Abstract:Background/aims: The link between child maltreatment (CM) and substance use (SU) in young adulthood is established. The sympathetic nervous system (SNS) division of the autonomic nervous system may mediate this link. However, less is known on the indirect link between CM and SU via SNS functioning. Due to individual variability in the link between SNS functioning and SU risk, we aimed to examine the moderating role of distress tolerance (DT). Methods: A longitudinal sample of 118 young adults (YAs) from a low socioeconomic status background were assessed twice (between 9 and 12 months apart). CM, DT, and galvanic skin response (GSR) stress reactivity were measured during the initial study visit, while SU was assessed at both timepoints. Stress reactivity was assessed by measuring the GSR reactivity during a stress task. We tested the indirect associations between CM and changes in SU problems via GSR stress reactivity, and the moderation effects of DT on these indirect associations. A mediation model in the structural equation modeling (SEM) framework was then followed by a moderated mediation model to analyze these data. Results: YA's stress reactivity mediated the association between CM and alcohol use problems, and this indirect effect was weaker among YAs who had higher levels of DT. This pattern did not emerge with drug use problems. Conclusions: Findings suggest that intervention and prevention efforts for SU outcomes should consider incorporating strategies that increase at-risk individuals' levels of DT. Providing strategies to help individuals stem their stress reactivity may reduce their risk for alcohol use.

Abstract:Childhood maltreatment is a rising social issue worldwide. Notably, childhood maltreatment is suggested to incur non-suicidal self-injury (NSSI), which is also considered as a profound public health concern, especially among Chinese youth and adolescence. Although the relationship between the two has been reported, little is known about the roles of psychological suzhi and inferiority complex in it. The current study aimed to examine the link between childhood maltreatment and NSSI among Chinese adolescents, and determine the mediating role of inferiority complex and the moderating effect of psychological suzhi. A total of 648 students (55.6% female; M age = 14.59) were guided to complete the self-report questionnaires regarding the mentioned study variables. Results demonstrated that childhood maltreatment was positively related with NSSI, and inferiority complex mediated such association. Furthermore, structural equation modeling (SEM) results showed that the effect of high childhood maltreatment on NSSI was weaker for adolescents who reported high psychological suzhi. In addition, the effect of inferiority on NSSI was also weaker for adolescents with high psychological suzhi. The current study demonstrated the indirect effects of inferiority complex and psychological suzhi in the link between childhood maltreatment and NSSI, advancing our understanding of the potential mechanism of such link.

Childhood Maltreatment and Affective Symptoms and Severity of Drug Addiction among Chinese Male Drug Users: Variable-Centered and Person-Centered Approaches

Abstract:Childhood maltreatment has been extensively linked to adult psychopathology, but the specificity and heterogeneity within this association remains inadequately understood. Research with non-Western samples also remains sparse. Using data from Chinese male adult drug users (<i>N</i> = 239) and utilizing both variable-centered and person-centered approaches, this study examined the link between childhood maltreatment (i.e., physical abuse and neglect, emotional abuse and neglect, and sexual abuse) and adult psychopathology (i.e., depressive symptoms, anxious symptoms, and severity of drug addiction). Variable-centered analyses indicated that when different types of maltreatment were considered in a single model, only emotional maltreatment stood out as a risk predictor for psychopathology, especially emotional abuse. Person-centered analyses identified three groups in which various types of maltreatment were configurated within individuals in distinctive patterns: the "rarely maltreated", the "multiply abused", and the "highly neglected" groups. Adult psychopathology varied across profiles. This study highlights the importance of utilizing both variable-centered and person-oriented approaches for a more complete delineation of the link between childhood maltreatment and adult psychopathology. Whereas variable-centered approaches are helpful for obtaining increased specificity, the person-oriented approaches are useful for tackling the heterogeneity. The findings have implications for interventions aimed at assisting drug users in Chinese cultural contexts.
Resilience to substance use disorder following childhood maltreatment: association with peripheral biomarkers of endocannabinoid function and neural indices of emotion regulation

Abstract:Childhood maltreatment (CM) is a risk factor for substance use disorders (SUD) in adulthood. Understanding the mechanisms by which people are susceptible or resilient to developing SUD after exposure to CM is important for improving intervention. This case-control study investigated the impact of prospectively assessed CM on biomarkers of endocannabinoid function and emotion regulation in relation to the susceptibility or resilience to developing SUD. Four groups were defined across the dimensions of CM and lifetime SUD (N = 101 in total). After screening, participants completed two experimental sessions on separate days, aimed at assessing the behavioral, physiological, and neural mechanisms involved in emotion regulation. In the first session, participants engaged in tasks assessing biochemical (i.e., cortisol, endocannabinoids), behavioral, and psychophysiological indices of stress and affective reactivity. During the second session, the behavioral and brain mechanisms associated with emotion regulation and negative affect were investigated using magnetic resonance imaging. CM-exposed adults who did not develop SUD, operationally defined as resilient to developing SUD, had higher peripheral levels of the endocannabinoid anandamide at baseline and during stress exposure, compared to controls. Similarly, this group had increased activity in salience and emotion regulation regions in task-based measures of emotion regulation compared to controls, and CM-exposed adults with lifetime SUD. At rest, the resilient group also showed significantly greater negative connectivity between ventromedial prefrontal cortex and anterior insula compared to controls and CM-exposed adults with lifetime SUD. Collectively, these peripheral and central findings point to mechanisms of potential resilience to developing SUD after documented CM exposure.
The Moderating Role of Family Communication Patterns in the Relationship Between Childhood Trauma and Tendency Toward Addiction in Adolescent Boys

Abstract:Objective: The present study aimed to investigate the moderating role of family communication patterns in the relationship between childhood trauma and the tendency toward addiction in adolescent boys. Methods and Materials: The research method was descriptive-correlational. The statistical population consisted of all adolescents aged 12 to 17 years, from seventh to twelfth grade, in the year 2023. The research sample included 380 individuals selected through multistage cluster sampling. They were evaluated using the Ahvaz Addiction Tendency Questionnaire, the Childhood Trauma Questionnaire (CTQ), and the Revised Family Communication Patterns (RFCP) questionnaire. Data analysis was performed using SPSS 21 software at both descriptive and inferential levels, and hypothesis testing was conducted using hierarchical regression analysis. Findings: The results showed a positive and significant relationship between dimensions of childhood trauma and the tendency toward addiction (p ≤ 0.05), as well as a positive and significant relationship between conformity and the tendency toward addiction (p ≤ 0.05). However, there was a negative and significant relationship between conversation orientation and the tendency toward addiction (p ≤ 0.05). Additionally, family communication patterns moderated the relationship between physical abuse and the tendency toward addiction, as well as the relationship between emotional abuse and the tendency toward addiction. However, family communication patterns did not moderate the relationship between physical neglect and the tendency toward addiction, nor the relationship between emotional neglect and the tendency toward addiction. Family communication patterns did moderate the relationship between sexual abuse and the tendency toward addiction. Conclusion: It can be concluded that family communication patterns mediate the relationship between childhood trauma and the tendency toward addiction in adolescent boys.

Abstract:Background and aim: Many studies have investigated the association between maltreatment and substance use in adulthood. In this study, we sought to explore the association between substance use during adolescence, diverse forms of child maltreatment, and psychological symptoms within a cohort of individuals under the purview of child protection services in France. Method: The dataset was culled from a retrospective, population-based study encompassing minors and young adults aged 0 to 21, who were under the care of child protection services. Specifically, we conducted a comparative analysis between minors exhibiting substance use (N = 72) and those without such use ( N = 776). Result: The odds ratios predominantly illuminated a significant correlation between Substance Use and the manifestation of self-destructive behavior (OR = 4.35; CI 2.02–9.59), as well as aggressive behavior (OR = 5.75; CI 2.87–11.84). Univariate analysis also hinted at an association between SUD and suicidal ideation (OR = 3.52; CI 2.1–5.90). Conclusion: Children in France who are in the care of child protection services and who use psychoactive substances are at greater risk of dropping out of school and of having other psychological symptoms. It is important that the public authorities take account of these results in order to adjust the care given to these minors, who often do not receive psychological support.

Abstract:ABSTRACT Introduction: This paper discusses the efficacy and safety of alendronate and risedronate in the treatment of postmenopausal osteoporosis. Methods: The literature was searched with the PubMed from 1996 to the present, with respect to strictly conducted systematic reviews with homogeneity, meta-analyses with homogeneity, and randomized controlled trials (RCTs) with narrow Confidence Interval. Results: According to the results of large randomized controlled trials (RCTs), bisphosphonates (alendronate, risedronate, and ibandronate), raloxifene, calcitonin, parathyroid hormone (PTH), and strontium ranelate effectively prevent vertebral fractures in postmenopausal women with osteoporosis. Because raloxifene has been shown to be effective in preventing the initial vertebral fracture in postmenopausal osteoporotic women without prevalent vertebral fractures, it is considered in the treatment of postmenopausal women with mild osteoporosis or osteopenia with some risk factors for fractures. RCTs have also demonstrated that alendronate, risedronate, PTH, and strontium are useful to prevent non-vertebral fractures and that alendronate and risedronate prevent hip fractures, thus alendronate or risedronate are primarily considered as the first-line drugs in the treatment of elderly women with osteoporosis having some risk factors for falls. While it has been suggested that PTH may be considered in patients with severe osteoporosis, the use of PTH in the treatment for osteoporosis is limited to 2 years or less, and it may be appropriate to use other anti-resorptive drugs after the completion of PTH treatment to maintain the skeletal effects gained during the treatment. RCTs have demonstrated that the incidence of gastrointestinal tract adverse events in postmenopausal osteoporotic women treated with bisphosphonates and placebo are similar, and also the long-term efficacy and safety of alendronate and risedronate. Conclusion: The evidence derived from the literature, based on strict evidence-based medicine guidelines, suggests that there is long-term efficacy and safety with alendronate and risedronate in the treatment of osteoporosis in postmenopausal women.

Abstract:Background and aims: Cognitive emotion regulation strategies enhance students’ ability to handle emotional and stressful situations and lead to resilient behavior and suitable behavioral performance. The present study aimed to investigate the association of addiction tendency with distress tolerance, self-differentiation, and emotion regulation difficulties mediated by resilience in university students. Methods: This was a descriptive-correlational study using structural equation modeling (SEM). Cluster sampling was used to enroll 314 individuals among all students at the Islamic Azad University of Tehran in the academic year 2020-2021. The research instruments included the Shorter PROMIS Questionnaire (SPQ), the Distress Tolerance Scale-Short Form (DTS-SF), the Self-Other Differentiation Scale (SODS), the Emotion Regulation Questionnaire (ERQ), and Connor-Davidson Resilience Scale (CD-RISC). Data were analyzed by the SEM method using SPSS-26 and SmartPLS. Results: The results showed that distress tolerance (β=0.25, P<0.001) and emotion regulation difficulties (β=0.35, P<0.001) had a significant direct association with resilience. In addition, distress tolerance (β=0.14, P<0.001), self-differentiation (β=0.25, P<0.001), emotion regulation difficulties (β=0.27, P<0.001), and resilience (β=0.22, P<0.001) had a significant direct relationship with addictive tendencies. Moreover, resilience mediated the association of distress tolerance and emotion regulation difficulties with addictive tendencies in college students. Conclusion: The proposed model had the desired fitting based on the results. It is a great step toward understanding factors associated with addiction tendencies in university students. Furthermore, it is an appropriate model, according to which various programs can be developed and designed to prevent addiction tendencies among university students.

Abstract:A double-integrating-spheres system, the basic principle of measuring technology of radiation, and an optical model of biological tissues were used for the study. Optical properties of human normal bladder tissue at 476.5, 488, 496.5, 514.5 and 532 nm of laser and their linearly polarized laser irradiation were studied. The results of measurement showed that total attenuation coefficient and scattering coefficient of human normal bladder tissue at these wavelengths of laser and their linearly polarized laser irradiation increased with decreasing wavelengths. And these was an obvious distinction between the results at these wavelengths of laser and their linearly polarized laser irradiation. Absorption coefficient of human normal bladder tissue at these wavelengths of laser and their linearly polarized laser irradiation was tardily increased with decreasing wavelengths. But there were a number of gurgitations. And these were independent of the wavelengths of laser or their linearly polarized laser irradiation. Mean cosine of scattering of human normal bladder tissue at these wavelengths of laser and their linearly polarized laser irradiation also increased with decreasing wavelengths. And these was an obvious distinction with these wavelengths of laser and their linearly polarized laser irradiation. But penetration depth of human normal bladder tissue at these wavelengths of laser and their linearly polarized laser irradiation also increased with increasing wavelengths. But there were a number of gurgitations. Refractive index of human normal bladder tissue at these wavelengths of laser ranged from 1.37 to 1.44. Absorption coefficient, scattering coefficient, total attenuation coefficient, and effective attenuation coefficients of human normal bladder tissue in Kubelka-Munk two-flux model at the same wavelength of laser and the linearly polarized laser irradiation do not exhibit prominent distinction (P > 0.05). Some absorption coefficient, scattering coefficient, total attenuation coefficient, and effective attenuation coefficient of human normal bladder tissue in Kubelka-Munk two-flux model at different wavelengths of laser and their linearly polarized laser irradiation exhibit obvious distinction.
A Preliminary Investigation of a Conceptual Model Describing the Associations Between Childhood Maltreatment and Alcohol Use Problems

Abstract:Background/Objectives: Childhood maltreatment has been linked to numerous adverse outcomes in adulthood, including problem substance use. However, not all individuals exposed to childhood maltreatment develop substance use problems, indicating the role of other factors in influencing this outcome. Past work suggests that adverse early life experiences, including childhood maltreatment, lead to neurobiological changes in frontolimbic functions that, in turn, result in altered stress and reward responses, heightened impulsivity, affect dysregulation, and, ultimately, increased risk for maladaptive behaviors such as substance use. The aim of this preliminary investigation using cross-sectional data was to test associations between these factors in the relationship between childhood maltreatment and alcohol use problems in a sample of emerging adults. Methods: Emerging adults (18–30 years old) who identified as regular drinkers (i.e., drinking at least 2–4 times in the past month) were recruited from a crowd-sourcing platform (Prolific) as well as community samples. Participants completed online standardized questionnaires assessing reward sensitivity and responsiveness, impulsivity, emotion regulation, and alcohol consequences. Results: Path analyses demonstrated good fit for the data (SRMR = 0.057, RMSEA = 0.096, 90% CI [0.055, 0.142], CFI = 0.957). Childhood maltreatment was associated with reward responsiveness (β = −0.026, Z = −4.222, p < 0.001) and emotion dysregulation (β = 0.669, Z = 9.633, p < 0.001), which in turn was associated with urgency and, subsequently, alcohol consequences (β = 0.758, Z = 7.870, p < 0.001). Conclusions: Although these findings are preliminary, the current study is one of the first to test a comprehensive model addressing the relationship between childhood maltreatment and alcohol use problems. The findings have the potential to inform treatment strategies that target motivation and goal-directed action for reducing and managing consequences associated with childhood maltreatment. Future research should test the model using longitudinal data to address the limitations of a cross-sectional study and assess temporal associations between constructs.

Investigating the Effect of Substance Desire and Child Abuse in Adolescent Suicide Attempt

Abstract:Background: This study aimed to investigate the impact of child abuse and substance desire on adolescent suicide. Methods: This cross-sectional analytical study was conducted on all adolescents aged 12 to 19 who attempted suicide in 2018 at Khorshid Hospital, Isfahan, Iran. A checklist of the patients' demographic information, toxicological data, and 2 standard questionnaires, including substance desire (family, personal, social) and child abuse questionnaire (emotional abuse, physical abuse, and neglect abuse), were collected. The data were analyzed using SPSS version 15. Comparisons between the 2 groups were performed using t tests, chi-square tests, regression analysis, and crude model analysis. Odds ratio (OR) and 95% confidence interval were calculated based on logistic regression. Findings: A total of 196 teenagers were included in the study, with a mean age of 16.48±1.6 years. Out of these, 155 individuals (79.1%) were female, resulting in a female-to-male ratio of 3.78. There was a significant relationship between gender and alcohol consumption in the personal aspect, as well as between the history of psychiatric diseases and alcohol consumption in the physical aspect (P<0.005). The previous history of suicide was the only variable that showed significance in all aspects of both the substance desire and child abuse questionnaires. History of neglect abuse (OR: 1.2, 95% CI [1.07-1.41]; P=0.009) was a predictive factor for suicide attempt. However, being male (OR: 0.12, 95% CI [0.039-0.37], P=0.000), having no psychiatric history (OR: 0.23, 95% CI [0.10-0.52], P<0.001) and not consuming alcohol (OR 0.33, 95% CI 0.15-0.71, P=0.005) were identified as protective factors for attempted suicide. Conclusion: A public health strategy for suicide prevention includes implementing prevention strategies aimed at reducing risk factors, such as alcohol consumption and the risk of neglect abuse.

Abstract:Objective: Polysubstance use as a severity index in individuals with substance abuse disorder is associated with various psychological, social, cultural, and genetic factors. This research aims to examine the predictive role of personality traits, personality functions, traumatic life events, and emotion-regulation difficulties in polysubstance use and the severity of dependence. Methods: A total of 300 subjects (75.2% male) with the mean age of 40.07 years and standard deviation of 12.26 were chosen via the available sampling method. The subjects included individuals abusing one substance (30.6%), a group abusing multiple substances (37.5%), and a nonclinical group (31.7%). The participants completed the questionnaires of short-form personality inventory based on The Diagnostic and Statistical Manual of Mental Disorders, fifth edition, levels of personality functioning scale, dysfunctional emotion regulation scale, and life events checklist. Results: The results of ranked regression and structural equation modeling indicated that the variables of personality traits (2.69), emotional regulation strategies (2.04), personality functioning (1.17), and stressful factors (1.65) had a significant role in predicting drug abuse. Conclusion: Personality affects all life dimensions. Emotion regulation strategies, by affecting maladaptive traits, especially negative affectivity and antagonism, as well as personality functions can predispose the person to polysubstance use.

Abstract:Background: Existing evidence suggests that children and adolescents who had experienced childhood maltreatment (CM) are at higher suicidal risk. However, the mediation role of resilience in this association remains unclear. We aim to investigate the mediation via resilience in the associations between CM and three suicidal risk indicators (suicidal ideation, SI; suicidal plan, SP; suicidal attempt, SA) among a large sample of Chinese children and adolescents. Methods: A population-based cross-sectional survey was conducted in southwestern China Yunnan province. A total of 9723 children and adolescents were included and analyzed by using a multi-stage stratified cluster sampling design. Univariate and multivariate logistic regression models were fitted to explore the associations between CM, resilience, and the three suicidal risk indicators, dose-response trends further elucidated by using the restricted cubic splines. Path models were adopted to estimate the mediation of resilience. Results: The estimated prevalence rates for one-year SI, SP and SA were 32.86% (95% CI: 31.93-33.80%), 19.36% (95% CI: 18.57-20.16%) and 9.07% (95% CI: 8.51-9.66%). After adjustment, CM significantly associated with all 3 suicidal risk indicators, and the odds ratios (ORs) were 2.13 (95% CI: 1.91-2.37), 2.45 (95% CI: 2.13-2.81), and 3.61 (95% CI: 2.90-4.52) for one-year SI, SP, and SA, respectively. Path models revealed that resilience significantly mediated the associations between CM and the three suicidal risk indicators, and among all dimensions of resilience, family support presented the strongest mediation consistently. Conclusions: Our study results suggest that intervention measures which focusing on improving psychological resilience might be effective in reducing suicidal risk for children and adolescents who had experienced maltreatment. Prospective studies should be done to corroborate our findings.

The statistical population of the research comprised all adolescents with an addicted parent in Ahvaz City, Iran, in the school year 2019-2020. In this research, 30 adolescents with an addicted parent were selected through convenience sampling and equally divided into experimental and control groups (n=15). The distress tolerance scale and the sensation-seeking questionnaire were used to collect data. The experimental group underwent MBSR intervention for eight 60-minute sessions. The control group received no intervention. The data were analyzed using analysis of covariance through SPSS software, version 23. Results: The results suggested that MBSR was effective in increasing distress tolerance and reducing sensation seeking of adolescents with an addicted parent in Ahvaz (P<0.001). Conclusion: Accordingly, this treatment can be used to reduce distress and sensation seeking in adolescents with an addicted parent and decrease their stress.
Effect of childhood maltreatment on internet addiction among college students: The mediators of social support and self-esteem

Abstract:Many studies have determined the existence of a close association between childhood maltreatment and internet addiction; however, the internal mechanism has not been fully investigated. This study explored the effect of childhood maltreatment on internet addiction among college students, focusing on the mediating effects of social support and self-esteem. Respondents were 442 college students, who completed the Childhood Trauma Questionnaire–Short Form, the Multidimensional Scale of Perceived Social Support, the Rosenberg Self-Esteem Scale, and the Internet Addiction Test. Results showed that childhood maltreatment, social support, and self-esteem were all significantly correlated with internet addiction. Structural equation modeling results indicated that social support and self-esteem partially mediated the effect of childhood maltreatment on internet addiction. Limitations and implications of the current study are discussed.

Abstract:Although the effects of insecure attachment on vulnerability, incidence, and developing mental disorders have been confirmed by many studies, the mechanism of this effect is still unknown. Therefore, the main aim of this study was to investigate the mediating and moderating role of the three-dimensional model of personality self-regulation in the relationship between insecure attachment and subclinical depression, anxiety, and stress. Four hundred Iranian students at Shiraz University were recruited and completed the following scales: the Revised Adult Attachment, Depression Anxiety Stress, Integrative Self-Knowledge, Mindful Attention Awareness, Self-Control, and Self-Compassion. Results showed that there was a moderate correlation among all the variables under study in the expected directions. Multiple mediating models analyses indicated that regarding the relationship between insecure attachment and depression, the components of integrative self-knowledge, self-control, and self-compassion functioned as mediators. However, regarding the relationship between insecure attachment and anxiety and stress, the components of integrative self-knowledge, mindfulness, and self-compassion relatively functioned as mediators. Further, our results showed that only mindfulness and self-compassion were identified as moderators in the relationship between insecure attachment and depression. It is concluded that insecure attachment may cause psychological damage due to deficiency in the components of the three-dimensional model of personality self-regulation, and that mindfulness and self-compassion may play a protective role in the relationship between insecure attachment and depression.
Mediating role of anxiety and impulsivity in the association between child maltreatment and lifetime non-suicidal self-injury with and without suicidal self-injury

Abstract:Background Child maltreatment can increase the risk of lifetime non-suicidal self-injury (NSSI) and suicidal self-injury (SSI), but there is limited knowledge regarding the differences of potentially psychological mechanisms between NSSI with and without SSI. Methods Participants, 3918 community-based Chinese young men aged 18–34 years in Chengdu, were included in this study. We investigated the association between depression, anxiety, psychosis, child maltreatment, adulthood traumatic events, impulsivity, alcohol dependence, drug abuse, and lifetime of NSSI among participants with and without SSI. Parallel mediation analysis was utilized to explore the mediators for the relation between child maltreatment and NSSI. Results The prevalence of lifetime NSSI was 6.1 % (95 % CI: 5.4 %–6.9 %) among young men. Anxiety and impulsivity partially mediated the effect of child maltreatment on NSSI either with (indirect effect: 51.2 %) or without SSI (indirect effect: 34.3 %). Depression was independently and significantly associated with only NSSI but not with NSSI+SSI. Alcohol dependence and psychosis were independently and significantly associated with NSSI+SSI and mediated the effect of child maltreatment on NSSI+SSI. Limitations The cross-sectional survey data limits the robustness of the proof to the causal relationships. Conclusions Anxiety and impulsivity are associated with NSSI either with or without SSI and partially mediate the effect of child maltreatment on NSSI. Depression is associated with only NSSI, while alcohol dependence and psychosis are only associated with NSSI+SSI. It could be crucial to improve treatment and recovery of alcohol dependence and psychosis for preventing young men engaged in NSSI from attempting SSI.

Abstract:Background Research has demonstrated positive associations between adverse childhood experiences (ACEs) and substance use behaviours in young adults. However, many of these are based on cross-sectional data. The pathway between ACEs and substance misuse among emerging adults is also not fully understood and few studies have investigated potential mediating factors. Methods We analysed data from the Child Cohort of the Growing Up in Ireland study, a nationally representative longitudinal sample of children recruited at 9 years of age (n = 4,729). Logistic regression was used to examine individual and cumulative ACE exposure relationships with substance use at age 20 years. Mediation analyses were used to assess whether parental and peer relationships and school engagement mediate ACE history associations with substance use behaviours. Results ACE exposure relationships with current cannabis use, other illicit drug use and problematic drug use were observed. In particular, young adults who had experienced 2 ACEs or 3 + ACEs had an approximate two-fold (OR=1.80, 95 % CI: 1.29, 2.51) and approximate three-fold (OR=2.94, 95 % CI: 2.06, 4.19) increased odds of problematic drug use, respectively, when compared to participants who had experienced no ACEs during the study period. Mediation analyses suggested that parent and peer attachment and liking school partially mediate relationships between ACEs and substance use behaviours. Conclusions Children who have experienced ACEs are at higher risk of future substance use and problematic drug use. In supporting these children, identified mediators such as interpersonal relationships and school engagement may help guide selection of prevention interventions.

Abstract:Sir, Up to 10% of patients with advanced cancer experience at least one bleeding event during their care [1]. Bleeding may occur directly from the primary tumour or from a metastasis. Some of the factors that confer an increased risk of haemorrhage include frequently prescribed medication such as nonsteroidal anti-inflammatory drugs, heparin and steroids. Patients who are bleeding are often given vitamin K, a practice of uncertain efficacy probably extrapolated from its use in the treatment of bleeding associated with liver failure. In a previous study, we (authors DH and TB) estimated that 22% of patients with advanced cancer are vitamin K deficient [2]. The suboptimal availability or impaired utilization of vitamin K impedes the c-carboxylation of vitamin K-dependent (VKD) proteins, leading to the release into the circulation of undercarboxylated VKD proteins that have reduced biological activity (known as PIVKAs, protein-induced vitamin K absence) [3]. Anorexia is common in patients with advanced cancer. Poor appetite can rapidly deplete hepatic stores of vitamin K potentially exposing patients with chronic anorexia to the development of vitamin K deficiency and increased haemorrhagic risk [4–6]. Some cancers directly suppress appetite, as can chemotherapy which is also associated with epithelial damage to the gastrointestinal tract, resulting in fat-soluble vitamin malabsorption. Vitamin K deficiency may also be induced by metastases to the liver which prevent bile or pancreatic secretions critical for the solubilization and absorption of dietary vitamin K. Surgery presents additional challenges, particularly if the proximal intestine is removed, because this greatly impairs vitamin K absorption. Bone marrow transplantation is also linked with a rapid depletion of vitamin K stores [7]. To better understand the role that vitamin K deficiency may play in the increased risk of haemorrhage in advanced cancer patients, we evaluated the vitamin K status of eight adults referred to the Guys and St Thomas’s NHS Foundation Trust palliative care inpatient team (between July 2009 and June 2011) with a bleeding episode within the last 24 h. The inclusion and exclusion criteria are shown in Table 1. Recruitment to this study was challenging. Of the 52 potential patient recruits admitted with recent bleeding, 42 could not be included because of their deteriorating medical condition or previous treatment. Two further patients did not complete the protocol.
