Abstract:BACKGROUND:Although most patients with tetralogy of Fallot undergo radical repair during infancy and childhood, patients that remain undiagnosed and untreated until adulthood can still be treated. This study aimed to evaluate longterm outcomes of adult patients with tetralogy of Fallot who were treated surgically, and to determine the predictors of postoperative pulmonary regurgitation.METHODS:Fifty-six adult patients underwent complete surgical repair. Forty-three patients (76.8%) required a transannular patch. Systolic, diastolic, and mean pressure in the main pulmonary artery were measured after repair.RESULTS:The early mortality rate was 3.6%. The 16-year survival rate was (84.4 ± 11.5)%. Late echocardiography revealed 41 patients with transannular patch who had pulmonary regurgitation, consisting of mild pulmonary regurgitation in 28 patients, moderate in eight, and severe regurgitation in five patients. In addition, there was right ventricular outflow tract stenosis in nine patients, moderate/severe tricuspid valve regurgitation in seven, and residual ventricular septal defect in five. Logistic regression analysis demonstrated that the mean pulmonary pressure measured just after repair predicted late pulmonary regurgitation.CONCLUSIONS:The long-term survival of surgically treated adult patients with tetralogy of Fallot is acceptable. The mean pressure >20 mmHg in the main pulmonary artery measured right after surgical repair may be a feasible reference to time the reconstruction of the pulmonary valve.

Effect of Somatostatin in the Syrian Hamster Bearing a Transplantable Islet-Cell Tumor*

Abstract:The influence of somatostatin (SRIF) on blood glucose, plasma insulin and plasma glucagon was studied in hamsters bearing a transplantable islet-cell tumor secreting insulin and glucagon as well as in normal controls. Fed anesthetized animals were infused intraperitoneally either at a dose of 10 microgram in 15 min or of 150 microgram in 30 min, and intravenously at a dose of 250 microgram in 30 min. Blood was withdrawn from the jugular vein before and after infusion. Before the infusions, tumor bearing animals (TB) had lower blood glucose, markedly elevated plasma glucagon and slightly lower plasma insulin by comparison with normal hamsters (N). Both doses of somatostatin infused by the intraperitoneal route produced a slight but significant hypoglycemia in TB hamsters but not in normals. Ten microgram SRIF did not affect insulin and plasma glucagon levels whereas 150 microgram SRIF significantly depressed plasma insulin in both types of hamsters (N and TB). This latter dose of SRIF decreased plasma glucagon in normal but not in tumor-bearing hamsters. Intravenous infusion of 250 microgram SRIF did not reduce the hyperglucagonemia of TB hamsters either. These results indicate that somatostatin does not reduce the hyperglucagonemia due to the transplantable islet-cell tumor but nevertheless decreases blood glucose and plasma insulin.

Abstract:Objective To summarize the experience of surgical treatment of tetralogy of Fallot (TOF) in adult. Methods Sixty-three adult patients receiving total correction of TOF was analyzed retrospectively from May 1995 to Dec 2002. To use the vertical right ventricular incision to close the VSD (ventricular septal defect) with Dacron patch and relieve the RVOT (right ventricular outflow tract) obstruction, other abnormalities were corrected at the same time. Fourteen patients were enlarged the outflow chamber with a PTFE (polytetrafluoroethylene) patch simply, and others were constructed a transannular patch. Results Two patients died in hospital, the mortality was 3.17% , one case died of low cardiac output syndrome, one of bleeding after tracheotomy, others were all recovered with a follow-up ranging from 6 months to 7 years. Conclusion Adult patients of TOF were in long-term anoxic condition, and the hypertrophy and fibrosis of cardiac muscle do harm to the cardiac function before surgery. They were liable to severe low cardiac output syndrome after operation. To resects proper cardiac muscle when relieving RVOT. The VSD of adult TOF was large and the edge was crisp. Suturing the VSD discontinuously fixing with the long felt to prevent tearing the muscle. Adult TOF must receive surgical treatment earlier, and the effect of open correction is impressive in the near future and at a specified future date.

Abstract:Objective: To summarize the experience of perioperative treatment of TOF(tetralogy of Fallot) in adult. Methods: 82 adult patients receiving total correction of TOF from May 1995 to Dec 2002 were analysed. In adult TOF case the VSD was usually large and the edge was friable. Closing VSD with interrupted sutures,and with a long felt for preventing the tearing of the muscle was done. It is suggested that adult TOF must receive earlier surgical correction for achieveing better outcomes.

Abstract:Objective: To review the clinical experience in surgical treatment for Tetralogy of Fallot in adults over 15 years old. Methods:From October 1997 to January 2006,50 patients over 15 years old(15-57 years) underwent corrective repairment for Tetralogy of Fallot. Ventricular septal defects were repaired with Dacron patches in all patients. Forty-one cases were operated to widen the right ventricular outflow with patches and 9 cases underwent single repairment of right ventricular outflow. Results:One patient died from severe low cadiac output syndrome after operation. Heart functions and clinical manifestations were improved significantly in other patients. Conclusion:Proper surgical correction and careful perioperative management play an important role in the treatment of Tetralogy of Fallot in adults.

Abstract:Objective: To report the experience of surgical intervention for tetralogy of fallot(TOF). Method: A total of 34 patients with TOF,age 2～16 years,32 patients received the complete repair through mediam sternotomy under Moderate-hypothermic cardiopulmonary bypass.1 patient received modified Blalock-Taussing shunt without cardiopulmonary bypass,1 TOF patient with right ventricle severe hypoplasia received bidirectional Glenn shunt and complete repair.Results:One patient died of low cardiac output syndrome(LOS)in early postoperative period,noneof them died in later period.Conclusion:The complete repair of TOF should be performed as early as possible, TOF with pulmonary arterial hypoplasia should received modified Blalock-Taussing shunt,TOF with right ventricle severe hypoplasia should received bi-directional Glenn shunt and complete repair.The main cause of death is low cardiac output syndrome,it should be prevent by appropriate cardiac surgical procedures and other multiple therapies in perioperative period.

Abstract:Objective To summarize the experience of surgical treatment of tetralogy of Fallot (TOF) in adults. Methods Twenty-seven adult patients with TOF(18-47years old)received redical corrective repair of malformatcons.The operations were performed via the right atrium or right ventricle.The ventricular septal defects were closed by dacron patch in all patients,and right ventricular outflow tract obstruction was eliminated by dacron and pericardial patch in 21 cases and transannular patch in 5 cases. Results Two patients died during hospitalization, with a hospital mortality of 7.4%. Postoperative complications were found in 4 patients: lung infection in 1 case, reexploration for postoperative hemorrhage in 2 cases and reintubation 1 cases. Conclusion The total corrective repair of TOF may have an excellent result in adult patients.Complete correction of malformation and intensive perioperative management were the key factors for successful surgical treatment of TOF.

Abstract:ABSTRACT Large-scale storage of CO2 within sub-sea geological formations is a viable option for reducing the volume of this greenhouse gas released directly to the atmosphere from anthropogenic activities. Risks to benthic marine life following possible leakage of gas through the seabed from this carbon capture and storage (CCS) initiative are not yet well established. This study examined behavior (activity patterns) in brown shrimp (Crangon crangon), exposed to a range of reduced seawater pH conditions (7.6, 7, or 6.5) simulating leakage scenarios of varying scales. Brown shrimp have an endogenous rhythmicity associated with their activity, which dictates they are most active during hours of darkness, presumably as protection against vision-dependent predators. This endogenous rhythm in activity continues to be expressed when shrimp are held under constant low-light conditions in the lab and provides an ecologically relevant endpoint to measure when examining the influence of reduced pH on the behavior of these animals. No marked differences in activity pattern were observed between control shrimp maintained at pH 8.1 and those at pH 7.6. However, changes in activity were evident at pH 7 and pH 6.5, where significant shifts in timing and intensity of activity occurred. There was an unexpected increase in activity within periods of expected light, probably signaling efforts by shrimp to migrate away from reduced seawater pH conditions. The loss of this important member of the benthic community due to migration may have important consequences for many of the resilient species that remain.

Abstract:Tetralogy of Fallot with pulmonary atresia (ToF-PA) is a rare diagnosis that includes an extraordinarily heterogeneous group of complex anatomical findings with significant implications for physiology and prognosis. In addition to the classic findings of ToF, this particular diagnosis is characterized by complete failure of forward flow from the right ventricle to the pulmonary arterial system. As such, pulmonary blood flow is entirely dependent on shunting from the systemic circulation, most frequently via a patent ductus arteriosus, major aortopulmonary collaterals, or a combination of the two. The pathophysiology of ToF-PA is largely attributable to the abnormalities of the pulmonary vasculature. Ultimately, these patients require operative intervention to create a reliable, controlled source of pulmonary blood flow and ideally complete intracardiac repair. Even after operative correction, these patients remain at risk for pulmonary arterial stenoses and pulmonary hypertension. Although there have been significant advances in surgical and interventional management of ToF-PA leading to dramatic improvements in survival and long-term functional status, there is ongoing debate about the optimal management strategy given the risk of development of irreversible abnormalities of the pulmonary vasculature and the morbidity and mortality associated with sometimes multiple, complex operative interventions often occurring early in infancy. This review will discuss the findings in patients with ToF-PA with a focus on the perioperative and anesthetic management and will highlight challenges faced by the anesthesiologist in caring for these patients.
Pulmonary Valve Replacement in Primary Repair of Tetralogy of Fallot in Adult Patients

Abstract:Background Adults with unrepaired tetralogy of Fallot (ToF) are common in developing countries. Long-term overload of the right ventricle places adult patients at risk for postoperative right heart failure after primary repair, which contributes to morbidity and mortality. The effect of pulmonary valve replacement (PVR) in reducing postoperative morbidity and mortality in adults has never been validated. Methods We conducted a retrospective cohort study in adults (age ≥18 years) with ToF undergoing primary repair from January 2014 to December 2019 at our institution. Patients were divided into three groups according to techniques used to enlarge the right ventricle outflow tract (RVOT). Baseline variables and perioperative outcomes were collected. The primary endpoint was operative mortality. Secondary endpoints were incidences of right heart failure and stage 3 acute kidney injury (AKI). Results A total of 56 patients were enrolled (mean age 41.5±11.7 years, 30 females, 53.6%). They were divided into three groups designated as the following: TA-PVR group for trans-annular patch enlargement with PVR; TA group for trans-annulus patch enlargement without PVR; and group AP for annulus preservation. Four patients (7.1%) died postoperatively, all due to right heart failure. All twelve patients in the TA-PVR group survived. There was no significant difference in mortalities among groups. Ten patients (17.9%) developed right heart failure after surgery with no significant difference among groups. Three patients (5.4%) developed stage 3 AKI after surgery, none belonging to the TA-PVR group, however, not statistically significant. Conclusions Right heart failure is a common complication after primary repair of adult ToF. Trans-annulus patch enlargement should be cautiously selected in this population. PVR with trans-annulus patch enlargement may be a promising technique to protect against postoperative right heart failure and mortality when annulus preservation is not feasible.

Abstract:Animal models of facial nerve healing and regeneration are becoming increasingly important as new techniques of facial nerve repair and grafting are developed. We have described in detail the peripheral branching pattern of the rat facial nerve and performed axon counts on the main trunk and peripheral branches. Similarities and differences between the human and rat facial nerve are emphasized and the branches of the rat nerve amenable to experimental manipulation are identified. In addition, electrophysiologic data from the rat show a more diffuse pattern of innervation of the face from the individual branches than seen in humans.
Prevalence and optimal management strategy for aortic regurgitation in tetralogy of Fallot

Abstract:Objective: Aortic regurgitation (AR) in the tetralogy of Fallot (TOF) is not frequent, but when present it impacts significantly on surgical management. Furthermore, the incidence of late AR development has been increasing, along with surgical interest in current practices. Methods: Pre- and post-operative studies on 427 patients (TOF, 374; TOF/PA (TOF with pulmonary atresia), 53) who survived corrective operation were reviewed. AR (> or =mild) was detected in 28. Results: Nine had AR preoperatively, while 25 (including six with preoperative AR) exhibited AR post-operatively. In the 19 who developed AR post-operatively, the aortic root diameter (AoRoD) and indexed AoRoD (%AoRoD) were 42+/-11 mm and 166+/-36%, increased from the preoperative values of 30+/-10mm and 149+/-24%. AR-free rate at 20 years was 95.1% of all cases studied, 84.3 vs 96.5% in TOF/PA vs classic TOF (P<0.0001), and 82.2 vs 97.0% in bulboventricular VSD vs infracristal VSD (P<0.0001). Older age at repair, and bulboventricular VSD were identified as risk factors for the progression of AR. Aortic valvuloplasty (AVP; n=5) or replacement (AVR; n=4) was performed nine times in eight patients before (n=1), during (n=4), or late after TOF repair (n=4); all showed improvement of NYHA class. Survival- and reoperation-free survival curves showed no significant difference between patients with or without AR. Conclusions: After repair of TOF, careful observation for a late progression of AR is needed for the optimal timing of surgical intervention, especially in patients who repaired at higher age with a dilated aortic root or in patients with bulboventricular VSD.
Long-term Outcomes of Tetralogy of Fallot Repair: A 30-year experience with 960 patients

Abstract:Objective This study is to evaluate the long-term results of TOF repair and assess the risk factors for adverse outcomes. Methods This retrospective study involved 960 patients who underwent trans-atrial trans-pulmonary TOF repair between 1990 and 2020. Results Trans-annular patch (TAP) was placed in 722 patients and pulmonary valve preservation (PVP) was achieved in 233 patients. The median age at TOF repair was 9.4 (IQR, 6.2-14.2) months. The median follow-up duration was 10.6 (IQR, 5.4-16.3) years. There were 8 early deaths (0.8%) and 20 late deaths (2.1%). Genetic syndrome and pulmonary valve annulus Z score < -3 were risk factors for mortality. The survival rate was 97.7 % [95% CI: 96.4-98.5] and 94.5% [95% CI: 90.9-96.7] at 10 and 30 years, respectively. Freedom from any reoperation was 86.4% [95% CI: 83.6-88.7.] and 65.4% [95% CI:59.8-70.4] at 10 and 20 years. Post-operative right ventricular outflow tract (RVOT) peak gradient ≥ 25mmHg correlated with reoperation. Propensity score matched analysis demonstrated that freedom from pulmonary valve replacement (PVR) at 15 years was higher in patient with PVP compared to those with TAP (98.2% vs 78.4%, p = 0.004). Freedom form reoperation for RVOT obstruction at 15 years was lower in PVP group compared to TAP group (p = 0.006). Conclusions The long-term outcomes of TOF repair are excellent. A post-operative RVOT peak gradient less than 25 mmHg appears to be optimal to prevent reoperation. If pulmonary valve size is suitable, PVP reduced the risk of PVR, yet increased reoperation rate for RVOT obstruction.

Abstract:Objective To summarize and review the pathophysiology,diagnosis and management of tetralogy of Fallot(TOF) with absent pulmonary valve.Methods The clinical data of three cases with tetrology of Fallot with absent pulmonary valve who were diagnosed from Novermber 2007 to February 2009 were summarized,including clinical features,iconography characteristics,treatment and prognosis.Results These 3 patients included 1 boy and 2 girls,aged 39 d,73 d and 4 months respectively.All the cases had symptoms of respiratory distress and variable cyanosis,and physical examinations revealed a to-and-fro murmur at the upper and mid-left sternal borders.Both echocardiography and angiocardiogram showed a large subaortic ventricular septal defect with overriding of the aorta,distally displaced pulmonary annulus with valve stenosis and severe regurgitation,massive aneurysm of the pulmonary trunk and its branches.One patient was with isolated and hypoplastic left pulmonary artery,and none of them was with ductus arteriosus patent.Two patients with severe respiratory distress symptoms received corrective surgery,with good result in one and perioperative death in the other one with isolated and hypoplastic left pulmonary artery.The parents of the other mild symptomatic baby preferred to follow up.Conclusions The anomaly of TOF with absent pulmonary valve should be kept in mind when meeting those patients who had repeated and refractory respiratory infection and distress in their early life.Early corrective surgery was the only effective management for those symptomatic patients,and even an asymptomatic child should receive elective surgery in the first 6 months of life.The surgical mortality of them was much higher than that of the isolated TOF patients,especially in those patients with one pulmonary artery hypoplasia or isolated.

Abstract:Tetralogy of Fallot (TOF) is an important lesion for all pediatric and congenital heart surgeons. In designing the most appropriate operation for children with TOF, the postoperative physiology should be taken into account, both in the short and long term. The balance between pulmonary stenosis (PS) and pulmonary insufficiency (PI) may be critical for preservation of ventricular function. A unified repair strategy that limits both residual PS and PI is presented, along with supporting experimental evidence, a strategy for dealing with coronary anomalies, and comments regarding best timing of operation.

Abstract:Introduction: Modified Blalock-Taussig shunt (BT shunt) is a palliative operation used for cyanotic heart diseases with decreased pulmonary blood supply. The definitive management of tetralogy of Fallot (TOF) is total corrective surgery, but these patients can be palliated with BT shunt. In the modern world, the BT shunt is getting out of favour in patients with TOF. In this article, we will share our 5-year experience at our institute, which also shows a decreasing trend. Patients and methods: It is a retrospective study. Files of all the patients admitted in our department from January 2019 to December 2023 were reviewed. Age, weight, hospital stay, inotropic support duration, mechanical ventilation duration, and outcomes were studied. Results: From January 2019 to December 2023, 173 patients underwent BT shunt for TOF. The mean age was 31 months, and the mean weight was 9.3 kg. The overall mortality for BT shunt was 15% after BT shunt. Hypercyanotic spell not controlled by medical management was the most common indication for BT shunt in our setup. Most of the patients with hypercyanotic spells were also candidates for total correction but due to the emergency, BT shunt was performed. Conclusion: The role of BT shunt in patients with TOF is decreasing due to PDA/RVOT stenting, it is likely that the BT shunt in TOF will become a thing of the past in the future even in developing countries like ours.

Abstract:Objective:To analyze the operative time,operative methods,perioperative risk factors and management of tetralogy of Fallot(TOF) in infants. Method:The clinical data of 178 infants of TOF in Ward 2,Fu Wai Hospital in 2009 were analyzed retrospectively,including 98 boys and 80 girls,aged from 4 months to 8 years old, average 1.7 years.The 76 cases less than one year old,weight less than 10Kg in 102 cases,146 cases of surgery with median sternotomy,right lateral axillary incision in 32 cases,routine balance ultrafictration and modified ultrafictration in all patients.The 109 cases needed transannular patchs,including to enlarge the left pulmonary in 31 patients,underwent angiography and embolization in 37 patients one-step. Result:One case dead,the mortality was 0.58%.The main causes of death were serious low cardiac output syndrome and respiratory distress syndrome.The patients have been followed up for 6 months to 18 months.Six cases with remnant obstruction(>30 mmHg) had no symptom,three cases with small residual shunt. Conclusion:The results of primary repair of TOF in infants are very satisfactory.The key points in decreasing the surgical mortality lie in: the cardiac deformity being satisfied put right,the reasonable perioperative managements.The diagnosis and operation in TOF at an early date will be admittedly.

Abstract:Objectives: The strategies of repair of tetralogy of Fallot change with the age of patients. In children older than 4 years and adults, the optimal strategy may be to use different method of reconstruction of the right ventricular outflow tract from those followed in Younger children, so as to avoid, or reduce, the pulmonary insufficiency that is increasingly known to compromise right ventricular function. Methods: From April, 2001, through May, 2008, we undertook complete repair in 312 patients, 180 male and 132 female, with a mean age of 11.3 years +/- 0.4 years, and a range from 4 to 48 years, with typical clinical and morphological features of tetralogy of Fallot, including 42 patients with the ventriculo-arterial connection of double outlet right ventricle. The operation was performed under moderate hypothermia using blood cardioplegla. The ventricular septal defect was closed with a Dacron patch. When it was considered necessary to resect the Musculature within the right ventricular outflow tract, or perform pulmonary valvotomy, we Sought to preserve the function of the Pulmonary valve by protecting as far as possible the native leaflets, or creating a folded monocusp of autologous pericardium. Results: The repair was achieved completely through right atrium in 192, through the right ventricular Outflow tract in 83, and through the right atrium, the Outflow tract, and the pulmonary trunk in 36 patients. A transjunctional patch was inserted in 169 patients, non-valved in all but 9. There were no differences regarding the periods of aortic cross-clamping or cardiopulmonary bypass. Of the patients, 5 died (1.6%), with no influence noted for the transjunctional patch. Of those having a non-valved patch inserted, three-tenths had pulmonary regurgitation of various degree, while those having a valved patch had minimal pulmonary Insufficiency and good right ventricular function postoperatively, this being maintained after follow-up Of 8 to 24-months. Conclusions: Based on our experience, we suggest that the current strategy of repair of tetralogy of Fallot in older children and adults should be based on minimizing the insertion of transjunctional patches, this being indicated only in those with very small ventriculo-pulmonory junctions. If such a patch is necessary, then steps should be taken to preserve the function of the pulmonary valve.
