Abstract:Objective To analyse the reasons of allergen-specific IgE(sIgE) failing to conform the actual condition of the objects and investigate the influences of IgE to carbohydrate cross-reactive determinants(CCD-IgE) on the detection of serum sIgE.Methods The information of 1 715 cases was collected by questionnaires,and 19 kinds of serum sIgE and CCD-IgE were detected by dot-immunoblot test(IBT).Based on questionnaires and the results of sIgE detection,the inconformity of sIgE with the questionnaires and the influence of CCD-IgE on the detection results of sIgE were analyzed.Results The inconformity of sIgE with the questionnaires was present in 19 kinds of allergen detected in this study.The highest inconformity rate(47.06%) was found in peanut allergen.The rate of inconformity was relatively higher in the cases with low level of positive sIgE and the cases with positive CCD-IgE than that in the cases of negative sIgE(P<0.05).Comparing the positive rates of CCD-IgE between the doubtful allergic subjects and healthy controls who showed inconformity of sIgE with clinical manifestation in questionnaires,a difference with statistical significance was found(P<0.05).Conclusion CCD may be the reason of inconformity of sIgE detection with the questionnaires resulting by cross-reaction.If sIgE results showed inconformity with the actual clinical symptoms,CCD should be taken into account.
Role of Aspergillus Fumigatus-Specific IgE in the Diagnosis of Allergic Bronchopulmonary Aspergillosis

Abstract:Introduction: Allergic bronchopulmonary aspergillosis (ABPA) has been regarded as a rare disease in China due to the lack of quantitative detection of Aspergillus fumigatus-specific IgE (sIgE). We compared the diagnostic rate of ABPA among asthma patients with or without A. fumigatus-sIgE screening tests to evaluate the benefit of the tests in diagnosing ABPA. Methods: We reviewed the detection rate of A. fumigatus-sIgE and the diagnostic rate of ABPA in 1842 asthma patients in the First Affiliated Hospital of Zhejiang University from 2014 to 2016. Additionally, we collected 144 asthma cases from November 2016 to March 2017 to detect the total serum IgE, A. fumigatus-sIgE and sIgE against mixed mold extract, the ABPA diagnostic rate of these patients was then compared with the total cohort. Total serum IgE, A. fumigatus-sIgE and sIgE against mixed mold extract were also tested in 30 patients identified with Aspergillus-positive sputum culture to analyze the incidence of ABPA. Results: Among the 1,842 asthma cases, 566 were inspected for total IgE; 308 (55.40%) were total IgE-positive and 58 (10.43%) had total IgE > 1,000 IU/mL. In contrast, only 126 cases were tested for A. fumigatus-sIgE (6.84%), and 28 had A. fumigatus-sIgE > 0.35 kUA/L (22.22%). Eleven patients were finally diagnosed with ABPA. Of 1,842 asthma patients, only 0.6% were diagnosed with ABPA if the A. fumigatus-sIgE was not detected at first. Moreover, among the 144 asthma cases that were selected for total IgE, A. fumigatus-sIgE, and sIgE against mixed mold extract screening tests, 12 had total IgE > 1,000 IU/mL (8.33%), 11 had A. fumigatus-sIgE > 0.35 kUA/L (7.64%), and 14 had sIgE against mixed mold extract > 0.35 (9.72%); 7 of these patients were confirmed as having ABPA according to the ISHAM guidelines (4.86%) but only 2 without A. fumigatus-sIgE screening test were diagnosed with ABPA (1.39%) (p = 0.000). Of the 30 Aspergillus-positive sputum culture cases, 4 had A. fumigatus-sIgE > 0.35 kUA/L (13.33%), but none was diagnosed with ABPA. Conclusions: Routine A. fumigatus-sIgE screening for asthma patients can significantly improve the diagnostic rate of ABPA.
Detection of specific IgE in serum of children with allergic rhinitis and its clinical significance

Abstract:Objective: Investigating the main allergen distribution and characteristics in the children with allergic rhinitis, in order to guide the clinical prevention and treatment. Design: 20 common allergens and specific IgE antibodies in serum of patents were determined through Western blot. Results: All the 938 patients were positive to at least one allergen, including 487 cases of children with total IgE was greater than 400 IU/ml, 230 cases ranging from 200 IU/ml ~ 400 IU/ml. In 20 specific allergens, the leading allergens were house dust and mite; the positive rates were 90.0% and 80.8%. Conclusion: At least one specific IgE antibody increases significantly in children with allergic rhinitis, the main allergens for the children with allergic rhinitis were house dust, dust mite, pet skin scraps, mutton and milk. Children are susceptible to allergic rhinitis in spring and autumn. © 2013 Japan International Cultural Exchange Foundation & Japan Health Sciences University.
Detection and analysis of serum immunoglobulin E levels in healthy volunteers from Jiangsu and Anhui

Abstract:Objective Serum total immunoglobulin E (tIgE)and allergen-specific immunoglobulin E (sIgE)levels were detected in a healthy population to evaluate their clinical value and significance.Methods A total of 379 healthy volunteers were recruited for this study,including 207 males and 172 females who were aged 17-64 years with a median age of 27 years old.All volunteers underwent a healthy physical examination in the Jiangsu or Anhui area.Serum levels of tIgE and sIgE were detected using a capture enzyme-linked immunosorbent assay and the enzyme-league immune method (Phadiatop),respectively.A tIgE level ≤100 kU/L was regarded negative,while a level >100 kU/L was regarded positive.For the sIgE level,Phadiatop values <0.35 kU/L (class 0) were regarded negative,and values≥0.35 kU/L (class ≥1)were positive.Data were analyzed using SPSS 18.0 software.A P-value less than 0.05 was considered statistically significant,with a correction ofα=0.05/6 (P<0.0083)in multiple comparisons.Results Among the 379 healthy volunteers,the tIgE serum level was 1.48-1903.22 (median 53.70)kU/L,and it was signifi-cantly higher in males than in females (P<0.001).The serum level of sIgE (Phadiatop)was 0.01-45.20 (median 0.07)kUA/L,with no significant difference between males and females (P=0.207).tIgE was positive and negative in 129 subjects (34.04%)and 250 subjects (65.96%),respectively,and Phadiatop was positive and negative in 88 subjects (23.22%)and 291 subjects (76.78%),respectively.The serum level of tIgE was highest in the group that was 17-30 years old (median 62.18 kU/L,P=0.006)among the four age groups,especially in males (80.60 kU/L, P=0.009).No significant difference was fund in the serum level of sIgE among the four age groups (P=0.318). There were 67 subjects (76.14%)with a tIgE level >100 kU/L in the positive sIgE group and 62 subjects (21.31%) with a tIgE level >100 kU/L in the negative sIgE group.Moreover,in the negative tIgE group,the majority were negative for sIgE (91.60%,P<0.001).The Spearman rank correlation coefficient between serum tIgE and sIgE lev-els was 0.650,which was statistically significant (positive correlation,P<0.001).Conclusion There was a positive correlation between the serum levels of tIgE and sIgE in this healthy population from the Jiangsu and Anhui areas.A lower serum level of tIgE (less than 100 kU/L)may be a reference value to exclude allergic sensitization.

Abstract:IgG4 is a subclass of IgG antibody with a unique molecular feature of (Fragment antigen- binding) Fab-arm exchange, allowing bispecific antigen binding in a mono-valent manner. With low binding affinity to C1q and Fc gamma receptors, IgG4 is incapable of forming immune complexes and activating the complement pathway, exhibiting a non-inflammatory feature. IgG4 is produced similarly to IgE and is considered a modified reaction to IgE class-switching response under certain conditions. It could also counteract IgE-activated inflammation. However, the clinical significance of IgG4 in allergic diseases is complex and controversial. Three viewpoints have been suggested to describe the role of IgG4. IgG4 can act as a tolerance-inducer to play a protective role under repeated and rapid incremental dosing of allergen exposure in allergen immunotherapy (AIT), supported by allergies in cat raisers and venom desensitization in beekeepers. Another viewpoint accepted by mainstream specialists and guidelines of Food Allergy and Management in different countries points out that food-specific IgG4 is a bystander in food allergy and should not be used as a diagnostic tool in clinical work. However, eosinophilic esophagitis (EoE) investigation revealed a direct clinical relevance between physiopathology and serum IgG4 in cow milk and wheat. These factors indicate that allergen-specific IgG4 plays a multifaceted role in allergic diseases that is protective or pathogenic depending on different allergens or exposure conditions.

Abstract:The diagnosis of IgE-mediated type Ⅰ allergy requires detailed clinical history,as well as in vivo testing and in vitro serological tests for allergen-specific IgE antibodies.The existence of allergenic crossreactivity has increased the difficulty in etiological diagnosis.This paper is aimed at illustrating the common molecular mechanisms of allergenic cross-reactivity,such as the structural similarity in protein families and cross-reactivity of carbohydrate determinants (CCD),and describing strategies to manage this condition in clinical practice.It is recommended to measure specific IgEs against allergen components in patients with allergen-specific IgEs when the interference from anti-CCD IgE is excluded,which may provide not only a more personalized allergen profile but also more information for specific immunotherapy.

Abstract:Objective In order to study the population distributions of IgE to cross-reactive carbohydrate determinants(anti-CCDIgE) and specific IgE(sIgE) in Beijing and their relationship with age and sex in order to provide information for anti-CCDIgE prevention and clinical diagnosis.Methods The dot-IBT method was utilized to examine anti-CCDIgE and 19 kinds of sIgE among 1 377 people living in Beijing.The relationship between anti-CCDIgE and sIgE and the positive rate difference between different people were analyzed.Results The prevalence of anti-CCDIgE was 23.7%(327 /1 377) in Beijing district.The prevalence of anti-CCDIgE increased with a higher number of sIgE positive items(r = 0.986,P < 0.05).The prevalence of anti-CCDIgE was a statistically higher among female(25.3%,260 /1 028) compared with male(19.2%,67 / 349).The prevalence of anti-CCDIgE was higher among people with allergic symptoms(45.2%,80 /177) compared with those without allergic symptoms(20.6%,247 /1 200,χ2 = 5.344 and 51.610,P < 0.05).Age was not associated with the prevalence of anti-CCDIgE.No difference was found between the prevalence of anti-CCDIgE among medical care personal and administration personal.The top sIgE allergens among those with anti-CCDIgE included: dust(44.6%),cockroaches(22.9%),and hair of the dogs(22.6%),trees(22.1%) and shrimps(21.7%).Conclusion Our study showed that the prevalence of anti-CCDIgE was high among the population in Beijing.If there are multiple positive sIgE items,it suggests anti CCDIgE may have affected the sIgE detection.This needs to be taken into consideration during clinical diagnosis of the allergies.
Analysis on Relationship Between Specific IgG Antibodies of 14 Food Allergens and Allergic Skin Diseases

Abstract:Purpose: To analyze the relationship between the specific IgG antibodies of 14 food allergens and allergic skin diseases. Method: 340 patients of allergic skin diseases treated in our hospital during March 2012-June 2014 are selected, including 164 males and 176 females. In addition, 30 cases of healthy people without anaphylactic reaction medical history based on the physical examination center of our hospital are selected as the control group. The food intolerance test kits (American BIOMERICA Corporation) are adopted, and the Enzyme Linked Immunosorbent Assay (ELISA) semi-quantitative detection method is applied to detect the concentrations of the antigenic IgG antibodies in 14 commonly seen foods including beef, milk, chicken and pork. Results: The specific IgG antibodies of the 14 food allergens have different concentrations in serums of the patients; in which cod, crab, egg, milk, shrimp and soybean have relatively high contents of 15.43%, 41.76%, 52.04%, 32.43%, 21.45% and 8.92%, respectively. The specific IgG antibody positive rates of the three types of skin disease patients are significantly higher than that of the control group, in which the positive rate of the urticaria group is 77.73%, with significant difference with that of the control group (X2=173.24, P<0.05). The positive rate of the eczema group is 89.35%, which is also significantly higher than that of the control group (X2=481.52, P<0.05). The positive rate of the specific antibody of the atopic dermatitis group is 81.19%, which is also significantly higher than that of the control group (X2=274.38, P<0.05). With the growth in ages, the positive rates of the specific IgG antibodies in serums of three groups show down trends of varied extents. The chi-square test is conducted on the antibody positive rates of the patients with ages under 3 y old and that during 25-50 y old, with the result that the patients of eczema have significant difference (X2=164.23, P<0.05). Conclusion: The serums of the allergic skin disease patients have food specific IgG antibodies, illustrating the correlation between allergic skin diseases and food intolerance. The detection on food specific IgG antibodies plays an important role in diagnosis on allergic skin diseases and prevention of food allergy.
A study of allergen detection panel in Guangzhou, southern China based on real-world data from the past 7 years

Abstract:This study aims to reduce the cost of allergen testing for Guangzhou, China by limiting the number of allergens for which patients are tested, and provide a testing panel to improve diagnostic and therapeutic efficiency. This retrospective study of real-world data from 2012 to 2019 included 39,570 patients with suspected allergies in Guangzhou, southern China. All the patients were tested for one or more of the following allergens serum specific immunoglobulin E (sIgE): Dermatophagoides pteronyssinus, Dermatophagoides farinae, cat dander, dog dander, Artemisia vulgaris, Aspergillus fumigatus, Alternaria alternata, Blattella germanica, egg whites, milk, wheat, peanuts, soybeans, Cancer pagurus, and Penaeus monodon by PhadiaCAP 1000. Totally, only the positive rates of allergens sIgE in D. farinae, D. pteronyssinus, milk, egg whites, B. germanica, C. pagurus, A. alternata, and P. monodon were > 10%, the other allergens were between 4-7%. Moreover, among the allergic diseases, dust mites exhibited the overall highest positive rate, followed by milk and B. germanica. In children, milk was the main allergen, whereas in adults, mites, cockroaches, shrimp, and crab allergens had higher positive rates. The optimal scale analysis shows that the multiple sensitization classification of patients can be divided into three categories: I D. farinae and D. pteronyssinus; II. C. pagurus, P. monodon, and B. germanica; III. Milk and egg whites. Generally, a panel including 4 allergens can detect > 90% of the potential allergy in this local population. In Guangzhou, southern China, D. farinae, milk, B. germanica, and A. alternata as a panel screening allergy for suspected allergic patients was suggested base on this study.
A Multi-centre Analysis of Serum IgE Levels in Atopic Dermatitis

Abstract:Objective To assess the characteristics of total immunoglobulin E (IgE) and allergen-specific IgE (sIgE) to 20 common allergens in 154 patients with atopic dermatitis (AD). To assess the correlation of clinical food allergy with positive food allergens’ sIgE results. We further discuss the significance of IgE as a potential biomarker for AD disease severity. Methods A total of 154 AD patients were collected from 15 hospitals nationwide in China from 2019 to 2021. Serum IgE was measured using reverse-enzyme immuno capture test (REAST). Patients were required to have at least one positive sIgE (N ≥ 0.35 IU/mL). Patients were divided into groups according to gender, age, disease severity, and region. SPSS 26.0 software was used for statistical analysis. Results Compared with adolescent and adult, AD in infancy and childhood showed significantly higher frequencies of positive sIgE to food allergens, including egg, cow milk, and wheat (P < 0.01). However, adolescent and adult AD showed significantly higher frequencies of positive sIgE to inhaled allergens, dermatophagoides farinae, and house dust mite. In addition, sIgE in different sexes were different. Compared with women, men showed higher frequencies of positive allergen-specific IgE level to wheat, dermatophagoides farinae, and house dust mite. The most common food allergens with elevated sIgE levels were egg (71%), cow milk (39%) and wheat (32%). However, AD patients reported seafood, including crab, shrimp, and fish, as the most frequent food allergens which aggravate their disease in their daily life. Only 18 (12%) patients reported definite correlation of clinical practice with positive food allergens’ IgE results. Among 154 sIgE-positive patients, 99 patients had an increase of total IgE (≥60 IU/ml). TotalIgE (tIgE) levels were significantly different between mild (193 ± 239 IU/mL), moderate (170 ± 202 IU/mL), and severe (375 ± 343 IU/mL) forms of AD patients (P < 0.01). AD patients with accompanied allergic diseases showed significantly higher tIgE levels than those without accompanied allergic symptoms (280 ± 286 IU/mL vs 194 ± 248 IU/mL). Conclusion Neither sIgE nor tIgE levels can be used to evaluate the condition or severity of AD. AD patients with accompanied allergic diseases showed significantly higher tIgE levels than those without accompanied allergic symptoms. Infantile AD patients are more allergic to food, while adolescents and adults are more allergic to environmental antigens. IgE tests must be interpreted by combining with clinical history to avoid unnecessary food avoidance. Early food allergen introduction for infants may be promising for the prevention of food allergies.

Abstract:Objective The purpose of the study is to detect common allergens in patients with allergic diseases using different methods and make contrasted analysis of the results. Methods Common allergens were tested by three methods skin prick test, FEIA, Immunoblot in 58 patients from January to April 2006 in our allergy outpatient department and paediatric outpatient department. Results There is certain concordance between the three methods, and they have own advantages and disadvantages. Skin prick test is easy to operate with rapid result, but the false positivity is high and it is affected by medicine. For single allergen specific IgE quantitative determination, ImmunoCAP is the most accurate, but its cost is high. As allergens screening tool for newly patients, AllergyScreen has confident results and moderate cost, so it has promising application in clinical. Conclusion Three methods were believable, and the doctor could make choice according to the patients, and if the specific Immunotherapy is considered, it is advisable that the doctor should co-evaluate both the in-vivo and in-vitro tests. Early detection of allergens and taking measures to avoid contact is the principle and the most effective method for prevention and treatment of allergic diseases. The study is to make contrasted analysis of the AllergyScreen system and Pharmacia UniCAP in detecting Inhalant-allergen-specific IgE in Type I allergy.
Analysis of allergen positivity rates in relation to gender, age, and cross-reactivity patterns

Abstract:Objective: This study aims to gain a deeper understanding of the patterns of allergen detection positivity rates among different age and gender groups, and to explore cross-reactivity patterns among allergens. Methods: We conducted a retrospective analysis of patients who underwent allergen testing at our hospital. The sample for this study included all patients who underwent allergen testing. We utilized immune blotting to detect specific IgE antibodies to 20 allergens in patient sera. Results: The study results showed a higher proportion of female samples (72.35%) compared to male samples (27.65%). Among all participants, the age distribution was primarily concentrated in the 25–35 age group, accounting for 37.43% of the total sample, followed by the 45 years and older age group, accounting for 27.00%. This indicates that allergic symptoms may occur not only in children and adolescents but also at any time after adulthood. We further observed significant influences of factors such as gender and age on individual sensitivity to specific allergens. For example, compared to males, females were found to be more sensitive to certain allergens such as cat hair, dust mites, and dog epithelium. Similarly, we also found variations in sensitivity to specific allergens among different age groups. Conclusion: Close monitoring of allergen distribution in populations facilitates active engagement of allergic individuals in self-management, while gender and age may be important factors influencing individual sensitivity to specific allergens. These findings provide valuable insights for understanding the pathogenesis of allergic diseases and designing more effective prevention and treatment strategies.

Abstract:BackgroundAllergen sources are complex and heterogeneous mixtures of proteins, which may contain from innocuous IgE-binding structures such as CCDs (cross-reactive carbohydrate determinants), which are not associated with symptoms, to a panallergen such as profilin, which is associated with multiple sensitizations to pollen and pollen-food-latex syndromes. CCDs are present in many plant and animal allergens and cause strong cross-reactivity due to the high similarity of their structure. They complicate the interpretation of positive in vitro diagnostic results because they do not associate symptomatology and, thats why, the determination of specific IgE antibodies against CCDs can provide useful information when positive IgE results are not consistent with the clinical picture. Profilins are ubiquitous proteins present in all eukaryotic cells and identified as allergens in pollen, latex and plant foods. The highly conserved structure explains the cross-reactivity of IgE antibodies against plant profilins and their designation as panallergens. As a panallergen, profilin is associated with multiple pollen sensitization and pollen-food-latex syndromes that the allergist must be aware of for accurate diagnosis and successful treatment of allergic diseases. We intend to evaluate the prevalence of these two markers with a new diagnostic tool, since profilins and CCDs are the substances most likely to cause in vitro cross-reactivity.MethodsIn our study 8,242 adults who presented rhinoconjunctivitis and visited their primary care center were included. The samples were analyzed in serum samples which were routinely processed in our laboratory with the EUROLINE Mediterranean Inhalation 2 (IgE) assay from Euroimmun® from February to July 2023, according to the manufacturer's protocols. Patients with specific IgE levels ≥ 0.35 kU/L were identified as sensitized to allergen.ResultsA total of 8,242 patients were enrolled in this study, 5,173 women and 3,069 men, ranging in age from 15 to 65 years. Of these, 279 patients (3.39%) had antibodies against CCD, 570 (6.92%) antibodies against profilins and 183 (2.22%) had both. A higher prevalence of these antibodies was observed in men (CCD, n = 168, 5.47%; Profilins, n = 308, 10.04%) than in women (CCD, n = 111, 2.15%; Profilins, n = 262, 5.06%). Both markers were more frequently found in the presence of specific IgE antibodies to grass and weed pollens.ConclusionsThe possibility of detecting the presence of IgE antibodies against CCD and panallergenic profilins is useful to detect cross-reactivity in vitro , which could explain the differences found between laboratory results and patients' symptoms, helping the allergist to make a diagnosis with greater certainty and accuracy.

Abstract:BackgroundPollen allergens are vital contributors to allergic diseases. The frequency and coreactivity pattern of allergens are closely related to geographical distribution.ObjectiveIn this study, we aimed to characterize the prevalence of the molecular components of the common weed pollen allergens, birch pollen, walnut, and cross-reactive carbohydrate determinant (CCD), as well as investigate the relationship between the allergens and CCD in Chinese pollen-sensitized patients with allergic diseases.MethodsBased on previous vegetation surveys, serum samples from 165 pollen-sensitized patients with allergic diseases in Guangdong Province in southern China were used to test 19 crude allergen extracts, their components, and CCD using component-resolved diagnosis (CRD). Moreover, the potential associations among CCD, allergens, and their components were described.ResultsIn the 165 samples, the most common sensitized allergens were goosefoot (43.0%), ragweed (40.6%), walnut (37.6%), walnut tree (37.6%), and mugwort (37.0%), followed by platane (35.2%), cocklebur (27.9%), and birch (24.2%). The positivity rate of CCD was 39.4%. Among the samples positive for mugwort, 11 (18.0%), 15 (24.6%), and 15 (24.6%) were positive for Art v 1, Art v 2, and Art v 3, respectively. Among the 67 patients sensitized to ragweed, only five (7.5%) were positive for Amb a 1. In the 40 patients sensitized to birch, Bet v 2 had the highest positivity rate (40.0%). There were 62 patients who were sensitized to walnut. Their components had a lower positivity rate (less than 15%). The hierarchical clustering and optimal scale analysis showed that Art v 4 and Bet v 2 were closely related, and 91.9% of CCD-positive samples were polysensitized. Meanwhile, Spearman's rank correlation method showed that CCD was closely correlated with the sensitization of crude allergen extracts, and there was a low correlation between CCD and allergen components. CCD was highly correlated with goosefoot, ragweed, and walnut trees (r>0.8). Moreover, there was a strong relationship between the levels of Jug r 3 and Art v 3 (r = 0.78; P < 0.001).ConclusionsIn southern China, the weed pollens (ragweed, cocklebur, and goosefoot) exhibited higher positivity rates in adults and had a stronger relationship with CCD but not with mugwort. The positivity rate of allergen components was not high. CCD-positive samples were always polysensitized.

Abstract:Objective: The study aims to preliminarily investigate the prevalence characteristics of allergen-specific immunoglobulin E (IgE) in 57558 patients over the past decade by examining its distribution in the province and exploring its associations with age, sex, temperature, and relative humidity, providing insights for the prevention and diagnosis of allergic diseases in the Sichuan region. Methods: A retrospective analysis was conducted on a cohort of 57558 patients who underwent allergen testing (by means of EUROIMMUN immunoblotting method) at West China Hospital, Sichuan University between August 2012 and February 2022. The clinical data of these patients were collected to establish a comprehensive database, while the temperature and humidity records of the corresponding timeframe were gathered for further analysis. The positive results from the allergen tests were categorized into four levels, including weakly positive (±), positive (+), moderately positive (++), and strongly positive (+++). Statistical analyses were performed using SPSS 25.0, with Chi-square tests conducted to compare count data and Pearson's correlation tests done conducted to assess the relationships between different types of allergens and temperature/relative humidity. P<0.05 was applied to determine statistically significant differences. GraphPad Prism 9.0.0 was utilized to generate visual representations of the data. Results: The overall positivity rate of allergen-specific IgE among the 57558 samples was 30.69%. The top five allergens that elicited positive results were dust mite mix 1 (14.46%), crab (6.67%), soybean (4.72%), fish mix 1 (4.64%), and cockroach (4.34%). Notably, weakly positive (±) results were predominant for allergens such as eggs, peanuts, soybeans, cow's milk, beef, mutton, crab, shrimp, fish mix 1, cockroach, humulus japonicus, ambrosia artemisifolia, artemisia vulgaris, tree mix 2, house dust, and mold mix 1, collectively constituting over 40% of the positive outcomes. In contrast, cat hair and dog dander exhibited an equal distribution of approximately 25% for each positive levels, while mite mix 1 demonstrated the highest proportion of strongly positive results (+++), accounting for 37.66% of all positive results. Sex disparities in positivity rates were evident for various allergens, with significant differences observed for peanut, soybean, crab, shrimp, fish mix 1, cockroach, ambrosia artemisifolia, tree mix 2, cat hair, dog dander, and mite mix 1. Furthermore, the study identified age-related trends in allergen positivity rates, with a general decline observed across most allergens with increasing age. The positive rate of at least one food allergen was highest in the 0-10 age group (36.18%), and the positive rate of at least one inhalation allergen was highest in the 11-20 age group (45.35%). Noteworthy correlations were observed between allergen-specific IgE positivity and environmental factors, including a strong negative correlation between cow's milk allergy and relative humidity ( r=-0.640, P<0.05), a strong negative correlation of artemisia vulgaris sensitivity with temperature ( r Mean high temperature=-0.695, r Mean low temperature=-0.692, P<0.05), and a very strong positive correlation of mold mix 1 sensitivity with relative humidity ( r=0.704, P<0.05). Conclusion: Allergen-specific IgE positivity is associated with genetic factors, demonstrates significant sex- and age-related characteristics in the population, and is influenced by changes in local temperature and relative humidity.

Abstract:Objective: To investigate the distribution characteristics of specific immunoglobulin E (IgE) to dust mite components in patients with atopic dermatitis (AD), aiming to provide a reference basis for the development of microarray protein chips for allergen component discrimination diagnosis and personalized desensitization therapy. Methods: A total of 55 patients who visited Huishan District People's Hospital of Wuxi City, Xishan People's Hospital of Wuxi City, and People's Hospital of Wuxi City from January 2023 to December 2023 were selected. The results of serum total IgE (total IgE) and specific IgE detection were analyzed. Results: Among the 55 patients, 54 (98.2%) were positive (cutoff value > 0.35 Ua/mL, the normal value for serum total IgE is under 150–400 IU/mL) for serum total IgE antibodies, 52 (94.6%) were positive for Dermatophagoides pteronyssinus (Dp)–specific IgE antibodies, and 50 (90.9%) were positive for Dermatophagoides farinae (Df)–specific IgE antibodies. The protein chip results were consistent with the existing Phadia detection results. Conclusion: The development of chips based on CRD (component‐resolved diagnosis) cannot only rely on IgE binding rate to assist in precise treatment of patients but also choose personalized desensitization therapy, providing more practical recommendations for preventing cross‐allergy, avoiding allergic environments, or ingesting sensitizing foods for susceptible individuals.

Abstract:Background: The role of specific IgE (sIgE) against Der p 1 and Der p 2 in Chinese patients with house dust mite (HDM) allergy has not yet been well investigated. Methods: One hundred patients were enrolled, based on sensitization and doctor-diagnosed allergy to HDM. Questionnaires were administered to document demographic and clinical characteristics. Serum IgE reactivity to Dermatophagoides pteronyssinus (Dp) extract, Der p 1, Der p 2 and Der p 10 was measured by ImmunoCAP. Results: Almost all patients were sensitized to Der p 1 (95%) and Der p 2 (93%), with both allergens together being largely responsible for the total anti-HDM IgE response. No evidence for a significant role of Der p 10 was found. Overall, IgE responses to HDM and its 2 major allergens were higher in children than in adults in this cross-sectional study. With increasing age, IgE responses to Der p 2 become more important. A positive correlation was observed between the reaction of sIgE against Dp, Der p 1 and Der p 2 and the number of organs (including the eyes, nose, lungs and skin) that were affected in patients. Conclusions: In China, Der p 1 and Der p 2 are the dominant allergens in patients with HDM allergy. The relative importance of Der p 1 and Der p 2 changes with age, in favor of Der p 2. Abstract:Background: IgE to cross-reacting carbohydrate determinants has already been described by several authors, but their function and distribution are still a matter of debate. In previous studies we showed how the presence of IgE to bromelain could be a useful and simple marker of the presence of IgE to carbohydrate epitopes. Methods: A survey of 1,831 subjects with a suspected allergic respiratory disease has been carried out by detecting IgE to bromelain. Data were analysed on the basis of demographical and allergological parameters. To find out whether a glycoprotein is capable of triggering an allergic reaction, 1,076 subjects were also skin tested with several purified molecules bearing carbohydrate side chains differing in number, composition and complexity. Results: An overall prevalence of 23% of positive IgE to cross-reacting carbohydrate determinants was recorded. Prevalence varied when subsets of non-allergic (5%), non-pollen-allergic (10%), and pollen-allergic (31%) subjects were considered. Prevalence further increased in subsets with multiple pollen sensitization (71%), and with a previous pollen immunotherapy course (46%), whereas minor differences were found in gender and age distribution. Almost all the allergenic extracts recorded negative in the skin test gave a positive IgE test in vitro. A higher correlation was found mainly with plant-derived allergenic extracts, whereas a lower one was recorded with mites and fungi. Horseradish peroxidase was the only glycoprotein capable of exerting a positive skin test in 21% of the subjects with IgE to cross-reacting carbohydrate determinants, 80% of them having IgE to the HRP molecule. Conclusions: IgE to cross-reacting carbohydrate determinants are common among the allergic population and the binding to skin test negative allergenic extracts further confirms their poor biological activity. Further studies on horseradish peroxidase should be carried out to define the role of the glycan side chains in its allergenic activity.
A multicenter analysis on the changes of sIgE in China during the early period of COVID-19 pandemic

Abstract:Objective: This study aims to analyze the changes in allergen composition ratio during the early stages of the COVID-19 outbreak in China and analyze the underlying factors contributing to these alterations. Methods: A multicenter study approach was employed. A total of 618 male and female patients (0-89 years old) were recruited from the dermatology, pediatrics, and allergy departments of 17 hospitals across 15 Chinese cities between January 2020 and June 2021. Serum samples were collected and subjected to allergen-specific immunoglobulin E (sIgE) detection using the reversed enzyme allegro-sorbent test. The allergens included Dermatophagoides pteronyssinus (D. pteronyssinus), Dermatophagoides farina (D. farina), Ambrosia artemisiifolia (A. artemisiifolia), Salix babylonica (S. babylonica), dog dander, alternaria, cockroach, Artemisia argyi (A. argyi), cat dander, house dust, milk, hen's egg, mutton, cod, peanut, beef, soybean, shrimp, crab, and wheat. Each participant was required to exhibit at least one positive sIgE detection result (≥0.35 IU/mL). The chi-square test was used to analyze the differences between groups. Results: The positive sIgE proportion of inhalation allergens were significantly lower than that of ingestion allergens. As restrictions on outdoor activities and seafood imports persisted, the positive sIgE proportion of wheat and shrimp exhibited a significant decrease in the spring of 2021 compared to the corresponding period in 2020. Conversely, there was a substantial increase in the positive proportion of crab. The fluctuation in the ratio of D. pteronyssinus and D. farina appeared to be influenced more by seasonal factors rather than the COVID-19 pandemic. However, no noteworthy disparities were observed in the proportions of other allergens. Conclusion: The alterations in allergen composition during the initial phase of the epidemic may be attributed to several factors, such as decreased travel, increased mask usage, reduced carbohydrate consumption, and changes in seafood consumption. However, factors such as season, cultural practices, and customs may also influence the composition of allergens.

Abstract:An 86 year-old male presented with progressive shortness of breath and a murmur. Six years earlier, he had undergone mitral valve surgery. An echocardiogram revealed a significant left ventricular-to-right atrial connection (Gerbode defect). The defect was closed percutaneously using an Amplatzer Septal Occluder device. After the procedure, the patient developed transient hemolysis and renal dysfunction. A year later, there was no residual flow on echocardiography and the patient reported marked improvement in exercise tolerance.
