Abstract:Background: Uncontrollable body movements are typical symptoms of Parkinson's disease (PD), which results in inconsistent findings regarding resting-state functional connectivity (rsFC) networks, especially for group difference clusters. Systematically identifying the motion-associated data was highly demanded. Purpose: To determine data censoring criteria using a quantitative cross validation-based data censoring (CVDC) method and to improve the detection of rsFC deficits in PD. Study Type: Prospective. Subjects: Forty-one PD patients (68.63 +/- 9.17 years, 44% female) and 20 healthy controls (66.83 +/- 12.94 years, 55% female). Field Strength/Sequence: 3-T, T1-weighted gradient echo and EPI sequences. Assessment: Clusters with significant differences between groups were found in three visual networks, default network, and right sensorimotor network. Five-fold cross-validation tests were performed using multiple motion exclusion criteria, and the selected criteria were determined based on cluster sizes, significance values, and Dice coefficients among the cross-validation tests. As a reference method, whole brain rsFC comparisons between groups were analyzed using a FMRIB Software Library (FSL) pipeline with default settings. Statistical Tests: Group difference clusters were calculated using nonparametric permutation statistics of FSL-randomize. The family-wise error was corrected. Demographic information was evaluated using independent sample t-tests and Pearson's Chi-squared tests. The level of statistical significance was set at P < 0.05. Results: With the FSL processing pipeline, the mean Dice coefficient of the network clusters was 0.411, indicating a low reproducibility. With the proposed CVDC method, motion exclusion criteria were determined as frame-wise displacement >0.55 mm. Group-difference clusters showed a mean P-value of 0.01 and a 72% higher mean Dice coefficient compared to the FSL pipeline. Furthermore, the CVDC method was capable of detecting subtle rsFC deficits in the medial sensorimotor network and auditory network that were unobservable using the conventional pipeline. Data Conclusion: The CVDC method may provide superior sensitivity and improved reproducibility for detecting rsFC deficits in PD. Level of Evidence: 1 Technical Efficacy: Stage 2

Abstract:Objectives: Parkinson's disease (PD) is the second most common neurodegenerative disorder which resists around 10 million people worldwide. It develops when nerve cells in a region of the brain that regulates movement become damaged; the symptoms usually begin gradually and become critical over time. In this study, we proposed to investigate the topological properties of functional brain networks within healthy controls (HCs) and PD patients. Also, we evaluated the gender difference among PD patients through graph theoretical approach.Materials and Methods: The rs-fMRI (resting-state functional magnetic resonance imaging) data of fifty-one PD patients and healthy controls was applied to generate the brain functional connectome. The functional whole-brain connectome was constructed by thresholding partial correlation matrices of 160 regions from Dosenbach brain atlas. From the graph theory approach, global and nodal metrics were analysed, and we observed considerable changes in PD patients in comparison with healthy controls.Results: Findings suggest that there is a significant difference in the topological characteristics of PD patients, and this was found to be evident in the default mode network (DMN) and occipital regions.Conclusion: This study provides essential insights from network changes to the clinically relevant information for the PD progression.

Drift in Centrality of Different Brain Regions in an Anatomical Neural Network with Parkinson's Disease: A View from Complex Network Analysis.

Abstract:Understanding the role of brain regions in anatomical neural networks with Parkinson's disease (PD) is essential for improving the clinical protocol or finding new targets for deep brain stimulation (DBS). Although numerous changes have been reported in local functional studies, few studies have reported on the anatomical network of the entire brain. Here, by developing a series of algorithms, this study provided a whole anatomical neural network of the macaque monkey. Then, the drifts in centrality from normal to PD networks were described in terms of complex network analysis and summarized with principal component analysis. Results revealed that the areas including the striatum, globus pallidus, amygdala, prefrontal lobe, thalamus, hippocampus, visual cortex, insula, etc., showed relatively notable drifts in their own patterns. The present study also demonstrated that the current targets of DBS shared a common feature: their centrality values being relatively low in the normal brain while intensely drifting with PD.

Abstract:The purpose of this study is to investigate whether the topological organization of whole-brain functional network is disrupted in patients with Parkinson’s disease (PD). We employed resting-state functional MRI (R-fMRI) and graph theory to investigate the topological organization of the functional connectome in 47 early-stage drug-naïve PD patients and 47 healthy control subjects. Correlations between network properties and clinical variables were tested. Both the PD and control groups showed small-world architecture in brain functional networks. However, the PD patients had lower clustering coefficient and local efficiency relative to control subjects, indicating disrupted topologic organization and a shift toward randomization in their functional brain network. At node and connection level, reduced node centralities and connectivity strength were found mainly in temporal-occipital regions and also in sensorimotor regions of PD patients. In PD patients, altered global network properties correlated with cognitive function, while motor impairment was correlated with local connection changes. This study demonstrates a disruption of whole-brain topological organization of the functional brain networks in early-stage drug-naïve PD patients and this disruption might contribute to preclinical changes in cognitive process in these patients.

Abstract:Parkinson's disease (PD) is a neurodegenerative disorder affecting dopaminergic neurons in the substantia nigra leading to dysfunctional cortico-striato-thalamic-cortical loops. In addition to the characteristic motor symptoms, PD patients often show cognitive impairments, affective changes and other non-motor symptoms, suggesting system-wide effects on brain function. Here, we used functional magnetic resonance imaging and graph-theory based analysis methods to investigate altered whole-brain intrinsic functional connectivity in PD patients (n = 37) compared to healthy controls (n = 20). Global network properties indicated less efficient processing in PD. Analysis of brain network modules pointed to increased connectivity within the sensorimotor network, but decreased interaction of the visual network with other brain modules. We found lower connectivity mainly between the cuneus and the ventral caudate, medial orbitofrontal cortex and the temporal lobe. To identify regions of altered connectivity, we mapped the degree of intrinsic functional connectivity both on ROI- and on voxel-level across the brain. Compared to healthy controls, PD patients showed lower connectedness in the medial and middle orbitofrontal cortex. The degree of connectivity was also decreased in the occipital lobe (cuneus and calcarine), but increased in the superior parietal cortex, posterior cingulate gyrus, supramarginal gyrus and supplementary motor area. Our results on global network and module properties indicated that PD manifests as a disconnection syndrome. This was most apparent in the visual network module. The higher connectedness within the sensorimotor module in PD patients may be related to compensation mechanism in order to overcome the functional deficit of the striato-cortical motor loops or to loss of mutual inhibition between brain networks. Abnormal connectivity in the visual network may be related to adaptation and compensation processes as a consequence of altered motor function. Our analysis approach proved sensitive for detecting disease-related localized effects as well as changes in network functions on intermediate and global scale.

Abstract:Objectives: This study investigates disruptions in functional brain networks in Parkinson's Disease (PD), using advanced modeling and machine learning. Functional networks were constructed using the Nonlinear Autoregressive Distributed Lag (NARDL) model, which captures nonlinear and asymmetric dependencies between regions of interest (ROIs). Key network metrics and information-theoretic measures were extracted to classify PD patients and healthy controls (HC), using deep learning models, with explainability methods employed to identify influential features. Methods: Resting-state fMRI data from the Parkinson's Progression Markers Initiative (PPMI) dataset were used to construct NARDL-based networks. Metrics, such as Degree, Closeness, Betweenness, and Eigenvector Centrality, along with Network Entropy and Complexity, were analyzed. Convolutional Neural Networks (CNNs), Recurrent Neural Networks (RNNs), and Long Short-Term Memory (LSTM) models, classified PD and HC groups. Explainability techniques, including SHAP and LIME, identified significant features driving the classifications. Results: PD patients showed reduced Closeness (22%) and Betweenness Centrality (18%). CNN achieved 91% accuracy, with Network Entropy and Eigenvector Centrality identified as key features. Increased Network Entropy indicated heightened randomness in PD brain networks. Conclusions: NARDL-based analysis with interpretable deep learning effectively distinguishes PD from HC, offering insights into neural disruptions and potential personalized treatments for PD.

Abstract:(1) Background: Directed functional connectivity (DFC) alterations within brain networks are described using fMRI. EEG has been scarcely used. We aimed to explore changes in DFC in the sensory-motor network (SMN), ventral-attention network (VAN), dorsal-attention network (DAN), and central-executive network (CEN) using an EEG-based mapping between PD patients and healthy controls (HCs). (2) Methods: Four-minutes resting EEG was recorded from 29 PD patients and 28 HCs. Network’s hubs were defined using fMRI-based binary masks and their electrical activity was calculated using the LORETA. DFC between each network’s hub-pairs was calculated for theta, alpha and beta bands using temporal partial directed coherence (tPDC). (3) Results: tPDCs percent was lower in the CEN and DAN in PD patients compared to HCs, while no differences were observed in the SMN and VAN (group*network: F = 5.943, p < 0.001) in all bands (group*band: F = 0.914, p = 0.401). However, in the VAN, PD patients showed greater tPDCs strength compared to HCs (p < 0.001). (4) Conclusions: Our results demonstrated reduced connectivity in the CEN and DAN, and increased connectivity in the VAN in PD patients. These results indicate a complex pattern of DFC alteration within major brain networks, reflecting the co-occurrence of impairment and compensatory mechanisms processes taking place in PD.

Abstract:Background: Functional imaging methods, such as resting-state functional magnetic resonance imaging, reflect changes in neural connectivity and may help to assess the widespread consequences of disease-specific network changes in Parkinson's disease. In this study we used a relatively new graph analysis approach in functional imaging: eigenvector centrality mapping. This model-free method, applied to all voxels in the brain, identifies prominent regions in the brain network hierarchy and detects localized differences between patient populations. In other neurological disorders, eigenvector centrality mapping has been linked to changes in functional connectivity in certain nodes of brain networks. Objectives: Examining changes in functional brain connectivity architecture on a whole brain and network level in patients with Parkinson's disease. Methods: Whole brain resting-state functional architecture was studied with a recently introduced graph analysis approach (eigenvector centrality mapping). Functional connectivity was further investigated in relation to eight known resting-state networks. Cross-sectional analyses included group comparison of functional connectivity measures of Parkinson's disease patients (n = 107) with control subjects (n = 58) and correlations with clinical data, including motor and cognitive impairment and a composite measure of predominantly non-dopaminergic symptoms. Results: Eigenvector centrality mapping revealed that frontoparietal regions were more prominent in the whole-brain network function in patients compared to control subjects, while frontal and occipital brain areas were less prominent in patients. Using standard resting-state networks, we found predominantly increased functional connectivity, namely within sensorimotor system and visual networks in patients. Regional group differences in functional connectivity of both techniques between patients and control subjects partly overlapped for highly connected posterior brain regions, in particular in the posterior cingulate cortex and precuneus. Clinico-functional imaging relations were not found. Conclusions: Changes on the level of functional brain connectivity architecture might provide a different perspective of pathological consequences of Parkinson's disease. The involvement of specific, highly connected (hub) brain regions may influence whole brain functional network architecture in Parkinson's disease.

Brain Network Analysis Between Parkinson's Disease and Health Control Based on Edge Functional Connectivity

Abstract:Parkinson's Disease (PD) is the second largest neurodegenerative disease. Brain functional connectivity (FC) studies for PD were useful. In this study, we employed a novel brain network construction method, edge functional connectivity (eFC), to explore FC differences between healthy control (HC) subjects and PD patients. The data used in this study included 34 HCs and 47 PDs from Huashan Hospital, Fudan University, China. Resting state functional magnetic resonance imaging (rsfMRI) and clinical information were selected. Firstly, we constructed eFC brain network and calculated network matrix for the HC and PD groups. Then, we compared brain network matrix between eFC and the traditional nodal functional connectivity (nFC) method. Receiver operating characteristic curve (ROC) analysis was applied to validate the efficiency of the eFC brain network. The results showed that both nFC and eFC brain networks could identify significantly different characteristics between the HC and PD groups. Important hubs were mainly concentrated in visual network, sensorimotor network, subcortex and cerebellum. In addition, new hubs in basal ganglia and cerebellum regions were found in eFC. Furthermore, eFC achieved better classification results (AUC=0.985) than nFC (AUC=0.861) in discriminating PD from CN subjects.

Abstract:In Parkinson's disease (PD), cortical–subcortical interplay plays a relevant role in affecting clinical performance. Functional MRI sequences described changes in functional connectivity at different stages of disease. Scarce are, instead, the investigations examining brain connectivity in patients with PD at early stages of disease. For this aim, here we analyzed the differences in functional connectivity between de novo , never treated, PD patients and healthy controls. The analyses were based upon custom-written scripts on the Matlab platform, combined with high-level functions of Fieldtrip, Brainstorm, and Brain Connectivity toolboxes. First, we proceeded to the spectral analysis of the EEG data in the five frequency bands (δ-θ-α-β-γ). Second, we calculated functional connectivity matrices based on both coherency (COH) and imaginary part of coherency (iCOH), in the δ-θ-α-β-γ frequency bands. Then, four network measures (density, transitivity, global efficiency, and assortativity) were computed in identified connectivity matrices. Finally, we compared the spectral density, functional connectivity matrices, and network measured between healthy controls and de novo PD patients through two-samples T -test. A total of 21 de novo PD patients and 20 healthy subjects were studied. No differences were observed in spectral analysis between the two groups, with the exception of the γ band where a significant increase in power density was found in PD patients. A reduced connectivity in the main EEG frequency bands (α-β frequency bands) was observed in PD patients compared to controls, while a hyperconnectivity was found in PD patients in γ band. Among the network measures, a reduced assortativity coefficient was found in de novo PD patients in α frequency band. Our results show the occurrence of early EEG functional connectivity alterations from the initial stages of PD. From this point of view, connectivity analysis may ease a better understanding of the complexity of PD physiopathology.

[Pharmacoresistant temporal epilepsy: surgical treatment and analysis of factors influencing on its results].

Abstract:An objective of the study is an analysis of the results of surgical treatment and influence of resection volume on the treatment outcome. Sixty-four patients, aged 7-46 years, with an epileptic focus in a temporal lobe have been studied. Patients have been stratified into 2 groups. The first group included 55 patients with epileptogenic lesions of a temporal lobe (monoregional lesions) and the second one--9 patients who, along with the lesion of one temporal lobe, had the signs of lesions in other brain areas (multiregional lesions). In the group of patients with monoregional lesions, a resection of the affected area has been performed. In other patients, the techniques of resection of frontal-medial temporal lobe regions within the scope of the area of steady epileptiform activity have been used. The results have been assessed with the modified Engel scale: class I--an absence of clinically manifested seizures (auras are possible); class II--seizures that do not impact quality of life; class III--a decrease of seizures frequency by more than 80%; class IV--an absence of substantial improvement. In the monoregional lesions, the satisfactory results (I-II classes) were observed in 39 patients (74%), including outcomes of class I (28 patients). In other 16 cases outcomes of classes III and IV were observed. In the multiregional lesions and cryptogenic (possibly symptomatic) partial epilepsy, outcomes of classes III and IV were observed in all patients, regardless of the resection volume. In conclusion, the highest efficacy with regard to frequency and severity of seizures is achieved by resections including the whole area of visualized lesion and border-line area of steady epileptic activity, the identification of which demands the intrasurgery neurophysiologic study. In the extensive and multiregional lesions, the resection surgery confined to one temporal lobe has low effect.
Clinically relevant connectivity features define three subtypes of Parkinson's disease patients

Abstract:Parkinson's disease (PD) is characterized by complex clinical symptoms, including classic motor and nonmotor disturbances. Patients with PD vary in clinical manifestations and prognosis, which point to the existence of subtypes. This study aimed to find the fiber connectivity correlations with several crucial clinical symptoms and identify PD subtypes using unsupervised clustering analysis. One hundred and thirty-four PD patients and 77 normal controls were enrolled. Canonical correlation analysis (CCA) was performed to define the clinically relevant connectivity features, which were then used in the hierarchical clustering analysis to identify the distinct subtypes of PD patients. Multimodal neuroimaging analyses were further used to explore the neurophysiological basis of these subtypes. The methodology was validated in an independent data set. CCA revealed two significant clinically relevant patterns (motor-related pattern and depression-related pattern;r= .94,p < .001 andr= .926,p= .001, respectively) among PD patients, and hierarchical clustering analysis identified three neurophysiological subtypes ("mild" subtype, "severe depression-dominant" subtype and "severe motor-dominant" subtype). Multimodal neuroimaging analyses suggested that the patients in the "severe depression-dominant" subtype exhibited widespread disruptions both in function and structure, while the other two subtypes exhibited relatively mild abnormalities in brain function. In the independent validation, three similar subtypes were identified. In conclusion, we revealed heterogeneous subtypes of PD patients according to their distinct clinically relevant connectivity features. Importantly, depression symptoms have a considerable impact on brain damage in patients with PD.

Abstract:Previous studies have demonstrated a close relationship between early Parkinson's disease and functional network abnormalities. However, the pattern of brain changes in the early stages of Parkinson's disease has not been confirmed, which has important implications for the study of clinical indicators of Parkinson's disease. Therefore, we investigated the functional connectivity before and after treatment in patients with early Parkinson's disease, and further investigated the relationship between some topological properties and clinicopathological indicators. We included resting state-fMRI (rs-fMRI) data from 27 patients with early Parkinson's disease aged 50–75 years from the Parkinson's Disease Progression Markers Initiative (PPMI). The results showed that the functional connectivity of 6 networks, cerebellum network (CBN), cingulo_opercular network (CON), default network (DMN), fronto-parietal network (FPN), occipital network (OCC), and sensorimotor network (SMN), was significantly changed. Compared to before treatment, the main functional connections were concentrated in the CBN after treatment. In addition, the coefficients of these nodes have also changed. For betweenness centrality (BC), the FPN showed a significant improvement in treatment ( p < 0.001). In conclusion, the alteration of functional networks in early Parkinson's patients is critical for clarifying the mechanisms of early diagnosis of the disease.

Unique Brain Network Identification Number for Parkinson's Individuals Using Structural MRI

Abstract:We propose a novel algorithm called Unique Brain Network Identification Number, UBNIN for encoding the brain networks of individual subjects. To realize this objective, we employed structural MRI on 180 Parkinsons disease PD patients and 70 healthy controls HC from the National Institute of Mental Health and Neurosciences, India. We parcellated each subjects brain volume and constructed an individual adjacency matrix using the correlation between the gray matter volumes of every pair of regions. The unique code is derived from values representing connections for every node i, weighted by a factor of 2^1-i. The numerical representation UBNIN was observed to be distinct for each individual brain network, which may also be applied to other neuroimaging modalities. This model may be implemented as a neural signature of a persons unique brain connectivity, thereby making it useful for brainprinting applications. Additionally, we segregated the above datasets into five age cohorts to study the variation in network topology over age. Sparsity was adopted as the threshold estimate to binarize each age-based correlation matrix. For each age cohort, a decreasing trend was observed in the mean clustering coefficient with increasing sparsity. Significantly different clustering coefficients were noted in PD between age cohort B and C, C and E, and in HC between E and B, E and C, E and D, and C and D. Our findings suggest network connectivity patterns change with age, indicating network disruption may be due to the underlying neuropathology. Varying clustering coefficients for different cohorts indicate that information transfer between neighboring nodes changes with age. This provides evidence of age related brain shrinkage and network degeneration. We also discuss limitations and provide an open-access link to software codes and a help file for the entire study.

Abstract:The aim of our study was to investigate differences in whole brain connectivity at different levels between drug-naïve individuals with early Parkinson’s disease (PD) and healthy controls (HCs). Resting-state functional magnetic resonance imaging data were collected from 47 patients with early-stage, drug-naïve PD and 50 HCs. Functional brain connectivity was analyzed at the integrity, network, and edge levels; UPDRS-III, MMSE, MOCA, HAMA, and HAMD scores, reflecting the symptoms of PD, were collected for further regression analysis. Compared with age-matched HCs, reduced functional connectivity were mainly observed in the visual (VSN), somatomotor (SMN), limbic (LBN), and deep gray matter networks (DGN) at integrity level [ p < 0.05, false discovery rate (FDR) corrected]. Intra-network analysis indicated decreased functional connectivity in DGN, SMN, LBN, and ventral attention networks (VAN). Inter-network analysis indicated reduced functional connectivity in nine pairs of resting-state networks. At the edge level, the LBN was the center of abnormal functional connectivity ( p < 0.05, FDR corrected). MOCA score was associated with the intra-network functional connectivity strength (FC) of the DGN, and inter-network FC of the DGN-VAN. HAMA and HAMD scores were associated with the FC of the SMN and DGN, and either the LBN or VAN, respectively. We demonstrated variations in whole brain connections of drug-naïve patients with early PD. Major changes involved the SMN, DGN, LBN, and VSN, which may be relevant to symptoms of early PD. Additionally, our results support PD as a disconnection syndrome.

Abstract:The aim of this study is to explore functional and structural properties of abnormal brain networks associated with Parkinson's disease (PD). F-18-Fluorodeoxyglucose positron emission tomography (F-18-FDG PET) and T1-weighted magnetic resonance imaging from 20 patients with moderate-stage PD and 20 age-matched healthy controls were acquired to identify disease-related patterns in functional and structural networks. Dual-modal images from another prospective subject of 15 PD patients were used as the validation group. Scaled Subprofile Modeling based on principal component analysis method was applied to determine disease-related patterns in both modalities, and brain connectome analysis based on graph theory was applied to verify these patterns. The results showed that the expressions of the metabolic and structural patterns in PD patients were significantly higher than healthy controls (PD1-HC, p = 0.0039, p = 0.0058; PD2-HC, p < 0.001, p = 0.044). The metabolic pattern was characterized by relative increased metabolic activity in pallidothalamic, pons, putamen, and cerebellum, associated with metabolic decreased in parietal-occipital areas. The structural pattern was characterized by relative decreased gray matter (GM) volume in pons, transverse temporal gyrus, left cuneus, right superior occipital gyrus, and right superior parietal lobule, associated with preservation in GM volume in pallidum and putamen. In addition, both patterns were verified in the connectome analysis. The findings suggest that significant overlaps between metabolic and structural patterns provide new evidence for elucidating the neuropathological mechanisms of PD.
Resting-state brain network in Parkinson's disease with different degrees of depression

Abstract:Objective: The aim of this study is to explore the neural network mechanism of Parkinson's disease (PD) with different degrees of depression using independent component analysis (ICA) of the functional connectivity changes in the forehead, limbic system, and basal ganglia regions. Methods: A total of 106 patients with PD were divided into three groups: PD with moderate-severe depression (PDMSD, n = 42), PD with mild depression (PDMD, n = 29), and PD without depression (PDND, n = 35). Fifty gender- and age-matched healthy subjects were recruited as a control group (HC). Three-dimensional T1-weighted image and resting-state functional magnetic resonance imaging (RS-fMRI) data were collected. Results: Different functional connectivity was observed in the left precentral gyrus, right precuneus, right inferior frontal gyrus, right medial and paracingulate gyrus, left supplementary motor area, right brain insula, and the inferior frontal gyrus of the left orbit among the four groups (ANOVA, P 5). Both PDMD and PDMSD exhibited increased functional connectivity in the superior-posterior default-mode network (spDMN) and left frontoparietal network (LFPN); they also exhibited a decreased functional connectivity in the interior Salience Network (inSN) when compared with the PDND group. The functional connectivity within the inSN network was decreased in the PDMSD group when compared with the PDMD group (Alphasim correction, P 5). Conclusion: PD with different degrees of depression has abnormal functional connectivity in multiple networks, which is an important neurobiological basis for the occurrence and development of depression in PD. The degree of decreased functional connectivity in the inSN network is related to the degree of depression in patients with PD-D, which can be an imaging marker for PD to judge the severity of depression.

Abstract:Background. Levodopa-based drugs are widely used for mitigating the complications induced by Parkinson’s disease (PD). Despite the positive effects, several issues regarding the way that levodopa changes brain activities have remained unclear. Methods. A combined strategy using EEG data and graph theory was used for investigating how levodopa changed connectome and processing hubs of the brain during resting-state. Obtained results were subjected to ANOVA test and multiple-comparison post-hoc correction procedure. Results. Outcomes showed that graph topology was not significantly different between PD and healthy groups during the eyes-closed condition, while in the eyes-open condition, statistically significant differences were found. The main effect of levodopa medication was observed for gamma-band activity in which levodopa changed the brain connectome toward a star-like topology. Considering the beta subband of EEG data, graph leaf number increased following levodopa medication in PD patients. Enhanced brain connectivity in the gamma band and reduced beta band connections in the basal ganglia were also observed after levodopa medication. Furthermore, source localization using dipole fitting showed that levodopa suppressed the activity of collateral trigone. Conclusion. Our combined EEG and graph analysis showed that levodopa medication changed the brain connectome, especially in the high-frequency range of brain electrical activities (beta and gamma).

Abstract:This study shows the transition between Cognitive Impairment stages of the Parkinson's disease. Different intra‐ and inter‐network functional connectivity can be observed between groups that represent this transition. Introduction Cognitive impairment (CI) is a significant non‐motor symptoms in Parkinson's disease (PD) that often precedes the emergence of motor symptoms by several years. Patients with PD hypothetically progress from stages without CI (PD‐normal cognition [NC]) to stages with Mild CI (PD‐MCI) and PD dementia (PDD). CI symptoms in PD are linked to different brain regions and neural pathways, in addition to being the result of dysfunctional subcortical regions. However, it is still unknown how functional dysregulation correlates to progression during the CI. Neuroimaging techniques hold promise in discriminating CI stages of PD and further contribute to the biomarker formation of CI in PD. In this study, we explore disparities in the clinical assessments and resting‐state functional connectivity (FC) among three CI stages of PD. Methods We enrolled 88 patients with PD and 26 healthy controls (HC) for a cross sectional clinical study and performed intra‐ and inter‐network FC analysis in conjunction with comprehensive clinical cognitive assessment. Results Our findings underscore the significance of several neural networks, namely, the default mode network (DMN), frontoparietal network (FPN), dorsal attention network, and visual network (VN) and their inter–intra‐network FC in differentiating between PD‐MCI and PDD. Additionally, our results showed the importance of sensory motor network, VN, DMN, and salience network (SN) in the discriminating PD‐NC from PDD. Finally, in comparison to HC, we found DMN, FPN, VN, and SN as pivotal networks for further differential diagnosis of CI stages of PD. Conclusion We propose that resting‐state networks (RSN) can be a discriminating factor in distinguishing the CI stages of PD and progressing from PD‐NC to MCI or PDD. The integration of clinical and neuroimaging data may enhance the early detection of PD in clinical settings and potentially prevent the disease from advancing to more severe stages.

Abstract:Objective: The objective of this study was to use functional connectivity and graphic indicators to investigate the abnormal brain network topological characteristics caused by Parkinson's disease (PD) and the effect of acute deep brain stimulation (DBS) on those characteristics in patients with PD. Methods: We recorded high-density EEG (256 channels) data from 21 healthy controls (HC) and 20 patients with PD who were in the DBS-OFF state and DBS-ON state during the resting state with eyes closed. A high-density EEG source connectivity method was used to identify functional brain networks. Power spectral density (PSD) analysis was compared between the groups. Functional connectivity was calculated for 68 brain regions in the theta (4-8 Hz), alpha (8-13 Hz), beta1 (13-20 Hz), and beta2 (20-30 Hz) frequency bands. Network estimates were measured at both the global (network topology) and local (inter-regional connection) levels. Results: Compared with HC, PSD was significantly increased in the theta (p = 0.003) frequency band and was decreased in the beta1 (p = 0.009) and beta2 (p = 0.04) frequency bands in patients with PD. However, there were no differences in any frequency bands between patients with PD with DBS-OFF and DBS-ON. The clustering coefficient and local efficiency of patients with PD showed a significant decrease in the alpha, beta1, and beta2 frequency bands (p < 0.001). In addition, edgewise statistics showed a significant difference between the HC and patients with PD in all analyzed frequency bands (p < 0.005). However, there were no significant differences between the DBS-OFF state and DBS-ON state in the brain network, except for the functional connectivity in the beta2 frequency band (p < 0.05). Conclusion: Compared with HC, patients with PD showed the following characteristics: slowed EEG background activity, decreased clustering coefficient and local efficiency of the brain network, as well as both increased and decreased functional connectivity between different brain areas. Acute DBS induces a local response of the brain network in patients with PD, mainly showing decreased functional connectivity in a few brain regions in the beta2 frequency band.
