Abstract:AIM: To determine the prevalence of some retinal pathologies in people over 60y and their association with demographic and ocular factors. METHODS: A cross-sectional study was conducted in Tehran using multistage cluster sampling. After selecting subjects aged 60 and over, optometric, and ophthalmic examinations were done. For retinal examination, a 90 D lens was used and indirect ophthalmoscopy was performed after instilling tropicamide drops. Biometry was done using the IOL Master for all participants. RESULTS: Of 3791 people that were invited through cluster sampling, 3310 participated in the study (response rate=82%). The prevalence of retinal pigmented epithelium (RPE) change, drusen, geographic atrophy (GA), hypertensive retinopathy (HTR), nonproliferative diabetic retinopathy (NPDR), proliferative diabetic retinopathy (PDR), choroidal neovascularization (CNV), central retinal artery occlusion (CRAO), myopic retinopathy (MR), branch retinal vein occlusion (BRVO), and central retinal vein occlusion (CRVO) was 27.42%, 11.08%, 4.52%, 3.03%, 4.05%, 0.54%, 0.82%, 0.39%, 0.20%, 0.49%, and 0.19%, respectively. After removing the effect of age, the odds of NPDR were 1.68 times higher in women compared to men (P=0.014). After removing the effect of sex, the odds of drusen, RPE change, GA, CNV, BRVO, and CRVO increased with age. CONCLUSION: There is a higher prevalence of RPE change, drusen, GA, CNV and a lower prevalence of MR and CRAO in the elderly population of Tehran aged over 60y compared to global average values. Considering the correlation of most of the diseases with age and their effects on vision, attention should be paid to these diseases and the related screening programs to prevent vision impairment.

Abstract:Abstract Purpose To determine the distribution of subfoveal choroidal thickness (SFCT) and its associated demographic, ocular, and systemic factors in an elderly population. Methods This report is part of the Tehran Geriatric Eye Study (TGES); a population-based cross-sectional study that was conducted on the urban elderly population of Tehran, aged 60 years and above using multi-stage stratified random cluster sampling. Choroidal imaging was performed using Spectralis SD-OCT with enhanced depth imaging mode. Results The average SFCT was 265.3 ± 25.9 μm (95% CI: 262.8-267.7) in the whole sample. According to the multiple generalized estimating equation (GEE) model, pseudophakia had a statistically significant direct relationship with SFCT (coefficient = 5.69), and history of cerebrovascular accident (CVA) was significantly inversely related to SFCT (coefficient=-4.77). Moreover, there was a significant interaction between age and sex in the average SFCT so that with increasing age, the SFCT increased in men and decreased in women. Conclusion The normal values of SFCT in the present study can be used as a reference database for clinical and research purposes. Age-sex interaction, pseudophakia, and history of CVA were significantly associated with SFCT in the elderly population. It is recommended that these factors be taken into account when interpreting SFCT data.

Abstract:To determine the distribution of keratometry and corneal astigmatism (CA) and their association with demographic factors, systemic parameters, anthropometric measures, ocular biometric indices, and refractive errors in people aged 60 years and above. In this cross-sectional study, 160 clusters were randomly selected from Tehran city (Iran) using the multi-stage cluster sampling method. All participants underwent optometric examinations including testing uncorrected and best-corrected distance visual acuity, non-cycloplegic autorefraction, and subjective refraction. Pentacam imaging for all participants was carried out using Pentacam AXL. Keratometry and CA were reported based on Pentacam's data. The average, standard deviation (SD) and 95% confidence interval (CI) of flat keratometry (Kf), steep keratometry (Ks), mean keratometry (mean K), and CA were 44.02 ± 1.58 D (95% CI 43.94–44.1), 44.86 ± 1.67 D (95% CI 44.78–44.94), 44.44 ± 1.58 D (95% CI 44.36–44.52), and 0.84 ± 0.74 D (95% CI 0.81–0.87), respectively. The 95% and 99% percentiles of mean K were 47.1 and 48.6 D, respectively. According to the multiple generalized estimating equation model, the mean K was significantly higher in males, in myopes, and in those with higher systolic blood pressure. Moreover, the mean K was inversely related to the axial length, height, anterior chamber depth (ACD), corneal diameter, and central corneal thickness (CCT). The prevalence of various types of CA based on a cut-off > 0.50 D was as follows; with-the-rule: 32.5% (95% CI 30.6–34.4), against-the-rule: 18.2% (95% CI 16.7–19.7), and oblique: 10.0% (95% CI 9.1–11.0). The present study investigated the normal distribution of keratometry and CA in individuals ≥ 60 years, and results can be used in clinical matters, especially in intraocular lens power calculation. Sex, systolic blood pressure, height, and some biometric components such as ACD, corneal diameter, and CCT were significantly related to keratometry and should be considered.

Abstract:The Persian Eye Cohort Study, a population-based cross-sectional study from 2015 to 2020, examined refractive error prevalence among 48,618 Iranian adults aged 31 to 70. The study encompassed six centers in Iran, employing random cluster sampling for demographic, medical, and socioeconomic data collection through interviews. Ophthalmic exams included visual acuity, automated and manual objective refraction, subjective refraction, slit lamp, and fundus examinations. Using the spherical equivalent definition, the sample population was categorized into groups. Results indicated a mean age of 49.52 ± 9.31 and a mean refractive error of 0.26 diopters (D) ± 1.6 SD (95% CI − 0.27 to -0.24), ranging from -26.1 to + 18.5 SD. Prevalence of myopia ( + 0.5D) was 22.6% (95% CI 22.2–23%) and 12.5% (95% CI 12.1–12.8%), respectively. Regarding different age groups, the prevalence of hyperopia and astigmatism exhibited a steady and significant rise with increasing age ( p -value < 0.001 for both). The prevalence of Myopia, however, showed a distinctive pattern, initially increasing in adults under 45, declining in those aged 55–64, and rising again among individuals aged 60 and older. Female gender, older age, urban residency, higher education, higher income, and Fars ethnicity were significantly related to a higher prevalence of myopia ( p -value < 0.001 for all). Female gender ( p -value < 0.001), aging ( p -value < 0.001), urban residency ( p -value = 0.029), and lower-income ( p -value = 0.005) were significantly related to higher prevalence of hyperopia. Astigmatism (> 1D) was prevalent in 25.5% of participants (95% CI 25.1–25.9%) and correlated with male gender, aging, urban residency, illiteracy, and higher income ( p -value < 0.001, < 0.001, < 0.001, < 0.001, 0.014, respectively). The study's comparison with regional and international surveys highlighted the increase in myopia among those over 65 due to higher nuclear cataract rates in older adults. Myopia positively related to education, income, and urban residency, while hyperopia did not exhibit such associations.

Abstract:BACKGROUND: To determine the prevalence of dry eye syndrome in the general 40- to 64-year-old population of Shahroud, Iran.DESIGN: Population-based cross-sectional study.PARTICIPANTS: Through cluster sampling, 6311 people were selected and 5190 participated. Assessment of dry eye was done in a random subsample of 1008 people.METHODS: Subjective assessment for dry eye syndrome was performed using Ocular Surface Disease Index questionnaire. In addition, the following objective tests of dry eye syndrome were employed: Schirmer test, tear break-up time, and fluorescein and Rose Bengal staining using the Oxford grading scheme.MAIN OUTCOME MEASURES: Those with an Ocular Surface Disease Index score ≥23 were considered symptomatic, and dry eye syndrome was defined as having symptoms and at least one positive objective sign.RESULTS: The prevalence of dry eye syndrome was 8.7% (95% confidence interval 6.9-10.6). Assessment of signs showed an abnormal Schirmer score in 17.8% (95% confidence interval 15.5-20.0), tear break-up time in 34.2% (95% confidence interval 29.5-38.8), corneal fluorescein staining (≥1) in 11.3% (95% confidence interval 8.5-14.1) and Rose Bengal staining (≥3 for cornea and/or conjunctiva) in 4.9% (95% confidence interval 3.4-6.5). According to the Ocular Surface Disease Index scores, 18.3% (95% confidence interval 15.9-20.6) had dry eye syndrome symptoms. The prevalence of dry eye syndrome was significantly higher in women (P = 0.010) and not significantly associated with age (P = 0.291). The objective dry eye syndrome signs significantly increased with age.CONCLUSIONS: Based on the findings, the prevalence of dry eye syndrome in the studied population is in the mid-range. The prevalence is higher in women. Also, objective tests tend to turn abnormal at higher age. Pterygium is associated with dry eye syndrome and increased its symptoms.

Abstract:Aim: To determine the prevalence of red-green (RG) color vision deficiency (CVD) in an elderly population and its related factors. Methods: This report is a part of the Tehran Geriatric Eye Study: a cross-sectional population-based study that was conducted on the elderly population (≥60y) of Tehran, Iran using multi-stage stratified random cluster sampling. All study participants underwent complete ocular examination, including the measurement of uncorrected and best-corrected visual acuity, objective and subjective refraction, and slit-lamp biomicroscopy. The color vision was tested using Ishihara plates with the near optical correction in place. Results: Of the 3791 invitees, 3310 participated in the study. The data of 2164 individuals were analyzed after applying the exclusion criteria. The prevalence of R-G CVD was 3.73% (95%CI: 2.37%-5.09%) in the whole sample; the prevalence of protanomaly, protanopia, and deuteranopia was 1.51%, 1.76%, and 0.45%, respectively. The prevalence of R-G CVD was significantly higher in males than in females. The prevalence of RG CVD increased with advancing age from 2.91% in the age group 60-64y to 5.8% in the age group ≥80y (P=0.070). According to the multiple logistic regression model, male sex, and glaucoma were significantly related to RG CVD. Older age and hypertension also had a marginally significant relationship with RG CVD. Conclusion: Changes in color vision occur in the elderly due to the aging process and some physiological and pathological factors. Since the change in visual perception may affect the person's performance, this aspect of the visual system's function should also be taken into consideration in the examinations of the elderly.
Corneal topographic indices of Scheimpflug camera in type 2 diabetic and non‐diabetic elderly population

Abstract:Aims/Purpose: To determine the corneal topographic indices of Scheimpflug camera in type 2 diabetic and non‐diabetic in elderly population of Tehran, Iran. Methods: A total of 1105 subjects were selected using random cluster sampling from Tehran, Iran and categorized into two groups including diabetics and non‐diabetics. The diabetic group had HbA1c levels ≥6.4% with no other systemic problems. The non‐diabetic participants had normal eye findings and no systemic diseases. The keratometry values, Q‐value, index of surface variance (ISV), index of vertical asymmetry (IVA), central keratoconus index (CKI), keratoconus index (KI), index of height asymmetry (IHA), index of height decentration (IHD), anterior average radius of curvature (ARC), posterior average radius of curvature (PRC), and pachymetric progression index (PPI) values were measured using the Pentacam AXL. Results: A total participants included 429 diabetic (38.46% male) and 676 non‐diabetic (38.76% male). Only PPIavg and PPImax were higher in the diabetics versus non‐diabetics ( p = 0.019 and p = 0.010, respectively). The HbA1c level was 5.47 ± 0.34, 6.70 ± 0.17, and 8.72 ± 1.16 in the first, second, and third group, respectively. There was a significant difference in PPImax between the three groups ( p = 0.036). There were significant differences only in IVA, CKI, PPIAvg, and PPImax between different stages of DR ( p = 0.045, p = 0.005, p = 0.002, and p = 0.004, respectively). The diabetic subjects were divided into two groups based on the presence of DR. There was a significant difference in ISV, IVA, PPIavg, and PPImax between diabetic subjects with and without DR ( p = 0.016, p = 0.022, p < 0.001, and p < 0.001, respectively). Conclusions: The results of this study showed that DM and DR change several topographic indices and the HbA1c level may affect PPImax. Therefore, special attention should be paid to these patients for different treatment strategies.

Five year changes in central and peripheral corneal thickness: The Shahroud Eye Cohort Study

Abstract:Purpose: To determine five year changes in corneal thickness from the apex to the 8mm periphery and related factors through a longitudinal population-based study of middle-aged Iranians. Methods: In the first phase, 4670 of the 5190 participants, and in the second phase, 4666 of the 4737 participants were examined with the Pentacam. In this report, analysis was done on right eye data of 2509 people who had no diabetes, pterygium, or history of eye surgery, and their image quality was displayed as "ok". Thickness changes in different parts of the cornea and their relation with age, gender, refractive error, and intraocular pressure (IOP) were assessed using repeated measures analysis of covariance. Results: Corneal thickness reduced by 1.5±11.7μm in the apex, 2.6±11.7μm in the thinnest point, and 5.3±12.2, 7.7±14.3, and 11.4±18.6μm in peripheral rings of 2, 3, and 4mm radius, respectively (all p<0.001 with and without adjusting for baseline thickness). Of the studied thickness variables, only changes in the 4mm ring significantly related with age (p<0.001) and gender (p<0.001); there was less change in older age and in men. Thickness changes were not related to refractive error or IOP (all p>0.05). Conclusion: Corneal thickness decreased with age in this sample of 40-64year olds. There was significantly greater thinning in the periphery compared to the corneal center even after controlling for baseline thickness. Results of this longitudinal study can be helpful in understanding age-related changes in the cornea and the eye.
Prevalence and associations of epiretinal membrane in an elderly urban Chinese population in China: the Jiangning Eye Study

Abstract:Aims To describe the prevalence and associations of epiretinal membrane (ERM) in an elderly urban Chinese population in China. Methods A population-based cross-sectional study was conducted using a cluster random sample of residents aged 50 years or older living in the Jiangning Road Sub-district, Shanghai, China. All participants underwent a standardised interview and comprehensive eye examinations, including digital retinal photography and spectral domain optical coherence tomography (SD-OCT) examinations of both eyes between November 2012 and February 2013. ERM was identified and classified as either cellophane macular reflex (CMR) or preretinal macular fibrosis (PMF) based on the fundus photography and OCT features. Results Of the 2044 subjects who participated (82.5% response rate), 2005 had fundus photographs and OCT results of sufficient quality for grading of ERM signs. ERM was present in 8.4% of participants, including 5.0% with CMR and 3.4% with PMF. After age and gender standardisation to the 2010 Chinese census population, the prevalence rate of ERM in mainland Chinese individuals of 50 years of age or older was estimated to be 7.3%. After adjusting for age and/or gender, idiopathic ERM was positively associated with age (OR, 1.06; 95% CI 1.04 to 1.08), female gender (OR 1.67, 95% CI 1.16 to 2.40), myopia (OR 2.21, 95% CI 1.51 to 3.22) and hyperlipaemia (OR 1.80, 95% CI 1.04 to 3.12). Conclusions The prevalence of ERM in elderly urban Chinese was similar to that in Caucasians. Risk factors for idiopathic ERM were older age, female gender, myopia and hyperlipaemia.

Abstract:Aims/Purpose: To determine the association between dry eye with higher order aberrations (HOAs) and contrast sensitivity in 60 years old people and over. Methods: This population‐based study was conducted in Tehran, the capital of Iran, using stratified multistage random cluster sampling. All participants underwent complete ocular examination including measurement of uncorrected and best‐corrected visual acuity, refraction, slit‐lamp biomicroscopy. Zywave was used for measuring HOAs aberrations. Contrast sensitivity was measured with best correction visual acuity using the CVS1000 grating charts. Schirmer and tear break‐up time (TBUT) tests were used for evaluating dry eye in participants. Results: After applying exclusion criteria, the data of 397 individuals were analysed. A total of 415 people were invited, of whom 397 (62.8%) were females. The mean age of the participants was 68.32 ± 5.49 years. After adjusting with age and sex, in the multiple regression model that the TBUT test has a direct association with spherical aberration ( p = 0.017) and an inverse association with coma, trefoil and the third, fourth and fifth order aberrations ( p < 0.05). Schirmer test results also had a positive relationship with spherical aberration ( p = 0.046) and an inverse relationship with coma, trefoil and the third, fourth and fifth order aberrations ( p < 0.05). Contrast sensitivity had a significant association with Schirmer test result only in spatial frequency of 3CPD. Conclusions: The results of this study indicated the association between dry eyes and HOAs, and that the TBUT and Schirmer test results are of the most important factors in this relationship. Treating dry eye, can improve HOAs and quality of vision in elderly people.

With conventional methods, disseminated HAdV disease could only be diagnosed with delay. Quantification of the HAdV load by real‐time PCR in peripheral blood promised to solve this diagnostic dilemma. Here we review the development, applications and significance of quantitative HAdV PCR.
Presbyopia, Dry Eye, and Retinal Thickness in the Middle-Aged Population: Focusing on Sex Differences

Abstract:Purpose: Risk factors for presbyopia have not been fully determined although previous studies suggested presbyopia was associate with age, dry eye, and retinal ganglion cell complex thickness (GCC). We accessed these signs and common ocular symptoms in the middle-aged population focusing on sex differences when women have drastic hormonal change. Methods: This cohort study consecutively enrolled 2743 patients aged 36-45 years (n=1000), 46-55 years (n=1000), and 56-65 years (n=743). All underwent ocular surface tests and had near add power and GCC measured. Common ocular symptoms were asked using questionnaire. Results: Among female participants, visual symptoms (eye strain and photophobia) were more prevalent in the age group 46-55, whereas non-visual symptoms (dryness, irritation, and pain) were not. We identified symptomatic presbyopia (near add power ≥ 1.5D) in 14.4%, 73.8%, and 97.8%, positive corneal staining in 29.1%, 23.8%, and 23.9%, and a mean GCC of 98.2 μm, 105.3 μm, and 89.6 μm in the age groups 36-45, 46-55, and 56-65, respectively. Mean tear break-up time were 3.3, 3.5, and 3.3 seconds, respectively. Results indicated a large progression of presbyopia (P<0.01) from the period of 36-45 years onward and significantly increased GCC (P<0.01) in women of age group 46-55. No notable tendency was observed in symptoms and GCC for male participants. Conclusion: Visual symptoms in women were worse between 46 and 55 years than before or after these ages. The increase of symptomatic presbyopia and GCC may be contributing to visual symptoms in addition to menopausal transition symptoms in this age group.
A Population-Based Study of Social Demographic Factors, Associated Diseases and Recurrent Corneal Erosion in Taiwan

Abstract:Purpose To investigate the association of recurrent corneal erosion (RCE) with sociodemographic factors and associated ocular conditions or systemic diseases. Methods This nationwide, population-based, retrospective, matched case-controlled study included 98,895 RCE patients, identified by the International Classification of Diseases, Ninth Revision, Clinical Modification (ICD-9-CM) code 371.42, were selected from the Taiwan National Health Insurance Research Database. The age-, sex-, and index date- matched control group included 98,895 non-RCE control group also selected from the Taiwan Longitudinal Health Insurance Database 2000. Sociodemographic factors and associated ocular conditions or systemic diseases were examined using univariate logistic regression analyses, and continuous variables were analyzed using paired t -test. The odds ratio (OR) of developing RCE were compared using adjusted logistic regression analysis. Results Patients with ocular conditions including corneal abrasion, ocular allergic conditions, and corneal dystrophy were more likely to have RCE than the control group (adjusted OR = 63.56, 95% CI = 42.06–96.06, p < 0.0001; adjusted OR = 24.27, 95% CI = 20.51–28.72, p < 0.0001; adjusted OR = 17.10, 95% CI = 5.14–59.93, p < 0.0001, respectively). Patients with systemic diseases such as diabetes mellitus, hyperlipidaemia, and atopy trait have significantly higher ORs for RCE development. Patients residing in either Northern Taiwan or a metropolis city had higher odds of developing RCE; however, there were no significant differences in income or occupation on the probability to develop RCE. Conclusion RCE is strongly associated with corneal abrasion, ocular allergic conditions, corneal dystrophy, diabetes mellitus, hyperlipidaemia, and atopy trait.

Abstract:Background: To estimate the prevalence of posterior segment eye lesions and to identify their ocular and systemic associations within the Gutenberg Health Study (GHS) in Germany. Methods: Assessment and grading of fundus images as well as physical examination and history taking were performed in the cross-sectional analysis of 15,010 subjects (aged 35-74 years) using standardised procedures to determine the prevalence and associations of various posterior segment eye lesions. Results: Fundus photographs of both eyes were available for 12,782 (85.2%; 50% female) subjects. The prevalence weighted to the region of Mainz and Mainz-Bingen in Germany was for choroidal nevi 2.4%, drusen of the optic nerve head 0.2%, tilted discs 1.5%, chorioretinal scars suggestive of toxoplasmosis 0.2%, retinitis pigmentosa 0.04% and persistent hyaloid artery 0.02%. Choroidal nevi were positively associated with a history of myocardial infarction (OR = 2.7, 95% confidence interval 1.2-6.2, p value = 0.017). Tilted discs were positively associated with increased intraocular pressure (OR = 1.09 per mm Hg (1.02-1.16), p = 0.011) and negatively associated with smoking (OR 0.4 (0.3-0.7), p = 0.0022). Participants with tilted discs had a mean spherical equivalent of - 3.6 dioptres (standard deviation 4.0) compared with - 0.4 dioptres (2.4) to those without. Conclusion: Our study is-to the best of our knowledge-the first to determine the prevalence of drusen of optic nerve head among Caucasians, to show a positive association between tilted discs and increased intraocular pressure and questions a possible link between choroidal nevi and myocardial infarction. It also showed that participants with tilted discs had a lower mean spherical equivalent than those without.

Abstract:PURPOSE:To describe the prevalence of AMD in an elderly urban Chinese population in China. METHODS:A population-based, cross-sectional study was conducted using a cluster random sample of residents aged 50 years or older living in the Jiangning Road Subdistrict, Jing'an District, Shanghai, China. All participants underwent a standardized interview and comprehensive eye examinations, including digital retinal photography and spectral-domain optical coherence tomography (OCT) examinations of both eyes between November 2012 and February 2013. Trained graders assessed the presence and severity of AMD lesions based on a modified version of the Wisconsin Age-Related Maculopathy Grading System. RESULTS:Of the 2044 subjects who participated (82.5% response rate), 2005 had fundus photographs and OCT results of sufficient quality for grading of AMD signs. Early and late AMD were present in 206 (10.3%) and 23 (1.1%) participants, respectively. After age standardization, the prevalence of early AMD in Chinese persons aged 50 years or older was 9.5% (95% confidence interval [CI], 8.2-10.8) and that of late AMD was 1.0% (95% CI, 0.5-1.5). CONCLUSIONS:The prevalence of early and late AMD in this urban Chinese sample was higher than that reported in the Beijing and Handan studies. Age-related macular degeneration is highly prevalent among the elderly urban Chinese population in mainland China. : Chinese Abstract.

Abstract:Purpose: To evaluate the association of five different polymorphisms from a genome-wide-associated study with susceptibility to glaucoma in the northeast Iranian population. Methods: Hundred and thirty patients with primary angle closure glaucoma (PACG) and 130 healthy controls were genotyped for the polymorphic regions with the aid of tetra-amplification refractory mutation system-polymerase chain reaction. The association of these variants with the disease susceptibility was measured statistically with the logistic regression method. Results: Hundred and thirty patients with PACG (53 males, 77 females) with a mean age of 64.5 ± 6.2 years and 130 healthy control subjects (51 males, 79 females) with a mean age of 64.0 ± 5.7 years were selected for evaluation. There was a significant association between rs3816415 (P = 0.005), rs736893 (P < 0.001), rs7494379 (P < 0.001), and rs1258267 (P = 0.02) with PACG susceptibility. This association could not be shown for rs3739821. Conclusion: It was revealed that studied variants in GLIS3, EPDR1, FERMT2, and CHAT genes can contribute to the incidence of PACG. Additional studies in other populations are needed to evaluate DPM2-FAM102A.

Abstract:PurposeTo determine the prevalence of visual impairment and blindness in underserved Iranian villages and to identify the most common cause of visual impairment and blindness.Patients and methodsMultistage cluster sampling was used to select the participants who were then invited to undergo complete examinations. Optometric examinations including visual acuity, and refraction were performed for all individuals. Ophthalmic examinations included slit-lamp biomicroscopy and ophthalmoscopy. Visual impairment was determined according to the definitions of the WHO and presenting vision.ResultsOf 3851 selected individuals, 3314 (86.5%) participated in the study. After using the exclusion criteria, the present report was prepared based on the data of 3095 participants. The mean age of the participants was 37.6±20.7 years (3–93 years). The prevalence of visual impairment and blindness was 6.43% (95% confidence interval (CI): 3.71–9.14) and 1.18% (95% CI: 0.56–1.79), respectively. The prevalence of visual impairment varied from 0.75% in participants aged less than 5 years to 38.36% in individuals above the age of 70 years. Uncorrected refractive errors and cataract were the first and second leading causes of visual impairment; moreover, cataract and refractive errors were responsible for 35.90 and 20.51% of the cases of blindness, respectively.ConclusionThe prevalence of visual impairment was markedly high in this study. Lack of access to health services was the main reason for the high prevalence of visual impairment in this study. Cataract and refractive errors are responsible for 80% of visual impairments which can be due to poverty in underserved villages.

Abstract:The prevalence of epiretinal membrane (ERM) and associated factors in the phakic eyes have not been fully elucidated yet. This cross-sectional study included 2,354 phakic eyes without retinal diseases or surgical history. Ocular parameters, such as uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), spherical equivalent (SE), intraocular pressure (IOP), white-to-white corneal diameter (WTW), mean keratometric value (Km) of total corneal refractive power at 4-mm diameter (TCRP4), astigmatism of TCRP4, total corneal irregular astigmatism (TCIA), pupil diameter, axial length (AXL), anterior chamber depth (ACD), lens thickness (LT), and posterior vitreous detachment (PVD) were compared between ERM group and control group. Additionally, an age-matched control group was selected by individual matching and compared with the ERM group to eliminate the confounders. Multiple logistic regression analysis was performed to evaluate the factors associated with the presence of ERM. Among 2,354 eyes, 429 eyes (18.2%) had ERM based on spectral-domain optical coherence tomography. The ERM group showed higher prevalence of PVD, worse CDVA, higher astigmatism of TCRP4, higher TCIA, smaller pupil size, longer AXL, and thicker LT than control group (P < 0.001, P < 0.001, P = 0.011, P < 0.001, P = 0.023, P < 0.001, and P < 0.001, respectively). Only PVD, CDVA, SE, astigmatism of TCRP4, TCIA, and AXL maintained the significance when compared with the age-matched control group (P < 0.001, P = 0.026, P < 0.001, P = 0.001, P = 0.003, and P < 0.001, respectively). Multivariate logistic regression analysis showed that age, PVD, CDVA, and TCIA were independently associated with the presence of ERM (P < 0.001, P < 0.001, P = 0.011, and P = 0.002). The prevalence of ERM detected using SD-OCT was 18.2% in the middle aged phakic population. Eyes with TCIA, in addition to older age and PVD, were more likely to have ERM.
