Abstract:Cities are increasingly adopting CeaseFire, an evidence-based public health program that uses specialized outreach workers, called violence interrupters (VIs), to mediate potentially violent conflicts before they lead to a shooting. Prior research has linked conflict mediation with program-related reductions in homicides, but the specific conflict mediation practices used by effective programs to prevent imminent gun violence have not been identified. We conducted case studies of CeaseFire programs in two inner cities using qualitative data from focus groups with 24 VIs and interviews with eight program managers. Study sites were purposively sampled to represent programs with more than 1 year of implementation and evidence of program effectiveness. Staff with more than 6 months of job experience were recruited for participation. Successful mediation efforts were built on trust and respect between VIs and the community, especially high-risk individuals. In conflict mediation, immediate priorities included separating the potential shooter from the intended victim and from peers who may encourage violence, followed by persuading the parties to resolve the conflict peacefully. Tactics for brokering peace included arranging the return of stolen property and emphasizing negative consequences of violence such as jail, death, or increased police attention. Utilizing these approaches, VIs are capable of preventing gun violence and interrupting cycles of retaliation.

Preventing Urban Firearm Homicides during COVID-19: Preliminary Results from Three Cities with the Advance Peace Program

Abstract:The years 2020-2021 during the COVID-19 pandemic witnessed increases in firearm violence in many cities across the USA. We present data from Sacramento, Stockton, and Richmond, California that suggests firearm homicides during the pandemic did not increase in all communities or disproportionately burden the African American community. More specifically, we found that in these cities, there was a 5-52% decrease in gun homicides during the 2020/2021 period compared to the 2018/2019 period for neighborhoods with a gun violence prevention program operating there. We also found a 24-83% reduction in gun homicides in census tracts with > 20% Black populations in Sacramento and Stockton during the 2020/2021 period compared to the 2018/2019 period. In two cities, there was a 15-42% decrease in the number of African American men under 35 years old that were victims of a gun homicide in 2021 compared to 2018. We also found that the gun violence program operating in these cities called Advance Peace interrupted 202 street-level conflicts where guns were present across the three cities in 2020/2021 compared to 178 of the same conflicts in 2018/2019. These interruptions likely saved hundreds of lives and we estimate contributed to between US $65 and $494 million in savings. Advance Peace is a program that engages those at the center of gun violence, frequently young, Black men under 35 years old, and offers them the Peacemaker Fellowship, an intensive, 18-month program of 24/7 mentorship, social services, and life opportunities. The program is delivered by community resident "credible messengers," who conduct the mentorship and interrupt conflicts in the streets. While these findings are descriptive and preliminary, we know of no other program that was in operation before and during the pandemic in each of these cities that engaged the hard-to-reach but highly influential population at the center of gun violence.

Abstract:Abstract Urban gun violence is the result of and contributes to trauma for both individuals and communities. In the US, African American males between 15 and 34 years old bear the greatest mortality burden from gun violence. Community-based approaches that use credible, street-level outreach workers to interrupt conflicts, mentor the small number of offenders in each community, and offer them alternatives to violent conflict resolution, have demonstrated success in reducing firearm homicides. Yet, few of these approaches explicitly aim to also address the traumas of structural violence that contribute to gun crime, including dehumanizing policing, extreme poverty, and institutional racism. This commentary describes a program called Advance Peace that aims to explicitly use a healing-centered approach to address the traumas associated with violence as a means to reduce gun crime in urban communities. We describe the trauma-informed, healing-centered approach used by Advance Peace, the components of its intensive outreach strategy called the Peacemaker Fellowship, and some impacts the program is having on trauma and healing. The evidence comes from observations, interviews, and the voices of Advance Peace participants and staff. We suggest that exploring the inner workings of the Advance Peace model is critical for identifying ways to support trauma-informed healing-centered approaches in Black and brown communities that have been ravaged by racism, incarceration, and heavy-handed state violence.
The Art and Science of Urban Gun Violence Reduction: Evidence from the Advance Peace Program in Sacramento, California

Abstract:Urban gun violence is a critical human health and social justice issue. Strategies to reduce urban gun violence are increasingly being taken out of the domain of police and into community-based programs. One such community-driven gun violence reduction program analyzed here is called Advance Peace. Advance Peace (AP) uses street outreach workers as violence interrupters and adult mentors to support the decision making and life chances of those at the center of urban gun violence. We reported on the impact Advance Peace had on gun violence and program participants in the City of Sacramento, California, from 2018–2019. Using an interrupted time series model, we attributed a gun violence reduction of 18% city wide and up to 29% in one of the AP target neighborhoods from the intervention. We also found that of the 50 participants in the Advance Peace Sacramento program 98% were alive, 90% did not have a new gun charge or arrest, 84% reported an improved outlook on life, all received cognitive behavioral therapy, and 98% reported that their AP outreach worker was one of the most important adults in their life. Advance Peace is a viable community-driven, urban gun violence, and healing-focused program.

Abstract:Scholars and practitioners alike in recent years have suggested that real and lasting progress in the fight against gun violence requires changing the social norms and attitudes that perpetuate violence and the use of guns. The Cure Violence model is a public health approach to gun violence reduction that seeks to change individual and community attitudes and norms about gun violence. It considers gun violence to be analogous to a communicable disease that passes from person to person when left untreated. Cure Violence operates independently of, while hopefully not undermining, law enforcement. In this article, we describe the theoretical basis for the program, review existing program evaluations, identify several challenges facing evaluators, and offer directions for future research.

Abstract:Abstract Cities around the world continue to grapple with safety, security and the role for law enforcement in reducing gun crime. Recent calls for alternatives to militarized policing in cities and addressing racism in urban crime policies and practices gives new urgency to explore community-led strategies. Advance Peace is a program that aims to reduce urban gun violence using formerly incarcerated community members as street outreach mentors and violence interrupters. Yet, few urban policy makers know of Advance Peace and how it is distinct from other community-based urban gun violence interruption programs, often called focused deterrence. In this paper, we describe the innovative approach used by Advance Peace, what distinguishes it from other municipal gun violence reduction strategies, and examine the elements of its unique, public health informed program called the Peacemaker Fellowship®. The Peacemaker Fellowship enrolls the small number of the most violent and hard to reach members of a community at the center of gun violence in an intensive 18-month program of trauma-informed, healing-centered, anti-racist mentorship, education, social services, and life opportunities. We suggest that cities around the world seeking transformations in their approach to public safety, including addressing structural racism and centering community expertise, explore the unique features of the Advance Peace approach.
Conflict Resolution Interventions and Tertiary Violence Prevention Among Urban Nonintimate Adults: A Review of the Literature

Abstract:Several hundred U.S. conflict resolution and restorative justice programs are addressing community violence using neutral facilitation to help people in conflict, or those who have experienced a crime, to talk things out face-to-face and come up with self-determined solutions. Very little quantitative intervention research has been conducted on the capacity of these programs to reduce violence, violent crime, and criminal recidivism. The scientific literature pertaining to the association of conflict resolution interventions with violence prevention are identified, screened, and sorted. Study design, sampling, measurement, and analyses are assessed using objective standards. Individual criminal recidivism outcomes and neighborhood gun violence rates are charted. In the 10 included studies, disparate conflict resolution and restorative justice interventions each appears to be related to modest reductions in individual criminal recidivism for participants, when compared with standard criminal justice system treatment. Conflict interruption interventions show a decrease in neighborhood gun violence in most districts. Future studies of conflict resolution and violence should mitigate selection bias, control for possibly confounding factors, operationalize all intervention components, select the correct units of analyses, and link "what works" outcome data to "how it works" intervention data. Four key gaps include measuring self-reported violence, including victimization, studying adults, and examining "upstream" interventions.
Evaluating the impact of a street outreach intervention on participant involvement in gun violence

Abstract:The past several years have witnessed increased calls for community violence interventions (CVIs) that address firearm violence while centering local expertise and avoiding the criminal legal system. Currently, little evidence exists on CVI effectiveness at the individual level. This study presents an evaluation of the impact of a street outreach-based CVI [Chicago CRED (Create Real Economic Destiny)] on participant involvement in violence. We used a quasiexperimental design with a treatment sample of 324 men recruited by outreach staff from 2016 to 2021 and a balanced comparison sample of 2,500 men from a network of individuals arrested in CRED's service areas. We conducted a Bayesian survival analysis to evaluate CRED's effect on individual violence-related outcomes on three levels of treatment: All enrolled participants, a subsample that made it through the initial phase, and those who completed programming. The intervention had a strong favorable effect on the probability of arrest for a violent crime for those completing the program: After 24 mo, CRED alumni experienced an 11.3 percentage point increase in survival rates of arrest for a violent crime relative to their comparisons (or, stated differently, a 73.4% reduction in violent crime arrests). The other two treatment levels experienced nontrivial declines in arrests but did not reach statistical significance. No statistically significant reduction in victimization risk was detected for any of the treatment levels. Results demonstrate that completion of violence intervention programming reduces the likelihood of criminal legal involvement for participants, despite the numerous systemic and environmental factors that impede personal success.
Do Cure Violence Programs Reduce Gun Violence? Evidence from New York City

Abstract:Cure Violence is a community violence intervention program that aims to reduce gun violence by mediating conflicts, "treating" high-risk individuals, and changing community norms. Using NYC shootings data from 2006-2023, we assess the efficacy of Cure Violence using both difference-in-differences and event study models. We find that, on average, Cure Violence is associated with a 14% reduction in shootings relative to the counterfactual. This association persists in the years after treatment, neither increasing nor decreasing much over time. We exploit variation in the geography and timing of Cure implementation to argue against alternative explanations. Additionally, we find suggestive evidence of spillover effects into nearby precincts, as well as increasing returns to opening new Cure programs. Interpreted causally, our results imply that around 1,300 shootings were avoided between 2012-2023 due to Cure -- generating a net social surplus of $2.45 billion and achieving a benefit-cost ratio of 6.5:1.

Abstract:Background: Cure Violence interruption programs are evidence-based interventions aimed at reducing the transmission of gun violence and its related injuries. Assessing the implementation of these programs can include the metric of "reach." This study evaluated one such program - 414LIFE - in Milwaukee, Wisconsin. The evaluation reconceptualized "reach" as a metric for reaching the individuals and neighborhoods at greatest risk for gun violence. Methods: 414LIFE's reach was analyzed descriptively and geospatially through its program evaluation dataset from May 2019 through September 2020 using a cross-sectional design. Program referral criteria includes patients who sustained a gunshot wound, are less than 36 years old, and a resident of, or injured in, the city of Milwaukee. A choropleth map visualized location of participants' residence, which justified a global Moran's I, and then a local Moran's I calculation to identify statistically significant clustering of referrals. Results: In the first 1.5 years of the program's partnership with the local level I trauma center and affiliated academic medical institution, 398 patients were referred. Three hundred referrals (75.4%) met program criteria; 53.8% were Black men. Statistically significant clusters were identified and mapped. Half of the top 10 neighborhoods with referrals were the city's identified priority neighborhoods. Conclusions: 414LIFE successfully reaches its intended population and geographic locations. Geospatial reach should be considered routinely in program evaluations of Cure Violence programs to track growth and reach over time.

Abstract:Gun violence is a leading cause of premature death and a driver of racial disparities in life expectancy in the United States. Community-based interventions are the foremost policy strategy for reducing gun violence without exacerbating harm associated with criminal justice approaches. However, little is known about the interventionist workforce. In 2021, we used a researcher-guided survey to obtain a near-census of Chicago violence interventionists (n = 181, 93% response rate). Workers were mostly male (84%) and Black (80.9%), with a mean age of 43.6 years. Interventionists commonly experienced work-related exposure to violence and direct victimization. A total of 59.4% witnessed someone being shot at, whereas 32.4% witnessed a victim struck by gunfire. During work hours, 19.6% were shot at, while 2.2% were nonfatally shot. Single-year rates of gun violence victimization exceeded those of Chicago police. Results suggest that investment in community violence intervention should prioritize improving worker safety and reducing violence exposure while developing support for vulnerable frontline practitioners.

Gun violence survivors especially face unique and long-term physical, mental, economic, and safety-related challenges. Hospital-based violence intervention programs (HVIPs) connect survivors with violence prevention professionals at their hospital bedsides, then provide them with wraparound services in the months to come. Promising research shows that HVIPs reduce risks of reinjury, retaliation, and recidivism.

Abstract:Kansas City is a microcosm for USA. Although Kansas City shows a relatively diverse population, it is one that is segregated along the lines of race and income. This is an inequity that is common to all cities across the country. With this inequity comes unequal opportunity to survive and to thrive. Firearm violence is a core component of this societal inequity. In this article, we present the proceedings of the 2019 Kansas City Firearm Violence Symposium, where distinguished experts in trauma convened to share their experience, evidence and voices of gun violence—directly and indirectly. There were discussions on topics such as the human toll of gun violence, the role of structural violence in its perpetuation, the intersectional nature of race with both violence and medical care, and guidance on measures that could be taken to advocate for the reduction and elimination of gun violence. This was a symposium that started a country-wide conversation between academia, healthcare, survivors and the community on the most pressing public health crisis facing USA today.
The Data Dilemma:: How Delaware is Responding to the CDC's Recommendations on Gun Violence

Abstract:In 2015, in response to a rising problem of gun violence in the City of Wilmington, Delaware, and due to the urging of local public officials, the Centers for Disease Control and Prevention (CDC) conducted a groundbreaking study around the public health crisis and issued three recommendations on opportunities for prevention. The ideal solutions centered on the creation of a predictive analytic tool that would help social service providers determine who is most likely, based on a set of weighted risk factors, to engage in gun violence. As various entities started to lay the foundation for implementing the CDC's recommendations, they faced several hurdles directly related to this new technological solution. After careful consideration and thorough vetting, which was also recommended by the CDC, led by Governor Carney's Family Services Cabinet Council (FSCC), Delaware concluded two things: a tool of this nature presents ethical issues, and there are evidence-based strategies to identify those engaging or likely to engage in gun violence; and notwithstanding the ethical concerns surrounding the tool, Delaware lacked the technology infrastructure and staffing to develop such a tool. Ultimately, good collaboration (facilitated by Social Contract, a local consulting firm) through the FSCC fostered an alternate path forward in keeping with the spirit of the CDC's recommendations; while the CDC's recommendations were not precisely enacted, their contribution has led to investments and capacity building in Delaware to support individuals and families most proximal to the problem. Ultimately, convening stakeholders to fully examine an issue and ideate solutions with the most potential for impact resulted in two meaningful outcomes: (1) an innovative approach to ultimately reduce gun violence in the City of Wilmington through widespread collaboration of state services, developing meaningful relationships with those directly engaged in gun violence; and (2) the creation of a statewide data-sharing system that will help improve service delivery and outcomes for Delawareans in need.
Using EMS data to explore community-level factors associated with firearm violence in North Carolina

Abstract:Background: Firearm violence is a significant public health issue. However, it is unclear if there is an association between the Social Vulnerability Index (SVI) and the intent of both fatal and nonfatal firearm injuries, and if these associations are modified by community race and ethnic composition. This study examines the association between community-level social vulnerability and firearm injury incidence in North Carolina (NC) using 2021-2022 emergency medical services (EMS) data. Additionally, it investigates how these associations vary by the intent of injury (assault, self-inflicted, and unintentional), and whether they are modified by community racial/ethnic composition. Methods: This cross-sectional study utilized NC EMS data, capturing firearm incidents from January 1, 2021, to December 31, 2022. The SVI from the Centers for Disease Control and Prevention (CDC) was used to assess community-level vulnerability. The SVI's racial/ethnic minority status component was removed for stratification analysis. Firearm injury rates were calculated per 100,000 population, and negative binomial regression models were used to estimate Incidence Rate Ratios (IRRs) for different SVI levels and intents of firearm injuries. Results: During the study period, we identified 7,250 EMS encounters at non-healthcare locations related to firearm injuries, encompassing 2,648 NC census tracts. Assault was the leading cause of firearm injuries (n = 3,799), followed by self-inflicted (n = 1,498), and unintentional injuries (n = 722). High-SVI communities had significantly higher rates of firearm injuries compared to low-SVI communities, particularly for assault-related injuries. When the minority status component was excluded from SVI, racial/ethnic minority status emerged as a significant modifier, with higher rates of firearm injuries being observed in communities with larger racial/ethnic minority populations. Conclusion: Community-level social vulnerability is significantly associated with firearm injury incidence, with the effect being more pronounced in racial/ethnic minority communities. These findings underscore the need for targeted public health interventions that address underlying social determinants of health (e.g., access to education) to reduce firearm violence. Future research should further explore the intersection of social vulnerability and racial/ethnic composition to develop effective prevention strategies.
Using synthetic control methodology to estimate effects of a Cure Violence intervention in Baltimore, Maryland

Abstract:Objective: To estimate the long-term impact of Safe Streets Baltimore, which is based on the Cure Violence outreach and violence interruption model, on firearm violence. Methods: We used synthetic control methods to estimate programme effects on homicides and incidents of non-fatal penetrating firearm injury (non-fatal shootings) in neighbourhoods that had Safe Streets' sites and model-generated counterfactuals. Synthetic control analyses were conducted for each firearm violence outcome in each of the seven areas where Safe Streets was implemented. The study also investigated variation in programme impact over time by generating effect estimates of varying durations for the longest-running programme sites. Results: Synthetic control models reduced prediction error relative to regression analyses. Estimates of Safe Streets' effects on firearm violence varied across intervention sites: some positive, some negative and no effect. Beneficial programme effects on firearm violence reported in prior research were found to have attenuated over time. Conclusions: For highly targeted interventions, synthetic control methods may provide more valid estimates of programme impact than panel regression with data from all city neighbourhoods. This research offers new understanding about the effectiveness of the Cure Violence intervention over extended periods of time in seven neighbourhoods. Combined with existing Cure Violence evaluation literature, it also raises questions about contextual and implementation factors that might influence programme outcomes.
