Abstract:Background: Assessing the coverage by public or private resources in meeting health-related and social-related needs may be useful for service planning and guide optimization of care, important especially in view of an increase in the prevalence of multiple sclerosis (MS). Methods: An ad hoc questionnaire assessed satisfaction of health-related and social care-related needs in a cross-sectional study of 1014 people with MS identified through MS outpatient clinics and local branches and social media channels of the Italian MS Society. Results: 87.1% and 79.8% of the responders had experienced at least one health-related or social-related need, respectively. The study demonstrated significant gaps between perceived needs and service provision. Rehabilitation, residential care, and psychological support were most frequently unsatisfied health-related needs, while the more commonly unmet social-related needs were financial support, elimination of architectural barriers, workplace adaptations, and career guidance. The multivariate analysis highlighted that the satisfaction of health-related needs was primarily associated with geographic area of residence. Social-related needs correlated with both clinical and sociodemographic aspects. Conclusion: The results provide insight into the range of interventions, care, and support people with MS report to be important to them at different points in their disease trajectory. More emphasis should be put on the inequitable distribution of NHS services in different geographic areas of Italy as well as on particularly fragile subgroups of the MS population (older individuals, and those with higher levels of disability) because the care of these individuals continues to be assumed by the family.

Abstract:(1) This paper aims to propose a telematic method for assessing the state of disability by a social worker member of the Medico-Legal Commissions of INPS. (2) We have proceeded to delve into and compare the current methods with new strategies to enhance the experience linked to the assessment of the status of disability in terms of timing and accessibility, eliminating the need for a direct visit. (3) The proposed protocol can be applied in cases where patients cannot be physically moved, following a medical visit at home, and when the mere evaluation of documents is sufficient. In both cases, a remote session with the Commission is necessary to fill in the required information about the socio-environmental section. This protocol can be applied to different platforms such as SkypeTM since it is free and widely used throughout the country. (4) It should be noted that telemedicine solutions cannot completely replace face-to-face interaction; however, in some limited cases, they can optimize the process and timing, avoiding the need to move or interact only by telephone.

Abstract:International Journal of Social Psychiatry, Ahead of Print. Background:Work functioning impairment is a key diagnostic and prognostic criterion in patients with psychiatric disorders and work inclusion is a major goal of their therapeutic pathway. Since 2009, the Regional Innovative Program (PIR) TR106, promoted by ASST Fatebenefratelli-Sacco of Milan in collaboration with other Departments of Mental Health and Addictions (DSMDs) in the town of Milan (Italy), has been developing the employment inclusion of psychiatric patients.Aims:The objective of this study is to evaluate its outcomes over 8 years of observation.Method:We reported the results of a retrospective epidemiologic analysis on 2,142 interventions on 1,066 patients recruited, investigating PIR TR106 outcomes per year focusing on different subgroups. We focused on 'positive', 'negative', and 'other' outcomes.Results:We preliminary calculated job maintenance interventions (5%, 107) and excluded these interventions from the overall. We observed 29 job firing (1.4%) and 15 job resignations (0.7%) as negative results (equal to 2.2% of the total) and 388 job hiring (16.6%), 647 traineeships (31.8%), and 413 work formation (20.3%) as positive outcomes (equal to 68.75%). In other outcomes (29.1%) we found 305 dismissals from PIR TR 106 (15%) and transitory outcomes (14.1%).Job hiring increased from 8.9% in 2012 to 23.8 % in 2019 (p < .001), while the dismissals diminished from 26.7% to 13.3% (p < .001). The effectiveness of traineeships in terms of job hiring increased in the ratio of annual job hiring versus job traineeship (+48.8%). The majority of hired patients (15.1%) were affected by a psychotic disorder. A significant hiring increase was observed in patients with psychotic disorders and personality disorders (p < .005).Conclusions:PIR-TR106 represents a territorial employment inclusion program with progressively increasing effectiveness and specificity, as suggested by changes in outcomes during the 8-year observation. The adaptive capacity and sustainability of the intervention are worth further investigation.

Social inequalities in health within the City of Milan (Lombardy Region, Northern Italy): An ecological assessment

Abstract:Objectives: to document existing geographical inequalities in health in the city of Milan (Lombardy Region, Northern Italy), examining the association between area socioeconomic disadvantage and health outcomes, with the aim to suggest policy action to tackle them. Design: the analysis used an ecological framework; multiple health indicators were considered in the analysis; socioeconomic disadvantage was measured through indicators such as low education, unemployment, immigration status, and housing crowding. For each municipal statistical area, Bayesian Relative Risks of the outcomes (using the Besag-Yorkand-Mollié model) were plotted on the city map. To evaluate the association between social determinants and health outcomes, Spearman correlation coefficients were estimated. Setting and participants: residents in the City of Milan aged between 30 and 75 years who were residing in Milan as of 01.01.2019, grouped in 88 statistical areas. Main outcomes measures: all-cause mortality, type-2 diabetes mellitus, hypertension, neoplasms, respiratory diseases, metabolic syndrome, antidepressants use, polypharmacy, and multimorbidity. Results: the results consistently demonstrated a significant association between socioeconomic disadvantage and various health outcomes, with low education exhibiting the strongest correlations. Neoplasms displayed an inverse social gradient, while the relationship with antidepressant use varied. Conclusions: these findings provide valuable insights into the distribution of health inequalities in Milan and contribute to the existing literature on the social determinants of health. The study highlights the need for targeted interventions to address disparities and promote equitable health outcomes. The results can serve to inform the development of effective public health strategies and policies aimed at reducing health inequalities in the city.
Applicability of the adjusted morbidity groups algorithm for healthcare programming: results of a pilot study in Italy

Abstract:Background: Population-based Health Risk Assessment (HRA) tools are strategic for the implementation of integrated care. Various HRA algorithms have been developed in the last decades worldwide. Their full adoption being limited by technical, functional, and economical factors. This study aims to apply the Adjusted Morbidity Groups (AMG) algorithm in the context of an Italian Region, and evaluate its performance to support decision-making processes in healthcare programming. Methods: The pilot study used five Healthcare Administrative Databases (HADs) covering the period 2015-2021. An iterative semi-automated procedure was developed to extract, filter, check and merge the data. A technical manual was developed to describe the process, designed to be standardized, reproducible and transferable. AMG algorithm was applied and descriptive analysis performed. A dashboard structure was developed to exploit the results of the tool. Results: AMG produced information on the health status of Marche citizens, highlighting the presence of chronic conditions from age 45 years. Persons with high and very high level of complexity showed elevated mortality rates and an increased use of healthcare resources. A visualization dashboard was intended to provide to relevant stakeholders accessible, updated and ready-to-use aggregated information on the health status of citizens and additional insight on the use of the healthcare services and resources by specific groups of citizens. Conclusion: The flexibility of the AMG, together with its ability to support policymakers and clinical sector, could favour its implementation in different scenarios across Europe. A clear strategy for the adoption of HRA tools and related key elements and lessons learnt for a successful transferability at the EU level were defined. HRA strategies should be considered a pillar of healthcare policies and programming to achieve person-centred care and promote the sustainability of the EU healthcare systems.

Abstract:Introduction: Due to the ongoing demographic and epidemiological changes, today stakeholders need to have available information, based on population stratification, in order to plan the most suitable organizational model to meet the population health needs. Methods: The legally adult population assisted and resident in Lazio Region on 31/12/2019 was equally and casually divided in two samples: the training sample (to define the model) and the validation sample (to measure model performances). On the base of the more complex model of Lazio Region, three population strata were defined: multi-chronic population, multi-chronic population with a high clinical complexity, multi-chronic population with socioeconomic vulnerability. Hospital discharge records were identified in the previous five years prior to 31/12/2019. Through appropriate classification models, it was evaluated the level with which the simplified system "from SDO" is able to approximate the more complex algorithm developed by the Lazio Region. Results: Model performances, which has examined only information "from SDO", results inadequate. In fact, the Positive Predictive Value (PVV) results equal to 46.3%, 16.3% and 30.3%, respectively for the three analyzed strata. Discussion: This study demonstrated that using the hospital system as the only Health Information System reduces the possibility to stratify and predict population health needs. For this reason, the improvement of the completeness and of the quality of data from different social and health information systems and their interconnection, represent the essential starting point. In addition, updating record track is needed to simplify the workload of compilers and improve data availability. These actions must also be accompanied by supportive interventions by central bodies for the regions that show the greatest weaknesses, as well as training actions that improve the level of knowledge of compilers.

Abstract:Objective: To examine the functioning profile of people with neurological disorders who access rehabilitation services through ClinFIT Generic-30. Methods: The functioning profile of people with neurological disorders accessing rehabilitation services was examined using the ClinFIT Generic-30, and the results compared with existing core set (neurological health conditions acute and post-acute,stroke, Multiple Sclerosis, Traumatic Brain Injury,Spinal Cord Injury). Results: Data for 364 people were analysed. The 10 most commonly impaired ICF categories included 3 for Body Functions (exercise tolerance functions (b455), mobility of joint functions (b710), and muscle power functions (b730)) and 7 for Activities and Participation (carrying out daily routine (d230), handling stress and other psychological demands (d240), changing basic body position (d410), maintaining a body position (d415), transferring oneself (d420), walking (d450), and moving around (d455)), while the ICF categories that were severely impaired (ICF qualifiers 3 and 4) in more than 30% of the study cohort were: muscle power functions (b730), carrying out daily routine (d230), walking (d450), moving around (d455), doing housework (d640), and assisting others (d660). Discussion: The current study data suggests that ClinFIT Generic-30 appears to effectively identify impairments and/or restrictions, as perceived by individuals affected by selected health conditions. Conclusion: ClinFIT Generic-30 is a tool that can be used to characterize functioning profile in people with different neurological disorders and to collect important information not addressed by the disease-specific core sets (neurological health conditions acute and post-acute,stroke, Multiple Sclerosis, Traumatic Brain Injury,Spinal Cord Injury).

Separating the effects of albedo from eco‐physiological changes on surface temperature along a successional chronosequence in the southeastern United States

Abstract:In the southeastern United States (SE), the conversion of abandoned agricultural land to forests is the dominant feature of land‐cover change. However, few attempts have been made to quantify the impact of such conversion on surface temperature. Here, this issue is explored experimentally and analytically in three adjacent ecosystems (a grass‐covered old‐field, OF, a planted pine forest, PP, and a hardwood forest, HW) representing a successional chronosequence in the SE. The results showed that changes in albedo alone can warm the surface by 0.9°C for the OF‐to‐PP conversion, and 0.7°C for the OF‐to‐HW conversion on annual time scales. However, changes in eco‐physiological and aerodynamic attributes alone can cool the surface by 2.9 and 2.1°C, respectively. Both model and measurements consistently suggest a stronger over‐all surface cooling for the OF‐to‐PP conversion, and the reason is attributed to leaf area variations and its impacts on boundary layer conductance.
Segmental lichen aureus in a young woman with spontaneous improvement

Abstract:Lichen aureus (LA), first described by Martin in 1958 and termed as “lichen purpuricus”, is a rare subtype of pigmented purpuric dermatoses, characterized by isolated golden to rust-brown patches or plaques [1–4]. It is always chronic and asymptomatic, and often affects young adults [1–4]. The lesions are frequently localized on the legs, but can also involve the arms, hands, trunk and even the eyelids [1–4]. We report a case with segmental distribution on the left hip and leg which improved spontaneously. A 30-year-old Chinese woman presented with a 4-month history of asymptomatic purpuric eruptions on her left leg, which had increased gradually in number and size; some lesions had already shown spontaneous regression. The patient Clinical letter denied any other medical problems. There was no history of prior trauma at this site. No family history of similar disorders was noted. Cutaneous examination showed irregular, discrete, lichenoid erythematous-purpuric macules and plaques in a segmental distribution located on the left hip and the backside of the left leg from thigh to calf (Figure 1a, b, c). The lesional distribution did not follow a dermatome or the lines of Blaschko. Routine laboratory tests including complete blood counts, urine analysis were unremarkable. Histopathology of a macule revealed normal epidermis, band-like infiltrate of lymphocytes and extravasated erythrocytes with hemosiderin deposition in the papillary dermis (Figure 2a–c), and deep dermal perivascular, perineural and peri-appendageal lymphocytic infiltrates (Figure 2d, e). The lesions responded poorly to a methylprednisolone 8 mg t.i.d. for 2 weeks along with topical halcinonide ointment. The patient refused further treatment, but the lesions tended to slowly resolve over the next 13 months (Figure 1d, e) although a few new lesions occurred occasionally. LA has a predilection for young men, and less frequently for children [2–4]. The lesions always present as unilateral, discrete or confluent golden to rust-brown lichenoid macules and papules, which may persist unchangeably for many years
The Sociodemographic Factors Related to Disability of Applicants of Welfare Benefits in Greece: A Cross-Sectional Survey Based on the World Health Organization Disability Assessment Schedule (WHODAS) 2.0

Abstract:Introduction: The aim of the present study was to report on the prevalence of disability and its association with sociodemographic factors among welfare benefit applicants in Greece. The study also compared the disability scores between different health conditions using the WHODAS 2.0 (12-item version), a biopsychosocial-model-based measure. Methods: The Greek WHODAS 2.0, 12-item version, was administered by interview. A three-member medical committee assessed the medical records of the applicants and assigned a disability percentage based on the biomedical measure of disability percentage determination (Barema scale). Results: The majority of the participants were female (56.65%). Certain health conditions were presented more frequently among welfare benefit applicants (mental health disorders and neoplasms). The domains with the highest rate of difficulty were the "participation" and "life activities" domains. Significant differences were found between WHODAS 2.0 and Barema scores for all eight different health condition categories. The factorial ANOVA (8x2) showed a significant interaction effect between health condition category and gender with respect to the WHODAS 2.0 score (F = 19.033, p <.001, η2 = 0.13). The WHODAS 2.0 score was negatively correlated to gender, years of studies, and marital status and positively correlated to age, working status, and the Barema score. The results revealed that male participants with a partner who were younger, had more studies, were actively working, and had a lower Barema score would have lower WHODAS scores. Conclusion: Sociodemographic characteristics of welfare benefit applicants are associated with disability levels based on WHODAS 2.0. Certain health conditions, like mental health or neuromusculoskeletal conditions, are associated with higher disability scores. There are differences between the biopsychosocial and the biomedical approaches to disability assessment. The implementation of WHODAS 2.0 may contribute to a better understanding of the lived experience of patients and is a feasible and efficient tool. Combining biomedical and biopsychosocial approaches may enhance the procedures of disability assessment and help in the development of policies that support people with disabilities.

Incidence and prevalence of Amyotrophic Lateral Sclerosis in three Italian Regions: a study based on health administrative databases

Abstract:Objectives: to estimate Amyotrophic Lateral Sclerosis (ALS) incidence and prevalence in three Italian Regions (Lazio, Tuscany, and Umbria), using health administrative databases. Design: retrospective population-based study. Setting and participants: ALS patients residing in Lazio, Umbria, and Tuscany were identified through an algorithm based on three different administrative databases: hospital discharge records, exemptions from health care co-payment, and emergency departments (study period 2014-2019). Crude, age- and gender-specific prevalence were calculated on 31.12.2019 and incidence rates of ALS were standardised by region, year, and gender between 2014-2019. Using a clinical dataset available in the Lazio Region, the proportion of individuals residing in the region correctly identified as ALS cases by the algorithm were calculated. Main outcomes measures: prevalence and incidence rates. Results: a total of 1,031 ALS patients (>=18 years) were identified: 408 cases in Tuscany, 546 in Lazio, and 77 in Umbria. ALS standardised prevalence (per 100,000) was similar among regions: 12.31 in Tuscany, 11.52 in Lazio, and 9.90 in Umbria. The 5-year crude rates were higher in men, and in people aged 65-79 years. Among 310 patients included in the clinical dataset, 263 (84.8%) were correctly identified by the algorithm based on health administrative databases. Conclusions: ALS prevalence and incidence in three Central Italy Regions are rather similar, but slightly higher than those previously reported. This finding is plausible, given that previous results relate to at least ten years ago and evidenced increasing trends. Overall, the results of this paper encourage the use of administrative data to produce occurrence estimates, useful to both epidemiological surveillance and research and healthcare policies.
Development and Internal Validation of a Disability Algorithm for Multiple Sclerosis in Administrative Data

Abstract:Objective: We developed and internally validated an algorithm for disability status in multiple sclerosis (MS) using administrative data. Methods: We linked administrative data from Manitoba, Canada to a clinical dataset with Expanded Disability Status Scale (EDSS) scores for people with MS. Clinical EDSS scores constituted the reference standard. We created candidate indicators using the administrative data. These included indicators based on use of particular health care services (home care, long-term care, rehabilitation admission), use of specific diagnostic codes (such as spasticity, quadriplegia), and codes based on use of Employment and Income Insurance. We developed algorithms to predict severe disability (EDSS ≥6.0), and to predict disability as a continuous measure. We manually developed algorithms, and also employed regression approaches. After we selected our preferred algorithms for disability, we tested their association with health care use due to any cause and infection after potential confounders. Results: We linked clinical and administrative data for 1,767 persons with MS, most of whom were women living in urban areas. All individual indicators tested had specificities >90% for severe disability, and all but a diagnosis of visual disturbance had positive predictive values (PPV) >70%. The combination of home care or long-term care use or rehabilitation admission had a sensitivity of 61.9%, specificity of 90.76%, PPV of 70.06% and negative predictive of 87.21%. Based on regression modeling, the best-performing algorithm for predicting the EDSS as a continuous variable included age, home care use, long-term care admission, admission for rehabilitation, visual disturbance, other paralytic syndromes and spasticity. The mean difference between observed and predicted values of the EDSS was −0.0644 (95%CI −0.1632, 0.0304). Greater disability, whether measured using the clinical EDSS or either of the administrative data algorithms was similarly associated with increased hospitalization rates due to any cause and infection. Conclusion: We developed and internally validated an algorithm for disability in MS using administrative data that may support population-based studies that wish to account for disability status but do not have access to clinical data sources with this information. We also found that more severe disability is associated with increased health care use, including due to infection.

PROFILES OF ICF DISABILITY IN ALZHEIMER AND VASCULAR DEMENTIA

Abstract:Backgrounds: The International Classification of Functioning, Disability and Health (ICF) is a suitable tool to standardize the status of health and disability. A previous study, carried out on 546 subjects included in the database ANASTE (National Association of Nursing Home for Third Age) Calabria, showed that 78.43% of the patients suffered from cognitive impairment whereas 52% had a severe degree of dementia. 65% of them was suffering from Alzheimer’s Disease (AD), whereas 23% from vascular dementia (VD). Objectives: Aim of this study was to analyse the prevalence of functional impairments, activity limitations and participation restrictions of patients affected by AD and VD. Design: Observational descriptive study. Setting: Nursing Homes ANASTE Calabria. Partecipants: 10 patients with probable AD (ADPr) and 10 patients affected by probable VD (VDPr). Measurements: All patients were underwent multidimensional geriatric assessment. The profiles of disability ICF, were expressed in terms of Capacity and Performance, and coded according to mild, medium, severe and complete disability. Environmental factors were skilled in facilitator or no facilitator. Results: The patients with ADPr displayed a severe impairment of functional status, and advanced clinical stage requiring higher care burden compared with VDPr patients. The ICF assessment showed that the global and specific Mental Functions, Communication and Interpersonal Relationships were more reduced in patients with ADPr respect those with VDPr. Conclusions: The identification of a ICF checklist of various forms of dementia, might provides a more detailed description of the profiles of disability and improving therapeutic, rehabilitative interventions and psico-social care.

Abstract:Background: Suspending ordinary care activities during the COVID-19 pandemic made it necessary to find alternative routes to comply with care recommendations not only for acute health needs but also for patients requiring follow-up and multidisciplinary visits. We present the 'Contactless' model, a comprehensive operational tool including a plurality of services delivered remotely, structured according to a complexity gradient, aimed to cover diagnostic procedures and monitor disease progression in chronic pediatric patients. Methods: A multidisciplinary and multiprofessional project team was recruited, in collaboration with patients' associations, to map a panel of available Evidence-Based solutions and address individual needs in full respect of the concept of personalized medicine. The solutions include a number of services from videoconsultations to more structure videotraining sessions. Results: A modular framework made up of four three Macro-levels of complexity - Contactless Basic, Intermediate and Advanced - was displayed as an incremental set of services and operational planning establishing each phase, from factors influencing eligibility to the delivery of the most accurate and complex levels of care. Conclusion: The multimodal, multidisciplinary 'Contactless' model allowed the inclusion of all Units of our Pediatric Department and families with children with disability or complex chronic conditions. The strengths of this project rely on its replicability outside of pediatrics and in the limited resources needed to practically impact patients, caregivers and professionals involved in the process of care. Its implementation in the future may contribute to reduce the duration of hospital admissions, money and parental absence from work.
New accessibility measures based on unconventional big data sources

Abstract:In health econometric studies we are often interested in quantifying aspects related to the accessibility to medical infrastructures. The increasing availability of data automatically collected through unconventional sources (such as webscraping, crowdsourcing or internet of things) recently opened previously unconceivable opportunities to researchers interested in measuring accessibility and to use it as a tool for real-time monitoring, surveillance and health policies definition. This paper contributes to this strand of literature proposing new accessibility measures that can be continuously feeded by automatic data collection. We present new measures of accessibility and we illustrate their use to study the territorial impact of supply-side shocks of health facilities. We also illustrate the potential of our proposal with a case study based on a huge set of data (related to the Emergency Departments in Milan, Italy) that have been webscraped for the purpose of this paper every 5 minutes since November 2021 to March 2022, amounting to approximately 5 million observations.

Urban diabetes: analysis of diabetes prevalence in cities of the Lombardy region participating in the cities changing diabetes project

Abstract:Aim: The urban population increases by about 60 million people/year. Urbanization, unhealthy lifestyle and aging of the population are reflected in a constant growth in the prevalence of diabetes. In 2014, Steno Diabetes Centre in Copenhagen, University College London and Novo Nordisk, launched the Cities Changing Diabetes® program with the aim of creating a unified movement that would stimulate policy-makers to prioritize urban diabetes. Methods: The socio-demographic data derive from (1) ISTAT (National Institute of Statistics of Italy), (2) ATS Metropolitan City of Milan, (3) ATS Val Padana-Cremona, (4) ATS Insubria-Varese, (5) The unemployment rates of the various municipalities have been extrapolated from an ISTAT-MEF elaboration published by Sole 24 Ore journal. Results: In the different sanitary districts of the Metropolitan City of Milan, a strong linear correlation was found between the prevalence of diabetes and the prevalence of heart disease (R = 0.695, p < 0.001), as well as between the prevalence of diabetes and of nephropathies (R = 0.316, p < 0.001). The analysis concerning the province of Cremona showed a fair correlation between the prevalence of diabetes and cardiovascular disease (R = 0.658, p < 0.001). Even for the municipalities of Varese, the analysis documented a good correlation between the prevalence of diabetes and heart disease (R = 0.419, p < 0.001), but not between diabetes and nephropathies. Conclusions: Interesting differences in the relationship of diabetes prevalence with several diseases and socio-demographic factors have been found when comparing the metropolitan City of Milan with two smaller size cities as Varese and Cremona. Our present data confirm the hypothesis that urban diabetes will be the challenge for our society during the next decades.
The Role of Local Factors in Dentin and Pulp in Intradental Pain Mechanisms

Abstract:Pain mechanisms in the dental pulp are not fully understood, and this is also true for most other peripheral tissues. Nevertheless, it is in the periphery that the origin of pain is usually found, and, consequently, there is an urgent need to learn more about causal factors so that a firm rationale for treatment can be established. Since pulpal pain may be present clinically, in cases with normal pulps ("physiological pain"), and also at different stages of pulp pathosis ("pathophysiological pain"), it is apparent that disparate pain mechanisms exist. Therefore, more than one method is needed for the local treatment of pain. The quality of pain may differ and depend on the status of the pulp. On the other hand, the diagnostic value of pain, in this context, is obscured by the fact that the function of the pain fibers is influenced by changes in the local environment, which mayor may not be associated with pulp inflammation. For example, changes in the extracellular dentinal fluid composition due to dentin exposure, caries, or the application of dental materials may influence nerve excitability. Changes in blood microcirculation and intrapulpal pressure also modulate sensory nerve function. Experiments in cat dental pulp have shown that nerve sensitivity can even be abolished by such factors. This may explain the difficulty in correlating any single clinical symptom or test result with the inflammatory process within the pulp, as Langeland found recently (1981). We also know from recent research that there are further explanations for the variability in pain sensations emerging from the pulp. Thus, several investigators (see Johnsen and Narhi, this conference) have independently presented morphological and neurophysiological evidence that more than one type of sensory nerve fiber exists in the mammalian pulp. As would be expected, the pulp, like other peripheral tissues, has a complex sensory innervation which can give rise to a spectrum of uncomfortable sensations. Some of these studies have recently discovered that small-diameter pain fibers in the pulp contain vasoneuroactive substances that can initiate and maintain inflammatory-like functional disturbances in the tissue. In this paper, methods of measuring sensory nerve activity in teeth will be briefly reviewed, with special emphasis on intradental recording techniques used in the cat and man. Some of the results accumulated during the last decade which throw light on local factors, influencing the responses in intradental nerves (mainly of the A-type), will be discussed.
Access to Dental Care-A Survey from Dentists, People with Disabilities and Caregivers

Abstract:The literature highlights differences in the dental conditions of people with disabilities compared with the general population. The aim of this study was to provide an overview of the dental health of people with disabilities in order to understand if their needs are met and to identify their most critical issues as per dentists. A paper and a Google Form platform were used in conducting a survey in Central Italy (the Abruzzo region), by performing an analysis on different points of view as reported by people with disabilities and dentists. The results showed that only 69.2% of dentists treat persons with disabilities. Of these, 73.5% treat less than 10 patients with physical disabilities per year. However, 54% of dentists do not treat people with cognitive impairment and a poor ability to collaborate during treatment. More than 80% of respondent dentists report that people with disabilities do not have good oral hygiene. On the other hand, 49.1% of people with disabilities (or their caregivers in cases where the patient was unable to answer) report that they rarely or never go to the dental office. Moreover, when they do go, it is mainly for emergencies. Despite this, respondents are well aware of their dental problems. However, they have difficulties in communicating their dental problems to their dentist. The 50% of dentists who treat people with cognitive impairment do not include them in follow-up, while only 20% of these patients reported being regularly recalled. This illustrates the importance of the implementation of follow-up. In addition, training courses could help clinicians to reduce this gap and create barrier-free dental offices.
