Abstract:INTRODUCTION:Traumatic thoracolumbar burst fracture is a common condition without a clear consensus on the best treatment approach. Percutaneous pedicle screw fixation (PPSF) techniques are widely used in practice, while its ability to correct fracture deformity is relatively weak, especially for the central area of the endplate. In this study, we reported a novel technique to reduce the fractured central endplate in thoracolumbar burst fractures.METHODS:The new reduction technique uses six percutaneous pedicle screws for the fractured vertebra and its adjacent vertebrae. Pedicle screws implanted in the two adjacent vertebrae were parallel to the superior vertebral endplate, as routinely required. Two monoaxial pedicle screws implanted in the fractured vertebra were placed toward the anteroinferior portion of the fractured vertebral body. After routine instrumentation and ligamentotaxis reduction, the bolt heads of the four screws implanted in the adjacent vertebrae were first tightened, and then the bolt heads of the screws implanted in the fractured vertebra were gradually tighten to elevate the collapsed endplate. A fundamental principle of this technique is to implant the pedicle screw in the fractured vertebra towards the anteroinferior portion of the vertebra in such a way that the angle between the pedicle screw and the rod is oblique on lateral fluoroscopy. As such, when the bolt heads were tightened, the pedicle screws can be swung up to reduce the endplate fragments.RESULTS:The novel technique was performed in 24 patients with neurologically intact thoracolumbar AO type A3 fractures. The middle vertebral height ratio was significantly improved from 69.7%±7.6% after routine reduction to 85.1%±4.5% postoperatively (p<0.01). No complication was noticed for this new reduction technique. At 6-month follow-up, no significant correction loss of the middle and posterior vertebral height ratios, Cobb angle, and vertebral wedge angle was observed, while 5.8% of correction loss was observed for the anterior vertebral height ratio.CONCLUSION:The described reduction technique is simple, safe, and effective in reducing the collapsed central endplate in thoracolumbar burst fractures. Such a practical reduction strategy does not need additional medical costs.

Abstract:Object To explore the feasibility and efficiency of treatment of minimally invasive percutaneous pedicle screws osteosynthesis(MIPPSO) and vertebroplasty by filling the injured vertebrae with injectable artificial bones on thoracolumbar fractures.Methods From Feb 2008 to Sep 2011,10 patients were treated with MIPPSO and vertebroplasty by filling the injured vertebrae with injectable artificial bones on thoracolumbar fractures.Results The average operative time was 80 minutes ﹙range,70~90min﹚,with a mean blood loss of 72ml﹙range,50~100ml﹚ and mean postoperative hospital stay of 8 days ﹙range,7~10 days﹚.23 cases were followed up 3~5 months ﹙average 3.5 months﹚,During the following-up,no case showed internalfixation device loosening or breakage;No case had anterior height loss of body of spine or collapse.CT results showed all injected artificial bones were absorbed within 3~4 months postoperatively.The anterior height and Cobb angle of the injured vertebrae body were significantly less than before treatment[﹙10.78�.53﹚%、﹙53.41�.35﹚%,P<0.0l;8.6�.9��22.72�.6� P<0.0l].Conclusion Fixation with vertebral pedicle screw system and vertebroplasty using injectable artificial bones is a safe and effective minimally invasivesurgical method.

Abstract:INTRODUCTION: Lateral lumbar interbody fusion (LLIF) including the anterior-to-psoas oblique lumbar interbody fusion (OLIF) has conventionally relied on pedicle screw placement (PSP) for construct stabilization. Single-position surgery with lumbar interbody fusion (LIF) in the lateral decubitus position with concomitant PSP has been associated with increased operative efficiency. What remains unclear is the accuracy of PSP with robotic guidance when compared to the more familiar prone patient positioning. METHODS: We identified all consecutive patients treated with interbody fusion and PSP in the prone or lateral position by a single surgeon between January 2019 and October 2022. All pedicle screws placed were analyzed using CT imaging to determine appropriate positioning according to the Gertzbein-Robbins Classification Grading System. Multivariate logistic regression models were constructed to identify risk factors for the occurrence of a radiographically significant breach. RESULTS: 89 consecutive patients were included (n = 690 screws), of whom 46 were treated in the prone position (n = 477 screws) and 43 in the lateral decubitus position (n = 213 screws). There were fewer breaches in the prone (n = 13; 2.7%) versus lateral decubitus group (n = 15; 7.0%; p = 0.012). Nine (1.9%) radiographically significant breaches occurred in the prone group compared to 10 (4.7%) in the lateral decubitus group (p = 0.019), for a prone vs. lateral decubitus pedicle screw accuracy rate of 98.1% vs. 95.3%. There were no significant differences in BMI between prone vs. lateral decubitus cohorts (30.1 vs. 29.6) or breached patients vs. non-breached patients (31.2 vs. 29.5). In multivariate models, prone position was the only significant protective factor for screw accuracy; we did not identify any other significant risk factors for screw breach. CONCLUSIONS: The present data suggest pedicle screws placed with robotic assistance have higher placement accuracy in the prone position.

Vertebral fracture at the caudal end of a surgical fusion for thoracic vertebral fracture in a patient with diffuse idiopathic skeletal hyperostosis (DISH)

Abstract:The patient was an 86-year-old woman with back pain after a fall. She had no neurological findings at the initial visit. Plain radiographs and magnetic resonance imaging (MRI) showed diffuse idiopathic skeletal hyperostosis (DISH) and a Th10 fracture. Two weeks later, she started gait exercise with immobilization by a rigid orthosis. Twenty-five days later, she presented with paralysis and numbness of her legs. Computed tomography (CT) showed anterior expansion in the vertebral body of Th10. MRI showed an intramedullary high-intensity area on T2-weighted images at the same level. She was diagnosed as having delayed paraplegia after a Th10 fracture and transferred to our hospital for surgery. Laminectomy of Th10, posterior fusion from Th7 to L1 with pedicle screws and hooks to Th6 and L1 laminae, anterior fusion from Th9 to Th11 with a plate, and autologous bone grafting were performed simultaneously. The patient's paralysis improved, and she started gait exercise with no limitation of bed rest and without an orthosis after surgery. At 8 days after surgery, she again presented with low back pain and paralysis in her legs. CT revealed an L1 fracture, which was the caudal end of the surgical fusion. The decreased kyphosis after surgery compared to that at pre-injury might have caused a subsequent horizontal shear force to L1 when the patient sat on the bed and when she walked. In conclusion, to avoid postoperative adjacent vertebral fracture after fusion, appropriate correction of spinal alignment to that at pre-injury is needed for vertebral fractures in patients with DISH.
Efficacy of One-Stage Posterior-Anterior Approach for Treatment of Severe Thoracolumbar Fractures

Abstract:Objective To observe the outcomes of one-stage posterior short-level pedicle screw fixation combined with anterior fixation of severe thoracolumbar fractures.Methods A retrospective case series study was performed on 21 patients with severe thoracolumbar fractures stabilized by posterior short-level pedicle fixation combined with anterior internal fixation at one stage from January 2012 to December 2014.There were 16 males and 5 females,at age of 17 and 64 years [(38.7 ± 11.4) years].The involved segments included T11 in 2 patients,T12 in 5,Lt in 6 and L2 in 8.For AO fracture classification,type A fractures were seen in 4 patients,type B in 7 and type C in 10.Thoracolumbar injury classification and severity score (TLICS) was (8.12 ± 0.87) points (range,7-10 points).Frankel neurological performance scale was Grade B in 8 patients,Grade C in 11 and Grade D in 2.Operation time,blood loss,nerve function,kyphosis correction and complications were reported.Results Operation time was (234.5 ±57.3)min (range,180-360 min),and blood loss was (387.4 ± 124.4) ml (range,260-950 ml).Time of follow-up was (19.8 ± 3.5)months (range,14-25 months).Nerve function of 18 patients was improved by at least one Frankel scale.Cobb angle was (4.1 ±5.3)° at postoperative 3 days and (4.0 ± 4.9)°at the final follow-up,showing significant differences from that before operation [(-9.3 ± 4.2) °] (P ＜ 0.05).While the difference of Cobb angle did not differ significantly at postoperative 3 days and at final follow-up.No cerebrospinal fluid leakage,vascular injury,incision infection or nerve function deterioration occurred.Conclusion One-stage posterior short-level pediele screw fixation combined with anterior decompression and bone graft fixation is characterized by short operation time,few blood loss,good correction of traumatic kyphosis and good neurological recovery,indicating a good surgical choice for severe thoracolumbar fractures.
Types of Spinal Instability That Require Interbody Support in Posterior Lumbar Reconstruction: an in Vitro Biomechanical Investigation.

Abstract:Study Design. Pedicle screw fixation alone for sequential spinal instabilities was biomechanically compared with pedicle screw fixation using interbody cages.Objective. To evaluate biomechanical effects of interbody cages on construct stiffness, pedicle-screw strain, and the adjacent level in posterior lumbar reconstruction using pedicle screw fixation.Summary of Background Data. It remains undetermined what types of spinal instability require interbody support in posterior lumbar reconstruction.Methods. For this study, 10 calf spines (L3-L6) were used. Sequential destabilization was performed at L4-L5 followed by posterior reconstruction using pedicle screw fixation (PS) and interbody cages as follows: intact + PS (I-PS), medial facetectomy + PS (MF-PS), total facetectomy + PS (TF-PS), partial discectomy + PS (D-PS), and D-PS + interbody cages (PLIF). Biomechanical testing was performed under flexion and extension loading modes. Construct stiffness (L4-L5), rod-screw bending strain, and range of motion (ROM) at the upper adjacent level (L3-L4) were analyzed.Results. In terms of construct stiffness (L4-L5), all the reconstructions except D-PS demonstrated higher construct stiffness than the intact spine (P < 0.05). The PLIF showed the highest stiffness among all the reconstructions (P < 0.05). In terms of ROM (L3-L4), all the reconstructions increased the ROM, as compared with the intact state (P < 0.05). Importantly, PLIF showed significantly greater ROM than all the other reconstructions except I-PS (P < 0.05). In terms of rod-screw strain, the D-PS resulted in higher strain than the other groups (P < 0.05). The PLIF presented less strain than the other reconstructions (P < 0.05).Conclusions. For spinal instability with preserved anterior load sharing, pedicle screw fixation alone is biomechanically adequate, and the interbody cages should not be used because they further increase segmental motion at the adjacent segment. However, PS alone provides insufficient stability and high implant strain in case of damaged anterior column. In such cases, additional interbody cages significantly increase construct stiffness and decrease hardware strain. However, they increase ROM at the adjacent segment as well.
Oblique Lateral Interbody Fusion Combined With Lateral Plate Fixation for the Treatment of Lumbar Degenerative Diseases: a Preliminary Clinical Study

Abstract:Abstract Background The oblique lateral interbody fusion (OLIF) is a minimally invasive indirect decompression technique for the treatment of lumar spine disease. The OLIF has usually combined with supplemental posterior pedicle screw fixation for decreasing the perioperative complications. The purpose of this study was to evaluate clinical efficacy and complications of oblique lateral interbody fusion (OLIF) combined with lateral plate instrumentation for the treatment of lumbar degenerative diseases.Methods From May 2020 to August 2020, the clinical data of 20 patients who underwent OLIF combined with lateral plate instrumentation were retrospectively analyzed. The operation time, blood loss, and the complications were recorded. Also, the radiological parameters, Visual Analog Scale (VAS) score and Oswestry Disability Index (ODI) were evaluated before and after surgery.Results The average operation time, blood loss, and length of hospital stay were 75.41±11.53 min, 39.57±9.22 ml, and 7.22±1.85 days, respectively. The VAS and ODI had both significantly improved after surgery (7.23±1.26 VS 2.15±0.87; 60.27±7.91 VS 21.80±6.32, P < 0.001). The postoperative disk height (DH) was 13.02±8.83 mm, which is much higher than before (P < 0.001); The postoperative foraminal height (FH) improved significantly (16.18±3.49 VS 21.54±2.12 mm, P < 0.01), and the cross-sectional area (CSA) had improved from 88.95±14.79 mm2 to 126.53±8.83 mm2 (P < 0.001). The radiological fusion rate was 95% at the last follow-up, while cage subsidence was found in one case. No major complications, such as ureteral injury, vascular injury and vertebral body fracture occurred.Conclusions OLIF combined with lateral plate fixation can avoid the lumbar posterior surgery, minimize the operation time, blood loss and the posterior ligament complex disruption. It can realize one-stage intervertebral fusion and instrumentation through a single small incision.
Hybrid kyphoplasty with short-versus intermediate- and long-segment pedicle screw fixations for the management of thoracolumbar burst fractures

Abstract:Abstract Background This study aimed to determine if the hybrid short-segment (HSS) technique is a good alternative to the intermediate-segment (IS) and long-segment (LS) techniques in pedicle screw fixations for acute thoracolumbar burst fractures (TLBFs). Methods In this retrospective evaluation, we examined 43 patients who underwent surgical treatments, including one- or two-level suprajacent (U) and infrajacent (L) pedicle screw fixations, for acute single-level TLBFs with neurological deficits between the T11 and L2 levels from July 2013 to December 2019. Among these patients, 15 individuals underwent HSS (U1L1), 12 received IS (U2L1), and 16 underwent LS (U2L2) fixations. Supplemental kyphoplasty of the fractured vertebral bodies was performed exclusively in the HSS group. Our analysis focused on assessing blood loss and surgical duration. Additionally, we compared postoperative thoracolumbar kyphotic degeneration using the data on Cobb angles on lateral radiographic images acquired at three time points (preoperatively, postoperative day 1, and follow-up). The end of follow-up was defined as the most recent postoperative radiographic image or implant complication occurrence. Results Blood loss and surgical duration were significantly lower in the HSS group than in the IS and LS groups. Additionally, the HSS group exhibited the lowest implant complication rate (2/15, 13.33%), followed by the LS (6/16, 37.5%) and IS (8/12, 66.7%) group. Implant complications occurred at a mean follow-up of 7.5 (range: 6–9), 9 (range: 5–23), and 7 (range: 1–21) months in the HSS, IS, and LS groups. Among these implant complications, revision surgeries were performed in two patients in the HSS group, two in the IS group, and one in the LS group. One patient treated by HSS with balloon kyphoplasty underwent reoperation because of symptomatic cement leakage. Conclusions The HSS technique reduced intraoperative blood loss, surgical duration, and postoperative implant complications, indicating it is a good alternative to the IS and LS techniques for treating acute single-level TLBFs. This technique facilitates immediate kyphosis correction and successful maintenance of the corrected alignment within 1 year. Supplemental kyphoplasty with SpineJack® devices and high-viscosity bone cements for anterior reconstruction can potentially decrease the risk of cement leakage and related issues.

Percutaneous Screw Distraction for Anatomic Restoration: Case Series

Abstract:Background and objectives: Percutaneous pedicle screw fixation with distraction reduces morbidity after traumatic thoracolumbar burst fractures; however, there are substantial limitations, particularly for correction of kyphosis. The use of fixed-angle screws may offer improved anatomic restoration, facilitating greater postdistraction vertebral height restoration and spinal canal fragment reduction. We examined the radiographic results of distraction across fixed-angle screws immediately after surgery and in long-term follow-up. Methods: Demographic and clinical characteristics were captured for patients with traumatic thoracolumbar fractures undergoing percutaneous pedicle screw fixation by a single surgeon. Radiographic measurements were collected at predistraction, postdistraction, and long-term follow-up time points. Paired t-tests, Student's t-tests, Mann-Whitney U tests, and χ2 tests were used to assess data where appropriate. Results: The case series included 22 patients (77.3% male; mean age 42.0 ± 18.4 years). Hounsfield density consistent with osteopenia was seen in 13.6% of patients at the time of injury. Sporting injuries and motor vehicle accidents were common (both 31.8%). Most injuries occurred at L1 (45.5%). Upon long-term follow-up, the mean injured-level predistraction cross-sectional area improved from 2.1 to 2.9 cm2 (P < .01). Compared with the superadjacent level, the injured-level cross-sectional canal area improved by 28.6% (P < .01). Vertebral body index also improved significantly (18.8° mean change, P < .01). The mean bisegmental Cobb angle improved by 6.2° (P = .01), and injured vertebral body compression decreased by 22.4% (P < .01). Significant improvement in correction was achieved with experience, with final technique yielding superior cross-sectional area (P = .04) and compression ratios (P = .03). Conclusion: Distraction across fixed-angle percutaneous screw instrumentation systems stabilizes traumatic thoracolumbar burst fractures, corrects deformity, and decompresses the spinal canal. Further comparative research is necessary to demonstrate whether outcomes are different between percutaneous instrumentation vs open fusion for thoracolumbar trauma.
An Innovative Prone Position Using a Body-Shape Plaster Bed and Skull Traction for Posterior Cervical Spine Fracture Surgeries

Abstract:Background: An innovative prone cervical spine surgical position using a body-shape plaster bed with skull traction (BSPST) was compared with the traditional prone surgical position with horseshoe headrests. Methods: A total of 47 patients, undergoing posterior cervical spine surgery for cervical spine fracture, were retrospectively classified into two groups, the BSPST group (n = 24) and the traditional group (n = 23), and underwent a posterior instrumented fusion with or without decompression. Multiple indicators were used to evaluate the advantages of the BSPST compared with the traditional position. Results: All the operations went smoothly. The mean recovery rate was 56.30% in the BSPST group and 48.55% in the traditional group (p = 0.454), with no significant difference. The intraoperative blood loss (177.5 ml vs. 439.1 ml, p = 0.003) and the total incidence of complications (8.3 vs. 47.8%, p = 0.004) were significantly less in the BSPST group than in the traditional group. In addition, the BSPST position provided a greater comfort level for the operators and allowed convenient intraoperative radiography. Conclusions: This is the first study to describe a combined body-shape plaster bed and skull traction as an innovative cervical spine-prone surgical position that is simple, safe, and stable, intraoperative traction direction adjustable, reproducible, and economical for posterior cervical spine fracture surgery, and potentially other cervical and upper dorsal spine surgeries in the prone position. Additionally, this position provides the surgeons with a comfortable surgical field and can be easily achieved in most orthopedic operation rooms.
[Effectiveness of injured vertebra fixation with inclined-long pedicle screws combined with interbody fusion for thoracolumbar fracture dislocation with disc injury]

Abstract:Objective: To investigate the effectiveness of injured vertebra fixation with inclined-long pedicle screws combined with interbody fusion for thoracolumbar fracture dislocation with disc injury. Methods: Between January 2017 and June 2022, 28 patients with thoracolumbar fracture dislocation with disc injury were underwent posterior depression, the injured vertebra fixation with inclined-long pedicle screws, and interbody fusion. There were 22 males and 6 females, with a mean age of 41.4 years (range, 22-58 years). The causes of injury included falling from height in 18 cases, traffic accident in 5 cases, and bruise in 5 cases. Fracture segment included 1 case of T 11, 7 cases of T 12, 9 cases of L 1, and 11 cases of L 2. According to the American Spinal Injury Association (ASIA) scale, the spinal injuries were graded as grade A in 4 cases, grade B in 2 cases, grade C in 11 cases, and grade D in 11 cases. Preoperative spinal canal encroachment ratio was 17.7%-75.3% (mean, 44.0%); the thoracolumbar injury classification and severity score (TLICS) ranged from 9 to 10 (mean, 9.9). Seventeen patients were associated with other injuries. The time from injury to operation ranged from 1 to 4 days (mean, 2.3 days). The perioperative indicators (operation time, intraoperative blood loss, and the occurrence of complications), clinical evaluation indicators [visual analogue scale (VAS) score and Oswestry Disability Index (ODI)], radiologic evaluation indicators [anterior vertebral height ratio (AVHR), kyphosis Cobb angle (KCA), intervertebral space height (ISH), vertebral wedge angle (VWA), displacement angle (DA), and percent fracture dislocation displacement (PFDD)], neurological function, and interbody fusion were recorded. Results: The operation time was 110-159 minutes (mean, 130.2 minutes). The intraoperative blood loss was 200-510 mL (mean, 354.3 mL). All incisions healed by first intention, and no surgical complications such as wound infection or hematoma occurred. All patients were followed up 12-15 months (mean, 12.7 months). The chest and lumbar pain significantly relieved, VAS scores and ODI after operation were significantly lower than those before operation, and further decreased with the extension of postoperative time, with significant differences ( P<0.05). At last follow-up, the ASIA classification of neurological function of the patients was grade A in 3 cases, grade B in 1 case, grade C in 1 case, grade D in 10 cases, and grade E in 13 cases, which was significantly different from preoperative one ( Z=-4.772, P<0.001). Imaging review showed that AVHR, KCA, ISH, VWA, DA, and PFDD significantly improved at 1 week, 3 months and last follow-up ( P<0.05). There was no significant difference between different time points after operation ( P>0.05). At last follow-up, according to the modified Brantigan score, all patients achieved good intervertebral bone fusion, including 22 complete fusion and 6 good intervertebral fusion with a few clear lines. No complications such as internal fixation failure or kyphosis occurred during follow-up. Conclusion: The injured vertebra fixation with inclined-long pedicle screws combined with interbody fusion is an effective treatment for thoracolumbar fracture dislocation with disc injury, which can correct the fracture dislocation, release the nerve compression, restore the injured vertebral height, and reconstruct spinal stabilization.

Abstract:Objective: To compare the differences of spinopelvic morphology among patients with DISH, patients without DISH and normal elderly and to assess the impact of ossification extent on sagittal alignment. Methods: Patients with and without DISH aged > 50 years who required surgery because of lumbar spinal stenosis were enrolled in this cohort(DISH and Non-DISH groups). Also, we collected age-matched normal old outpatients as the control group(Normal group). According to ossification extent, DISH group were divided into two subgroups(T-DISH and L-DISH subgroups). Spinopelvic parameters were measured. Distribution differences of Roussouly classification were analyzed between DISH and Non-DISH group, T-DISH and L-DISH subgroup, respectively. Additionally, distribution difference of kyphotic apex vertebrae between T-DISH and L-DISH subgroup was also investigated. Results: A total of 429 patients (300 males and 129 females) were enrolled in our study, with a mean age of 64.1 +/- 5.8 years. Compared to the Normal group, DISH and Non-DISH groups both had significantly higher CSVA, PT, OH, SVA, TPA and lower LL, SS, C7 Tilt, SSA, SPA. Compared to Non-DISH group, DISH group, regardless of ossification extent, had significantly higher T1 slope, CSVA, TK and SVA. Besides, T-DISH subgroup showed significant higher LL, PI, SS and SSA than L-DISH subgroup. There were significant differences of Roussouly classification distribution between T-DISH and L-DISH subgroup. In terms of kyphotic apex location, compared to relatively higher locations in T-DISH subgroup, L-DISH subgroup had apical locations predominantly in the lower thoracic. Conclusion: Sagittal spinopelvic alignment is influenced by the presence of DISH and the extent of ossification. Patients with L-DISH have not only increased thoracic kyphosis and forward trunk, but also insufficient lumbar lordosis.
Prospective evaluation of thoracic pedicle screw placement using fluoroscopic imaging

Abstract:Background: In this prospective 18-month study, 29 patients underwent posterior thoracic instrumentation with placement of 209 transpedicular screws guided by intraoperative fluoroscopic imaging and anatomic landmarks. We assessed the safety, accuracy, complications, and early stability of this technique. Methods: Pedicle and pedicle-rib units were measured, and screw cortical penetrations were graded on anatomy and depth of penetration. All 29 patients underwent preoperative computed tomographic (CT) imaging, and 28 underwent postoperative CT imaging (199/209 screws). Results: From T2 to T12, screw diameters were >or=5 mm with mean medial screw angulation measuring 20-25 degree. Of the 209 screws placed from T1 to T12, 111 had diameters greater than or equal to the pedicle width. From T3 to T9, the mean diameter of the pedicle screws exceeded the mean pedicle width. Lateral pedicle wall penetration occurred significantly more often than superior, inferior, and medial pedicle wall penetrations and anterolateral vertebral body penetration. Five of six high-risk screw penetrations occurred in one patient when intraoperative technique was compromised. We observed no new postoperative neurologic deficits, visceral injuries, or pedicle screw instrumentation failures. The three high-risk anterolateral vertebral body penetrations at T1 and T2 were associated with a significantly decreased mean screw transverse angle; the three high-risk medial pedicle wall penetrations occurring from T3 to T9 were associated with a significantly increased mean screw transverse angle. Among all 26 patients available at postoperative follow-up (mean 11.9 months), the mean loss of kyphosis correction was 2.0 degree. Conclusions: Guided by intraoperative fluoroscopic imaging and anatomic landmarks, thoracic pedicle screws can be placed safely. Early clinical follow-up reveals excellent results with minimal loss of kyphosis correction.

Abstract:Background and objectives: Percutaneous pedicle screw (PPS) placement is a minimally invasive spinal procedure that has been rapidly adopted over the last decade. However, PPS placement has elicited fear of increased radiation exposure from some surgeons, medical staff, and patients. This is because PPS placement is performed using a K-wire, and the operator must perform K-wire insertion into the pedicle under fluoroscopy. In order to prevent erroneous insertion, there are many occasions when direct insertion is required during radiation exposure, and the amount of radiation exposure to hands and fingers in particular increases. Although these problems are being addressed by navigation systems, these systems are still expensive and not widely available. Attempts have been made to address this situation using instrumentation commonly used in spinal surgery. First, it was considered to visualize anatomical bone markers using a tubular retractor and a microscope. In addition, the use of a self-drilling pin was adopted to locate the pedicle in a narrower field of view. Based on these considerations, a minimally invasive and highly accurate pedicle screw placement technique was developed while avoiding direct radiation exposure. This study evaluated radiation exposure and accuracy of pedicle screw placement using this new procedure in one-level, minimally invasive, transforaminal lumbar interbody fusion (MIS-TLIF). Materials and methods: Data were collected retrospectively to review pedicle screw placement in single-level MIS TLIFs using a tubular retractor under a microscope. The total fluoroscopy time, radiation dose, and screw placement accuracy were reviewed. Extension of operating time was also evaluated. Results: Twenty-four patients underwent single-level MIS TLIFs, with placement of 96 pedicle screws. There were 15 females and 9 males, with an average age of 64.8 years and a mean body mass index of 25.5 kg/m2. The mean operating time was 201.8 min. The mean fluoroscopic time was 26.8 s. The mean radiation dose of the area dose product was 0.0706 mGy∗m2. The mean radiation dose of air kerma was 6.0 mGy. The mean radiation dose of the entrance skin dose was 11.31 mGy. Postoperative computed tomography scans demonstrated 93 pedicle screws confined to the pedicle (97%) and three pedicle screw breaches (3.2%; two lateral, one medial). A patient with screw deviation of the medial pedicle wall developed right-foot numbness necessitating reoperation. There were no complications after reoperation. The average added time with this combined procedure was 39 min (range 16-69 min) per patient. Conclusions: This novel pedicle screw insertion technique compares favorably with other reports in terms of radiation exposure reduction and accuracy and is also useful from the viewpoint of avoiding direct radiation exposure to hands and fingers. It is economical because it uses existing spinal surgical instrumentation.

Abstract:Objective: To compare correction of kyphotic deformity (KD) and implant failure (IF) in percutaneous short-segment pedicle screws fixation (SSPF) with index level versus long-segment pedicle screws fixation (LSPF) without index level for traumatic thoracolumbar (TL) fractures. Methods: This prospective study comprised 56 patients who met the study's inclusion criteria from the Department of Neurosurgery at the Punjab Institute of Neurosciences in Lahore, Pakistan presented between June 2022 and May 2023. We separated them into two groups: Group-A and Group-B, each with 28 patients. Group-A consisted of percutaneous SSPF with incorporated screws in the fractured vertebra, whereas Group-B consisted of percutaneous LSPF without index level involvement for traumatic TL fractures. We reviewed the patient's preoperative, postoperative, and follow-up radiographs. The quantitative factors such as Cobb's angle and implant stability were investigated. Results: The study comprised individuals with a mean age of 31.5 ± 10.6 SD years. Out of 56 patients, 38 (67.85%) were male and 18 (32.14%) were female. The fracture level distribution was 37 (67.07%) patients with L1 fracture, 15 (26.78%) with D12 fracture, 2 (3.57%) with D11 fracture, and 2 (3.57%) with L2 fracture. Group-A had a preoperative Cobb's angle of 18.8° ± 5.0° SD, whereas Group-B had 19.8° ± 6.3° SD (P-value=0.23). Immediate postoperative Cobb's angle was 6.4° ± 3.4° SD in Group-A and 7.3° ± 3.7° SD in Group-B (P-value 0.66). After three months, Group-A had a Cobb's angle of 7.1° ± 3.6° SD, whereas Group-B had 7.8° ± 3.7° SD (P-value = 0.78). Six-month follow-up Cobb's angle was 7.9° ± 3.6° SD in Group-A and 8.4° ± 3.8° SD in Group-B (P-value=0.502). There were no implant failures in any group. Conclusions: For a single level traumatic TL fracture, SSPF with index level can preserve Cobb's angle better than LSPF without index level, and it has high IF stability. Abstract:Introduction. Spinal fractures with subsequent bone fragment dislocation are among the injuries most feared by patients and physicians. The surgical strategy is tailored to the individual patient's characteristics and often consists of pedicle instrumentation with rod-screw systems. Short instrumentation has been associated with worse spinal correction and increased complications. However, recent studies have suggested similar results in terms of kyphosis correction and the maintenance of sagittal alignment compared to longer instrumentation. Material and methods. This single-center retrospective study was conducted between January 2018 and April 2021. We included 35 single lumbar burst fractures AO Spine grade A3 or A4 with evidence of intra-canal fragments. Patients underwent minimally invasive percutaneous posterior lumbar instrumentation with pedicle screws. Patients received short segmental fixation involving only one level above and below the fractured vertebra. Results. An immediate postoperative computed tomography (CT) scan demonstrated a significant reduction in vertebral kyphotic deformation (11.7° ± 1.6 vs 16.7° ± 5, p<0.001) and sagittal Cobb angle (9.8° ± 1.3 vs 11.7° ± 1.5, p < 0.001). The correction was slightly reduced but remained significant at 12 months for both kyphotic (12.3° ± 1.4, p = 0.03) and sagittal Cobb (10.3° ± 0.9, p = 0.04). Upper lumbar vertebrae showed even larger correction indices compared to lower lumbar segments. No implant failure or screws pullout was seen at the last follow-up. Conclusions. Short spinal fixation is a safe and effective treatment of complete and incomplete burst fractures with posterior bone fragment dislocation. All included patients fared well and achieved good kyphotic correction with no perioperative or long-term complications.

Accuracy and safety of pedicle screw fixation in thoracic spine trauma

Abstract:Object: The authors evaluated the accuracy of placement and safety of pedicle screws in the treatment of unstable thoracic spine fractures. Methods: Patients with unstable fractures between T-1 and T-10, which had been treated with pedicle screw (PS) placement by one of five spine surgeons at a referral center were included in a prospective cohort study. Postoperative computed tomography scans were obtained using 3-mm axial cuts with sagittal reconstructions. Three independent reviewers (C.B., V.S., and D.G.) assessed PS position using a validated grading scale. Comparison of failure rates among cases grouped by selected baseline variables were performed using Pearson chi-square tests. Independent peri- and postoperative surveillance for local and general complications was performed to assess safety. Twenty-three patients with unstable thoracic fractures treated with 201 thoracic PSs were analyzed. Only PSs located between T-1 and T-12 were studied, with the majority of screws placed between T-5 and T-10. Of the 201 thoracic PSs, 133 (66.2%) were fully contained within the pedicle wall. The remaining 68 screws (33.8%) violated the pedicle wall. Of these, 36 (52.9%) were lateral, 27 (39.7%) were medial, and five (7.4%) were anterior perforations. No superior, inferior, anteromedial, or anterolateral perforations were found. When local anatomy and the clinical safety of screws were considered, 98.5% (198 of 201) of the screws were probably in an acceptable position. No baseline variables influenced the incidence of perforations. There were no adverse neurological, vascular, or visceral injuries detected intraoperatively or postoperatively. Conclusions: In the vast majority of cases, PSs can be placed in an acceptable and safe position by fellowship-trained spine surgeons when treating unstable thoracic spine fractures. However, an unacceptable screw position can occur.

Abstract:Abstract Background There have been few comparisons between dual positions, which require a position change, and a single position, which does not require position change, and it is not clear whether there is a difference in indirect decompression achieved by the two procedures. Therefore, the purpose of this study was to compare perioperative and radiographic outcomes following lateral lumbar interbody fusion (LLIF) in two cohorts of patients who underwent surgery in a single position or dual position. Methods This study involved 45 patients who underwent indirect decompression at 68 levels, with LLIF and percutaneous pedicle screw (PPS) fixation for lumbar degenerative spondylolisthesis with spinal canal stenosis. Patient demographics and perioperative data were compared between two groups: patients who remained in the lateral decubitus position for pedicle screw fixation (SP group) and those turned to the prone position (DP group). Results A total of 26 DP and 19 SP patients were analyzed. The operation time was approximately 31 min longer for the DP group (129.7 ± 36.0 min) than for the SP group (98.4 ± 41.3 min, P < 0.01). We also evaluated the pre- and postoperative image measurements, there was no significant difference for lumbar lordosis, segmental disc angle, slipping length, and disc height between the groups. The CSA of the dural sac (DP group, from 55.3 to 78.4 mm 2 ; SP group, from 54.7 to 77.2 mm 2 ) and central canal diameter (DP group, from 5.9 to 7.9 mm; SP group, from 5.6 to 7.7 mm) was significantly larger after surgery in both groups. However, there were no statistically significant differences between the two groups ( P = 0.684). Conclusions SP surgery could reduce the average surgery time by about 31 min. We found that the effect of indirect decompression by SP-PPS fixation following LLIF was considered to be a useful technique with no difference in dural sac enlargement or disc angle obtained compared with DP-PPS fixation.

Misplacement pattern of pedicle screws in pediatric patients with spinal deformity: a computed tomography study.

Abstract:Study Design: A retrospective study. Objective: To assess the accuracy and safety of the placement of pedicle screws (PSs) on pediatric patients with congenital scoliosis younger than 10 years through postoperative computed tomography scanning. Summary of Background Data: The accuracy of the placement of PSs in adolescent and adult population with various spinal problems has been thoroughly studied. As to pediatric patients with congenital scoliosis, the related studies were inadequate. Methods: A retrospective review was carried out on 96 pediatric patients with congenital scoliosis younger than 10 years, who received posterior instrumentation from February 2006 to June 2012. Postoperative computed tomography scan was utilized to assess the accuracy of PS placement. Pedicle perforations were classified as medial, lateral, or anterior and categorized into 1 of the 4 grades: grade 1, <= 2 mm; grade 2, 2.1-4.0 mm; grade 3, 4.1-6.0 mm; and grade 4, >= 6.1 mm. The positions of screws were also classified as acceptable (fully contained screws or with screws either <= 2 mm of medial wall perforation or <= 6 mm of lateral wall perforation and without impingement of visceral organs) or unacceptable. The variables of PS position as defined by fully contained were statistically examined using the Fisher (2-tailed) exact test with regard to: (1) side of the curvature (convex or concave); (2) region (thoracic or lumbar); (3) magnitude of the curvature (< 80 or >= 80 degrees); (4) apical region and overall; (5) deformed vertebrae versus normal vertebrae. Results: A total of 625 screws were inserted; 512 screws (81.9%) were fully contained within the cortical boundaries of the pedicle. Five hundred and sixty-nine (91.0%) screws were regarded as acceptably positioned, whereas 56 (9.0%) as unacceptably positioned. Among the 625 screws inserted, 317 were inserted in the thoracic spine, of which 67 (21.1%) led to perforations, and 308 screws were inserted in the lumbar spine, of which 46 (14.9%) led to perforations. The breach rate was found to be higher in thoracic spine than in the lumbar spine (P < 0.05). Of the 625 screws inserted, 306 screws were inserted on the concave side of the curve, of which 50 (16.3%) led to perforations, and 319 were inserted on the convex side, of which 63 (19.7%) led to perforations. There is no difference with regard to the breach rates between the concave side and the convex side of the curve (P > 0.05). Of the 625 screws, 328 were inserted in patients with a Cobb angle < 80 degrees, of which 41 (12.5%) screws perforated the pedicles, and 297 screws were inserted in the patients with a Cobb angle > 80 degrees, of which 72 (24.2%) screws perforated the pedicles. The breach rate was higher in the patients with the Cobb angle > 80 degrees than in the patients with the Cobb angle < 80 degrees (P < 0.05). One hundred and fifty-four screws were inserted in the apical vertebrae, of which 57 (37.1%) screws perforated the pedicle walls. The breach rate in apical vertebrae was higher than the overall (55/154 vs. 113/625) (P < 0.05). Among the 625 screws, 252 screws were inserted in the deformed vertebrae, of which 58 (23.0%) perforated the pedicle walls, and 373 screws were inserted in the normal vertebrae, of which 55 (14.7%) perforated the pedicle walls. The breach rate in the deformed vertebrae was higher than the normal vertebrae (P < 0.05). Conclusions: PSs insertion in pediatric patients with congenital scoliosis was less accurate than that in adult patients. However, the safety of placement was quite satisfactory. The possibility of pedicle breaching increased when placing PSs at deformed vertebra, on the apex of the curve, and in the thoracic spine, which reminds surgeons to be more cautious when placing PSs in these regions.
