Abstract:The COVID-19 outbreak has caused significant stress in our lives, which potentially increases frustration, fear, and resentful emotions. Managing stress is complex, but helps to alleviate negative psychological effects. In order to understand how the public coped with stress during the COVID-19 pandemic, we used Macao as a case study and collected 104,827 COVID-19 related posts from Facebook through data mining, from 1 January to 31 December 2020. Divominer, a big-data analysis tool supported by computational algorithm, was employed to identify themes and facilitate machine coding and analysis. A total of 60,875 positive messages were identified, with 24,790 covering positive psychological themes, such as "anti-epidemic", "solidarity", "hope", "gratitude", "optimism", and "grit". Messages that mentioned "anti-epidemic", "solidarity", and "hope" were the most prevalent, while different crisis stages, key themes and media elements had various impacts on public involvement. To the best of our knowledge, this is the first-ever study in the Chinese context that uses social media to clarify the awareness of solidarity. Positive messages are needed to empower social media users to shoulder their shared responsibility to tackle the crisis. The findings provide insights into users' needs for improving their subjective well-being to mitigate the negative psychological impact of the pandemic.
Alternative health groups on social media, misinformation, and the (de)stabilization of ontological security

Abstract:New Media &Society, Ahead of Print. Through 19 semistructured interviews, we explore why people join and participate in "alternative health" Facebook groups and Reddit subs. Participation in these groups creates an ontological circle where people's feelings of fear, desperation, and distrust in the systems and actors that comprise our government, health, and news systems inspire alternative information and support-seeking in these groups and subs. While participation assuages those feelings and stabilizes people's sense of ontological security, it also destabilizes it, reinforcing and legitimizing feelings of fear, desperation, and distrust. This circle contributes to favorable receptive conditions for spreading unproven and dangerous health misinformation. We argue that it will be difficult to address misinformation (1) without considering and rectifying people's often valid reasons for feelings of fear, desperation, distrust, and their desires for information and (2) without considering the relationships between alternative health social media groups and subs and the (de)stabilization of ontological security.

Health Information Seeking Behaviors on Social Media During the COVID-19 Pandemic Among American Social Networking Site Users: Survey Study

Abstract:Background In recent years, medical journals have emphasized the increasingly critical role that social media plays in the dissemination of public health information and disease prevention guidelines. However, platforms such as Facebook and Twitter continue to pose unique challenges for clinical health care providers and public health officials alike. In order to effectively communicate during public health emergencies, such as the COVID-19 pandemic, it is increasingly critical for health care providers and public health officials to understand how patients gather health-related information on the internet and adjudicate the merits of such information. Objective With that goal in mind, we conducted a survey of 1003 US-based adults to better understand how health consumers have used social media to learn and stay informed about the COVID-19 pandemic, the extent to which they have relied on credible scientific information sources, and how they have gone about fact-checking pandemic-related information. Methods A web-based survey was conducted with a sample that was purchased through an industry-leading market research provider. The results were reported with a 95% confidence level and a margin of error of 3. Participants included 1003 US-based adults (aged ≥18 years). Participants were selected via a stratified quota sampling approach to ensure that the sample was representative of the US population. Balanced quotas were determined (by region of the country) for gender, age, race, and ethnicity. Results The results showed a heavy reliance on social media during the COVID-19 pandemic; more than three-quarters of respondents (762/1003, 76%) reported that they have relied on social media at least “a little,” and 59.2% (594/1003) of respondents indicated that they read information about COVID-19 on social media at least once per week. According to the findings, most social media users (638/1003, 63.6%) were unlikely to fact-check what they see on the internet with a health professional, despite the high levels of mistrust in the accuracy of COVID-19–related information on social media. We also found a greater likelihood of undergoing vaccination among those following more credible scientific sources on social media during the pandemic (χ216=50.790; φ=0.258; P<.001). Conclusions The findings suggest that health professionals will need to be both strategic and proactive when engaging with health consumers on social media if they hope to counteract the deleterious effects of misinformation and disinformation. Effective training, institutional support, and proactive collaboration can help health professionals adapt to the evolving patterns of health information seeking.

Abstract:Objective The initial emergence of SARS-CoV-2 created uncertainty for humanity, driving people to seek assistance on social media. This study aims to understand the role of social media in coping with crises and to offer guidance for future uncertainties by examining the experiences of Wuhan during the early stages of the pandemic. Methods Using quantitative content analysis, this study investigated 2207 Weibo posts tagged with "COVID-19 Mutual Aid" from individuals located in Wuhan during the early lockdown period from January 23, 2020, to March 23, 2020. Results At the start of pandemic, messages seeking tangible support were most common. A hurdle regression model showed that deeper self-disclosure led to more retransmission of help-seeking messages. The Chi-Square and Mann-Whitney U tests revealed that health professionals and laypeople had different self-disclosure strategies. Conclusions This study provides insight into the online social support exchange during the early stages of the COVID-19 pandemic in Wuhan, highlighting the importance of self-disclosure on message retransmission, and the differences in self-disclosure strategies between health professionals and laypeople in online help-seeking.

Abstract:Online health information is critical during pandemics. Previous research has focused on examining antecedents or consequences of particular information behaviors (e.g., seeking, sharing), but the process by which one information behavior influences or transforms into other information behaviors remains poorly understood. Guided by theories of information behavior and the literature on online misinformation, this study proposes an interaction model of online information behaviors that theorizes relationships among online information scanning, misinformation exposure, misinformation elaboration, information sharing, and information avoidance. Conducting a two-wave representative panel survey in Hong Kong during the COVID-19 pandemic ( N = 1501), we found that online information scanning at Wave 1 had a direct, positive impact on misinformation exposure and information sharing at Wave 2, while it did not have an impact on information avoidance at Wave 2. Additionally, misinformation exposure was positively related to both information sharing and information avoidance at Wave 2. Importantly, we underlined that evaluations of crisis-related misinformation are aided by misinformation elaboration, which plays a moderating role in catalyzing appropriate information behaviors. Results of this study could help scholars and practitioners propose evidence-based interventions for enhancing the public's ability to manage crisis information on the Internet in times of heightened uncertainty.

Abstract:BACKGROUND: Social network sites (SNSs) are being increasingly used to exchange health information between patients and practitioners, pharmaceutical companies, and research centers. Research contributions have explored the contents of such exchanges discussed online. They have categorized the topics discussed and explored the engagement levels of these discussions.OBJECTIVE: This research aimed at investigating the potential role of SNSs in health care. Specifically it provides an information-clustering analysis of the health information available on SNSs and develops a research design that allows an investigation of this information in enhancing health care research and delivery. In addition, this research aims at testing whether SNSs are valid tools for sharing drug-related information by patients.METHODS: This research is based on a specific chronic disease: multiple sclerosis. We searched Facebook to identify and research the social media groups related to this condition. The analysis was restricted to public groups for privacy concerns. We created a database by downloading posts from two main groups (in the English language). Subsequently, we performed a content analysis and statistical analysis; this allowed us to explore the differences between categories, their engagement levels, and the types of posts shared. The mean level of engagement for each topic was analyzed using a 1-way analysis of variance.RESULTS: From a sample of 7029 posts, initial results showed that there were 8 information categories that resonated (percentage of times the topic appears in our sample) with those who post on Facebook: information and awareness (4923/7029, 70.04%), event advertising and petitions (365/7029, 5.19%), fundraising (354/7029, 5.04%), patient support (217/7029, 3.09%), drug discussion (144/7029, 2.05%), clinical trials and research studies (59/7029, 0.84%), product and drug advertising (48/7029, 0.68%), and other (919/7029, 13.07%). Initial analysis showed that comments and likes (as measures of engagement level) are the most frequent indicators and measures of level of engagement. Our results show a high engagement level (in terms of views, likes, comments, etc) for patient support and information and awareness. In addition, although drug discussion had a low resonance, it had an unexpected highly engagement level which we found worthy of further exploration.CONCLUSIONS: SNSs have become important tools for patients and health care practitioners to share or seek information. We identified the type of information shared and how the public reacted to it. Our research confirmed that the topics discussed in social media related to specific diseases such as multiple sclerosis are similar to the information categories observed by other researchers. We unexpectedly found other categories such as drug discussion. These and other results of our study enhance our understanding of how content is disseminated and perceived within a specific disease-based community. We concluded that this information has useful implications in the design of prevention campaigns, educational programs, and chronic disease management.
Using Tweets to Understand How COVID-19–Related Health Beliefs Are Affected in the Age of Social Media: Twitter Data Analysis Study

Abstract:Background The emergence of SARS-CoV-2 (ie, COVID-19) has given rise to a global pandemic affecting 215 countries and over 40 million people as of October 2020. Meanwhile, we are also experiencing an infodemic induced by the overabundance of information, some accurate and some inaccurate, spreading rapidly across social media platforms. Social media has arguably shifted the information acquisition and dissemination of a considerably large population of internet users toward higher interactivities. Objective This study aimed to investigate COVID-19-related health beliefs on one of the mainstream social media platforms, Twitter, as well as potential impacting factors associated with fluctuations in health beliefs on social media. Methods We used COVID-19-related posts from the mainstream social media platform Twitter to monitor health beliefs. A total of 92,687,660 tweets corresponding to 8,967,986 unique users from January 6 to June 21, 2020, were retrieved. To quantify health beliefs, we employed the health belief model (HBM) with four core constructs: perceived susceptibility, perceived severity, perceived benefits, and perceived barriers. We utilized natural language processing and machine learning techniques to automate the process of judging the conformity of each tweet with each of the four HBM constructs. A total of 5000 tweets were manually annotated for training the machine learning architectures. Results The machine learning classifiers yielded areas under the receiver operating characteristic curves over 0.86 for the classification of all four HBM constructs. Our analyses revealed a basic reproduction number R0 of 7.62 for trends in the number of Twitter users posting health belief–related content over the study period. The fluctuations in the number of health belief–related tweets could reflect dynamics in case and death statistics, systematic interventions, and public events. Specifically, we observed that scientific events, such as scientific publications, and nonscientific events, such as politicians’ speeches, were comparable in their ability to influence health belief trends on social media through a Kruskal-Wallis test (P=.78 and P=.92 for perceived benefits and perceived barriers, respectively). Conclusions As an analogy of the classic epidemiology model where an infection is considered to be spreading in a population with an R0 greater than 1, we found that the number of users tweeting about COVID-19 health beliefs was amplifying in an epidemic manner and could partially intensify the infodemic. It is “unhealthy” that both scientific and nonscientific events constitute no disparity in impacting the health belief trends on Twitter, since nonscientific events, such as politicians’ speeches, might not be endorsed by substantial evidence and could sometimes be misleading.

Abstract:Purpose Health-related online fake news (HOFN) has become a major social problem. HOFN can lead to the spread of ineffective and even harmful remedies. The study aims to understand Internet users' responses to HOFN during the coronavirus (COVID-19) pandemic using the protective action decision model (PADM). Design/methodology/approach The authors collected pandemic severity data (regional number of confirmed cases) from government websites of the USA and China (Studies 1 and 2), search behavior from Google and Baidu search engines (Studies 1 and 2) and data regarding trust in two online fake news stories from two national surveys (Studies 2 and 3). All data were analyzed using a multi-level linear model. Findings The research detected negative time-lagged relationships between pandemic severity and regional HOFN search behavior by three actual fake news stories from the USA and China (Study 1). Importantly, trust in HOFN served as a mediator in the time-lagged relationship between pandemic severity and search behavior (Study 2). Additionally, the relationship between pandemic severity and trust in HOFN varied according to individuals' perceived control (Study 3). Originality/value The authors' results underscore the important role of PADM in understanding Internet users' trust in and search for HOFN. When people trust HOFN, they may seek more information to implement further protective actions. Importantly, it appears that trust in HOFN varies with environmental cues (regional pandemic severity) and with individuals' perceived control, providing insight into developing coping strategies during a pandemic.

Abstract:Social media is the most crucial part of our daily lives. Today the internet and World Wide Web are being used to influence people around the globe. However, the information available on social media struggles to maintain its credibility because anyone may upload content. Currently, it has been observed that Facebook being the biggest social media platform plays a significant role in the dissemination of information about the COVID-19 pandemic. The objective of this study was to investigate the information about COVID-19 on Pakistan's biggest page dedicated to COVID-19 known as Corona Recovered Warriors. The researcher conducted a qualitative and quantitative Facebook content analysis. The content was restricted to COVID-19 information available on the Corona Recovered Warriors Facebook page. The posts from June, September, and December were selected to analyze the change in themes of information over time. In Pakistan, in June, the highest number of daily COVID-19 positive cases are reported, September because of the ease in lockdown and the lowest number of COVID-19 daily cases, and then December when the graph of COVID-19 positive case again rises. A total of 28381 posts were analyzed in three months, and the focus of the posts was on Experience sharing (n=7934), Seeking or clarifying personal status (n=8419), Raising awareness on COVID-19 (n=2614), Support for patient and caregiver (n= 2597), and Product and service promotion (n=3341). A significant link between the number of posts on the Facebook page and the crucial and easy months related to COVID-19 positive cases has also been detected. COVID-19-positive patients and their relatives or family members share their health information widely with other Facebook users, also highlighting the need for collaboration between healthcare providers and media experts to design appropriate interventions for responding to pandemic diseases.
What social media told about us in the time of COVID-19: a scoping review

Abstract:With the onset of COVID-19 pandemic, social media has rapidly become a crucial communication tool for information generation, dissemination, and consumption. In this scoping review, we selected and examined peer-reviewed empirical studies relating to COVID-19 and social media during the first outbreak starting in November 2019 until May 2020. From an analysis of 81 studies, we identified five overarching public health themes concerning the role of online social platforms and COVID-19. These themes focused on: (i) surveying public attitudes, (ii) identifying infodemics, (iii) assessing mental health, (iv) detecting or predicting COVID-19 cases, (v) analyzing government responses to the pandemic, and (vi) evaluating quality of health information in prevention education videos. Furthermore, our review highlights the paucity of studies on the application of machine learning on social media data related to COVID-19 and a lack of studies documenting real-time surveillance developed with social media data on COVID-19. For COVID-19, social media can play a crucial role in disseminating health information as well as tackling infodemics and misinformation.
Tracking Self-reported Symptoms and Medical Conditions on Social Media During the COVID-19 Pandemic: Infodemiological Study

Abstract:Background: Harnessing health-related data posted on social media in real time can offer insights into how the pandemic impacts the mental health and general well-being of individuals and populations over time. Objective: This study aimed to obtain information on symptoms and medical conditions self-reported by non-Twitter social media users during the COVID-19 pandemic, to determine how discussion of these symptoms and medical conditions changed over time, and to identify correlations between frequency of the top 5 commonly mentioned symptoms post and daily COVID-19 statistics (new cases, new deaths, new active cases, and new recovered cases) in the United States. Methods: We used natural language processing (NLP) algorithms to identify symptom- and medical condition-related topics being discussed on social media between June 14 and December 13, 2020. The sample posts were geotagged by NetBase, a third-party data provider. We calculated the positive predictive value and sensitivity to validate the classification of posts. We also assessed the frequency of health-related discussions on social media over time during the study period, and used Pearson correlation coefficients to identify statistically significant correlations between the frequency of the 5 most commonly mentioned symptoms and fluctuation of daily US COVID-19 statistics. Results: Within a total of 9,807,813 posts (nearly 70% were sourced from the United States), we identified a discussion of 120 symptom-related topics and 1542 medical condition-related topics. Our classification of the health-related posts had a positive predictive value of over 80% and an average classification rate of 92% sensitivity. The 5 most commonly mentioned symptoms on social media during the study period were anxiety (in 201,303 posts or 12.2% of the total posts mentioning symptoms), generalized pain (189,673, 11.5%), weight loss (95,793, 5.8%), fatigue (91,252, 5.5%), and coughing (86,235, 5.2%). The 5 most discussed medical conditions were COVID-19 (in 5,420,276 posts or 66.4% of the total posts mentioning medical conditions), unspecified infectious disease (469,356, 5.8%), influenza (270,166, 3.3%), unspecified disorders of the central nervous system (253,407, 3.1%), and depression (151,752, 1.9%). Changes in posts in the frequency of anxiety, generalized pain, and weight loss were significant but negatively correlated with daily new COVID-19 cases in the United States (r=-0.49, r=-0.46, and r=-0.39, respectively; P<.05). Posts on the frequency of anxiety, generalized pain, weight loss, fatigue, and the changes in fatigue positively and significantly correlated with daily changes in both new deaths and new active cases in the United States (r ranged=0.39-0.48; P<.05). Conclusions: COVID-19 and symptoms of anxiety were the 2 most commonly discussed health-related topics on social media from June 14 to December 13, 2020. Real-time monitoring of social media posts on symptoms and medical conditions may help assess the population's mental health status and enhance public health surveillance for infectious disease.
Exploring the health information seeking behavior of social media users under the background of COVID-19 pandemic: An empirical study based on social cognitive theory

Abstract:With the outbreak of COVID-19 in late 2019, people's awareness of actively searching for health information has been growing. Coupled with the promotion of "Internet + medical and health," social media, as an important platform for health information dissemination, has become one of the important information sources for users to obtain health information. However, health information seeking behavior in public health emergencies are quite different from those in daily life. Therefore, the research question of this paper is to explore the influencing factors of health information seeking behavior of social media users in the context of COVID-19. To this end, based on the research framework of social cognition theory, this paper selects six variables to construct a structural equation modeling, including information and platform quality, experience, social support, emotional arousal, self-efficacy, and social media users' health information seeking behavior. The empirical study is carried out by collecting 219 valid questionnaires. The research results show that: (1) Information and platform quality, and use experience have a significant positive effect on emotional arousal; (2) Experience, social support, and emotional arousal have a significant positive effect on self-efficacy; (3) Emotional arousal and self-efficacy have a significant positive effect on social media users' health information seeking behavior. The research provides an important theoretical reference for social media users and operators to cope with the huge demand for health information in the post-COVID-19 era.
Platform Effects on Public Health Communication: A Comparative and National Study of Message Design and Audience Engagement Across Twitter and Facebook

Abstract:Background: Public health agencies widely adopt social media for health and risk communication. Moreover, different platforms have different affordances, which may impact the quality and nature of the messaging and how the public engages with the content. However, these platform effects are not often compared in studies of health and risk communication and not previously for the COVID-19 pandemic. Objective: This study measures the potential media effects of Twitter and Facebook on public health message design and engagement by comparing message elements and audience engagement in COVID-19–related posts by local, state, and federal public health agencies in the United States during the pandemic, to advance theories of public health messaging on social media and provide recommendations for tailored social media communication strategies. Methods: We retrieved all COVID-19–related posts from major US federal agencies related to health and infectious disease, all major state public health agencies, and selected local public health departments on Twitter and Facebook. A total of 100,785 posts related to COVID-19, from 179 different accounts of 96 agencies, were retrieved for the entire year of 2020. We adopted a framework of social media message elements to analyze the posts across Facebook and Twitter. For manual content analysis, we subsampled 1677 posts. We calculated the prevalence of various message elements across the platforms and assessed the statistical significance of differences. We also calculated and assessed the association between message elements with normalized measures of shares and likes for both Facebook and Twitter. Results: Distributions of message elements were largely similar across both sites. However, political figures ( P <.001), experts ( P =.01), and nonpolitical personalities ( P =.01) were significantly more present on Facebook posts compared to Twitter. Infographics ( P <.001), surveillance information ( P <.001), and certain multimedia elements (eg, hyperlinks, P <.001) were more prevalent on Twitter. In general, Facebook posts received more (normalized) likes (0.19%) and (normalized) shares (0.22%) compared to Twitter likes (0.08%) and shares (0.05%). Elements with greater engagement on Facebook included expressives and collectives, whereas posts related to policy were more engaged with on Twitter. Science information (eg, scientific explanations) comprised 8.5% (73/851) of Facebook and 9.4% (78/826) of Twitter posts. Correctives of misinformation only appeared in 1.2% (11/851) of Facebook and 1.4% (12/826) of Twitter posts. Conclusions: In general, we find a data and policy orientation for Twitter messages and users and a local and personal orientation for Facebook, although also many similarities across platforms. Message elements that impact engagement are similar across platforms but with some notable distinctions. This study provides novel evidence for differences in COVID-19 public health messaging across social media sites, advancing knowledge of public health communication on social media and recommendations for health and risk communication strategies on these online platforms.

Abstract:Background: The COVID-19 vaccine is an effective tool in the fight against the COVID-19 outbreak. As the main channel of information dissemination in the context of the epidemic, social media influences public trust and acceptance of the vaccine. The rational application of health behavior theory is a guarantee of effective public health information dissemination. However, little is known about the application of health behavior theory in web-based COVID-19 vaccine messages, especially from Chinese social media posts. Objective: This study aimed to understand the main topics and communication characteristics of hot papers related to COVID-19 vaccine on the WeChat platform and assess the health behavior theory application with the aid of health belief model (HBM). Methods: A systematic search was conducted on the Chinese social media platform WeChat to identify COVID-19 vaccine-related papers. A coding scheme was established based on the HBM, and the sample was managed and coded using NVivo 12 (QSR International) to assess the application of health behavior theory. The main topics of the papers were extracted through the Latent Dirichlet Allocation algorithm. Finally, temporal analysis was used to explore trends in the evolution of themes and health belief structures in the papers. Results: A total of 757 papers were analyzed. Almost all (671/757, 89%) of the papers did not have an original logo. By topic modeling, 5 topics were identified, which were vaccine development and effectiveness (267/757, 35%), disease infection and protection (197/757, 26%), vaccine safety and adverse reactions (52/757, 7%), vaccine access (136/757, 18%), and vaccination science popularization (105/757, 14%). All papers identified at least one structure in the extended HBM, but only 29 papers included all of the structures. Descriptions of solutions to obstacles (585/757, 77%) and benefit (468/757, 62%) were the most emphasized components in all samples. Relatively few elements of susceptibility (208/757, 27%) and the least were descriptions of severity (135/757, 18%). Heat map visualization revealed the change in health belief structure before and after vaccine entry into the market. Conclusions: To the best of our knowledge, this is the first study to assess the structural expression of health beliefs in information related to the COVID-19 vaccine on the WeChat public platform based on an HBM. The study also identified topics and communication characteristics before and after the market entry of vaccines. Our findings can inform customized education and communication strategies to promote vaccination not only in this pandemic but also in future pandemics.
Investigating the Role of Perceived Information Overload on COVID-19 Fear: A Moderation Role of Fake News Related to COVID-19

Abstract:During crises and uncertain situations such as the coronavirus disease 2019 (COVID-19) pandemic, social media plays a key function because it allows people to seek and share news, as well as personal views and ideas with each other in real time globally. Past research has highlighted the implications of social media during disease outbreaks; nevertheless, this study refers to the possible negative effects of social media usage by individuals in the developing country during the COVID-19 epidemic lockdown. Specifically, this study investigates the COVID-19 fear using the survey data collected from a developing country. In total, 880 entries were used to analyze the COVID-19 fear using the AMOS software. Findings indicated that information-seeking and sharing behavior of individuals on social media has a significant impact on perceived COVID-19 information overload. Perceived COVID-19 information overload has a positive impact on COVID-19 fear. In addition, fake news related to COVID-19 strengthens the relationship between perceived COVID-19 information overload and COVID-19 fear. The implication and limitations of the study are also discussed in the final section of the study.
Health belief model and social media engagement: A cross-national study of health promotion strategies against COVID-19 in 2020

Abstract:Background: Using the Health Belief Model (HBM), this study analyzed tweets related to COVID-19 published by national health departments of the United States, the South Korea, the United Kingdom, Japan, Germany, and India to explore their differences in (1) the health measures against COVID-19, (2) the health promotion strategies, (3) the social media engagements that those measures and strategies have triggered. Method: We conducted a content analysis with 1,200 randomly selected COVID-19-related tweets from six national health departments' Twitter accounts from 1 January 2020 to 31 December 2020. We coded the six HBM constructs and 21 sub-themes of the HBM constructs for each tweet. Results: Results showed that all six HBM constructs were used in the full sample. The most commonly used HBM construct was cues to action, followed by susceptibility, benefits, self-efficacy, severity, and barriers. All the HBM constructs were positively related to Twitter engagement variables except barriers. Further analysis illustrated that people from the six countries responded differently to the HBM constructs and the HBM sub-themes. Twitter users in Germany, India, the U.S., and Japan positively reacted to the clear directions of "what to do against COVID-19" (cues to action), while Twitter users in the U.S. and Japan were also eager to know the justifications for such directions (benefits); people in South Korea and the U.K. were mainly seeking a diagnosis of the severity and susceptibility of COVID-19, instead of health measures, of COVID-19 in the year 2020. Conclusions: This study showed the use of HBM constructs is generally effective in inducing Twitter engagement. The further comparison illustrated a homogenization in the promotion strategies that the health departments implemented and the health measures they promoted, yet responses to such promotions varied across nations. This study broadened the scope of HBM applications from predicting health behaviors in surveys to guiding the design of health promotion messages online.

Abstract:Background The COVID-19 pandemic has created a global health crisis that is affecting economies and societies worldwide. During times of uncertainty and unexpected change, people have turned to social media platforms as communication tools and primary information sources. Platforms such as Twitter and Sina Weibo have allowed communities to share discussion and emotional support; they also play important roles for individuals, governments, and organizations in exchanging information and expressing opinions. However, research that studies the main concerns expressed by social media users during the pandemic is limited. Objective The aim of this study was to examine the main concerns raised and discussed by citizens on Sina Weibo, the largest social media platform in China, during the COVID-19 pandemic. Methods We used a web crawler tool and a set of predefined search terms (New Coronavirus Pneumonia, New Coronavirus, and COVID-19) to investigate concerns raised by Sina Weibo users. Textual information and metadata (number of likes, comments, retweets, publishing time, and publishing location) of microblog posts published between December 1, 2019, and July 32, 2020, were collected. After segmenting the words of the collected text, we used a topic modeling technique, latent Dirichlet allocation (LDA), to identify the most common topics posted by users. We analyzed the emotional tendencies of the topics, calculated the proportional distribution of the topics, performed user behavior analysis on the topics using data collected from the number of likes, comments, and retweets, and studied the changes in user concerns and differences in participation between citizens living in different regions of mainland China. Results Based on the 203,191 eligible microblog posts collected, we identified 17 topics and grouped them into 8 themes. These topics were pandemic statistics, domestic epidemic, epidemics in other countries worldwide, COVID-19 treatments, medical resources, economic shock, quarantine and investigation, patients’ outcry for help, work and production resumption, psychological influence, joint prevention and control, material donation, epidemics in neighboring countries, vaccine development, fueling and saluting antiepidemic action, detection, and study resumption. The mean sentiment was positive for 11 topics and negative for 6 topics. The topic with the highest mean of retweets was domestic epidemic, while the topic with the highest mean of likes was quarantine and investigation. Conclusions Concerns expressed by social media users are highly correlated with the evolution of the global pandemic. During the COVID-19 pandemic, social media has provided a platform for Chinese government departments and organizations to better understand public concerns and demands. Similarly, social media has provided channels to disseminate information about epidemic prevention and has influenced public attitudes and behaviors. Government departments, especially those related to health, can create appropriate policies in a timely manner through monitoring social media platforms to guide public opinion and behavior during epidemics.

Abstract:The growing popularity of social media and its ubiquitous presence in our lives brings associated risks such as the spread of mis- and disinformation, particularly when these may be unregulated in times of global crises. Online communities are able to provide support by enabling connection with others and also provide great potential for dynamic interaction and timely dissemination of information compared with more traditional methods. This study evaluates interactions within the Essex Coronavirus Action/Support Facebook private group, which set out to prevent the spread of COVID-19 infection by informing Essex residents of guidance and helping vulnerable individuals. At the outset, 18 community administrators oversaw the group, which attracted approximately 37,900 members. Longitudinal Facebook group interactions across five periods spanning the UK lockdowns 2020–2021 were analysed using psychological discourse analysis and supplementary computed-mediated analysis to further explore sentiment and linguistic features. The findings endorsed that the group provided a protected space for residents to express their feelings in times of crises and an opportunity to address confusion and concern. The effective communication of public health messages was facilitated by promoting desired interaction and the construction of group identities. Administrators worked with group members to achieve a shared understanding of others' perspectives and the COVID-19 evidence base, which led to a mobilisation of the provision of support in the community. This was accomplished through the application of rhetorical and interactional devices. This study demonstrates how online groups can employ discursive strategies to engage audiences, build cohesion, provide support, and encourage health protective behaviours. This has implications for public health teams in terms of designing, implementing, or evaluating such interventions.
Online Information Exchange and Anxiety Spread in the Early Stage of the Novel Coronavirus (COVID-19) Outbreak in South Korea: Structural Topic Model and Network Analysis

Abstract:Background In case of a population-wide infectious disease outbreak, such as the novel coronavirus disease (COVID-19), people’s online activities could significantly affect public concerns and health behaviors due to difficulty in accessing credible information from reliable sources, which in turn causes people to seek necessary information on the web. Therefore, measuring and analyzing online health communication and public sentiment is essential for establishing effective and efficient disease control policies, especially in the early stage of an outbreak. Objective This study aimed to investigate the trends of online health communication, analyze the focus of people’s anxiety in the early stages of COVID-19, and evaluate the appropriateness of online information. Methods We collected 13,148 questions and 29,040 answers related to COVID-19 from Naver, the most popular Korean web portal (January 20, 2020, to March 2, 2020). Three main methods were used in this study: (1) the structural topic model was used to examine the topics in the online questions; (2) word network analysis was conducted to analyze the focus of people’s anxiety and worry in the questions; and (3) two medical doctors assessed the appropriateness of the answers to the questions, which were primarily related to people’s anxiety. Results A total of 50 topics and 6 cohesive topic communities were identified from the questions. Among them, topic community 4 (suspecting COVID-19 infection after developing a particular symptom) accounted for the largest portion of the questions. As the number of confirmed patients increased, the proportion of topics belonging to topic community 4 also increased. Additionally, the prolonged situation led to a slight increase in the proportion of topics related to job issues. People’s anxieties and worries were closely related with physical symptoms and self-protection methods. Although relatively appropriate to suspect physical symptoms, a high proportion of answers related to self-protection methods were assessed as misinformation or advertisements. Conclusions Search activity for online information regarding the COVID-19 outbreak has been active. Many of the online questions were related to people’s anxieties and worries. A considerable portion of corresponding answers had false information or were advertisements. The study results could contribute reference information to various countries that need to monitor public anxiety and provide appropriate information in the early stage of an infectious disease outbreak, including COVID-19. Our research also contributes to developing methods for measuring public opinion and sentiment in an epidemic situation based on natural language data on the internet.

Abstract:The COVID-19 outbreak, a recent major public health emergency, was the first national health crisis since China entered the era of mobile social media. In this context, the public posted many quarantine-related posts for help on social media. Most previous studies of social media during the pandemic focused only on people's emotional needs, with less analysis of quarantine help-seeking content. Based on this situation, this study analyzed the relationship between the number of quarantine help-seeking posts and the number of new diagnoses at different time points in the pandemic using Zhihu, the most comprehensive topic discussion platform in China. It showed a positive correlation between the number of help-seeking posts and the pandemic's severity. Given the diversity of people's help-seeking content, this study used topic model analysis and sentiment analysis to explore the key content of people's quarantine help-seeking posts during the pandemic. In light of the framework of uses and gratifications, we found that people posted the most questions in relation to help with information related to pandemic information and quarantine information. Interestingly, the study also found that the content of people's quarantine posts during the pandemic was primarily negative in sentiment. This study can thus help the community understand the changes in people's perceptions, attitudes, and concerns through their reactions to emergencies and then formulate relevant countermeasures to address pandemic control and information regulation, which will have implications for future responses to public health emergencies. Moreover, in terms of psychological aspects, it will help implement future mental health intervention strategies and better address the public's psychological problems.
