Purpose To develop and validate a deep learning (DL) method to detect and segment enhancing and nonenhancing cellular tumor on pre- and posttreatment MRI scans of patients with glioblastoma and to predict overall survival (OS) and progression-free survival (PFS). Materials and Methods This retrospective study included 1397 MRIs in 1297 patients with glioblastoma, including an internal cohort of 243 MRIs (January 2010-June 2022) for model training and cross-validation and four external test cohorts. Cellular tumor maps were segmented by two radiologists based on imaging, clinical history, and pathology. Multimodal MRI with perfusion and multishell diffusion imaging were inputted into a nnU-Net DL model to segment cellular tumor. Segmentation performance (Dice score) and performance in detecting recurrent tumor from posttreatment changes (area under the receiver operating characteristic curve [AUC]) were quantified. Model performance in predicting OS and PFS was assessed using Cox multivariable analysis. Results A cohort of 178 patients (mean age, 56 years ± [SD]13; 121 male, 57 female) with 243 MRI timepoints, as well as four external datasets with 55, 70, 610 and 419 MRI timepoints, respectively, were evaluated. The median Dice score was 0.79 (IQR:0.53-0.89) and the AUC for detecting residual/recurrent tumor was 0.84 (95% CI:0.79- 0.89). In the internal test set, estimated cellular tumor volume was significantly associated with OS (hazard ratio [HR] = 1.04/mL, P < .001) and PFS (HR = 1.04/mL, P < .001) when adjusting for age, sex and gross total resection status. In the external test sets, estimated cellular tumor volume was significantly associated with OS (HR = 1.01/mL, P < .001) when adjusting for age, sex and gross total resection status. Conclusion A DL model incorporating advanced imaging could accurately segment enhancing and nonenhancing cellular tumor, classify recurrent/residual tumor from posttreatment changes, and predict OS and PFS in patients with glioblastoma. ©RSNA, 2024.

Abstract:Extent of resection after surgery is one of the main prognostic factors for patients diagnosed with glioblastoma. To achieve this, accurate segmentation and classification of residual tumor from post-operative MR images is essential. The current standard method for estimating it is subject to high inter- and intra-rater variability, and an automated method for segmentation of residual tumor in early post-operative MRI could lead to a more accurate estimation of extent of resection. In this study, two state-of-the-art neural network architectures for pre-operative segmentation were trained for the task. The models were extensively validated on a multicenter dataset with nearly 1000 patients, from 12 hospitals in Europe and the United States. The best performance achieved was a 61\% Dice score, and the best classification performance was about 80\% balanced accuracy, with a demonstrated ability to generalize across hospitals. In addition, the segmentation performance of the best models was on par with human expert raters. The predicted segmentations can be used to accurately classify the patients into those with residual tumor, and those with gross total resection.

Auto-segmentation of Adult-Type Diffuse Gliomas: Comparison of Transfer Learning-Based Convolutional Neural Network Model Vs. Radiologists

Abstract:Segmentation of glioma is crucial for quantitative brain tumor assessment, to guide therapeutic research and clinical management, but very time-consuming. Fully automated tools for the segmentation of multi-sequence MRI are needed. We developed and pretrained a deep learning (DL) model using publicly available datasets A ( n = 210) and B ( n = 369) containing FLAIR, T2WI, and contrast-enhanced (CE)-T1WI. This was then fine-tuned with our institutional dataset ( n = 197) containing ADC, T2WI, and CE-T1WI, manually annotated by radiologists, and split into training ( n = 100) and testing ( n = 97) sets. The Dice similarity coefficient (DSC) was used to compare model outputs and manual labels. A third independent radiologist assessed segmentation quality on a semi-quantitative 5-scale score. Differences in DSC between new and recurrent gliomas, and between uni or multifocal gliomas were analyzed using the Mann–Whitney test. Semi-quantitative analyses were compared using the chi-square test. We found that there was good agreement between segmentations from the fine-tuned DL model and ground truth manual segmentations (median DSC: 0.729, std-dev: 0.134). DSC was higher for newly diagnosed (0.807) than recurrent (0.698) ( p < 0.001), and higher for unifocal (0.747) than multi-focal (0.613) cases ( p = 0.001). Semi-quantitative scores of DL and manual segmentation were not significantly different (mean: 3.567 vs. 3.639; 93.8% vs. 97.9% scoring ≥ 3, p = 0.107). In conclusion, the proposed transfer learning DL performed similarly to human radiologists in glioma segmentation on both structural and ADC sequences. Further improvement in segmenting challenging postoperative and multifocal glioma cases is needed.
A Fully Automated Post-Surgical Brain Tumor Segmentation Model for Radiation Treatment Planning and Longitudinal Tracking

Abstract:Glioblastoma (GBM) has a poor survival rate even with aggressive surgery, concomitant radiation therapy (RT), and adjuvant chemotherapy. Standard-of-care RT involves irradiating a lower dose to the hyperintense lesion in T2-weighted fluid-attenuated inversion recovery MRI (T2w/FLAIR) and a higher dose to the enhancing tumor on contrast-enhanced, T1-weighted MRI (CE-T1w). While there have been several attempts to segment pre-surgical brain tumors, there have been minimal efforts to segment post-surgical tumors, which are complicated by a resection cavity and postoperative blood products, and tools are needed to assist physicians in generating treatment contours and assessing treated patients on follow up. This report is one of the first to train and test multiple deep learning models for the purpose of post-surgical brain tumor segmentation for RT planning and longitudinal tracking. Post-surgical FLAIR and CE-T1w MRIs, as well as their corresponding RT targets (GTV1 and GTV2, respectively) from 225 GBM patients treated with standard RT were trained on multiple deep learning models including: Unet, ResUnet, Swin-Unet, 3D Unet, and Swin-UNETR. These models were tested on an independent dataset of 30 GBM patients with the Dice metric used to evaluate segmentation accuracy. Finally, the best-performing segmentation model was integrated into our longitudinal tracking web application to assign automated structured reporting scores using change in percent cutoffs of lesion volume. The 3D Unet was our best-performing model with mean Dice scores of 0.72 for GTV1 and 0.73 for GTV2 with a standard deviation of 0.17 for both in the test dataset. We have successfully developed a lightweight post-surgical segmentation model for RT planning and longitudinal tracking.

Abstract:Glioblastoma changes during chemoradiotherapy are inferred from high-field MRI before and after treatment but are rarely investigated during radiotherapy. The purpose of this study was to develop a deep learning network to automatically segment glioblastoma tumors on daily treatment set-up scans from the first glioblastoma patients treated on MRI-linac. Glioblastoma patients were prospectively imaged daily during chemoradiotherapy on 0.35T MRI-linac. Tumor and edema (tumor lesion) and resection cavity kinetics throughout the treatment were manually segmented on these daily MRI. Utilizing a convolutional neural network, an automatic segmentation deep learning network was built. A nine-fold cross-validation schema was used to train the network using 80:10:10 for training, validation, and testing. Thirty-six glioblastoma patients were imaged pre-treatment and 30 times during radiotherapy (n = 31 volumes, total of 930 MRIs). The average tumor lesion and resection cavity volumes were 94.56 ± 64.68 cc and 72.44 ± 35.08 cc, respectively. The average Dice similarity coefficient between manual and auto-segmentation for tumor lesion and resection cavity across all patients was 0.67 and 0.84, respectively. This is the first brain lesion segmentation network developed for MRI-linac. The network performed comparably to the only other published network for auto-segmentation of post-operative glioblastoma lesions. Segmented volumes can be utilized for adaptive radiotherapy and propagated across multiple MRI contrasts to create a prognostic model based for glioblastoma on multiparametric MRI.

Abstract:Glioblastoma multiforme (GBM) is the most prevalent and aggressive primary brain tumour that has the worst prognosis in adults. Currently, the automatic segmentation of this kind of tumour is being intensively studied. Here, the automatic three‐dimensional segmentation of the GBM is achieved with its related subzones (active tumour, inner necrosis, and peripheral oedema). Preliminary segmentations were first defined based on the four basic magnetic resonance imaging modalities and classic image processing methods (multithreshold Otsu, Chan–Vese active contours, and morphological erosion). After an automatic gap‐filling post processing step, these preliminary segmentations were combined and corrected by a supervised artificial neural network of multilayer perceptron type with a hidden layer of 80 neurons, fed by 30 selected radiomic features of gray intensity and texture. Network classification has an overall accuracy of 83.9%, while the complete combined algorithm achieves average Dice similarity coefficients of 89.3%, 80.7%, 79.7%, and 66.4% for the entire region of interest, active tumour, oedema, and necrosis segmentations, respectively. These values are in the range of the best reported in the present bibliography, but even with better Hausdorff distances and lower computational costs. Results presented here evidence that it is possible to achieve the automatic segmentation of this kind of tumour by traditional radiomics. This has relevant clinical potential at the time of diagnosis, precision radiotherapy planning, or post‐treatment response evaluation.

Abstract:Background Neuroimaging differentiation of glioblastoma, primary central nervous system lymphoma (PCNSL) and solitary brain metastasis (BM) remains challenging in specific cases showing similar appearances or atypical features. Overall, advanced MRI protocols have high diagnostic reliability, but their limited worldwide availability, coupled with the overlapping of specific neuroimaging features among tumor subgroups, represent significant drawbacks and entail disparities in the planning and management of these oncological patients. Objective To evaluate the classification performance metrics of a deep learning algorithm trained on T1-weighted gadolinium-enhanced (T1Gd) MRI scans of glioblastomas, atypical PCNSLs and BMs. Materials and Methods We enrolled 121 patients (glioblastoma: n=47; PCNSL: n=37; BM: n=37) who had undergone preoperative T1Gd-MRI and histopathological confirmation. Each lesion was segmented, and all ROIs were exported in a DICOM dataset. The patient cohort was then split in a training and hold-out test sets following a 70/30 ratio. A Resnet101 model, a deep neural network (DNN), was trained on the training set and validated on the hold-out test set to differentiate glioblastomas, PCNSLs and BMs on T1Gd-MRI scans. Results The DNN achieved optimal classification performance in distinguishing PCNSLs (AUC: 0.98; 95%CI: 0.95 - 1.00) and glioblastomas (AUC: 0.90; 95%CI: 0.81 - 0.97) and moderate ability in differentiating BMs (AUC: 0.81; 95%CI: 0.70 - 0.95). This performance may allow clinicians to correctly identify patients eligible for lesion biopsy or surgical resection. Conclusion We trained and internally validated a deep learning model able to reliably differentiate ambiguous cases of PCNSLs, glioblastoma and BMs by means of T1Gd-MRI. The proposed predictive model may provide a low-cost, easily-accessible and high-speed decision-making support for eligibility to diagnostic brain biopsy or maximal tumor resection in atypical cases.

Deep Learning-Based Automated Post-Operative Gross Tumor Volume Segmentation in Glioblastoma Patients

Abstract:Precise automated delineation of post-operative gross tumor volume in glioblastoma cases is challenging and time-consuming owing to the presence of edema and the deformed brain tissue resulting from the surgical tumor resection. To develop a model for automated delineation of post-operative gross tumor volumes in glioblastoma, we proposed a novel 3D double pocket U-Net architecture that has two parallel pocket U-Nets. Both U-Nets were trained simultaneously with two different subsets of MRI sequences and the output from the models was combined to do the final prediction. We strategically combined the MRI input sequences (T1, T2, T1C, FL) for model training to achieve improved segmentation accuracy. The dataset comprised 82 post-operative studies collected from 23 glioblastoma patients who underwent maximal safe tumor resection. All had gross tumor volume (GTV) segmentations performed by human experts, and these were used as a reference standard. The results of 3D double pocket U-Net were compared with baseline 3D pocket U-Net models and the ensemble of 3D pocket U-Net models. All the models were evaluated with fivefold cross-validation in terms of the Dice similarity coefficient and Hausdorff distance. Our proposed double U-Net model trained with input sequences [T1, T1C, FL + T2, T1C] achieved a better mean Dice score of 0.8585 and Hausdorff distance of 4.1942 compared to all the baseline models and ensemble models trained. The presence of infiltrating tumors and vasogenic edema in the post-operative MRI scans tends to reduce segmentation accuracy when considering the MRI sequences T1, T2, T1C, and FL together for model training. The double U-Net approach of combining subsets of the MRI sequences as distinct inputs for model training improves segmentation accuracy by 7% when compared with the conventional method of model training with all four sequences.

Abstract:Abstract: The treatment for bilateral testicular tumors is associated with infertility and hypogonadism. We carried out testis‐preserving therapy treatment for three patients with synchronous bilateral tumors with seminoma. Our protocol of testis preservation consisted of unilateral orchiectomy for the larger tumor followed by three courses of chemotherapy with a cisplatin, etoposide and bleomycin regimen. All three patients showed no recurrence 24, 24, and 82 months after treatment. The serum testosterone level was maintained around the lower limit of the normal range in all patients without androgen replacement, and sexual function was not impaired. One patient showed oligospermia, preoperatively, and the number of sperm was preserved at a similar level after treatment. Azoospermia was observed in two patients before the treatment, and still persisted after treatment. Testis preservation with a chemotherapy‐based new protocol was carried out successfully without recurrence. In addition, our protocol preserved a good quality of life in all patients.
ReGiSegNet: a Post-Operative Glioma Segmentation Based on Magnetic Resonance Imaging

Abstract:Glioblastoma is a highly malignant tumor. In recent years, many scholars have conducted research on the automatic segmentation of preoperative primary glioblastoma magnetic resonance imaging (MRI) and achieved good results. The automatic segmentation of postoperative residual glioblastoma MRI plays a crucial role in treatment planning. However, there is still no research specifically focusing on the segmentation of postoperative residual glioblastoma MRI due to the limited data and the difficulty in establishing standards. In this study, a large amount of preoperative tumor data was utilized to pretrain the segmentation model. Postoperative residual tumor MRI data from 53 patients were collected and annotated by medical students specializing in radiology. The pre-trained segmentation model was then applied to segment the postoperative residual tumor data, obtaining preliminary segmentation results that roughly indicate the location of the residual tumor. Based on the similarity between the preliminary segmentation results and the residual tumor annotations, a simple and effective active learning strategy is designed to select the cases that need to be reannotated. The preliminary segmentation results, along with the postoperative residual tumor data, were fed into a new segmentation network to achieve precise segmentation of the residual tumor after surgery. Ultimately, the proposed network achieved a Dice coefficient of 0.871 for the segmentation of residual tumor data.
Assessment of Glioma Response to Radiotherapy Using Multiple MRI Biomarkers with Manual and Semiautomated Segmentation Algorithms

Abstract:Background and purpose: Multimodality magnetic resonance imaging (MRI) can provide complementary information in the assessment of brain tumors. We aimed to segment tumor in amide proton transfer-weighted (APTw) images and to investigate multiparametric MRI biomarkers for the assessment of glioma response to radiotherapy. For tumor extraction, we evaluated a semiautomated segmentation method based on region of interest (ROI) results by comparing it with the manual segmentation method. Methods: Thirteen nude rats injected with U87 tumor cells were irradiated by an 8-Gy radiation dose. All MRI scans were performed on a 4.7-T animal scanner preradiation, and at day 1, day 4, and day 8 postradiation. Two experts performed manual and semiautomated methods to extract tumor ROIs on APTw images. Multimodality MRI signals of the tumors, including structural (T2 and T1 ), functional (apparent diffusion coefficient and blood flow), and molecular (APTw and magnetization transfer ratio or MTR), were calculated and compared quantitatively. Results: The semiautomated method provided more reliable tumor extraction results on APTw images than the manual segmentation, in less time. A considerable increase in the ADC intensities of the tumor was observed during the postradiation. A steady decrease in the blood flow values and in the APTw signal intensities were found after radiotherapy. Conclusions: The semiautomated method of tumor extraction showed greater efficiency and stability than the manual method. Apparent diffusion coefficient, blood flow, and APTw are all useful biomarkers in assessing glioma response to radiotherapy.

Abstract:Arachnoid cysts of the spinal canal are relatively common lesions that may be either intra- or extradural. These cysts are usually asymptomatic but may produce symptoms by compressing the spinal cord or nerve roots. We report a case in which an intradural thoracic arachnoid cyst became symptomatic after a routine decompressive lumbar laminectomy for spinal stenosis. Myelography revealed no abnormality, although magnetic resonance imaging and computed tomography after myelography demonstrated a mass within the posterior aspect of the thoracic spinal canal associated with anterior displacement and compression of the spinal cord. A change in the flow dynamics of the cerebrospinal fluid probably allowed the development of spinal cord compression due to one of the following: expansion of the cyst, decreased cerebrospinal fluid buffer between the cord and the cyst, or epidural venous engorgement. A concomitant and more cephalad lesion such as an arachnoid cyst should be considered when myelopathic complications arise after lumbar surgery. Magnetic resonance imaging and computed tomography after myelography are useful to demonstrate the additional pathological processes.
Automatic Brain Tumor Segmentation And Overall Survival Prediction Using Machine Learning Algorithms

Abstract:Purpose: This study was designed to evaluate the ability of a U-net neural net-work to properly identify three regions of a brain tumor and an ELM for the prediction of patient overall survival after gross tumor resection using preoperative MR images.Methods: 210 GBM patients were used for training, while 66 LGG and GBM patients were used for validation. Multiple preprocessing steps were performed on each patient's data before loading them into the model. The segmentation model consists of three different U-nets, one for each region of interest. These created segmentations were then analyzed by use of common quantitative metrics with respect to physician created contours. Regarding the patient overall survival prediction, 59 high grade glioma patients with gross total resection (GTR) were provided for training. 28 patients with GTR were used to validate the algorithm.Results: The average [s.d] DSC for the whole tumor, enhanced tumor, and tumor core contours were 0.882 [0.080], 0.712 [0.294], and 0.769 [0.263], respectively. The average [s.d.] Hausdorff distance were 7.09 [11.57], 4.46 [8.32], and 9.57 [14.08], respectively. The average [s.d.] sensitivity for the whole tumor, enhanced tumor, and tumor core contours were 0.887 [0.126], 0.770 [0.245], and 0.750 [0.293], respectively. The average [s.d.] specificity was 0. 993 [0.005], 0.998 [0.003], 0.998 [0.002], respectively. The predictive power of patient overall survival is 0.607 using an extreme learning machine algorithm.Conclusion: The U-Net model was very effective in determining accurate location of the whole tumor and segmenting the whole tumor, enhancing tumor and tumor core. The most predictive features of patient overall survival are both age and location of the tumor when all 163 validation cases were utilized.
Robust Deep Learning-based Segmentation of Glioblastoma on Routine Clinical MRI Scans Using Sparsified Training

Abstract:Purpose: To improve the robustness of deep learning-based glioblastoma segmentation in a clinical setting with sparsified datasets. Materials and methods: In this retrospective study, preoperative T1-weighted, T2-weighted, T2-weighted fluid-attenuated inversion recovery, and postcontrast T1-weighted MRI from 117 patients (median age, 64 years; interquartile range [IQR], 55-73 years; 76 men) included within the Multimodal Brain Tumor Image Segmentation (BraTS) dataset plus a clinical dataset (2012-2013) with similar imaging modalities of 634 patients (median age, 59 years; IQR, 49-69 years; 382 men) with glioblastoma from six hospitals were used. Expert tumor delineations on the postcontrast images were available, but for various clinical datasets, one or more sequences were missing. The convolutional neural network, DeepMedic, was trained on combinations of complete and incomplete data with and without site-specific data. Sparsified training was introduced, which randomly simulated missing sequences during training. The effects of sparsified training and center-specific training were tested using Wilcoxon signed rank tests for paired measurements. Results: A model trained exclusively on BraTS data reached a median Dice score of 0.81 for segmentation on BraTS test data but only 0.49 on the clinical data. Sparsified training improved performance (adjusted P < .05), even when excluding test data with missing sequences, to median Dice score of 0.67. Inclusion of site-specific data during sparsified training led to higher model performance Dice scores greater than 0.8, on par with a model based on all complete and incomplete data. For the model using BraTS and clinical training data, inclusion of site-specific data or sparsified training was of no consequence. Conclusion: Accurate and automatic segmentation of glioblastoma on clinical scans is feasible using a model based on large, heterogeneous, and partially incomplete datasets. Sparsified training may boost the performance of a smaller model based on public and site-specific data.Supplemental material is available for this article.Published under a CC BY 4.0 license.
Automated post-operative brain tumour segmentation: A deep learning model based on transfer learning from pre-operative images

Abstract:Automated brain tumour segmentation from post-operative images is a clinically relevant yet challenging problem. In this study, an automated method for segmenting brain tumour into its subregions has been developed. The dataset consists of multimodal post-operative brain scans (T1 MRI, post-Gadolinium T1 MRI, and T2-FLAIR images) of 15 patients who were treated with post-operative radiation therapy, along with manual annotations of their tumour subregions. A 3D densely-connected U-net was developed for segmentation of brain tumour regions and extensive experiments were conducted to enhance model accuracy. A model was initially developed using the publicly available BraTS dataset consisting of pre-operative brain scans. This model achieved Dice Scores of 0.90, 0.83 and 0.78 for predicting whole tumour, tumour core, and enhancing tumour subregions when tested on BraTS20 blind validation dataset. The acquired knowledge from BraTS was then transferred to the local dataset. For augmentation purpose, the local dataset was registered to a dataset of MRI brain scans of healthy subjects. To improve the robustness of the model and enhance its accuracy, ensemble learning was used to combine the outputs of all the trained models. Even though the size of the dataset is very small, the final model can segment brain tumours with a high Dice Score of 0.83, 0.77 and 0.60 for whole tumour, tumour core and enhancing core respectively.

Abstract:(1) Background: Gliomas are the most common primary brain neoplasms accounting for roughly 40–50% of all malignant primary central nervous system tumors. We aim to develop a deep learning-based framework for automated segmentation and prediction of biomarkers and prognosis in patients with gliomas. (2) Methods: In this retrospective two center study, patients were included if they (1) had a diagnosis of glioma with known surgical histopathology and (2) had preoperative MRI with FLAIR sequence. The entire tumor volume including FLAIR hyperintense infiltrative component and necrotic and cystic components was segmented. Deep learning-based U-Net framework was developed based on symmetric architecture from the 512 × 512 segmented maps from FLAIR as the ground truth mask. (3) Results: The final cohort consisted of 208 patients with mean ± standard deviation of age (years) of 56 ± 15 with M/F of 130/78. DSC of the generated mask was 0.93. Prediction for IDH-1 and MGMT status had a performance of AUC 0.88 and 0.62, respectively. Survival prediction of <18 months demonstrated AUC of 0.75. (4) Conclusions: Our deep learning-based framework can detect and segment gliomas with excellent performance for the prediction of IDH-1 biomarker status and survival.

Abstract:For patients suffering from brain tumor, prognosis estimation and treatment decisions are made by a multidisciplinary team based on a set of preoperative MR scans. Currently, the lack of standardized and automatic methods for tumor detection and generation of clinical reports represents a major hurdle. In this study, we investigate glioblastomas, lower grade gliomas, meningiomas, and metastases, through four cohorts of up to 4000 patients. Tumor segmentation models were trained using the AGU-Net architecture with different preprocessing steps and protocols. Segmentation performances were assessed in-depth using a wide-range of voxel and patient-wise metrics covering volume, distance, and probabilistic aspects. Finally, two software solutions have been developed, enabling an easy use of the trained models and standardized generation of clinical reports: Raidionics and Raidionics-Slicer. Segmentation performances were quite homogeneous across the four different brain tumor types, with an average true positive Dice ranging between 80% and 90%, patient-wise recall between 88% and 98%, and patient-wise precision around 95%. With our Raidionics software, running on a desktop computer with CPU support, tumor segmentation can be performed in 16 to 54 seconds depending on the dimensions of the MRI volume. For the generation of a standardized clinical report, including the tumor segmentation and features computation, 5 to 15 minutes are necessary. All trained models have been made open-access together with the source code for both software solutions and validation metrics computation. In the future, an automatic classification of the brain tumor type would be necessary to replace manual user input. Finally, the inclusion of post-operative segmentation in both software solutions will be key for generating complete post-operative standardized clinical reports.

Abstract:Abstract Background Longitudinal measurement of glioma burden with MRI is the basis for treatment response assessment. In this study, we developed a deep learning algorithm that automatically segments abnormal fluid attenuated inversion recovery (FLAIR) hyperintensity and contrast-enhancing tumor, quantitating tumor volumes as well as the product of maximum bidimensional diameters according to the Response Assessment in Neuro-Oncology (RANO) criteria (AutoRANO). Methods Two cohorts of patients were used for this study. One consisted of 843 preoperative MRIs from 843 patients with low- or high-grade gliomas from 4 institutions and the second consisted of 713 longitudinal postoperative MRI visits from 54 patients with newly diagnosed glioblastomas (each with 2 pretreatment “baseline” MRIs) from 1 institution. Results The automatically generated FLAIR hyperintensity volume, contrast-enhancing tumor volume, and AutoRANO were highly repeatable for the double-baseline visits, with an intraclass correlation coefficient (ICC) of 0.986, 0.991, and 0.977, respectively, on the cohort of postoperative GBM patients. Furthermore, there was high agreement between manually and automatically measured tumor volumes, with ICC values of 0.915, 0.924, and 0.965 for preoperative FLAIR hyperintensity, postoperative FLAIR hyperintensity, and postoperative contrast-enhancing tumor volumes, respectively. Lastly, the ICCs for comparing manually and automatically derived longitudinal changes in tumor burden were 0.917, 0.966, and 0.850 for FLAIR hyperintensity volume, contrast-enhancing tumor volume, and RANO measures, respectively. Conclusions Our automated algorithm demonstrates potential utility for evaluating tumor burden in complex posttreatment settings, although further validation in multicenter clinical trials will be needed prior to widespread implementation.

Clinical capability of modern brain tumor segmentation models

Abstract:Purpose: State-of-the-art automated segmentation methods achieve exceptionally high performance on the Brain Tumor Segmentation (BraTS) challenge, a dataset of uniformly processed and standardized magnetic resonance generated images (MRIs) of gliomas. However, a reasonable concern is that these models may not fare well on clinical MRIs that do not belong to the specially curated BraTS dataset. Research using the previous generation of deep learning models indicates significant performance loss on cross-institutional predictions. Here, we evaluate the cross-institutional applicability and generalzsability of state-of-the-art deep learning models on new clinical data. Methods: We train a state-of-the-art 3D U-Net model on the conventional BraTS dataset comprising low- and high-grade gliomas. We then evaluate the performance of this model for automatic tumor segmentation of brain tumors on in-house clinical data. This dataset contains MRIs of different tumor types, resolutions, and standardization than those found in the BraTS dataset. Ground truth segmentations to validate the automated segmentation for in-house clinical data were obtained from expert radiation oncologists. Results: We report average Dice scores of 0.764, 0.648, and 0.61 for the whole tumor, tumor core, and enhancing tumor, respectively, in the clinical MRIs. These means are higher than numbers reported previously on same institution and cross-institution datasets of different origin using different methods. There is no statistically significant difference when comparing the dice scores to the inter-annotation variability between two expert clinical radiation oncologists. Although performance on the clinical data is lower than on the BraTS data, these numbers indicate that models trained on the BraTS dataset have impressive segmentation performance on previously unseen images obtained at a separate clinical institution. These images differ in the imaging resolutions, standardization pipelines, and tumor types from the BraTS data. Conclusions: State-of-the-art deep learning models demonstrate promising performance on cross-institutional predictions. They considerably improve on previous models and can transfer knowledge to new types of brain tumors without additional modeling.
