Abstract:Locally advanced head and neck squamous cell carcinoma requires a multidisciplinary approach, including surgery followed by radiotherapy, with or without chemotherapy, or definitive concurrent chemoradiotherapy. The application of induction chemotherapy remains controversial. Cetuximab has achieved satisfactory results in the treatment of head and neck squamous cell carcinoma. Immunocheckpoint inhibitors against the receptor programmed death-1 (PD-1) and programmed death ligand-1 (PD-L1) have demonstrated clinical efficacy in head and neck squamous carcinoma. However, only a small subset of patients can benefit from anti-PD-1 and anti-PD-L1 monotherapy. To improve the effectiveness of immunotherapy, combined of immunotherapy and other treatments has become an alternative strategy. Abstract:Esophageal carcinoma is one of the most common malignant tumors, especially in China which is the high incidence area. As a result of mild symptoms of early-stage esophageal cancer, the majority of patients cannot be diagnosed until they develop to advanced cancer, and the treatment outcome of surgery or chemoradiotherapy is still unsatisfactory at present. The guidelines of esophageal cancer issued by National Comprehensive Cancer Network （NCCN） are regarded as important reference tools by clinical oncologists, and provide uniform criteria for the diagnosis and treatment of esophageal carcinoma. However, the guidelines are not always suitable for Chinese patients because the data come from European and American population which have significant ethnical difference from Chinese. We retrospectively analyzed the changes of treatment strategy of esophageal cancer in NCCN guidelines and the advance of treatment for esophageal carcinoma in China, aiming to provide our oncologists with new research ideas. We also hope to set up clinical cancer cooperation organizations, and release our own cancer guidelines to serve Chinese patients and oncologists.

Abstract:Head and neck cancer (HNC) is a disease of the upper aerodigestive tract and is one of the most frequently diagnosed cancers worldwide. A high rate of cancers involving the head and neck are reported across the Asian region, with notable variations between countries. Disease prognosis is largely dependent on tumor stage and site. Patients with early stage disease have a 60-95% chance of cure with local therapy. Early diagnosis and appropriate treatment are important to increase the likelihood of cure and survival. However, the majority of patients present with locally advanced disease and require multimodality treatment. This necessitates, a multidisciplinary approach which is essential to make appropriate treatment decisions, particularly with regards to tolerability, costs, available infrastructure and quality of life issues. Unfortunately, majority of the studies that dictate current practice have been developed in the west where diseases biology, patient population and available infrastructure are very different from those in the Asian continent. With this in mind an expert panel of Head and Neck Oncologists was convened in May 2012 to review the National Comprehensive Cancer Network (NCCN) and the European Society for Medical Oncology (ESMO) clinical practice guidelines and develop practical recommendations on the applicability of these guidelines on the management of head and neck cancer for Asian patients. The objective of this review and consensus meeting was to suggest revisions, to account for potential differences in demographics and resources, to the NCCN and ESMO guidelines, to better reflect current clinical management of head and neck cancer within the Asian region for health care providers. These recommendations, which reflect best clinical practice within Asia, are expected to benefit practitioners when making decisions regarding optimal treatment strategies for their patients. Abstract:The interaction between Cu(II) and the growth-modulating tripeptide glycyl-L-histidyl-L-lysine in the presence and absence of L-histidine was investigated by potentiometric titration and visible-absorption spectrophotometry at 25 degrees C in 0.15 M-NaCl. Analyses of the results in the pH range 3.5--10.6 indicated the presence of multiple species in solution in the binary system and extensive amounts of the ternary complexes in the ternary system. The species distribution and the stability constants, as well as the visible-absorption spectra of the species, were evaluated. The combined results were used to propose the structure of some of the complexes. The influence of the epsilon-amino group of the peptide in the enhancement of the stability constants was reflected prominently when compared with those complexes formed by either glycyl-L-histidine or glycyl-L-histidylglycine. The results obtained from the equilibrium-dialysis experiments showed that this tripeptide was able to compete with albumin for Cu(II) at pH 7.5 and 6 degrees C. At equimolar concentrations of albumin and the peptide, about 42% of the Cu(II) was bound to the peptide. At the physiologically relevant concentrations of Cu(II), albumin, L-histidine and this peptide, about 6% of the Cu(II) was associated with the low-molecular-weight components. This distribution could be due to the binary as well as the ternary complexes. The possible physiological role of these complexes in the transportation of Cu(II) from blood to tissues is discussed.

Abstract:PURPOSE:Target delineation in nasopharyngeal carcinoma (NPC) often proves challenging because of the notoriously narrow therapeutic margin. High doses are needed to achieve optimal levels of tumour control, and dosimetric inadequacy remains one of the most important independent factors affecting treatment outcome.METHOD:A review of the available literature addressing the natural behaviour of NPC and correlation between clinical and pathological aspects of the disease was conducted. Existing international guidelines as well as published protocols specified by clinical trials on contouring of clinical target volumes (CTV) were compared. This information was then summarized into a preliminary draft guideline which was then circulated to international experts in the field for exchange of opinions and subsequent voting on areas with the greatest controversies.RESULTS:Common areas of uncertainty and variation in practices among experts experienced in radiation therapy for NPC were elucidated. Iterative revisions were made based on extensive discussion and final voting on controversial areas by the expert panel, to formulate the recommendations on contouring of CTV based on optimal geometric expansion and anatomical editing for those structures with substantial risk of microscopic infiltration.CONCLUSION:Through this comprehensive review of available evidence and best practices at major institutions, as well as interactive exchange of vast experience by international experts, this set of consensus guidelines has been developed to provide a practical reference for appropriate contouring to ensure optimal target coverage. However, the final decision on the treatment volumes should be based on full consideration of individual patients' factors and facilities of an individual centre (including the quality of imaging methods and the precision of treatment delivery).

Abstract:PURPOSE The aim of this joint guideline is to provide evidence-based recommendations to practicing physicians and other healthcare providers on definitive-intent chemoradiotherapy for patients with stage II-IVA nasopharyngeal carcinoma (NPC). METHODS The Chinese Society of Clinical Oncology (CSCO) and ASCO convened an expert panel of radiation oncology, medical oncology, surgery, and advocacy representatives. The literature search included systematic reviews, meta-analyses, and randomized controlled trials published from 1990 through 2020. Outcomes of interest included survival, distant and locoregional disease control, and quality of life. Expert panel members used this evidence and informal consensus to develop evidence-based guideline recommendations. RESULTS The literature search identified 108 relevant studies to inform the evidence base for this guideline. Five overarching clinical questions were addressed, which included subquestions on radiotherapy (RT), chemotherapy sequence, and concurrent, induction, and adjuvant chemotherapy options. RECOMMENDATIONS Evidence-based recommendations were developed to address aspects of care related to chemotherapy in combination with RT for the definitive-intent treatment of stage II to IVA NPC. Interpretation of NCCN clinical practice guidelines for primary central nervous system lymphoma

Abstract:Objective To provide references of the standard treatment of primary central nervous system lymphoma (PCNSL) for neurosurgeons in China. Methods The thorough interpretation was performed on the clinical practice guidelines (version 1. 2012) for PCNSL which was made by National Comprehensive Cancer Network (NCCN). Results According to NCCN guidelines, four steps that were initial evaluation, staging workup, initial treatment and treatment at the progressive stage were recommended for treatment of PCNSL. The initial evaluation was to make a definite diagnosis and the neuroimaging performed under conditions without use of steroids was very important to the definite diagnosis. The staging workup was to evaluate the condition of the whole body. According to the guidelines, high-dose methotrexate was recommended for the initial treatment of PCNSL with KPS score ≥ 40 or creatinine clearance rate ≥ 50mL/min and the steroids or whole brain radiotherapy was recommended for the initial treatment of PCNSL with KPS score <40 or creatinine clearance rate <50mL/min. For treatment at the progressive stage of PCNSL, the high-dose methotrexate or whole brain radiotherapy should be chosen according to the specific conditions of the patients. The median survival time ranged from 33 to 60 months in patients with PCNSL treated according to the NCCN guideline. Conclusions The high-dose methotrexate is the first-line choice for treatment of PCNSL and whole brain radiotherapy is the adjuvant treatment according to the NCCN guidelines. It is very important to treat PCNSL according to the NCCN guidelines in order to increase the survival time of patients with PCNSL.

Abstract:Oral cancer is designated as carcinomas of the oral mucosa and sometimes as carcinomas of the minor salivary glands in the oral cavity. Carcinoma of the lip should not be included. Oral cancer is the most common cancer among oral and maxillofacial malignancies. The anatomical structures of the oral and maxillofacial regions are very complicated and closely related not only to facial contour and esthetics, but also to the functions of speech, mastication, swallowing, respiration, facial expression, etc. Oral cancer is a life threatening disease which can destroy facial contour and functions of the oral and maxillofacial organs, seriously influencing the quality of life of patients. The ultimate goals of oral cancer therapy are to cure the cancer, preserve or restore facial contour and function, minimize the sequence of treatment, and to prevent multiple primary cancers.1 In the past 30 years, an important breakthrough and achievement in oral cancer management has been the development of functional surgery, leading to a greatly improved quality of life for patients. Included in the functional surgeries are conservative functional surgery, which deals with the preservation of the original organs and their function, and reconstructive functional surgery, which relates to repairing the functions of organs after resection of the cancer. The typical procedure of the former is functional neck dissection, which can treat the disease, preserve the functions of the related structures, reduce facial edema and cervical deformities, and avoid the occurrence of shoulder syndrome, all leading to a better quality of life.2 This procedure is applied in elective neck dissection of NO cases. A representative technique of the latter is functional reconstruction of maxillary and mandibular defects by use of a vascularised free fibular flap or combined free fibular flap for repair of long distance mandibular or maxillary defects. It may also restore the integrity of the dentition by implant techniques upon the basis of the bone graft take. The facial contour and masticatory function are thus repaired.3,4 It is regrettable that the survival rate of oral cancer has not been raised significantly in the last 30 years - according to the USA National Cancer Database.5 The overall 5-year survival is around 64% for head and neck cancer. Therefore, there still a lot needs to be done to improve the survival rate. EARLY DIAGNOSIS AND EARLY TREATMENT The “Two Earlies” are still the key to improve the survival rate of patients. It is convenient to apply the “Two Earlies” because the oral structures are superficial and can readily be approached. However, most patients are in an advanced stage of disease (stages III and IV) at their first visit. So, for those diseases of the oral mucosa with malignant transformation tendency, such as oral leukoplakia, oral lichen planus, it is necessary to strengthen surveillance and to remove the lesion before it becomes invasive. Modern molecular biology techniques have provided effective measures for diagnosis of early cancer. One report shows that early cancer may be detected from saliva samples.6 This may be a new measure for early diagnosis though the value thereof needs further investigation. APPLICATION OF MULTIDICIPLINARY COMBINED AND SEQUENTIAL THERAPY For early oral cancer (stages I and II), surgery alone and/or radiation therapy, can treat the disease. For advanced cases, however, the results of single therapy alone are not usually effective and combined treatment modalities should be applied. The problem of how to combine different methods and what sequence should be adopted remain to be solved. Surgery, radiation oncology, medical oncology, rehabilitation, etc., all may be part of the therapy. There needs to be further investigated as to how the various disciplines can be best applied in the formation of a multidiciplinary combined modality treatment protocol. Dr. Zheng et al's1 review in this issue has provided a good answer. TO DEVELOP NEW-THERAPEUTIC MODALITY The traditional modalities in cancer therapy - surgery, radiation and chemotherapy - are still the main modalities for oral cancer treatment. Use of traditional Chinese medicine in cancer therapy is limited; however, it can improve the general status and reinforce the immunity of the patients, and may be used clinically.7 Basic research on oral cancer must be reinforced. Modern molecular biology techniques have expanded greatly in the field of diagnosis and treatment of oral cancer. Some researches8,9 have demonstrated the role of genes in the origin and development of oral cancers. There are also many applied basic studies, such as immunotherapy, cytokine therapy, gene therapy. As regards new methods and techniques, they should be faced positively and carefully. The use of laboratory findings in the clinic should be fast and examined during their application, but controlled by ethics and regulations.10 TO DEVELOP MULTICENTRIC, PROSPECTIVE, LARGE SAMPLE SIZE CLINICAL STUDY Owing to significant individual variations and variation in biological behavior inherent in the location of oral cancers and small sample size, it is often difficult to achieve a precise evaluation. This is particularly true with chemotherapy treatment. So, multicentric, prospective, large sample size studies are necessary to obtain objective conclusions for clinical guidance. TO STRENGTHEN STUDIES ON REHABILITATION THERAPY After surgery and radiation therapy of oral cancer, patients present with various types of damage to oral structures and functions, such as limitations in opening the mouth and speech and swallowing dysfunctions; rehabilitation therapy can improve these functions. At present, a set of systematic therapeutic techniques and measures are still lacking. Established measures for treating different organic and functional impairments, with the goal of improving the quality of life of the patient, are urgently required. Abstract:Purpose Nasopharyngeal carcinoma (NPC) is a malignant epithelial tumor originating in the nasopharynx and is particularly prevalent in southern China. Unfortunately, international guidelines, such as NCCN or ESMO, fail to adequately coincide with clinical practice in China, making it difficult to achieve precision personalized therapy in China. The aim of this guideline is to better promote a “Multidisciplinary Team to Holistic Integrative Medicine" (MDT to HIM) system for the prevention, screening, diagnosis, treatment, and rehabilitation of NPC. Methods The China Anti-Cancer Association (CACA) invited domestic multi-disciplinary experts, involving radiologists, oncologists, surgeons, pathologists, herbalists, physiatrists, and psychologists, to write, discuss, and revise the guidelines. Based on the integration of research evidence, clinical experience, and patient needs, the domestic experts have iteratively developed these guidelines to provide proper and feasible management of NPC. Results and conclusion The CACA Guidelines for Holistic Integrative Management of Nasopharyngeal Carcinoma are more suitable for China's clinical practice, highlight Chinese characteristics, and have important clinical significance.
Variations of Clinical Target Volume Delineation for Primary Site of Nasopharyngeal Cancer Among Five Centers in China

Abstract:Purpose: The purpose of this study is to investigate the current status of clinical target volume (CTV) delineation for primary site of nasopharyngeal cancer (NPC) among five large tertiary cancer centers in China.Materials and Methods: The simulation CT and MR images of a patient with T3N2M0 NPC were sent to the centers participating. Fourteen experienced physicians contoured the targets independently, and the outlined structures were compared. The consistency and differences among these 14 CTVs are discussed.Results: Two different CTV designs were used in the centers. "One-CTV" design defines one CTV with a dose of 60 Gy, whereas "two-CTV" design has a high-risk CTV with dose of 60 Gy and a medium risk CTV with dose of 54 Gy. We found that the coverage of prophylactic area is very consistent between these two designs. The variances on the coverage of some sites were also significant among physicians, including covering cavernous sinus at un-involved side, posterior space of styloid process, and caudal border on posterior pharyngeal wall.Conclusions: Standardization is the main requirement for personalization of care; our study shows that among the 14 physicians in the five centers the coverage of prophylactic areas is in excellent agreement. Two distinct strategies on CTV design are currently being used, and multiple controversies were found, suggesting further optimization of CTV for primary site of NPC is needed.

Abstract:Nasopharyngeal carcinoma (NPC) is a malignant epithelial tumor originating in the nasopharynx and has a high incidence in Southeast Asia and North Africa. To develop these comprehensive guidelines for the diagnosis and management of NPC, the Chinese Society of Clinical Oncology (CSCO) arranged a multi-disciplinary team comprising of experts from all sub-specialties of NPC to write, discuss, and revise the guidelines. Based on the findings of evidence-based medicine in China and abroad, domestic experts have iteratively developed these guidelines to provide proper management of NPC. Overall, the guidelines describe the screening, clinical and pathological diagnosis, staging and risk assessment, therapies, and follow-up of NPC, which aim to improve the management of NPC.

Abstract:Objective To explore the role and significance of the standardized treatment program in the clinical treatment of gastric cancer.Methods Collected from Feb.2010 to Mar.2011,52 cases of gastric cancer patients with clinical standardization of treatment,and summarized the clinical,pathological data and the treatment process.Results 3 cases were confirmed transfer,palliative treatment was used.1 cases of preoperative staging T1 directly treated by surgery.Pre-laparoscopic exploration in 48 cases before treatment,15 cases of exploratory peritoneal metastasis,confirmed that the M1 line and perfusion chemotherapy was applied,and palliative chemotherapy,post-tolerated palliative surgery in 10 cases.The remaining 33 patients according to preoperative procedure for drawing up of direct surgery in 15 cases,the first new adjuvant chemotherapy,efficiency ratings 18 cases after surgery,all surgery reached the D2 standards.Length of stay was 7~70 days,average 11 days.Operative time was 120~190 min,and average of 145 min.Blood loss of about 50~200 mL,125 mL average.Conclusion Emphasis on the comprehensive treatment of gastric cancer mode,reasonable and effective treatment for patients have important clinical implications for standardizing treatment of gastric cancer.

Abstract:Oral cancer accounts for around 30 percent of all cancers in India. These cancers are usually managed either by surgery, radiotherapy, chemotherapy or a combination of these modalities. Dental oncologists play an integral part in the multidisciplinary team, including surgeons, radiation and medical oncologists, nurses, physician assistants, nutritionists, psychologists and social workers to efficiently manage cancer patients. Oral complications associated with cancer therapy can range from minor mucositis or infections to severely debilitating conditions such as osteonecrosis of jaws, which can disrupt the ongoing cancer therapy and jeopardize the overall quality of life of the patient. This highlights the primary role of a dentist in the efficient identification and management of potential oral foci of infection and subsequent prevention of the onset of these complications. This necessitates accurate, evidence-based knowledge and extensive training of dental oncologists to provide state-of-the-art management strategies. This article aims to review oral management before, during and after therapy in patients undergoing treatment for cancers affecting the head and neck region.

Abstract:Background: Developing guidelines for the diagnosis and treatment of common cancers in China based on the evidence-based practice, the availability of diagnosis and treatment products, and the up-to-date advances in precision medicine is one of the basic tasks of the Chinese Society of Clinical Oncology Breast Cancer (CSCO BC) Committee. Methods: Protocols with high evidence level and good availability are used as the Level I recommendations; protocols with relatively high evidence level but slightly lower expert consensus or with poor availability are used as the Level II recommendations; and protocols that are clinically applicable but with low evidence level are regarded as the Level III recommendations. Based on the findings of clinical research at home and abroad and the opinions of CSCO BC experts, the CSCO BC guidelines determine the levels of recommendations for clinical application. Results: For human epidermal growth factor receptor 2 (HER2)-positive breast cancer, a combination of trastuzumab and pertuzumab regimen were recommended as Level I recommendation for neoadjuvant and first line metastatic breast cancer. Pyrotinib is also recommended as Level I recommendation in first line and second line therapy according to the latest studies conducted in China. Antibody drug conjugates was also recommended for patients with trastuzumab progression. For triple negative breast cancer, immunotherapy in early and metastatic breast cancer was highlighted and listed as new chapters in this version of guideline. For hormone receptor (HR)-positive breast cancer, cyclin dependent kinase 4/6 (CDK4/6) was recommended in different stages, especially in adjuvant therapy. There was also a new chapter for HER2-low breast cancer stratified by HR status. Conclusions: We firmly believe that evidence-based, availability-concerned, and consensus-based guidelines will be more feasible for clinical practice in China and in other countries with similar situations.

Abstract:Head and neck cancer (HNC) is malignant, genetically complex and difficult to treat and is the sixth most frequent cancer, with tobacco, alcohol and human papillomavirus being major risk factors. Based on epigenetic data, HNC is remarkably heterogeneous, and treatment remains challenging. There is a lack of significant improvement in survival and quality of life in patients with HNC. Over half of HNC patients experience locoregional recurrence or distal metastasis despite the current multiple traditional therapeutic strategies and immunotherapy. In addition, resistance to chemotherapy, radiotherapy and some targeted therapies is common. Therefore, it is urgent to explore more effective and tolerable targeted therapies to improve the clinical outcomes of HNC patients. Recent targeted therapy studies have focused on identifying promising biomarkers and developing more effective targeted therapies. A well understanding of the pathogenesis of HNC contributes to learning more about its inner association, which provides novel insight into the development of small molecule inhibitors. In this review, we summarized the vital signaling pathways and discussed the current potential therapeutic targets against critical molecules in HNC, as well as presenting preclinical animal models and ongoing or completed clinical studies about targeted therapy, which may contribute to a more favorable prognosis of HNC. Targeted therapy in combination with other therapies and its limitations were also discussed.

Abstract:Most of the patients with oral and maxillofacial malignancy are in the middle and advanced stages at diagnosis and the incidence rate is increasing in recent years. Chemotherapy alone is difficult to benefit the survival of patients with advanced oral and maxillofacial malignancy. Ultrasound hyperthermia is a new and effective treatment for malignant tumor, which is developing rapidly in addition to conventional treatment. However, at present, ultrasound hyperthermia has not been widely used in the treatment of oral and maxillofacial malignancy. Therefore, formation of a guideline on ultrasound hyperthermia for oral and maxillofacial malignancy is mandatory, in order to promote and standardize the clinical practice of ultrasound hyperthermia in this field, and improve the long-term survival rate and quality of life of patients.

Abstract:The treatment of oral cavity cancer primarily depends on the location, tumor size, and feasibility of organ preservation in patients with negative margins. A liquid biopsy is a minimally invasive approach for detecting blood-based signatures such as circulating tumor cells (CTCs) and circulatory tumor DNA (ctDNA). To assess the presence of occult metastatic disease, sentinel lymph node biopsy (SLNB) is recommended in early-stage oral cavity cancer (OCC). In India, surgery is the preferred choice of treatment for oral cancers. Selected patients, who are medically inoperable or refuse surgery, should be given definitive radiotherapy (RT) as an alternative to surgery. Concurrent chemoradiation (CCRT) may be considered an alternative option to definitive RT in patients with unresectable oral cancer.

Abstract:Oral cancer is a significant global health challenge, with conventional treatments often resulting in substantial side effects and limited effectiveness. Phototherapy, encompassing photodynamic and photothermal therapy, presents a promising alternative by selectively targeting and destroying cancer cells with minimal systemic toxicity. However, issues such as insufficient light penetration and limited tumor specificity have restricted their clinical use. Recent advancements in nanosystems have addressed these challenges by enhancing the solubility, stability, and tumor-targeting capabilities of phototherapy agents. This review explores the latest progress in phototherapeutic nanosystems for oral cancer, highlighting innovative nanoformulations, targeted delivery strategies, and phototherapy-based combination therapies. These developments have the potential to significantly improve treatment outcomes and reduce side effects in oral cancer therapy.

Abstract:Radiotherapy is the mainstay treatment modality for nasopharyngeal carcinoma (NPC). Intensity-modulated radiation therapy (IMRT), as the standard technique, achieves the purpose of improving target coverage and better sparing of normal tissue. Increased attention has been given to explore various strategies for deescalating treatment intensity. The optimization of clinical target volume (CTV) is one of the most active research areas being widely discussed. Although the International Guidelines for the delineating of CTV in NPC had provided important references for clinicians, there are marked variations in practice among different institutions. This article reviews the development of CTV delineation in non-metastatic NPC patients among centers, and compares the similarities and differences in CTV delineation of various current guidelines in the hope of providing insights for future investigation. This review aims to provide a comprehensive summary of the development and evolution of CTV delineation on primary tumor and lymph nodes for definitive radiotherapy in non-metastatic NPC through historical lens. We also compare the differences of CTV delineation ways. In addition, we look into the clinical and practical challenges of CTV delineation, hoping to provide direction for future research.
