Abstract:ObjectiveTo evaluate the repeatability of pupillary light reflex metrics measured by the RAPDx® dynamic pupillometer in healthy subjects and clinical application in patients with unilateral optic neuritis (ON).MethodsSixty eyes of 30 healthy volunteers were measured three times consecutively by the same technician. The amplitude of constriction (AC), the latency of constriction (LOC), the velocity of peak constriction (VC) of light-evoked pupillary constriction, RAPD score for amplitude and latency were measured using RAPDx® dynamic pupillometer. The repeatability of above metrics was assessed by the intraclass correlation coefficient (ICC) and coefficient of variation (Cov). Furthermore, pupillary light reflex measurements were performed in 48 eyes of 24 patients diagnosed with unilateral optic neuritis (ON). Interocular symmetry was evaluated both in the healthy subjects and the ON-involved patients.ResultsHigh repeatability of AC, LOC, and VC in healthy subjects was displayed, presenting with the ICC value over 0.80 and the Cov less than 8.00%. But the RAPD score for amplitude (ICC: 0.67) and RAPD score for latency (ICC: 0.65) showed only moderate repeatability. Furthermore, a slight declining trend was found in amplitude and peak velocity when continuous and multiple measurements in the healthy subjects. Good symmetry of the AC, LOC, and VC of pupillary light constriction between the two eyes was displayed in the healthy subjects (P > 0.05). By contrast, there was a distinct decrease of AC and VC (P < 0.01), and a mild increase of LOC (P < 0.01) in the ON-involved eye in direct pupillary light reflex.ConclusionsPupillary light reflex measured by the RAPDx® pupillometer achieved overall good repeatability and interocular symmetry in healthy subjects. The device also presented decent preliminary results in patients with unilateral ON, suggesting its potential value to be developed as a tool in optic nerve diseases.

Abstract:Abnormalities in pupillary light reflex can indicate optic nerve disorders that may lead to permanent visual loss if not diagnosed in an early stage. In this study, we focus on relative afferent pupillary defect (RAPD), which is based on the difference between the reactions of the eyes when they are exposed to light stimuli. Incumbent RAPD assessment methods are based on subjective practices that can lead to unreliable measurements. To eliminate subjectivity and obtain reliable measurements, we introduced an automated framework to detect RAPD. For validation, we conducted a clinical study with lab-on-a-headset, which can perform automated light reflex test. In addition to benchmarking handcrafted algorithms, we proposed a transfer learning-based approach that transformed a deep learning-based generic object recognition algorithm into a pupil detector. Based on the conducted experiments, proposed algorithm RAPDNet can achieve a sensitivity and a specificity of 90.6% over 64 test cases in a balanced set, which corresponds to an AUC of 0.929 in ROC analysis. According to our benchmark with three handcrafted algorithms and nine performance metrics, RAPDNet outperforms all other algorithms in every performance category.

Diagnostic Value of Point-of-Care Ultrasound-Guided Assessment of Relative Afferent Pupillary Defect in Adult Ocular Trauma Patients Presenting to the Emergency Department: A Prospective Cohort Study

Abstract:Objectives: Early diagnosis of relative afferent pupillary defects (RAPDs) in patients with ocular trauma is crucial for timely management and improved outcomes. However, clinical examination can be challenging for patients with periorbital ecchymosis. This study aimed to compare the diagnostic accuracy of point-of-care ultrasound (POCUS) and clinical examination by emergency physicians for detecting RAPD in adult ocular trauma patients and to evaluate the proportion of RAPD in patients with ocular trauma who presented to the ED. Methods: This prospective cohort study was conducted at an academic emergency department in South India. Adult ocular trauma patients were assessed for RAPD using clinical examinations by emergency physicians and POCUS. The diagnostic accuracies of both methods were compared, with the ophthalmologist's final diagnosis serving as the gold standard. The sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) were calculated for both techniques. Results: A total of 376 patients (median age, 35 years) were included in this study. RAPD was identified in 14.63% of the patients. The sensitivity and specificity of POCUS in detecting RAPD were 92.73% and 99.38%, respectively, which were higher than those of clinical examination, with a sensitivity of 81.82% and specificity of 99.07%. The PPV and NPV of the clinical examination were 93.75% and 96.95%, respectively, whereas the PPV and NPV of POCUS were 96.23% and 98.76%, respectively. POCUS accurately diagnosed RAPD in patients with periorbital ecchymosis. Conclusion: POCUS-guided RAPD assessment proves to be a better diagnostic adjunct compared to clinical examination in patients with ocular trauma presenting to the emergency department.
Automated Pupillary Light Reflex Test on a Portable Platform

Abstract:In this paper, we introduce a portable eye imaging device denoted as lab-on-a-headset, which can automatically perform a swinging flashlight test. We utilized this device in a clinical study to obtain high-resolution recordings of eyes while they are exposed to a varying light stimuli. Half of the participants had relative afferent pupillary defect (RAPD) while the other half was a control group. In case of positive RAPD, patients pupils constrict less or do not constrict when light stimuli swings from the unaffected eye to the affected eye. To automatically diagnose RAPD, we propose an algorithm based on pupil localization, pupil size measurement, and pupil size comparison of right and left eye during the light reflex test. We validate the algorithmic performance over a dataset obtained from 22 subjects and show that proposed algorithm can achieve a sensitivity of 93.8% and a specificity of 87.5%.

Abstract:The relative afferent asymmetry between two eyes can be diagnosed using swinging flashlight test, also known as the alternating light test. This remains one of the most used clinical tests to this day. Despite the swinging flashlight test's straightforward approach, a number of factors can add variability into the clinical methodology and reduce the measurement's validity and reliability. This includes small and poorly responsive pupils, dark iris, anisocoria, uneven illumination in both eyes. Due to these limitations, the true condition of relative afferent asymmetry may create confusion and various observers may quantify the relative afferent pupillary defect differently. Consequently, the results of the swinging flashlight test are subjective and ambiguous. In order to eliminate the limitations of traditional swinging flashlight test and introduce objectivity, we propose a novel approach to the swinging flashlight exam, VR-SFT, by making use of virtual reality (VR). We suggest that the clinical records of the subjects and the results of VR-SFT are comparable. In this paper, we describe how we exploit the features of immersive VR experience to create a reliable and objective swinging flashlight test.
Evaluation of a Novel Virtual Reality Simulated Alternate Cover Test to Assess Strabismus: A Prospective, Masked Study

Abstract:Purpose: A sensorimotor examination is the gold standard for strabismus diagnosis and quantification but requires a highly skilled examiner and may be limited by a child's cooperation. Virtual reality (VR) employs eye-tracking technology to monitor eye position and may be able to measure strabismus. The aim of this study was to assess a prototype VR-simulated alternate cover test to detect and measure strabismus. Design: Prospective, masked diagnostic test study. Methods: Patients aged 5-18 years with visual acuity of 20/80 or better were prospectively enrolled to undergo strabismus measurements using a VR simulated alternate cover test (Olleyes, Inc., Summit, NJ) followed by an alternate cover test performed by a masked pediatric ophthalmologist or orthoptist. The main outcome measure was correlation between gold standard and VR-obtained strabismus measurements (in prism diopters [PD]) in primary gaze at near using Pearson correlation coefficients and Bland-Altman analysis with limits of agreement (LOA). A secondary measure was the diagnostic accuracy for the VR headset to detect strabismus. Results: A total of 85 participants were enrolled, mean ± standard deviation age was 10.8 ± 3.8 years, 45.9% (39/85) male. 40.0% (34/85) had strabismus: 17.7% (15/85) esotropia, 22.4% (19/85) exotropia, and 5.9% (5/85) vertical strabismus. 52.9% (18/34) of strabismus was intermittent. The overall correlation between VR and gold standard strabismus measurements was moderate but significant (r = 0.42, 95% CI 0.22, 0.58, P < .001), and correlation was strong for esotropia and constant deviations (r = 0.74, 95% CI 0.38, 0.91, P = .001 and r = 0.74, 95% CI 0.39, 0.91, P < .001, respectively). In participants with horizontal strabismus, Bland-Altman analysis showed a mean difference between standard and VR measurements of 3.55 ± 8.33 PD for esotropia (upper and lower LOA 19.89, -12.78 PD) and 17.15 ± 11.20 PD for exotropia (LOA 39.09 and -4.79 PD). Sensitivity for detecting strabismus was low: 27.6% (95% CI 12.7, 47.2), but specificity was high: 87.5% (95% CI 75.9, 94.8). Conclusions: A prototype VR simulated alternate cover test showed a moderate but significant correlation with the gold standard sensorimotor examination and correlation was strong in those with esotropia and constant deviations. While the level of agreement demonstrated by this novel VR technology is promising, further improvements are needed before clinical deployment. However, this study demonstrates that VR has the potential to expand our ability to detect, measure, and monitor strabismus.
Binocular head-mounted chromatic pupillometry can detect structural and functional loss in glaucoma

Abstract:Aim: The aim of this study is to evaluate the utility of binocular chromatic pupillometry in detecting impaired pupillary light response (PLR) in patients with primary open-angle glaucoma (POAG) and to assess the feasibility of using binocular chromatic pupillometer in opportunistic POAG diagnosis in community-based or telemedicine-based services. Methods: In this prospective, cross-sectional study, 74 patients with POAG and 23 healthy controls were enrolled. All participants underwent comprehensive ophthalmologic examinations including optical coherence tomography (OCT) and standard automated perimetry (SAP). The PLR tests included sequential tests of full-field chromatic stimuli weighted by rods, intrinsically photosensitive retinal ganglion cells (ipRGCs), and cones (Experiment 1), as well as alternating chromatic light flash-induced relative afferent pupillary defect (RAPD) test (Experiment 2). In Experiment 1, the constricting amplitude, velocity, and time to maximum constriction/dilation were calculated in three cell type-weighted responses, and the post-illumination response of ipRGC-weighted response was evaluated. In Experiment 2, infrared pupillary asymmetry (IPA) amplitude and anisocoria duration induced by intermittent blue or red light flashes were calculated. Results: In Experiment 1, the PLR of POAG patients was significantly reduced in all conditions, reflecting the defect in photoreception through rods, cones, and ipRGCs. The variable with the highest area under the receiver operating characteristic curve (AUC) was time to max dilation under ipRGC-weighted stimulus, followed by the constriction amplitude under cone-weighted stimulus and the constriction amplitude response to ipRGC-weighted stimuli. The impaired PLR features were associated with greater visual field loss, thinner retinal nerve fiber layer (RNFL) thickness, and cupping of the optic disk. In Experiment 2, IPA and anisocoria duration induced by intermittent blue or red light flashes were significantly greater in participants with POAG than in controls. IPA and anisocoria duration had good diagnostic value, correlating with the inter-eye asymmetry of visual field loss. Conclusion: We demonstrate that binocular chromatic pupillometry could potentially serve as an objective clinical tool for opportunistic glaucoma diagnosis in community-based or telemedicine-based services. Binocular chromatic pupillometry allows an accurate, objective, and rapid assessment of retinal structural impairment and functional loss in glaucomatous eyes of different severity levels.

Abstract:INTRODUCTION: The pupillary light reflex (PLR) parameters are quantitative biomarkers that can be used to detect neurological disease. Vasospasm complicates aneurysmal subarachnoid hemorrhage (aSAH) trajectory, causing morbidity and mortality. METHODS: We prospectively collected PLR parameters from patients with aSAH admitted to a neurological intensive care unit twice daily using quantitative smartphone pupillometry recordings. PLR parameters included: Maximum pupil diameter (MAX), minimum pupil diameter (MIN), percent change in pupil diameter (CHANGE), latency in beginning of pupil constriction to light (LAT), mean constriction velocity (MCV), maximum constriction velocity (MAXCV), mean dilation velocity (MDV). One-tailed t-tests were performed to determine changes in PLR parameters between the following comparisons: (1) patients with and without radiographic vasospasm (defined by computed tomography angiography (CTA) or digital subtraction angiography (DSA)); (2) patients with and without delayed cerebral ischemia (DCI); and (3) patients with and without neurological exam change due to vasospasm. RESULTS: 42 total subjects were enrolled. For patients with (n = 13) and without (n = 29) radiographic vasospasm, significant PLR parameter differences were observed in: MAX (p = .024) and MIN (p = .0018). For patients with (n = 14) and without (n = 28) DCI, significant PLR parameter differences were observed in: CHANGE (p = .008). For patients with (n = 13) and without (n = 29) neurological exam change due to vasospasm, significant PLR parameter differences were observed in: MIN (34.7 vs 31.7, p = .014), CHANGE (20.6 vs 24.7, p = .025), and LAT (.36 vs .2, p = .0002). All other PLR parameters were not significantly different between groups for the three comparisons. CONCLUSIONS: Quantitative smartphone pupillometry may be of clinical use in the detection of radiographic vasospasm, DCI, and neuro exam changes via separate and distinct parameters from the PLR.

Abstract:Computerized pupillary light reflex assessment devices (CPLRADs) may serve as an effective screening tool for glaucomatous optic neuropathy, since they can dynamically detect abnormal pupillary responses from a novel sequence of light stimuli and functionally-shaped stimuli. The aim of this study was to systematically evaluate the current state of advanced CPLRADs and accuracy of application in detecting glaucoma. An electronic literature search of PubMed, MEDLINE, and Embase from database inception to December 2019 was performed. Studies that reported data on the use of computer-aided pupillometry with monocular and/or binocular monitoring in glaucoma patients were included. Two review authors independently conducted the study selection and extracted study data. A total of twenty-five studies were included in this review; eight studies with a total of 829 subjects were included in this meta-analysis. Data were pooled using a random-effect model, since the significant heterogeneity (P< 0.1,I-2> 50%). Our meta-analysis of eight studies showed reasonably high summary sensitivity and specificity estimates of 0.81 (95% CI 0.73-0.89) and 0.83 (95% CI: 0.75-0.91), respectively. Simpler monochromatic devices, such as Pupilmetrix(TM)PLR60, generally performed as well as or slightly better than more complex chromatic devices. This review suggests that CPLRADs may facilitate direct clinical decision making for glaucoma diagnosis and evaluation, and may provide a deeper understanding of the pathomechanism of glaucoma.
Repeatability and reproducibility of dynamic pupillary parameters using an automated quantitative pupillometer

Abstract:Introduction and Aim: The examination of the pupils is an integral part of any patient undergoing neuropsychological and ophthalmological evaluation. The manual examination of pupils has its own limitations. A more reliable technique for determining the size and reactivity of pupils is required. Hence, the study was done to examine the repeatability and reproducibility of dynamic pupillary parameters using an infrared pupillometer in the Indian population in routine clinical settings. Materials and Methods: A total of three paired pupillary measurements were completed within 6 minutes by two observers under identical ambient conditions with NPi-200 pupillometer in 30 healthy participants aged 18 to 60 years, providing a total of 60 paired measurements. Results: The ICC values for Neurological Pupil Index, Maximum Pupil Diameter, and Minimum Pupil Diameter were greater than 0.90, signifying excellent agreement. For Change in Pupil Size, Constriction Velocity, Maximum Constriction Velocity, Latency, and Dilatation Velocity, the ICC ranged from 0.75 to 0.90, indicating good agreement between measurements by Observer 1 and between measurements (No. 2 and 3) by Observer 1 and Observer 2. Conclusion: We found excellent agreement regarding repeatability and reproducibility of dynamic pupillary parameters using an automated quantitative pupillometer. In general, the findings of this study affirm the effective performance of automated quantitative pupillometry in routine clinical settings.

Abstract:Objective To evaluate the use of virtual reality-based infrared pupillometry (VIP) to detect individuals suffering long COVID. Design Prospective, case-control cross-sectional study. Participants Participants aged 20-60 were recruited from a community eye screening programme. Methods Pupillary Light Responses (PLR) were recorded in response to 3 intensities of light stimuli (L6, L7 and L8) using a virtual reality head-mount display (VR-HMD). 9 PLR waveform features for each stimulus, were extracted by 2 masked observers and statistically analyzed. We also use various methods on the whole PLR waveform including trained, validated and tested (6:3:1) by machine learning models including Multi-layer Perceptron, Support Vector Machine, K-nearest Neighbors, Logistic Regression, Decision Tree, Random Forest and Long Short-Term Memory (LSTM) models for two and three-class classification into long-COVID (LCVD), post-COVID (PCVD) or control.Main Outcome Measures:Accuracies/AUC of individual or combination of PLR features and ML models using PLR features or whole pupillometric waveform. Results PLR from a total of 185 subjects including 112 LCVD, 44 PCVD and 29 age/sex-matched controls were analysed. Models examined the independent effects of age and sex. Constriction Time(CT) after the brightest stimulus(L8) is significantly associated with LCVD status(two-way ANOVA, false discovery rate(FDR)<0.001; multinominal logistic regression, FDR<0.05). The overall accuracy/AUC of CT-L8 alone in differentiating LCVD from control or from PCVD were 0.7808/0.8711 and 0.8654/0.8140 respectively. Using cross-validated backward stepwise variable selection, CT-L8, CT-L6, Constriction Velocity(CV)-L6 were most useful to detect LCVD while CV-L8 for PCVD from other groups. The accuracy/AUC of selected features were 0.8000/0.9000 (control versus LCVD) and 0.9062/0.9710 (PCVD versus LCVD), better than when all 27 pupillometric features were combined. An LSTM model analyzing whole pupillometric waveform achieved the highest accuracy/AUC at 0.9375/1.000 in differentiating LCVD from PCVD and a slightly lower accuracy of 0.7838 for three-class classification (LCVD-PCVD-control). Conclusions We reported, for the first time, specific pupillometric signatures in differentiating LCVD from PCVD or control subjects using a VR-HMD. Combining statistical methods to identify specific pupillometric features and ML algorithms to analyse the performance further enhance the performance of VIP as a non-intrusive, low-cost, portable and objective method to detect and monitor long COVID.

Abstract:In this work, we developed a wearable, head-mounted device that automatically calculates the precise Relative Afferent Pupillary Defect (RAPD) value of a patient. The device consists of two RGB LEDs, two infrared cameras, and one microcontroller. In the RAPD test, the parameters like LED on-off durations, brightness level, and color of the light can be controlled by the user. Upon data acquisition, a computational unit processes the data, calculates the RAPD score and visualizes the test results with a user-friendly interface.Multiprocessing methods used on GUI to optimize the processing pipeline. We have shown that our head-worn instrument is easy to use, fast, and suitable for early-diagnostics and screening purposes for various neurological conditions such as RAPD, glaucoma, asymmetric glaucoma, and anisocoria.

Abstract:Purpose To investigate the robustness and variability of a novel kinetic visual field (VF) screening method termed rapid campimetry (RC). Methods In RC visual field (VF) screening is enabled via kinetic-based testing on any computer (10°/4.7 s at 40-cm viewing distance) and high contrast in a dark room (1 cd/cm 2 ). In experiment (1): 30 participants [20 healthy participants (HC), 5 glaucoma patients (GLA) and 5 patients with cataract (CAT)] were included to test the intra-session variability (fatigue effect) and the following effects on RC: room illumination (140 cd/m 2 ), ±3 D refractive errors, media opacity. In experiment (2): Inter-session variability (1–3 weeks apart) was assessed in 10 HC and 10 GLA. Since RC detects absolute scotomas, the outcome measure was the size of physiological (blindspot) and pathological (glaucoma) scotomas in degrees. A repeated measures ANOVA was employed in experiment 1 and intraclass correlation (ICC) in experiment 2. Results Neither the size of the blindspot nor the VF defects differed significantly between the different testing conditions. For intra-session variability, the average bias of blindspot size was −0.6 ± 2.5°, limits of agreement (LOA), in comparison to 0.3 ± 1.5° for VF defects, both with ICC of 0.86 and 0.93, respectively. For the inter-session repeatability, the average bias and LOA for blindspot size was 0.2 ± 3.85° in comparison 1.6 ± 3.1° for VF defects, both with ICC of 0.87 and 0.91, respectively. Conclusion RC was robust to suboptimal testing VF conditions and showed good-to-excellent reliability between VF testing visits holding high potential for teleophthalmology.

Abstract:Purpose: Compare estimated sensitivities of SITA-Standard to the RATA-Standard algorithm of the Radius virtual reality perimeter (VRP), and measure concordance in glaucoma staging.Methods: One hundred adult glaucoma patients-half with suspect or mild glaucoma, and half with moderate or severe-from five clinics performed four 24-2 visual field tests during a single visit, two with the Humphrey Field Analyzer (HFA) and two with Radius, in randomized order: HRHR or RHRH. Only one eye was tested per participant. We used the Wilcoxon rank sum test with Bonferroni correction to compare distributions of estimated sensitivities across all 54 test locations over the 15 to 40 dB measurement range of the Radius. Weighted kappa measured concordance in glaucoma staging between two masked glaucoma experts using Medicare definitions of severity.Results: A total of 62 OD and 38 OS eyes were tested. Estimated sensitivities for SITA-Standard and RATA-Standard were not significantly different for OD, but were for OS-likely because of SITA-Standard OD and OS being significantly different in our sample, but not for RATA-Standard. Low agreement was observed between 15 to 22 dB. Concordance in glaucoma staging was high for both graders: kappa = 0.91 and kappa = 0.93. Average test duration was 298 seconds for RATA-Standard and 341 seconds for SITA-Standard. The correlation in mean deviation was 0.94.Conclusions: Estimated sensitivities of RATA-Standard are comparable to SITA-Standard between 23 to 40 dB with high concordance in glaucoma staging.Translational Relevance: Radius VRP is statistically noninferior to HFA when staging glaucoma using Medicare definitions.

Automated Quantitative Pupillometry in the Critically Ill: A Systematic Review of the Literature

Abstract:Objective: A systematic literature search has been performed to identify potential confounders for outcome prediction using pupillary light reflex in adult critically ill patients, as measured by handheld automated pupillometry devices. Methods: Three digital databases (PubMed, EmBase, Cochrane) were systematically searched. Articles published between 1990 and 2019 in adult patients using monocular automated handheld devices were considered. Studies were classified according to the Oxford Centre for Evidence-Based Medicine classification (level 1 represents the highest and level 5 the lowest level of evidence). Case reports, original research, and systematic reviews were included and cross-referenced. Results: With the use of 202 search terms, 58 eligible articles reporting the use of handheld pupillometry in the critically ill could be identified, considering 3,246 patients. The highest level of evidence came from 10 randomized trials and 19 prospective observational studies. The level of evidence was mostly 2 to 3 and highest with studies regarding the potential confounding effects of pain, the use of opioids, and increased intracranial pressure. Additional potential confounders found are selective serotonin reuptake inhibitors, α2-adregenic receptor agonists, and NMDA antagonists. Conclusions: The pupillary light reflex is susceptible to factors resulting from underlying comorbid conditions and effects of treatment regimens. Scenarios frequently encountered in critical care such as pain, use of opioids, and proof of increased intracranial pressure have potential confounding effects on outcome and pupillary reflexes. When treatment is guided by pupillary metrics, such confounders put patients at risk of overtreatment or undertreatment. Future research should validate and identify additional confounders, because our review suggests that even more unexplored confounders may exist.

Abstract:Pupillometry technique has gained an increased interest over the years as it allows to objectively assess patient’s consciousness and neuro-ophthalmological status by measuring pupillary light reflex (PLR). Chromatic pupillometry emerged with the discovery of intrinsically photosensitive retinal ganglion cells (ipRGCs), blue light sensitive and contributors to PLR. Automated pupillometers have been developed over the years to quantitatively measure PLR but have a reduced possibility to be a widespread screening tool as they are expensive and not portable. In this study, a smartphone-based pupillometer was developed, taking advantage of this technology accessibility, low-cost price, and portability. Chromatic stimuli were considered in this smartphone-based pupillometer to assess the contribution of ipRGCs to PLR and allow the screening of the neuro-ophthalmological diseases. In this preliminary study, six healthy individuals participated and pupillometric data was collected using the smartphone application developed, testing different protocols and background light conditions. The system presented good quality of eye images acquired and good behaviour in pupil data extraction. The acquisition protocols tested showed promising results to be used for chromatic pupillometry, although increasing the number of participants is mandatory and further research is needed. Nevertheless, this study shows the potential of the developed pupillometer to function as low cost, portable and accessible screening tool for neuro-ophthalmological diseases.
Gaze-Contingent Flicker Pupil Perimetry Detects Scotomas in Patients With Cerebral Visual Impairments or Glaucoma

Abstract:Background: The pupillary light reflex is weaker for stimuli presented inside as compared to outside absolute scotomas. Pupillograph perimetry could thus be an objective measure of impaired visual processing. However, the diagnostic accuracy in detecting scotomas has remained unclear. We quantitatively investigated the accuracy of a novel form of pupil perimetry. Methods: The new perimetry method, termed gaze-contingent flicker pupil perimetry, consists of the repetitive on, and off flickering of a bright disk (2 hz; 320 cd/m2; 4° diameter) on a gray background (160 cd/m2) for 4 seconds per stimulus location. The disk evokes continuous pupil oscillations at the same rate as its flicker frequency, and the oscillatory power of the pupil reflects visual sensitivity. We monocularly presented the disk at a total of 80 locations in the central visual field (max. 15°). The location of the flickering disk moved along with gaze to reduce confounds of eye movements (gaze-contingent paradigm). The test lasted ~5 min per eye and was performed on 7 patients with cerebral visual impairment (CVI), 8 patients with primary open angle glaucoma (age >45), and 14 healthy, age/gender-matched controls. Results: For all patients, pupil oscillation power (FFT based response amplitude to flicker) was significantly weaker when the flickering disk was presented in the impaired as compared to the intact visual field (CVI: 12%, AUC = 0.73; glaucoma: 9%, AUC = 0.63). Differences in power values between impaired and intact visual fields of patients were larger than differences in power values at corresponding locations in the visual fields of the healthy control group (CVI: AUC = 0.95; glaucoma: AUC = 0.87). Pupil sensitivity maps highlighted large field scotomas and indicated the type of visual field defect (VFD) as initially diagnosed with standard automated perimetry (SAP) fairly accurately in CVI patients but less accurately in glaucoma patients. Conclusions: We provide the first quantitative and objective evidence of flicker pupil perimetry's potential in detecting CVI-and glaucoma-induced VFDs. Gaze-contingent flicker pupil perimetry is a useful form of objective perimetry and results suggest it can be used to assess large VFDs with young CVI patients whom are unable to perform SAP.

Abstract:Purpose: The VisuALL S is an automated, static threshold, virtual reality-based perimeter for mobile evaluation of the visual field. We examined same-day and 3-month repeatability. Methods: Adult participants with a diagnosis of glaucoma or ocular hypertension underwent two VisuALL 24-2 Normal T- Full threshold strategy tests at baseline and one additional exam at 3 months for each eligible eye. Spearman, intraclass correlation coefficients (ICCs), and Bland-Altman plots were used to assess the correlation of individual point sensitivities and mean deviation (MD) among three tests. Results: Eighty-eight eyes (44 participants) were included. Average age was 68.1 ± 14.3 years, and 60.7% were male. VisuALL MD was highly correlated between tests (intravisit: r = 0.89, intervisit: r = 0.82; P < 0.001 for both). Bland-Altman analysis showed an average difference in intravisit MD of -0.67 dB (95% confidence interval [CI], -6.04 to 4.71 dB) and -0.15 dB (95% CI, -8.04 to 7.73 dB) for intervisit exams. Eight-five percent of pointwise intravisit ICCs were above 0.75 (range, 0.63 to 0.93), and 65% of pointwise intervisit ICCs were above 0.75 (range, 0.55 to 0.91). Conclusions: VisuALL demonstrated high correlation of MD between tests and good repeatability for individual point sensitivities among three tests in 3 months, except at the points around the blind spot and superiorly. Translational relevance: The preliminary reproducibility results for VisuALL are encouraging. Its portable design makes it a potentially useful tool for patients with glaucoma, enabling more frequent assessments both at home and in clinical settings.
