Abstract:Introduction Validation of functional free-breathing MRI involves a comparison to more established or more direct measurements. This procedure is cost-intensive, as it requires access to patient cohorts, lengthy protocols, expenses for consumables, and binds working time. Therefore, the purpose of this study is to introduce a synthetic lung model (ASYLUM), which mimics dynamic MRI acquisition and includes predefined lung abnormalities for an alternative validation approach. The model is evaluated with different registration and quantification methods and compared with real data. Methods A combination of trigonometric functions, deformation fields, and signal combinations were used to create 20 synthetic image time series. Lung voxels were assigned either to normal or one of six abnormality classes. The images were registered with three registration algorithms. The registered images were further analyzed with three quantification methods: deformation-based or signal-based regional ventilation (JVent/RVent) analysis and perfusion amplitude (QA). The registration results were compared with predefined deformations. Quantification methods were evaluated regarding predefined amplitudes and with respect to sensitivity, specificity, and spatial overlap of defects. In addition, 36 patients with chronic obstructive pulmonary disease were included for verification of model interpretations using CT as the gold standard. Results One registration method showed considerably lower quality results (76% correlation vs. 92/97%, p ≤ 0.0001). Most ventilation defects were correctly detected with RVent and QA (e.g., one registration variant with sensitivity ≥78%, specificity ≥88). Contrary to this, JVent showed very low sensitivity for lower lung quadrants (0–16%) and also very low specificity (1–29%) for upper lung quadrants. Similar patterns of defect detection differences between RVent and JVent were also observable in patient data: Firstly, RVent was more aligned with CT than JVent for all quadrants ( p ≤ 0.01) except for one registration variant in the lower left region. Secondly, stronger differences in overlap were observed for the upper quadrants, suggesting a defect bias in the JVent measurements in the upper lung regions. Conclusion The feasibility of a validation framework for free-breathing functional lung imaging using synthetic time series was demonstrated. Evaluating different ventilation measurements, important differences were detected in synthetic and real data, with signal-based regional ventilation assessment being a more reliable method in the investigated setting.

Lung Volume Dependence and Repeatability of Hyperpolarized 129xe MRI Gas Uptake Metrics in Healthy Volunteers and Participants with COPD.

Abstract:Purpose:To assess the effect of lung volume on measured values and repeatability of xenon 129 (129Xe) gas uptake metrics in healthy volunteers and participants with chronic obstructive pulmonary disease (COPD).Materials and Methods:This Health Insurance Portability and Accountability Act-compliant prospective study included data (March 2014-December 2015) from 49 participants (19 with COPD [mean age, 67 years ± 9 (SD)]; nine women]; 25 older healthy volunteers [mean age, 59 years ± 10; 20 women]; and five young healthy women [mean age, 23 years ± 3]). Thirty-two participants underwent repeated 129Xe and same-breath-hold proton MRI at residual volume plus one-third forced vital capacity (RV+FVC/3), with 29 also undergoing one examination at total lung capacity (TLC). The remaining 17 participants underwent imaging at TLC, RV+FVC/3, and residual volume (RV). Signal ratios between membrane, red blood cell (RBC), and gas-phase compartments were calculated using hierarchical iterative decomposition of water and fat with echo asymmetry and least-squares estimation (ie, IDEAL). Repeatability was assessed using coefficient of variation and intraclass correlation coefficient, and volume relationships were assessed using Spearman correlation and Wilcoxon rank sum tests.Results:Gas uptake metrics were repeatable at RV+FVC/3 (intraclass correlation coefficient = 0.88 for membrane/gas; 0.71 for RBC/gas, and 0.88 for RBC/membrane). Relative ratio changes were highly correlated with relative volume changes for membrane/gas (r = -0.97) and RBC/gas (r = -0.93). Membrane/gas and RBC/gas measured at RV+FVC/3 were significantly lower in the COPD group than the corresponding healthy group (P ≤ .001). However, these differences lessened upon correction for individual volume differences (P = .23 for membrane/gas; P = .09 for RBC/gas).Conclusion:Dissolved-phase 129Xe MRI-derived gas uptake metrics were repeatable but highly dependent on lung volume during measurement.Keywords: Blood-Air Barrier, MRI, Chronic Obstructive Pulmonary Disease, Pulmonary Gas Exchange, Xenon Supplemental material is available for this article © RSNA, 2023.

Abstract:Rationale and Objectives: Chronic obstructive pulmonary disease (COPD) is highly heterogeneous and not well understood. Hyperpolarized xenon-129 (Xe129) magnetic resonance imaging (MRI) provides a unique way to assess important lung functions such as gas uptake. In this pilot study, we exploited multiple imaging modalities, including computed tomography (CT), gadolinium-enhanced perfusion MRI, and Xe129 MRI, to perform a detailed investigation of changes in lung morphology and functions in COPD. Utility and strengths of Xe129 MRI in assessing COPD were also evaluated against the other imaging modalities. Materials and Methods: Four COPD patients and four age-matched normal subjects participated in this study. Lung tissue density measured by CT, perfusion measures from gadolinium-enhanced MRI, and ventilation and gas uptake measures from Xe129 MRI were calculated for individual lung lobes to assess regional changes in lung morphology and function, and to investigate correlations among the different imaging modalities. Results: No significant differences were found for all measures among the five lobes in either the COPD or age-matched normal group. Strong correlations (F? > 0.5 or < -0.5, p < 0.001) were found between ventilation and perfusion measures. Also gas uptake by blood as measured by Xe129 MRI showed strong correlations with CT tissue density and ventilation measures (R > 0.5 or < -0.5, p < 0.001) and moderate to strong correlations with perfusion measures (R > 0.4 or < -0.5, p < 0.01). Four distinctive patterns of functional abnormalities were found in patients with COPD. Conclusion: Xe129 MRI has high potential to uniquely identify multiple changes in lung physiology in COPD using a single breath-hold acquisition.

Abstract:Background Non‐contrast‐enhanced 1H magnetic resonance imaging (MRI) with full lung coverage shows promise for assessment of regional lung ventilation but a comparison with direct ventilation measurement using 19F MRI is lacking. Purpose To compare ventilation parameters calculated using 3D phase‐resolved functional lung (PREFUL) MRI with 19F MRI. Study Type Prospective. Population Fifteen patients with asthma, 14 patients with chronic obstructive lung disease, and 13 healthy volunteers. Field Strength/Sequence A 3D gradient‐echo pulse sequence with golden‐angle increment and stack‐of‐stars encoding at 1.5 T. Assessment All participants underwent 3D PREFUL MRI and 19F MRI. For 3D PREFUL, static regional ventilation (RVent) and dynamic flow‐volume cross‐correlation metric (FVL‐CM) were calculated. For both parameters, ventilation defect percentage (VDP) values and ventilation defect (VD) maps (including a combination of both parameters [VDPCombined]) were determined. For 19F MRI, images from eight consecutive breaths under volume‐controlled inhalation of perfluoropropane were acquired. Time‐to‐fill (TTF) and wash‐in (WI) parameters were extracted. For all 19F parameters, a VD map was generated and the corresponding VDP values were calculated. Statistical Tests For all parameters, the relationship between the two techniques was assessed using a Spearman correlation (r). Differences between VDP values were compared using Bland–Altman analysis. For regional comparison of VD maps, spatial overlap and Sørensen–Dice coefficients were computed. Results 3D PREFUL VDP values were significantly correlated to VDP measures by 19F (r range: 0.59–0.70). For VDPRVent, no significant bias was observed with VDP of the third and fourth breath (bias range = −6.8:7.7%, P range = 0.25:0.30). For VDPFVL‐CM, no significant bias was found with VDP values of fourth–eighth breaths (bias range = −2.0:12.5%, P range = 0.12:0.75). The overall spatial overlap of all VD maps increased with each breath, ranging from 61% to 81%, stabilizing at the fourth breath. Data Conclusion 3D PREFUL MRI parameters showed moderate to strong correlation with 19F MRI. Depending on the 3D PREFUL VD map, the best regional agreement was found to 19F VD maps of third–fifth breath. Level of Evidence 3 Technical Efficacy Stage 2

An automated pipeline for computation and analysis of functional ventilation and perfusion lung MRI with matrix pencil decomposition: TrueLung

Abstract:Purpose: To introduce and evaluate TrueLung, an automated pipeline for computation and analysis of free-breathing and contrast-agent free pulmonary functional MRI. Material and Methods: time-resolved ultra-fast bSSFP acquisitions are transferred to TrueLung, which includes image quality checks, image registration, and computation of perfusion and ventilation maps with matrix pencil decomposition. Neural network whole-lung and lobar segmentations allow quantification of impaired relative perfusion (RQ) and fractional ventilation (RFV). TrueLung delivers functional maps and quantitative outcomes, reported for clinicians in concise documents. We evaluated the pipeline in 75 cystic fibrosis children. Whole-lung and lobar segmentations were manually refined when necessary, and the impact on RQ and RFV was quantified. Results: Functional imaging was performed at 7.9 $\pm$ 1.8 (mean $\pm$ SD) coronal slice positions per patient, totaling on average 6min 20s scan time per patient. The whole pipeline required 20min calculation time per subject. TrueLung delivered the functional maps of all the subjects for radiological assessment. Quality controlling maps and segmentations lasted 1min 12s per patient. The automated segmentations and quantification of whole-lung defects were satisfying in 88% of patients (97% of slices) and the lobar quantification in 73% (93% of slices). The segmentations refinements required 16s per patient for the whole-lung, and 2min 10s for the lobe masks. The relative differences in RFV and RQ between fully-automated and manually refined data were marginal. Conclusions: TrueLung quickly delivers functional maps and quantitative outcomes in an objective and standardized way, suitable for radiological and pneumological assessment with minimal manual input. TrueLung can be used for clinical research in cystic fibrosis and might be applied across various lung diseases.

Abstract:Background In respiratory medicine, there is a need for sensitive measures of regional lung function that can be performed using standard imaging technology, without the need for inhaled or intravenous contrast agents. Purpose To describe VOxel‐wise Lung VEntilation (VOLVE), a new method for quantifying regional lung ventilation (V) and perfusion (Q) using free‐breathing proton MRI, and to evaluate VOLVE in healthy never‐smokers, healthy people with smoking history, and people with chronic obstructive pulmonary disease (COPD). Study Type Prospective pilot. Population Twelve healthy never‐smoker participants (age 30.3 ± 12.5 years, five male), four healthy participants with smoking history (>10 pack‐years) (age 42.5 ± 18.3 years, one male), and 12 participants with COPD (age 62.8 ± 11.1 years, seven male). Field Strength/Sequence Single‐slice free‐breathing two‐dimensional fast field echo sequence at 3 T. Assessment A novel postprocessing was developed to evaluate the MR signal changes in the lung parenchyma using a linear regression‐based approach, which makes use of all the data in the time series for maximum sensitivity. V/Q‐weighted maps were produced by computing the cross‐correlation, lag and gradient between the respiratory/cardiac phase time course and lung parenchyma signal time courses. A comparison of histogram median and skewness values and spirometry was performed. Statistical Tests Kruskal–Wallis tests with Dunn's multiple comparison tests to compare VOLVE metrics between groups; Spearman correlation to assess the correlation between MRI and spirometry‐derived parameters; and Bland–Altman analysis and coefficient of variation to evaluate repeatability were used. A P‐value <0.05 was considered significant. Results Significant differences between the groups were found for ventilation between healthy never‐smoker and COPD groups (median XCCV, LagV, and GradV) and perfusion (median XCCQ, LagQ, and GradQ). Minimal bias and no significant differences between intravisit scans were found (P range = 0.12–0.97). Data Conclusion This preliminary study showed that VOLVE has potential to provide metrics of function quantification. Level of Evidence 2 Technical Efficacy Stage 1

Abstract:Abstract Objectives 3D phase-resolved functional lung (PREFUL) MRI offers evaluation of pulmonary ventilation without inhalation of contrast agent. This study seeks to compare ventilation maps obtained from 3D PREFUL MRI with a direct ventilation measurement derived from 129 Xe MRI in both patients with chronic obstructive pulmonary disease (COPD) and healthy volunteers. Methods Thirty-one patients with COPD and 12 healthy controls underwent free-breathing 3D PREFUL MRI and breath-hold 129 Xe MRI at 1.5 T. For both MRI techniques, ventilation defect (VD) maps were determined and respective ventilation defect percentage (VDP) values were computed. All parameters of both techniques were compared by Spearman correlation coefficient ( r ) and the differences between VDP values were quantified by Bland–Altman analysis and tested for significance using Wilcoxon signed-rank test. In a regional comparison of VD maps, spatial overlap and Sørensen–Dice coefficients of healthy and defect areas were computed. Results On a global level, all 3D PREFUL VDP values correlated significantly to VDP measure derived by 129 Xe ventilation imaging (all r > 0.65; all p < 0.0001). 129 Xe VDP was significantly greater than 3D PREFUL derived VDP RVent (mean bias = 10.5%, p < 0.001) and VDP FVL-CM (mean bias = 11.3%, p < 0.0001) but not for VDP Combined (mean bias = 1.7%, p = 0.70). The total regional agreement of 129 Xe and 3D PREFUL VD maps ranged between 60% and 63%. Conclusions Free-breathing 3D PREFUL MRI showed a strong correlation with breath-hold hyperpolarized 129 Xe MRI regarding the VDP values and modest differences in the detection of VDs on a regional level. Clinical relevance statement 3D PREFUL MRI correlated with 129 Xe MRI, unveiling regional differences in COPD defect identification. This proposes 3D PREFUL MRI as a ventilation mapping surrogate, eliminating the need for extra hardware or inhaled gases. Deriving Pulmonary Ventilation Images From Clinical 4D-CBCT Using a Deep Learning-Based Model

Abstract:Purpose: The current algorithms for measuring ventilation images from 4D cone-beam computed tomography (CBCT) are affected by the accuracy of deformable image registration (DIR). This study proposes a new deep learning (DL) method that does not rely on DIR to derive ventilation images from 4D-CBCT (CBCT-VI), which was validated with the gold-standard single-photon emission-computed tomography ventilation image (SPECT-VI). Materials and Methods: This study consists of 4D-CBCT and 99mTc-Technegas SPECT/CT scans of 28 esophagus or lung cancer patients. The scans were rigidly registered for each patient. Using these data, CBCT-VI was derived using a deep learning-based model. Two types of model input data are studied, namely, (a) 10 phases of 4D-CBCT and (b) two phases of peak-exhalation and peak-inhalation of 4D-CBCT. A sevenfold cross-validation was applied to train and evaluate the model. The DIR-dependent methods (density-change-based and Jacobian-based methods) were used to measure the CBCT-VIs for comparison. The correlation was calculated between each CBCT-VI and SPECT-VI using voxel-wise Spearman's correlation. The ventilation images were divided into high, medium, and low functional lung regions. The similarity of different functional lung regions between SPECT-VI and each CBCT-VI was evaluated using the dice similarity coefficient (DSC). One-factor ANONA model was used for statistical analysis of the averaged DSC for the different methods of generating ventilation images. Results: The correlation values were 0.02 ± 0.10, 0.02 ± 0.09, and 0.65 ± 0.13/0.65 ± 0.15, and the averaged DSC values were 0.34 ± 0.04, 0.34 ± 0.03, and 0.59 ± 0.08/0.58 ± 0.09 for the density change, Jacobian, and deep learning methods, respectively. The strongest correlation and the highest similarity with SPECT-VI were observed for the deep learning method compared to the density change and Jacobian methods. Conclusion: The results showed that the deep learning method improved the accuracy of correlation and similarity significantly, and the derived CBCT-VIs have the potential to monitor the lung function dynamic changes during radiotherapy.

Comparison of image registration based measures of regional lung ventilation from dynamic spiral CT with Xe-CT

Abstract:PURPOSE: Regional lung volume change as a function of lung inflation serves as an index of parenchymal and airway status as well as an index of regional ventilation and can be used to detect pathologic changes over time. In this paper, the authors propose a new regional measure of lung mechanics-the specific air volume change by corrected Jacobian. The authors compare this new measure, along with two existing registration based measures of lung ventilation, to a regional ventilation measurement derived from xenon-CT (Xe-CT) imaging.METHODS: 4DCT and Xe-CT datasets from four adult sheep are used in this study. Nonlinear, 3D image registration is applied to register an image acquired near end inspiration to an image acquired near end expiration. Approximately 200 annotated anatomical points are used as landmarks to evaluate registration accuracy. Three different registration based measures of regional lung mechanics are derived and compared: the specific air volume change calculated from the Jacobian (SAJ); the specific air volume change calculated by the corrected Jacobian (SACJ); and the specific air volume change by intensity change (SAI). The authors show that the commonly used SAI measure can be derived from the direct SAJ measure by using the air-tissue mixture model and assuming there is no tissue volume change between the end inspiration and end expiration datasets. All three ventilation measures are evaluated by comparing to Xe-CT estimates of regional ventilation.RESULTS: After registration, the mean registration error is on the order of 1 mm. For cubical regions of interest (ROIs) in cubes with size 20 mm × 20 mm × 20 mm, the SAJ and SACJ measures show significantly higher correlation (linear regression, average r(2) = 0.75 and r(2) = 0.82) with the Xe-CT based measure of specific ventilation (sV) than the SAI measure. For ROIs in slabs along the ventral-dorsal vertical direction with size of 150 mm × 8 mm × 40 mm, the SAJ, SACJ, and SAI all show high correlation (linear regression, average r(2) = 0.88, r(2) = 0.92, and r(2) = 0.87) with the Xe-CT based sV without significant differences when comparing between the three methods. The authors demonstrate a linear relationship between the difference of specific air volume change and difference of tissue volume in all four animals (linear regression, average r(2) = 0.86).CONCLUSIONS: Given a deformation field by an image registration algorithm, significant differences between the SAJ, SACJ, and SAI measures were found at a regional level compared to the Xe-CT sV in four sheep that were studied. The SACJ introduced here, provides better correlations with Xe-CT based sV than the SAJ and SAI measures, thus providing an improved surrogate for regional ventilation.

Lobar Ventilation in Patients with COPD Assessed with the Full-Scale Airway Network Flow Model and Xenon-enhanced Dual-Energy CT

Abstract:Background The full-scale airway network (FAN) flow model shows excellent agreement with limited functional imaging data but requires further validation prior to clinical use. Purpose To validate the ventilation distributions computed with the FAN flow model with xenon ventilation from xenon-enhanced dual-energy (DE) CT in participants with chronic obstructive pulmonary disease (COPD). Materials and Methods In this prospective study, the FAN model extracted structural data from xenon-enhanced DE CT images of men with COPD scanned between June 2012 and July 2013 to compute gas ventilation dynamics. The ventilation distributions on the middle cross-section plane, percentage lobar ventilation, and ventilation heterogeneity quantified by the coefficient of variation (CV) were compared between xenon-enhanced DE CT imaging and the FAN model. The relationship between the ventilation parameters with the densitometry and pulmonary function test results was demonstrated. The agreements and correlations between the parameters were measured using the concordance correlation coefficient and the Pearson correlation coefficient. Results Twenty-two men with COPD (mean age, 67 years ± 7 [standard deviation]) were evaluated. The percentage lobar ventilation computed with FAN showed a strong positive correlation with xenon-enhanced DE CT data (r = 0.7, P < .001). Ninety-five percent of lobar ventilation CV differences lay within 95% confidence intervals. Correlations of the percentage lobar ventilation were negative for percentage emphysema (xenon-enhanced DE CT: r = -0.38, P < .001; FAN: r = -0.23, P = .02) but were positive for percentage normal tissue volume (xenon-enhanced DE CT: r = 0.78, P < .001; FAN: r = 0.45, P < .001). Lung CVs of FAN revealed negative correlations with the spirometry results (CVFAN vs percentage predicted forced expiratory volume in 1 second: r = -0.75, P < .001; CVFAN vs ratio of forced expiratory volume in 1 second to forced vital capacity: r = -0.67, P < .001). Abstract:Purpose Various parameters of regional lung ventilation can be estimated using phase‐resolved functional lung (PREFUL)‐MRI. The parameter "ventilation correlation coefficient (Vent‐CC)" was shown advantageous because it assesses the dynamics of regional air flow. Calculating Vent‐CC depends on a voxel‐wise comparison to a healthy reference flow curve. This work examines the effect of placing a reference region of interest (ROI) in various lung quadrants or in different coronal slices. Furthermore, algorithms for automated ROI selection are presented and compared in terms of test–retest repeatability. Methods Twenty‐eight healthy subjects and 32 chronic obstructive pulmonary disease (COPD) patients were scanned twice using PREFUL‐MRI. Retrospective analyses examined the homogeneity of air flow curves of various reference ROIs using cross‐correlation. Vent‐CC and ventilation defect percentage (VDP) calculated using various reference ROIs were compared using one‐way analysis of variance (ANOVA). The coefficient of variation was calculated for Vent‐CC and VDP when using different reference selection algorithms. Results Flow‐volume curves were highly correlated between ROIs placed at various lung quadrants in the same coronal slice (r > 0.97) with no differences in Vent‐CC and VDP (ANOVA: p > 0.5). Abstract:Introduction: There is a need for sensitive measures of regional lung function that can be performed using standard imaging technology. We propose extending current free-breathing 1 H MRI analysis methods to provide novel measures of pulmonary V/Q. Aims: To demonstrate the application of VOxel-wise Lung VEntilation (VOLVE) analysis, a new MRI method for quantifying regional V and Q, in chronic obstructive pulmonary disease (COPD). Methods: People with COPD (n=12) and healthy never smoker controls (n=12) were recruited to undergo a free-breathing fast field echo (FFE) MRI exam. VOLVE analysis was performed by comparing the respiratory and cardiac phase to the MRI signal in each lung voxel to generate ventilation- and perfusion-weighted correlation maps. As well as the correlation, the magnitude and delay of the V/Q response can be investigated. Results: Representative ventilation (CC V ) and perfusion (CC Q ) correlation maps are shown (Figure 1). Significant differences between the groups were found for the median CC V (p<0.0001) and CC Q (p<0.0001). Conclusion: The VOLVE method provides a novel assessment of lung function that is sensitive to lung disease, enabling V/Q assessment from a single, safe, cost-effective scan. Abstract:Purpose To implement and validate an algorithm to determine the statistical errors in self‐gated non‐contrast‐enhanced functional lung imaging. Methods A bootstrapping residuals approach to determine the error in quantitative functional lung imaging is proposed. Precision and accuracy of the median error over the lungs, as well as reproducibility of the approach were investigated in 7 volunteers. The algorithm was additionally applied to data acquired in a patient with cystic fibrosis. Results The obtained bootstrapping error maps appear comparable to the error maps determined from repeated measurements, and median absolute error values for both methods show comparable median errors when reducing the number of averages. In a volunteer in whom 10 consecutive measurements were carried out, the median functional parameters were ventilation = 0.22 mL gas/mL lung tissue, perfusion amplitude = 0.028, perfusion timing = −82 ms, whereas precision and accuracy of the median error were below 3.2 × 10−3 mL gas/mL lung for ventilation tissue, 4.4 × 10−4 for perfusion amplitude, and 11 ms for perfusion timing. In the measurement of the patient, low errors in areas with reduced ventilation support the assessment as real defects. Conclusion Using a bootstrapping residuals method, the error of functional lung MRI could be determined without the need for repeated measurements. The error values can be determined reproducibly and can be used as a future means of quality control for functional lung MRI.

Abstract:Rationale and Objectives Emergent evidence in several respiratory diseases supports translational potential for Phase-Resolved Functional Lung (PREFUL) MRI to spatially quantify ventilation but its feasibility and physiological relevance have not been demonstrated in patients with asthma. This study compares PREFUL-derived ventilation defect percent (VDP) in severe asthma patients to healthy controls and measures its responsiveness to bronchodilator therapy and relation to established measures of airways disease. Materials and Methods Forty-one adults with severe asthma and seven healthy controls performed same-day free-breathing 1 H MRI, 129 Xe MRI, spirometry, and oscillometry. A subset of participants (n = 23) performed chest CT and another subset of participants with asthma (n = 19) repeated 1 H MRI following the administration of a bronchodilator. VDP was calculated for both PREFUL and 129 Xe MRI. Additionally, the percent of functional small airways disease was determined from CT parametric response maps (PRM fSAD ). Results PREFUL VDP measured pre-bronchodilator (19.1% [7.4–43.3], p = 0.0002) and post-bronchodilator (16.9% [6.1–38.4], p = 0.0007) were significantly greater than that of healthy controls (7.5% [3.7–15.5]) and was significantly decreased post-bronchodilator (from 21.9% [10.1–36.9] to 16.9% [6.1–38.4], p = 0.0053). PREFUL VDP was correlated with spirometry (FEV 1 % pred : r = −0.46, p = 0.0023; FVC% pred : r = −0.35, p = 0.024, FEV 1 /FVC: r = −0.46, p = 0.0028), 129 Xe MRI VDP (r = 0.39, p = 0.013), and metrics of small airway disease (CT PRM fSAD : r = 0.55, p = 0.021; Xrs 5 Hz : r = −0.44, p = 0.0046, and AX: r = 0.32, p = 0.044). Conclusion PREFUL-derived VDP is responsive to bronchodilator therapy in asthma and is associated with measures of airflow obstruction and small airway dysfunction. These findings validate PREFUL VDP as a physiologically relevant and accessible ventilation imaging outcome measure in asthma.

Abstract:Despite a myriad of technical advances in medical imaging, as well as the growing need to address the global impact of pulmonary diseases, such as asthma and chronic obstructive pulmonary disease, on health and quality of life, it remains challenging to obtain in vivo regional depiction and quantification of the most basic physiological functions of the lung—gas delivery to the airspaces and gas uptake by the lung parenchyma and blood—in a manner suitable for routine application in humans. We report a method based on MRI of hyperpolarized xenon-129 that permits simultaneous observation of the 3D distributions of ventilation (gas delivery) and gas uptake, as well as quantification of regional gas uptake based on the associated ventilation. Subjects with lung disease showed variations in gas uptake that differed from those in ventilation in many regions, suggesting that gas uptake as measured by this technique reflects such features as underlying pathological alterations of lung tissue or of local blood flow. Furthermore, the ratio of the signal associated with gas uptake to that associated with ventilation was substantially altered in subjects with lung disease compared with healthy subjects. This MRI-based method provides a way to quantify relationships among gas delivery, exchange, and transport, and appears to have significant potential to provide more insight into lung disease.

Abstract:This research aims to improve ventilation protocols in critical care by using appropriate computer models that take into account the essential lung mechanics. This paper automatically tracks the boundary of individual or groups of alveoli, and converts this into a pressure volume curve for three different types of alveoli. Schiller et al. investigate the inflation and deflation of alveoli in a surfactant deactivation model of lung injury in pigs. Three different types of alveoli were tracked using dynamic ventilation data from Schiller et al. The boundaries of all the tracked alveoli compared well visually to the movies. Pressure versus area curves were derived for both inflation and deflation, they captured the expected physiological behaviour, and were qualitatively similar to the quasi-static pressure area curves derived by Schiller et al. Quantitative differences are due to the dynamic effects of ventilation which were not investigated by Schiller et al.

Abstract:Fourier decomposition is a contrast agent-free 1H MRI method for lung perfusion (Q) and ventilation (V) assessment. After image registration, the time series of each voxel is analyzed with regard to the cardiac and breathing frequency components. Using a standard 2D spoiled gradient-echo sequence with a temporal resolution of ~300 ms, an image-sorting algorithm was developed to produce phase-resolved functional lung imaging (PREFUL) with an increased temporal resolution. Thus, it is feasible to evaluate regional flow volume loops (FVL) during tidal volume breathing and depict the propagation of the pulse wave during the cardiac cycle. This method can be applied at 1.5T or 3T with standard MR hardware without the necessity for sequence programming, as the described protocol can be implemented with the default SPGRE sequence on most systems. PREFUL ventilation MRI has been validated using 129Xe and 19F gas imaging with good regional agreement. Perfusion-weighted PREFUL MRI has been validated using SPECT as well as dynamic contrast enhanced (DCE) MRI. PREFUL has been tested in a dual center dual vendor setting and is currently applied in several ongoing multicenter trials. Furthermore, it is feasible across a range of field strengths (0.55T-3T) and different age groups, including newborns. Quantitative V/Q PREFUL MRI has been used in patients with cystic fibrosis, chronic obstructive pulmonary disease, chronic thromboembolic pulmonary hypertension, and corona virus disease-2019 to quantify disease and monitor treatment change after therapy. Furthermore, PREFUL V/Q imaging has been shown to predict transplant loss due to chronic lung allograft dysfunction in patients after lung transplantation. In summary, PREFUL MRI is a validated technique for quantitative ventilation and pulmonary pulse wave/perfusion imaging for regional pulmonary disease detection, quantification, and treatment monitoring with potential added value to the current clinical routine.
Patient-specific prediction of regional lung mechanics in ARDS patients with physics-based models: A validation study

Abstract:The choice of lung protective ventilation settings for mechanical ventilation has a considerable impact on patient outcome, yet identifying optimal ventilatory settings for individual patients remains highly challenging due to the inherent inter- and intra-patient pathophysiological variability. In this validation study, we demonstrate that physics-based computational lung models tailored to individual patients can resolve this variability, allowing us to predict the otherwise unknown local state of the pathologically affected lung during mechanical ventilation. For seven ARDS patients undergoing invasive mechanical ventilation, physics-based, patient-specific lung models were created using chest CT scans and ventilatory data. By numerically resolving the interaction of the pathological lung with the airway pressure and flow imparted by the ventilator, we predict the time-dependent and heterogeneous local state of the lung for each patient and compare it against the regional ventilation obtained from bedside monitoring using Electrical Impedance Tomography. Excellent agreement between numerical simulations and experimental data was obtained, with the model-predicted anteroposterior ventilation profile achieving a Pearson correlation of 96% with the clinical reference data. Even when considering the regional ventilation within the entire transverse chest cross-section and across the entire dynamic ventilation range, an average correlation of more than 81% and an average root mean square error of less than 15% were achieved. The results of this first systematic validation study demonstrate the ability of computational models to provide clinically relevant information and thereby open the door for a truly patient-specific choice of ventilator settings on the basis of both individual anatomy and pathophysiology.

Development and prospective in-patient proof-of-concept validation of a surface photogrammetry + CT-based volumetric motion model for lung radiotherapy

Abstract:Purpose: We develop and validate a motion model that uses real-time surface photogrammetry acquired concurrently with four-dimensional computed tomography (4DCT) to estimate respiration-induced changes within the entire irradiated volume, over arbitrarily many respiratory cycles. Methods: A research, couch-mounted, VisionRT (VRT) system was used to acquire optical surface data (15 Hz, ROI = 15 × 20 cm2 ) from the thoraco-abdominal surface of a consented lung SBRT patient, concurrently with their standard-of-care 4DCT. The end-exhalation phase from the 4DCT was regarded as reference and for each remaining phase, deformation vector fields (DVFs) with respect to the reference phase were computed. To reduce dimensionality, the first two principal components (PCs) of the matrix of nine DVFs were calculated. In parallel, ten phase-averaged VRT surfaces were created. Surface DVFs and corresponding PCs were computed. A principal least squares regression was used to relate the PCs of surface DVF to those of volume DVFs, establishing a relationship between time-varying surface and the underlying time-varying volume. Proof-of-concept validation was performed during each treatment fraction by concurrently acquiring 30 s time series of real-time surface data and "ground truth" kV fluoroscopic data (FL). A ray-tracing algorithm was used to create a digitally reconstructed fluorograph (DRF), and motion trajectories of high-contrast, soft-tissue, anatomical features in the DRF were compared with those from kV FL. Results: For five of the six fluoroscopic acquisition sessions, the model out-performed 4DCT in predicting contour Dice coefficient with respect to fluoroscopy-derived contours. Similarly, the model exhibited a marked improvement over 4DCT for patch positions on the diaphragm. Model patch position errors varied from 5 to -15 mm while 4DCT errors ranged between 5 and -22.4 mm. For one fluoroscopic acquisition, a marked change in the a priori internal-external correlation resulted in model errors comparable to those of 4DCT. Conclusions: We described the development and a proof-of-concept validation for a volumetric motion model that uses surface photogrammetry to correlate the time-varying thoraco-abdominal surface to the time-varying internal thoraco-abdominal volume. These early results indicate that the proposed approach can result in a marked improvement over 4DCT. While limited by the duration of the fluoroscopic acquisitions as well as the resolution of the acquired images, the DRF-based proof-of-concept technique developed here is model-agnostic, and therefore, has the potential to be used as an in-patient validation tool for other volumetric motion models.
