Abstract:Intrauterine adhesion (IUA) is primarily caused by endometrial injury, and hysteroscopic adhesiolysis is presently the main treatment. However, postoperative recurrence and poor pregnancy outcomes remain intractable. In this study, we aim to assess the effects of different treatments on clinical symptoms and reproductive outcomes in IUA. This retrospective study was conducted in a tertiary university-affiliated women’s hospital. The study included 1449 consecutive women who desired to have a baby and were diagnosed with IUA through hysteroscopy from January 2016 to December 2021. Patients with IUA underwent hysteroscopic electric resection (E) or cold scissors separation (C), as well as hormone therapy and one or both of the following secondary prevention measures: intrauterine devices (IUD) and hyaluronic acid gel (HA). The pregnancy rate (PR) was significantly higher in the E + IUD + HA (90.23% CI: 85.82, 94.64%) than in other groups (p = 0.000) groups. The rates of full-term birth (p = 0.000) and live birth (p = 0.000) were significantly higher in the E + IUD + HA (67.82% and 68.97%, respectively) and E + HA (62.41% and 63.91%, respectively) groups. Multivariate logistic regression analysis revealed a significantly higher PR in women who received second-look hysteroscopy (OR 1.571, 95% CI: 1.009–2.224, p = 0.013) and E + IUD + HA (OR 4.772, 95% CI: 2.534–8.987, p = 0.000). Combining hysteroscopic electric resection with IUDs and HA gel could prevent adhesion recurrence and improve postoperative pregnancy and live birth outcomes in IUA. Furthermore, postoperative second-look hysteroscopy may increase the PR and shorten the waiting period.
Effect of Early Second-Look Hysteroscopy on Reproductive Outcomes after Hysteroscopic Adhesiolysis in Patients with Intrauterine Adhesion, a Retrospective Study in China

Abstract:Objective: The aim of this study was to assess the effect of early second-look hysteroscopy after hysteroscopic adhesiolysis for intrauterine adhesions (IUAs) on the pregnancy rate (PR) and live birth rate (LBR). Study design: It is a retrospective, observational study of 151 women were referred to our institution from January 2010 to January 2016. Operative hysteroscopy for treatment of IUAs and office hysteroscopic follow-up to assess IUAs. We investigated demographic characteristics, menstrual pattern, previous intrauterine surgery, reproductive history, ovarian preservation assessment, tubal patency examination, husband's semen test, obstetric parameters, and surgical variables to evaluate the effect of early second-look hysteroscopy after hysteroscopic adhesiolysis on the PR and LBR. Results: Of 151 women treated for IUAs, the general PR was 71.5% and LBR was 53.0%. The PR and LBR were higher in the earlier second-look group (compared with second hysteroscopy later than 2 months group) and the group which received less than three times adhesiolysis (p < 0.05). The PR was higher in the amenorrhea group (compared with normal menses group) and recurrent miscarriage group (compared with infertility group) (p < 0.05). Logistic regression showed that the second-look time interval, times of operation to relieve adhesion, and pregnancy history were associated with the PR, while age and the second-look time interval were associated with the LBR. Conclusions: Early second-look hysteroscopic examinations within 2 months may increase the cumulative PR and LBR.
Analysis of Risk Factors for Obstetric Outcomes after Hysteroscopic Adhesiolysis for Asherman Syndrome: A Retrospective Cohort Study.

Abstract:Objective To investigate the factors influencing placenta accreta in pregnant women who previously underwent hysteroscopic adhesiolysis (HA). Methods This retrospective study enrolled 265 women with intrauterine adhesions (IUAs) at the Sir Run Run Shaw Hospital from January 2014 to December 2018. We followed up their pregnancy outcomes and maternal complications. Results The menstrual pattern and gestational history before operation were significantly different between the live birth and pregnancy loss groups. The age, extent of cavity involved, type of adhesions, times of adhesiolysis performed, and time interval from surgery to pregnancy were not significantly different between these two groups. In the third trimester, 48 of 140 patients had 53 perinatal complications, including placenta accreta (27), gestational diabetes mellitus (10), pregnancy-induced hypertension (6), postpartum hemorrhage (4), intrahepatic cholestasis of pregnancy (2), placenta previa (1), oligohydramnios (1), and intrauterine growth restriction (1). Logistic regression analysis showed that extent of cavity involved and times of adhesiolysis performed were associated with placenta accreta. Conclusion The extent of cavity involved and times of adhesive separation surgeries were risk factors for placenta accreta in patients. The menstrual model and gestational history may provide the main predictive factors for pregnancy loss.
A retrospective cohort study to examine factors affecting live birth following hysteroscopic treatment of intrauterine adhesions

Abstract:Structured abstract Objective To evaluate independent factors that affect the chance of live birth following hysteroscopic adhesiolysis in patients with intrauterine adhesions (IUAs). Design Retrospective cohort study Setting Hysteroscopic center of Fuxing Hospital in Beijing, China. Patient(s) Patients diagnosed with Asherman syndrome between June, 2020, and February, 2022. Intervention(s) Hysteroscopic adhesiolysis followed by second look hysteroscopy to assess outcome and follow-up for a year. Main Outcome Measure(s) Live birth rate without the use of assisted reproductive technologies at 12 months follow-up. Result (s): Of the 544 women included in the cohort, the pregnancy rate at the end of one year follow-up was 47.6% (95%CI 45.5%-49.7%) and the live birth rate was 41.0% (95%CI 38.9%-43.1%). Stepwise multiple logistic regression analysis identified 3 independent predictors of live birth in decreasing order of significance: increase in menstrual flow after surgery (OR 3.69, 95%CI 1.77–8.21), postoperative endometrial thickness in the mid-luteal phase (OR 1.53, 95%CI 1.31–1.80), and the severity of recurred adhesion at second-look hysteroscopy (OR 0.62, 95%CI 0.50–0.76). Among subjects with good independent prognostic factors, namely, increased menstrual flow after surgery, postoperative endometrial thickness in the mid luteal phase greater than 6mm, no or minimal recurrence of adhesions at second-look hysteroscopy, the live birth rate was 69.0% (95%CI 65.4%-72.6%). On the other hand, in women (n=26) without any of the three good prognostic factors, none had a successful live birth (0%). Conclusion (s): Overall, the live birth rate following treatment of Asherman syndrome was 41.0%. The prognosis is dependent on three outcome measures following surgery, namely, improvement in menstrual flow, post-operative endometrial thickness and minimal degree of recurrent adhesions at second-look hysteroscopy.

Abstract:STUDY QUESTION Does intrauterine infusion of granulocyte colony-stimulating factor (G-CSF) prevent adhesion reformation and promote endometrial growth after hysteroscopic adhesiolysis? SUMMARY ANSWER Intrauterine perfusion of G-CSF can increase endometrial thickness but does not prevent the recurrence of intrauterine adhesions (IUAs) in patients with Asherman syndrome (AS) after surgery. WHAT IS KNOWN ALREADY Intrauterine infusion of G-CSF has been used in attempts to treat patients with recurrent miscarriage and an idiopathic thin endometrium for either fresh or frozen-thawed embryo transfer cycles but without uniform efficacy. There have been no reports on the effect of G-CSF on the recurrence of IUAs, endometrial regrowth or pregnancy results in specific populations with AS. STUDY DESIGN, SIZE, DURATION This two-center prospective double-blind randomized controlled trial ran between April 2016 and August 2021. In it, 245 patients with moderate to severe AS were randomized to G-CSF and control groups at a 1:1 ratio; 229 women were included in the adhesion recurrence analysis; and 164 patients were analyzed for pregnancy outcomes. PARTICIPANTS/MATERIALS, SETTING, METHODS All eligible patients received the first hysteroscopic adhesion separation and balloon placement procedure. Patients who met our inclusion and exclusion criteria were randomized after surgery. These patients returned for balloon removal and underwent intrauterine perfusion with 300 mu g (1.8 ml) G-CSF or 1.8 ml normal saline according to randomization at 7 days after surgery. A second-look hysteroscopy was carried out 1-2 months later. The primary outcome was the rate of formation of new adhesions at the second hysteroscopy. The secondary outcomes included endometrial thickness in the periovulatory period after surgery, as well as the clinical pregnancy and live birth rates. MAIN RESULTS AND THE ROLE OF CHANCE Age, menstrual cycle characteristics, pregnancy history and IUA score before surgery were similar between groups. There were no statistically significant differences in the adhesion reformation rate or median adhesion score reduction. However, G-CSF perfusion significantly improved endometrial thickness (7.91 +/- 2.12 mm vs 7.22 +/- 2.04 mm; P = 0.019, 95% CI for difference: -1.26 to -0.12), as well as cumulative pregnancy and live birth rate over time (P = 0.017 and P = 0.042). Furthermore, multivariate logistic regression analysis showed that postoperative endometrial thickness was an independent prognostic factor for pregnancy and live birth rates. LIMITATIONS, REASONS FOR CAUTION These results cannot be extended to older patients or those without AS, as our subjects had moderate or severe AS and were aged WIDER IMPLICATIONS OF THE FINDINGS Intrauterine perfusion of G-CSF could be an effective adjuvant therapy for patients with AS to increase endometrial thickness. Abstract:To compare the efficacy of heart-shaped intrauterine balloon and intrauterine contraceptive device (IUD) in the prevention of adhesion reformation after hysteroscopic adhesiolysis.Prospective, randomized, controlled trial.University hospital.A total of 201 women with Asherman syndrome.Women were randomized to having either a heart-shaped intrauterine balloon or an IUD fitted after hysteroscopic adhesiolysis. The devices were removed after 7 days. A second-look hysteroscopy was carried out 1 to 2 months after the surgery.Incidence of adhesion reformation and reduction of adhesion score before and after surgery.Initially 201 cases were recruited; 39 cases dropped out, resulting in 82 cases in the balloon group and 80 cases in IUD group. The age, menstrual characteristics, pregnancy history, and American Fertility Society score before surgery were comparable between the two groups. The median adhesion score reduction (balloon group, 7; IUD group, 7) and the adhesion reformation rate (balloon group, 30%; IUD group, 35%) were not significantly different between the two groups.The heart-shaped intrauterine balloon and IUD are of similar efficacy in the prevention of adhesion reformation after hysteroscopic adhesiolysis for Asherman syndrome.ISRCTN 69690272.

Abstract:OBJECTIVE:To compare the efficacy of intrauterine balloon, intrauterine contraceptive device and hyaluronic acid gel in the prevention of the adhesion reformation after hysteroscopic adhesiolysis for Asherman's syndrome.STUDY DESIGN:Retrospective cohort study of 107 women with Asherman's syndrome who were treated with hysteroscopic division of intrauterine adhesions. After hysteroscopic adhesiolysis, 20 patients had intrauterine balloon inserted, 28 patients had intrauterine contraceptive device (IUD) fitted, 18 patients had hyaluronic acid gel instilled into the uterine cavity, and 41 control subjects did not have any of the three additional treatment measures. A second-look hysteroscopy was performed in all cases, and the effect of hysteroscopic adhesiolysis was scored by the American Fertility Society classification system.RESULTS:Both the intrauterine balloon group and the IUD group achieved significantly (P<0.001) greater reduction in the adhesion score than that of the hyaluronic acid gel group and control group. The efficacy of the balloon was greater than that of the IUD (P<0.001). There was no significant difference in results between the hyaluronic acid gel group and the control groups.CONCLUSION:The insertion of an intrauterine balloon or intrauterine device is more effective than the use of hyaluronic acid gel in the prevention of intra-uterine adhesion reformation.
The influencing factors of infertility patients with pelvic adhesions and fallopian tube recanalization

Abstract:Objective To analyze the impact factors of infertility patients with pelvic adhesions,observed the effects of pelvic adhesions of fallopian tube recanalization.Methods Collected the clinical data of 386 cases of tubal obstruction infertility patients,divided into adhesions group and control group,adhesions group fallopian tube recanalization sepa ration of adhesions and laparoscopic surgery,the control group fallopian tube recanalization.Fallopian tube recanal ization was observed and pregnancy outcomes.Results Case of infertility >10 years were more,previous miscarriage,induced abortion,ectopic pregnancy,pelvic disease history and the uterine cavity operation history with higher propor tion(P<0.05).Miscarriage,ectopic pregnancy,pelvic disease history and the history of the uterine cavity operation infertility were main factors of pelvic adhesions(P<0.05).Percentage differences of different levels of adhesion ectopic pregnancy,pelvic disease,uterine operation were statistically significant,the frequency of ectopic pregnancy,the pelvic disease rate and uterine number of operations and pelvic adhesions were positively correlated(P<0.05),recanalization rate of adhesions group were lower than the control group(P<0.05),adhesion and control groups after 12 months of normal pregnancy,ectopic pregnancy,miscarriage rate,again blocking rate,the difference were statistically significant(P<0.05),differences of them among different degree of adhesion were statistically significant(P<0.05).Conclusion Miscarriage,ectopic pregnancy,pelvic disease history and the uterine cavity operation history are the main factors affecting of infertility patients with pelvic adhesions.Fallopian tube recanalization rate of pelvic adhesions and normal pregnancy rate after 12 months are low,risks of abortion,ectopic pregnancy,obstruction again are high.
Intrauterine adhesions treated with hysteroscopic adhesiolysis and subsequent obstetric outcome: A retrospective matched cohort study

Abstract:Objective: To examine whether a history of hysteroscopic adhesiolysis (HA)-treated intrauterine adhesions (IUAs) was associated with an increased risk of adverse obstetrical outcomes in subsequent pregnancies. Design: Retrospective cohort study. Setting: A tertiary-care hospital in Shanghai, China. Population: A cohort of 114 142 pregnant women who were issued an antenatal card and received routine antenatal care in Shanghai First Maternity and Infant Hospital, between January 2016 and October 2021. Methods: From the cohort of 114 142 pregnant women, each woman with a history of HA-treated IUA prior to the current pregnancy (n = 780) was matched with four women without a history of IUAs (n = 3010) using propensity score matching. The matching variables were maternal age and parity, mode of conception, pre-pregnancy body mass index and prior history of abortion. Main outcome measures: Pregnancy complications, placental abnormalities, postpartum haemorrhage and adverse birth outcomes. Results: Compared with women with no history of IUAs, women with a history of HA-treated IUAs were at higher risk of pre-eclampsia (RR 1.69, 95% CI 1.23-2.33), placenta accreta spectrum (RR 4.72, 95% CI 3.9-5.73), placenta praevia (RR 4.23, 95% CI 2.85-6.30), postpartum haemorrhage (RR 2.86, 95% CI 1.94-4.23), preterm premature rupture of membranes (RR 3.02, 95% CI 1.97-4.64) and iatrogenic preterm birth (RR 2.86, 95% CI 2.14-3.81). Those women were also more likely to receive cervical cerclage (RR 5.63, 95% CI 3.95-8.02) during pregnancy and haemostatic therapies after delivery (RR 2.17, 95% CI 1.75-2.69). Moreover, we observed that the RRs of those adverse obstetrical outcomes increased with the increasing number of hysteroscopic surgeries. Conclusions: This study found that a history of HA-treated IUAs, especially a history of repeated HAs, was associated with an increased risk of adverse obstetrical outcomes.

Abstract:Structured abstract: Background Asherman's syndrome refers to the presence of intrauterine adhesions which have clinical implications, including infertility. There are few studies in the literature assessing the effect of serial hysteroscopies for adhesiolysis on reproductive and pregnancy outcomes amongst women who subsequently undergo in-vitro fertilization (IVF), and none that have looked at maternal, neonatal or placental pregnancy complications. Objective This study aimed to explore the effect of hysteroscopic adhesiolysis amongst a cohort of patients who subsequently undergo IVF Study design This was a retrospective cohort study of all patients who underwent hysteroscopic adhesiolysis for intrauterine adhesions at our center between 2005-2020, and subsequently attempted conception by IVF. A control group of patients who underwent IVF for non-uterine factor infertility and no prior history of intrauterine adhesions was chosen for comparison. Results There were 691 patients included in this study, of whom 168 were intrauterine adhesion cases. Implantation rate (41.3% in both groups), and live birth rate (aRR 0.93, 95% CI 0.76-1.14) were not statistically different between cases and controls. When grouped by number of previous adhesiolysis surgeries, patients who underwent 2 or more adhesiolysis surgeries did have a lower live birth rate compared to controls (aRR 0.53 (0.28-0.99). Endometrial thickness prior to transfer was significantly reduced in cases versus controls (8.23 vs 10.25mm, aRR 0.84; 95% CI 0.78-0.90). Adverse placental outcomes, including placenta accreta spectrum, placenta previa, or vasa previa, PP or vasa previa) were significantly more likely to occur in cases compared to controls (aRR 2.08; 95% CI 1.25-3.46). When grouped by number of prior adhesiolysis surgeries, the risk appeared to increase as the number of prior surgeries increased. This is likely due to the increased severity of these adhesions. Conclusions Overall, patients with a history of treated intrauterine adhesions have the same live birth rate as patients undergoing IVF for non-uterine factor indications. However, the subgroup of patients who require multiple surgeries for correction of intrauterine adhesions do have a lower live birth rate after IVF compared to controls. Patients with a history of treated intrauterine adhesions are at significantly increased risk of placenta accreta syndrome disorder compared to controls who underwent IVF for non-uterine factor indications.

Pregnancy and obstetric outcomes were compared. There were no significant differences in clinical pregnancies, ectopic pregnancies, live births, preterm births, and obstetric outcomes between the two groups (P>0.05). However, the TCRA group had a higher risk of miscarriage than the control group (P=0.048).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusions</h3><p>TCRA improved the reproductive outcomes of IUA patients, but the risk of miscarriage was higher than that in the general population. To avoid miscarriage, careful monitoring is critical for pregnant patients with a history of TCRA who undergo embryo transfers during IVF treatment.</p>

Abstract:Background: Intrauterine adhesions (IUA) are a challenging clinical problem in reproductive infertility. The most common causes are intrauterine surgery and abortions. We aimed to investigate whether early second-look office hysteroscopy can prevent IUA. Methods: A single-center, prospective, two-armed, randomized controlled trial was designed to explore the efficacy of early office hysteroscopy after first-trimester induced abortion (suction dilatation and curettage [D&C]) and to further analyze fertility outcomes. Women aged 20-45 years undergoing suction D&C and desiring to conceive were recruited. Between October 2019 and September 2022, 66 women were enrolled, of whom 33 were allocated to group A (early hysteroscopy intervention). The women in intervention group A were planned to receive 2 times of hysteroscopies (early and late). In group B, women only underwent late (6 months post suction D&C) hysteroscopy. Results: The primary outcome was the IUA rate assessed using office hysteroscopy 6 months after artificial abortion. Secondary outcomes included menstrual amount/durations and fertility outcomes. In intervention group A, 31 women underwent the first hysteroscopy examination, and 15 completed the second. In group B (late hysteroscopy intervention, 33 patients), 16 completed the hysteroscopic exam 6 months after an artificial abortion. Twenty-one women did not receive late hysteroscopy due to pregnancy. The IUA rate was 16.1% (5/31) at the first hysteroscopy in group A, and no IUA was detected during late hysteroscopy. Neither group showed statistically significant differences in the follow-up pregnancy and live birth rates. Conclusions: Early hysteroscopy following suction D&C can detect intrauterine lesions. IUA detected early by hysteroscopy can disappear on late examination and become insignificant for future pregnancies. Notably, the pregnancy outcomes showed a favorable trend in the early hysteroscopy group, but there were no statistically significant differences. Exploring Risk Factors for Early Pregnancy Loss: A Retrospective Hysteroscopy Study in a Single Institution

Abstract:Background: To explore the risk factors for early pregnancy loss and to study the relationship between early pregnancy loss and intrauterine adhesion (IUA) confirmed by hysteroscopy. Methods: A retrospective study was conducted on 226 early pregnancy loss and 51 early pregnancy patients who received medical management combined with hysteroscopy at a regional institution from March 2020 to February 2021. The uterine cavity’s shape was evaluated by hysteroscopy 6 hours after treatment with medicine. Risk factors included maternal age, gestational weeks, gravidity, parity, number of prior early pregnancy losses, number of prior induced abortions, mean sac diameter (MSD) and IUA. Results: Our results showed that the prevalence of IUA increased significantly in early pregnancy loss cases (31.42%) compared with early pregnancy cases (9.8%) (p < 0.01). Multivariate logistic regression analysis showed that maternal age (odds ratio (OR): 1.195, 95% confidence interval (95% CI): 1.077–1.326), gestational weeks (OR: 2.919, 95% CI: 2.028–4.201) and IUA (OR: 8.631, 95% CI: 2.455–30.336) were positively associated with early pregnancy loss, while MSD (OR: 0.943, 95% CI: 0.899–0.990) and parity (OR: 0.194, 95% CI: 0.088–0.428) were inversely associated with early pregnancy loss. Conclusions: Maternal age, gestational weeks and, IUA were the risk factors for early pregnancy loss. Special attention is to be given to cases of combined IUA when managing early pregnancy loss.

Abstract:Asherman's syndrome, characterized by intrauterine adhesions (IUAs), represents a significant challenge in the field of female infertility. Hysteroscopic adhesiolysis has emerged as the gold standard for both the diagnosis and treatment of Asherman's syndrome. Understanding the intricate relationship between Asherman's syndrome, uterine adhesiolysis, and infertility is crucial for guiding comprehensive and effective management strategies. The success of the treatment is contingent upon preventing adhesion recurrence, particularly in cases of severe IUAs. This is the first case, in which we employed a multifaceted preventive approach, utilizing hyaluronic gel, Foley balloon, hormonal therapy, and platelet-rich plasma, achieving successful pregnancy following embryo transfer despite the presence of severe IUAs. The patient, a 35-year-old female, underwent one cesarean section following in vitro fertilization and required dilation and curettage due to retained products of conception. The patient presented with oligohemorrhage, and the uterine lining was thin while using hormones for endometrial preparation. The diagnosis of severe IUAs was confirmed through ultrasound and hysteroscopic examination of the uterine cavity. The patient underwent hysteroscopic adhesiolysis with a preventive approach using a combination of methods. Subsequently, the patient underwent a second-look hysteroscopy to assess the uterine cavity and achieved successful embryo transfer. The patient carried the pregnancy to 38 weeks and underwent repeated cesarean section due to the vertex-vertex presentation of the twins.
Extended balloon stent placement for reducing intrauterine adhesion recurrence: a retrospective cohort study

Abstract:Research question To compare the efficacy, safety, and reproductive outcomes of intrauterine balloon stent placement for 4 and 6 weeks after hysteroscopic adhesiolysis. Design This retrospective cohort study was conducted at a university-affiliated hospital, and 155 women with moderate-to-severe intrauterine adhesions who underwent hysteroscopic adhesiolysis between March 2016 and December 2019 were included. Patients were divided according to whether the heart-shaped balloon stent was left in place for 4 (Group 1) or 6 weeks (Group 2) after surgery. Stents removed at the second-look hysteroscopy 4 or 6 weeks after surgery were sent for a culture of common bacteria. The incidence of adhesion reformation and adhesion score reduction, bacterial colonisation of the intrauterine balloon stent, live birth rate, and time to live birth were analysed. Results Group 2 had a significantly lower adhesion reformation rate (45.7% vs 28.2%, P =.024) and a more significant reduction in adhesion score (5.2 ± 2.1 vs 6.3 ± 2.2, P =.003) than Group 1. However, no statistical difference was observed in the percentage of bacterial colonisation of the intrauterine balloon stent (55.9% vs 66.7%, P =.174), live birth rate (52.4% vs 42.3%, P =.331), or time to live birth (hazard ratio: 1.09, 95% confidence interval: 0.60–1.96, P =.778) between the two groups. Conclusions Extending intrauterine balloon stent use from 4 to 6 weeks further reduces the adhesion reformation rate after hysteroscopic adhesiolysis in patients with moderate-to-severe intrauterine adhesion. No increase in bacterial colonisation of the balloon stent was observed. Extending the duration of intrauterine balloon stent placement did not significantly affect live birth rates.

Abstract:Objective: This meta-analysis aims to evaluate the efficacy and safety of autologous platelet-rich plasma (PRP) administration in reducing adhesion recurrence and improving pregnancy outcomes in patients with intrauterine adhesion (IUA). Methods: We conducted a comprehensive search of Pubmed, Embase, the Cochrane Library, Web of Science, Scopus, and China National Knowledge Internet (CNKI) from inception to February 10, 2023, without any language or regional restrictions. We used random-effects models to assess odds ratios (OR) and weight mean differences (WMD) with 95% confidence intervals (CI). Results: Our meta-analysis included a total of 730 patients from 10 clinical studies (6 RCTs and 4 non-RCTs). The results showed that PRP administration significantly increased endometrial thickness (WMD = 0.79, 95% CI: 0.40-1.19; P < 0.001; I2 = 0.0%), menstrual volume (WMD = 2.96, 95% CI = 2.30-3.61; P < 0.001; I2 = 0.0%), and days of menstruation (WMD = 1.13, 95% CI = 0.86-1.41; P < 0.001; I2 = 0.0%). Additionally, the clinical pregnancy rate was also improved (OR = 1.82, 95% CI: 1.19-2.78; P = 0.006; I2 = 0.0%). However, there was insufficient evidence to reach a conclusion regarding the effects of PRP on the recurrence rate of moderate to severe IUA, changes in AFS scores, miscarriage rate, and live birth rate. Conclusions: Our analysis confirms that autologous PRP is an effective treatment for IUA. However, the limited sample size suggests that the results should be interpreted with caution. Therefore, larger and well-designed studies are necessary in the future to confirm these findings and explore the optimal PRP dosing regimens further. Reproductive outcomes following copper‐containing intrauterine device after hysteroscopic lysis for intrauterine adhesions

Abstract:The present study aimed to investigate the reproductive outcomes of copper-containing intrauterine devices (IUDs) after hysteroscopic lysis in patients with mild to severe intrauterine adhesions (IUAs), according to the American Fertility Society (AFS) classification. Therefore, a prospective randomized controlled study was conducted at the Affiliated Jinhua Hospital of Wenzhou Medical University (Jinhua, China). A total of 173 women with IUAs were initially recruited between January 2020 and June 2021 and were then randomized to the copper-containing IUD group or the no barrier device group. Following hysteroscopic procedure, the fertility and obstetric outcomes were analyzed. Among the 173 patients enrolled, a total of 109 participants completed the study protocol. The results showed that AFS scores were not significantly different between the two groups prior to hysteroscopy. In addition, no statistically significant differences were recorded in pregnancy and live birth rates between the copper-containing IUD and no barrier device groups. Overall, the results of the current study indicated that the copper-containing IUDs had no positive effect on pregnancy and live birth rates in patients with mild to severe IUAs after hysteroscopic adhesiolysis. Analysis of factors affecting the prognosis of patients with intrauterine adhesions after transcervical resection of adhesions

Abstract:Objectives: To study the factors affecting the prognosis of patients with intrauterine adhesions (IUAs) after transcervical resection of adhesions (TCRA), analyze the reproductive outcome, and guide prognostic improvements. Design: Prospective study. Patients: Our study included 292 patients diagnosed with IUAs who underwent follow-up office hysteroscopy at Shenyang Women's and Children's Hospital between June 2018 and June 2022. Interventions: Patients were divided into case (52 patients whose hysteroscopy results indicated the presence of IUAs) and nocase (240 patients whose uterine cavity had returned to normal shape without obvious adhesion) groups on the basis of the results of a 2-month follow-up hysteroscopy following TCRA. Clinical data were collected and compared with various influencing factors, and the combined effect of these factors was assessed using multifactorial logistic regression analysis. A nomogram prediction model was constructed and internally validated on the basis of multifactorial analysis. Main outcome measures: Intrauterine re-adhesion was observed at a 2-month follow-up after TCRA. Results: Postoperative re-adhesion occurred in 52 of 292 patients with IUAs. Multifactorial binary logistic regression analysis showed that IUA barrier gel reapplication 5 days after TCRA was a protective factor. In contrast, the preoperative American Fertility Society scores demonstrated that severe IUAs and chronic endometritis were risk factors. The results of the multifactorial analysis were used to build a nomogram model, and the area under the curve value of the nomogram model for predicting postoperative recurrence was 0.914 (95% confidence interval: 0.864-0.956). The bootstrap method was subsequently used to resample 1,000 times for internal validation. The results showed that the internal validation C-index was 0.9135, and the calibration and ideal curves were well-matched. Conclusion: The prognosis of patients with IUAs after TCRA is related to the severity of preoperative IUAs, presence of chronic endometritis, and IUA barrier gel reapplication 5 days after TCRA. Therefore, clinicians should monitor patients using targeted data to reduce recurrence risk after TCRA and improve the prognosis of patients with IUAs.
