Abstract:Background: Diffuse uterine leiomyomatosis is a benign disease with a low incidence rate, mainly manifested as menorrhagia and infertility. Due to its clinical manifestations similar to multiple uterine myomas or adenomyosis, it is often misdiagnosed in clinical practice. Research has found that they can be distinguished through histopathological examination. According to existing reported cases, Diffuse uterine leiomyomatosis mainly occurs during the child-bearing period and has a significant impact on fertility. Hysterectomy is recognized as a radical cure, but uterine-sparing treatment methods need to be continuously explored. Case: A 41-year-old woman presented with recurrent menorrhagia 5 years after myomectomy. B-ultrasound examination indicated that the uterus was enlarged and the myometrium was full of hypoechoic, Considering the patient's desire to preserve the uterus, we performed laparotomy myomectomy again. Result: Hysteroscopic myomectomy has been shown to have a certain therapeutic effect on infertile patients in the early stage of DUL. Laparotomy myomectomy can remove a wider range of lesions and can be combined with gonadotropin releasing hormone agonists or antagonists for pretreatment. Uterine artery embolization is also a commonly administered therapy. High-intensity focused ultrasound ablation has been used with significant success in reducing uterine volume but requires further evaluation for its impact on fertility. Conclusion: A DUL patient underwent extensive myomectomy. This atudy further discussed uterine-sparing treatments, and reviewed existing research reports.
Management of Primary Female Urethral Adenocarcinoma: Two Rare Case Reports and Literature Review

Abstract:Primary urethral adenocarcinoma in females is an extremely rare malignancy with unclear origin and only a few retrospective cases have been reported. The controversy continues to exist over the origin of primary urethral adenocarcinoma from periurethral glands (which include the Skene’s glands), urethritis glandularis or intestinal metaplasia. Herein, we report one case of a 49-year-old female with distal urethral adenocarcinoma who presented with obstructive voiding. Abdominal and pelvic CT scans and chest radiology were unremarkable. Biopsy of the mass confirmed urethral adenocarcinoma. The patient underwent partial ureterectomy and was disease-free at the 2-years follow-up period. We also present another extremely rare case of primary urethral adenocarcinoma with mucinous features in a 58-year-old female who initially complained of external urethral orifice itching with painless urethral bleeding and was treated with local excision. The patient has not received any neoadjuvant or adjuvant therapy, and experienced tumor recurrence, inguinal lymph nodes metastasis, and even local iliopsoas metastasis during over 10-years follow-up. In conclusion, our current study emphasizes the importance of imaging studies and biopsy in making an accurate preoperative diagnosis of this rare disease, and further highlights the role of multimodal therapy. A combination of radiotherapy, chemotherapy and surgery is recommended for the optimal local and distant disease control. Moreover, better medical compliance and regular follow-up are required in these patients.

Abstract:Introduction and importance: Leiomyoma is a rare benign bladder tumor, classified into intravesical, intramural and extravesical types according to the location. Because of the difficulty of accurate preoperative diagnosis, resection is performed in the majority of the cases. Case presentation: A 37-year-old Japanese man presented to the hospital with a chief complaint of abdominal swelling. Abdominal computed tomography (CT) revealed a large solid mass (20 cm in size) from the abdominal wall to bladder. The tumor was successfully removed by a combination of laparoscopic and open surgery. The histological diagnosis was compatible with leiomyoma, and the patient remained free from recurrence at 3 years after surgery. Clinical discussion: The possibility of urachal carcinoma could not be ruled out preoperatively because of the location and internal heterogeneous findings by contrast CT. Although imaging is useful in the diagnosis of leiomyoma, the need for histological examination for a conclusive diagnosis has been noted. Therefore, surgical intervention is reported as a major treatment option. In the present case, laparoscopic approach was performed in accordance with partial cystectomy. The procedure was useful for observation of the positional relationship between the tumor and adjacent intestinal organs, and antegrade resection was performed without incident. Conclusion: Laparoscopic approach may be a useful and safe procedure for the resection of extravesical bladder leiomyoma.

Abstract:Abstract Background Most bladder tumors are derived from the urothelium. Benign mesenchymal tumors are rare. Leiomyoma of the bladder is the most common benign neoplasm. We present a case of leiomyoma of the bladder presenting with acute urinary retention in a female patient and report on the post-operative change in urodynamic findings. To our knowledge, few cases of this kind have been reported. Case Presentation A 56-year-old woman presented with acute urinary retention. Evaluations including ultrasound, magnetic resonance imaging, cystoscopy, and urodynamics contributed to a diagnosis of leiomyoma of the bladder. Various medications were ineffective for solving her lower urinary tract symptoms; therefore, a transurethral resection was performed. The final pathological report was leiomyoma. After the operation, her symptoms resolved; this improvement was confirmed by an urodynamic analysis. The postoperative urodynamics demonstrated a lower frequency of detrusor overactivity during filling cystometry and an increase in the uroflow rate, with reduced detrusor pressure in a pressure flow study. Conclusions Leiomyoma of the bladder can cause female outlet obstruction. A review of the literature and disease management is discussed.

Abstract:Leiomyoma is a rare benign tumour of the urinary bladder. Typically, bladder leiomyomas are treated with transurethral resection, which yields favourable results. We present a clinical case of a 29-year-old man with a symptomatic bladder tumour, initially diagnosed on flexible cystoscopy and CT scan. Subsequent transurethral resection and MRI scan confirmed a transmural bladder leiomyoma invading the urachal remnant. The patient was subsequently treated with robotic partial cystectomy. The presentation and management, including imaging and histopathology results, are discussed with a brief review of the literature.

Abstract:Introduction Radical cystectomy with dissection of pelvic lymph nodes and urethral diversion is the standard surgical treatment for muscle-invasive non-metastatic bladder cancer. In rare cases where patients with bladder cancer without distant metastasis have pelvic multi-organ invasion, the cancer compresses or invades the ureter and, in severe cases, leads to bilateral upper urinary tract obstruction and renal damage. The treatment recommended by guidelines often cannot improve the patients’ clinical symptoms immediately, and patients cannot complete the treatment owing to severe side effects, resulting in poor survival benefits. Case presentation A 69-year-old woman with facial edema was treated at the First Affiliated Hospital of Jinan University. The serum creatinine and potassium values were 1244 umol/L and 5.86 mmol/L, respectively. Pelvic magnetic resonance and abdominal computed tomography revealed that the bladder tumor had infiltrated the uterus, anterior vaginal wall, rectum, right ureter, right fallopian tube, and right ovary and metastasized to multiple pelvic lymph nodes. Tumor invasion of the right ureter resulted in severe hydronephrosis of the right kidney and loss of function and obstructive symptoms in the left kidney. Four days later, the patient’s creatinine level decreased to 98 u mol/L, the general condition significantly improved, and the patient and family members strongly desired surgical treatment of the tumor. Through a comprehensive preoperative discussion, possible intraoperative and postoperative complications were evaluated. Right nephrectomy, right ureterectomy, total pelvic organ resection, extended pelvic lymph node dissection, and bowel and urinary diversion were conducted under 3D laparoscopy-assisted treatment. The patient was followed-up for 1.5 years and showed good tumor control, self-care, and mental status. Conclusion Minimally invasive surgery is a curative option for patients with bladder cancer with pelvic multi-organ invasion without distant metastasis. Surgeons should strictly control the indications for surgery and warn patients about the occurrence of related post-surgical complications.

Abstract:The diagnostic complexities posed by lesions within the urinary bladder underscore the need for personalised management approaches. Endocervicosis, a rare condition stemming from Müllerian tissue, is characterised by the benign infiltration of endocervical glands, predominantly affecting the urinary bladder. Despite the absence of definitive symptoms, meticulous preoperative assessment is imperative to ensure precise diagnosis and optimal surgical intervention. While typically benign, recent cases have hinted at a potential association with adenocarcinoma, underscoring the necessity for meticulous management. The management of endocervicosis lacks consensus, with suggested surgical modalities including transurethral resection or partial cystectomy. In this case, a 47-year-old woman presented to our department with chronic pelvic pain following hysterectomy for adenomyosis. Imaging tests revealed a solid lesion situated at the dome of the urinary bladder. Consequently, a segmental cystectomy with circular tumour-only excision, with minimal free surgical margins, was undertaken to preserve bladder integrity. Histopathological analysis confirmed the diagnosis of endocervicosis. While the reported cases of urinary bladder endocervicosis remain limited, this instance contributes valuable insights into its understanding and management. It underscores the pivotal role of accurate diagnosis and tailored surgical intervention in optimising patient outcomes and mitigating postoperative complications, with tumour-only excision emerging as a promising and feasible approach.
A Role of Multimodality Bladder-Preserving Therapy in Patients with Muscle-Invasive Bladder Cancer Plus Hydronephrosis with or Without Pelvic Nodal Involvement.

Abstract:BACKGROUND/PURPOSE:To retrospectively evaluate the failure patterns of multimodality bladder-preserving therapy in patients with muscle-invasive bladder cancer.METHODS:Patients with muscle-invasive bladder cancer underwent maximal transurethral resection of bladder tumor and induction chemotherapy, followed by concurrent chemoradiotherapy (CCRT). Radiotherapy was given with 45 Gy to the pelvis, 50.4 Gy to the bladder, and 64.8 Gy to the tumor bed. Three protocols of trimodality treatment were used: Protocol A, three cycles of cisplatin and fluorouracil (CF), followed by CCRT with 6 weekly cisplatin; Protocol B, three cycles of weekly paclitaxel plus CF, followed by CCRT with 6 weekly paclitaxel and cisplatin; Protocol C, three cycles of gemcitabine and cisplatin, followed by CCRT with 6 weekly cisplatin. Interval cystoscopy confirmed complete response (CR) after induction chemotherapy and 40-50 Gy of radiotherapy. Patients without CR were referred for salvage cystectomy.RESULTS:A total of 60 patients were enrolled, including 11 patients with unfavorable factors defined as hydronephrosis and/or pelvic nodal involvement. After a median follow-up of 86.7 months, the 5-year overall, progression-free, and bladder preservation-specific survival rates were 76.3%, 62.9%, and 71.5%, respectively. Three patients underwent salvage cystectomy for invasive bladder recurrence. Of 45 surviving patients, 42 patients (93.3%) retained functioning bladders. Patients with unfavorable factors had significantly lower metastasis-free survival (p=0.002), but not bladder preservation-specific survival (p=0.25).CONCLUSION:With trimodality treatment involving visually complete transurethral resection of bladder tumor, cisplatin-based induction chemotherapy, and CCRT, patients with unfavorable factors maintained satisfactory bladder preservation but not systemic control.

Abstract:Objectives This study aims to provide an overview of the diagnosis, treatment, and follow-up management of vulvar and vaginal leiomyomas through the presentation of two rare cases. Methods Detailed clinical presentations, surgical procedures, histopathological examinations, and follow-up outcomes of two cases of vulvar and vaginal leiomyomas are described. Relevant literature is also reviewed to contextualize the findings. Results Both patients underwent successful surgical excision of the leiomyomas with no perioperative or postoperative complications. Histopathological examinations confirmed the diagnosis of leiomyoma based on characteristic microscopic features and immunohistochemical analyses. Conclusion Vulvar and vaginal leiomyomas are rare benign tumors that require careful evaluation for accurate diagnosis and appropriate management. Surgical excision remains the primary treatment modality, and long-term follow-up is essential for monitoring recurrence and ensuring favorable outcomes.

Abstract:Background: Inflammatory myofibroblastic tumor (IMT) is a rare neoplasm with malignant potential. Bladder IMT is even rarer and mainly treated by surgical resection However, partial or radical cystectomy would affect the quality of life of patients due to major surgical trauma, and classical TURBT is hard to avoid intraoperative complications including obturator nerve reflex and bleeding etc. Therefore, the safe and effective better choice of surgical approaches become critical to bladder IMT. Case presentation: A 42-year-old male patient was admitted to the department of urology with persistent painless gross hematuria for more than 10 days without the presentation of hypertension. Preoperative routine urine examination of red blood cells was 7738.9/HPF (normal range ≤ 3/HPF). CTU indicated a space occupying lesion (6.0 cm×5.0 cm) in the left posterior wall of the bladder with heterogeneous enhancement in the excretory phase. MRI also indicated bladder tumor with slightly equal SI on T1WI and mixed high SI on T2WI (6.0 cm×5.1cm×3.5cm) in the left posterior wall of the bladder. En bloc resection of bladder IMT with 1470 nm diode laser in combination of removing the enucleated tumor by the morcellator system was performed. Postoperative pathological examination revealed bladder IMT, with IHC positive for Ki-67 (15-20%), CK AE1/AE3, SMA, and Desmin of bladder IMT and negative for ALK of bladder IMT as well as FISH negative for ALK gene rearrangement. Second TUR with 1470 nm diode laser was performed within 6 weeks to reduce postoperative risk of recurrence due to highly malignant potential for the high expression of Ki-67 (15-20%) and negative ALK in IHC staining. The second postoperative pathology report showed chronic inflammation concomitant with edema of the bladder mucosa without bladder IMT, furthermore no tumor was observed in muscularis propria layer of bladder. No recurrence occurred during the period of 24-month follow-up. Conclusion: En bloc resection of bladder IMT in combination of the following second transurethral resection with 1470 nm diode laser is a safe and effective surgical approach for the huge bladder IMT with highly malignant potential.

Abstract:There has recently been a major change in the design of aircraft. Electrical systems are being used in applications, which have traditionally been powered by hydraulic or pneumatic sources. The Boeing 787 and the Airbus A380 both have significantly larger electrical systems than any previous aircraft and this has led to a wealth of technology developments. Electrical systems are now being use for aircraft actuation systems, wing ice protection, environmental control systems and fuel pumping. These new systems are helping to make future aircraft more fuel efficient and quieter, improving the environment for everyone. However, this is just the start of the changes to aircraft design. A roadmap leading to a fully electric aircraft for civil applications is now emerging. The technology challenges involved for power electronics, electrical systems, control and electrical machines are being defined and these requirements give a view of the fantastic opportunities for research teams to make a real impact. The issues to be resolved include equipment weight, volume, cost and reliability. The More Electric Aircraft concept offers many potential benefits in the design and efficiency of future large, manned aircraft. In the future it is predicted that the trend in transportation electrification will lead to the development of all electric civilian aircraft concepts. In this paper typical aircraft electrical power systems and the loads associated with these requirements are described as well as the challenges for the aerospace industry in this implementation. The importance of Power Electronics as an enabling technology for this step change in aircraft design is considered and examples of typical system designs are discussed.
Leiomyoma of the prostate: A case report and literature review

Abstract:Leiomyomas are benign tumors that can develop in any organ, with the uterus being the most common site. The prevalence of leiomyomas in women can reach as high as 70% to 80% by the age of 50, with the highest incidence among women of African descent. [ 1 ] Leiomyomas originating in the bladder are rare, representing only 0.05% of all bladder tumors, with approximately 250 cases reported globally, affecting men and women equally. Even fewer leiomyomas originate in the prostate, with fewer than 100 documented cases worldwide to date. Most leiomyomas are diagnosed postoperatively, though they may also present with symptoms of local irritation, such as urinary retention, irritative urethral symptoms, and hematuria. The prognosis for leiomyomas is generally positive, with a low recurrence rate. However, accurately assessing the tumor's origin, size, and boundaries via ultrasound, CT, or MRI is often challenging. Moreover, no clinical guidelines have been established for diagnosing and managing this condition or for distinguishing it from malignant bladder tumors. In this report, we describe a case of prostate leiomyoma in a 51-year-old male patient. Additionally, we review the available literature on prostate leiomyomas, covering their epidemiology, etiology, clinical presentation, imaging characteristics, and the diagnostic and therapeutic approaches used in the differential diagnosis and treatment of this condition in clinical practice.

Abstract:Inflammatory myofibroblastic tumors (IMTs) are particularly rare tumors that have been described in various anatomic locations, of which the urinary bladder is the most common. These benign tumors are amendable to conservative therapy but are notoriously difficult to diagnose given their mimicry of malignant sarcomas and sarcomatoid carcinomas, making an accurate diagnosis paramount to spare a patient radical and unnecessary treatment. We hereby present the case of a 37-year-old female patient who was diagnosed with an IMT of the urinary bladder during workup for painless gross hematuria. Patient was successfully managed with a laparoscopic partial cystectomy and is free of recurrence 5 years after surgery. IMTs are rare benign tumors that share the same clinical presentation as malignant bladder tumors. Deep biopsy and experienced pathologist are crucial in establishing diagnosis and avoiding patient radical treatment. This case is a classical demonstration of a remarkably rare tumor that was adequately managed with conservative therapy, achieving excellent clinical outcomes.
[Comparative toxicity of different cereals for subjects intolerant of gluten].

Abstract:Coeliac disease is caused by prolamines, the storage proteins of some cereals, located in the endosperm. Cereals do not all have the same toxicity. The four wheat prolamine groups (alpha, beta, gamma and omega gliadins), visible in electrophoresis at acid pH, have been isolated and their toxicities compared by observing the morphological changes in intestinal biopsies cultured in vitro when peptic-tryptic digests of the studied proteins were added to the culture medium. The toxicity was found to be mainly located in the alpha and beta-gliadins and in peptides of 5 to 10 000 molecular weight. Peptides, resulting from peptic-tryptic hydrolysis, varied in length as a direct function of their proline content. In fact, peptide bond splitting by pepsin and trypsin is known to be blocked by proline. Thus, proline content determines peptide length and toxicity. Wheat, rye and barley toxicities were compared on the basis of a correlation between toxicity and the alpha- and beta-gliadin-like prolamine contents of these cereals. Electrophoretic estimation of alpha- and beta-gliadin-like prolamine content gave the following prediction of relative toxicity (in decreasing order): wheat, triticale, rye, barley and oats.

Abstract:Purpose To share our experience in the diagnosis and treatment of an inflammatory myofibroblastic tumor of the urinary bladder (IMTUB). Materials and Methods A database searches in the pathology archives by using the term “inflammatory myofibroblastic tumor” and” bladder” in our hospital department of pathology from 2010 to 2021. Patient characteristics, clinical features, histopathological results, immunohistochemical staining results, and treatment outcomes were reviewed. Results Fourteen cases of IMTUB were retrieved. The mean age was 44.7 ± 18.9 years (range 12–74). Nine (64.3%) of the patients presented with hematuria, followed by seven (50%) with odynuria, five (35.7%) with urgent urination, and one (7.1%) with dysuria. Ten (71.4%) of the patients were treated with partial cystectomy (PC), three (21.4%) with transurethral resection of bladder tumor (TURBT), and one (7.1%) with radical cystectomy (RC). Histopathologically, eight (57.1%) had a compact spindle cell pattern. Anaplastic lymphoma kinase (ALK) staining was positive in six (75%) of 8 cases. During a mean follow-up period of 43.9 ± 38 months (range 3–117), a patient had recurrence within half a month. Then, the patient was treated with further TURBT surgery and had no recurrence within 6 months. Thirteen of the patients had no local recurrence or distant metastasis. Conclusion Inflammatory myofibroblastic tumor of the urinary bladder (IMTUB) is clinically rare and has a good prognosis. The disease is mainly treated with surgery to remove the tumor completely. It can easily be misdiagnosed as bladder urothelial carcinoma, leiomyosarcoma, or rhabdomyosarcoma, which may result in overtreatment and poor quality of life of patients.

Abstract:Uterine fibroids, or leiomyomas, are common benign tumors of the uterus, generally asymptomatic but potentially causing severe symptoms and complications in some cases, as demonstrated in this report. This case presents significant management challenges due to the fibroids' size, number, and location, including an unusual complication involving adhesion to the ileum. A 40-year-old female with a history of P2L1D1 and no significant comorbidities presented with three months of progressive abdominal pain and a rapidly enlarging mass resembling a 30- to 32-week gravid uterus and heavy menstrual bleeding. Clinical findings included severe anemia with a hemoglobin level of 5.5 g/dL. Imaging studies revealed a bulky uterus with numerous multilobulated, well-defined, solid, hypoechoic fibroids subserosally and intramurally, raising suspicions of sarcomatous conversion. The patient underwent a laparotomy, which involved the resection of multiple large subserosal fibroids and a total abdominal hysterectomy, necessitated by extensive uterine distortion and the patient's preference against fertility preservation. A significant intraoperative discovery was the adhesion of fibroids to the ileum, which required bowel resection and anastomosis. This case emphasizes the complexity of managing extensive uterine fibroids, highlighting the need for thorough preoperative assessment, preparation for potential intraoperative complications, and the importance of a multidisciplinary surgical approach. The successful management and uneventful recovery underscore the effectiveness of proactive and comprehensive surgical intervention in cases with significant fibroid burden and associated anatomical challenges.
Organ-Sparing Strategies in Muscle-Invasive Bladder Cancer

Abstract:Radical cystectomy (RC) is the treatment of choice and is strongly recommended for patients with pT2-4aN0M0 bladder cancer in both the European Association of Urology (EAU) and American Urological Association (AUA) muscle-invasive bladder cancer (MIBC) guidelines. RC is a challenging operation, with significant perioperative and postoperative morbidity and mortality, having short-term complication rates between 14.4% and 21.7%, and long-term oncological survival rates ranging from 60% after 5 years to 43% after 10 years. The impact on quality of life (QoL) in patients after treatment for bladder cancer is significantly worse than in other pelvic cancers. Although RC is strongly recommended as the gold standard, there is a need for bladder-sparing options in MIBC. Attempts to improve mortality and morbidity rates after RC have been made by implementing Enhanced Recovery After Surgery (ERAS), robot-assisted RC, and sexual function-preserving techniques. None of these significantly improves QoL or functional outcome. Because of the invasiveness of RC, other therapeutic options have been evaluated. Transurethral resection of the bladder tumor (TURB) plays an important role in the diagnostic evaluation of MIBC and has also been reviewed as a curative option, although the oncological results appear inferior to RC. Furthermore, improved radiotherapy (RT) and widely used chemotherapy, both as monotherapeutic options in bladder cancer, are not as effective as radical surgery, with lower survival rates. Trimodality treatment (TMT) in bladder cancer combines TURB with chemotherapy and RT. The goal of TMT is preserving the bladder and QoL without compromising oncological outcome. A 2018 review showed no difference in overall survival rates between RC and TMT (30.9% vs 35.1%), with lower survival rates after RC than TMT in the first year of follow-up, probably due to higher postoperative mortality. For a selected group of patients, TMT is to be recommended, and it is the most favorable option out of the organ-sparing strategies in MIBC.
