Abstract:New data sources, and artificial intelligence (AI) methods to extract information from them are becoming plentiful, and relevant to decision making in many societal applications. An important example is street view imagery, available in over 100 countries, and considered for applications such as assessing built environment aspects in relation to community health outcomes. Relevant to such uses, important examples of bias in the use of AI are evident when decision-making based on data fails to account for the robustness of the data, or predictions are based on spurious correlations. To study this risk, we utilize 2.02 million GSV images along with health, demographic, and socioeconomic data from New York City. Initially, we demonstrate that built environment characteristics inferred from GSV labels at the intra-city level may exhibit inadequate alignment with the ground truth. We also find that the average individual-level behavior of physical inactivity significantly mediates the impact of built environment features by census tract, as measured through GSV. Finally, using a causal framework which accounts for these mediators of environmental impacts on health, we find that altering 10% of samples in the two lowest tertiles would result in a 4.17 (95% CI 3.84 to 4.55) or 17.2 (95% CI 14.4 to 21.3) times bigger decrease on the prevalence of obesity or diabetes, than the same proportional intervention on the number of crosswalks by census tract. This work illustrates important issues of robustness and model specification for informing effective allocation of interventions using new data sources.

Artificial intelligence–based assessment of built environment from Google Street View and coronary artery disease prevalence

Abstract:Background and Aims Built environment plays an important role in the development of cardiovascular disease. Tools to evaluate the built environment using machine vision and informatic approaches have been limited. This study aimed to investigate the association between machine vision–based built environment and prevalence of cardiometabolic disease in US cities. Methods This cross-sectional study used features extracted from Google Street View (GSV) images to measure the built environment and link them with prevalence of coronary heart disease (CHD). Convolutional neural networks, linear mixed-effects models, and activation maps were utilized to predict health outcomes and identify feature associations with CHD at the census tract level. The study obtained 0.53 million GSV images covering 789 census tracts in seven US cities (Cleveland, OH; Fremont, CA; Kansas City, MO; Detroit, MI; Bellevue, WA; Brownsville, TX; and Denver, CO). Results Built environment features extracted from GSV using deep learning predicted 63% of the census tract variation in CHD prevalence. The addition of GSV features improved a model that only included census tract-level age, sex, race, income, and education or composite indices of social determinant of health. Activation maps from the features revealed a set of neighbourhood features represented by buildings and roads associated with CHD prevalence. Conclusions In this cross-sectional study, the prevalence of CHD was associated with built environment factors derived from GSV through deep learning analysis, independent of census tract demographics. Machine vision–enabled assessment of the built environment could potentially offer a more precise approach to identify at-risk neighbourhoods, thereby providing an efficient avenue to address and reduce cardiovascular health disparities in urban environments.

Google Street View Images as Predictors of Patient Health Outcomes, 2017-2019

Abstract:Collecting neighborhood data can both be time- and resource-intensive, especially across broad geographies. In this study, we leveraged 1.4 million publicly available Google Street View (GSV) images from Utah to construct indicators of the neighborhood built environment and evaluate their associations with 2017-2019 health outcomes of approximately one-third of the population living in Utah. The use of electronic medical records allows for the assessment of associations between neighborhood characteristics and individual-level health outcomes while controlling for predisposing factors, which distinguishes this study from previous GSV studies that were ecological in nature. Among 938,085 adult patients, we found that individuals living in communities in the highest tertiles of green streets and non-single-family homes have 10-27% lower diabetes, uncontrolled diabetes, hypertension, and obesity, but higher substance use disorders-controlling for age, White race, Hispanic ethnicity, religion, marital status, health insurance, and area deprivation index. Conversely, the presence of visible utility wires overhead was associated with 5-10% more diabetes, uncontrolled diabetes, hypertension, obesity, and substance use disorders. Our study found that non-single-family and green streets were related to a lower prevalence of chronic conditions, while visible utility wires and single-lane roads were connected with a higher burden of chronic conditions. These contextual characteristics can better help healthcare organizations understand the drivers of their patients' health by further considering patients' residential environments, which present both risks and resources.
Chronic disease and where you live: Built and natural environment relationships with physical activity, obesity, and diabetes

Abstract:Background: Diabetes is among the most prevalent non-communicable diseases causing significant morbidity and mortality globally. The aetiology and disease development of diabetes are influenced by genetic, lifestyle, and environmental factors. Due to an increasing number of cases each year, it is imperative to improve the understanding of modifiable environmental risk and protective factors. In this study we aimed to analyse associations between built and natural environment features and diabetes prevalence; and two major risk factors: physical activity and obesity and their mediation effects. Methods: We analysed relationships between walkability and park availability with physical activity, obesity, and diabetes, using self-reported data from a large cross-sectional survey in British Columbia, Canada (n = 22,418). We validated results with an independent cohort (n = 11,972) in a subset of the analyses. The outcome measures included walking, moderate to vigorous physical activity (MVPA), body mass index (BMI), and diabetes. Built and natural environment features within a 1 km road network buffer around residential postal code centroids were assessed using validated indicators of walkability and park availability. We used general linear multivariable models (GLM) to examine the direct relationship between environmental features, physical activity, obesity, and diabetes respectively. Path models were developed to analyse mediation effects of physical activity and obesity on the association between environmental indicators and diabetes. The relative contribution of direct versus indirect effects was assessed. All models were adjusted for age, gender, income. ethnicity, years lived in neighbourhood and regional accessibility. Results: Walkable neighbourhoods and areas with greater park availability were associated with lower rates of diabetes. There was a direct association of walkability and park availability on physical activity (highest vs. lowest quintile OR = 1.15; 95% CI: 1.00, 1.33 and OR = 1.28, 95% CI: 1.13, 1.45 respectively), obesity (highest vs. lowest quintile OR = 0.58, 95% CI: 0.49, 0.70 and OR = 0.57, 95% CI: 0.48, 0.68 respectively) and diabetes (highest vs. lowest quintile OR = 0.62, 95% CI: 0.45, 0.85, and OR = 0.63, 95% CI: 0.47, 0.84 respectively). Results were similar in the independent cohort. The associations between diabetes and walkability and park availability respectively were partly mediated by obesity (41% of total association for walkability and 53% of total association for park availability). The mediating effect of physical activity was negligible. Conclusion: Results support investments in walkability through active transportation and transit infrastructure. Changes in zoning and subdivision regulations governing land use actions are required to enable compact mixed-use environments with access to parks and high quality transit service. Future studies including cost-benefit analyses of health-related economic impacts of such investments can contribute to evidence-based decisions for healthier cities.

Abstract:The built environment characteristics associated with walkability range from neighborhood-level urban form factors to street-level urban design factors. However, many existing walkability indices are based on neighborhood-level factors and lack consideration for street-level factors. Arguably, this omission is due to the lack of a scalable way to measure them. This paper uses computer vision to quantify street-level factors from street view images in Atlanta, Georgia, USA. Correlation analysis shows that some streetscape factors are highly correlated with neighborhood-level factors. Binary logistic regressions indicate that the streetscape factors can significantly contribute to explaining walking mode choice and that streetscape factors can have a greater association with walking mode choice than neighborhood-level factors. A potential explanation for the result is that the image-based streetscape factors may perform as proxies for some macroscale factors while representing the pedestrian experience as seen from eye-level.

Using Deep Learning to Examine the Association between the Built Environment and Neighborhood Adult Obesity Prevalence

Abstract:More than one-third of the adult population in the United States is obese. Obesity has been linked to factors such as, genetics, diet, physical activity and the environment. However, evidence indicating associations between the built environment and obesity has varied across studies and geographical contexts. Here, we used deep learning and approximately 150,000 high resolution satellite images to extract features of the built environment. We then developed linear regression models to consistently quantify the association between the extracted features and obesity prevalence at the census tract level for six cities in the United States. The extracted features of the built environment explained 72% to 90% of the variation in obesity prevalence across cities. Outof-sample predictions were considerably high with correlations greater than 80% between predicted and true obesity prevalence across all census tracts. This study supports a strong association between the built environment and obesity prevalence. Additionally, it also illustrates that features of the built environment extracted from satellite images can be useful for studying health indicators, such as obesity. Understanding the association between specific features of the built environment and obesity prevalence can lead to structural changes that could encourage physical activity and decreases in obesity prevalence.

Abstract:Urban physical environments are the physical settings and built environments in neighbourhoods and cities which provide places for human activities. Evidence suggests that there are substantial associations between urban physical environments and various health outcomes, e.g. people's physical activities might be influenced by surrounding physical environments, thereby affecting their health behaviours; more exposure to urban physical environments may benefit human mental health. Street view imagery enables us to capture the landscape at eye-level, making it a promising data source for observing and analysing the realistic dynamics of urban physical environments. Compared with traditional in-person assessments and field observations, street view imagery-based data collection is relatively time-effective and cost-effective. Researchers from epidemiology, psychology, and geography have used street view imagery to quantify the built environment and understand its impacts on public health. To summarize current research trends, this paper systematically reviews the use of street view images for sensing urban environments in public health studies. Specifically, we describe the characteristics of street view imagery and review the methodology for image processing and semantic understanding. We then summarize the challenges that remain for quantifying urban environments in terms of data and methodology. Several future research directions that would benefit public health research and practices are recommended in urban environment research.

Abstract:Background: Social and Environmental Determinants of Health (SEDH) provide us with a conceptual framework to gain insights into possible associations among different human behaviors and the corresponding health outcomes that take place often in and around complex built environments. Developing better built environments requires an understanding of those aspects of a community that are most likely to have a measurable impact on the target SEDH. Yet data on local characteristics at suitable spatial scales are often unavailable. We aim to address this issue by application of different data disaggregation methods. Methods: We applied different approaches to data disaggregation to obtain small area estimates of key behavioral risk factors, as well as geospatial measures of green space access and walkability for each zip code of Allegheny County in southwestern Pennsylvania. Results: Tables and maps of local characteristics revealed their overall spatial distribution along with disparities therein across the county. While the top ranked zip codes by behavioral estimates generally have higher than the county's median individual income, this does not lead them to have higher than its median green space access or walkability. Conclusion: We demonstrated the utility of data disaggregation for addressing complex questions involving community-specific behavioral attributes and built environments with precision and rigor, which is especially useful for a diverse population. Thus, different types of data, when comparable at a common local scale, can provide key integrative insights for researchers and policymakers.

Abstract:Stimulating more citizens to walk plays an essential role in building a healthy city. This paper explores the mismatch between walkability and walking behavior, using mobile phone data, street view images, and various sources of open data. Using Shenzhen as our case study, we identified walking trips of 6 months in 2021 from cellular mobile data, taking the rule-based heuristics approach. We collected ground truth GPS data to validate the walking trip extraction method. Open data and deep learning enabled quantifying walkability from the perspective of four pedestrian needs: safety, convenience, continuity, and attractiveness. We employed geospatial techniques to identify the mismatch areas between walkability and walking behavior in the city. We also explored the spatially varying effects of walkability on walking behavior. Our results showed that the mismatch areas with high-level walking trips but low-level walkability mainly occurred in the fringe areas of the central business district (CBD) and subcenters that require prioritizing more interventions. Moreover, walkability showed strong effects on walking trips in the inner suburbs. For the four aspects of our walkability framework, safety and convenience had greater positive effects on walking trips in suburbs than in urban areas. Continuity promotes walking trips mainly in the city's western sector. The positive effect of attractiveness on walking trips clustered in the central and western parts of the city. Based on the findings, we provide prioritized and contextualized built-environment intervention strategies and policy recommendations for urban designers and transportation planners.

Abstract:Introduction The exposome consists of factors an individual is exposed to across the life course. The exposome is dynamic, meaning the factors are constantly changing, affecting each other and individuals in different ways. Our exposome dataset includes social determinants of health as well as policy, climate, environment, and economic factors that could impact obesity development. The objective was to translate spatial exposure to these factors with the presence of obesity into actionable population-based constructs that could be further explored. Methods Our dataset was constructed from a combination of public-use datasets and the Center of Disease Control's Compressed Mortality File. Spatial Statistics using Queens First Order Analysis was performed to identify hot- and cold-spots of obesity prevalence; followed by Graph Analysis, Relational Analysis, and Exploratory Factor Analysis to model the multifactorial spatial connections. Results Areas of high and low presence of obesity had different factors associated with obesity. Factors associated with obesity in areas of high obesity propensity were: poverty / unemployment; workload, comorbid conditions (diabetes, CVD) and physical activity. Conversely, factors associated in areas where obesity was rare were: smoking, lower education, poorer mental health, lower elevations, and heat. Discussion The spatial methods described within the paper are scalable to large numbers of variables without issues of multiple comparisons lowering resolution. These types of spatial structural methods provide insights into novel variable associations or factor interactions that can then be studied further at the population or policy levels.

Abstract:As obesity is emerging as a serious social issue, urban planners and public health practitioners are making efforts to establish urban environments that can help to reduce obesity. Despite many studies on the correlation between built environments and obesity, little attention has been devoted to micro-scale street environments related to obesity. This study fills this gap by empirically analyzing the effects of both micro- and macro-scale walkable environments on obesity through the mediating factor of walking behaviors. In particular, we employed the deep learning technique of semantic segmentation based on Google Street View panorama images, to estimate visual street environments at the pedestrian eye level. Based on an online survey collected from 1000 respondents in Seoul, Korea, this study used the Structural Equation Model to examine the systematic relationships between walkable environments, walking behaviors, and obesity. Our findings revealed that walkable environments had an indirect effect on obesity through the mediating factor of walking behaviors. Specifically, utilitarian walking behavior had no significant effect on obesity while leisure walking behavior had an effect on reducing obesity. Moreover, visual street environments (i.e., street greenery and sidewalk pavement) and macro-scale built environments (i.e., land-use mix and intersection density) had significant indirect effects on obesity. The results of this study provide the evidence for policymakers, urban planners, and public health practitioners to effectively retrofit built environments to be more physically active-friendly, so as to reduce obesity.

Abstract:Many current studies have suggested that herpes simplex virus is a probable cause of Bell's palsy, and that treatment with antiviral agents such as acyclovir might benefit the patients. In the present study, 69 patients with Bell's palsy were treated with oral administration of acyclovir (2000 mg/day) and prednisolone (60-40 mg/day) at Ehime University Hospital between Oct. 1995 and Dec. 1998. Patients enrolled in this study met the following criteria: 1) severe or complete paralysis with a score lower than 20 by the 40-point Japanese grading system, and 2) treatment started within 7 days of onset. The overall recovery rate was 95.7% (66/69). The rate in patients who started this treatment within 3 days after disease onset was 100%, and this early treatment was highly efficacious in the prevention of nerve degeneration and resulted in a significantly better recovery. By comparison, the recovery rate in patients whose treatment was started 4 days or more after onset was only 84.2%. All patients who were given a diagnosis of zoster sine herpete and treated with acyclovir-prednisolone had a good outcome. These results suggest that early treatment, within 3 days after palsy onset, is necessary for effective acyclovir-prednisolone therapy of Bell's palsy.

Abstract:Outdoor jogging, as a physical exercise beneficial for health, has proliferated worldwide. However, understanding the nonlinear and heterogeneous associations between environmental factors and jogging behavior remains challenging. This study established an explainable spatial machine learning framework combining Geographically Weighted-Random Forest (GW-RF) and SHapley Additive exPlanation (SHAP) model to address the nonlinearity, spatial heterogeneity, and interpretability. Using large-scale GPS trajectories and multi-source big data, this study provides the global and local explanations of nonlinear associations in Beijing, China. Our findings highlight that (1) Built Environment (BE) factors play a more important role than visual landscape factors in determining jogging behavior, with sports facilities such as tracks and parks having higher contributions. (2) All environmental variables, including the BE, visual landscape, and social economics, exhibit nonlinear and threshold effects on jogging behavior. (3) Certain factors such as population density, number of parks, Greening View Index, and Sky View Index exhibit differential effects across different periods, regions, and mobility patterns of jogging. (4) Compared with the Ordinary Least Squares model, Geographically Weighted Regression model, and Random Forest model, GW-RF model demonstrates improved performance in modeling and predicting the jogging flow. The findings have important implications for urban planners seeking to create a supportive environment that promotes outdoor jogging.
Predicting Perceptions of the Built Environment using GIS, Satellite and Street View Image Approaches

Abstract:Background: High quality built environments are important for human health and wellbeing. Numerous studies have characterized built environment physical features and environmental exposures, but few have examined urban perceptions at geographic scales needed for population-based research. The degree to which urban perceptions are associated with different environmental features, and traditional environmental exposures such as air pollution or urban green space, is largely unknown. Objective: To determine built environment factors associated with safety, lively and beauty perceptions across 56 cities. Methods: We examined perceptions collected in the open source Place Pulse 2.0 dataset, which assigned safety, lively and beauty scores to street view images based on crowd-sourced labelling. We derived built environment measures for the locations of these images (110,000 locations across 56 global cities) using GIS and remote sensing datasets as well as street view imagery features (e.g. trees, cars) using deep learning image segmentation. Linear regression models were developed using Lasso penalized variable selection to predict perceptions based on visible (street level images) and GIS/remote sensing built environment variables. Results: Population density, impervious surface area, major roads, traffic air pollution, tree cover and Normalized Difference Vegetation Index (NDVI) showed statistically significant differences between high and low safety, lively, and beauty perception locations. Visible street level features explained approximately 18% of the variation in safety, lively, and beauty perceptions, compared to 3-10% explained by GIS/remote sensing. Large differences in prediction were seen when modelling between city (R2 67-81%) versus within city (R2 11-13%) perceptions. Important predictor variables included visible accessibility features (e.g. streetlights, benches) and roads for safety, visible plants and buildings for lively, and visible green space and NDVI for beauty. Conclusion: Substantial within and between city differences in built environment perceptions exist, which visible street level features and GIS/remote sensing variables only partly explain. This offers a new research avenue to expand built environment measurement methods to include perceptions in addition to physical features.

Abstract:Abstract The public health burden caused by overweight, obesity (OO) and type-2 diabetes (T2D) is very significant and continues to rise worldwide. The causation of OO and T2D is complex and highly multifactorial rather than a mere energy intake (food) and expenditure (exercise) imbalance. But previous research into food and physical activity (PA) neighbourhood environments has mainly focused on associating body mass index (BMI) with proximity to stores selling fresh fruits and vegetables or fast food restaurants and takeaways, or with neighbourhood walkability factors and access to green spaces or public gym facilities, making largely naive, crude and inconsistent assumptions and conclusions that are far from the spirit of 'precision and accuracy public health'. Different people and population groups respond differently to the same food and PA environments, due to a myriad of unique individual and population group factors (genetic/epigenetic, metabolic, dietary and lifestyle habits, health literacy profiles, screen viewing times, stress levels, sleep patterns, environmental air and noise pollution levels, etc.) and their complex interplays with each other and with local food and PA settings. Furthermore, the same food store or fast food outlet can often sell or serve both healthy and non-healthy options/portions, so a simple binary classification into 'good' or 'bad' store/outlet should be avoided. Moreover, appropriate physical exercise, whilst essential for good health and disease prevention, is not very effective for weight maintenance or loss (especially when solely relied upon), and cannot offset the effects of a bad diet. The research we should be doing in the third decade of the twenty-first century should use a systems thinking approach, helped by recent advances in sensors, big data and related technologies, to investigate and consider all these factors in our quest to design better targeted and more effective public health interventions for OO and T2D control and prevention.

Abstract:BACKGROUND: Street imagery is a promising and growing big data source providing current and historical images in more than 100 countries. Studies have reported using this data to audit road infrastructure and other built environment features. Here we explore a novel application, using Google Street View (GSV) to predict travel patterns at the city level.METHODS: We sampled 34 cities in Great Britain. In each city, we accessed 2000 GSV images from 1000 random locations. We selected archived images from time periods overlapping with the 2011 Census and the 2011-2013 Active People Survey (APS). We manually annotated the images into seven categories of road users. We developed regression models with the counts of images of road users as predictors. The outcomes included Census-reported commute shares of four modes (combined walking plus public transport, cycling, motorcycle, and car), as well as APS-reported past-month participation in walking and cycling.RESULTS: We found high correlations between GSV counts of cyclists ('GSV-cyclists') and cycle commute mode share (r = 0.92)/past-month cycling (r = 0.90). Likewise, GSV-pedestrians was moderately correlated with past-month walking for transport (r = 0.46), GSV-motorcycles was moderately correlated with commute share of motorcycles (r = 0.44), and GSV-buses was highly correlated with commute share of walking plus public transport (r = 0.81). GSV-car was not correlated with car commute mode share (r = -0.12). However, in multivariable regression models, all outcomes were predicted well, except past-month walking. The prediction performance was measured using cross-validation analyses. GSV-buses and GSV-cyclists are the strongest predictors for most outcomes.CONCLUSIONS: GSV images are a promising new big data source to predict urban mobility patterns. Predictive power was the greatest for those modes that varied the most (cycle and bus). With its ability to identify mode of travel and capture street activity often excluded in routinely carried out surveys, GSV has the potential to be complementary to new and traditional data. With half the world's population covered by street imagery, and with up to 10 years historical data available in GSV, further testing across multiple settings is warranted both for cross-sectional and longitudinal assessments.

The Association of Urban Greenness and Walking Behavior: Using Google Street View and Deep Learning Techniques to Estimate Residents’ Exposure to Urban Greenness

Abstract:Many studies have established that urban greenness is associated with better health outcomes. Yet most studies assess urban greenness with overhead-view measures, such as park area or tree count, which often differs from the amount of greenness perceived by a person at eye-level on the ground. Furthermore, those studies are often criticized for the limitation of residential self-selection bias. In this study, urban greenness was extracted and assessed from profile view of streetscape images by Google Street View (GSV), in conjunction with deep learning techniques. We also explored a unique research opportunity arising in a citywide residential reallocation scheme of Hong Kong to reduce residential self-selection bias. Two multilevel regression analyses were conducted to examine the relationships between urban greenness and (1) the odds of walking for 24,773 public housing residents in Hong Kong, (2) total walking time of 1994 residents, while controlling for potential confounders. The results suggested that eye-level greenness was significantly related to higher odds of walking and longer walking time in both 400 m and 800 m buffers. Distance to the closest Mass Transit Rail (MTR) station was also associated with higher odds of walking. Number of shops was related to higher odds of walking in the 800 m buffer, but not in 400 m. Eye-level greenness, assessed by GSV images and deep learning techniques, can effectively estimate residents’ daily exposure to urban greenness, which is in turn associated with their walking behavior. Our findings apply to the entire public housing residents in Hong Kong, because of the large sample size.

Abstract:Introduction: The rising prevalence of obesity has become a public health concern, requiring efficient and comprehensive prevention strategies. Methods: This study innovatively investigated the combined influence of individual and social/environmental factors on obesity within the urban landscape of Seoul, by employing advanced machine learning approaches. We collected 'Community Health Surveys' and credit card usage data to represent individual factors. In parallel, we utilized 'Seoul Open Data' to encapsulate social/environmental factors contributing to obesity. A Random Forest model was used to predict obesity based on individual factors. The model was further subjected to Shapley Additive Explanations (SHAP) algorithms to determine each factor's relative importance in obesity prediction. For social/environmental factors, we used the Geographically Weighted Least Absolute Shrinkage and Selection Operator (GWLASSO) to calculate the regression coefficients. Results: The Random Forest model predicted obesity with an accuracy of >90%. The SHAP revealed diverse influential individual obesity-related factors in each Gu district, although 'self-awareness of obesity', 'weight control experience', and 'high blood pressure experience' were among the top five influential factors across all Gu districts. The GWLASSO indicated variations in regression coefficients between social/environmental factors across different districts. Conclusion: Our findings provide valuable insights for designing targeted obesity prevention programs that integrate different individual and social/environmental factors within the context of urban design, even within the same city. This study enhances the efficient development and application of explainable machine learning in devising urban health strategies. We recommend that each autonomous district consider these differential influential factors in designing their budget plans to tackle obesity effectively.

Countrywide natural experiment reveals impact of built environment on physical activity

Abstract:While physical activity is critical to human health, most people do not meet recommended guidelines. More walkable built environments have the potential to increase activity across the population. However, previous studies on the built environment and physical activity have led to mixed findings, possibly due to methodological limitations such as small cohorts, few or single locations, over-reliance on self-reported measures, and cross-sectional designs. Here, we address these limitations by leveraging a large U.S. cohort of smartphone users (N=2,112,288) to evaluate within-person longitudinal behavior changes that occurred over 248,266 days of objectively-measured physical activity across 7,447 relocations among 1,609 U.S. cities. By analyzing the results of this natural experiment, which exposed individuals to differing built environments, we find that increases in walkability are associated with significant increases in physical activity after relocation (and vice versa). These changes hold across subpopulations of different genders, age, and body-mass index (BMI), and are sustained over three months after moving.The added activity observed after moving to a more walkable location is predominantly composed of moderate-to-vigorous physical activity (MVPA), which is linked to an array of associated health benefits across the life course. A simulation experiment demonstrates that substantial walkability improvements (i.e., bringing all US locations to the walkability level of Chicago or Philadelphia) may lead to 10.3% or 33 million more Americans meeting aerobic physical activity guidelines. Evidence against residential self-selection confounding is reported. Our findings provide robust evidence supporting the importance of the built environment in directly improving health-enhancing physical activity, in addition to offering potential guidance for public policy activities in this area.
