Abstract:Firearm availability is correlated with increased risk of suicide but its link with suicidal ideation remains unclear. Previous studies are limited by retrospective reports and prospective designs with lengthy gaps between assessments that are ill-suited for measuring fluctuations in suicidal ideation. This study used ecological momentary assessment (EMA) to repeatedly assess suicidal ideation in a sample of 138 U.S. adults (81 handgun owners, 57 non-owners). Participants received six EMA prompts per day for 28 consecutive days. Results revealed no group differences in the frequency, maximum amplitude, or variability of suicidal ideation across male and female handgun owners and non-owners. Stability of suicidal ideation significantly differed across groups, however ( F (1,132) = 4.5, p = 0.036); male handgun owners had the strongest stability and male non-owners had the weakest stability. Stability of suicidal ideation was significantly lower when participants reported a firearm was nearby as compared to when no firearm was nearby ( F (4,17732) = 5.6, p < 0.001). Results suggest firearm availability increases reactivity to the environment, slows recovery from acutely elevated risk states, and may increase vulnerability to sudden shifts to higher risk states characterized by increased probability of suicidal behavior. Although these effects were observed in both handgun owners and non-owners, they disproportionately impact handgun owners because they report being near firearms more often.

Associations among exaggerated threat perceptions, suicidal thoughts, and suicidal behaviors in U.S. firearm owners

Abstract:Firearm availability has traditionally been conceptualized as an environmental risk factor for suicide mortality. Firearm acquisition for the primary purpose of self-protection (i.e., protective gun ownership) is often motivated by exaggerated threat expectancies-a central component of anticipatory anxiety-but firearm acquisition may paradoxically reinforce or exacerbate threat expectancies and contribute to behavioral disinhibition. Firearm ownership may therefore confer increased risk for suicide mortality through biobehavioral mechanisms. The present study conducted an initial test of this hypothesis by examining associations among firearm ownership, intentions to acquire additional firearms, threat expectancies, and past-month suicidal thoughts and behaviors using data collected from a national sample of 6200 U.S. adults. Results indicated threat expectancies and rates of suicide-related behaviors were significantly elevated among protective gun owners and participants who intended to acquire a firearm within the next year as compared to non-gun owners and non-protective gun owners. Threat expectancies were associated with significantly increased risk for past-month suicide-related behavior. Results suggest that threat expectancies in particular, and anticipatory anxiety more generally, may be biobehavioral processes associated with the correlation of firearm ownership and increased suicide risk.

Abstract:Introduction Firearms account for more than half of suicide deaths in the United States (US) and both ownership and access are associated with increased risk of intentional and unintentional injury. Despite evidence linking ownership and access to suicide risk, individuals may be reticent to answer questionnaire items assessing ownership. The current study examined characteristics of individuals who do not provide a response (nonresponders) to a firearm ownership item in a community sample. Methods Data were collected from a cross‐sectional online survey of 10,625 US adults. Univariate and multivariate analyses were conducted to examine demographic, emotional distress, and suicide risk differences across three groups (firearm owners, nonowners, and nonresponders). Results Nonresponders were significantly younger, more likely to be female and non‐White than firearm owners and nonowners. Nonresponders were less likely to endorse recent suicidal ideation and probable PTSD than firearm owners, but more likely to endorse probable PTSD than nonowners. Firearm owners were significantly more likely to report several correlates of suicide risk than nonowners. Conclusions Nonresponders may be a unique subgroup with distinct demographic, emotional distress, and suicide risk profiles compared to both firearm owners and nonowners. Implications of these findings for future directions are discussed.

Testing the interpersonal theory of suicide in a sample of sexual minority young adults: Attention to within-group differences

Abstract:Introduction: This study investigated demographic differences in interpersonal theory of suicide factors and their associations with suicide attempts among sexual minority young adults. Methods: 784 sexual minority young adults ages 18-29 (42.7% cisgender men, 42.2% cisgender women, 15.1% transgender/gender diverse; 62.2% non-Hispanic White; 50.5% gay/lesbian, 49.5% bisexual+) completed an online survey assessing lifetime suicide attempts and interpersonal theory of suicide factors. Results: Demographic differences included (1) greater perceived burdensomeness among transgender/gender diverse participants compared to other gender groups; (2) greater acquired capability for suicide and suicide attempts among cisgender men compared to cisgender women; (3) greater acquired capability for suicide among bisexual+ compared to gay/lesbian participants; and (4) a lower number of suicide attempts among Asian/Asian American sexual minority participants compared to most other sexual minority participants. All interpersonal theory of suicide factors were significantly associated with a higher number of suicide attempts, though only perceived burdensomeness and acquired capability for suicide remained significant when examining all three simultaneously. No two- or three-way interactions between interpersonal theory of suicide factors were significant. Conclusion: The interpersonal theory of suicide may be useful for understanding suicide attempts in this population, with perceived burdensomeness and acquired capability being particularly relevant to consider.

Abstract:Introduction Men have an increased risk to die by suicide compared to women but are underrepresented in suicide research. To improve individual risk prediction for suicide‐related thoughts and behaviors (STBs), risk factors are increasingly being studied at an individual level. The possible affect‐regulatory function of suicidal ideation has hardly been investigated ideographically and has not yet been tested in a male sample. Methods We investigated the bidirectional associations between suicidal ideation and negative and positive affect in a sample of n = 21 male inpatients with unipolar depression and a history of suicidal ideation both at the group level and at the individual level. Participants underwent an intensive ecological momentary assessment for 6 days with 10 data points per day. Results We found no evidence for an affect‐regulatory function of suicidal ideation at the group level, neither for effects of affect on subsequent suicidal ideation (antecedent affect‐regulatory hypothesis) nor for effects of suicidal ideation on subsequent affect (consequence affect‐regulatory hypothesis). Person‐specific analyses revealed substantial variability in strength and direction of the considered associations, especially for the associations representing the antecedent hypothesis. Conclusion The demonstrated between‐person heterogeneity points to the necessity to individualize the investigation of risk factors to enhance prediction and prevention of STBs.

Abstract:Most theories of suicide propose within-person changes in psychological states cause suicidal thoughts/behaviors; however, most studies use between-person analyses. Thus, there are little empirical data exploring current theories in the way they are hypothesized to occur. We used a form of statistical modeling called group iterative multiple model estimation (GIMME) to explore one theory of suicide: The Interpersonal Theory of Suicide (IPTS). GIMME estimates personalized statistical models for each individual and associations shared across individuals. Data were from a real-time monitoring study of individuals with a history of suicidal thoughts/behavior (adult sample: participants = 111, observations = 25,242; adolescent sample: participants = 145, observations = 26,182). Across both samples, none of theorized IPTS effects (i.e., contemporaneous effect from hopeless to suicidal thinking) were shared at the group level. There was significant heterogeneity in the personalized models, suggesting there are different pathways through which different people come to experience suicidal thoughts/behaviors. These findings highlight the complexity of suicide risk and the need for more personalized approaches to assessment and prediction.

Abstract:Psychological Reports, Ahead of Print. Access to firearms among individuals with mental health problems has been a source of protracted debate among policymakers, the media, and the public, writ large. At the center of this controversy are questions about the nature and consequences of gun access in the context of mental illness. The lack of substantial empirical evidence, due in part to limited access to quality data, plays a significant role in perpetuating ongoing debate. To address this problem, this study uses data from the National Comorbidity Survey Replication to evaluate the relative importance of several clinical, cultural, and criminological factors in explaining gun access and carrying among adults with and without mental illnesses. Multivariate analyses reveal that, whereas past year disorder (of any type or severity) and other clinical characteristics were unrelated to firearm access, several cultural factors such as childhood rurality (e.g., ORrural vs. urban: 3.59; 95% CI: 2.52, 5.12) and the criminological experience of early intimate partner violence (e.g., ORvictim only vs. noexp: 1.84; 95% CI: 1.50, 2.26) were. None were predictive of carrying. Further, none of the relationships observed were conditioned on any of the clinical characteristics. These results indicate that people with mental illnesses likely own and carry guns for the same reasons and in the same contexts as others. Additional updated and quality data is needed to further explore these issues; however, these finding suggest that suicide and violence prevention efforts targeting people with mental illnesses need to be sensitive to the cultural and personal significance of guns.

Abstract:Introduction: Self-directed violence (SDV), comprising both non-suicidal self-injury (NSSI) and suicide, is elevated among sexual minority populations. NSSI age of onset has been shown to impact severity of SDV outcomes. Components of the Interpersonal Theory of Suicide (ITS), which includes perceived burdensomeness (PB), thwarted belongingness (TB), and acquired capability for suicide (ACS), may be useful to understanding NSSI subgroups, especially among under-researched groups such as sexual minority persons. Methods: This study examined subtypes of NSSI and associations with constructs from the ITS. A sample of 473 sexual minority young adults with a history of NSSI completed self-report measures assessing demographics, SDV, and ITS constructs. More than half of the sample (59.7%) reported engaging in NSSI during their lifetime. For suicidal thoughts and behaviors, a majority of the sample reported having thoughts of suicide in their lifetime (87.3%), and one-third reported a lifetime suicide attempt (33.0%). Results: Latent class analysis revealed three classes: (1) lower NSSI, (2) moderate NSSI, and (3) higher NSSI. NSSI age of onset, intensity of suicidal thoughts, suicide plan likelihood, duration of NSSI, and number of NSSI methods differentiated groups. Multivariate Analysis of Variance (MANOVA) revealed that the lower NSSI group reported lower PB and TB than the moderate NSSI group. No differences were found for ACS across NSSI classes. Discussion: Results are discussed in terms of their theoretical implications and application for counseling at-risk sexual minority young adults. This study identifies factors associated with subgroups of self-directed violence among sexual minority young adults. Such findings inform gaps in self-directed violence prevention research and clinical practice for sexual minority persons.

The Association of Suicidal Ideation With Firearm Purchasing During a Firearm Purchasing Surge

Abstract:Importance: Firearm access is associated with risk for suicide, and, since early 2020, the US has experienced an unprecedented surge in firearm sales. Objective: To assess frequency of suicidal ideation among individuals who purchased firearms during the surge period (surge purchasers), other firearm owners, and non-firearm owners. Design, setting, and participants: Cross-sectional online survey data were collected from January to June 2021. Participants were recruited via quota sampling through Qualtrics Panels. Within Minnesota, zip codes from Minneapolis and St Paul were oversampled. Participants included 6404 US adults recruited from 3 states: New Jersey (n = 3197), Minnesota (n = 1789), and Mississippi (n = 1418). Participants identified as becoming a first-time firearm owner during the surge period, being an established firearm owner who purchased a firearm during the surge period, being a firearm owner who did not buy firearms during the surge period, and not owning firearms. Main outcomes and measures: The primary outcomes were lifetime, past year, and past month suicidal ideation as measured by the Self-injurious Thoughts and Behaviors Interview-Revised. Results: The mean (SD) age within the full sample was 44.81 (18.45) years, with 3132 males (48.8%), 4706 White adults (73.4%), 2674 reporting annual household income less than $50 000 (41.7%), and 1546 (24.1%) reporting current firearm ownership. In the full sample, individuals who purchased firearms during the surge period were more likely than were non-firearm owners to report lifetime suicidal ideation (odds ratio [OR], 2.21; 95% CI, 1.82-2.68), past-year suicidal ideation (OR, 2.10; 95% CI, 1.72-2.55), and past-month suicidal ideation (OR, 1.80; 95% CI, 1.41-2.29). In addition, among individuals who purchased firearms during the surge period, first-time owners were more likely than established firearm owners to report lifetime suicidal ideation (OR, 2.12; 95% CI, 1.43-3.14) and past-year suicidal ideation (OR, 2.37; 95% CI, 1.59-3.53). Results were largely consistent across states. Conclusions and relevance: In this survey study, results highlighted that individuals who acquired firearms during the purchasing surge were more likely than other firearm owners and non-firearm owners to have experienced suicidal thoughts. This is particularly true for individuals who purchased a firearm for the first time during the surge period. This illustrates the need to implement policies and interventions that increase safety among firearm purchasers (eg, safe firearm storage) as well as those that promote the acquisition of alternative forms of protection (eg, home alarm systems).
Person-specific dynamics between negative emotions and suicidal thoughts

Abstract:Introduction Recent technology has enabled researchers to collect ecological momentary assessments (EMA) to examine within-person correlates of suicidal thoughts. Prior studies examined generalized temporal dynamics of emotions and suicidal thinking over brief periods, but it is not yet known how variable these processes are across people. Method We use data EMA data delivered over two weeks with youth/young adults ( N = 60) who reported past year self-injurious thoughts/behaviors. We used group iterative multiple model estimation (GIMME) to model group- and person-specific associations of negative emotions (i.e., fear, sadness, shame, guilt, and anger) and suicidal thoughts. Results 29 participants (48.33%) reported at least one instance of a suicidal thought and were included in GIMME models. In group level models, we consistently observed autoregressive effects for suicidal thoughts (e.g., earlier thoughts predicting later thoughts), although the magnitude and direction of this link varied from person-to-person. Among emotions, sadness was most frequently associated with contemporaneous suicidal thoughts, but this was evident for less than half of the sample, while other emotional correlates of suicidal thoughts broadly differed across people. No emotion variable was linked to future suicidal thoughts in >14% of the sample, Conclusions Emotion-based correlates of suicidal thoughts are heterogeneous across people. Better understanding of the individual-level pathways maintaining suicidal thoughts/behaviors may lead to more effective, personalized interventions.

[Surgical treatment of unroofed coronary sinus].

Abstract:We have experienced four cases of unroofed coronary sinus. Three cases were partial forms and one case was a complete form. Two of them were successfully diagnosed before operation. Surgical viewpoints were discussed. In the absence of LSVC, the coronary sinus defect may be simply closed. In the cases with PLSVC, several types of repair were discussed. It has been reported that this defect is usually associated with simple or complex cardiac malformations, including tricuspid atresia or tetralogy of Fallot. In our case with TA, oversight of this anomaly before and during operation resulted in the urgent reoperation. A surgeon should always keep in mind this anomaly, even if the coronary sinus is not large.

Abstract:Introduction This study investigated the trajectories of interpersonal stress and psychological pain and their effects on self‐harm behaviors in adolescents and explored the reciprocal associations between interpersonal stress and NSSI/SA. Methods A delay discounting approach was applied to evaluate the degree to which these choices were influenced by two dimensions of reinforcer efficacy—delay and magnitude. The decision‐making task sampled choice behavior across various durations of temporary voluntary removal of firearms (Delay) and differences in stated potential risk of suicide (Magnitude) as a result of that choice. Results Results of mixed‐effects modeling indicated that the subjective value of immediate access to a firearm was differentially sensitive to both delay and magnitude. Additionally, the scaling of these effects was linked to various other indicators of firearm‐specific safety (e.g., use of trigger locks) and suicidality risk (e.g., intolerance of uncertainty). Conclusions These results provide additional support for behavioral models of decision‐making (i.e., delay discounting) when evaluating how specific environmental arrangements and framing may support (or potentially discourage) engagement in means safety activities, inclusive of temporary firearms access restriction. These findings suggest that further analysis of the ecological underpinnings of these choices may help to guide more targeted efforts to engage with firearm owners in safety planning when there are concerns about the potential for suicide.

Abstract:A simple and unique route to synthesize the nano-size Pt particles using mesoporous silica support including single-site Ti-oxide moiety (Ti-HMS) under UV-light irradiation has been developed. By the photo-assisted deposition (PAD) method, a Pt precursor can be deposited from aqueous solution of H2PtCl6 directly on the photo-excited isolated Ti-oxide moiety. The subsequent reduction with H2 generates the nano-sized Pt metal particles (PAD-Pt/Ti-HMS). The deposition of Pt precursor onto the Ti-HMS can be recognized by the observation of the decrease in the intensity of the preedge in the Ti K-edge XANES spectrum, demonstrating that the Pt precursor underwent anchoring on the single framework Ti4+ center of Ti-HMS under UV-light irradiation. The lower intensity of the Pt-Pt bond of the PAD-Pt/Ti-HMS than that of Pt foil in the Fourier transforms of Pt L(III)-edge EXAFS spectra clearly suggests the formation of the Pt nanoparticles. The PAD-Pt/Ti-HMS exhibited the higher catalytic activity for the hydrogenation of nitrobenzene to aniline and the NO reduction than the catalyst prepared by the conventional impregnation.
Testing rurality as a moderator of associations between masculinity constructs, help‐seeking, and firearm storage practices

Abstract:Introduction Self‐reliance, emotional control, and honor ideology may prevent help‐seeking when suicidal ideation emerges. Furthermore, these factors are associated with an increased likelihood of firearm ownership and unsecure storage, which could facilitate suicide attempts. The rurality of residence may impact these associations, as rural regions report increased independence, honor culture, and firearm prevalence. Therefore, this study examined how emotional control, self‐reliance, and honor ideology relate to firearm storage and help‐seeking for suicidal ideation, with rurality moderating these associations. Methods Participants were 733 adults (63.6% female, 33.5% male, and 2.9% transgender/other) who reported past‐month suicidal ideation. Analyses tested associations between emotional control, self‐reliance, and honor ideology with help‐seeking for suicidal ideation and firearm storage, with rurality moderating these associations. Results The association of self‐reliance and decreased help‐seeking willingness for suicidal ideation was significant for those in non‐urban areas. The association of emotional control and decreased help‐seeking willingness for suicidal ideation was significant for those in urban areas. No variables predicted firearm storage. Conclusions Self‐reliance and emotional control could be barriers for help‐seeking regardless of gender identity that depend on rurality. Clinicians may target emotional control/self‐reliance via strengths‐based approaches with an emphasis on when these constructs could be maladaptive, as this could promote help‐seeking.

Research examining how perceived access to firearms is related to SI and SA among youth with marginalized identities is limited and often neglects to consider intersectionality. Abstract:Importance: Since 2004, the suicide rate among US Army soldiers has exceeded the rate of death from combat injury. It is critical to establish factors that increase the risk of acting on suicidal thoughts to guide early intervention and suicide prevention.Objective: To assess whether firearm ownership, use, storage practices, and accessibility are associated with increased risk of suicide.Design, Setting, and Participants: In this case-control study, suicide cases (n = 135) were defined as US Army soldiers who died by suicide while on active duty between August 1, 2011, and November 1, 2013. Next-of-kin and Army supervisors of soldiers who died by suicide (n = 168) were compared with propensity-matched controls (n = 137); those soldiers with a suicidal ideation in the past year (n = 118) provided structured interview data. Data were analyzed from April 5, 2018, to April 2, 2019.Main Outcomes and Measures: Firearm ownership, storage, and accessibility were assessed by using items from the World Health Organization Composite International Diagnostic Interview screening scales along with items created for the purpose of the Army Study to Assess Risk and Resilience in Servicemembers (Army STARRS) study.Results: Among the 135 suicide decedents, next-of-kin reported that they had greater accessibility to firearms compared with propensity-matched controls. Specifically, suicide decedents were more likely to own 1 or more handguns compared with propensity-matched controls (odds ratio [OR], 1.9; 95% CI, 1.0-3.7; χ21 = 4.2; false discovery rate [FDR] P = .08), store a loaded gun at home (OR, 4.1; 95% CI, 1.9-9.1; χ21 = 12.2; FDR P = .003), and publicly carry a gun when not required for military duty (OR, 3.2; 95% CI, 1.4-7.3; χ21 = 7.4; FDR P = .02). The combination of these 3 items was associated with a 3-fold increase in the odds of suicide death (OR, 3.4; 95% CI, 1.2-9.4; χ21 = 5.4; FDR P = .05). Storing a loaded gun with ammunition at home or publicly carrying a gun when not on duty was associated with a 4-fold increase in the odds of suicide death (OR, 3.9; 95% CI, 1.9-7.9; χ21 = 14.1; FDR P = .002).Conclusions and Relevance: In this study, in addition to gun ownership, ease and immediacy of firearm access were associated with increased suicide risk. Discussion with family members and supervisors about limiting firearm accessibility should be evaluated for potential intervention.

Abstract:Previous evidence has highlighted the potential roles of both physical and psychological distance to suicide methods as an important factor in conferring suicide risk; however, less is known about the temporal stability of and associations between these constructs, other facets of capability for suicide, and suicide-related outcomes. The present study examined fluctuations in and associations between physical and psychological distance to suicide methods, fearlessness about death, and suicidal intent using ecological momentary assessment. A sample of 237 adults at high risk for suicide (61.6% female, Mage = 27.12 years) responded to six prompts daily for 2 weeks assessing their physical and psychological distance to preferred suicide methods, fearlessness about death, and suicidal intent. Results indicated that physical and psychological distance to suicide methods exhibited both trait- and state-like properties, that lower physical and psychological distance and higher fearlessness about death were associated with higher concurrent suicidal intent, and that lower psychological distance was uniquely predictive of higher subsequent time-point suicidal intent, controlling for concurrent suicidal intent. Suicide attempt history and preferred suicide methods were explored as potential moderating factors. Overall, these findings highlight the importance of assessing and intervening upon psychological distance to suicide methods, in addition to physical proximity to these methods, in lethal means counseling. Abstract:Introduction One factor that can influence whether someone will engage in secure firearm storage is having a suicide exposure (SE). Daruwala et al. (2018) examined this and found that individuals with an SE, without considering perceived closeness, did not significantly differ from those who did not have an SE in their firearm storage practices. Thus, the present study aimed to replicate and extend the research of Daruwala et al. (2018) by examining if the method of suicide (by firearm or other means) and closeness of suicide decedent relate to secure firearm storage practices. Methods 308 male firearm owners completed self‐report measures assessing SE, perceived closeness to decedent, and current firearm storage practices. Chi square and logistic regression analyses were conducted. Results Suicide loss exposure, regardless of method used in suicide, did not relate to firearm storage practices in this sample. We also found that there was no association between perceived closeness to the decedent who died by suicide by firearm and secure storage, without considering age. Conclusions Results underscore the difficulty in helping male firearm owners develop personally salient reasons to increase safety with firearms. Implications for clinical care, firearm safety, and public health initiatives are explored.

Abstract:Decedents with no known mental disorder comprise 5–40% of suicides, suggesting that suicide ideation (SI) and behavior may occur in the psychiatrically healthy with important implications for suicide risk screening. Healthy Volunteers (HV) and patients with Major Depressive Disorder (MDD) provided 7 days of Ecological Momentary Assessment (EMA) data about SI and stressors. Longitudinal mixed effects logistic regression models compared HV and patient SI and stressors. Mixed effects linear regression models compared HVs' and patients' SI score change from the previous epoch's SI score when each stressor occurred. HVs ( n = 42) reported less frequent ( p < 0.001) and less intense SI ( p < 0.003) than patients ( n = 80), yet did endorse SI and/or SI-related items in 44% of EMA epochs, endorsing SI items in 25% of epochs with non-zero SI scores. For 7 of 8 stressors, patients reported stressors more often than HVs (all p < 0.001) responding to them with increased SI (0.0001 < p < 0.0472). HVs were relatively resilient to stressors, reporting SI increases only in response to neglect ( p < 0.0147). Although SI and SAs are documented among psychiatrically healthy individuals, scientific attention to these observations has been scant. Real-time SI measurement showed that HVs' SI was less pronounced than MDD patients', but was endorsed, nonetheless. Patients were more likely to report stressors than HVs, perhaps due to greater sensitivity to the environment, and reported SI in response to stressors, which was less common in HVs. Both MDD patients and HVs most often manifested passive SI (viz, "decreased wish to live"). However, passive SI (viz, "desire for death"), may predict suicide, even absent SI per se (thinking about killing yourself). This study validates the utility of real-time SI assessment, showing that HVs endorse SI items in 11% of epochs, which implies that suicide risk screening focused on those with mental disorders may be too narrow an approach.
