Abstract:Background: We examined whether there are racial disparities in pain management, opioid medicine prescriptions, symptom severity, and quality of life constructs in breast cancer survivors. Methods: We conducted a secondary analysis of longitudinal data from the Women's Hormonal Therapy Initiation and Persistence (WHIP) study (n = 595), a longitudinal study of hormonal receptor-positive breast cancer survivors. Upon study enrollment, patients completed a survey assessing an array of psychological, behavioral, and treatment outcomes, including adjuvant endocrine therapy (AET)-induced symptoms, and provided a saliva biospecimen. Opioid prescription records were extracted from the health maintenance organizations (HMOs) pharmacy database. The final analytic sample included women with complete HMO pharmacy records for 1 year. Results: There were 251 eligible patients, of which 169 (67.3%) were White. The average age was 61.09 years old (SD = 11.07). One hundred seventy-two patients (68.5%) had received at least one opioid medication and 37.1% were prescribed opioids longer than 90 days (n = 93). Sixty-four Black patients (78%) had a record of being prescribed with opioids compared to 64% of White patients (n = 108, p = 0.03). Black patients reported worse vasomotor, neuropsychological, and gastrointestinal symptoms, as well as lower quality of life and greater healthcare discrimination than White patients (p's < 0.05). Black patients were more likely to be prescribed opioids for 90 days or longer compared to White patients, when controlling for age, marital status, income, body mass index (BMI), cancer stage, and chemotherapy status (adjusted Odds Ratio = 2.72, p = 0.014). Conclusion: Findings indicate that there are racial differences in opioid prescriptions supplied for pain management and symptomatic outcomes. Future research is needed to understand the causes of disparities in cancer pain management and symptomatic outcomes.

Abstract:Abstract Cancer-related pain affects a large proportion of all patients with cancer yet remains inadequately managed, particularly among patients from certain racialized backgrounds. Recently, there has been increased research and clinical interest in the use of medical cannabis for cancer pain management, including its potential to ameliorate race-based disparities in cancer pain control. Although medical cannabis is not currently a US Food and Drug Administration–approved treatment option for cancer-related pain, many oncologists discuss and recommend its use with their patients, underscoring the need for researchers and clinicians to proactively identify barriers to cannabis for cancer pain management that may disproportionately affect patients from certain racial groups. In this commentary, we highlight challenges that patients from racialized backgrounds may face when incorporating cannabis into their palliative care regimens and discuss opportunities for researchers and clinicians to address these challenges should medical cannabis become a recommended treatment option for cancer pain management. In particular, we identify challenges at the structural (eg, lack of insurance coverage), clinician (eg, racialized stereotypes regarding addiction and pain), and individual (eg, internalized stigma) levels and emphasize the importance of multilevel approaches in combating these challenges as the evidence base regarding medical cannabis and its potential harms and therapeutic benefits continues to accumulate.

Abstract:Racial and ethnic disparities in pain management pose major challenges to equitable cancer care delivery. These disparities are driven by complex interactions between patient-, provider-, and system-related factors that resist reductionistic solutions and require innovative, holistic approaches. On September 19, 2022, the Society for Integrative Oncology and the American Society of Clinical Oncology published a joint guideline to provide evidence-based recommendations on integrative medicine for cancer pain management. Integrative medicine, which combines conventional treatments with complementary modalities from cultures and traditions around the world, are uniquely equipped to resonate with diverse cancer populations and fill existing gaps in pain management. Although some complementary modalities, such as music therapy and yoga, lack sufficient evidence to make a specific recommendation, other modalities, such as acupuncture, massage, and hypnosis, demonstrated an intermediate level of evidence, resulting in moderate strength recommendations for their use in cancer pain management. However, several factors may hinder real-world implementation of the Society for Integrative Oncology and the American Society of Clinical Oncology guideline and must be addressed to ensure equitable pain management for all communities. These barriers include, but are not limited to, the lack of insurance coverage for many complementary therapies, the limited diversity and availability of complementary therapy providers, the negative social norms surrounding complementary therapies, the underrepresentation of racial and ethnic subgroups in the clinical research of complementary therapies, and the paucity of culturally attuned interventions tailored to diverse individuals. This commentary examines both the challenges and the opportunities for addressing racial and ethnic disparities in cancer pain management through integrative medicine.

Abstract:Background: In the United States, colorectal cancer (CRC) is a public health concern, as it is the second leading cause of cancer-related death, accounting for over 50,000 deaths annually. In addition to its high mortality, CRC is associated with significant comorbidities including pain and psychiatric symptoms, particularly in late-stage disease. Supportive care medications (SCM) are frequently used for symptom management and improving health-related Quality of Life (HRQoL). Racial disparities in clinical outcomes including HRQoL have been reported in CRC. While racial disparities have been described in the use of SCM for some cancer types, no investigation of SCM use in late-stage CRC have been performed. The purpose of our study was to explore the relationship between SCM use and race/ethnicity in patients with metastatic CRC. Methods: We used the National Cancer Institute Surveillance Epidemiology and End Results (NCI SEER)- Medicare-linked database to analyze all patients with a CRC diagnosis and a newly initiated outpatient prescription claim for an SCM of interest from 2009-2018. Patients on SCM before cancer diagnosis or who had non-metastatic CRC at diagnosis were excluded. We assessed six SCM classes: opioids, non-opioid analgesics, skeletal muscle relaxants, antidepressants, anxiolytics, and antipsychotics. Results: After application of inclusion/exclusion criteria, 28, 212 patients were included in the final analysis (White: 72.8%, Black: 12.7%, Hispanic: 9.5%, Asian: 4.6%, Native American: 0.4%). After controlling for socioeconomic factors and compared to non-Hispanic White patients, there were significant differences across multiple SCM drug classes. When considering pain medications, these differences were bi-directional. Black patients were less likely to receive opioids (adjusted Odds Ratio [aOR] 0.86, 95% CI: 0.80-0.93), while Hispanic patients were more likely to receive opioids (aOR: 1.12, 95% CI: 1.03-1.22) and non-opioid analgesics (aOR 1.22, 95% CI: 1.06-1.40). Asian patients were more likely to receive non-opioid analgesics (aOR 1.71, 95% CI: 1.45-2.03), but less likely to receive skeletal muscle relaxants (aOR 0.60, 95% CI: 0.43-0.82). When considering psychiatric medications, decreased use among all racial groups was noted for antidepressants (Black: aOR 0.53, 95% CI: 0.50-0.60; Hispanic: aOR 0.82, 95% CI: 0.75-0.90; Asian: aOR 0.48, 95% CI: 0.42-0.56), and anxiolytics (Black: aOR 0.43, 95% CI: 0.38-0.49; Hispanic: aOR 0.76, 95% CI: 0.67-0.85; Asian: aOR 0.45, 95% CI: 0.37-0.55). Among antipsychotics, only Asian patients had less use compared to non-Hispanic White patients. Conclusion: Racial disparities in the use of pain and psychiatric medications among patients with late-stage CRC were present, even after controlling for multiple confounders. Abstract:Background: Persistent opioid use (POU) in breast cancer survivors after they have completed active cancer treatment is an emerging area of concern. Disparities affecting the risk of POU remains understudied in breast cancer survivors. We evaluated disparities in examining the risk with POU in breast cancers survivors in the year after active cancer treatment along with a comparison group. Methods: We assembled a cohort of 14,347 female breast cancer (BC) survivors diagnosed in 2009-2019 treated for early-stage disease (in situ, local, regional), who had at least two opioid prescriptions after one year of survivorship and 51,369 age-matched women without cancer. Data were captured from the tumor registry, electronic health records, and pharmacy databases. The primary outcome was POU defined as continuously receiving at least 90 days supplies of opioids during follow-up. Patients were followed through the date once they met POU criteria, died, disenrolled from health plan, or reached study's end (12/31/2021), whichever came first. We used the multivariable proportional hazard regression to estimate the association [hazard ratios (HR), 95% confidence interval (CI)] between race ethnicity and POU. Results: Overall, the cohort was diverse: 7% Asian/Pacific Islander (API); 14% Black; 28% Hispanic; and 50% White women. During a median follow-up of 6.4 years, 11134 (17%) women developed POU. Patients who developed POU had longer annualized median opioid duration, especially in Black patients (175 vs. 5 days) and were prescribed stronger opioids [median maximum daily morphine milligram equivalent (62.5 vs 42.9)], compared to those who did not develop POU. In the multivariable analyses adjusted for sociodemographic characteristics, opioid type, and other pain medications, Black, Hispanic and API women were 28%, 42% and 54% less likely to develop POU compared to White women [HR (95%CI): 0.72 (0.68-0.76), 0.58 (0.55-0.61), and 0.46 (0.42-0.52), respectively]. However, Black women were only 19% less likely [HR (95%CI): 0.81(0.72-0.92)] to develop POU compared to White women in BC survivors. Also, patients with older age, Elixhauser Comorbidity Index ≥3; smoking, type of opioid (Oxycodone and hydrocodone), and who used other psychiatric and pain medications had significantly greater POU risk. Conclusion: Findings suggest that women of color were less likely to develop POU. Abstract:Prescription opioids still account for a large proportion of overdose deaths and contribute to opioid use dependence (OUD). Studies earlier in the epidemic suggest clinicians were less likely to prescribe opioids to racial/ethnic minorities. As OUD-related deaths have increased disproportionately amongst minority populations, it is essential to understand racial/ethnic differences in opioid prescribing patterns to inform culturally sensitive mitigation efforts. The purpose of this study is to estimate racial/ethnic differences in opioid medication use among patients prescribed opioids. Using electronic health records and a retrospective cohort study design, we estimated multivariable hazard models and generalized linear models, assessing racial/ethnic differences in OUD diagnosis, number of opioid prescriptions, receiving only one opioid prescription, and receiving ≥18 opioid prescriptions. Study population (N=22,201) consisted of adult patients (≥18years), with ≥3 primary care visits (ensuring healthcare system linkage), ≥1 opioid prescription, who did not have an OUD diagnoses prior to the first opioid prescription during the 32-month study period. Relative to racial/ethnic minority patients, White patients, in both unadjusted and adjusted analyses, had a greater number of opioid prescriptions filled, a higher proportion received ≥18 opioid prescriptions, and a greater hazard of having an OUD diagnosis subsequent to receiving an opioid prescription (all groups p<0.001). Although opioid prescribing rates have declined nationally, our findings suggest White patients still experience a high volume of opioid prescriptions and greater risk of OUD diagnosis. Racial/ethnic minorities are less likely to receive follow-up pain medications, which may signal low care quality. Identifying provider bias in pain management of racial/ethnic minorities could inform interventions seeking balance between adequate pain treatment and risk of opioid misuse/abuse.

Although there are multiple sources of this staggering issue, one inciting factor has been the use of opioids to manage pain. Although recent efforts have changed care pathways for patients with chronic pain, the first-line management of cancer pain remains opioids. Despite this, there is very little evidence and no guidelines/regulations to assist in the management of these patients. Although the literature suggests a number of current barriers to identifying and managing the challenges (such as the preferred management of patients with opioid use disorder (OUD), the optimal approach to taperering, or approaches to disparities), there are no concrete strategies for providers to manage these circumstances. Investing in further research utilizing the national opioid settlement funding, exploring the equity gaps using community based participatory research and community health worker models, and increasing provider education pathways are all potential approaches to improving this issue. These solutions could help identify and address some of the gaps that affect cancer patients taking opioids for pain. The number of overdose related deaths has reached abysmal new heights in the United States, greater than 106,000 in 2022. 1 Publications such as a 2016 summary in NEJM have noted that a percentage of deaths were presumed related to Opioid Use Disorder (OUD) resulting from pain management with opioids which prompted a vast change to the approach to chronic pain management with limitations in prescribing opioids long-term. 2 CDC guidelines, as well as state regulations (for example, Virginia) 3 exclude cancer patients from their recommendations. Although this allows for individualized treatment planning, it has also led to a lack of standardized approaches to opioid prescribing for cancer patients. It is still recommended that patients with cancer pain are managed with opioids according to the most recent ASCO guidelines. 4 These guidelines do note that there is a paucity of data to guide management of patients with OUD or non-medical opioid use and cancer pain requiring opioids. Furthermore, it has been well noted that there are disparities in pain management due to a variety of factors, and these findings have even been noted in patients who are nearing end of life. 5 A further exploration of the underlying challenges in the management of pain in cancer patients will help define strategies that may improve treatment pathways. Balance is necessary on this topic, as it is also known that there are a number of current barriers that affect the ability of cancer patients to access opioids at times, which may cause inadequate pain treatment. 6 Opioid use disorder that arises in the context of treating cancer pain is unfortunately not well described. A systematic review of chronic pain management studies from 2022 concluded that, although heterogeneity of the studies meant the result of approximately 8% for the prevalence of OUD was inconclusive, the rate of prevalence of the risk of use disorder was 23.5%. 7 In addition to the unknown rates of OUD in cancer patients, little is known on how to manage this circumstance even when diagnosed. Given the lack of evidence to guide therapeutic options, a consensus-based publication for patients with advanced cancer as well as suspicion of opioid misuse or OUD, was developed using a qualitative survey of 120 palliative or addiction medicine specialists. For patients with untreated OUD there was more consensus on an approach using buprenorphine/naloxone whereas there was less certainty on treatment planning for patients with suspected misuse but undiagnosed OUD. 8 Tapering is an expected outcome according to guidelines for patients whose source of pain resolves. If cancer is likely to be cured or treated successfully long-term, the patient should have expectations for an eventual tapering. During the course of the opioid epidemic, tapering became so fraught as patients were abruptly taken off opioids that the FDA released multiple warnings regarding the risks of harm. 9 The CDC has developed materials to guide clinicians on how to manage the tapering process. 10 Many reviews on tapering, even with a focus on palliative or cancer patients, note extrapolation from chronic non-cancer pain. 11 Despite the lack of evidence, there are considerations that -Abstract Truncated-

This disparity results from undertreatment of pain and is compounded by low adherence to prescribed non-opioid and opioid analgesics among African American patients with cancer. While nearly one in four patients use cannabis to manage cancer-related symptoms, less is known about how cannabis use influences pain relief in this patient population. Methods: This study is based on preliminary data from an ongoing study of longitudinal outcomes of opioid therapy among African American and White patients with cancer. Linear mixed-effects models were utilized to assess the interaction of race and cannabis use on pain relief using "least pain" item scores from the Brief Pain Inventory (BPI) averaged across three time points. Models were adjusted for sociodemographic and clinical variables. Results: This analysis included 136 patients (49 African American, 87 White). Overall, 30.1% of the sample reported cannabis use for cancer pain. The mean "least pain" score on BPI was 3.3 (SD=2.42) on a scale of 0– 10. African American patients had a mean "least pain" score 1.32± 0.48 units higher (indicating lower pain relief) than White patients ( p =0.006). Cannabis use did not have a significant main effect ( p =0.28). However, cannabis use was a significant moderator of the relationship between race and "least pain" ( p =0.03). In the absence of cannabis use, African Americans reported higher "least pain" scores compared to Whites (mean difference=1.631± 0.5, p =0.001). However, this disparity was no longer observed in African American patients reporting cannabis use (mean "least pain" difference=0.587± 0.59, p =0.32). Conclusion: These findings point to the possible role of cannabis in cancer pain management and its potential to reduce racial disparities. These findings are preliminary and further research into the role of cannabis in cancer pain outcomes is needed. A Narrative Review of the Current Research in Cancer-Related Pain Inequities: The Necessity of Applying Intersectionality to Advance Cancer Pain Research

Abstract:Cancer-related pain has a significant impact on quality of life for patients with cancer. In populations without cancer, there are documented pain inequities associated with minoritized racial and/or ethnic groups, women, and low socioeconomic status. However, our understanding of pain inequities specifically among patients with cancer remains incomplete. We narratively synthesized published quantitative research on cancer-related pain inequities in the US in the past decade. A search identified 17 English-language articles examining pain for patients with various cancer types at different treatment stages. Our review revealed mixed findings comparing cancer-related pain by racial group (e.g., Black vs White) and sex (male vs female), but consistent findings indicating that people with lower (vs higher) socioeconomic status and younger (vs older) patients report more cancer-related pain. Research on cancer pain among sexual and gender minorities remains scant. Key research gaps include a need for more research that incorporates an intersectional perspective by exploring intersecting subgroups and measuring social and structural processes that drive pain inequities. These findings underscore an important need for researchers to use an intersectional approach to cancer pain to help elucidate key populations at-risk for exacerbated cancer-related pain and identify ways to mitigate social and structural processes that drive these inequities.
Unraveling Racial Disparities in Supportive Care Medication Use among End-of-Life Pancreatic Cancer Patients: Focus on Pain Management and Psychiatric Therapies.

Abstract:AbstractBackground:Supportive care medication use differences may contribute to racial disparities observed in health-related quality of life in patients with pancreatic cancer.Methods:In this observation study using the Surveillance, Epidemiology, and End Results-Medicare linked database, we sought to examine supportive care medication use disparities in patients with pancreatic cancer from 2005 to 2017 by race and ethnicity.Results:Among 74,309 patients included in the final analysis, racial and ethnic disparities in the use of supportive care medications were identified. After adjustment for confounding factors and compared with non-Hispanic Whites, minorities had significantly less use of opioids [Black: adjusted OR (aOR), 0.84; 95% confidence interval (CI), 0.79–0.88; Asian: aOR, 0.84; 95% CI, 0.79–0.90), and skeletomuscular relaxants (Black: aOR, 0.90; 95% CI, 0.82–0.99; Hispanic: aOR, 0.82; 95% CI, 0.74–0.91; Asian: aOR, 0.59; 95% CI, 0.51–0.68), and increased use of non-opioid analgesics (Hispanic: aOR, 1.16; 95% CI, 1.01–1.14; Asian: aOR, 1.37; 95% CI, 1.26–1.49). Racial and ethnic minorities had less use of antidepressants (Black: aOR, 0.56; 95% CI, 0.53–0.59; Hispanic: aOR, 0.77; 95% CI, 0.73–0.82; Asian: aOR, 0.47; 95% CI, 0.44–0.51), anxiolytics (Black: aOR, 0.78; 95% CI, 0.74–0.82; Hispanic: aOR, 0.66; 95% CI, 0.62–0.71; Asian: aOR, 0.52; 95% CI, 0.48–0.57), and antipsychotics (Hispanic: aOR, 0.90; 95% CI, 0.82–0.99; Asian: aOR, 0.84; 95% CI, 0.74–0.95).Conclusions:Racial and ethnic disparities in the use of supportive care medications among patients with pancreatic cancer were observed, with the differences unexplained by sociodemographic factors.Impact:Future studies should identify strategies to promote equitable use of supportive care medications among racial minorities and explore factors that may influence their use in these populations.
Patient Preference and Risk Assessment in Opioid Prescribing Disparities: A Secondary Analysis of a Randomized Clinical Trial

Abstract:Importance: Although racial disparities in acute pain control are well established, the role of patient analgesic preference and the factors associated with these disparities remain unclear. Objective: To characterize racial disparities in opioid prescribing for acute pain after accounting for patient preference and to test the hypothesis that racial disparities may be mitigated by giving clinicians additional information about their patients' treatment preferences and risk of opioid misuse. Design, setting, and participants: This study is a secondary analysis of data collected from Life STORRIED (Life Stories for Opioid Risk Reduction in the ED), a multicenter randomized clinical trial conducted between June 2017 and August 2019 in the emergency departments (EDs) of 4 academic medical centers. Participants included 1302 patients aged 18 to 70 years who presented to the ED with ureter colic or musculoskeletal back and/or neck pain. Interventions: The treatment arm was randomized to receive a patient-facing intervention (not examined in this secondary analysis) and a clinician-facing intervention that consisted of a form containing information about each patient's analgesic treatment preference and risk of opioid misuse. Main outcomes and measures: Concordance between patient preference for opioid-containing treatment (assessed before ED discharge) and receipt of an opioid prescription at ED discharge. Results: Among 1302 participants in the Life STORRIED clinical trial, 1012 patients had complete demographic and treatment preference data available and were included in this secondary analysis. Of those, 563 patients (55.6%) self-identified as female, with a mean (SD) age of 40.8 (14.1) years. A total of 455 patients (45.0%) identified as White, 384 patients (37.9%) identified as Black, and 173 patients (17.1%) identified as other races. After controlling for demographic characteristics and clinical features, Black patients had lower odds than White patients of receiving a prescription for opioid medication at ED discharge (odds ratio [OR], 0.42; 95% CI, 0.27-0.65). When patients who did and did not prefer opioids were considered separately, Black patients continued to have lower odds of being discharged with a prescription for opioids compared with White patients (among those who preferred opioids: OR, 0.43 [95% CI, 0.24-0.77]; among those who did not prefer opioids: OR, 0.45 [95% CI, 0.23-0.89]). These disparities were not eliminated in the treatment arm, in which clinicians were given additional data about their patients' treatment preferences and risk of opioid misuse. Conclusions and relevance: In this secondary analysis of data from a randomized clinical trial, Black patients received different acute pain management than White patients after patient preference was accounted for. These disparities remained after clinicians were given additional patient-level data, suggesting that a lack of patient information may not be associated with opioid prescribing disparities. The Impact of Patient Race and Prior Misuse Behaviors on Opioid Tapering Decisions

Abstract:Given the recent opioid crisis, providers are being encouraged to taper prescription opioids for chronic pain. Racial disparities in pain treatment are well documented, but little is known about how patient race may influence provider opioid tapering decisions. Research and theory also suggest that patient race may interact with opioid use and adherence behaviors to impact provider tapering decisions. In this online study, 136 providers made opioid tapering decisions for 8 computer-simulated patients (4 Black, 4 White) with chronic pain. Text vignettes described all patients as taking prescription opioids, but half reported previous "yellow flag" misuse behaviors (e.g., early refill) and half reported medication adherence. Multilevel modeling indicated that patient history of opioid misuse increased providers' odds of changing the opioid prescription (OR=4.7, p=.03) and of endorsing the goal of discontinuing opioids (vs. maintaining at a lower dose; OR=11.0, p<.01). Patient history of opioid misuse also increased providers' odds of starting a long-acting opioid and decreasing the dose of the current opioid (vs. discontinuing opioids immediately; OR=4.2, p=.06). For providers who started a long-acting opioid, there was a significant race-by-misuse interaction on their target MED/day. For White patients, a history of opioid misuse increased providers' odds of prescribing a higher dose of the long-acting opioid (OR=3.4, p<.01), but no differences emerged for Black patients. Neither patient race nor opioid use history impacted the rate of tapering or the MED/day target for short-acting opioids. Our results indicate that the clinically-relevant information of patient opioid use history impacted provider opioid tapering decisions. Although patient race largely did not affect the tapering decisions assessed herein, prior work has found Black patients undergo more opioid-related monitoring than White patients. Future studies are needed to further understand the nuances of racial disparities in pain care during the opioid crisis. Abstract:Background: Opioid use disorder (OUD) is a rising public health concern in the United States. It poses substantial challenges in the management of head and neck cancers (HNC), given the pain associated with the disease. This study examines the sociodemographic differences in OUD among patients hospitalized for HNC in the United States as well as in-patient outcomes of HNC patients with OUD. Methods: We analyzed weighted retrospective data from the 2016 to 2020 National Inpatient Sample database. We included patients 18 years or older with principal discharge diagnoses of HNC with or without secondary diagnoses of OUD using specific ICD-10 codes. OUD was defined as opioid abuse, dependence, poisoning or adverse effects. Baseline sociodemographic and hospital characteristics of HNC patients with or without OUD were compared using χ2 for categorical variables and Student's t-test for continuous variables. We examined sociodemographic characteristics associated with OUD using multivariable logistic regression models. We assessed three in-patient outcomes including mortality, length of stay and total hospitalization charge (THC) adjusted for inflation. Further we included interaction terms to explore the combined effects of relevant sociodemographic predictors with depression. Results: There were 117,085 hospitalizations for HNC between 2016 and 2020. Among HNC hospitalizations, 3.2% were opioid users and 2.36% had a secondary diagnosis of OUD. The mean age of HNC pts was 63.3 years (SD 0.08). 78.3% were Non-Hispanic White, 11.8% Non-Hispanic Black, 6.7% Hispanic and 3.2% Asians/Pacific Islanders. HNC patients with OUD had higher odds of being female (aOR 1.32; 95% CI 1.09, 1.59) and having more than one comorbidity (aOR 1.088; 95% CI 1.047, 1.132). There were lower odds of OUD with older age >70 years (aOR: 0.25; 95% CI 0.10, 0.59), private insurance (aOR 0.66; 95% CI 0.50, 0.87), higher income quartiles (aOR 0.66; 95% CI 0.50, 0.87) and with hospital regions in the South compared to the North-East United States (aOR 0.57; 95% CI 0.43, 0.74). There was no statistically significant interaction between MDD and gender (aOR 1.27; 95% CI 0.80, 2.04) among patients with HNC and OUD. OUD was not significantly associated with increased mortality in patients hospitalized for HNC (aOR 1.39, 95% CI 0.83, 2.33), however OUD was associated with a greater LOS by 3.6 days (95% CI 2.52, 4.60) and an increased THC of $26,947 (95% CI 13232, 40662). Conclusion: Our study demonstrates significant sociodemographic disparities in the prevalence of OUD among head and neck cancer hospitalizations in the US. Female patients and those with multiple comorbidities had higher odds of OUD. Additionally, OUD was significantly associated with an increased LOS and THC. These findings highlight the need for targeted interventions that address the unique needs of vulnerable populations with OUD and HNC. Depression and Long-Term Prescription Opioid Use and Opioid Use Disorder: Implications for Pain Management in Cancer

Abstract:Preventing depression in cancer patients on long-term opioid therapy should begin with depression screening before opioid initiation and repeated screening during treatment. In weighing the high morbidity of depression and opioid use disorder in patients with chronic cancer pain against a dearth of evidence-based therapies studied in this population, patients and clinicians are left to choose among imperfect but necessary treatment options. When possible, we advise engaging psychiatric and pain/palliative specialists through collaborative care models and recommending mindfulness and psychotherapy to all patients with significant depression alongside cancer pain. Medications for depression should be reserved for moderate to severe symptoms. We recommend escitalopram/citalopram or sertraline among selective serotonin reuptake inhibitors (SSRIs), or the serotonin and norepinephrine reuptake inhibitors (SNRIs) duloxetine, venlafaxine, or desvenlafaxine if patients have a significant component of neuropathic pain or fibromyalgia. Tricyclic antidepressants (TCAs) (consider nortriptyline or desipramine, which have better anticholinergic profiles) should be considered for patients who do not respond to or tolerate SSRI/SNRIs. Existing evidence is inadequate to definitively recommend methylphenidate or novel agents, such as ketamine or psilocybin, as adjunctive treatments for cancer-related depression and pain. Physicians who treat patients with cancer pain should utilize universal precautions to limit the risk of non-medical opioid use (non-medical opioid use). Patients should be screened for non-medical opioid use behaviors at initial consultation and at regular intervals during treatment using a non-judgmental approach that reduces stigma. Co-management with an addiction specialist may be indicated for patients at high risk of non-medical opioid use and opioid use disorder. Buprenorphine and methadone are indicated for the treatment of opioid use disorder, and while they have not been systematically studied for treatment of opioid use disorder in patients with cancer pain, they do provide analgesia for cancer pain. While an interdisciplinary team approach to manage psychological stress may be beneficial, this may not be possible for patients treated outside of comprehensive cancer centers.
Racial Inequality in Receipt of Medications for Opioid Use Disorder

Abstract:Background: Since 2010, Black persons in the United States have had a greater increase in opioid overdose-related mortality than other groups, but national-level evidence characterizing racial and ethnic disparities in the use of medications for opioid use disorder (OUD) is limited. Methods: We used Medicare claims data from the 2016-2019 period for a random 40% sample of fee-for-service beneficiaries who were Black, Hispanic, or White; were eligible for Medicare owing to disability; and had an index event related to OUD (nonfatal overdose treated in an emergency department or inpatient setting, hospitalization with injection drug use-related infection, or inpatient or residential rehabilitation or detoxification care). We measured the receipt of medications to treat OUD (buprenorphine, naltrexone, and naloxone), the receipt of high-risk medications (opioid analgesics and benzodiazepines), and health care utilization, all in the 180 days after the index event. We estimated differences in outcomes according to race and ethnic group with adjustment for beneficiary age, sex, index event, count of chronic coexisting conditions, and state of residence. Results: We identified 25,904 OUD-related index events among 23,370 beneficiaries, with 3937 events (15.2%) occurring among Black patients, 2105 (8.1%) among Hispanic patients, and 19,862 (76.7%) among White patients. In the 180 days after the index event, patients received buprenorphine after 12.7% of events among Black patients, after 18.7% of those among Hispanic patients, and after 23.3% of those among White patients; patients received naloxone after 14.4%, 20.7%, and 22.9%, respectively; and patients received benzodiazepines after 23.4%, 29.6%, and 37.1%, respectively. Racial differences in the receipt of medications to treat OUD did not change appreciably from 2016 to 2019 (buprenorphine receipt: after 9.1% of index events among Black patients vs. 21.6% of those among White patients in 2016, and after 14.1% vs. 25.5% in 2019). In all study groups, patients had multiple ambulatory visits in the 180 days after the index event (mean number of visits, 6.6 after events among Black patients, 6.7 after events among Hispanic patients, and 7.6 after events among White patients). Conclusions: Racial and ethnic differences in the receipt of medications to treat OUD after an index event related to this disorder among patients with disability were substantial and did not change over time. The high incidence of ambulatory visits in all groups showed that disparities persisted despite frequent health care contact. Abstract:Background Opioid pain management in cancer survivorship is a complex and understudied topic. Methods The authors conducted in‐depth, qualitative interviews to understand clinician approaches to opioid pain management in chronic cancer pain and to generate ideas for improvement. They used a rigorous, inductive, qualitative, descriptive approach to examine clinician (n = 20) perspectives about opioid pain management in survivorship, including oncologists (n = 5), palliative care clinicians (n = 8), primary care clinicians (n = 5), and pain management specialists (n = 2). Results The findings indicated that no consistent medical home exists for chronic pain management in cancer survivors and that there are fundamental differences in how each subspecialty approaches chronic pain management in survivorship (e.g., "Do we think of this as noncancer pain or cancer pain?... This is in this limbo zone—this gray zone—because it's cancer‐related pain, right?"). Simultaneously, clinicians are influenced by their peers' perceptions of their opioid prescribing decisions, sparking intraprofessional tension when disagreement occurs. In these instances, clinicians described overthinking and doubting their clinical decision‐making as well as a sense of judgment, pressure, and/or shame. Finally, clinicians acknowledged a fear of consequences for opioid prescribing decisions. Specifically, participants cited conflict with patients, sometimes escalating to aggression and threats of violence, as well as potential disciplinary actions and/or legal consequences. Conclusions Participants suggested that opportunities to improve chronic cancer pain care include developing clear, systematic guidance for chronic cancer pain management, facilitating clinician communication and consultation, creating tailored survivorship care plans in partnership with patients, and developing accessible, evidence‐based, complementary pain treatments.

Abstract:Background: Beyond its well-known lethality, pancreatic cancer (PC) is associated with a wide variety of physical and psychological complications. These complications collectively lead to an increase in symptom burden experienced by patients with PC, as compared to other cancer types, leading to lower health-related quality of life (HRQoL) and worsened clinical outcomes. Reducing symptom burden is often achieved through the use of supportive care medications (SCM). Racial disparities have been described in the use of pain and psychiatric medications but limited research has explored potential disparities in access to and utilization of other SCM among diverse racial and ethnic groups. The goal of this study was to illustrate the presence of racial/ethnic disparities in the treatment patterns of supportive care medications in end-of-life care of PC patients Methods: We used data from the Surveillance, Epidemiology, and End Results (SEER)-Medicare-linked database from 2005 to 2017. Participants were identified using ICD9 and ICD10 diagnostic codes. Patients diagnosed with PC and have died in 2005-2007 with more than 1 year of Medicare enrollment before death were included. Logistic regression models to assess the association between use of supportive care medications and race, adjusting for covariates (sex, race, age, marital status, education, income, residence, CCI, year of diagnosis of PC, cancer stage). Results: After the application of the inclusion/exclusion criteria, 74,309 patients were included in the final analysis (White: 72.53%, Black 11.01%, Hispanic 9.60%, Asian/Pacific Islander 6.58%) Racial and ethnic disparities in the use of multiple SCM drug classes were identified. The results are summarized in Table 1. Conclusion: Disparities in the use of SCM were found in patients with PC. Abstract:Background: Research describes inequities in substance use treatment, but few studies focus specifically on racial and ethnic disparities in a range of aspects of substance use treatment among women with opioid use disorder (OUD). Objective: To examine whether substance use treatment (i.e. receipt, sources, barriers) differs by race and ethnicity among women with opioid use disorder (OUD) and to identify factors associated with treatment gap (i.e. needing treatment but not receiving it). Methods: We performed cross-sectional analyses using National Survey on Drug Use and Health 2015–2019 data, restricted to non-Hispanic Black, non-Hispanic White, and Hispanic women with past-year OUD (unweighted n = 1089). We estimated the prevalence of aspects of treatment among racial and ethnic groups, and used modified Poisson regression to estimate correlates of reported treatment gap. Results: Approximately 68% of White versus 87% of Black and 81% of Hispanic women with OUD had a treatment gap (p-value 0.0034). Commonly reported barriers to treatment included prioritization, affordability, and stigma. Older age was associated with lower prevalence of treatment gap among all women [prevalence ratio (PR) = 0.83, and 95% confidence interval (CI): 0.76, 0.92], while criminal legal involvement and healthcare coverage was associated with a lower prevalence of treatment gap among Hispanic and White women only (past year arrest: Hispanic women PR = 0.38, 95% CI: 0.17, 0.86; White women PR = 0.62, 95% CI: 0.47, 0.82). Conclusions: Receipt of treatment is low among women with OUD, especially Black and Hispanic women. Intersectional intervention approaches are needed to increase access and reduce inequities.
