Abstract:Background: Diabetes mellitus is the most challenging and fastest-growing global public health concern. Approximately 10.5% of the global adult population is affected by diabetes, and almost half of them are undiagnosed. The growing at-risk population exacerbates the shortage of health resources, with an estimated 10.6% and 6.2% of adults worldwide having impaired glucose tolerance and impaired fasting glycemia, respectively. All current diabetes screening methods are invasive and opportunistic and must be conducted in a hospital or laboratory by trained professionals. At-risk participants might remain undetected for years and miss the precious time window for early intervention to prevent or delay the onset of diabetes and its complications. Objective: We aimed to develop an artificial intelligence solution to recognize elevated blood glucose levels (≥7.8 mmol/L) noninvasively and evaluate diabetic risk based on repeated measurements. Methods: This study was conducted at KK Women's and Children's Hospital in Singapore, and 500 participants were recruited (mean age 38.73, SD 10.61 years; mean BMI 24.4, SD 5.1 kg/m2). The blood glucose levels for most participants were measured before and after consuming 75 g of sugary drinks using both a conventional glucometer (Accu-Chek Performa) and a wrist-worn wearable. The results obtained from the glucometer were used as ground-truth measurements. We performed extensive feature engineering on photoplethysmography (PPG) sensor data and identified features that were sensitive to glucose changes. These selected features were further analyzed using an explainable artificial intelligence approach to understand their contribution to our predictions. Results: Multiple machine learning models were trained and assessed with 10-fold cross-validation, using participant demographic data and critical features extracted from PPG measurements as predictors. A support vector machine with a radial basis function kernel had the best detection performance, with an average accuracy of 84.7%, a sensitivity of 81.05%, a specificity of 88.3%, a precision of 87.51%, a geometric mean of 84.54%, and F score of 84.03%. Conclusions: Our findings suggest that PPG measurements can be used to identify participants with elevated blood glucose measurements and assist in the screening of participants for diabetes risk.

Abstract:Noninvasive glucose detection is highly required for more convenient and less pain glycaemic monitoring. Most of currently used methods are invasive. In this paper, a near-infrared reflectance spectroscopy (NIRS) is proposed to detect blood glucose to protect patient absent of pain. NIRS is a safe, simple and efficient technology applied in many fields. Experiments, based on Oral Glucose Tolerance Test (OGTT), were conducted to collect data modeling with partial least squares (PLS) regression. 42 samples of fingertip blood and palm were measured by commercially available blood glucose meter and NIRS separately at the same time. The glucose concentration range is between 5 and 12 mmol·L-1. With leave-one-out cross-validation, we obtained a result of root mean square error of cross-validation (RMSECV) of 1.16 mmol·L-1 for all the data. With the pre-processing methods of normalization and un-informative variables elimination reducing noise and eliminating some additional effects, we get a better result of 0.79 mmol·L-1. A RMSECV of 0.41 mmol·L-1 for individual modeling is much less than the total modeling. It has a broad application prospect in individual customization.

Abstract:Biomedical Engineering: Applications, Basis and Communications, Ahead of Print. The increase in diabetic class population is witnessed every year and hence frequent need of checking blood glucose levels is highly required in both diabetic and healthy subjects. This requirement has led many researchers to develop efficient, accurate and easy estimation of blood glucose level without painful blood pricking or extraction. In line with this objective, this study is an effort for waveform analysis of autonomic response of photo pulse plethysmogram (PPG) and to evaluate PPG features that may provide better non-invasive detection of the disease. A total of 300 (150 diabetic and 150 control) subjects aged more than 18 years are considered for this study. Relevant patient details were obtained, and finger PPG recording in digital form was performed for 5 min with the digitization sampling rate of 200 sample points per second. After signal baseline correction, features are calculated from averaged normalized PPG and its first and second derivatives. A total of 41 features were derived from normalized PPG pulse and its derivatives for the assessment of hyperglycemia. But in practice, computation of many features is time-consuming and truly a tedious job for any model design. Further, model performance and accuracy are affected significantly. Applying six different statistical and machine learning methods for the identification of important features, 20 features were identified out of 41 features whose values are highly significantly different for control and study (diabetic) groups. This work was focused on feature extraction stage for a PPG-based diagnostic system. Though our study is aimed to identify significant features that can discriminate between diabetic and control groups non-invasively, these features have the potential for much clinical assessment and other pathological disorders without any doubt.

Abstract:Objective. We present the design and validation of a non-invasive smart-phone based screening tool for atherosclerosis and coronary arterial disease (CAD), which is the leading cause of mortality worldwide. Approach. We designed a three-channel photoplethysmography (PPG) device that connects to a smart phone application for measuring pulse transit time (PTT) and pulse wave velocity (PWV) using PPG probes that are simultaneously clipped onto to the ear, index finger, and big toe, respectively. Validation was performed through a clinical study with 100 participants (age 20 to 77) at a research hospital in Nagpur, India. Study subjects were stratified by age and divided into three groups corresponding to the disease severity: CAD, hypertensive ('Pre-CAD'), and Healthy. Main results. PWV measurements derived from the Ear-Toe probe measurements yielded the best performance, with median PWV values increasing monotonically as a function of disease severity and age, as follows: 14.2 m s−1 for the older-patient CAD group, 12.2 m s−1 for the younger-patient CAD group, 11.6 m s−1 for the older-patient Pre-CAD group, 10.2 m s−1 for the younger-patient Pre-CAD group, 9.7 m s−1 for the older healthy controls, and 8.4 m s−1 for the younger healthy controls. Using just two simple features, the PTT and patient height, we demonstrate a machine learning prediction model for CAD with a median accuracy of 0.83 (AUC). Significance. This work demonstrates the ability to predict atherosclerosis and CAD using a single simple physiological measurement with a multi-site PPG tool that is electrically powered by a mobile phone and does not require any electrocardiogram reference. Furthermore, this method only requires a single anthropometric measurement, which is the patient's height.

Abstract:Diabetes mellitus has become very important issue over the last decade. It affects several complications such as kidney failure, heart failure, stroke and blindness. The risk of developing diabetic complications is related to hyperglycemia and duration of having diabetes. Therefore, early detection can improve the chances of therapeutic interventions that may alleviate its effects. This research proposed second derivative of Photoplethysmography waveform (SDPPG) technique for monitoring the arterial condition in estimated the states of diabetic (HbA1c level greater than 10 percent). The proposed technique is simple and low cost optical techniques for detecting and tracking blood volume change. Results showed that the risk of having HbA1c level greater than 10 percent can be estimated. We conclude that the proposed technique on extracting the information from PPG signal is useful for estimating HbA1c level greater than 10 percent among diabetic patients.

Abstract:Blood glucose level needs to be monitored regularly to manage the health condition of hyperglycemic patients. The current glucose measurement approaches still rely on invasive techniques which are uncomfortable and raise the risk of infection. To facilitate daily care at home, in this article, we propose an intelligent, noninvasive blood glucose monitoring system which can differentiate a user's blood glucose level into normal, borderline, and warning based on smartphone photoplethysmography (PPG) signals. The main implementation processes of the proposed system include 1) a novel algorithm for acquiring PPG signals using only smartphone camera videos; 2) a fitting-based sliding window algorithm to remove varying degrees of baseline drifts and segment the signal into single periods; 3) extracting characteristic features from the Gaussian functions by comparing PPG signals at different blood glucose levels; 4) categorizing the valid samples into three glucose levels by applying machine learning algorithms. Our proposed system was evaluated on a data set of 80 subjects. Experimental results demonstrate that the system can separate valid signals from invalid ones at an accuracy of 97.54$%$ and the overall accuracy of estimating the blood glucose levels reaches 81.49$%$. The proposed system provides a reference for the introduction of noninvasive blood glucose technology into daily or clinical applications. This article also indicates that smartphone-based PPG signals have great potential to assess an individual's blood glucose level.

Abstract:Introduction Type 2 diabetes is preventable in subjects with impaired glucose tolerance based on 2-hour plasma glucose (2hPG) during 75 g oral glucose tolerance test (OGTT). We incorporated routine biochemistry to improve the performance of a non-invasive diabetes risk score to identify individuals with abnormal glucose tolerance (AGT) defined by 2hPG≥7.8 mmol/L during OGTT. Research design and methods We used baseline data of 1938 individuals from the community-based "Better Health for Better Hong Kong - Hong Kong Family Diabetes Study (BHBHK-HKFDS) Cohort" recruited in 1998–2003. We incorporated routine biochemistry in a validated non-invasive diabetes risk score, and evaluated its performance using area under receiver operating characteristics (AUROC) with internal and external validation. Results The AUROC of the original non-invasive risk score to predict AGT was 0.698 (95% CI, 0.662 to 0.733). Following additional inclusion of fasting plasma glucose, serum potassium, creatinine, and urea, the AUROC increased to 0.778 (95% CI, 0.744 to 0.809, p<0.001). Net reclassification improved by 31.9% (p<0.001) overall, by 30.8% among people with AGT and 1.1% among people without AGT. The extended model showed good calibration ( 2 =11.315, p=0.1845) and performance on external validation using an independent data set (AUROC=0.722, 95% CI, 0.680 to 0.764). Conclusions The extended risk score incorporating clinical and routine biochemistry can be integrated into an electronic health records system to select high-risk subjects for evaluation of AGT using OGTT for prevention of diabetes.

Abstract:Objective To assess the clinical utility of point‐of‐care (POC) capillary blood glucose (CBG) testing in the assessment of gestational diabetes mellitus (GDM) during oral glucose tolerance test (OGTT). Design Prospective cohort study. Setting Antenatal clinics at King's College Hospital. Population Women screened for GDM between March and June 2020. Methods The CBG was measured using the POC StatStrip® test and the venous plasma glucose (VPG) was measured by Roche analyser (Cobas 8000 c702). GDM was diagnosed based on the 2015 National Institute for Health and Clinical Excellence (NICE) Clinical Guideline criteria. The two methods were compared statistically using Analyse‐It 5.40.2. Main outcome measures Diagnostic sensitivity, specificity, positive and negative predictive values (PPV and NPV) for the POC StatStrip® test, compared with VPG measured by reference laboratory method. Results A total of 230 women were included. The number and percentage of women with glucose concentrations above the GDM threshold using the POC StatStrip® test versus laboratory VPG measurement was 15 (6.5%) versus eight (3.4%) at fasting and 105 (45.6%) versus 72 (31.1%) at 2 h, respectively. The sensitivity and specificity values (and 95% CIs) for the POC StatStrip® test were 88% (52%–99%) and 97% (93%–98%) at fasting and 97% (91%–99%) and 79% (71%–84%) at 2 h, respectively. However, the specificity and the NPV for the POC StatStrip® test for concentrations of ≤5.0 mmol/L at fasting or 9.5 mmol/L at 2 h were 100%. Conclusions In our cohort the POC measurement of CBG cannot entirely replace the laboratory method for the OGTT; however, it can be used to rule out/rule in GDM for glucose concentrations of ≤5.0 mmol/L at fasting or 9.5 mmol/L at 2 h.

Abstract:Arterial Blood Pressure (ABP) and photoplethysmography (PPG) are both useful techniques to monitor cardiovascular status. Though ABP monitoring is more widely employed, this procedure of signal acquisition whether done invasively or non-invasively may cause inconvenience and discomfort to the patients. PPG, however, is simple, noninvasive, and can be used for continuous measurement. This paper focuses on analyzing the similarities in time and frequency domains between ABP and PPG signals for normotensive, prehypertensive and hypertensive subjects and the feasibility of the classification of subjects considering the results of the analysis performed. From a database with 120 records of ABP and PPG, each 120 s in length, the records where separated into epochs taking into account 10 heartbeats, and the following statistical measures were performed: Correlation (r), Coherence (COH), Partial Coherence (pCOH), Partial Directed Coherence (PDC), Directed Transfer Function (DTF), Full Frequency Directed Transfer Function (ffDTF) and Direct Directed Transfer Function (dDTF). The correlation coefficient was r > 0.9 on average for all groups, indicating a strong morphology similarity. For COH and pCOH, coherence (linear correlation in frequency domain) was found with significance (p < 0.01) in differentiating between normotensive and hypertensive subjects using PPG signals. For the dataset at hand, only two synchrony measures are able to convincingly distinguish hypertensive subjects from normotensive control subjects, i.e., ffDTF and dDTF. From PDC, DTF, ffDTF, and dDTF, a consistent, a strong significant causality from ABP→PPG was found. When all synchrony measures were combined, an 87.5 % accuracy was achieved to detect hypertension using a Neural Network classifier, suggesting that PPG holds most informative features that exist in ABP.

Abstract:Abstract: There are 422 million diabetes patients worldwide according to 2023 census of WHO, and around 100 million cases in India. 8.4 million type 1 diabetes cases were recorded as of 2021. By 2040, it is projected that the number of people living with Type 1 Diabetes will reach 13.5–17.4 million. The surge in diabetes cases has created the critical need for regular glucose monitoring to effectively manage this prevalent health issue. Currently, glucose levels are predominantly measured invasively by pricking the finger for a blood sample and involves invasive methods, such as fingertip pricks, which can be uncomfortable and inconvenient, particularly for paediatric patients. This method utilizes blood glucose monitoring devices, which measure sugar levels in a small blood sample placed on a disposable test strip and have a certain degree of inaccuracy. Patients do not test as often as they should because of the pain and hassle of finger pricks and many do not achieve optimal glycaemic control. However, the shift toward non-invasive continuous glucose monitoring is becoming increasingly imperative and a promising alternative. With current biotechnological advances, many techniques offer a non-invasive, continuous means of monitoring glucose levels and have garnered significant attention in recent years. The objective is to consolidate and summarize recent advancements in non-invasive continuous glucose monitoring, emphasizing integration into wearable platforms for seamless and real-time monitoring. Furthermore, paediatric patients with Type 1 Diabetes face elevated risks of severe hypoglycaemic and hyperglycaemic events due to challenges in glycaemic control. Frequently, monitoring blood glucose is crucial to mitigate these risks and achieve optimal glycaemic management. The objective of ongoing research is to validate AI-based algorithms utilizing ECG signals collected through non-invasive devices to detect glycaemic events. These advancements, once commercially available, have the potential to revolutionize glucose monitoring, offering a convenient, efficient, and less intrusive approach, particularly benefiting paediatric patients and improving overall healthcare management. Despite substantial research and development, challenges persist, including standardizing sweat collection methods, addressing sample degradation, accounting for variations in sweat composition among individuals, optimizing diverse detection methods, improving glucose detection sensitivity, and ensuring overall commercial viability. Overcoming these challenges is paramount to bridge the gap between research and market availability, paving the way for a technological revolution in glucose monitoring. In this study, we compared potential non-invasive glucose monitoring devices and techniques to evaluate the most accurate, pain-free, fast, and cost-friendly device. Upon examination and survey, we have theorized the drawbacks persisting in the existing devices and hypothesized techniques. Looking forward this study has a broader application in not only understanding the limitations and creating more accurate devices but also in continuous glucose monitoring for diabetic patient care.
Postprandial Plasma Glucose between 4 and 7.9 h May Be a Potential Diagnostic Marker for Diabetes

Abstract:Postprandial glucose levels between 4 and 7.9 h (PPG4–7.9h) correlate with mortality from various diseases, including hypertension, diabetes, cardiovascular disease, and cancer. This study aimed to assess if predicted PPG4–7.9h could diagnose diabetes. Two groups of participants were involved: Group 1 (4420 participants) had actual PPG4–7.9h, while Group 2 (8422 participants) lacked this measure but had all the diabetes diagnostic measures. Group 1 underwent multiple linear regression to predict PPG4–7.9h using 30 predictors, achieving accuracy within 11.1 mg/dL in 80% of the participants. Group 2 had PPG4–7.9h predicted using this model. A receiver operating characteristic curve analysis showed that predicted PPG4–7.9h could diagnose diabetes with an accuracy of 87.3% in Group 2, with a sensitivity of 75.1% and specificity of 84.1% at the optimal cutoff of 102.5 mg/dL. A simulation on 10,000 random samples from Group 2 revealed that 175 participants may be needed to investigate PPG4–7.9h as a diabetes diagnostic marker with a power of at least 80%. In conclusion, predicted PPG4–7.9h appears to be a promising diagnostic indicator for diabetes. Future studies seeking to ascertain its definitive diagnostic value might require a minimum sample size of 175 participants.

Abstract:Aims: The aim of the study was to evaluate usefulness of capillary blood glucose (CBG) for diagnosis of gestational diabetes mellitus (GDM) in resource-constrained settings where venous plasma glucose (VPG) estimations may be impossible. Methods: Consecutive pregnant women (n = 1031) attending antenatal clinics in southern India underwent 75-g oral glucose tolerance test (OGTT). Fasting, 1- and 2-h VPG (AU2700 Beckman, Fullerton, CA) and CBG (One Touch Ultra-II, LifeScan) were simultaneously measured. Sensitivity and specificity were estimated for different CBG cut points using the International Association of Diabetes in Pregnancy Study Groups (IADPSG) criteria for the diagnosis of GDM as gold standard. Bland-Altman plots were drawn to look at the agreement between CBG and VPG. Correlation and regression equation analysis were also derived for CBG values. Results: Pearson's correlation between VPG and CBG for fasting was r = 0.433 [intraclass correlation coefficient (ICC) = 0.596, p < 0.001], for 1H, it was r = 0.653 (ICC = 0.776, p < 0.001), and for 2H, r = 0.784 (ICC = 0.834, p < 0.001). Comparing a single CBG 2-h cut point of 140 mg/dl (7.8 mmol/l) with the IADPSG criteria, the sensitivity and specificity were 62.3 and 80.7 %, respectively. If CBG cut points of 120 mg/dl (6.6 mmol/l) or 110 mg/dl (6.1 mmol/l) were used, the sensitivity improves to 78.3 and 92.5 %, respectively. Conclusions: In settings where VPG estimations are not possible, CBG can be used as an initial screening test for GDM, using lower 2H CBG cut points to maximize the sensitivity. Those who screen positive can be referred to higher centers for definitive testing, using VPG.
Machine Learning-Based Diabetes Detection Using Photoplethysmography Signal Features

Abstract:Diabetes is a prevalent chronic condition that compromises the health of millions of people worldwide. Minimally invasive methods are needed to prevent and control diabetes but most devices for measuring glucose levels are invasive and not amenable for continuous monitoring. Here, we present an alternative method to overcome these shortcomings based on non-invasive optical photoplethysmography (PPG) for detecting diabetes. We classify non-Diabetic and Diabetic patients using the PPG signal and metadata for training Logistic Regression (LR) and eXtreme Gradient Boosting (XGBoost) algorithms. We used PPG signals from a publicly available dataset. To prevent overfitting, we divided the data into five folds for cross-validation. By ensuring that patients in the training set are not in the testing set, the model's performance can be evaluated on unseen subjects' data, providing a more accurate assessment of its generalization. Our model achieved an F1-Score and AUC of $58.8\pm20.0\%$ and $79.2\pm15.0\%$ for LR and $51.7\pm16.5\%$ and $73.6\pm17.0\%$ for XGBoost, respectively. Feature analysis suggested that PPG morphological features contains diabetes-related information alongside metadata. Our findings are within the same range reported in the literature, indicating that machine learning methods are promising for developing remote, non-invasive, and continuous measurement devices for detecting and preventing diabetes.

Abstract:Public health costs ascribable to air pollution are socialized in our society. To quantify the damage to public health, epidemiologic studies are needed. We present the methods and epidemiologic background data which form the basis for estimating the public health effect ascribable to air pollution. The figures are presented per 1 million "average" Swiss population and per 10 micrograms/m3 increase in long-term annual mean particulate pollution (PM10). Quantification was restricted to the health effects given below (due to lack of complete data for other effects or to avoid duplicating health effects which may be described by overlapping measurements). In parenthesis we present (1) the mean effect estimates (+/-1 SE) (% increase per 10 micrograms/m3 increment in PM10) derived from national and international epidemiologic studies, and (2) the expected absolute additional health effects (+/-1 SE) per 1 million Swiss population and per 10 micrograms/m3 increment in PM10, based on Swiss population statistics: total mortality (long-term estimates from the 2 US cohort studies) (+4.4% [+/-1.1]/349 [+/-91] premature deaths per year); prevalence of chronic bronchitis in adults (+25% [+8.4]/increase in long-term prevalence by 3,513 [+/-1.475]); bronchitis among children (35% [+/-13]/5,180 [+/-2,590] additional children sufferers in a year); repeated cough among children (+54% [+/-8.8]/23,490 [+/-5,873] additional children per year); cough/phlegm in adults (+12.8% [+/-7.8]/1.56 [+/-1.08] million person-days per year); hospital admissions for respiratory diseases (+1.47% [+/-0.5]/71 [+/-24] additional admissions or 1,104 (+/-390) hospital days); admissions for cardiovascular diseases (+0.9% [+/-0.25]/70 [+19] admissions; 970 (+/-270) hospital days); restricted activity days (+10.5% [+/-0.77]/0.42 [+/-0.03] million person-days); days with asthma attacks among the 6.7% asthmatics (Swiss prevalence) (+5.3% [+/-2.1]/ 240,000 [+/-102,000] additional person-days). Conservative assumptions were used throughout the study and thus the true effects are likely to be larger. Estimates of Swiss population exposure distribution would be needed to apply these results to the entire population. This step, and monetary quantification of these total effects, were the second and third elements (not shown in this report) of this Swiss government funded project. Although individual relative risks of air pollution are rather slight, the public health burden of even moderate pollution may be substantial.
Evaluation of point-of-care maternal glucose measurements for the diagnosis of gestational diabetes mellitus

Abstract:Objective: Using updated laboratory standards as the reference, we aimed to compare point-of-care (POC) maternal capillary glucose testing with the diagnostic accuracy of reference and customary venous samples. Design, setting, population: Women screened selectively with a one-step 75-g oral glucose tolerance test (OGTT) at 24-28 weeks' gestation were conveniently recruited to this prospective observational study. Methods: Two venous samples and one capillary sample were taken at each OGTT time point. Venous sample one was a fluoride-EDTA (FE) tube placed on an ice-slurry until cell separation and analysis within 30 minutes (reference standard). Venous sample two was transported in a tube containing FE (without ice) (customary practice). A capillary sample was used for POC testing. Various cut-off points for the POC sample were examined to evaluate diagnostic accuracy. Main outcome measures: The sensitivity, specificity, positive and negative predictive values and accuracy of POC capillary glucose for the diagnosis of GDM. Results: Of 108 women, GDM was detected in 47.2% (n = 51), 17.6% (n = 19) and 24.1% (n = 26) using the reference standard, customary practices and POC, respectively (P < 0.001). However, based on adjustment of the POC fasting diagnostic threshold from ≥5.1 to ≥4.8 mol/l (aPOC), sensitivity, specificity, PPV, NPV and accuracy improved to 92.5, 76.5, 69.8, 94.5 and 94.5%, respectively. Conclusions: POC capillary maternal glucose tests were superior to customary laboratory practices for diagnosing GDM. This has considerable potential, particularly in healthcare settings where facilities for phlebotomy are distant from the laboratory or pre-analytical sample handling is substandard. Abstract:Regional ecological risk assessment (ERA) is more complex than general ERA, as it requires that risk receptors, risk sources, risk exposure, uncertainty and especially spatial heterogeneity all be taken into account. Taking the catchment area along the Weihe River in Shaanxi Province as a study area, a mathematical model of regional ERA was developed and tested on the basis of elementary principles and methods of risk measurement. Through selecting four main risk sources of drought, flood, pollution and soil erosion, with the assistance of geographical information system(GIS) technique, the integrated risk value which was calculated by indices and formulas was classified into five grades (the lowest, lower, moderate, higher and the highest). Each risk area was assigned to one of the five classes. The results can provide quantitatively theoretic basis and data support for environmental management and ecological risk decision-making in the lower Weihe River.
Diagnostic Potential of Combined Photoplethysmography and Ankle-Brachial Index in Peripheral Arterial Disease: A Duplex Ultrasonography-Based Comparative Study

Abstract:Background: Although ankle-brachial index (ABI) and photoplethysmography (PPG) have also shown adequate sensitivity in detecting peripheral arterial disease, their diagnostic performance is less reliable in asymptomatic cases or those with high atherosclerotic cardiovascular risks. Methods and results: We evaluated 130 participants using ABI, PPG, and duplex ultrasonography, diagnosing 65 with peripheral arterial disease. From the PPG, we derived 2 parameters: PPG amplitude ratio of the lower-to-upper extremities (PPGratio) and the PPG amplitude of the lower extremity (PPGamp). Sensitivity, specificity, accuracy, and the area under receiver operating characteristic (ROC) curve were calculated for PPG parameters and ABI, and their combination of both methods. Univariate and multivariate logistic regression assessed the prognostic potential of these parameters. ROC analysis revealed optimal cutoff values in diagnosing peripheral arterial disease were 0.417 for PPGratio and "58" for PPGamp. Both PPGratio and PPGamp demonstrated significantly higher sensitivities, 78.4% and 75.7%, respectively, compared with 55.9% for ABI <0.9 (P<0.05). The areas under the ROC curves of combination models, including model 1 (ABI <0.9 and PPGratio), model 2 (ABI <0.9 and PPGamp), and model 3 (ABI <0.9, PPGratio, and PPGamp), exhibited improved performance with areas under the ROC curves of 0.922, 0.922, and 0.931 (all P<0.01) compared with ABI alone (area under the ROC curve, 0.822). Additionally, the PPG parameters, both alone and combined with ABI, were associated with major adverse cardiac events and all-cause mortality after adjusting for other relevant factors. Conclusions: On the basis of duplex ultrasonography, combining ABI and PPG markedly improves peripheral arterial disease diagnosis in high-risk individuals compared with either method alone and provides crucial insights into major adverse cardiac events and all-cause mortality risks.

Abstract:Introduction: Globally, hypertension (HT) is a substantial risk factor for cardiovascular disease and mortality; hence, rapid identification and treatment of HT is crucial. In this study, we tested the light gradient boosting machine (LightGBM) machine learning method for blood pressure stratification based on photoplethysmography (PPG), which is used in most wearable devices. Methods: We used 121 records of PPG and arterial blood pressure (ABP) signals from the Medical Information Mart for Intensive Care III public database. PPG, velocity plethysmography, and acceleration plethysmography were used to estimate blood pressure; the ABP signals were used to determine the blood pressure stratification categories. Seven feature sets were established and used to train the Optuna-tuned LightGBM model. Three trials compared normotension (NT) vs. prehypertension (PHT), NT vs. HT, and NT + PHT vs. HT. Results: The F1 scores for these three classification trials were 90.18%, 97.51%, and 92.77%, respectively. The results showed that combining multiple features from PPG and its derivative led to a more accurate classification of HT classes than using features from only the PPG signal. Discussion: The proposed method showed high accuracy in stratifying HT risks, providing a noninvasive, rapid, and robust method for the early detection of HT, with promising applications in the field of wearable cuffless blood pressure measurement.

Abstract:The use of observed wearable sensor data (e.g., photoplethysmograms [PPG]) to infer health measures (e.g., glucose level or blood pressure) is a very active area of research. Such technology can have a significant impact on health screening, chronic disease management and remote monitoring. A common approach is to collect sensor data and corresponding labels from a clinical grade device (e.g., blood pressure cuff) and train deep learning models to map one to the other. Although well intentioned, this approach often ignores a principled analysis of whether the input sensor data have enough information to predict the desired metric. We analyze the task of predicting blood pressure from PPG pulse wave analysis. Our review of the prior work reveals that many papers fall prey to data leakage and unrealistic constraints on the task and preprocessing steps. We propose a set of tools to help determine if the input signal in question (e.g., PPG) is indeed a good predictor of the desired label (e.g., blood pressure). Using our proposed tools, we found that blood pressure prediction using PPG has a high multi-valued mapping factor of 33.2% and low mutual information of 9.8%. In comparison, heart rate prediction using PPG, a well-established task, has a very low multi-valued mapping factor of 0.75% and high mutual information of 87.7%. We argue that these results provide a more realistic representation of the current progress toward the goal of wearable blood pressure measurement via PPG pulse wave analysis.
