Abstract:Objective: To observe the therapeutical efficacy and safety of photodynamic therapy with aminolevulinic acid （ALA-PDT） in urethral condyloma acuminatum. Methods: ALA-PDT was applied in the treatment of 44 patients with urethral condyloma acuminata; as control, CO; laser was performed in patients with urethral condyloma acuminata. Local adverse reac tions, extent of resolution of the lesions, time for resolution of the lesions and relapse rate were evaluated after treatment. Results: Total effective rate in ALA group reached 95.5%（42/44）, and the time for resolution was about （13.24±4.83） days, showing no statistical difference （P>0.05） compared with those in CO; laser group. The incidence of either scar formation or urethral stricture was 0% in ALA group, while it was 19.4%（7/36） and 11.1%（4/36） respectively in CO; laser group, showing statistical differ- ences （P<0.01）. Furthermore, the overall relapse rate in ALA group was 17.6%（6/34）, which was significantly lower than that in CO; laser group（21/31, 67.7%）. Conclusion: ALA-PDT therapy has the advantages of a higher therapeutic efficacy, a lower relapse rate and fewer side effects in the treatment of urethral condyloma acuminatum.

Abstract:<p>Infections with human papillomavirus in the anogenital area result in warty, papillary, and condylomatous lesions. The giant anogenital warts are relatively uncommon. Treatment of giant warts is challenging, especially in the immunosuppressive population. Here, a patient with systemic lupus erythematosis had extensive, fast-growing, recurrent anogenital condylomatosis shaped as giant cauliflowers. We reported this case and the successful treatment of photodynamic therapy combined with surgical management. It provided the feasibility of surgical ablation combined with ALA-PDT performed on these kinds of challenging cases.</p>

Abstract:OBJECTIVES:Condyloma acuminatum (CA) is a sexually transmitted disease with a high recurrence rate due to the rapid replication of human papillomavirus (HPV) and its subtle immune escape mechanism, which makes the diagnosis and treatment of CA in the male urethra particularly difficult. This study aims to evaluate the efficacy of comprehensive treatments for male urethral CA after accurate localization of warts under ultrasound guidance.METHODS:The study included 15 men with intraurethral CA. Before treatment, the urethra was examined by ultrasonography and HPV-PCR. After examination of the invisible urethral warts, wart curettage (penetrating operation with a special stainless steel medical curettage tool) combined with ALA-PDT was used for treatment. The ultrasound and HPV load were reviewed 1 week after treatment, and again at 1, 3, and 6 months after treatment.RESULTS:All patients achieved satisfactory results 1 week after the last treatment. The viral load of human papilloma was significantly reduced or turned negative, ultrasound imaging exploration showed no neoplasm in the urethra, and no obvious intraoperative or postoperative complications were observed. The side effects in patients included a mild burning or tingling sensation confined to the treated area. After a 6 month follow-up period, only 2 patient relapsed.CONCLUSION:The combined diagnosis and treatment of CA in the male urethra under the guidance of multi-mode ultrasound imaging is an effective, economical, safe, and well-tolerated treatment method.

Abstract:Background: Condyloma acuminatum is a benign proliferative lesion of the genitalia and perianal region caused by human papillomavirus infection, which is rare in children. Traditional treatments include topical medications, CO2 laser therapy, cryotherapy, and surgical resection, which are often associated with pain, scarring, and recurrence. In recent years, aminolevulinic acid/photodynamic therapy has been applied as a new method for treating condyloma acuminatum in adults. However, its application in young children is limited. We herein report a case of condyloma acuminatum treated by aminolevulinic acid/photodynamic therapy in a young boy.Case report: The patient was a 2-year-old boy who presented with a 10-day history of warty masses on his glans penis. Condyloma acuminatum was diagnosed based on skin lesions and histological findings. Aminolevulinic acid/photodynamic therapy was selected as the treatment method after obtaining informed consent from his guardians. We performed five rounds with a treatment interval of 7 days and achieved a complete disappearance of the skin lesions without chapping, flushing, or scarring. There was no recurrence during the 6-month follow-up period.Conclusion: Photodynamic therapy can be used as an alternative treatment for condyloma acuminatum in young children. More case studies are needed to confirm the value of this treatment.

Abstract:•Condyloma acuminata and lichen planus are different cutaneous diseases with disparate etiologies. Simultaneous appearance of two diseases in the same area is rare.•This report aims to share the clinical and pathological features of a rare case; introduce the effective and safe therapy of it so as to broaden the therapeutic spectrum of Photodynamic Therapy (PDT) indications.•PDT has its therapeutic advantages in cutaneous diseases, especially inflammatory disorders, tissue proliferative diseases such as CA and malignant tumors.•Limited side effects and low recurrence are two significant advantages of noninvasive ALA-PDT.•This case shows that topical PDT appears to be a viable option for the treatment of CA with LP and adds another application to ALA-PDT.

Abstract:Condyloma acuminatum is a common sexually transmitted disease caused by human papillomavirus infection and is a benign hyperplastic lesion of the genital and perianal areas. The principle of its treatment is to remove the visible warts as much as possible and to prevent recurrence. Traditional treatment methods of condyloma acuminatum, such as CO 2 laser, liquid nitrogen freezing, surgery, and topical medications, can remove warts. However, these methods have disadvantages such as pain, high recurrence rates, long treatment cycles, and scarring. Aminolevulinic acid/photodynamic therapy (ALA-PDT), a safe and effective method, has been widely used to treat condyloma acuminatum in recent years. Condyloma acuminatum occurs relatively rarely in elderly patients, in whom treatment is difficult owing to poorer physiological function. We successfully treated an 87-year-old patient with a giant condyloma acuminatum of the glans penis using six sessions of ALA-PDT at 7-day intervals and obtained satisfactory results. No recurrence was observed during a 6-month follow-up. Therefore, ALA-PDT is worth popularizing in clinical practice.

Abstract:Condylomata acuminata (CA) is caused by human papillomavirus (HPV). It is one of the most common sexually transmitted diseases (STD). The lesions mainly occur in the external genitalia and perianal areas, rarely involves in urethral and usually localized at the distal 3 cm of the urethral orifice. Because of the special anatomical site, treating urethral CA is challenging and it has high recurrence rate after treatment. 5-aminolevulinic acid photodynamic therapy (ALA-PDT) can successfully treat urethral CA, however, the experience of using ALA-PDT combined with wart curettage to treat intractable urethral CA is still very limited. In here, we reported an intractable urethral CA case with effective remission after receiving combination therapy. Wart curettage combine with ALA-PDT is an expeditious, economical, and well-tolerated treatment method.

Abstract:AbstractCondyloma acuminatum is a common sexually transmitted disease caused by human papillomavirus. Its prevalence and recurrence rates are high. While there are multiple treatment options for condyloma acuminatum, no single therapy form is effective in achieving complete remission. Photodynamic therapy (PDT) was shown to be very promising as a treatment option for condyloma acuminatum. However, for some patients the therapeutic result after a single PDT session is not satisfactory. Thus, it is necessary to develop an effective treatment method for recalcitrant condyloma acuminatum.We hypothesize that suboptimal therapeutic results after a single PDT may be caused by an insufficient penetration of the photosensitizer into the tissue. Here, we applied a combination of superficial shaving and PDT to address this issue. All patients were satisfied with the effect of the treatment, and their quality of life improved significantly. Pain was the major adverse event during treatment but was acceptable to all patients. Therefore, superficial shaving in combination with PDT is safe and effective.

Abstract:Background Condyloma acuminatum (CA) of the vagina is a sexually transmitted disease due to infection by human papilloma virus (HPV). The treatment efficacy of the conventional methods for vaginal CA is often unsatisfactory with a high recurrence rate. Topical 5-aminolevulinic acid-mediated photodynamic therapy (ALA-PDT) combined with CO 2 laser pretreatment is a feasible approach for vaginal CA, but the effectiveness and safety need further evaluation. Methods This study enrolled 15 patients with vaginal CA. All patients underwent CO 2 laser ablation and then ALA-PDT for two or three cycles. The clinical efficacy and side effects were evaluated and analyzed during the treatment and 6 months after the treatment. Results The wart lesions in 4 cases (26.7%) disappeared after the first treatment. The wart lesions in 5 cases (33.3%) disappeared after the second treatment. And 6 cases (40%) needed three treatment cycles before the lesions disappeared completely. The complete response (CR) rate was 93.3% (14/15) at 2 weeks after three treatment cycles. There were 5 cases (83.3%) which have complete remission after 2 treatments in warts diameter 1 cm group. All patients had CR without reoccurrence at 6 months after treatment. The side effects mainly included a mild or moderate burning or stinging sensation (26.7%). There were no infection, ulcers and scars after treatment. Conclusion Topical 5-aminolevulinic acid-mediated photodynamic therapy combined with CO 2 laser pretreatment is a safe and effective treatment for vaginal CA.

Abstract:INTRODUCTION:With the younger onset age of female lower genital tract diseases, there are increasing demands for protecting organ and tissue structures to preserve fertility and, therefore, effective fertility-sparing treatments that cause minimal normal tissue damage and less adverse reactions are urgently needed.OBJECTIVE:This study is aimed at reviewing information and achieving consensus on recommendations on the clinical applications of aminolevulinic acid-based photodynamic therapy (ALA-PDT) in female lower genital tract diseases.METHODS:Members of the expert panel held online and in-person meetings to discuss and revise drafts created by the steering committee based on the literature review and the clinical experiences of the expert panel. Opinions of the experts were transcribed and discussed in detail to ensure that the consensus statement best reflects the current advances in the field and the experts' view.RESULTS:After numerous rounds of meetings, experts unanimously agreed on the importance of ALA-PDT in the treatment of cervical squamous intraepithelial lesions (SIL), vaginal SIL, vulvar SIL, vulvar lichen sclerosus (VLS), and condyloma acuminatumon (CA). Experts also reached consensus on the recommended treatment regimen and treatment methods.CONCLUSION:This consensus aimed to provide practical basis and guidance for the clinical applications of ALA-PDT in female lower genital tract diseases in China. Of note, this is the only expert consensus prepared by board-certified specialists in gynecology and obstetrics in China. More evidence-based clinical studies should be made to update and expand the current recommendations.

Abstract:Background Human papilloma virus (HPV) infection and cervical condyloma acuminatum (CA) often co-exist. Although there are many methods to treat cervical CA, high recurrence rate and cervical scars are still troublesome. Biopsy forceps excision combined with 5-aminolevulinic acid photodynamic therapy (ALA-PDT) is a feasible approach for cervical CA, but its efficacy and limitation need to be evaluated. Methods This retrospective study consisted of 49 patients aged 18–50 years with a histologically or colposcopic confirmed cervical CA and with HPV infection. Patients were treated with biopsy forceps excision and ALA-PDT. The efficacy was evaluated through HPV typing and colposcopy directed biopsy. Results Three months after the combination treatment the total lesion remission rate was 93.88% (46/49) and the HPV clearance rate was 83.67 % (41/49). One patients showed some residual lesions and two patients showed new lesions. Recurrence rate was 4.34% at 6 months follow-up. There was no significant difference in HPV clearance rate at 3 and 6 months follow-up. Univariate analysis showed that the combination treatment was less effective for patients who had size of visible lesion > 1.5 cm 2 . Adverse effects were minimal and no structural complications were reported. The main side effects were abdominal pain and increased vaginal secretions. Conclusion Combination of biopsy forceps excision and ALA-PDT is safe and effective for eliminating cervical condylomata lesion and eradicating HPV infection. Colposcopic evaluation is recommended before and after treatment.

The effect of pidotimod combined photodynamic therapy on Langerhans cells in female patients with condyloma acuminate

Abstract:Objective To explore the effect of pidotimod combined photodynamic therapy (PDT) on Langerhans cells in female patients with condyloma acuminata (CA). Methods One hundred female patients with CA were selected from the department of dermatology of this hospital during June 2013 to June 2016, and they were divided into pi light group and single light group according to the random number table method,50cases in each group. Patients in single light group were given with 5-aminolevulinic acid photodynamic therapy (PDT) for treatment, and patients in pi light group were given with pidotimod on the basis of treatment in single light group,and immune functions were compared before and after the treatment, LC and effect of treatment and recurrence rate were compared between these two groups. Results Amount of CD3 +,CD4+, CD4+/CD8+ and number of LC,and density of positive cells in pi light group were significantly higher than those of single light group, and the number of CD8+ in pi ligt group was significantly lower than those of single light group, and the difference was statistically significant (P＜0.05). The therapeutic response rate of pi light group was 96.00％, it was significantly higher than those (84.00％) of single light group, and the difference was statistically significant (P＜0.05). The rates of adverse reactions in pi light group and single light group were 22.00％ and 18.00％ respectively, and the difference was not statistically significant (P＞0.05). After follow-up for 12months, the recurrence rate of pi light group was 6.25％, it was significantly lower than that (23.81％) of single light group, and the difference was statistically significant (P＜0.05), Conclusion The application of pidotimod combined photodynamic therapy can effectively improve the immune function of female patients with CA, it may facilitate the promotion of recovery of Langerhans' cells to improve the curative effect and reduce the recurrence, hence it is worthy to be further clinically promoted.

Abstract:1. Musculo-skeletal development was measured in males and females of two genetic lines of turkey breeding stock: one line had been selected mainly for egg production (E-line) while the other had been selected for meat production as well as egg production (M-line). 2. The M-line had a more rapid growth of body weight and breast muscle weight than the E-line but breast muscle yield (breast muscle weight/body weight) did not differ between lines. 3. From an early age, breast length and keel length were longer in the M-line than in the E-line. 4. In older birds, keel depth was greater in the M-line than in the E-line. 5. No differences between lines were detected for depth of the pectoralis muscle, number of muscle fibres in the depth of the pectoralis, or muscle fibre cross sectional areas.
Treatment of condyloma acuminata with carbon dioxide laser: a prospective study

Abstract:The common wart has been a ubiquitous problem throughout recorded history. In recent times, the genital wart has assumed more and more importance. These viral growths cause itching and burning as well as more severe problems. Treatment traditionally has included a large variety of preparations from nature. Subsequently, man-made chemicals were used along with freezing, burning, and surgical excision. Because of the pain, bleeding, scarring, and high recurrence rate with these treatments, a study was instituted in 1978 to evaluate the advantages of the CO2 laser for treatment of this condition. Results have been excellent, far exceeding what we expected. Recurrence rate has been under 2%. The control group had the usual high rate of persistence and recurrence. Recent work pointing to a strong association between human papilloma virus (HPV) and cancer of the female genitals suggests an increasing need for early, vigorous, and more effective treatment of these lesions.
Evaluation of Photodynamic Therapy Using Topical Aminolevulinic Acid Hydrochloride in the Treatment of Condylomata Acuminate.

Abstract:OBJECTIVES To investigate the efficacy and safety of topical application of 5-aminolaevulinic acid (ALA) photodynamic therapy (PDT) for the treatment of condylomata acuminata (CA) in larger population. METHODS Patients with CA were given a treatment of ALA-PDT once a week for 3 weeks and followed up at 4, 8, 12 and 24 weeks after the treatment finished. RESULTS In 531 patients, a clearance rate was observed 95.27%. The rates rouse with PDT cycles. The clearance rate of three PDT cycles was significant higher than one PDT cycles (P < 0.001) and two PDT cycles (P < 0.001). The clearance rate (88.73%) of small lesions (diameter small than 5mm) was significant higher than that (97.74%) of larger lesions (P < 0.001). The clearance rate varied with the location of the lesions. The clearance rate of urethral meatus was highest and that of perianal was lowest. Follow-up for patients with complete response lasted for 24 weeks. The recurrence rate was 5.65%, 11.30%, 15.07%, 15.44% and 16.20% after 1, 4, 8, 12 and 24 weeks. The recurrence rate varied with the location of the lesions. The recurrence rate of perianal was highest and that of labium was lowest. The side effects mainly included flare, pain, erosion, ulcer, and hyperpigmentation. The adverse reaction rate was 7.72%, 8.10%, 2.26%, 0.94% and 0.19%. Sexual dysfunction and urethral malformations were not observed during the 24 weeds visit. CONCLUSION Topical application of ALA-PDT is a simple and as effective therapy with a lower incidence of adverse effects in the treatment of condylomata acuminata.
Topical 5-Aminolevulinic Acid Photodynamic Therapy for the Treatment of Condyloma Acuminatum: a Multicenter, Randomized, CO2 Laser-Controlled Trial

Abstract:Objective To evaluate the safety and efficacy of topical 5-aminolevulinic acid photody- namic therapy (ALA-PDT) in the treatment of condyloma acuminatum.Methods This multicenter,ran- domized,open,parallel,CO_2 laser-controlled clinical trial was conducted at 5 research sites in China. Patients with condyloma acuminatum were randomly assigned to the ALA-PDT group or the CO_2 laser (control) group in an allocation ratio of 3:1.The treatment was repeated weekly if necessary,but no more than 3 times.The primary efficacy endpoint was the wart clearance rate 1 week after the last treatment.The recurrence rate was evaluated at weeks 4,8,and 12 after the treatment ended.The laboratory examinations were performed before and at the end of the treatment.Results A total of 453 patients with condyloma acuminatum were enrolled in the clinical trial.Of these,442 (97.57%) patients completed the trial (331 in the ALA-PDT group,and 111 in the control group).At 1 week after the last treatment,98.42% and 100% of the base line warts were eliminated in the ALA-PDT group and the control group,respectively (P0.05).The clearance rate of condylomata acuminata at male urethral orifice was 99.43% in the ALA-PDT group and 100% in the control group (P0.05).The overall recurrence rate calculated for the whole fol- low-up period was significantly lower in the ALA-PDT group than in the control group (10.77% vs 33.33%, P0.0001),and the recurrence of condyloma acuminatum at male urethra orifice was also significantly decreased in the ALA-PDT group than in the control group (10.53% vs 36.36%,P0.0001).There was no systemic adverse event in either group.The incidence of local adverse events was significantly lower in the ALA-PDT group than in the control group (7.67% vs.53.57%,P0.0001).The main adverse event in the ALA-PDT group was mild erosion,whereas the adverse events in the control group were more com- mon and severe,including pain,ulcer,and even scarring (n=14).Conclusions The results confirm that topical ALA-PDT is a safe and effective treatment with low recurrence rate for condyloma acuminatum, especially for this condition at urethral orifice.

Abstract:Objectives: To investigate the clinical efficacy and safety of topical application of 5-aminolevulinic acid (ALA) photodynamic therapy (PDT) for the treatment of condylomata acuminata in urethral meatus. Methods: In total of 213 subjects were enrolled in the trail. The patients were treated with 20% ALA solution occlusive dressing for 3 h Mowed by irradiation with the helium-neon laser. The treatment was repeated 1 week later if the lesion was not completely removed after the first treatment. Once more therapy was carried out one week after the lesions were cleared. Results: Two hundred and eight subjects were cured after thrice therapies. The cure rate was 97.7% and the recurrence rate 8.2%. No severe adverse effect was found. Conclusion: Photodynamic therapy exhibits good efficacy and safety in the treatment of condylomata acuminata in urethral meatus.

Abstract:Oral condyloma acuminatum (OCA) is a less prevalent subtype of condyloma acuminatum and is challenging to treat. We reported two cases of OCA successfully treated with 5-aminolevulinic acid photodynamic therapy (ALA-PDT). After three treatments spaced one week apart, both cases were completely cured without significant adverse reactions and were followed up for one year with no recurrence.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background</h3><p>Pain is a major concern associated with conventional photodynamic therapy (C-PDT).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Objective</h3><p>To evaluate the efficacy, pain and safety of modified photodynamic therapy(M-PDT) for the treatment of condylomata acuminata.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>A prospective, randomized, self-controlled study was conducted. Warts were randomized to the M-PDT or C-PDT side. 5-aminolevulinic acid (ALA; 20%) was incubated for 3 h before patients were exposed to LED red light (100 J/cm2 ) on the C-PDT side and for 30 minutes before being exposed to LED red light (300 J/cm2 ) on the M-PDT side. Treatment was administered with 1-week interval for three weeks. The clearance rates were determined at one week and the recurrence rates at 4, 8, and 12 weeks after treatment. The pain and other side effects were also investigated.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>A total of 24 patients with condylomata acuminata were enrolled in this trial. Twenty patients completed the trial. The clearance rates were 98.17% in the M-PDT side and 98.20% in the C-PDT side(P > 0.05). The recurrence rates were 11.11% and 10.53% (P > 0.05). However, M-PDT was almost painless (mean score 0.3 ± 0.47, range 0∼1), which was significantly less than that on the C-PDT side (mean score 3.6 ± 0.94, range 0∼1) (P < 0.05). Local erythema, mild edema and erosion were observed on both sides.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusion</h3><p>The modified PDT is basically painless with similar efficacy to conventional PDT, which is a major breakthrough of pain management in PDT.</p>

Abstract:The present study aimed to evaluate the efficacy of photodynamic therapy with topical applied 5-aminolevulinic acid (ALA-PDT) for the treatment of cervical condylomata accuminate (CA). 161 Patients with cervical CA were randomly divided into ALA-PDT group and CO2 laser (control) group. Patients (n=89) in the ALA-PDT group were treated with topical 5% ALA under occlusive dressing for 3 h followed by irradiation with semiconductor laser at a dose of 1000 J/cm(-2) and a power of 100 mW. Patients were treated 2 weeks later if necessary. Patients (n=72) in the control group were treated with CO2 laser. The treatment was repeated at 1-week interval when necessary. No response rate, complete response rate (CR) and recurrence rate of wart lesions as well as rate of eradication of HPVs were analyzed. The CR rate was 90.2% in the ALA-PDT group and 96.2% in the control group. The eradication rate was 90.2% in the ALA-PDT group and 65.8% in the control group after 3 months of follow-up. Both the eradication rate and recurrence rate in the ALA-PDT group were significantly lower than those in the control group (P<0.001). The adverse event in patients receiving ALA-PDT was mainly mild bleeding. ALA-PDT is a more effective and well-tolerated treatment for cervical CA compared with conventional CO2 laser therapy.
