Abstract:Objectives: To evaluate the timing of definitive fixation of tibial plateau fractures relative to fasciotomy closure with regards to alignment and articular reduction. Methods: Design : Retrospective Case Series Setting: Four Level I trauma centers Patient Selection Criteria: Patients with tibial plateau fractures with ipsilateral compartment syndrome treated with fasciotomy between 2006-2018 met inclusion criteria. Open fractures, patients under the age of 18, patients with missed or delayed treatment of compartment syndrome, patients with diagnosis of compartment syndrome after surgical fixation, and patients whose plateau fracture was not treated with open reduction and internal fixation (ORIF) were excluded. Patients were divided into two groups depending on the relative timing of fixation to fasciotomy closure: early fixation (EF) was defined as fixation before or at the time of fasciotomy closure and delayed fixation (DF) was defined as fixation after fasciotomy closure. Outcome Measures and Comparisons: Radiographic limb alignment (categorized as anatomic alignment (no varus/valgus), ≤ 5° varus/valgus, or > 5° varus/valgus) and articular reduction (categorized as anatomical alignment with no residual gap or step off, 5mm of articular surface step off) was compared between early and delayed fixation groups. Additionally, superficial and deep infection rates were compared between those in the EF and DF cohorts. Subgroup analysis within the EF cohort was performed to compare baseline characteristics and outcomes between those that received fixation prior to closure and those that underwent concurrent fixation and closure within one operative episode. Results: A total of 131 patients met inclusion criteria for this study. Sixty-four patients (48.9%) were stratified into the delayed fixation group and 67 patients (51.1%) were stratified into early fixation. In the EF cohort, 57 (85.1%) were male with an average age of 45.3 ± 13.6 years and an average BMI of 31.0 ± 5.9. The DF cohort was primarily male (44, 68.8%), with an average age of 46.6 ±13.9 and an average BMI of 28.4 ± 7.9. Fracture pattern distribution did not differ significantly between the early and delayed fixation cohorts (p = 0.754 for Schatzker classification and p = 0.569 for AO/OTA classification). The relative risk of infection for the DF cohort was 2.17 [95%CI = 1.04, 4.54] compared to the EF cohort. . Patients in the early fixation cohort were significantly more likely to have anatomic articular reduction compared to their delayed fixation counterparts (37.5% vs. 52.2%; p < 0.001). Conclusions: This study demonstrated higher rates of anatomic articular reduction in patients that underwent fixation of tibial plateau fractures prior to or at the time of fasciotomy closure for acute compartment syndrome compared to their counterparts that underwent definitive fixation for tibial plateau fracture after fasciotomy closure. The relative risk of overall infection in the for those that underwent fasciotomy closure after definitive fixation for tibial plateau fracture was 2.17 compared to the cohort that underwent closure before or concomitantly with definitive fixation. Abstract:The purpose of this study was to report on the treatment, results, and longer-term outcomes of patients who sustained a tibial plateau fracture with an associated leg compartment syndrome (CS). A total of 766 patients who sustained 766 tibial plateau fractures met inclusion criteria. Fourteen patients (1.8%) were diagnosed with CS in association with a tibial plateau fracture during their initial hospitalization, 13 at the time of presentation and 1 delayed. The treatment protocol consisted of initial external fixation and fasciotomy, followed by irrigation and debridement, and eventual closure. Fasciotomy cases included 2/14 (14.3%) single incision approaches and 12/14 (85.7%) dual incision approaches. Operative treatment of the tibial plateau fracture was performed at the time of final closure or once soft tissues were permitted. One case of CS that developed following definitive fixation was treated with fasciotomy and delayed primary closure after initial stabilization. Ten (71.4%) were available at 1-year follow-up. We compared these 10 cases to the patients with operative tibial plateau fractures without CS to assess for surgical, radiographic, clinical, and functional outcomes. We used a propensity match based on age, body mass index, sex, Charleson comorbidity index, and fracture type to reduce the presence of confounding biases. Standard statistical methods were employed. Those in the CS cohort were younger males ( p < 0.05). At latest follow-up, function did not differ between those in the CS group compared with the non-CS cohort ( p > 0.05). Clinically, knee flexion (130.7 vs. 126; p = 0.548), residual depression (0.5 vs. 0.2; p = 0.365), knee alignment (87.7 vs. 88.3; p = 0.470), and visual analog scale pain scores (3.0 vs. 2.4; p = 0.763) did not differ between the cohorts. Although infection was higher in the CS cohort, the overall complication rates did not differ between the CS patients and non-CS cohort ( p > 0.05). Early identification and standardized treatment protocols for the management of CS that develops in association with a tibial plateau fracture lead to outcome scores that were not significantly different from patients who did not develop CS. Temporary Stabilization of Tibia Fractures: Does External Fixation or Temporary Plate Fixation Result in Better Outcomes?

Abstract:Background: Provisional stabilization of high-energy tibia fractures using temporary plate fixation (TPF) or external fixation (ex-fix) prior to definitive medullary nailing (MN) is a strategy common in damage control orthopaedics. There is a lack of comprehensive data evaluating outcomes between these methods. This study compares outcomes of patients stabilized with either TPF or ex-fix, and with early definitive MN only, assessing complications including nonunion and deep infection. Methods: A retrospective review was performed on adult patients with tibia fractures treated with MN followed until fracture union (≥3 months) at a single level-1 trauma center from 2014 to 2022. Medical records were evaluated for nonunion and deep infection. Demographics, injury characteristics, and fixation methods were recorded. Significance between patients who underwent TPF and ex-fix was compared with a matched cohort of early MN using Pearson's exact tests, independent t-tests, and one-way ANOVA, depending on the appropriate variable. Results: 81 patients were included; 27 were temporized with TPF (n = 12) or ex-fix (n = 15). 54 early MN cases defined the matched cohort. All groups had similar patient and fracture characteristics. The difference in rates of nonunion between groups was significant, with TPF, ex-fix, and early MN groups at 17, 40, and 11% respectively (p = 0.027). Early MN had lower rates of nonunion (11% vs. 40%, p = 0.017) and deep infection (13% vs. 40%, p = 0.028) compared to ex-fix. Conclusion: Temporary ex-fix followed by staged MN was associated with higher rates of nonunion and deep infection. There was no difference in complication rates between TPF and early definitive MN. These data suggest that ex-fix followed by MN of tibia fractures should be avoided in favor of early definitive MN when possible. If temporization is needed, TPF may be a better option than ex-fix. Abstract:Purpose: The purpose of this study was to determine the rates of compartment syndrome and other early complications following outpatient open reduction and internal fixation (ORIF) of tibial plateau fractures. Methods: This was a retrospective cohort at a single US level I academic trauma centre of patients with tibial plateau fractures managed operatively. Inpatients received their definitive ORIF during their index hospital stay and were admitted post-operatively following ORIF. Outpatients were scheduled for ambulatory surgery during definitive ORIF. Exclusion criteria for outpatient surgery included compartment syndrome, polytrauma, open types IIIb/IIIc, and patients who received any internal fixation during index presentation. The primary outcome measure was post-operative compartment syndrome. Secondary outcomes were return to the 90-day return to the ED, 90-day readmission, surgical wound infection, thromboembolism, and 90-day mortality. An intention-to-treat (ITT) and as-treated (AT) analyses were performed. Results: Totally, 71 inpatients and 47 outpatients were included. There were no cases of post-operative compartment syndrome. In the ITT analysis, there were no differences for inpatients vs outpatients for 90-day re-admission (22.5% vs 12.8%, p = 0.275), 90-day return to the ED (35.2% vs 17.0%, p = 0.052), infection (12.7% vs 2.1%, p = 0.094), DVT (7% vs 4.3%, p = 0.819), or PE 1.4% vs 0.0%, p = 1.000). The AT analysis showed a significantly higher 90-day re-admission (26.9% vs 2.5%, p = 0.003) and 90-day ED visit (38.5% vs 7.5%, p = 0.001) rate in the inpatient group. Conclusions: Appropriately selected patients with isolated tibial plateau fractures can have non-inferior rates of compartment syndrome and post-operative complications when compared to inpatients.

Abstract:Background: Low energy tibia plateau fractures can be treated with definitive surgery within 48 hours. High energy tibia plateau fractures require a staged treatment plan that allows healing of the soft tissue envelope, with avoidance of devastating complications such as compartment syndrome and chronic infection. The aim of this study was to compare outcomes of definitive tibial plateau surgery performed within 21 days from the date of injury to those done after 21 days in a resource-restricted center. Methods: A retrospective cohort study was conducted. Group A consisted of patients that received their definitive surgery within 21 days of the injury. Group B were those who had definitive surgery after 21 days from the injury. Outcomes of interest included infectious complications (suggestive- and confirmed-fracture related infection (FRI)) and clinical outcomes (pain and range of motion). All patients were treated in one center between June 2018 and June 2020. Results: Forty-four (44) patients were included in the study. Twenty-three patients (23) were in group A while 21 were in group B. The median age was 40 (Inter quartile range [IQR]: 36 - 49) years in group A and 37 (IQR: 30 - 49) years in group B. Group A consisted of 15 males and 8 females while group B had 8 males and 13 females. Suggestive FRI rate in group A was 26% compared to 33% in group B. Confirmed FRI occurred in 2% of patients in group A compared to none in group B. The pain visual analogue scale (VAS) in group A had a median of 2 (IQR: 0 - 4) and 3 (IQR: 1 - 3) in group B. Range of motion was found to be 94.13 (range: 40 - 130) degrees on average for group A and 99.5 (range: 70 - 120) degrees for group B. Conclusion: Outcomes of surgeries performed after a delayed period of 21 days still were comparable to those performed before 21 days.

Abstract:OBJECTIVE:To analysis the early complications of tibial fracture and its related factors, and propose a solution.METHODS:From December 2003 to December 2013,38 patients with early complications of tibial plateau fracture after operation were retrospectively analyzed. There were 35 males and 3 females, aged from 37 to 69 years old (averaged 42.3 years). According to Schatzker classification, 3 cases were classified as type II, 2 cases as type III, 2 cases as type IV, 19 cases as type V, 12 cases as type VI. The intervals between injury and operation ranged from 9 hours to 9 days, 26 cases within 3 days. Fifteen cases were treated with internal fixation of plates and 23 were treated by plate fixation and bone transplantation. Early complications included skin necrosis in 15 cases, infection in 6 cases, osteofascial compartment syndrome in 3 cases, common peroneal nerve injury in 2 cases, the superficial peroneal nerve injury in 3 cases, popliteal artery injury in 2 cases, loss of reduction in 7 cases.RESULTS:The wound of 14 cases healed at the first stage and 24 cases healed delay. Hospitalization days ranged from 7 to 67 days (averaged 25.6 days). All patients were followed up for 12 to 36 months with an average of 16.4 months. The fracture healing time ranged from 3 to 9 months (averaged 6.9 months). According to Merchant knee function evaluation criteria, the results were excellent in 19 cases, good in 12, fair in 5 and poor in 2.CONCLUSION:Early complications of tibial fracture after operation is closely associated with the severe fracture complexity and related with preoperative preparation, surgical timing, operation incision selection and surgical technique. Early detection and timely processing reduce damage.
Effect of surgical fixation timing on in-hospital mortality and morbidity of distal femur fractures

Abstract:Introduction: The literature lacks a large-scale study investigating the effect of surgical fixation timing on early mortality and morbidity outcomes in distal femur fractures. The aims of this study were to determine the effect of fixation timing on in-hospital mortality and morbidity outcomes for operatively treated distal femur fractures retrospectively using large database data. Methods: This study is a retrospective analysis using data from the National Trauma Data Bank. Patients were stratified into a fragility cohort (age ≥ 60, ISS < 16) and polytrauma cohort (age < 60, ISS ≥16), with both cohorts analyzed separately. Within each cohort, patients were split into three fixation timing groups: within 24 h, between 24 and 48 h, and greater than 48 h from presentation to the hospital. Fixation-timing groups were compared based on the primary outcome of in-hospital mortality rate. Secondary outcomes included hospital length of stay (LOS), ICU length of stay (ICU LOS), days on a ventilator, and complications. Results: The fragility and polytrauma cohorts included 22,045 and 5,905 patients, respectively. The in-hospital mortality rate was 1.23 % in the fragility cohort and 2.56 % in the polytrauma cohort. Multivariate analysis of the fragility cohort showed that fixation greater than 48 h from time of presentation was associated with increased mortality compared to fixation within 24 h (OR 1.89, CI: 1.26-2.83, p=0.002) and between 24 and 48 h (OR 1.63, CI: 1.23-2.15, p<0.001). In the polytrauma cohort, multivariate analysis showed no significant mortality differences between fixation timing groups. Multivariate analysis of morbidity outcomes in both cohorts showed that fixation greater than 48 h was associated with increased LOS, ICU LOS, ventilator days, and complications compared to fixation within 24 h. In the polytrauma cohort, fixation between 24 and 48 h was associated with decreased LOS, ICU LOS, and complications compared to the other two timing groups. Conclusions: Fixation of distal femur fractures before 48 h from presentation may lead to improved mortality and morbidity in older, lower injury severity patients. No significant mortality benefit was observed in younger, polytrauma fractures. Further prospective work is needed to validate these findings.

Abstract:Introduction: Tibial plateau fractures are due to high energy trauma brought about by axial compression forces and associated varus or valgus component. Objective: Patients diagnosed with tibial plateau fractures from January to December 2018 treated with internal vs external fixation will be described according to their Schatzker classification. The study further aims to compare the functional outcomes between the two groups in terms of surgery done. Methods: A chart review determined the distribution of demographics. The Modified Rasmussen Score (MRS) was used to determine the clinical and radiographic parameters after taking a new knee radiograph and assessment from the rehabilitation department. The MRS determined the functional outcomes of the said patients. Ethical considerations and proper informed consent were upheld after being reviewed by the hospital's research committee. Results: Out of 48 patients, 35 underwent internal fixation via open reduction using plates and/or screws, while 13 underwent external fixation using hybrid external fixator. The demographic profile showed mostly males between ages 20 to 49 years old. Most cases were due to vehicular accidents affecting the left lower extremity. In terms of Schatzker classification, the most common was type VI. The computed mean MRS of the internal fixation group was 30.43 while the external fixation group was 30.00, generally showing no significant difference. Conclusion: Surgical intervention of tibial plateau fractures aims for anatomic reduction using internal or external fixation. There was no significant difference on the functional outcome of the two groups despite classifying the respondents according to Schatzker type, hence we can conclude that external fixation be chosen as the treatment of choice for tibial plateau fractures when properly indicated.
Strategies to minimize soft tissues and septic complications in staged management of high-energy proximal tibia fractures

Abstract:Background: Soft tissues (wound dehiscence, skin necrosis) and septic (wound infection, osteomyelitis) complications have been historically recognized as the most frequent complications in surgical treatment of high-energy proximal tibia fractures (PTFs). Staged management with a temporary external fixator is a commonly accepted strategy to prevent these complications. Nonetheless, there is a lack of evidence about when and how definitive external or internal definitive fixation should be chosen, and which variables are more relevant in determining soft tissues and septic complications risk. The aim of the present study is to retrospectively evaluate at midterm follow-up the results of a staged management protocol applied in a single trauma center for selective PTFs. Methods: The study population included 24 cases of high-energy PTFs treated with spanning external fixation followed by delayed internal fixation. Severity of soft tissues damage and fracture type, timing of definitive treatment, clinical (ROM, knee stability, WOMAC and IOWA scores) and radiographic results as well as complications were recorded. Results and conclusion: Complex fracture patterns were prevalent (AO C3 58.3%, Schatzker V-VI 79.1%), with severe soft tissues damage in 50% of cases. Mean time to definitive internal fixation was 6 days, with double-plate fixation mostly chosen. Clinical results were highly satisfying, with mean WOMAC and IOWA scores as 21.3 and 82.5, respectively. Soft tissue complication incidence was very low, with a single case of wound superficial infection (4.3%) and no cases (0%) of deep infection, skin necrosis or osteomyelitis. Staged management of high-energy PTFs leads to satisfying clinical and radiographic results with few complications in selected patients.

Abstract:Schatzker type IV tibial plateau fractures occur on the inner side of the tibial plateau and often involve the posterior lateral aspect. Schatzker type V tibial plateau fractures are bilateral plateau fractures, often resulting from high-energy trauma such as car accidents or falls. [ 1 ] In most cases, complex tibial plateau fractures are often associated with damage to the soft tissues, nerves, and blood vessels around the knee joint, and sometimes they may even involve dislocation of the knee joint. The tibial plateau is a critical bone structure in the human body, supporting the bending and extension functions of the knee joint. [ 2 ] As a major weight-bearing joint, when a partial tibial plateau fracture occurs in the knee joint, it severely limits its function, causing inconvenience in daily life, and may even lead to lower limb disability, having a profound impact on the patient's physical and psychological well-being. [ 3 ] When dealing with complex tibial plateau fractures, the key lies in accurately repositioning the damaged joint and providing stable internal fixation. This not only effectively corrects the alignment of the lower limb but also restores the integrity of the joint surface. Therefore, such surgeries not only require doctors to possess a high level of surgical skills but also demand the correct selection of appropriate internal fixation materials. Although traditional bilateral support plate fixation surgery provides good results in stabilizing Schatzker IV and Schatzker V tibial plateau fractures, during bilateral support plate fixation, the periosteum at the fracture site experiences additional pressure. This excessive compression may not only lead to periosteal damage but also affect the blood supply to the fracture site, thereby delaying the healing process of the fracture. [ 4 ] Compared to the limitations of traditional double-support steel plate treatment, locked plate fixation has shown significant advantages in the treatment of complex tibial plateau fractures. These advantages include providing stable fixation, employing minimally invasive techniques, and featuring point contact, all of which contribute to maintaining the stability of the biomechanical structure. This ensures solid fixation of the fracture site and ensures good blood supply, thereby promoting rapid fracture healing. Especially for patients with comminuted and extra-articular tibial plateau fractures, locked plate fixation has become the preferred method for treating such fractures. [ 5 ] In light of this, the aim of this study is to analyze the therapeutic effects of internal and external dual-plate fixation surgery on patients with posterior lateral tibial plateau fractures and investigate its safety. It is hoped that this study will provide empirical evidence for future research on the treatment methods of complex tibial plateau fractures.

Patients/Participants Patients 18 years and older with distal femur fractures treated with locked lateral plating Intervention Early full weight bearing (defined as less than 30 days from date of surgery) versus restricted post-operative weight bearing Main Outcome Measurements Composite complication comprising malunion, nonunion, surgical site infection, re-admission, or death Results 270 distal femur fractures were reviewed, with 165 meeting inclusion criteria. 21 patients had been allowed early full weight bearing. Fractures were divided into two groups based on when full weight bearing was allowed post-operatively. The two groups had similar fractures as determined by the distribution of AO distal femur fracture and Su periprosthetic femur fracture classifications. The early weight bearing group was significantly older and more comorbid. Despite being older, more comorbid, and allowed early full weight bearing on their fracture fixation construct, there was no difference in the rate of composite complications between groups. Conclusion Our data contributes to the small, but growing body of literature that has found no increased rate of fracture related complications in surgically treated distal femur fractures allowed early post-operative weight bearing. Abstract:Postoperative management of tibial plateau fractures classically involves a prolonged period between 10 and 12 weeks of nonweight bearing or partial weight bearing. In recent years, there has been some support for earlier weight-bearing protocols although this remains controversial. The goal of this study was to investigate the difference in outcomes between early weight-bearing (EWB) and traditional weight-bearing (TWB) protocols. This investigation is a retrospective review of 92 patients treated with open reduction and internal fixation of tibial plateau fractures at a single institution, from August 2018 to September 2020. Subjects were divided into EWB (< 10 weeks) and traditional nonweight bearing groups (≥ 10 weeks). Key outcome measures collected include injury classification, mechanism of injury, surgical fixation method, bone grafting, time to full weight bearing, radiographic time to union, range-of-motion, all-cause complications, and subsidence at an average follow-up time of 1 year. The EWB group had an earlier average time to weight bearing versus the TWB group (6.5 ± 1.4 vs. 11.8 ± 2.3 weeks, p < 0.0001). There was no difference in the classification of fractures treated between the two groups, with Arbeitsgemeinschaft für Osteosynthesefragen/Orthopaedic Trauma Association B3 fractures the most common in the EWB group, and C3 fractures the most common in the TWB group. Radiographic time to union was no different between the two groups (93.5 ± 53.7 days for EWB vs. 103.7 ± 77.6 days for TWB, p = 0.49). There was no significant difference in complication rates or subsidence. Following operative treatment of tibial plateau fractures, patients who underwent a weight-bearing protocol earlier than 10 weeks were able to recover faster with similar outcomes and complications compared with patients who started weight bearing after 10 weeks or more. Abstract:We address the problem of semi-supervised domain adaptation of classification algorithms through deep Q-learning. The core idea is to consider the predictions of a source domain network on target domain data as noisy labels, and learn a policy to sample from this data so as to maximize classification accuracy on a small annotated reward partition of the target domain. Our experiments show that learned sampling policies construct labeled sets that improve accuracies of visual classifiers over baselines.

Abstract:ObjectiveThe aim of this study was to identify risk factors for surgical site infections and to quantify the contribution of independent risk factors to the probability of developing infection after definitive fixation of tibial plateau fractures in adult patients.MethodsA retrospective analysis was performed at a level I trauma center between January 2004 and December 2010. Data were collected from a review of the patient’s electronic medical records. A total of 251 consecutive patients (256 cases) were divided into two groups, those with surgical site infections and those without surgical site infections. Preoperative and perioperative variables were compared between these groups, and risk factors were determined by univariate analyses and multivariate logistic regression. Variables analyzed included age, gender, smoking history, diabetes, presence of an open fracture, presence of compartment syndrome, Schatzker classification, polytrauma status, ICU stay, time from injury to surgery, use of temporary external fixation, surgical approach, surgical fixation, operative time, and use of a drain.ResultsThe overall rate of surgical site infection after ORIF of tibial plateau fractures during the 7 years of this study was 7.8 % (20 of 256). The most common causative pathogens was Staphylococcus aureus (n = 15, 75 %). Independent predictors of surgical site infection identified by multivariate analyses were open tibial plateau fracture (odds ratio = 3.9; 95 % CI = 1.3–11.6; p = 0.015) and operative time (odds ratio = 2.7; 95 % CI = 1.6–4.4; p < 0.001). The presence of compartment syndrome (odds ratio = 3.4; 95 % CI = 0.7–15.9; p = 0.119), use of temporary external fixation (odds ratio = 0.5; 95 % CI = 0.2–1.7; p = 0.298), and ICU stay (odds ratio = 1.0; 95 % CI = 1.0–1.1; p = 0.074) were not determined to be independent predictors of surgical site infection.ConclusionsBoth open fracture and operative time are independent risks factors for postoperative infection.

Abstract:There is still no consensus on the length of the non-bearing period required for tibial plateau fractures (TPFs) treated surgically. Several studies showed that full weight bearing immediately postoperatively does not affect fixation or cause joint collapse in patients with TPF. While there are concerns about weight bearing in the elderly due to fragile bone quality, their physical disability often makes it difficult to carry out activities of daily life when weight bearing is not allowed. The purpose of this study was to assess differences in clinical and radiographic outcomes between an early weight-bearing (EWB) group and a non-weight-bearing (NWB) group following TPF in elderly patients.

Abstract:Objectives: To evaluate the outcomes of staged management with external fixation (ex-fix) prior to definitive fixation of distal femur fractures. Methods: Design: Retrospective cohort. Setting: Single Level I Trauma Center. Patient selection criteria: Adults treated operatively between 2004 and 2019 for distal femur fractures (OTA/AO 33A/33C) were identified using Current Procedural Terminology codes. Excluded cases were those with screw only fixation, acute distal femur replacement, 33B fracture pattern, no radiographs available, or did not have 6-months of follow-up. Outcome measures and comparisons: Postoperative complication rates including surgical site infection [SSI], reoperation to promote bone healing, final knee arc of motion <90 degrees, heterotopic ossification formation, and reoperation for stiffness were compared between patients treated with ex-fix prior to definitive fixation and those not requiring ex-fix. Results: A total of 407 patients were included with a mean follow-up of 27 months (median [IQR] of 12 [7,33] months), (range 6-192 months). Most patients were male (52%) with an average age of 48 [Range: 18-92] years. Ex-fix was utilized in 150 (37%) cases and 257 (63%) cases underwent primary definitive fixation. There was no difference in SSI rates (p=0.12), final knee arc of motion <90 degrees (p=0.51), and reoperation for stiffness (p=0.41) between the ex-fix and no ex-fix groups. The 150 patients requiring ex-fix spent an average of 4.2 days (SD 3.3) in the ex-fix before definitive fixation. These patients were further analyzed by comparing the duration of time spent in ex-fix, <4 days (n=82) and ≥4 days (n=68). Despite longer time spent in ex-fix prior to definitive fixation, there was again no significant difference in any of the complication and reoperation rates when comparing the two groups, including final knee arc of motion <90 degrees (p=0.63), reoperation for stiffness (p=1.00), and SSI (p=0.79). Conclusion: Ex-fix of distal femur fractures as a means of temporary stabilization prior to definitive ORIF does not increase the risk of complications such as SSI, final knee arc of motion <90 degrees, or reoperations for bone healing or stiffness when compared to single stage ORIF of distal femur fractures. Relationship Between Perioperative Regional Anesthesia and the Risk of Missing Acute Compartment Syndrome Following Tibia Fractures

Abstract:Background: The aim of this study was to determine the incidence of missed compartment syndrome in tibia fractures treated with and without regional anesthesia. Methods: A retrospective chart review was performed of patients with operative tibial shaft or plateau fractures at a single level-one trauma hospital between January 2015 and April 2022 with a minimum of 3-month follow-up. Patients under 18 years of age, an ipsilateral knee dislocation, known neurologic injury at presentation, or prophylactic fasciotomy were excluded. We defined missed acute compartment syndrome (ACS) as a postinjury motor deficit still present at the 3-month postoperative appointment. For patients that received a peripheral nerve block, we recorded whether a continuous perineural catheter or one-time single-shot injection was performed, and the number of nerves blocked. Incidence rates for ACS were calculated with exact binomial 95% confidence intervals (CIs). Morphine milligram equivalents (MMEs) consumed 24 hours after surgery, use of nerve block, nerve block timing, and type of block were compared using Mann-Whitney and Kruskal-Wallis nonparametric tests. Statistical significance was defined as P < .05. Results: The incidence of compartment syndrome diagnosed and treated during index hospitalization was 2.2% (17/791, 95% CI, 1.3%-3.4%). The incidence of missed ACS was 0.9% (7/791, 95% CI, 0.4%-1.8%). The incidence of missed ACS was not different between those who received nerve block 0.7% (4/610, 95% CI, 0.2%-1.7%), and those who did not (1.7% (3/176, 95% CI, 0.4%-4.8%) P = .19). Within patients receiving a nerve block, all patients with missed ACS (n = 4) received a perineural catheter. Similar missed ACS rates were observed between tibial shaft and plateau fractures. Patients receiving a nerve block had lower MME compared to those who did not receive a nerve block (P < .001). Conclusions: The results do not provide evidence that perioperative regional anesthesia increases the incidence of missed ACS in patients with operative tibial shaft or plateau injuries. but does decrease postoperative opioid requirements.
