Abstract:Parenting is a process of interaction and mutual development ( Frosch et al. 2019 ). While parental involvement is essential for the development and overall well-being of their child ( Olson & Masur 2015 ; Peacock-Chambers et al. 2017 ; Curtin et al. 2021 ), the parenting experience also has an emotional effect on parents ( Frosch et al. 2019 ). Parents caring for children with disabilities have unique parenting experiences, filled with both victories and challenges. Parents have described the positive aspects of caring for children with disabilities including personal strength and development, more positive outlook on life, expanded communities with others who have similar experiences, and a source of happiness and fulfillment upon caring for their child ( Beighton & Wills 2019 ). However, parenting a child with disabilities can bring additional responsibilities as parents work to respond to their children's special needs ( Rani & Thomas 2019 ).

Abstract:Individuals who are d/Deaf or hard of hearing (d/DHH) may face maltreatment due to being d/DHH or exposure to negative attitudes toward hearing loss or being d/DHH, with variations across cultural, contextual, and socioeconomic settings ( Stockton et al. this issue pp. 4S–16S ). For example, individuals may experience interpersonal acts of discrimination perpetuated (or enacted) by others that result in unfair, negative treatment due to being d/DHH, which is known as experienced stigma ( Boyle 2018 ). According to evidence from qualitative studies conducted mostly in high-income Western societies, experienced stigma toward people who are d/DHH may range from being pitied, mocked, assumed to be of lower intelligence or ability, or socially isolated ( Wallhagen 2010 ; Barker et al. 2017 ; Ruusuvuori et al. 2021 ; Lash & Helme 2020 ; Mukuna & Maizere 2022 ). Further, individuals who are d/DHH develop their own understanding of the prevalence and degree of others' negative, stigmatizing attitudes toward hearing loss or being d/DHH, which constitutes perceived stigma ( DeFreitas et al. 2018 ; Čolić & Milačić-Vidojević 2021 ). Finally, individuals may internalize these experiences and perceptions and accept such negative treatment and attitudes as true and just, which constitutes internalized stigma ( Nyblade et al. 2021 ). For example, individuals who are d/DHH may report feeling shame or that they are "not as good as others" because they are d/DHH ( Mousley & Chaudoir 2018 ).

Abstract:This special supplement is a product of the Lancet Commission on Hearing Loss (LCHL), a multinational effort to map a new approach to reducing the global burden of disability associated with hearing loss ( Wilson et al. 2019 ). During early meetings of the LCHL, commissioners identified stigma as a cross-cutting issue that may affect the LCHL initiatives designed to mitigate the negative effect of hearing loss in both high-income and low- and middle-income (LMIC) settings. As such, the LCHL measures, models, and stigma-reduction subgroup (henceforth, the stigma subgroup) was convened; all members are coauthors of this article. The purpose of the stigma subgroup was to advance our understanding of hearing loss stigma and actions to address it. Learning from stigma research across multiple stigmatized health conditions, the stigma subgroup identified the lack of a comprehensive set of measures to capture the different types of stigma faced by diverse groups of d/Deaf and hard of hearing (d/DHH) populations. After much consultation with the stigma subgroup and review of current literature, we chose to use the term d/DHH here to encompass this diverse group, given the term has been used to inclusively describe deaf individuals who are culturally Deaf, deaf individuals who do not identify with the Deaf community, as well as others with a range hearing difficulties ( Linton 1998 ; Brueggemann 2009 ; McIlroy & Storbeck 2011 ; Canadian Association of the Deaf 2015 ; National Association of the Deaf 2024 ). Measures would allow future research to characterize the nature and magnitude of stigma; understand the relations between different types of stigma, health, and other life outcomes; and design and evaluate targeted stigma-reduction interventions. Thus, we developed and preliminarily validated survey measures of different types of stigma related to deafness and hearing loss in the United States (U.S., a high-income country) and Ghana (an LMIC), the results of which are presented in this special supplement of Ear and Hearing .

Abstract:Valid survey measures provide important evidence for program designers, advocates, and policymakers. They allow for the quantification of ideas, the calculation of their relationships with other outcomes, and the development of comparisons across geographies and populations as well as over time. Such measures, however, require the use of simple numeric values to describe complex phenomena. One such complex phenomenon is stigma, a relational phenomenon that occurs within the existing power structures of a society when a person possesses, or is thought to possess, attributes that others consider to be of little value, abnormal, or undesirable in a particular social context ( Becker 1981 ; Crocker et al. 1998 ; Nyblade et al. 2019 ). The intensely personal experience of stigma, combined with the complexity of the social structures that shape it, can create the impression that we cannot measure stigma. In reality, however, a systematic review of stigma measurement studies has found that stigma manifests in remarkably consistent ways across cultural settings, public health fields, and even health conditions ( Van Brakel 2006 ). Consistent measures for stigma, especially those that disaggregate different kinds of stigma, provide an important complement to qualitative methods and allow for a more comprehensive understanding of how stigma affects people's lives. In turn, this understanding can inform the development of specific interventions that account for different types of stigma, including interventions that directly address the stigma associated with a given health condition. In a systematic review of the stigma literature, evidence from health fields as disparate as human immunodeficiency virus (HIV), leprosy, tuberculosis, and mental health shows that interventions and strategies that explicitly account for stigma can meaningfully improve the quality of life of those affected by these conditions ( Heijnders & Van Der Meij 2006 ).

Abstract:The population of older adults continues to increase in the United States (U.S.), and many of these older adults are aging at home with physical, mental, or cognitive disabilities that require care provision from other individuals ( Roth et al. 2015 ; Administration on Aging 2021 ). According to recent estimates, more than 1 in 5 adults (21.3% or 53.0 million) in the U.S. provide unpaid support to persons who require varying forms of assistance with their social, functional, or health needs, usually related to a disability or illness ( Center for Disease Control and Prevention 2019 ; American Association of Retired Persons 2020 ). The estimated economic value of this unpaid care in the U.S. was approximately $470 billion in 2017 ( Reinhard et al. 2019 ), documenting its economic significance.

Abstract:Background: With few psychometrically evaluated HIV related stigma measures for adolescents and young adults (AYA) living with HIV, we examined the developmental applicability (i.e., validity) of two subscales of the commonly used stigma measure, the Social Impact Scale (SIS), among a cohort of adolescents and young adults with perinatally-acquired HIV. Setting: Data came from a New York City longitudinal study (N=340). This study primarily comprises Black and Latinx AYA with either perinatally-acquired HIV (PHIV) or those with perinatal exposure but who are uninfected. Data for this analysis came from the PHIV population and spanned approximately a 15-year survey period (2003-2018). Methods: A confirmatory factor analysis was used at seven time points to assess if the Social Rejection and Internalized Shame subscales were consistent in this cohort over time. Overall and individual Cronbach’s alphas were reported to show the strength of the internal consistency. Results: Mean age from Baseline to Follow-Up 6 ranged from 12-23 years over the study period. The Social Rejection subscale was acceptably valid across follow-up periods with strong factor loadings and Cronbach’s alphas higher than 0.70. However, the Internalized Shame subscale was less valid among younger adolescents. Starting at Follow-Up 2, we observed better validity with the Internalized Shame subscale performance. Conclusion: Future research must consider mechanisms for developing and adapting measures from a developmental perspective to best measure the experiences of HIV-related stigma among younger populations.

Abstract:AbstractSignificant numbers of children have diagnosable mental health problems, but only a small proportion of them receive appropriate services. Stigma has been associated with help-seeking for adult mental health problems and for Caucasian parents. The current study aims to understand factors, including stigma, associated with African American parents’ help-seeking behavior related to perceived child behavior problems. Participants were a community sample of African American parents and/or legal guardians of children ages 3–8 years recruited from an urban primary care setting (N = 101). Variables included child behavior, stigma (self, friends/family, and public), object of stigma (parent or child), obstacles for engagement, intention to attend parenting classes, and demographics. Self-stigma was the strongest predictor of help-seeking among African American parents. The impact of self-stigma on parents’ ratings of the likelihood of attending parenting classes increased when parents considered a situation in which their child’s behavior was concerning to them. Findings support the need to consider parent stigma in the design of care models to ensure that children receive needed preventative and treatment services for behavioral/mental health problems in African American families.

Abstract:Patients who are d/Deaf and hard of hearing (d/DHH) report lower-quality health care experiences, less than favorable communication with health care providers (HCPs) ( Mick et al. 2014 ), and numerous barriers to engaging with the health care system ( Schniedewind et al. 2021 ). Stigma may be one factor contributing to adverse interactions and resistance to engaging with the health care system for patients who are d/DHH. Social stigma related to hearing loss and hearing aids are commonly cited reasons why patients with acquired hearing loss deny their hearing status or reject hearing care ( Rawool 2018 ). If patients who are d/DHH experience instances in which they perceive, anticipate, or experience stigma because of a hearing loss, then such interactions may exacerbate their perceived social stigma of hearing loss and further contribute to their reluctance to pursue hearing health care. In addition, stigma enacted in health care settings erodes patient trust in HCPs ( Gudzune et al. 2014 ), precludes patient-centered communication ( Phelan et al. 2023 ), and deters patients from engaging in health care services and accepting treatments that could improve their quality of life ( Knaak et al. 2017 ; Nyblade et al. 2019 ). Stigma against people who are d/DHH could manifest through explicit forms, such as outright discrimination (e.g., denying of care [ Schniedewind et al. 2021 ]), or in more subtle behaviors borne out of a provider's implicit or unconscious biases against patients with disabilities ( Lagu et al. 2022 ).

'They Say HIV is a Punishment from God or from Ancestors': Cross-Cultural Adaptation and Psychometric Assessment of an HIV Stigma Scale for South African Adolescents Living with HIV (ALHIV-SS)

Abstract:Sub-Saharan Africa is home to 90 % of the world's adolescents living with HIV (ALHIV). HIV-stigma and the resultant fear of being identified as HIV-positive can compromise the survival of these youth by undermining anti-retroviral treatment initiation and adherence. To date, no HIV-stigma measures have been validated for use with ALHIV in Sub-Saharan Africa. This paper reports on a two-stage study in the Eastern Cape, South Africa. Firstly, we conducted a cross-cultural adaptation of an HIV stigma scale, previously used with US ALHIV. One-on-one semi-structured cognitive interviews were conducted with 9 urban and rural ALHIV. Three main themes emerged: 1) participants spoke about experiences of HIV stigma specific to a Southern African context, such as anticipating stigma from community members due to 'punishment from God or ancestors'; 2) participants' responses uncovered discrepancies between what the items intended to capture and how they understood them and 3) participants' interpretation of wording uncovered redundant items. Items were revised or removed in consultation with participants. Secondly, we psychometrically assessed and validated this adapted ALHIV stigma scale (ALHIV-SS). We used total population sampling in 53 public healthcare facilities with community tracing. 721 ALHIV who were fully aware of their status were identified and interviewed for the psychometric assessment. Confirmatory factor analysis confirmed a 3-factor structure of enacted, anticipated and internalized stigma. The removal of 3 items resulted in a significant improvement in model fit (Chi2(df) = 189.83 (33), p < .001) and the restricted model fitted the data well (RMSEA = .017; CFI/TLI = .985/.980; SRMR = .032). Standardized factor loadings of indicators onto the latent variable were acceptable for all three measures (.41-.96). Concurrent criterion validity confirmed hypothesized relationships. Enacted stigma was associated with higher AIDS symptomatology (r = .146, p < .01) and depression (r = .092, p < .01). Internalized stigma was correlated with higher depression (r = .340, p < .01), higher AIDS symptomatology (r = .228, p < .01) and low social support (r = -.265, p < .01). Anticipated stigma was associated with higher depression (r = .203, p < .01) and lower social support (r = -.142, p < .01). The resulting ALHIV-SS has 10 items capturing all three HIV stigma mechanisms experienced by ALHIV. ALHIV-SS will be valuable for evaluating rates and types of stigma, as well as effectiveness of stigma-reduction interventions among ALHIV in Southern Africa.

Abstract:Aim: Although there are many scales that measure stigma, there is no scale with the necessary adequacy to measure stigma in the perinatal period. The study aims to develop the stigma scale for women with mental illness in the perinatal period and test its validity and reliability. Materials and methods: Participants were reached via patients, visitors, and hospital staff who applied to Sakarya Training and Research Hospital between 01/06/2022 and 01/12/2022. Two hundred people (female n = 134, male n = 66) aged 18-65 participated in the study and "Sociodemographic data form," "Perinatal Mental Illness Stigma Scale (PMISS)," "Social Distance Scale," and "Beliefs Towards Mental Illness Scale" were used to collect data. Data were analyzed using SPSS 22 and the AMOS 26 program. Results: The Content Validity Index of the scale items was between 0.80-1. Cronbach's alpha coefficient score of the general scale was 0.94, the "Discrimination and Prejudice" sub-dimension was 0.93, and the "Labeling" sub-dimension was 0.88. It was determined that item-total score correlations varied between 0.410 and 0.799. P value calculated < 0.05 in Barlett's test and 0.94 in the Kaiser-Meyer Olkin test. These values show that factor analysis can be applied to the scale. According to the Exploratory Factor Analysis result, the scale has a 2-factor structure, explaining 60% of the total variance. The Guttman Split-Half coefficient of the scale was 0.882, and the Spearman-Brown coefficient was 0.883. The scale was reapplied to 30 participants with an interval of three weeks. The correlation coefficient between the two measurements was 0.91, indicating that the scale satisfies the invariance principle over time. Conclusion: The PMISS is a reliable measurement tool that can be used to investigate stigma towards mental illness during the perinatal period in the Turkish population.

Abstract:Limited research has been done to explore parental perception of stigma. The present study developed the Parental Perceptions of Public Attitudes Scale to examine perceived stigma among Serbian parents of children with autism spectrum disorder (ASD) and children with physical disabilities (PD). The convenience sample consisted of 82 participants. A confirmatory factor analysis was done to validate the scale and a 9-item solution clustered into two factors proved to be the best model. Overall, the parents reported a low-to-moderate level of perceived stigma. Parents of children with ASD reported higher degree of perceived stigma compared to the parents of children with PD. The scale was useful in measuring parental perceived stigma and could be used in other Eastern European countries.

Abstract:Background: Parents' attitudes and beliefs in vaccination are important to understand for shaping vaccine acceptance and demand interventions. Little research has focused on developing a validated scale to measure parents' attitudes towards vaccinations in low and middle-income countries; Ghana provided an opportunity develop a caregiver vaccination attitudes scale (CVAS) validated against childhood vaccine compliance. Methods: We conducted a cluster survey of 373 households with children aged 12-35 months of age from Northern Region, Ghana. Caregivers responded to 22 vaccination behavior and belief survey items and provided the child's vaccination status. In exploratory factor analysis (EFA) to assess CVAS content validity, we used parallel analysis to guide the number of factors to extract and principal axis factor analysis for factor extraction. Reliability of the scale was assessed using McDonald's Omega coefficient. Criterion validity of scale and subscales was assessed against receipt of vaccinations by 12 months of age and vaccination delay, using number of days undervaccinated. Results: EFA of CVAS responses resulted in removing 11 of 22 survey items due to loadings <0.30 and development of a 5-factor structure with subscales for Vaccine-Preventable Disease (VPD) Awareness, Vaccine Benefits, Past Behavior, Vaccine Efficacy and Safety, and Trust. The 5 factors accounted for 69% of the common variance and omega coefficients were >0.73 for all subscales. Validity analysis indicated that for every unit increase in the parent's scale score, the odds of the child being vaccinated decreased by 0.58 (95% confidence interval [CI]: 0.37, 0.68) and the number of days under-vaccinated increased by 86 (95%CI: 28, 143). The final 3-factor scale included Vaccine Benefits, Past Behavior, and Vaccine Efficacy and Safety. Discussion: The final CVAS included three factors associated with vaccine compliance in Ghana, although several survey items suggested for use in vaccine acceptance scales were dropped. Replicating this study in several country settings will provide additional evidence to assist in refining a tool for use in routine vaccine acceptance and demand surveillance efforts.

Abstract:Parents of sexual and gender minority (SGM) youth play an important role in supporting their SGM child's mental health in the face of stigma. Yet, parents of SGM youth may themselves experience stigma, including discrimination/rejection, and its emotional consequences, including vicarious stigma and shame. The present cross-sectional study leveraged a national sample of parents of SGM youth to investigate associations between parents' stigma experiences and self-reported anxiety and depression symptoms. Further, we additionally explored sociodemographic and contextual correlates of parents' stigma experiences. Participants included 264 parents ( M age = 46) who reported having at least one SGM child under age 30 ( M age = 18). The Lesbian, Gay, Bisexual-Affiliate Stigma Measure (LGB-ASM) assessed parents' experiences of discrimination/rejection (e.g., actual and anticipated rejection experiences due to having an SGM child), vicarious stigma (e.g., worry and concern for one's SGM child), and shame (e.g., feeling embarrassed for having an SGM child). Parents indicated their anxiety and depressive symptoms using respective Patient-Reported Outcomes Measurement Information System—short forms. Results showed that vicarious stigma and shame, but not discrimination/rejection, were uniquely associated with parents' increased symptoms of anxiety (vicarious stigma: β = 1.59, p < .001; shame: β = 2.15, p < .001) and depression (vicarious stigma: β = 0.90, p < .01; shame: β = 2.77, p < .001). Further, parents with more accepting religious, racial, ethnic, and/or cultural communities reported lower stigma experiences. This study advances understanding of how the psychological consequences of stigma extend beyond SGM people themselves and contribute to mental health difficulties in parents of SGM youth. Abstract:Stigma has grave consequences for persons living with HIV/AIDS. Stigma hampers prevention of HIV transmission to sexual partners and to unborn babies, diagnosis, and early treatment, and negatively affects mental and physical health, quality of life, and life satisfaction. Internalized stigma of HIV/AIDS may have even more severe consequences than perceived or enacted stigma. The purpose of this study was to develop an instrument to measure internalized stigma in those with HIV/AIDS. Data were drawn from the Rural Women's Health Project. Research assistants administered structured interviews at baseline, 3 months, and 6 months. Instruments used in these analyses included a demographic data form, the Centers for Epidemiological Studies Depression Scale (CES-D), the Perceived Stigma Scale (PSS), and the Internalized Stigma of AIDS Tool (ISAT). Exploratory factor analysis confirmed that the ten items of the ISAT measure a single factor that explains 88% of the variance in the construct. Internal consistency was demonstrated by a Cronbach's alpha of .91 (Time 1), .92 (Time 2), and .92 (Time 3). Convergent validity was supported with significant positive correlations with the CES-D (rho = 0.33, p < 0.0001) and the PSS (rho = 0.56, < 0.0001). The Internalized Stigma of AIDS Tool appears to be a reliable and valid instrument to measure internalization of the stigma of HIV/AIDS. It may be of value in research and clinical assessment.
The Role of Stigma in Parental Help-Seeking for Child Behavior Problems

Abstract:The present study examined the relationship between stigma and parental help-seeking after controlling for demographics, child behavior, and barriers to treatment. One hundred fifteen parents of children ages 4 to 8 years were surveyed during well-child visits in a rural pediatric primary care practice. Parental perceptions of stigma toward parents and children were both assessed. Parents believe that children are more likely to be stigmatized by the public and personally impacted by stigma. In linear regression analyses, parents rated themselves as more likely to attend parenting classes with lower levels of self-stigma and greater levels of personal impact of stigma. Stigma toward the child was not associated with help-seeking. Child behavior moderated the relationship between stigma and parental help-seeking. When referring parents to treatment, providers should address potential stigma concerns. Future research should assess both the impact of the stigma of attending treatment and the stigma of having a child with behavior problems.

Abstract:This article describes the development and initial psychometric testing of the Mental Health Provider Self-Assessment of Stigma Scale (MHPSASS), a 20-item instrument crafted in reflection of Charles’ (Social Work in Mental Health 11:360–375, 2013) empirically derived, experience-based, five-themed model of provider stigmatization. Following model and item review by construct experts, 220 mental health service providers in Virginia’s public mental health centers and in-patient facilities completed the survey package. Results indicate the refined MHPSASS is a reliable measure of provider-based stigma with promising face and content validity. However, rather than they hypothesized five-factors, analysis indicates a four-factor solution, a key finding signaling a discrepancy between what providers endorse and what clients’ experience. Notably absent from the MHPSASS’ were items related to blame and shame, in contrast to the experience of clients and families. Further refinement is indicated, particularly reconsideration of blame and shame items due to their practical and theoretical significance.

Abstract:Scandinavian Journal of Public Health, Ahead of Print. Aims:The Red Noses Culturally-Sensitive Stigma Survey (RN-CSS) contributes to the underexplored research domain of adolescents' stigmatising attitudes and behaviours towards peers with mental health difficulties and mental healthcare services. It also addresses the need for comprehensive and culturally-sensitive tools to assess stigma in this context.Methods:Drawing on insights from focus groups and building upon the existing Stigma in Global Context-Mental Health Study, we have successfully developed and implemented the first culturally-sensitive stigma survey tailored for school-aged adolescents of different migration/cultural backgrounds. The questionnaire includes an unlabelled case vignette depicting a peer with symptoms of depression and gathers data on various domains, including (1) sociodemographic variables; (2) education-related information; (3) COVID-19; (4) perceptions of mental health difficulties and mental healthcare services (i.e. severity assessment, causal attributions, care recommendations, personal stigma, perceived stigma, and service stigma); (5) subjective wellbeing and familiarity with mental health difficulties; (6) social support; (7) school context; (8) bullying; and (9) knowledge of anti-stigma campaigns.Results:Our final sample comprises 5075 pupils from 38 secondary schools in Flanders, Belgium.Conclusions:In this article, we present the study's background and rationale, the development of the questionnaire, and the sampling and recruitment methods employed. Furthermore, we provide a summary of the sample characteristics and preliminary descriptive results of the RN-CSS. Subsequent empirical studies will address the research objectives outlined in this protocol paper. The research opportunities provided by the developed materials and dataset are being discussed.
