Abstract:Objective The aims of the study were to describe the up-to-date trend of total, diagnosed, and undiagnosed diabetes and prediabetes, assess their associated disparities among population subgroups , and examine their relationship with sociodemographic factors among adults in the United States. Study design This was a cross-sectional study from a nationally representative sample of US adults (aged ≥20 years) who participated in the National Health and Nutrition Examination Survey. Methods Diagnosed diabetes was defined as a self-reported previous diagnosis of diabetes by a physician or any other health professionals (other than during pregnancy). Undiagnosed diabetes was defined as elevated levels of fasting plasma glucose (≥126 mg/dL) or HbA 1c (≥6.5%). Total diabetes included those who had either diagnosed or undiagnosed diabetes. Prediabetes was defined as an HbA 1c level of 5.7%–6.4% or a fasting plasma glucose level of 100–125 mg/dL. All estimates were age standardized to the 2010 US census population for age groups 20–44, 45–64, and 65+ years. All analyses accounted for the complex survey design. Logistic regressions were used to conduct the analyses. Results A total of 21,600 (mean, 47.2 years [SD, 14.7]) individuals were analyzed. From 1999 to 2018, the age-standardized prevalence increased significantly from 9.17% to 14.7% (difference, 5.52%; 95% confidence interval [CI], 2.69%–8.35%; P -trend <0.001) for total diabetes, increased from 6.15% to 11.0% (difference, 4.79%; 95% CI, 2.27%–7.32%; P -trend<0.001) for diagnosed diabetes and remained stable from 3.01% to 3.73% (difference, 0.72%; 95% CI, −0.47% to 1.91%; P -trend = 0.19) for undiagnosed diabetes. The age-standardized prevalence of prediabetes increased significantly from 29.5% to 48.3% (difference, 18.8%; 95% CI, 13.3%–24.4%; P -trend<0.001). Disparities persisted with higher prevalence among adults with obesity and populations that have been marginalized, including racial and ethnic minorities, low income, less educated Americans, and those living in food-insecure household. Conclusions The prevalence of diabetes and prediabetes increased significantly from 1999 to 2018 among US adults. There are substantial and persistent disparities among racial and ethnic minorities, populations experiencing socio-economic disadvantages, and adults with obesity.

Abstract:Prediabetes is the stage of abnormal glucose metabolism between diabetes and normal blood glucose level and mainly includes 2 states, namely, impaired fasting glucose (IFG, fasting plasma glucose of 5.6–6.9 mmol/L) and impaired glucose tolerance (IGT, postload plasma glucose of 7.8 to 11.0 mmol/L based on 2 hours oral glucose tolerance test). The American Diabetes Association (ADA) has a lower threshold for IFG compared with the WHO but the same threshold for IGT. The ADA also includes hemoglobin A1c (HbA1C) levels of 5.7% to 6.4% as a new high-risk category for diabetes. In 2021, about 541 million of adults have IGT and about 319 million have IFG; by 2045, these numbers are projected to increase by 11.4% and 6.9%, respectively. [ 1 ] This condition imposes a great burden on the society and economy; thus, early diagnosis and control is necessary. In light of this, the ADA 2022 guidelines pointed to a trend toward younger diabetes, bringing the age of diabetes screening earlier from 45 to 35 years of age, with or without diabetes risk factors such as obesity, [ 2 ] and emphasizing the identification of people at risk for prediabetes. Therefore, a comprehensive quantitative analysis of the current situation, focus fields, and prospects for prediabetes is critical for developing effective prevention and control strategies.

Predictive Efficiency of Prediabetes for Diabetes Among Chinese Middle-Aged and Older Populations: a 5-Year National Prospective Cohort Study

Abstract:Background Limited studies have explored the predictive efficiency of prediabetes based on two definitions for diabetes among Chinese middle-aged and older populations with prediabetes. Objective To evaluate the predictive efficiency of prediabetes based on two definitions for diabetes and the clinical and public health benefit in Chinese middle-aged and older populations. Design A 5-year cohort study from the China Health and Retirement Longitudinal Study. Participants A total of 5208 participants who had blood sample data at baseline in 2011. Main Measures The exposure was prediabetes based on American Diabetes Association (ADA) and World Health Organization (WHO) definition. The main outcome was incident diabetes. The ability of prediabetes for predicting diabetes was assessed by sensitivity, specificity, positive predictive value, and negative predictive value. Cox proportional hazards regression was used to explore the associations between prediabetes and the 5-year risk of diabetes and all-cause mortality. Key Results Among those with prediabetes according to the ADA definition, only 426 (15.45%) with baseline prediabetes progressed to total diabetes, while according to the WHO definition, 208 (21.89%) progressed to total diabetes. In terms of the ability of predicting the incident total diabetes in 5 years, the ADA definition has a higher sensitivity than the WHO definition (70.76% versus 34.55%, P < 0.001), while the WHO definition has a higher specificity than the ADA definition (84.09% versus 49.35%, P < 0.001). Positive predictive values based on the two definitions were low (< 24%); negative predictive values were high (> 90%). Conclusions Neither definition of prediabetes is robust for predicting diabetes development in Chineses middle-aged and older populations.

Abstract:Background: There is still controversy surrounding the precise characterization of prediabetic population. We aim to identify and examine factors of demographic, behavioral, clinical, and biochemical characteristics, and obesity indicators (anthropometric characteristics and anthropometric prediction equation) for prediabetes according to different definition criteria of the American Diabetes Association (ADA) in the Chinese population. Methods: A longitudinal study consisted of baseline survey and two follow-ups was conducted, and a pooled data were analyzed. Prediabetes was defined as either impaired fasting glucose (IFG), impaired glucose tolerance (IGT), or elevated glycosylated hemoglobin (HbA1c) according to the ADA criteria. Robust generalized estimating equation models were used. Results: A total of 5,713 (58.42%) observations were prediabetes (IGT, 38.07%; IGT, 26.51%; elevated HbA1c, 23.45%); 9.66% prediabetes fulfilled all the three ADA criteria. Among demographic characteristics, higher age was more evident in elevated HbA1c [adjusted OR (aOR)=2.85]. Female individuals were less likely to have IFG (aOR=0.70) and more likely to suffer from IGT than male individuals (aOR=1.41). Several inconsistency correlations of biochemical characteristics and obesity indicators were detected by prediabetes criteria. Body adiposity estimator exhibited strong association with prediabetes (D10: aOR=4.05). For IFG and elevated HbA1c, the odds of predicted lean body mass exceed other indicators (D10: aOR=3.34; aOR=3.64). For IGT, predicted percent fat presented the highest odds (D10: aOR=6.58). Conclusion: Some correlated factors of prediabetes under different criteria differed, and obesity indicators were easily measured for target identification. Our findings could be used for targeted intervention to optimize preventions to mitigate the obviously increased prevalence of diabetes.

Associations of Diabetes and Prediabetes With Mortality and Life Expectancy in China: A National Study

Abstract:OBJECTIVE To investigate the excess mortality and life-years lost associated with diabetes and prediabetes in China. RESEARCH DESIGN AND METHODS This national cohort study enrolled 135,405 participants aged 18 years or older from the general population in China. Cox proportional hazards regression models were used to estimate adjusted mortality rate ratio (RR). The life table method was used to estimate life expectancy. RESULTS Among the 135,405 participants, 10.5% had diabetes and 36.2% had prediabetes in 2013. During a median follow-up of 6 years, 5517 deaths were recorded, including 1428 and 2300 deaths among people with diabetes and prediabetes, respectively. Diabetes and prediabetes were significantly associated with increased risk of all-cause (diabetes: RR, 1.61 [95% CI 1.49, 1.73]; prediabetes: RR, 1.08 [95% CI 1.01, 1.15]), and cardiovascular disease (diabetes: RR, 1.59 [95% CI 1.41, 1.78]; prediabetes: RR, 1.10 [95% CI 1.00, 1.21]) mortality. Additionally, diabetes was significantly associated with increased risks of death resulting from cancer, respiratory disease, liver disease, and diabetic ketoacidosis or coma. Compared with participants with normoglycemia, life expectancy of those with diabetes and prediabetes was shorter, on average, by 4.2 and 0.7 years at age 40 years, respectively. The magnitude of the associations of diabetes and prediabetes with all-cause and cardiovascular disease mortality varied by age and residence. CONCLUSIONS In this national study, diabetes and prediabetes were significantly associated with reduced life expectancy and increased all-cause and cause-specific mortality risks. The disparities in excess mortality associated with diabetes and prediabetes between different ages and residences have implications for diabetes and prediabetes prevention and treatment programs.

Abstract:Introduction: Racial and ethnic minority groups and individuals with limited educational attainment experience a disproportionate burden of diabetes. Prediabetes represents a high-risk state for developing type 2 diabetes, but most adults with prediabetes are unaware of having the condition. Uncovering whether racial, ethnic, or educational disparities also occur in the prediabetes stage could help inform strategies to support health equity in preventing type 2 diabetes and its complications. We examined the prevalence of prediabetes and prediabetes awareness, with corresponding prevalence ratios according to race, ethnicity, and educational attainment. Methods: This study was a pooled cross-sectional analysis of the National Health and Nutrition Examination Survey data from 2011 to March 2020. The final sample comprised 10,262 U.S. adults who self-reported being Asian, Black, Hispanic, or White. Prediabetes was defined using hemoglobin A1c and fasting plasma glucose values. Those with prediabetes were classified as "aware" or "unaware" based on survey responses. We calculated prevalence ratios (PR) to assess the relationship between race, ethnicity, and educational attainment with prediabetes and prediabetes awareness, controlling for sociodemographic, health and healthcare-related, and clinical characteristics. Results: In fully adjusted logistic regression models, Asian, Black, and Hispanic adults had a statistically significant higher risk of prediabetes than White adults (PR:1.26 [1.18,1.35], PR:1.17 [1.08,1.25], and PR:1.10 [1.02,1.19], respectively). Adults completing less than high school and high school had a significantly higher risk of prediabetes compared to those with a college degree (PR:1.14 [1.02,1.26] and PR:1.12 [1.01,1.23], respectively). We also found that Black and Hispanic adults had higher rates of prediabetes awareness in the fully adjusted model than White adults (PR:1.27 [1.07,1.50] and PR:1.33 [1.02,1.72], respectively). The rates of prediabetes awareness were consistently lower among those with less than a high school education relative to individuals who completed college (fully-adjusted model PR:0.66 [0.47,0.92]). Discussion: Disparities in prediabetes among racial and ethnic minority groups and adults with low educational attainment suggest challenges and opportunities for promoting health equity in high-risk groups and expanding awareness of prediabetes in the United States.

We aimed to estimate the trends of DM over time among adults in the US. Hypothesis: We anticipated an increase in DM and persistent disparities by racial/ethnic and socioecnomic subgroups from 1999 to 2018. Methods: Data were from a nationally representative sample of US adults (≥20 years; NHANES 1999-2018). Diagnosed DM was defined as a self-reported previous diagnosis of DM by a physician or any other health professionals (other than during pregnancy). Undiagnosed DM was defined as elevated levels of fasting plasma glucose (FPG≥126 mg/dL) or HbA1c (≥6.5%). Total DM included those who had either diagnosed or undiagnosed diabetes. Prediabetes was defined as no DM but a HbA1c level of 5.7% - 6.4% or an FPG level of 100 mg/dL-125 mg/dL. All estimates were age-standardized to the 2010 US census population for age groups 20-44, 45-64, and 65+ years. All analyses accounted for the complex survey design. Logistic regressions were conducted to calculate a P-value for trend. Results: Our sample included 53,533 US adults. From 1999 to 2018, the age-adjusted prevalence of total DM increased significantly from 9.05% (95% CI, 7.80%-10.2%) to 13.9% (95% CI, 12.5%-15.4%) and the prevalence of prediabetes increased from 22.5% (20%-25.2%) to 40.2% (37.4%-43.1%) (P-trends<0.001). The rate of increase in prevalence was higher among Mexican Americans but lower among non-Hispanic black individuals compared to non-Hispanic white individuals (all P-trends<0.01, P-interaction=0.003). Trends in total DM by education and income levels were similar to the overall trend but disparities persisted between low- and high-socioeconomic groups (all P-trends 0.05) ( Figure 1 ). Conclusions: The prevalence of DM increased significantly from 1999 to 2018 among US adults. There are substantial and persistent disparities between racial/ethnic and socioeconomic subgroups.

Abstract:Background and aims: Prediabetes is defined as a state of impaired glucose metabolism with hemoglobin A1c (HbA1c) levels that precede those of a diabetic state. There is increasing evidence that suggests that hyperglycemic derangement in prediabetes leads to microvascular and macrovascular complications even before progression to overt diabetes mellitus. We aim to identify the association of prediabetes with acute cardiovascular events. Methods: We utilized the National inpatient sample 2018-2020 to identify adult hospitalizations with prediabetes after excluding all hospitalizations with diabetes. Demographics and prevalence of other cardiovascular risk factors were compared in hospitalizations with and without prediabetes using the chi-square test for categorical variables and the t-test for continuous variables. Multivariate regression analysis was further performed to study the impact of prediabetes on acute coronary syndrome, acute ischemic stroke, intracranial hemorrhage, and acute heart failure. Results: Hospitalizations with prediabetes had a higher prevalence of cardiovascular risk factors like hypertension, hyperlipidemia, obesity, and tobacco abuse. In addition, the adjusted analysis revealed that hospitalizations with prediabetes were associated with higher odds of developing acute coronary syndrome (OR-2.01; C.I:1.94-2.08; P<0.001), acute ischemic stroke (OR-2.21; 2.11-2.31; p<0.001), and acute heart failure (OR-1.41; C.I.: 1.29-1.55; p<0.001) as compared to hospitalizations without prediabetes. Conclusions: Our study suggests that prediabetes is associated with a higher odds of major cardiovascular events. Further prospective studies should be conducted to identify prediabetes as an independent causative factor for these events. In addition, screening and lifestyle modifications for prediabetics should be encouraged to improve patient outcomes.

Abstract:AIMS: To examine the predictive factors associated with the progression of different prediabetic status to diabetes.METHODS: A two-year retrospective cohort study was conducted on 5741 participants aged 40 years or older. Finally, 1685 participants with prediabetes defined by IFG (impaired fasting glucose), IGT (impaired glucose tolerance) and CGI (combined IFG and IGT) were included. Logistic regression model was used to evaluate the risk of prediabetes progression to diabetes.RESULTS: Of the 1685 subjects with prediabetes at baseline, 212 (12.6%) subjects progressed to diabetes and 1473 (87.4%) subjects did not. Logistic regression analysis demonstrated that people with CGI were associated with an increased risk of progressing to diabetes compared to those with IFG (OR, 95% CI: 3.127, 2.047-4.776). Moreover, males, obese people, people with increased BMI and WHR (Waist/ Hip ratio), and hypertension were positively associated with the progression to diabetes, while HOMA-β was negatively associated with the progression to diabetes.CONCLUSIONS: Subjects with CGI are prone to progressed to diabetes compared to those with IFG or IGT in middle-aged and older person in China. More attention should be paid to male and obese prediabetic subjects, and measures should be taken to control the increase in their BMI and WHR.

Diagnosis and Management of Prediabetes: A Review

Abstract:Prediabetes isa highly prevalent intermediate stage of glucose dysregulation that may precede type 2 diabetes. In the United States, approximately 10% of those with prediabetes will develop diabetes each year, and the prevalence of prediabetes is steadily increasing. The aim of this review is to summarize the epidemiology, diagnostic criteria, clinical outcomes, and management of pre -diabetes. A literature search was conducted for high-quality prospective cohort studies, randomized trials, systematic reviews, and meta-analyses. For the purpose of this review, outcomes associated with cases defined by fasting plasma glucose level, 2-hour plasma glucose level after a 75-g oral glucose challenge (2hPG), or glycosylated hemoglobin (HbA1c) level were in-cluded. Different diagnostic criteria for prediabetes are reported by the American Diabetes Association, the World Health Organization, and the International Expert Committee; however, a fasting plasma glucose of greater than 100, a 2hPG of greater than 140, or an HbA1c of greater than 6.0 would qualify by each organization's standards. Nationally representative surveys were queried to obtain estimates of disease frequency. A total of 110 studies were included, including 20 randomized clinical trials, 9 systematic reviews and meta-analyses, 7 guide-lines, 3 diagnostic studies, 58 cohort studies, and results of 13 nationally representative surveys. Longitudinal studies revealed an incident prediabetes rate of 6.2% among predominantly White participants, 7.8% among people those identifying as Pima In-dian, and 11% among Black or White adults with parental history of type 2 diabetes. Risk factors for the development of pre -diabetes include overweight or obesity, older age, physical inactivity, unhealthy diet, and genetic predisposition. A meta-analysis revealed that a history of diabetes in at least 1 first-degree relative was associated with increased odds of having prediabetes (odds ratio, 1.40; 95% confidence interval, 1.27-1.54). One observational study found that 102 of 265 participants with predi-abetes progressed to diabetes during 10 years of follow-up, whereas another found the cumulative diabetes incidence was 95.9% among participants with prediabetes without treatment over a 30-year follow-up period. Another study found that of the 3 diag-nostic methods the highest relative risk for development of diabetes from prediabetes was associated with a high HbA1c.A meta-analysis of 129 prospective studies found that prediabetes was associated with increased rates of cardiovascular disease (incidence rate per 10,000 person-years, 58.3 in those with normal glucose regulation vs 67.0 among those with pre -diabetes) and all-cause mortality (73.6 among those with normal glucose regulation and 81 among those with prediabetes). In addition, microvascular complications including retinopathy, peripheral neuropathy, and chronic kidney disease are frequent among those with prediabetes. Lifestyle modifications such as increased exercise anda healthy diet are the mainstays of treat-ment for prediabetes. Interventions targeting these pillars have shown a decrease in the incidence of diabetes compared with placebo or usual care among those with prediabetes and women with a history of gestational diabetes. In addition, several randomized clinical trials have shown metformin may reduce the incidence of diabetes among people with prediabetes; however, it was less efficacious than lifestyle intervention. The results of this review show that prediabetes is associated with increased risk of diabetes, cardiovascular events, and mortality and should be treated with lifestyle modification that includes weight loss and exercise with or without metformin.

Abstract:OBJECTIVE Prediabetes has been suggested to increase risk for death; however, the definitions of prediabetes that can predict death remain elusive. We prospectively investigated the association of multiple definitions of prediabetes with the risk of death from all causes, cardiovascular disease (CVD), and cancer in Japanese workers. RESEARCH DESIGN AND METHODS The study included 62,785 workers who underwent a health checkup in 2010 or 2011 and were followed up for death from 2012 to March 2019. Prediabetes was defined according to fasting plasma glucose (FPG) or glycated hemoglobin (HbA1c) values or a combination of both using the American Diabetes Association (ADA) or World Health Organization (WHO)/International Expert Committee (IEC) criteria. The Cox proportional hazards regression model was used to investigate the associations. RESULTS Over a 7-year follow-up, 229 deaths were documented. Compared with normoglycemia, prediabetes defined according to ADA criteria was associated with a higher risk of all-cause mortality (hazard ratio [HR] 1.53; 95% CI 1.12–2.09) and death due to cancer (HR 2.37; 95% CI 1.45–3.89) but not with death due to CVD. The results were materially unchanged when prediabetes was defined according to ADA FPG, ADA HbA1c, WHO FPG, or combined WHO/IEC criteria. Diabetes was associated with the risk of all-cause, CVD, and cancer deaths. CONCLUSIONS In a cohort of Japanese workers, FPG- and HbA1c-defined prediabetes, according to ADA or WHO/IEC, were associated with a significantly increased risk of death from all causes and cancer but not CVD.

Abstract:Aims Estimated glucose disposal rate (eGDR), a noninvasive and convenient measure of insulin resistance, has been demonstrated to be associated with mortality in both type 1 and type 2 diabetes. We aimed to explore whether eGDR is associated with cardiovascular disease (CVD) risk and mortality in prediabetic adults. Methods A nationwide population-based cohort of prediabetic individuals from the National Health and Nutrition Examination Survey 1999–2018 with available data on eGDR was included and categorized into eGDR ≥ 8 (reference), 6–7.99, 4–5.99, and < 4 mg/kg/min groups. Cox proportional hazards model was used to estimate the associations of eGDR with mortality. Results A total of 4725 prediabetic adults, 60.12% men, mean age 48 years were included. The odds ratio and 95% confidence interval (CI) for CVD risk were 1.74 (1.08–2.78), 2.90 (1.79–4.67), and 4.58 (2.15–9.76) for the eGDR 6–7.99, 4–5.99, and < 4 mg/kg/min groups, respectively, compared with the reference group. There were 410 deaths (116 CVD-related) during a median follow-up of 107 months in 4,332 participants without baseline CVD. The hazard ratios and 95%CI for the eGDR 6–7.99, 4–5.99, and < 4 mg/kg/min groups were 1.70 (1.23–2.35), 2.01 (1.45–2.77), and 1.84 (1.11–3.04), respectively, for all-cause mortality ( P for trend < 0.0001), and 3.84 (2.04–7.21), 4.01 (2.01–8.00), and 2.88 (1.03–8.06), respectively, for CVD mortality ( P for trend = 0.01). Smoking status significantly modified the associations between eGDR and all-cause or CVD mortality. Conclusions Increased insulin resistance, as indicated by a lower eGDR, is associated with increased risks of all-cause and CVD mortality in U.S. prediabetic adults.

Abstract:Background: To estimate secular trends and disease burden of diabetes and prediabetes among Chinese adults. Methods: Three population-based surveys were performed among Chinese adults in Shanghai in 2002-2003 (n = 12 302), 2009 (n = 7414), and 2017 ( n = 18 960). Diabetes and prediabetes were defined using the 1999 World Health Organization ( WHO) criteria. Cochran-Armitage trend test was used to examine the trends in prevalence, awareness, and glycemic control status. Disability adjusted life years (DALYs) were estimated to evaluate the disease burden of diabetes- related complications using the population attribution fraction approach based on published data. Results: The age-adjusted prevalence of diabetes increased during the 15-year period (p for trend <.001) and reached 23.0% (95% CI: 22.1 similar to 24.0%) in men and 15.7% ( 95% CI: 15.1 similar to 16.4%) among women in 2017. The prevalence of impaired glucose tolerance peaked in 2009, whereas that of impaired fasting glucose increased continuously (p for trend <.001). The awareness of diabetes was found to increase and the glycemic control rates decreased over the three surveys. The estimated DALYs of diabetes complications were found to have increased rapidly due to the increasing prevalence of diabetes and the decreasing glycemic control rates. Conclusions: Prediabetes and diabetes affect a considerable proportion of Chinese adults in Shanghai. Our results highlight the necessary to strengthen the community healthcare system in China to guarantee extensive management of diabetes and prediabetes.

It provides a series of country-specific case studies of computing education in schools that highlights the way in which curricula emerge from each country’s specific historical and cultural circumstances. As a result, not only is there much to learn from each of the case studies, but there are additional lessons in the commonalities and generalizations obtainable only by having a rich set of case studies such as these that can be viewed comparatively.

Abstract:Prediabetes presents a high-risk state for the development of various diseases and is reversible by adhering to a healthy lifestyle. We conducted this analysis to explore the associations of the Healthy Eating Index-2015 (HEI-2015) and the Alternate Mediterranean Diet Index (aMed index) with the risk of prediabetes. The data were derived from the National Health and Nutrition Examination Survey, including 20,844 participants. Multivariable-adjusted odds ratios (OR) of prediabetes and 95% confidence intervals (CI) by tertile of diet quality scores were estimated using a weighted logistic regression. Compared to those in the lowest tertile, the multivariable-adjusted OR of prediabetes for the highest tertile was 0.82 (95% CI: 0.72, 0.94; p for trend = 0.005) for HEI-2015 and 0.87 (95% CI: 0.76, 0.98; p for trend = 0.02) for the aMed index. After mutual adjustment, the association for HEI-2015 (p for trend = 0.03) but not for the aMed index (p for trend = 0.59) remained significant. Among the component food groups and nutrients, higher intakes of red and processed meat, sodium, and total saturated fatty acids were associated with a higher risk of prediabetes, while moderate alcohol consumption was associated with a lower risk. In conclusion, adherence to the 2015–2020 Dietary Guidelines for Americans, as compared with the Mediterranean Diet, appeared to be more strongly associated with a lower risk of prediabetes among adults in the United States.

Piroxicam poisoning in a 2-year-old child. A case report.

Abstract:This report describes the severe multisystem toxicity which followed ingestion of 5 piroxicam capsules (100 mg) by a 2-year-old child. Gastro-intestinal symptoms developed within 2 hours, resulting in severe fluid and electrolyte imbalance, mental confusion and a generalized seizure. Evidence of liver and renal dysfunction developed within 3 days. Haemopoietic toxicity was manifested by progressive peripheral pancytopenia, bone marrow aplasia and coagulopathy. Pseudomonas septicaemia developed during the period of neutropenia. Clinical, biochemical and haematological abnormalities slowly resolved over 3-4 weeks. In view of the increasing use of piroxicam as an anti-inflammatory agent it seemed important to draw attention to the potentially serious effects of accidental overdosage.
Prevalence of Diagnosed Type 1 and Type 2 Diabetes among US Adults in 2016 and 2017: Population Based Study.

Abstract:OBJECTIVE To estimate the prevalence of diagnosed total diabetes, type 1 diabetes, and type 2 diabetes in the US general population and the proportions of each among US adults with a diagnosis of diabetes. DESIGN Nationwide, population based, cross sectional survey. SETTING National Health Interview Survey, 2016 and 2017. PARTICIPANTS Adults aged 20 years or older (n=58 186), as a nationally representative sample of the civilian, non-institutionalized US population. MAIN OUTCOME MEASURES Prevalence of diagnosed diabetes, type 1 diabetes, and type 2 diabetes in the US general population, and the proportions of each subtype in participants with a diagnosis of diabetes. RESULTS Among the 58 186 included adults, 6317 had received a diagnosis of diabetes. The weighted prevalence of diagnosed diabetes, type 1 diabetes, and type 2 diabetes among US adults was 9.7% (95% confidence interval 9.4% to 10.0%), 0.5% (0.5% to 0.6%), and 8.5% (8.2% to 8.8%), respectively. Type 1 diabetes was more prevalent among adults with lower education level, and type 2 diabetes was more prevalent among older adults, men, and those with lower educational level, lower family income level, and higher body mass index (BMI). Among adults with a diagnosis of diabetes, the weighted percentage of type 1 and type 2 diabetes was 5.6% (4.9% to 6.4%) and 91.2% (90.4% to 92.1%), respectively. The percentage of type 1 diabetes was higher among younger adults (age 20-44 years), non-Hispanic white people, those with higher education level, and those with lower BMI, whereas the percentage of type 2 diabetes was higher among older adults (age >= 65 years), non-Hispanic Asians, those with lower education level, and those with higher BMI. CONCLUSION This study provided benchmark estimates on the national prevalence of diagnosed type 1 diabetes (0.5%) and type 2 diabetes (8.5%) among US adults. Among US adults with diagnosed diabetes, type 1 and type 2 diabetes accounted for 5.6% and 91.2%, respectively.
Incidence and Temporal Trends in Type 2 Diabetes by Weight Status: A Systematic Review and Meta-Analysis of Prospective Cohort Studies

Abstract:Background Diabetes is more prevalent among overweight/obese individuals, but has become a significant public health challenge among normal-weight populations. In this meta-analysis, we aimed to estimate diabetes/prediabetes incidence and its temporal trends by weight status.Methods PubMed, Embase, Web of Science, and Cochrane Library were searched until 8 December 2021. Prospective cohort studies reporting diabetes incidence by baseline body mass index (BMI) categories in adults were included. The median year of data collection was used to assess the temporal trends. Subgroup analyses and meta-regression were also performed.Results We included 94 studies involving 3.4 million adults from 22 countries. The pooled diabetes incidence in underweight, normal-weight, and overweight/obese adults was 4.5 (95% confidence interval (CI) = 2.8-7.3), 2.7 (95% CI = 2.2-3.3), and 10.5 (95% CI = 9.3-11.8) per 1000 person-years, respectively. The diabetes incidence in low-and middle-income countries (LMICs) was higher than in high-income countries among normal-weight (5.8 vs 2.0 per 1000 person-years) or overweight/obese (15.9 vs 8.9 per 1000 person-years) adults. European and American regions had a higher diabetes incidence than the non-Western areas, regardless of weight status. Underweight diabetes incidence decreased significantly from 1995-2000 to 2005-2010. Diabetes incidence in normal-weight populations has increased continuously since 1985 by an estimated 36% every five years. In overweight/obese adults, diabetes incidence increased between 1985-1990 and 1995-2000, stabilised between 2000 and 2010, and spiked suddenly after 2010.Conclusions Diabetes incidence and its temporal trends differed by weight status. The continuous upward trend of diabetes incidence among overweight/ obese individuals requires urgent attention, particularly in LMICs. Furthermore, diabetes among normal-weight individuals is becoming a significant public health problem.Registration PROSPERO (CRD42020215957).
Time trends in the incidence of clinically diagnosed type 2 diabetes and pre-diabetes in the UK 2009–2018: a retrospective cohort study

Abstract:Introduction To describe recent trends in the incidence of clinically diagnosed type 2 diabetes and pre-diabetes in people seen in UK general practice. Research design and methods A retrospective cohort study using IQVIA Medical Research Data looking at people newly diagnosed with type 2 diabetes and pre-diabetes through primary care registers in the UK between 1 January 2009 and 31 December 2018. Results A cohort of 426 717 people were clinically diagnosed with type 2 diabetes and 418 656 people met the criteria for a diagnosis of pre-diabetes in that time period. The incidence of clinically diagnosed type 2 diabetes per 1000 person years at risk (PYAR) in men decreased from a peak of 5.06 per 1000 PYAR (95% CI 4.97 to 5.15) in 2013 to 3.56 per 1000 PYAR (95% CI 3.46 to 3.66) by 2018. For women, the incidence of clinically diagnosed type 2 diabetes per 1000 PYAR decreased from 4.45 (95% CI 4.37 to 4.54) in 2013 to 2.85 (2.76 to 2.93) in 2018. The incidence rate of pre-diabetes tripled by the end of the same study period in men and women. Conclusions Between 2009 and 2018, the incidence rate of new clinical diagnoses of type 2 diabetes recorded in a UK primary care database decreased by a third from its peak in 2013–2014, while the incidence of pre-diabetes has tripled. The implications of this on timely treatment, complication rates and mortality need further longer term exploration.
