cycles (Rituximab 375 mg/m 2, cyclophosphamide 750 mg/m 2, doxorubicin 50 mg/m 2 IV, vincristine 0.5 mg/kg on day 1, prednisone 100 mg PO on days 1-5 in a 21-day cycle). The primary endpoint is complete remission (CR) rate. Secondary endpoints include objective response Rate (ORR), progression free survival (PFS), disease free survival (DFS), overall survival (OS), safety and tolerability. Results The median age of 15 patients was 59 years (range 25-71), with 10 males (66.7%). All patients were in advanced stage according to Ann Arbor staging system, with 13 patients in stage IV, and 2 in stage III. 11(73.3%) patients had ≥2 extranodal lesions and 9 (60%) patients had higher LDH level at baseline. 1 patient had TP53 mutation, and 6 patients (40%) had double expressor lymphoma (DEL) (Table 1). The overall best ORR and CR were 100% and 60%, respectively. The CR rate in 6 DEL patients were 50%; Patients with ≥2 extranodal lesions reached a CR of 54.5% (n=11), while 3 out of 4 patients with≤1 extranodal lesions completely responded. The CR rates of patients with Ki-67≥80% vs<80% were 50% vs 62.5%, respectively. Median PFS and OS were not reached yet with a median follow-up of 8 months (range 2-11 months). 2 patients had progressive disease with 1 progressed after C4 and the other 1 after C6. 4 patients with viral hepatitis (3 HBV, 1 HCV) were administered both anti-tumor and antiviral therapy, and their HBV-DNA and HCV-RNA levels continuously declined (till undetectable) along with their tumor load (3 CR ,1 PR). Common treatment-relate adverse events (grade 3/4, all grades) included: leukopenia (80%, 93%), neutropenia (73%, 87%), anemia (27%, 93%), thrombocytopenia (27%, 67%), lymphopenia (20%, 27%), anorexia (13%, 40%), pneumonia (13%,40%), hypertriglyceridemia (7%, 40%), diarrhea (7%, 20%). All AEs were managed with appropriate supportive care and dose adjustments. 8 out of 15 patients had a dose reduction of selinexor to 40mg (initial dose 60mg QW) due to AE, 3 in C2 and the other 5 in C4-C5. All patients were continuously treated after dose reduction, and no treatment-related deaths occurred. Overall, 6 patients completed 6 cycles of selinexor+R-CHOP treatment with sustained remission, and 6 patients were still receiving therapy at time of writing this abstract (4 in C6, 2 in C3). 3 patients ended study, 2 PD and 1 withdrew informed consent after PR at the end of C4 (Figure 1). Conclusion Once weekly 60mg selinexor can be safely combined with R-CHOP in ND high risk (IPI 3-5) GCB subtype DLBCL patients with ORR of 100% and CR of 60%. No viral reactivation was observed in patients with viral hepatitis with simultaneous antiviral therapy.

Abstract:Background: Atypical chronic myeloid leukemia (aCML) is a BCR::ABL1 negative myelodysplastic/myeloproliferative neoplasm with poor overall survival. Some patients can be treated by allogeneic hematopoietic stem cell transplantation (allo-HSCT) from suitable donors. The effectiveness of decitabine or azacitidine (AZA) has recently been reported; however, their combined efficacy with selinexor has not yet been reported. Case description: In this study, we report the case of a patient with aCML who was successfully treated with selinexor combined with AZA. A 67-year-old man with a history of gastric mucosa-associated lymphoid tissue (MALT) lymphoma was admitted to the hospital with fatigue and emaciation. He was diagnosed with aCML and no longer responded to decitabine treatment after undergoing seven cycles. The patient was subsequently administered hydroxyurea (HU), selinexor, and AZA. After four courses of combination therapy, his blood cell counts improved; he no longer required transfusions and was able to discontinue HU. The patient continued receiving selinexor and AZA without severe complications. This case is the first to show that combinatorial selinexor and AZA therapy can effectively treat aCML. Conclusion: Our case sheds light on the importance of selinexor and AZA combined therapy in the exploration of new treatment strategies for aCML. Moreover, this treatment approach offers the possibility of bridging with allo-HSCT.

Abstract:7025 Background: While the understanding of AML has been improved over decades, the outcome of relapsed/refractory AML remains dismal. Venetoclax-based regimen has not yet been systematically assessed in R/R AML. Clinical trials of VEN-based regimen designed for R/R AML are urgently needed to further evaluate its efficacy and safety among these patients. We innovatively combined VEN with azacytidine and cytarabine (termed as VAA). Methods: a) Patients: Eligible patients aged 18-70 years and had a confirmed diagnosis of relapsed/refractory AML. b) Trial design and regimens: Phase 2, open-label, single-arm, multicenter study of VAA regimen in R/R AML patients. A stepwise ramp-up of VEN dosing was adopted to achieve the target dose (400 mg), beginning with 100mg on day 1, and escalated to 200mg on day 2, and 400 mg on day 3-day 9. Cytarabine (10mg/m2) was administered by subcutaneous injection twice daily from day 1 to 10, and AZA (75mg/m2) was administered by intravenous injection once daily from day 1 to 7. c) End points and assessments: Primary efficacy endpoints were CRc including CR and CRi, PR and MLFS per IWG for AML. Secondary endpoints were OS and DOR. The safety endpoints were adverse events graded per CTCAE. d) Statistics analysis: As of the date of submission, 29 patients have completed efficacy evaluation and 1 patient is still in the trial. This study was designed to detect a target CRc rate of 65% (historical rate 40%), with 80% power and type 1 error rate at 0.05, and the planned sample size was 31 patients. All P values were two sided. Results: a) Patients: 30 R/R AML patients were enrolled.b) Efficacy: The CRc rate among all patients was achieved in 62.1% (18/29), including CR in 34.5% and CRi in 27.6% patients, following with 13.8% patients obtaining PR. CRc rates were preserved across subgroups with 59.1% (13/22) in ELN intermediate/adverse group, 62.5% (5/8) in those who ≥65 years and 50.0% (7/14) in those whose baseline bone marrow count ≥30%. Noticeably, CRc rate in patients diagnosed with M4/5 reached 72.7% (8/11). With a median follow-up of 6.95 months, median OS and median DOR are not yet reached. Estimated 12-month OS was 86%, and Estimated 12-month DOR was 62.6%. In ELN intermedia/adverse risk patients, the median DOR was 7.5 months. Estimated 12-month OS rate and DOR rate were 84.3% and 63.8%, respectively. Patients with prior bone marrow count less than 30% and with no prior HMA-based therapy were associated with favorable estimated 12-months DOR rate. The regimen had similar efficacy in patients younger than 65 yeas compared with those >65 years. c) Safety: No TLS events were observed. No treatment-related deaths occurred. Conclusions: VAA regimen represents a feasible and safe new choice for R/R AML patients, especially with satisfactory effect in ELN intermediate/adverse group and in those with advanced ages. Abstract:Background: Adult patients with acute myeloid leukemia (AML) have unsatisfactory complete remission (CR) and clinical outcomes. Venetoclax combined with azacytidine or low lose cytarabine has showed definite efficacy in elderly AML patients.Methods: We conducted a two-stage, phase 2 clinical trial to test whether venetoclax added to daunorubicin + cytarabine induction treatment could increase CR rate from 65% to more than 85% after one cycle of therapy. We enrolled 33 adult AML patients (18-60 years old). Induction treatment included daunorubicin (60 mg/m2, days 1-3), cytarabine (100 mg/m2, days 1-7) and venetoclax (100 mg day 4, 200 mg day 5, and 400 mg days 6-11) (DAV regimen). The primary endpoint was composite complete remission rate.Findings: The CR rate after one cycle of DAV regimen was 91% (95% confidence interval, 76 to 98) in 33 patients, and 100%, 83%, and 75% in ELN favorable, intermediate and poor-risk groups. 29 (97%) of 30 patients who achieved CR had undetectable measurable residual disease (MRD<0.1%). Grade 3 and 4 adverse events included neutropenia in 33 (100%), thrombocytopenia in 33 (100%), anemia in 33 (100%), febrile neutropenia in 28 (54%), pneumonia in 7 (21%), and sepsis in 4 (12%) patients. A rapid recovery of myelosuppression was found. With a median follow-up of 6 months, 30 patients were in continuous CR (29 with MRD-negative).Interpretation: DAV regimen represents an effective induction treatment for newly diagnosed adult AML patients, which was associated with high and deep remission, and rapid recovery of myelosuppression.Trial Registration Details: Clinical Trial Registry Number: ChiCTR2000041509. Efficacy and Safety of Selinexor-Based Combination Therapy in CNS Lymphoma: A Retrospective Experience

Abstract:Introduction ： Primary central nervous system lymphoma (PCNSL) is a rare diffuse large B-cell lymphoma (DLBCL) with distinct clinical and biological features, characterized by an aggressive course and poor prognosis. Patients with relapsed/refractory (R/R) disease have an unmet medical need due to a median overall survival (OS) of only 6.8 months. Novel therapies are urgently required for patients with R/R CNSL. In June 2020, the U.S. Food and Drug Administration granted accelerated approval to selinexor for the treatment of adult patients with R/R DLBCL. Selinexor has demonstrated central nervous system (CNS) penetration and activity in secondary CNS lymphoma (SCNSL) patients. Thus, we conducted a retrospective review of PCNSL/SCNSL cases treated with a selinexor-based regimen to preliminarily evaluate its effectiveness and safety. Methods: We retrospectively analyzed the medical records of 13 patients with CNSL who received selinexor-based combination therapy between June 2021 and June 2023 at three research centers in China. Among these cases, 2 were previously untreated, and 11 were relapsed or refractory patients. All patients had histopathologically confirmed DLBCL at the time of initial diagnosis. Data collection included baseline characteristics, treatment efficacy and safety evaluation. Results: A total of 13 patients (10 PCNSL and 3 SCNSL) were included in the study, with a median age of 62 years (range, 28-73), including three male patients. The median international extranodal lymphoma study group (IELSG) score for PCNSL cases was 3 (range, 1-3). The median International Prognosis Index (IPI) score for the three systemic DLBCL cases was 5 (range, 3-5), and three cases were at Ann Arbor stage IV. Among the patients, 11 had relapsed or refractory disease with a median of 2 prior treatments (range, 1-4), and 10 of them were refractory to previous treatments. The main patient characteristics, responses, and toxicities of the selinexor-based combination regimen are summarized in Table 1. Figure 1 presents the response of all evaluable patients. The overall response rate (ORR) of the patients was 84.6% (11 /13), with a complete response (CR) rate of 76.9% (10/13). Both treatment-naïve patients achieved CR after four and six cycles of the selinexor-containing first-line regimen, respectively. They are currently experiencing sustained remission, with the longest duration reaching 13 months. For relapsed/refractory CNSL, the ORR and CR rate were 81.8% (9/11) and 72.7% (8/11), respectively. Two patients had stable disease (SD). Among the 9 patients who achieved remission, 4 patients are still in remission, with the longest duration reaching 20 months, 2 patients received CART bridging therapy, one received radiation therapy, and 2 patients experienced disease progression after 5 months, but all are currently still alive. These patients are currently under continuous observation. Due to the heterogeneity of the selinexor-based regimens, the categories and severity of adverse events (AEs) varied greatly among the different regimens. The median treatment cycle number was six (range, 2-22). One patient died due to pneumonia infection, one patient was lost to follow-up, three patients successfully bridged to CART/RT, and eight patients completed the treatment as planned. The common AEs included thrombocytopenia, infection, nausea, vomiting, and gastrointestinal bleeding(Table 1). Conclusion: This report provides encouraging evidence of the efficacy and tolerable safety of the selinexor-based regimen in the treatment of CNSL. However, further prospective studies with a larger sample size are needed to provide more comprehensive evidence regarding the efficacy of selinexor in central nervous system lymphoma and to determine the appropriate dosage and optimal combination strategies.v
Selinexor in Combination with R-EPOCH for Patients with Previously Untreated HIV-Associated Diffuse Large B Cell Lymphoma(DLBCL)

Abstract:Introduction : The incidence of HIV-positive lymphoma remains significantly higher than HIV-negative patients in the era of combined antiretroviral therapy(cART). DLBCL is the most common histological type of HIV-positive non-Hodgkin's lymphoma(NHL). Exportin 1 protein(XPO1) inhibitor has demonstrated preliminary anti-HIV activity and synergy with anti-lymphoma drug(e.g. etoposide, doxorubicin, prednisone). Methods: The study was designed to evaluate safety and efficacy of selinexor (60mg Days 1, 8, 15) combined with R-EPOCH(rituximab 375 mg/m2d1, etoposide 50 mg/m2 d1-d4, doxorubicin 10mg/m2 d1-d4, vinblastine 0.4mg/m2 d1-d4, prednisone 60mg/m2 d1-d5 and cyclophosphamide 750mg/m2 d5) every 3 weeks (4-6 cycles), followed by selinexor maintenance (60 mg weekly) or transplantation as per physician's choice in patients with untreated HIV-positive DLBCL. All patients received cART concomitantly during chemotherapy. This study was registered at Chictr.org.cn(ChiCTR2300069941). Results: From Feb 24, 2023 to July 17, 2023, 6 patients were enrolled and received at least one cycles of study treatment. Median age was 55 years (range 43-67) with four patients(66.7%) was male. All six patients were in advanced stage according to Ann Arbor staging system, with aaIPI intermediate to high risk or IPI low to intermediate risk. Four patients(66.7%) were germinal center B-cell-like lymphoma (GCB) subtype and three patients(50%) had an Elevated lactate dehydrogenase level. Median HIV RNA load was 74450(10000-100000) copies/mL and Median CD4 cell count was 233 cells/μL (233 cells/mm3) with one patient having a CD4 cell count less than 200 cells/μL (200 cells/mm3). Two patients were HBV-positive and one was EB virus(EBV) coinfection at baseline. Three patients achieved PR after two treatment cycles, one patient of which had completed five treatment cycles and achieved complete response at the end of the fourth cycle (Figure 1). One patients after interim evaluation with PR stopped treatment because of loss to follow-up. The EBV DNA load of the patient with EBV coinfection at baseline turned to normal level after one treatment cycle. Similar result was observed in two HBV-positive patients, the one achieved HBV-negative after two cycles and the other's HBV DNA load level declined from 7.96×10^4 IU/mL to 1.12×10^3 after completing first cycle. HIV RNA load of the patient with CR had declined to <20 copies/mL from 58300 copies/mL after the fourth cycle, another patient likewise became HIV-negative before the initiation of the third cycle. The change of CD4+T cell count during treatment was presented in Figure 2. All patients experienced at least one treatment-emergent adverse event (TEAE), 3 (50%) patients with at least one Grade≥3 TEAE. The most common grade 3 or 4 adverse events were hematological toxicities including lymphopenia(83.3%), neutropenia (50%), leukopenia(33.3%), thrombocytopenia (33.3%). Two patients experienced Febrile neutropenia. The non-hematological toxicities were grade 1 or 2, including nausea(66.7%), vomit(50%), constipation(33.3%), cough(33%). One patient stopped treatment for one month due to pulmonary infection. All toxicities were manageable by supportive care. Conclusion: Selinexor plus R-EPOCH regimen demonstrated a favorable Interim efficacy and manageable safety profile in patients with HIV-associated DLBCL. The change of HIV RNA load and CD4+ cell count still requires further evaluation.

Safety and Efficacy of Selinexor Sequential Azacytidine in Newly Diagnosed Patients with Myelodysplastic Syndromes EB1 or EB2: A Single-Center, Single-Arm, Phase Ib/II Trial

Abstract:Background: Azacytidine is one of the hypomethylating agents (HMAs) for the treatment of higher-risk myelodysplastic syndromes (MDS). However, clinical outcomes of patients treated with azacytidine monotherapy were far from satisfactory with overall response rate (ORR) of 41%-59% and overall survival (OS) of 15-18 months. Selinexor is an oral, first-in-class selective inhibitor of nuclear export compound that inhibits exportin 1 (XPO1). Preclinical studies have shown that inhibition of XPO1 causes nuclear accumulation of p53 and disruption of NF-κB signaling, both relevant targets for myelodysplastic syndromes. Prior study reported an ORR of 26% and mOS of 8.7 months with selinexor in HMAs-refractory MDS patients. We therefore aimed to assess the safety and efficacy of selinexor sequential azacytidine in newly diagnosed patients with MDS-EB1 or EB2. Methods: The effectiveness of simultaneous and sequential treatment of azacytidine and selinexor was measured by cell death rate, cell cycle and colony formation rate in vitro. Western Blot was performed to detect the sublocation of p53. MOLM-13-luciferase xenograft murine model was established to verify the synergetic effect in vivo. Based on the experiments, we conducted a single-center, single-arm, phase Ib/II trial, which included newly diagnosed patients 18 years or older with MDS-EB1 or EB2. Eligible patients received 2-4 weeks long cycles of oral selinexor (40mg qw for 4 weeks; 40mg biw for 2 weeks; 60mg qw for 4 weeks). The primary endpoints were to find the recommended phase 2 dose (RP2D) and maximum tolerated dose (MTD). The secondary endpoint was ORR including complete remission (CR), partial remission (PR), marrow complete remission (mCR), and hematological improvement (HI). This trial was registered with the Chinese Clinical Trial Registry (#ChiCTR2200065167). Results: First we treated three AML cell lines: AML-OCI2, THP1, MV4-11 and three MDS cell lines: SKM-1, MOLM-13, MDSL with azacitidine and selinexor simultaneously or sequentially. Sequential treatment showed stronger synergetic effect (Figure 1A) and reduced colony formation by 90% in SKM-1 and MOLM-13 (Figure 1B). Cell cycle arrest were induced and the percentage of Annexin V cells obviously increased with sequential treatment (Figure 1C). Sequential treatment decreased leukemia burden and prolonged survival of MOLM-13-luciferase xenograft murine model (Figure 1D, E). Mechanistically, pretreatment of azacitidine on SKM-1 and MOLM-13 upregulated the expression level of p53, and selinexor led to massive nuclear accumulation of p53, resulted in better vulnerability lethal to MDS cells (Figure 1F). Secondly, 15 patients were enrolled in our trial with a median age of 57. 26.7% of patients were MDS-EB1, 73.3% were EB2. 93.3% of patients were high or very high risk by the Revised International Prognositic Scoring System (IPSS-R) (see the Table). At Phase Ib, the following selinexor dosages were administered: 40mg qw (n=3), 40mg biw (n=3) and 60mg qw (n=3). One patient in the 60mg qw group discontinued treatment because of nausea, vomiting, fatigue and Herpes simplex virus infection. Subsequently, in order to find RP2D, an extension cohort of 3 additional patients were treated with 40mg qw and 40mg biw, respectively. Therefore, 14 patients were evaluable for efficacy assessment. ORR was 78.6% in 11 patients including CR in 3 patients (21.4%); mCR+HI in 1 patient (7.1%); and mCR in 7 patients (50.0%) (see the swimmer plot). Especially, among the TP53 mutated patients (n=3), one achieved CR, the other two achieved mCR. The common adverse events were neutropenia (93.3%), anemia (86.7%), thrombocytopenia (80.0%), fatigue (53.3%), nausea (26.7%), dizziness (26.7%), vomiting (20.0%) and decreased appetite (20.0%). There were no treatment related deaths. Conclusion: We verified the activity of selinexor sequential azacytidine in MDS both in vitro and in vivo. The treatment showed with encouraging efficacy in newly diagnosed patients with MDS-EB1 or EB2. Adverse events were manageable with supportive care implementation. Enrollment is ongoing to assess the efficacy and safety of this sequential therapy, which may be a new treatment option for EB subjects.

Abstract:Purpose Selinexor is a first-in-class, oral selective-inhibitor-of-nuclear-export, granted accelerated approval by FDA (2019) for relapsed and refractory multiple myeloma (RRMM). We sought to quantitatively summarize the selinexor efficacy and safety in RRMM. Methods We searched PubMed, EMBASE, CENTRAL, clinicaltrial.gov, and google scholar, until May 2023, studies about selinexor use in RRMM. The outcome measures of interest were primarily efficacy outcomes, in addition to safety outcomes. Random-effect model analyses were performed, at statistical significance of P<0.05, using the RevMan software. Results Meta-analyses of eleven included clinical trials yielded a significant 56.21% overall clinical benefit, 46.91% overall response, 4.89% complete response, 23.41% very good partial response, 24.68% partial response, and 28.06% stable disease rates with selinexor. Due to safety reasons, selinexor caused significant increase in discontinuation rate, 16.80%. Subgroup analyses demonstrated higher efficacy with selinexor plus dexamethasone and proteasome inhibitor combinations than with selinexor alone. The multiple myeloma type, high cytogenetic risk, refractory state, and advanced disease state did not affect performance. Risk of selection, performance, and detection biases were unclear in the included trials. Conclusion Selinexor led to significant positive responses with an acceptable safety profile in RRMM patients, despite higher rates of safety-related discontinuations. Selinexor-based combinations further enhanced response.

Abstract:BACKGROUND Both the government and the pharmaceutical industry are interested in increasing the use of over-the-counter (OTC) medicines. The reaction on the part of general practitioners is more circumspect. AIM To investigate whether fundholding or dispensing status and patient exemption from, or prepayment of prescription charges influence the behaviour of general practitioners with respect to prescribing preparations otherwise available OTC. METHOD Regression analysis of data for all 105 Lincolnshire practices for the fiscal year 1993-94, using the number of items prescribed by the practice that were also available OTC as the outcome variable. Comparison of Audit Commission Thematic Analysis of Prescribing (ACTAP) data for fundholders' and non-fundholders' OTC prescribing in the same year. RESULTS The prescription of medicines otherwise available OTC is less likely when the practice is fundholding and more likely when the practice has dispensing status. Prescription of such medicines also increases as the proportion of patients exempt from, or having prepaid prescription charges increases.

Abstract:Background: Acute myeloid leukaemia (AML) patients still have unsatisfactory clinical outcomes. Venetoclax combined with chemotherapy or azacytidine or low-dose cytarabine has shown efficacy in adults with AML. Homoharringtonine is synergistically lethal with venetoclax in acute myeloid leukemia through down regulation with MCL-1. Aims: We aimed to investigate the activity and safety of venetoclax plus homoharringtonine-based induction regimens in adults with AML. Methods: We conducted a two-stage, single-arm, phase 2 trial at four public hospitals in China. We enrolled patients aged 18–60 years with previously untreated de novo AML. Patients received induction treatment with venetoclax (100 mg on day 2, 200 mg on day 3, and 300 mg on days 4–10), homoharringtonine (2 mg/m² on days 1–5), cytarabine (100 mg/m² on days 1–5), and aclarubicin (12 mg/m² on days 1–5) (VHAA regimen). The primary endpoint was the composite complete remission rate [complete remission (CR) plus complete remission with incomplete blood cell count recovery(CRi)] after one cycle of induction treatment, assessed in the as-treated population. Secondary endpoints were bone marrow measurable residual disease by flow cytometry, event-free survival, overall survival, and adverse events. This trial is ongoing and is registered with Chinese Clinical Trial Registry, ChiCTR2100053516. Results: Between Nov 25, 2021, and Mar 23, 2021, 35 patients were assessed for eligibility and 33 were enrolled. 16 (48.5%) patients were men and 17 (51.5%) were women, and all were Asian. The composite complete remission rate after one cycle of DAV regimen was 93.9% (31 of 33 patients, with CR rate of 87.8%, 28/31) in the entire cohort. 31 (100%) of 31 patients had undetectable measurable residual disease (ie, <0.01%). Grade 3 or worse adverse events included neutropenia in 33 (100%) of 33 patients, thrombocytopenia in 33 (100%), anaemia in 33 (100%), febrile neutropenia in 11 (33.3%), pneumonia in eighteen (54.5%), and sepsis in four (12%). No treatment-related deaths occurred. Summary/Conclusion: The VHAA regimen represents an effective induction therapy for newly diagnosed adults with acute myeloid leukaemia, which resulted in a high rate of composite complete remission. These findings showing a safe strategy to incorporate venetoclax combined homoharringtonine-based induction regimens to treat newly diagnosed acute myeloid leukaemia. Abstract:Abstract Introduction The oral anti-apoptotic B-cell lymphoma 2 protein inhibitor (venetoclax) combined with hypomethylating agents (azacitidine) has shown promising activity in patients with acute myeloid leukemia (AML). However, there are no single center real-world research results with relatively large sample size in China. The purpose of this study is to explore the safety and short-term efficacy of venetoclax combined with azacitidine (Ven + AZA) in previously untreated patients who were ineligible for standard chemotherapy and patients with relapsed/refractory (R/R) AML in China. Methods A retrospective study was conducted on 60 previously untreated patients who were ineligible for standard chemotherapy and patients with R/R AML who received Ven + AZA (venetoclax,100mg D1、200mg D2、400mg D3-28;azacitidine, 75mg/m2 D1-7) in the Peking University Institute of Hematology from March 15, 2019 to May 20, 2021. The incidence of adverse events (AE), complete remission (CR)/CR with incomplete hematological recovery (CRi) rate, objective remission rate (ORR) and minimal residual disease (MRD) status in patients with different risk stratification and different gene subtypes were analyzed. Results Patient Characteristics The median age was 54 (18-77) years, 33 males (55.0%), and the median follow-up time was 4.8 (1.4-26.3) months. Among the 60 patients, 24 (40.0%) were previously untreated patients who were ineligible for standard chemotherapy and 36 (60.0%) were R/R patients. The median cycles of Ven + AZA in the two groups were both 1 (1-5). The median follow-up time of them were 2.9 (1.5-13.2) and 118 (1.4-26.3) months, respectively. In the previously untreated patients who were ineligible for standard chemotherapy, 2 were over 75 years old, 16 cases were diagnosed with active infection, 6 had organ dysfunction. According to the risk stratification of NCCN, it was divided into 8 cases of favorable-risk, 2 cases of intermediate-risk and 14 cases of poor-risk. According to the time from CR to hematological recurrence, the R/R group was divided into 12 cases in the favorable-risk group (CR to hematological recurrence ≥18 months) and 24 cases in the poor-risk group (CR to hematological recurrence <18 months, no remission after 1 cycle of therapy, no remission after ≥2 cycles of therapy). Efficacy In the previously untreated patients who were ineligible for standard chemotherapy, after the first cycle of Ven + AZA treatment, 17/24 (70.8%) cases achieved CR/CRi, 3/24 (12.5%) cases achieved partial remission (PR), and the ORR was 83.3%. Among the CR/CRi patients, 8/17 (47.1%) achieved MRD negative after 2 cycles of therapy. Four patients achieved CR/CRi after the first cycle of Ven + AZA, ineligible factors (active infection) were eliminated, followed by chemotherapy, and MRD achieved negative after 1-2 cycles of consolidation therapy. The median duration of induction therapy in R/R group was 28 (14-28) days, and the total CR/CRi rate was 58.3%. 11/24 (45.8%) cases achieved CR/CRi after one cycle of Ven + AZA in the poor-risk R/R group, and 10/12 (83.3%) cases achieved CR/CRi in the favorable-risk R/R group, which was significantly superior than that in the poor-risk group (P = 0.031). The efficacy in patients with different risk stratification and gene subtypes are displayed in Table. Safety The incidence of hematological AEs was 100%, and the incidence of neutropenic fever was 40.0% (24/60). The incidence of grade 3-4 leukopenia was 86.7% (52/60), of which 60.0% (36/60) occurred before therapy, and the incidence of grade 3-4 leukopenia caused by therapy was 26.7% (16/60). The incidence of grade 3-4 anemia was 71.7% (43/60), of which 51.7% (31/60) occurred before therapy, and the incidence of grade 3-4 anemia caused by therapy was 20.0% (12/60). The incidence of grade 3-4 thrombocytopenia was 76.7% (46/60), of which 60.0% (36/60) occurred before therapy, and the incidence of grade 3-4 thrombocytopenia caused by therapy was 16.7%. The most common non hematological AE was infection, followed by gastrointestinal AE. Infection mainly included pneumonia (13.3%) and soft tissue infection (6.7%), both of which were grade 3-4. Conclusion Ven + AZA has acceptable safety in previously untreated patients who were ineligible for standard chemotherapy and patients with R/R AML, can achieve a high response rate, and some patients can achieve MRD negative in China. Abstract:Background: Myelofibrosis is a chronic hematologic malignancy characterized by constitutional symptoms, bone marrow fibrosis, extramedullary hematopoiesis resulting in splenomegaly and a propensity toward leukemic progression. Although JAK inhibitors have improved patients' splenomegaly and symptom burden, the clinical management of MF is still challenging for its limited treatment options, especially when patients become less responsive or intolerable to Ruxolitinib (RUX) treatment. Selinexor (SEL) is a novel, first-in-class, oral selective inhibitor of nuclear export agent that binds the karyopherin protein exportin 1 (XPO1). Preclinical studies showed SEL decreased viable cells and colony formation both in newly diagnosed and RUX-exposed MF cells. ESSENTIAL trial showed SEL's sustained spleen responses as a single agent in JAKi refractory MF. Besides, a phase I study reported, with SEL plus RUX in treatment-Naïve MF, 92% patients(pts) achieved SVR35, 69% had symptom response and 65% had stable or improved Hgb levels. We have presented promising efficacy and safety data in real-world setting of SEL plus RUX regimen in MF patients in 2023 EHA. Here we are to report the efficacy and safety of the regimen in RUX-treated patients in a prospective clinical trial.

Abstract:Purpose: Selinexor is an oral selective inhibitor of exportine-1 (XPO1) with efficacy as a single agent in heavily pretreated diffuse large B-cell lymphoma (DLBCL). We conducted a study investigating the combination of selinexor with rituximab and platinum-based chemotherapy in B-cell lymphoma. Patients and methods: We conducted a phase 1b, dose-escalation, and expansion trial, which enrolled patients with relapsed or refractory B-cell non-Hodgkin lymphoma. Patients received oral selinexor according to a 3 + 3 design in combination with rituximab and dexamethasone, high-dose cytarabine, oxaliplatine (DHAOX) or gemcitabine, dexamethasone, and cisplatin (GDP) chemotherapy. Results: A total of 39 patients were enrolled, 27 during the escalation phase and 12 during the expansion phase. Most patients had diffuse large B-cell lymphoma (DLBCL; 77%). Group R-DHAOX was prematurely closed to inclusion due to a recommendation from the French drug agency, independent of this trial. A recommended phase 2 dose (RP2D) of selinexor in association with R-GPD was established at 40 mg on days 1, 8, and 15 of each 21-day cycle. In a population of 18 patients treated at this dose of selinexor, the most frequent grade 3–4 adverse events were hematological. With this regimen, seven obtained a complete metabolic response and five a partial response. The median PFS was 5.8 months. Conclusions: Among the patients with R/R B-cell lymphoma, selinexor at a weekly dose of 40 mg with R-GDP is feasible for outpatients, with a generally acceptable safety profile.
