Abstract:Study Design.A finite element analysis.Objective.The aim of this study was to determine the biomechanical differences between atlantoaxial fusion cage combined with transoral atlantoaxial reduction plate fixation (TARP+Cage, modified TARP technique) and that combined with C1 lateral mass screw and C2 pedicle screw fixation (C1LS+C2PS+Cage, modified Goel technique) in the treatment of basilar invagination (BI) by finite element analysis.Summary of Background Data.Clinical studies have shown that transoral anterior atlantoaxial release followed by TARP fixation can achieve reduction, decompression, fixation, and fusion of C1-C2 through a transoral-only approach. Although cage has been used to reduce the BI through posterior approach, there are no studies referred to the cage combined with TARP for C1-C2 fusion.Methods.A finite element model was used to investigate and compare the stability between TARP+Cage fixation and C1LS+C2PS+Cage fixation in the treatment of BI. Vertical load of 40N was applied on the C0, to simulate head weight, and 1.5Nm torque was applied to the C0 to simulate flexion, extension, lateral bending, and rotation.Results.In comparison with the C1LS+C2PS+Cage model, the TARP+Cage model reduced the ROM by 44.7%, 30.0%, and 10.5% in extension, lateral bending, and axial rotation, while the TARP+Cage model increased the ROM by 30.0% in flexion, and the TARP+Cage model also led to lower screw stress in all motions with one exception (anterior C2PS stress in extension).Conclusion.Our results indicate that the TARP+Cage fixation may offer higher stability to C1LS+C2PS+Cage in extension, lateral bending, and axial rotation but lower stability in flexion. Compared with modified Goel technique, the modified TARP technique not only has the capability of transferring the load and distributing the stress but also can provide neural decompression, stabilization and fusion, and restore C1-C2 normal fusion angle.Level of Evidence: N/A
[Clinical Effect of Posterior Atlanto Axial Vertebra Internal Fixation for Treatment of Instability of Occipitocervical].

Abstract:OBJECTIVE:To explore radiographic results and clinical effects of posterior atlanto axial vertebra internal fixation in treating instability of occipitocervical.METHODS:The clinical data of 155 patients with instability of occipitocervical treated by posterior atlanto axial vertebra internal fixation were respectively analyzed from September 2005 to January 2011. There were 68 males and 87 females, ranging in age from 6 to 75 years old with an average of 45.6 years old. Of them, 53 cases were fresh odontoid fractures(Aderson type II C), 30 cases were os odontoideum, 20 cases were old odontoid fractures, 18 cases were unstable atlas fractures, 12 cases were atlanto axial rotatory dislocation, 11 cases were atlanto axial dislocation after rheumatoid arthritis, and 11 cases were basilar invagination. Radiographic results were evaluated in terms of atlas pedicle screw fixation, bone healing and bone graft fusion. Clinical effect evaluation included relief of pain in the occipital-cervical region by VAS score and JOA score.RESULTS:Totally 300 screws were set through atlas pedicle screw fixation in 150 patients. Five patients receivde hook fixation. Postoperative CT showed ideal nailing were 275 (91.7%),acceptable nailing were 14 (4.7%) and unacceptable nailing were 11 (3.6%). All patients were followed up, and the duration ranged from 16 to 40 months with an average of 25.4 months. The fresh fractures healed and 140 cases got bone graft fusion. Preoperative VAS and JOA score were respectively improved from (7.2 +/- 1.1), (7.3 +/- 2.4) to (3.2 +/- 1.1), (13.3 +/- 2.4) at the latest follow-up.CONCLUSION:Posterior atlanto axial vertebra internal fixation in treating instability of occipitocervical can effectively recover physiological curvature of cervical, provide mechanical stability, and obtain good clinical effect. For the young patients who require further activity, posterior fixation and non-fusion technology is a good choose, which can avoid bone graft.
Biomechanical stability of atlantoaxial and occipitocervical fixations with unilateral spinous process screw plus contralateral pedicle screw: a finite element analysis

Abstract:Objective:To determine the biomechanical stability of atlantoaxial and occipitocervical fixations with unilateral spinous process screw plus contralateral pedicle screw.Methods:After nonlinear finite element models of the intact upper cervical spine (C0-3) were constructed and validated, they were processed into 3 kinds with normal axis anatomy, a high-riding vertebral artery and a thin axis lamina, re-spectively.In the models, the odontoid fracture stabilized by atlantoaxial fixation and the atlas fracture sta-bilized by occipitocervical fixation were simulated.In the atlantoaxial fixation, the group of unilateral spinous process screw+contralateral pedicle screw+bilateral atlas lateral mass screws (spinous process screw group) was compared with that of bilateral axis pedicle screws+bilateral atlas lateral mass screws (pedicle screw group); in the occipitocervical fixation, the group of unilateral spinous process screw+contralateral pedicle screw+occipital screws (spinous process screw group) was compared with that of bilateral axis pedicle screws+occipital screws (pedicle screw group).Three different techniques in insertion of spinous process screws (horizontal, oblique and vertical) were applied.The range of motion (ROM) was measured in extension-flexion, lateral flexion and rotation of occipitocervical bodies (C0-C3) after internal fixation.Results:Compared with the intact models, the ROMs of extension-flexion, lateral flexion and rotation of C1-C2 were obviously reduced in the spinous process screw group and the pedicle screw group in the models with atlantoaxial and occipitocervical fixations.In the atlantoaxial fixation, the ROMs of extension-flexion, lateral flexion and rotation of C1-C2 were greater in the spinous process screw group than in the pedicle screw group; in the occipitocervical fixation, the spinous process screw group had greater ROM of lateral flexion of C1-C2 and greater ROM of rotation of C0-C2 than the pedicle screw group.There were unobvious differences in insertion of spinous process screws between the horizontal, oblique and vertical techniques.Conclusions:In the atlantoaxial and occipitocervical fixations, the unilateral spinous process screw plus contralateral pedicle screw can provide as fine stability as the bilateral axis pedicle screws.In the atlantoaxial fixation, the bilateral axis pedicle screws may lead to better stability of C1-C2 than the unilateral spinous process screw plus contralateral pedicle screw.In the occipitocervical fixation, the bilateral axis pedicle screws may lead to better stability of C1-C2 in lateral flexion and rotation than the unilateral spinous process screw plus contralateral pedicle screw. The 3 different techniques in insertion of spinous process screws lead to an unobvious difference in the stability of C1-C2.
Progression of Research of Posterior Screw Fixation on Atlas

Abstract:Posterior screw fixation on atlas includes posterior screw fixation within lateral mass of atlas and in atlas via posterior arch and lateral mass.Atlas is anatomically capable for screw fixation in its lateral mass and via its posterior arch and lateral mass.But it is necessary to pay attention to the thickness of the posterior arch at the thinnest part of the groove and avoid injuring the vertebral artery and the internal carotid artery.Routine lateral radiographs and CT scans of the cervical spines of patients must be performed before surgery,which can be of definite help in locating the entry point and regulating the screw direction.The screws have good pullout strength(even when placed unicortically),and constructs using C_1 lateral mass screws and C_2 pars screws connected with rods are as stable as transarticular screws with posterior wire-graft constructs.At present there are three popular techniques:the Harms technique,as well as Xia and Gupta,which is screw fixation within lateral mass of atlas,and the Tan and Ma technique,which are both screw fixation in atlas via posterior arch and lateral mass.Clinical results showed that posterior screw fixation on atlas could relieve spinal and neural oppression immediately,improve clinical symptoms,have good immediate postoperative stability,keep the movement of occipitocervical region,and showed its advantages in very high successful fusion rate,and very few instrument failure,morbidity and complications.

Abstract:BACKGROUND: Chinese anatomical parameters of antedor atlantoaxial transarticular screw fixation have been rarely reported although the technique is a novel method out of China for patients with C1-C2 instability. OBJECTIVE: To provide Chinese anatomical data for anterior C1-C2 transarticular screw fixation. DESIGN, TIME AND SETTING: A measurement experiment was performed at the Department of Anatomy, Southern Medical University and Department of Orthopedics, Wuhan General Hospital, Guangzhou Command of Chinese PLA between September 2006 and April 2008. MATERIALS: A total of 50 sets of dried Chinese adult human C1 and C2 specimens, without regard to gender and age, but no abnormality and breakage, were measured with an electronic digital caliper (precision 0.01 mm) and a goniometer(precision 0.5°) made in China. METHODS: One proper screw was drilled through the atlantoaxial joint respectively in the direction to middle part of laterosuperior angle in the posterior of C1 lateral mass, and the screw point should not break through the superior facet articularsurface of the C1. In the procedure, the screw drilling point was at the junction of the lateral border of C2 body to 4 mm above the inferior border of C2 anterior arch. border of transverse foramen of C2 body and the median line of C2 body, and the distance between the inserting point and the medial border of transverse foramen of C2 body. RESULTS: The data from all specimens were involved in the result analysis. In the sagittal plane, the minimum lateral angulation of the screw tract was (10.80±2.10)°(left) and (10.76±2.40)°(right) respectively, and the maximum lateral angulation was (25.13±3.12)°(left) and (25.12±2.86)°(right), respectively. In the coronal plane, the minimum posterior angulation was (8.85±2.12)° (left) and (9.28±2.65)° (right) respectively, and the maximum posterior angulation was (26.96±3.09)°(left) and (27.49±2.51)°(right), respectively. The left screw tract length was from (17.48±2.10) mm to (25.41±2.59) ram, and the right was from (17.49±2.23) mm to (25.58±2.42) mm. The left distance between the inserting point and the median line of C2 body was (9.84±0.69) mm, and the right was (9.81±0.66) mm. The left distance between the median line of C2 body and medial border of transverse foramen of C2 body was (14.12±1.28) mm, and the right was (14.60±1.36) mm. The left distance between the inserting point and medial border of transverse foramen of C2 body was (6.28±1.38) mm, and the right was (6.79±1.39) mm. CONCLUSION: It is optimal for the anterior C1-C2 transarticular screw fixation to place the antedor screw with a length of 17 to 25 mm in lateral angulation ranging from 10° to 25° and the posterior angulation ranging from 9° to 27°. During the procedure, the dissecting distance from the middle of C2 body to lateral should not exceed 14 mm.
Biomechanical and Clinical Study of Anterior Transarticular Screw Fixation for Treatment of Traumatic Instability of C1-C2 Joint

Abstract:Objective To evaluate the biomechanical stability of C1-C2 after anterior C1-C2 transarticular screw fixation and assess its curative effect clinically. Methods Eight human cadaveric occipitocervical spine specimens which have been harvested freshly were tested in three modes of loading,flexion-extension, lateral bending, and axial rotation, and four configurations including intact, after an odontoid fracture, and after anterior or posterior C1-C2 transarticular screw fixation. All tests were performed in load and torque control. Clinically, 20 patients who had C1 -C2 traumatic instability were instructed to be operated with anterior C1 -C2 transarticular screw fixation technique, planting cancellated granulate bone into the space between muscular tubercle of arias and odontoid bone. Results Both the anterior and posterior C1-C2 transarticular screw fixation techniques limited motion at the C1-C2 articulation at the tested directions and there were no significant differences between the two techniques. In the 20 patients, one with complete cervical cord injury died of pulmonary infection a month later, and the rest 19 were followed up about 7 months to 3 years (mean 18 months). They all had no trauma on vertebral artery and cervical cord during their cervical operations, and had aflantoaxial synostoses. Conclusion The anterior C1-C2 transarticular screw fixation not only is easy to manipulate and provides the same stability as posterior C1-C2 transarticular screw fixation, but has a less risk of injury on vertebral artery and cervical cord.
Anterior Cervical Transpedicular Screw Fixation System in Subaxial Cervical Spine: A Finite Element Comparative Study.

Abstract:Multilevel cervical corpectomy has raised the concern among surgeons that reconstruction with the anterior cervical screw plate system (ACSPS) alone may fail eventually. As an alternative, the anterior cervical transpedicular screw (ACTPS) has been adopted in clinical practice. We used the finite element analysis to investigate whether ACTPS is a more reasonable choice, in comparison with ACSPS, after a 2-level corpectomy in the subaxial cervical spine. These 2 types of implantation models with the applied 75 N axial pressure and 1 N • m pure moment of the couple were evaluated. Compared with the intact model, the range of motion (ROM) at the operative segments (C4–C7) decreased by 97.5% in flexion-extension, 91.3% in axial rotation, and 99.3% in lateral bending in the ACTPS model, whereas it decreased by 95.1%, 73.4%, 96.9% in the ACSPS model respectively. The ROM at the adjacent segment (C3/4) in the ACTPS model decreased in all motions, while that of the ACSPS model increased in axial rotation and flexion-extension compared with the intact model. Compared to the ACSPS model, whose stress concentrated on the interface between the screws and the titanium plate, the stress of the ACTPS model was well-distributed. There was also a significant difference between the maximum stress value of the 2 models. ACTPS and ACSPS are biomechanically favorable. The stability in reducing ROM of ACTPS may be better and the risk of failure for internal fixator is relatively low compared with ACSPS fixation except for under lateral bending in reconstruction the stability of the subaxial cervical spine after 2-level corpectomy.
Treatment of Atlantoaxial Instability with C1-C2 Posterior Transarticular Screw Fixation

Abstract:OBJECTIVE:To evaluate the technique of C1-C2 transarticular screw fixation for atlantoaxial instability or dislocation.METHODS:Among 14 patients with atlantoaxial instability, 10 were male and 4 patients were female, with an average age of 38.6 years ranging from 17 to 62 years. All patients were treated by internal fixation with transarticular screws.RESULTS:There were 28 screws applied in 14 patients. All patients were followed up. The average follow-up period was 16 months (range, 9 to 35 months). The postoperative JOA score was ranging from 13.8 to 15.8 with the average score of (14.50 +/- 0.66) and the improved rate of (76.12 +/- 4.94)%. No spinal injury and vertical artery injury was found and osseous fusion was completed in all patients.CONCLUSION:The technique of C1-C2 transarticular screw fixation is one of the best treatments for atlantoaxial instability. Without the help of structural bone graft and aided internal fixation, morselized cancellous bone graft can acquire effective osseous fusion.
Biomechanical Rationale for the Development of Atlantoaxial Instability and Basilar Invagination in Patients with Occipitalization of the Atlas: A Finite Element Analysis

Abstract:Objective: Occipitalization of the atlas (OA) often is associated with atlantoaxial dislocation and basilar invagination. The purpose of this study is to determine the biomechanical difference between normal and OA conditions in the craniovertebral junction and to further explore the rationale for development of atlantoaxial dislocation and basilar invagination using the finite element model (FEM). Methods: A ligamentous, nonlinear, sliding-contact, 3-dimensional FEM of the occipitoatlantoaxial complex was generated. Validation of the model was accomplished by comparing kinematic predictions with experimental data. We defined the atlantooccipital joint as a tie contact to simulate the OA deformity. The range of motion and the value of the maximum Von Mises stress were compared between the intact and OA models. Results: We found all of the predicted data in the intact FEM fell within 1 standard deviation of the cadaver data for all 6 loadings. The OA simulation significantly reduced the overall range of motion of the occipitoatlantoaxial complex at all loadings. The maximum Von Mises stress was predicted to increase at the transverse ligament and the superior articular facet of the axis for all the flexion, extension, lateral bending, and axial rotation loadings. Conclusions: The OA could result in hypermobility of the atlantoaxial segment and cause overstress in the transverse ligament and the lateral atlantoaxial joints. These changes explain the pathogenesis of atlantoaxial dislocation and basilar invagination associated with OA. Follow-up should be scheduled regularly due to the nature of the dynamic development of atlantoaxial dislocation and basilar invagination.
Biomechanical characteristics of posterior fixation for repairing atlantoaxial instability

Abstract:BACKGROUND:With the development of atlantoaxial morphology, applied anatomy and biomechanics, pathogenesis, diagnosis and treatment of atlantoaxial instability have attracted more and more attention. However, the development of effective fixation for atlantoaxial instability is relatively late, so scholars al over the world have made numerous studies. OBJECTIVE:To compare biomechanical functions of different fixations through atlantoaxial posterior approach, and to assess its stability. METHODS:We retrieved recent studies on comparative biomechanical evaluation and its primary clinical application of different posterior approaches in repair of atlantoaxial instability, and conducted a retrospective analysis by measuring the three-dimensional range of movement in normal atlantoaxial complex and atlantoaxial instability models. This analysis evaluated the stability of different fixations under normal three-dimensional atlantoaxial movement, and provided a biomechanical basis for reasonable fixator selection. <br> RESULTS AND CONCLUSION:Atlantoaxial posterior fixation included Gal ie wire fixation, Brooks fixation, Apofix and Halifax vertebral plate hook fixation, screw fixation through joint and atlantoaxial pedicle nail/rod fixation. Gal ie technique contributes to the reduction of anterior semiluxation, but its mechanical stability is poor. Brooks technique has strong rotation and stretch forces. Apofix and Halifix vertebral plate hook device provides strong anti-rotation and anti-antelocation strength, and is more stable than Gal ie technique in mechanics. The biomechanics of screw fixation through joint was better than wire technique and Halifax. The screw fixation avoids occipitocervical fusion, and has a high requirement to instal ation. When an internal fixator was selected, immediate cervical vertebra stability should be provided to protect spinal cord functions. Upper neck functions should be maintained to reach reduction and maintenance of occipital bone, atlas and axis. A suitable fixation method should be selected for each patient.

Abstract:To evaluate the stability of biomechanics of occipitoatlantoaxial reconstruction with transarticular screw fixation.Twelve fresh human cadaveric occipitocervical spine specimens were mounted in a custom-designed, spine-testing machine that applied pure moments while recording the three-dimensional angular movement at occiput (Oc)-C(1) and C(1 - 2) segments. The specimens were tested under seven different conditions: the intact (intact group), the occipitoatlantoaxial instability (Destabilized group), fixation with Ti-cable plus bone graft group (cable + graf group), fixation with the transarticular fixation (CTS group), fixation with SUMMIT occitocervical spinal fixation system (SUMMIT group), fixation with transarticular screw plus bone graft (CTS + graf group), and fixation with SMMIT system and plus graft group (SUMMIT + graf group). The data obtained were statistically analyzed.The CTS group reduced motion to well within the normal rang. In the Oc-C(1) segment, The CTS group and SUMMIT group allowed a very small rang of motion (ROM) and neutral zone (NZ) during lateral bending and axial rotation. The ROM and NZ during flexion and extension of the SUMMIT group, were significantly smaller than those of cable + graf group and CTS group (6.64 degrees +/- 0.59 degrees, 2.49 degrees +/- 0.26 degrees, 0.50 degrees +/- 0.03 degrees, 0.21 degrees +/- 0.01 degrees, 0.27 degrees +/- 0.07 degrees, 0.13 degrees +/- 0.01 degrees vs 10.01 degrees +/- 1.26 degrees, 3.80 degrees +/- 0.79 degrees, 7.93 degrees +/- 1.34 degrees, 3.18 degrees +/- 0.95 degrees, 9.54 degrees +/- 0.87 degrees, 5.93 degrees +/- 0.74 degrees, P < 0.05). In the C(1 - 2) segment, ROM and NZ in all directions of CTS group were smaller in rotation than SUMMIT group (1.64 degrees +/- 0.39 degrees, 0.61 degrees +/- 0.15 degrees, 0.14 degrees +/- 0.05 degrees, 0.02 degrees +/- 0.01 degrees, 0.32 degrees +/- 0.04 degrees, 0.08 degrees +/- 0.01 degrees, vs 0.21 degrees +/- 0.04 degrees, 0.07 degrees +/- 0.03 degrees, 0.21 degrees +/- 0.12 degrees, 0.10 degrees +/- 0.02 degrees, 2.92 degrees +/- 0.28 degrees, 1.27 degrees +/- 0.11 degrees, all P < 0.05). There was no significant difference in ROM and NZ in all directions between CTS + graf group and SUMMIT + Graf group (P > 0.05).In vivo biomechanics studies show that posterior occipitocervical transarticular fixation has unique features in reconstructing dynamic stability of the occipitoatlantoaxis, especially in controlling stability of rotation and lateral flexion, thus ensuring successful fusion of the implanted bone and allowing for clinical use of the technique.
Biomechanical Study of Atlanto-occipital Instability in Type II Basilar Invagination: A Finite Element Analysis

Abstract:Objective: Recent studies indicate that 3 morphological types of atlanto-occipital joint (AOJ) exist in the craniovertebral junction and are associated with type II basilar invagination (BI) and atlanto-occipital instability. However, the actual biomechanical effects remain unclear. This study aims to investigate biomechanical differences among AOJ types I, II, and III, and provide further evidence of atlanto-occipital instability in type II BI. Methods: Models of bilateral AOJ containing various AOJ types were created, including I-I, I-II, II-II, II-III, and III-III models, with increasing AOJ dysplasia across models. Then, 1.5 Nm torque simulated cervical motions. The range of motion (ROM), ligament and joint stress, and basion-dental interval (BDI) were analyzed. Results: The C0-1 ROM and accompanying rotational ROM increased progressively from model I-I to model III-III, with the ROM of model III-III showing increases between 27.3% and 123.8% indicating ultra-mobility and instability. In contrast, the C1-2 ROM changes were minimal. Meanwhile, the stress distribution pattern was disrupted; in particular, the C1 superior facet stress was concentrated centrally and decreased substantially across the models. The stress on the C0-1 capsule ligament decreased during cervical flexion and increased during bending and rotating loading. In addition, BDI gradually decreased across the models. Further analysis revealed that the dens showed an increase of 110.1% superiorly and 11.4% posteriorly, indicating an increased risk of spinal cord impingement. Conclusion: Progressive AOJ incongruity critically disrupts supportive tissue loading, enabling incremental atlanto-occipital instability. AOJ dysplasia plays a key biomechanical role in the pathogenesis of type II BI.
Improving C1 Pedicle Screw Placement for Atlantoaxial Instability with Ultrasonic Bone Burr Assistance

Abstract:Objective: This study aims to evaluate the safety and efficacy of the ultrasonic bone burr (UBB) in facilitating C1 pedicle screw placement for atlantoaxial instability treatment, compared to the conventional high-speed drill (HSD) technique. Methods: A retrospective analysis was conducted on patients undergoing posterior screw-rod fixation for atlantoaxial instability between December 2017 and July 2023. The patients were divided into 2 groups based on the tools used for screw placement: UBB and HSD. Data on surgical duration, estimated blood loss, spinal cord and arterial injury incidence, screw placement accuracy, neurological status measured by the Japanese Orthopedic Association score, and fusion rates were collected and analyzed. Results: Thirteen patients received C1 pedicle screw placement via UBB facilitation, while 8 were treated using the HSD approach. The UBB group showed a significant reduction in blood loss and operation time compared to the HSD group (P = 0.002 and P < 0.001, respectively). No spinal cord or arterial injuries occurred in either group. Optimal screw placement was confirmed in all UBB cases versus 87.5% in the HSD group (P = 0.139). Both groups demonstrated significant improvements in Japanese Orthopedic Association scores with no intergroup difference. The fusion rate was 100% in the UBB group and 87.5% in the HSD group, not statistically different (P = 0.381). Conclusions: UBB is a viable alternative for C1 pedicle screw placement, associated with reduced blood loss and shorter operation time, while achieving comparable clinical outcomes to the conventional HSD method. Nevertheless, further research with a larger sample size is needed.

Abstract:Objective To research the biomechanical characteristics and clinical applicant of posterior transarticular screws (Magerl technique) with bene graft in atlantoaxial instability.Methods 30 fresh cadaver human cervical spine specimens were obtained and randomized subdivided into 5 groups:intact group,atlantoaxial destabilized group,atlantoaxial destabilized and fixed with pedicle screws group,fixed with Magerl screws group and fixed with Maged screws combined with Brooks way group.ROM(range of motion)and NZ(neutral zone)of extend,flex,right and left lateral bend,right and left rotation of each specimen in 5 groups were measured.Transarticular screws were placed in 23 patients with atlantoaxial instability from 2000 Feb.to 2005 Jan.,with 16 males and 7 females,age from 17 to 62 years.20 of them were traumatic dislocation and 3 were congenital instable.Results Biomechanical study indicated that stability of all 6 degrees were Magerl+Brooks＞Magerl screws＞Pedicle screws＞Intact＞Destabilized.23 patient were fixed with 46 Maged screws in total,none were suffered from vertebral artery injury and neurological deterioration.All patients were followed-up from 4 to 36 months.2 of them complained that difficult in rotation of head.Conclusion Single use of Magerl screw can provide enough biomechanical stable for adantoaxial instability,and is an effective,low-risky way combined with intra-spinal-process bone graft.
Finite Element Analysis of Horizontal Screw-Screw Crosslink Used in C1-C2 Pedicle Screw-Rod Fixation

Abstract:BACKGROUND In the craniocervical junction, a C1-C2 pedicle screw-rod (PSR) fixation is applied to provide stability. The horizontal rod-rod crosslink (hR-R CL) is often used to enhance segmental posterior instrumentation. However, the biomechanics of the alternative horizontal screw-screw crosslink (hS-S CL) in the craniocervical junction are unclear. MATERIAL AND METHODS A nonlinear atlantoaxial instability 3-dimensional C1-C2 finite element model was constructed using computed tomography images. On this basis, 2 fixation models were established with C1-C2 PSR fixation using (1) a rod-rod crosslink (R-R CL), and (2) a screw-screw crosslink (S-S CL). Range of motion (ROM) of the atlantoaxial joint, stress distribution of the implants, and maximum stress value of the vertebral bodies were calculated and compared under 4 loading conditions, including flexion, extension, lateral bending, and axial rotation. RESULTS Atlantoaxial joint ROM was reduced by 90.19% to 98.5% with the hR-R CL, and by 90.1% to 98.7% with the hS-S CL, compared with the instability model. During axial rotation, the total stress peak of the PSR fixation was smaller with hS-S CL than with hR-R CL. The peak stress values of the vertebral bodies were comparable between the 2 fixation models. CONCLUSIONS The 2 tested crosslink models provided comparable stability. However, during axial rotation, the total stress peak of hS-S CL fixation was smaller than that of hR-R CL fixation. Since the atlantoaxial joint primarily functions as a rotational joint, our results suggested that the use of hS-S CL can provide a more stable environment for the implants.
A Technique for the In Vivo Study of Three-dimensional Cervical Segmental Motion Characteristics After Anterior Screw Fixation for Odontoid Process Fractures

Abstract:Study design: Kinematics of the cervical spine was investigated using cone beam computed tomography (CBCT) images combined with three dimensions to three dimensions (3D-3D) registration technology in patients after anterior odontoid screw fixation (AOSF) surgery. Objective: The aim of this study was to investigate in vivo 3D cervical motion characteristics of patients who had undergone AOSF surgeries. Summary of background data: AOSF surgery is a classic surgical method for the treatment of odontoid fracture, but there are few studies that investigated its effect on in vivo biomechanics of the cervical spine. Postoperative biomechanical characters of the atlantoaxial joint (C1-C2) and the caudal adjacent segment (C2-C3) have yet to be clarified. Methods: The study involved 14 patients subjected to a procedure of AOSF with lag screw. Subjects were matched with 14 healthy controls. All subjects underwent CBCT scanning of the cervical spine under seven functional positions. A 3D-3D registration was performed for each vertebra at each functional position to calculate the segmental motion characteristics. The ranges of motion (ROMs) of the C1-C2, C2-C3, and the overall cervical spine (C1-C7) for each of the functional positions were determined. Results: The ROMs of the AOSF group were significantly (P < 0.05) smaller than the control group in flexion-extension positions for the C1-C2 (7.0°vs.11.0°), C2-C3 (3.7°vs.6.7°) and C1-C7 (43.3°vs.54.4°). The twisting ROM of the C1-C2 was 39.3° in the AOSF group and 65.7° in the control (P < 0.05), the bending ROM of the C2-C3 was 2.8° in the AOSF group and 8.9° in the control (P < 0.05). The twisting ROM of C1-C7 segment was 63.2° for the AOSF and 98.1° for the healthy control groups (P < 0.05). Conclusion: Although AOSF surgery reduced the flexion-extension ROMs of all investigated spinal segments, additionally, it reduced twisting ROMs of C1-C2 and C1-C7, but only lateral bending ROM of C2-C3, when compared with the control group. The data implied that the AOSF surgery would result in different biomechanics changes in the atlantoaxial segment and caudal adjacent segment. Longer-term follow-up studies of larger patient cohorts are necessary to evaluate the clinical outcomes of patients after the AOSF surgery.Level of Evidence: 3.
Anatomical Study of Posterior Atlanto-Occipital Transarticular Screw Fixation

Abstract:Objective Explicit the technical parameters in posterior atlanto-occipital transarticular screw fixation; discuss the clinic significance of craniovertebral junction fixation with transarticular screws. Methods Posterior atlanto-occipital transarticular K-pin implantation was performed under visual control on 20 dried bony specimens that contained complete atlanto-occipital articulations. Then the angle and length of the atlanto-occipital transarticular screw trajectory were measured on the cephalocaudal and lateral X-ray filems. The atlanto-occipital articulations of 30 normal adult Chinese were three-dimensionally reconstructed by CT, on which the adjustable range of allowable angle of the posterior atlanto-occipital transarticular screw trajectory were measured. On 12 fresh craniovertebral junction specimens, posterior occipitoatlantoaxial transarticular screw fixation was performed. Roentgen rays were examined, and three-dimensional CT was reconstructed postoperatively to study the positions of screws. Results The atlanto-occipital transarticular screw trajectory was placed cephalad tilting in sagittal plane and medial tilting in coronary plane. There was little differences between leftside and rightside, but without significence(P0.05). The ideally placed angles of the screws were 53.3°±3.4° upper tilting in sagittal plane and 20.0°±2.6° inside tilting in coronary plane. The ideal length of the trajectory was (29.28±2.46) mm. The allowable range for adjusting the direction of the trajectory was 24.9°～74.6° on sagittal plane, and 0.7°～40.5° on coronary plane. Imageological examination on fresh specimens demonstrated that atlanto-occipital transarticular screw fixation according the above results ensured insertion of the trajectory passing through the atlanto-occipital articulation without running the risk of going into the neural tube. Conclusion Posterior atlano-occipital transarticular fixation is clinical feasible; but there are some directions for implantating the screws. When combining with C1,2 transarticular fixation, posterior atlano-occipital transarticular fixation can be considered as an ideal clinical fixation modality for craniovertebral fusion.

Abstract:BACKGROUND: Atlas occipitalization and congenital C2-3 fusion often result in atlantoaxial dislocation (AAD) and superior odontoid migration that requires occipitocervical fixation. The widely used technique is posterior occiput-C2 fixation with pedicle screws. However, congenital C2-3 fusion cases tend to have thinner C2 pedicles that are inadequate for normal-sized pedicle screw fixation. With the presence of AAD, the strength of the fixation is further compromised as the C2 pedicle screws (C2PS) sustain considerable cephalic shearing force during the reduction procedure. Therefore, a novel technique has been developed to augment the C2 pedicle screw fixation with a strengthening cable.OBJECTIVE: To introduce and assess this new technique.METHODS: Seventy-six patients who underwent this procedure were reviewed. The position of the instrument and resultant fusion were examined retrospectively. In the biomechanical test, 6 fresh specimens were subjected to 2 types of fixation in the order of Oc-C2 screw-plate fixation followed by additional use of strengthening cable. Under 3 loading modes (extension-flexion, lateral bending, and axial rotation), the relative movement between the occiput and C2 was measured and compared in the form of range of motion.RESULTS: The average follow-up time was 26 months. Solid fusion was achieved in 75 patients (98.7%) as assessed radiologically. The only patient who experienced hardware failure eventually obtained solid fusion between the occiput and C2 after revision. Biomechanically, there was significant difference between the occiput and C2 fixation and cable-strengthened fixation in range of motion for all modes.CONCLUSION: This technique is a promising option for the treatment of AAD with congenital C2-3 fusion and occipitalization. Biomechanically, this technique can reduce the occipital-axial motion significantly compared with occiput-C2 fixation.
Computed Tomographic Morphometric Analysis of Pediatric C1 Posterior Arch Crossing Screw Fixation for Atlantoaxial Instability

Abstract:Study design: A CT-based study of pediatric atlas. Objective: The aim of this study was to identify morphometric patterns and dimensions of the pediatric C1 posterior arch to establish guidelines for the posterior arch crossing screw (PACS) fixation. Summary of background data: The PACS fixation is an alternative or supplement to the C1 lateral mass screw or pedicle screw constructs. However, the anatomic feasibility of this technique has not been well documented in the pediatric population. Methods: A total of 123 pediatric patients were assigned into 6 groups based on age at an interval of 3 years. The C1 posterior arch unsuited to PACS fixation was classified as type 1 with synchondrosis, and type 2 without synchondrosis. Three subtypes of type 2 were included: the underdeveloped tubercle, the flat arch, and the undersized tubercle (height < 7 mm or width < 3.5 mm), respectively. The C1 posterior tubercle height and arch width, the projected PACS length and angle were measured, respectively. Results: There were 19 patients of type 1 and 68 patients of type 2. Type 1 was mostly found in patients under age 3 years, whereas type 2 was largely observed in patients older than 4 years. The tubercle size was the most limiting variable for successful PACS placement. The PACS fixation was feasible for 42% patients over age 7 years and only 5% patients under age 6 years. The posterior tubercle height increased with age, whereas the tubercle width, the PACS length, and axial-plane angle increased with age up to 12 years. The PACS length was between 16 and 18 mm with the angle between 18° and 23° for patients over age 7 years. Conclusion: The PACS fixation was anatomically feasible in patients over age 7 years, but contraindicated in patients under age 6 years. Pediatric patients for the PACS fixation should be assessed before surgery. Level of evidence: 4.
Independent Correlation of the C1–2 Cobb Angle With Patient-Reported Outcomes After Correcting Chronic Atlantoaxial Instability

Abstract:OBJECTIVE: To investigate three-planar radiographic results and patient-reported outcomes (PROs) after correcting chronic atlantoaxial instability (AAI) by translaminar screw (TLS) and pedicle screw (PS) fixation, and to explore the potential association of atlantoaxial realignment with PRO improvements.METHODS: Twenty-three patients who underwent C1 lateral mass screw (LMS)-C2 TLS and 29 who underwent C1 LMS-C2 PS with ≥ 2 years of follow-up were retrospectively analyzed. Three-planar (sagittal, coronal, and axial) radiographic parameters were measured. PROs including the Neck Disability Index (NDI), Japanese Orthopaedic Association (JOA) score and the Short Form 36 Physical Component Summary (SF-36 PCS) were documented. Factors potentially associated with PROs were identified.RESULTS: The radiographic parameters significantly changed postoperatively except the C1-2 midlines' intersection angle in the TLS group (p = 0.073) and posterior atlanto-dens interval in both groups (p = 0.283, p = 0.271, respectively). The difference in bilateral odontoid lateral mass interspaces at last follow-up was better corrected in the TLS group than in the PS group (p = 0.010). Postoperative PROs had significantly improved in both groups (all p < 0.05). Thereinto, NDI at last follow-up was significantly lower in the TLS group compared with PS group (p = 0.013). In addition, blood loss and operative time were obviously lesser in TLS group compared with PS group (p = 0.010, p = 0.004, respectively). Multivariable regression analysis revealed that a change in C1-2 Cobb angle was independently correlated to PROs improvement (NDI: β = -0.435, p = 0.003; JOA score: β = 0.111, p = 0.033; SF-36 PCS: β = 1.013, p = 0.024, respectively), also age ≤ 40 years was independently associated with NDI (β = 5.40, p = 0.002).CONCLUSION: Three-planar AAI should be reconstructed by C1 LMS-C2 PS fixation, while sagittal or coronal AAI could be corrected by C1 LMS-C2 TLS fixation. PROs may improve after atlantoaxial reconstruction in patients with chronic AAI. The C1-2 Cobb angle is an independent predictor of PROs after correcting chronic AAI, as is age ≤ 40 years for postoperative NDI.
