Abstract:Background The dental pulp's environment is essential for the regulation of mesenchymal stem cells' homeostasis and thus, it is of great importance to evaluate the materials used in regenerative procedures. Aim To assess in vitro (i) the effect of chitosan nanoparticles, 0.2% chitosan irrigation solution, Dual Rinse®, 17% EDTA, 10% citric acid and 2.5% NaOCl on DSCS viability; (ii) the effect of different concentrations of TGF‐β1 on DCSC proliferation; and (iii) whether treatment with TGF‐β1 following exposure to the different irrigation solutions could compensate for their negative effects. Methodology (i) DSCS were treated with three dilutions (1:10, 1:100 and 1:1000) of the six irrigation solutions prepared in DMEM for 10 and 60 min to assess the effect on viability. (ii) The effect of different concentrations (0, 1, 5 and 10 ng/mL) of TGF‐β1 on DCSC proliferation was assessed at 1, 3 and 7 days. (iii) The proliferative effect of TGF‐β1 following 10‐min exposure to 1:10 dilution of each irrigation solution was also tested. We used MTT assay to assess viability and proliferation. We performed statistical analysis using Prism software. Results (i) The different endodontic irrigation solutions tested showed a significant effect on cell viability (p ≤ .0001). Significant interactions between the endodontic irrigation solutions and their dilutions were also found for all parameters (p ≤ .0001). Chitosan nanoparticles and 0.2% chitosan irrigation solution were the least cytotoxic to DSCS whilst 2.5% NaOCl was the most cytotoxic followed by 17% EDTA. (ii) TGF‐β1 at concentrations of 1 and 5 ng/mL resulted in significantly higher proliferation compared to the control group. (iii) Exposure to 17% EDTA or 2.5% NaOCl for 10 min was sufficient to make DSCS cells refractory to the proliferative effects of TGF‐β1. DSCS groups treated with TGF‐β1 following exposure to chitosan nanoparticles, 0.2% chitosan irrigation solution, Dual Rinse® and 10% CA demonstrated significantly higher proliferation compared to non‐TGF‐β1‐treated groups (p ≤ .0001, p ≤ .0001, p ≤ .0001 and p = .01 respectively). Conclusions The current study offers data that can be implemented to improve the outcome of regenerative endodontic procedures by using less toxic irrigation solutions and adding TGF‐β1 to the treatment protocol.

In Vitro Efficacy of Apical Negative Pressure Irrigation in Primary Teeth: A Smear Layer Analysis

Abstract:Aim and background: This study aimed to assess the efficacy of root canal debris removal in primary teeth through irrigation using the Endo-Irrigator Plus (CWAIS). Materials and methods: A total of 25 primary mandibular first molars were divided into five groups, with five teeth in each group. After determining the working length and performing biomechanical preparation, groups I and II were subjected to hand and rotary instrumentation (ProTaper, Dentsply, United States of America) with conventional syringe irrigation with positive pressure, while groups III and IV were instrumented using hand and rotary instrumentation with positive and negative pressure irrigation employing the Endo-Irrigator Plus (CWAIS) system. Sodium hypochlorite (5.25%) and ethylenediaminetetraacetic acid (EDTA) (17%) were used as irrigants. Group V served as the control group, undergoing only root canal access opening. The removal of the smear layer was analyzed at the cervical, middle, and apical thirds of the canal using environmental scanning electron microscopy (SEM). Statistical significance was set at p < 0.05. Results: Statistically significant differences were observed among all groups at the cervical, middle, and apical levels (p < 0.05). Groups III and IV exhibited significantly lower smear scores at all levels, particularly in the apical third, as observed through SEM examination. Conclusion: The use of the Endo-Irrigator Plus (CWAIS) system demonstrated superior debridement of intracanal debris in primary teeth compared to the positive pressure irrigation method. The Effect of Rosmarinus Officinalis as a Potential Root Canal Medication on the Viability of Dental Pulp Stem Cells

Abstract:Aim: The objective of the current study was to assess and compare the impact of triple antibiotic paste (TAP) and calcium hydroxide (Ca(OH)2) with rosmarinic acid (RA) on the viability of dental pulp stem cells (DPSCs). Materials and methods: Dental pulp stem cells were isolated and characterized using flow cytometry. The cells were treated with (0.25, 0.5, 1, 2.5, and 5 mg/mL) concentrations for TAP and Ca(OH)2 and (6.25, 12.5, 25, 50, and 100 µM) concentrations for RA. Cell viability was evaluated after 3 days, with cell proliferation further analyzed over 3, 5, and 7 days utilizing the MTT assay. The optical density (OD) was quantified at 570 nm, subsequently enabling the determination of corrected OD and cell viability. ANOVA followed by the post hoc Tuckey test evaluated the statistical significance at p < 0.05. Results: Following the cell viability test, 0.25 and 0.5 mg/mL of TAP and Ca(OH)2 showed no significant difference for DPSCs compared to the control group. While dosages of 1 mg/mL, 2.5 mg/mL, and 5 mg/mL significantly reduced cell viability (p < 0.05). However, 6.25 µM and 12.5 µM concentrations of RA showed a significant increase in cell viability compared to untreated cells, 25 µM and 50 µM concentrations showed no significant difference compared to untreated cells while 100 µM concentration showed a decrease in cell viability (p < 0.05). Moreover, RA at a concentration of 12.5 µM exhibited a significant enhancement in cell proliferation rates after 5 and 7 days. Conclusion: Rosmarinic acid showed a significant increase in cell viability when used at 6.25 and 12.5 µM concentrations compared to TAP and CA(OH)2. Clinical significance: The assessment of cytotoxicity associated with bioactive compounds like RA, which processes antimicrobial and anti-inflammatory properties, holds importance. This evaluation could pave the way for novel intracanal medicaments that enhance the regenerative potential of DPSCs.

Abstract:This systematic review aims to analyse the available evidence concerning the use of citric acid (CA) in endodontics treatment and to assess its results in terms of different considerations: effect on smear layer removal, influence on sealer bond strength, activation effect by means of sonic or ultrasonic devices, effects on dentine surface, antibacterial activity, and effectiveness boost for regenerative procedures, releasing growth factors from dentin. To evaluate the results of CA as a final irrigant and compare them to other chelating agents. This review followed the PRISMA checklist. An electronic search was conducted in MEDLINE (OVID), Scopus (Elsevier) and the Web of Science (Thomson Reuters) databases. Risk of bias of included studies was evaluated using the modified CONSORT checklist and the PRIRATE checklist 2020 guidelines. 39 studies fulfilled the eligibility criteria to be included in this review: 27 in vitro studies using extracted human teeth, 10 in vitro studies using human dentin disks, and 2 RCT. Citric acid has proven to be effective in smear layer removal, showing better results in coronal and middle root thirds, improving its effect when combined with manual dynamic activation. There is no agreement regarding citric acid effect on sealer adhesion and adaptation to root canal walls due to heterogeneity within studies. Citric acid irrigation can decrease dentine microhardness and cause decalcification and erosion, especially when used before NaOCl. Citric acid has proven to be beneficial in regenerative endodontic procedures due to higher TGF-β1 release.

Abstract:Aim: To study the effect of glycyrrhizin (GA) on the viability and proliferation of dental pulp stem cells (DPSCs) compared with intracanal medicaments. Materials and methods: Third molars of an adult donor were used to obtain the DPSCs. Flow cytometry was utilized to conduct phenotypic analysis for DPSCs. The methyl-thiazol tetrazolium (MTT) test was used to detect the cell viability. Cell proliferation assay was conducted at distinct time intervals: 3, 5, and 7 days. Results: The flow cytometry analysis verified the positive expression of mesenchymal cell surface antigen molecules (CD73, CD90, and CD105) and the absence of hematological markers (CD14, CD34, and CD45) in the DPSCs. The cells that treated with concentrations more than 0.5 mg/mL of Ca(OH2) and triple antibiotic paste (TAP) gave significant decrease in viability in comparison to the untreated cells (p < 0.05). Also, the cells treated with concentrations 50 and 25 µM of GA showed no significant difference compared with the untreated cells (p > 0.05), while concentrations 12.5 and 6.25 µM expressed a significant increase in viability compared with the untreated cells (p < 0.05). At 7 days, cells treated with the three different concentrations of GA (12.5, 25, and 50 µM) demonstrated a significant increase in cell density compared with Ca(OH)2 and TAP-treated cells (p < 0.05). Conclusion: Based upon the potential of GA on DPSCs proliferation compared with Ca(OH)2 and TAP, It is conceivable to acknowledge that GA could be used as an intracanal medicaments for revascularization process of necrotic immature teeth. Clinical significance: This study emphasizes the significance of assessing alternative root canal medicaments and their impact on the proliferation and viability of DPSCs. The results regarding GA, specifically its impact on the viability and growth of DPSCs, provide essential understanding for its potential application as an intracanal medicine. This study adds to the continuous endeavors in identifying safer and more efficient intracanal therapies, which are essential for improving patient outcomes in endodontic operations. Abstract:Thin films of hydrogenated and unhydrogenated titanium carbide, known to be amorphous from x-ray and electron diffraction studies, nevertheless exhibit short-range order at the level of the first-nearest-neighbor shell when studied by extended x-ray-absorption fine structure and extended electron-energy-loss fine structure. This short-range order appears to consist of the same octahedrally coordinated units present in crystalline titanium carbide (NaCl structure), where a titanium atom is surrounded by an octahedron of six carbon atoms and vice versa. The persistence of this basic structural unit is attributed to the difference in atomic radii between C and Ti and to the strong nature of the Ti-C bond. A slight relaxation in interatomic distance relative to the crystalline state, which increases upon hydrogenation, is also observed. The atomic positions in the first coordination shell in amorphous titanium carbide are slightly displaced relative to the crystalline form. Yet, upon hydrogenation, this displacement decreases in the first shell, made of carbon atoms and vacancies, surrounding a Ti atom by 40% for the 4.5 at. % H and 60% for the 6 at. % H relative to the unhydrogenated amorphous ${\mathrm{TiC}}_{\mathrm{x}}$ films. By contrast, the structural disorder in the first shell of Ti atoms surrounding a C atom is practically unaffected by the addition of hydrogen. These results imply that the hydrogen atoms, instead of occupying tetrahedral vacancies, fill carbon vacancies in the amorphous phase as they do in crystalline ${\mathrm{TiC}}_{\mathrm{x}}$, in agreement with our previous findings from static secondary-ion mass spectroscopy and x-ray photoelectron spectroscopy.
Engineering a Microphysiological Model for Regenerative Endodontic Studies

Abstract:Dental pulp infections are common buccal diseases. When this happens, endodontic treatments are needed to disinfect and prepare the root canal for subsequent procedures. However, the lack of suitable in vitro models representing the anatomy of an immature root canal hinders research on regenerative events crucial in endodontics, such as regenerative procedures. This study aimed to develop a 3D microphysiological system (MPS) to mimic an immature root canal and assess the cytotoxicity of various irrigating solutions on stem cells. Utilizing the Dental Stem Cells SV40 (DSCS) cell line derived from human apical papilla stem cells, we analyzed the effects of different irrigants, including etidronic acid. The results indicated that irrigating solutions diminished cell viability in 2D cultures and influenced cell adhesion within the microphysiological device. Notably, in our 3D studies in the MPS, 17% EDTA and 9% 1-hydroxyethylidene-1, 1-bisphosphonate (HEBP) irrigating solutions demonstrated superior outcomes in terms of DSCS viability and adherence compared to the control. This study highlights the utility of the developed MPS for translational studies in root canal treatments and suggests comparable efficacy between 9% HEBP and 17% EDTA irrigating solutions, offering potential alternatives for clinical applications.

Progress of Research on the Application of Triple Antibiotic Paste and Hydrogel Scaffold Materials in Endodontic Revascularization: A Systematic Review

Abstract:Objective. To evaluate the application of hydrogel scaffold materials and triple antibiotic paste in endodontic regeneration through literature review. Methods. An electronic search of the literature published on PubMed, Wangfang database, and CNKI database using the search terms "endodontic regeneration," "pulp blood flow reconstruction," "recanalization," "triple antibiotic paste," and "scaffold material" was conducted. The searched literature was used for analysis. Results and Conclusion. Hydrogels regulate stem cell fates, modulate growth factor release, and encapsulate antibacterial and anti-inflammatory drugs. The triple antibiotic paste is composed of metronidazole, ciprofloxacin, and minocycline, which exhibits promising antibacterial effects and duration at appropriate concentrations, with low cytotoxicity, and effectively promotes the preservation and regeneration of pulp tissues and the formation of dental hard tissues. However, issues such as tooth discoloration and bacterial drug resistance also exist. The present article reviews the progress of research on the application of hydrogel scaffold materials and triple antibiotic paste in endodontic revascularization.

Impact of Irrigation Solutions on the Apical Sealing Potential of Different Endodontic Sealers Used with the Continuous-wave Obturation Technique: An In vitro Study

Abstract:Abstract Aim: This study is aimed to evaluate the impact of three irrigating solutions on the apical sealing ability of three endodontic sealers. Materials and Methods: One hundred maxillary central incisors were decoronated, and their roots were assigned into three experimental groups (n = 30 each) according to the nature of the irrigating solution and two control groups (n = 5 each). Each experimental group was divided equally into three subgroups (n = 10 each) according to the type of root canal sealer. All root canals were instrumented using ProTaper Universal rotary system and irrigated with 3% sodium hypochlorite (NaOCl/Group 1), 3% hydrogen peroxide (H2O2/Group 2), and 2% chlorhexidine gluconate (CHX/Group 3). The final irrigating solutions for all groups were 17% ethylenediaminetetraacetic acid (EDTA) and distilled water. The continuous-wave technique was used to fill the prepared canals with the following materials: MTA-Fillapex/Gutta-percha (Subgroup A), AH Plus/Gutta-percha (Subgroup B), and Realseal SE/Realseal (Subgroup C). The microleakage was assessed after 72 hours using the linear dye penetration method. The results were statistically analyzed with one-way analysis of variance. Results: Significant differences were found only between the experimental groups (P < 0.05). All root canal sealers after irrigation with 3% H2O2 showed the lowest amount of leakage. The highest apical leakage was observed when 3% NaOCl and 2% CHX were used. Conclusions: The type of irrigating solution could affect the apical sealing ability of experimental sealers used with the continuous-wave obturation technique. Root canal irrigation with 3% H2O2 +17% EDTA increased the apical sealing ability of all root canal sealers. Root canal irrigation with 3% NaOCl or 2% CHX +17% EDTA decreased the apical sealing ability of all root canal sealers.
Assessing the Impact of Stem Cell-based Therapy on Periodontal Health: A Meta-analysis of Clinical Studies

Abstract:Objective:: While clinical trials exploring stem cells for regenerating periodontal tissues have demonstrated positive results, there is a limited availability of systematic literature reviews on this subject. To gain a more comprehensive understanding of stem cell interventions in periodontal regeneration, this meta-analysis is undertaken to assess the beneficial effects of stem cells in human periodontal regeneration. Methods:: "PubMed," "Cochrane Library," "Web of Science," "Embase," "Wanfang," and "CNKI," were used to extract clinical studies related to the utilization of stem cells in repairing periodontal tissue defects. This search included studies published up until October 5, 2023. The inclusion criteria required the studies to compare the efficacy of stem cell-based therapy with stem cell-free therapy for regenerating periodontal tissues. Meta-analysis was conducted using Review Manager software (version 5.4). Results:: This meta-analysis synthesized findings from 15 selected studies investigating the impact of stem cell interventions on periodontal tissue regeneration. The "stem cell" group displayed a substantial reduction in clinical attachment level (CAL) compared to the "control" group within 3 to 12 months post-surgery. However, no significant differences in CAL gain were found between groups. Probing pocket depth (PPD) significantly decreased in the "stem cell" group compared to the "control" group, particularly for follow-up periods exceeding 6 months, and dental stem cell treatment exhibited notable improvements. Conversely, no significant differences were observed in PPD reduction. Gingival recession (GR) significantly decreased in the "stem cell" group compared to the "control" group at 3 to 12 months post-surgery. No significant differences were observed in GR reduction between groups. No significant differences were identified in cementoenamel junction-bone distance reduction, infrabony defect reduction, or bone mineral density increase between the two groups. Furthermore, no significant changes were observed in the gingival index, plaque index, or width of keratinized gingiva. Conclusion:: In conclusion, while stem cell-based therapy offers promising prospects for periodontal defect treatment, there are notable limitations in the current body of research. Larger, multicenter, double-blind RCTs with robust methodologies are needed to provide more reliable evidence for stem cell-based intervention in periodontitis.

Abstract:Statement of the problem: During endodontic therapy, irrigation solutions applied in the root canal may affect the physicochemical properties of the dentinal wall, thereby changing its microhardness. This may adversely affect the sealing ability and adhesion of dental materials. Therefore, many studies have focused on the search for an ideal root canal irrigant that has a minimal effect on dentinal microhardness. Purpose: This in vitro study was conducted to determine the changes in dentin microhardness after root canal irrigation with different endodontic irrigants. Materials and method: Ninety-five freshly extracted maxillary central incisor teeth with straight single canals were selected. These teeth were sectioned transversely at the level of the cementoenamel junction. The working length of each tooth was determined, and canal space was prepared by the HyFlex CM rotary file system. During instrumentation, normal saline was used for irrigation. Then, teeth were split longitudinally into two segments. According to the irrigating solution employed, samples were divided into five groups (n=19): normal saline (Group A), 3% sodium hypochlorite (Group B), 2% chlorhexidine (Group C), 5% calcium hypochlorite (Group D), and 0.2% nanochitosan (Group E).3 mL of the corresponding irrigating solution was administered for total15 minutes in each prepared sample. The Vickers micro-hardness tester was then used to assess micro-hardness. The data was analyzed using one-way analysis of variance (ANOVA). Results: All tested irrigating solutions decreased the dentinal microhardness. Samples irrigated with 5% calcium hypochlorite demonstrated dentinal microhardness of 42.43±1.62, which is the lowest among all the tested groups, followed by nano chitosan, sodium hypochlorite, and chlorhexidine. Samples treated with control group (saline) demonstrated the maximum microhardness of dentin in the present study. Conclusion: Within the limitations of this research, it can be concluded that the tested novel irrigating solutions, 5% calcium hypochlorite and 0.2% nanochitosan, were more detrimental to radicular dentin microhardness when compared with conventional endodontic irrigants.
Effectiveness of sodium hypochlorite gel and solutions in endodontics: A systematic review

Abstract:ABSTRACT Aim: The purpose of this systematic review was to evaluate the superiority of sodium hypochlorite (NaOCl) liquid or NaOCl gel in disinfecting root canals. Methods: Online electronic databases such as PubMed-MEDLINE, Scopus, and Embase were explored for suitable studies. In this evaluation, papers were deemed eligible for the data extraction if they included in vitro experiments using NaOCl liquid or NaOCl gel for canal irrigation and its influence on the smear layer, as well as their elimination of the bacteria from the root canal. The review was registered in International Prospective Register of Systematic Reviews with registration number CRD420223441126. Results: One hundred and sixty-one articles were found after initial screening and four articles were included for qualitative synthesis. Overall risk of bias was “high.” Two studies did not show significant variations among 2.5% NaOCl solution and gel. Conclusion: As per rigorous investigation done, NaOCl solution proved more efficient against Enterococcus faecalis than its gel form. More research is needed to determine the best effective NaOCl type, concentration, duration, and treatment regimens.

Abstract:This study of the fine structure of the neuroglia in the spinal cord of the newt, Triturus viridescens, has permitted the identification of three types of glial elements. One type is the ependymal cell whose processes are rich in filaments and glycogen and end on the pia in the deeper portion of the fissures of the cord. The two other types of glial cells display some morphological similarities to typical mammalian neuroglia although they appear to be more primitive. These are the astrocytes and oligodendrocytes. The astrocytes contain glial filaments, glycogen, specialized mitochondria, few ribosomes, and probably send end feet to the surface of the cord and to the intramedullary vessels. The oligodendrocytes present some variation in morphology but always exhibit an abundance of ribosomes. They are located either near the neurons, the intramedullary vessels, the pia, or the myelin sheath.

Abstract:Aim: The aim of this study was to compare the effect of three irrigating solutions on the apical sealing ability of three endodontic sealers when the continuous wave obturation technique was used.Materials and Methods: 100 maxillary central incisors were decoronated and roots were divided into three experimental groups (n=30 each) according to the type of irrigating solution, and two control groups (n=5 each). Each experimental group was divided equally into three subgroups according to the type of obturation system. All canals were instrumented using ProTaper Universal rotary system and irrigated with 3% sodium hypochlorite (Group 1), 2% chlorohexidine (Group 2), and 3% hydrogen peroxide (Group 3), followed by 17% EDTA and distilled water. Continuous wave technique was used to obturate root canals with the following combinations: MTA Fillapex/Gutta-percha (Subgroup A), AH Plus/Gutta-percha (Subgroup B) and Realseal SE/Realseal (Subgroup C). The apical microleakage was assessed using linear dye penetration method and the data were statistically analyzed. Results: Significant differences were found between all groups (P<0.05). All root canal sealers after irrigation with 3% hydrogen peroxide showed the lowest amounts of apical leakage. Samples irrigated with 3% sodium hypochlorite or 2% chlorohexidine showed the highest amounts of apical leakage. Conclusions: The type of irrigating solution could affect the apical sealing ability of experimental sealers when used with the continuous wave obturation technique. Root canal irrigation with 3% hydrogen peroxide + 17% EDTA increased the sealing ability of all root canal sealers. Root canal irrigation with 3% sodium hypochlorite 2% chlorohexidine + 17% EDTA decreased the sealing ability of all root canal sealers.
Evolution of regenerative endodontic in young permanent teeth: a literature review

Abstract:Objective: to compare different protocols available for regenerative endodontic. Methodology: articles published between 2001 and 2021 were searched on the database PubMed using the following keywords: “pulp revascularization” or “pulp revitalization” or “apexification”. Inclusion criteria were studies on humans, patients with immature permanent teeth with necrosis or irreversible pulpitis, a follow-up of at least 12 months, clearly described protocols and in English language. Results: The search produced 944 results. After applying the inclusion and exclusion criteria, only 35 were selected. 11 articles were added following a bibliographic review of the selected articles and a total of 46 cases were included. The success rate outcome for regenerative endodontic was of 80% observed in a total of 278 teeth. During irrigation, NaOCL was used in most of the cases with concentration varying from 0.5% to 6% and TAP has been the most used intracanal medication (32.7%). Periapical healing and resolution of the symptoms were observed in 95.7%, while apexogenesis was observed in 80.2%. Increase in root length reach 60%, root thickness 64.8% and apical closure 61,9%. Pulp vitality was found in 7.2% of the cases and discoloration in 29.5%. Conclusion: Regenerative endodontic showed signs of apexogenesis in most cases, and a pronounced increase in root thickness, root length and closure of the apex. Due to a high number of protocols and their heterogenicity, the superiority of one over the others cannot be established. More randomized clinical trials are needed to determine the most effective protocol.

Abstract:Objective: To explore the feasibility of injectable platelet-rich fibrin (i-PRF) in regenerative endodontics by comparing the effect of i-PRF and platelet-rich fibrin (PRF) on the biological behavior and angiogenesis of human stem cells from the apical papilla (SCAPs). Methodology: i-PRF and PRF were obtained from venous blood by two different centrifugation methods, followed by hematoxylin-eosin (HE) staining and scanning electron microscopy (SEM). Enzyme-linked immunosorbent assay (ELISA) was conducted to quantify the growth factors. SCAPs were cultured with different concentrations of i-PRF extract (i-PRFe) and PRF extract (PRFe), and the optimal concentrations were selected using the Cell Counting Kit-8 (CCK-8) assay. The cell proliferation and migration potentials of SCAPs were then observed using the CCK-8 and Transwell assays. Mineralization ability was detected by alizarin red staining (ARS), and angiogenesis ability was detected by tube formation assay. Real-time quantitative polymerase chain reaction (RT-qPCR) was performed to evaluate the expression of genes related to mineralization and angiogenesis. The data were subjected to statistical analysis. Results: i-PRF and PRF showed a similar three-dimensional fibrin structure, while i-PRF released a higher concentration of growth factors than PRF ( P <.05). 1/4× i-PRFe and 1/4× PRFe were selected as the optimal concentrations. The cell proliferation rate of the i-PRFe group was higher than that of the PRFe group ( P <.05), while no statistical difference was observed between them in terms of cell mitigation ( P >.05). More importantly, our results showed that i-PRFe had a stronger effect on SCAPs than PRFe in facilitating mineralization and angiogenesis, with the consistent result of RT-qPCR ( P <.05). Conclusion: This study revealed that i-PRF released a higher concentration of growth factors and was superior to PRF in promoting proliferation, mineralization and angiogenesis of SCAPs, which indicates that i-PRF could be a promising biological scaffold for application in pulp regeneration.
