Abstract:RATIONALE AND OBJECTIVES:Identification of muscle-invasive status (MIS) of bladder cancer (BCa) is critical for treatment decisions. The Vesical Imaging-Reporting and Data System (VI-RADS) has been widely used in preoperatively predicting MIS using tri-parametric MR imaging including T2-weighted (T2W), diffusion-weighted (DW), and dynamic contrast-enhanced (DCE) sequences. While the diagnostic values of radiomics features from bi-parametric MRI such as T2W + DW to identification of MIS have been reported, whether the tri-parametric MRI could provide additional diagnostic value to the radiomics prediction task, and how to integrate DCE features into the radiomics model, which is the objectives of this study, remain unknown.MATERIALS AND METHODS:Patients with postoperatively confirmed BCa lesions (150 in non-muscle-invasive BCa and 56 in muscle-invasive BCa groups) were retrospectively included. Their T2W, DW with apparent diffusion coefficient (ADC) maps, and DCE sequences were acquired using a 3.0T MR system. Regions of interest were manually depicted and VI-RADS scores were assessed by three radiologists. Three predictive models were developed by the radiomics features extracted from sequence combinations of T2W + DW (Model one), T2W + DCE (Model two), and T2W + DW + DCE (Model three), respectively, using the least absolute shrinkage and selection operator. The performance of each model was quantitatively assessed on both the training (n = 165) and testing (n = 41) cohorts. Then a 10 times five-fold cross validation was conducted to assess the overall performance.RESULTS:Three models achieved area under the curve of 0.888, 0.869, and 0.901 in the cross validation, respectively. The tri-parametric model performed significantly superior than the two bi-parametric models and VI-RADS. The analysis of feature coefficients derived from least absolute shrinkage and selection operator algorithm showed features from the tri-parametric MRI are effective in MIS discrimination.CONCLUSION:The tri-parametric MRI provides additional value to the radiomics-based identification of MIS.

Elaboration of a Multisequence MRI-based Radiomics Signature for the Preoperative Prediction of the Muscle-Invasive Status of Bladder Cancer: a Double-Center Study

Abstract:To develop a multisequence MRI-based radiomics signature for the preoperative prediction of the muscle-invasive status of bladder cancer (BCa). This retrospective study involved 106 eligible patients from two independent clinical centers. All patients underwent a preoperative 3.0 T MRI scan with T2-weighted image (T2WI) and multi-b-value diffusion-weighted image (DWI) sequences. In total, 1404 radiomics features were extracted from the largest region of the reported tumor locations on the T2WI, DWI, and corresponding apparent diffusion coefficient map (ADC) of each patient. A radiomics signature, namely the Radscore, was then generated using the recursive feature elimination approach and a logistic regression algorithm in a training cohort (n = 64). Its performance was then validated in an independent validation cohort (n = 42). The primary imaging and clinical factors in conjunction with the Radscore were used to determine whether the performance could be further improved. The Radscore, generated by 36 selected radiomics features, demonstrated a favorable ability to predict muscle-invasive BCa status in both the training (AUC 0.880) and validation (AUC 0.813) cohorts. Subsequently, integrating the two independent predictors (including the Radscore and MRI-determined tumor stalk) into a nomogram exhibited more favorable discriminatory performance, with the AUC improved to 0.924 and 0.877 in both cohorts, respectively. The proposed multisequence MRI-based radiomics signature alone could be an effective tool for quantitative prediction of muscle-invasive status of BCa. Integrating the Radscore with MRI-determined tumor stalk could further improve the discriminatory power, realizing more accurate prediction of nonmuscle-invasive and muscle-invasive BCa. • DWI is superior to T2WI sequence in reflecting the heterogeneous differences between NMIBC and MIBC, and multisequence MRI helps in the preoperative prediction of muscle-invasive status of BCa. • Co-occurrence (CM), run-length matrix (RLM), and gray-level size zone matrix (GLSZM) features were the favorable feature categories for the prediction of muscle-invasive status of BCa. • The Radscore (proposed multisequence MRI-based radiomics signature) helps predict preoperatively muscle invasion. Combination with the MRI-determined tumor stalk further improves prediction.

Abstract:Background Microvascular invasion (MVI) has been shown to be closely associated with postoperative recurrence and metastasis in patients with intrahepatic cholangiocarcinoma (ICC). We aimed to develop a radiomics prediction model based on contrast-enhanced CT (CECT) to distinguish MVI in patients with mass-forming ICC. Methods 157 patients were included and randomly divided into training (n=110) and test (n=47) datasets. Radiomic signatures were built based on the recursive feature elimination support vector machine (Rfe-SVM) algorithm. Significant clinical-radiologic factors were screened, and a clinical model was built by multivariate logistic regression. A nomogram was developed by integrating radiomics signature and the significant clinical risk factors. Results The portal phase image radiomics signature with 6 features was constructed and provided an area under the receiver operating characteristic curve (AUC) of 0.804 in the training and 0.769 in the test datasets. Three significant predictors, including satellite nodules (odds ratio [OR]=13.73), arterial hypo-enhancement (OR=4.31), and tumor contour (OR=4.99), were identified by multivariate analysis. The clinical model using these predictors exhibited an AUC of 0.822 in the training and 0.756 in the test datasets. The nomogram combining significant clinical factors and radiomics signature achieved satisfactory prediction efficacy, showing an AUC of 0.886 in the training and 0.80 in the test datasets. Conclusions Both CECT radiomics analysis and radiologic factors have the potential for MVI prediction in mass-forming ICC patients. The nomogram can further improve the prediction efficacy.

Abstract:Abstract Background: Bladder cancer is characterized by its frequent recurrence and progression. Effective treatment strategies need to be based on an accurate risk stratification, in which muscle invasiveness and tumor grade represent the two most important factors. Traditional imaging techniques provide preliminary information about muscle invasiveness but are lacking in terms of accuracy. Although as the gold standard, pathological biopsy is only available after the surgery and cannot be performed longitudinally for long-term surveillance. Methods: In this work, we developed a microfluidic approach that interrogates circulating tumor cells (CTCs) in the peripheral blood of bladder cancer patients to reflect the risk stratification of the disease. Results：In a cohort of 48 bladder cancer patients comprising 33 non-muscle invasive bladder cancer (NMIBC) cases and 15 muscle invasive bladder cancer (MIBC) cases, the CTC count was found to be considerably higher in the MIBC group compared with the NMIBC group (4.67 vs. 1.88 CTCs/3 mL, P=0.019), and was significantly higher in high-grade bladder cancer patients verses low-grade bladder cancer patients (3.69 vs. 1.18 CTCs/3mL, P=0.024). Conclusions: This microfluidic assay of CTCs is believed to be a promising complementary tool for the risk stratification of bladder cancer.Trial registration: This research was conducted under the approval of the Ethics Committee of the First Affiliated Hospital at Zhejiang University School of Medicine with the Registration No. 2015-218.

Abstract:AIM: To develop a radiomics model to predict the muscle-invasive status of bladder cancer (BC) in contrast-enhanced computed tomography (CECT) images, compared with radiologists??? interpretations. MATERIALS AND METHODS: One hundred and eighty-eight CECT images with histopathologically confirmed BC were retrieved retrospectively from November 2018 to December 2019 and were divided randomly into the training (n=120) and test dataset (n=68). The BC were annotated manually and validated on the venous phase by a general radiologist and an experienced radiologist, respectively. The radiomics analysis included radiomics feature extraction and model development. The same images were also evaluated by two radiologists. The diagnostic performance of radiomics was evaluated using receiver operating characteristic (ROC) curve analysis and the area under the ROC curve (AUC), sensitivity, and specificity were calculated. The predictive performance of radiomics was then compared to visual assessments of the two radiologists. RESULTS: The radiomics model reached an AUC (95% confidence interval [CI]) of 0.979 (0.935 -0.996) and 0.894 (0.796-0.956) in the training and test dataset, respectively. The radiomics model outperformed the visual assessment of radiologist A and B both in the training (0.865 [0.791-0.921], 0.894 [0.824-0.943]) and test dataset (0.766 [0.647-0.860], 0.826 [0.715 -0.907]). Pairwise comparisons showed that the specificities of the radiomics model were higher than the radiologists (85.3-96.7% versus 47.1-58.3%, all p 0.05), but the sensitivities were comparable between the radiomics and the radiologists (79.4-90% versus 91.2-96.7%; all p 0.05). CONCLUSIONS: A radiomics model was developed that outperformed the radiologists??? visual assessment in predicting the muscle-invasive status of BC in the venous phase of CT images. ?? 2022 The Royal College of Radiologists. Abstract:AbstractObjectivesThis study investigated the feasibility of a computed tomography (CT)-based radiomics prediction model to evaluate muscle invasive status in bladder cancer.MethodsPatients who underwent CT urography at two medical centers from October 2014 to May 2020 and had bladder urothelial carcinoma confirmed by postoperative histopathology were retrospectively enrolled. In total, 441 cases were collected and randomized into a training cohort (n = 293), an internal testing cohort (n = 73), and an external testing cohort (n = 75). The images were first filtered, and then, 1218 features were extracted. The best features related to muscle invasiveness of bladder cancer were identified by ANOVA. A prediction model was built by using the logistic regression method. Statistical analysis was performed by plotting the receiver operating characteristic curve. Indicators of the diagnostic performance of the prediction model, including sensitivity, specificity, accuracy, and area under curve (AUC), were evaluated.ResultsIn the training, internal testing, and external testing cohorts, the prediction model diagnosed muscle-invasive bladder cancer with AUCs of 0.885 (95% confidence interval [95% CI] 0.841–0.929), 0.820 (95% CI 0.698–0.941), and 0.784 (95% CI 0.674–0.893), respectively. In the internal testing cohort, the sensitivity, specificity, and accuracy of the model were 0.667 (95% CI 0.387–0.870), 0.845 (95% CI 0.721–0.922), and 0.782 (95% CI 0.729–0.827), respectively. In the external testing cohort, the sensitivity, specificity, and accuracy of the model were 0.742 (95% CI 0.551–0.873), 0.750 (95% CI 0.594–0.863), and 0.782 (95% CI 0.729–0.827), respectively.ConclusionsCT-based radiomics prediction model can evaluate muscle invasiveness of bladder cancer before surgery with a good diagnostic performance.Key Points• CT-based radiomics model can evaluate muscle invasive status in bladder cancer.• The radiomics model shows good diagnostic performance to differentiate muscle-invasive bladder cancer from non-muscle-invasive bladder cancer.• This preoperative CT-based prediction method might complement MR evaluation of bladder cancer and supplement biopsy.

Abstract:Background and Objectives Radiomics and deep learning are two popular technologies used to develop computer-aided detection and diagnosis schemes for analyzing medical images. This study aimed to compare the effectiveness of radiomics, single-task deep learning (DL) and multi-task DL methods in predicting muscle-invasive bladder cancer (MIBC) status based on T2-weighted imaging (T2WI). Methods A total of 121 tumours (93 for training, from Centre 1; 28 for testing, from Centre 2) were included. MIBC was confirmed with pathological examination. A radiomics model, a single-task model, and a multi-task model based on T2WI were constructed in the training cohort with five-fold cross-validation, and validation was conducted in the external test cohort. Receiver operating characteristic (ROC) curve analysis was performed to evaluate the diagnostic performance of each model. DeLong's test and a permutation test were used to compare the performance of the models. Results The area under the ROC curve (AUC) values of the radiomics, single-task and multi-task models in the training cohort were: 0.920, 0.933 and 0.932, respectively; and were 0.844, 0.884 and 0.932, respectively, in the test cohort. The multi-task model achieved better performance in the test cohort than did the other models. No statistically significant differences in AUC values and Kappa coefficients were observed between pairwise models, in either the training or test cohorts. According to the Grad-CAM feature visualization results, the multi-task model focused more on the diseased tissue area in some samples of the test cohort compared with the single-task model. Conclusions The T2WI-based radiomics, single-task, and multi-task models all exhibited good diagnostic performance in preoperatively predicting MIBC, in which the multi-task model had the best diagnostic performance. Compared with the radiomics method, our multi-task DL method had the advantage of saving time and effort. Compared with the single-task DL method, our multi-task DL method had the advantage of being more lesion-focused and more reliable for clinical reference.

Abstract:Purpose: To develop predictive models based on the integration of radiomics with the Vesical Imaging-Reporting and Data System (VIRADS) for determining muscle invasion of bladder cancer. Materials and Methods: One hundred ninety-one patients were retrospectively included in this study from January 2015 to March 2022. Of these, 121 data were randomly divided into training and validation sets at a ratio of 7:3. The remaining data (n = 70) served as the independent testing set. The radiomics features were extracted from bladder cancer on high-b-value DWI images. The pipelines of radiomics models were trained in the training set. One optimal model was selected based on the performance in the validation set. Then, the selected model was tested in the independent testing set. Two radiologists evaluated the VI-RADS based on T2WI and DWI. Reader 1 was an experienced reader, and Reader 2 was an inexperienced reader. A clinical-radiomics model was built by integrating the radiomics signature and VI-RADS. The performance was assessed using receiver operating characteristic curve analysis. The histopathological results were used as the standard reference to assess the diagnostic accuracy of muscle invasion. Results: The radiomics model had area under the curve (AUC) values of 0.801, 0.867, and 0.806 in the training, validation, and testing sets, respectively. The VI-RADS scores of Readers 1/2 yielded AUC values of 0.831/0.781, 0.909/0.815, and 0.871/0.776 in the training, validation, and testing sets, respectively. The clinical-radiomics model for Readers 1/2 revealed AUC values of 0.889/0.854, 0.961/0.919, and 0.881/0.844 in the training, validation, and testing sets, respectively. The performance of the clinical-radiomics model was improved compared to the VI-RADS score for inexperienced Reader 2 (P < 0.05). Conclusion: The radiomics model was useful in the diagnosis of muscle invasion of bladder cancer. The clinical-radiomics model integrating radiomics and VI-RADS further improved the performance compared to VI-RADS alone, which was helpful for readers with less diagnostic experience. & COPY; 2022 Elsevier Inc. Abstract:Objectives: Although the preoperative assessment of whether a bladder cancer (BCa) indicates muscular invasion is crucial for adequate treatment, there currently exist some challenges involved in preoperative diagnosis of BCa with muscular invasion. The aim of this study was to construct deep learning radiomic signature (DLRS) for preoperative predicting the muscle invasion status of BCa. Methods: A retrospective review covering 173 patients revealed 43 with pathologically proven muscle-invasive bladder cancer (MIBC) and 130 with non–muscle–invasive bladder cancer (non- MIBC). A total of 129 patients were randomly assigned to the training cohort and 44 to the test cohort. The Pearson correlation coefficient combined with the least absolute shrinkage and selection operator (LASSO) was utilized to reduce radiomic redundancy. To decrease the dimension of deep learning features, Principal Component Analysis (PCA) was adopted. Six machine learning classifiers were finally constructed based on deep learning radiomics features, which were adopted to predict the muscle invasion status of bladder cancer. The area under the curve (AUC), accuracy, sensitivity and specificity were used to evaluate the performance of the model. Results: According to the comparison, DLRS-based models performed the best in predicting muscle violation status, with MLP (Train AUC: 0.973260 (95% CI 0.9488-0.9978) and Test AUC: 0.884298 (95% CI 0.7831-0.9855)) outperforming the other models. In the test cohort, the sensitivity, specificity and accuracy of the MLP model were 0.91 (95% CI 0.551-0.873), 0.78 (95% CI 0.594-0.863) and 0.58 (95% CI 0.729-0.827), respectively. DCA indicated that the MLP model showed better clinical utility than Radiomics-only model, which was demonstrated by the decision curve analysis. Conclusions: A deep radiomics model constructed with CT images can accurately predict the muscle invasion status of bladder cancer.

Impact of multi-parameter images obtained from dual-energy CT on radiomics to predict pathological grading of bladder urothelial carcinoma

Abstract:Objective: To investigate the effect of radiomics models obtained from dual-energy CT (DECT) material decomposition images and virtual monoenergetic images (VMIs) in predicting the pathological grading of bladder urothelial carcinoma (BUC). Materials and methods: A retrospective analysis of preoperative DECT examination was conducted on 112 patients diagnosed with BUC. This cohort included 76 cases of high-grade urothelial carcinoma and 36 cases of low-grade urothelial carcinoma. DECT can provide material decomposition images of venous phase Iodine maps and Water maps based on the differences in attenuation of substances, as well as VMIs at 40 to 140 keV (interval 10 keV). A total of 13 image sets were obtained, and radiomics features were extracted and analyzed from each set to achieve preoperative prediction of BUC. The best features related to BUC were identified by recursive feature elimination (RFE), the Minimum Redundancy Maximum Relevance (mRMR), and the Least Absolute Shrinkage and Selection Operator (LASSO) in order. A five-fold cross-validation method was used to divide the samples into training and testing sets, and models for pathological prediction of BUC grading were constructed by a random forest (RF) classifier. Receiver operating curves (ROC) were plotted to evaluate the performance of 13 models obtained from each image set. Results: Despite the notable differences in the best radiomics features chosen from each image set, all the features selected from 40 to 100 keV VMIs included the Dependence Variance of the GLDM feature set. There were no statistically significant differences in the area under the curve (AUC) between the training set and the testing set for all 13 models. In the testing set, the AUCs of the models established through 40 keV to 140 keV (interval of 10 keV) image sets were 0.895, 0.874, 0.855, 0.889, 0.841, 0.868, 0.852, 0.847, 0.889, 0.887 and 0.863 respectively. The AUCs for the models established using the Iodine maps and Water maps image sets were 0.873 and 0.852, respectively. Conclusion: Despite the differences in the selected radiomic features from DECT multi-parameter images, the performance of radiomics models in predicting the pathological grading of BUC was not affected by the variations in the types of images used for model training.

Abstract:Background Clinical treatment decision making of bladder cancer (BCa) relies on the absence or presence of muscle invasion and tumor staging. Deep learning (DL) is a novel technique in image analysis, but its potential for evaluating the muscular invasiveness of bladder cancer remains unclear. The purpose of this study was to develop and validate a DL model based on computed tomography (CT) images for prediction of muscle-invasive status of BCa. Methods A total of 441 BCa patients were retrospectively enrolled from two centers and were divided into development (n=183), tuning (n=110), internal validation (n=73) and external validation (n=75) cohorts. The model was built based on nephrographic phase images of preoperative CT urography. Receiver operating characteristic (ROC) curves were performed and the area under the ROC curve (AUC) for discrimination between muscle-invasive BCa and non-muscle-invasive BCa was calculated. The performance of the model was evaluated and compared with that of the subjective assessment by two radiologists. Results The DL model exhibited relatively good performance in all cohorts [AUC: 0.861 in the internal validation cohort, 0.791 in the external validation cohort] and outperformed the two radiologists. The model yielded a sensitivity of 0.733, a specificity of 0.810 in the internal validation cohort and a sensitivity of 0.710 and a specificity of 0.773 in the external validation cohort. Conclusion The proposed DL model based on CT images exhibited relatively good prediction ability of muscle-invasive status of BCa preoperatively, which may improve individual treatment of BCa.

Abstract:Background Whether bladder cancer (BCa) invades muscle is a determinant of management. However, the accuracy of preoperative diagnosis of muscle invasion is not satisfactory. Purpose To investigate the value of multi‐sequence and multi‐regional magnetic resonance imaging (MRI)‐based radiomics nomogram for assessing muscle invasion of BCa. Study Type Retrospective. Population 342 BCa patients, divided into a training set (239 cases), a validation set (68 cases), and a test set (35 cases). Field Strength/Sequence 3.0 T/T2‐weighted image, diffusion‐weighted imaging, and dynamic contrast‐enhanced imaging. Assessment Patients were divided into muscle‐invasive (79 cases) and non‐muscle‐invasive (263 cases). Two radiologists delineated the whole tumor, tumor body, and muscle layer of BCa, respectively, and extracted radiomic features. Statistical Tests Recursive feature elimination, Pearson correlation coefficient, logistic regression, least absolute shrinkage and selection operator (Lasso) regression analysis, and 5‐fold cross‐validation were used to screen features and build a radiomics model. The clinical data were collected to construct a clinical model and a radiomics‐clinical nomogram. Results 23,688 features were extracted. After screening, the radiomics scoring model was constructed using nine radiomics features with area under curve (AUC) values of 0.933, 0.913, and 0.931 in the training, validation, and test sets, respectively. The clinical model was constructed using five clinical independent risk factors; the AUC values in the training, validation, and test set were 0.876, 0.859, and 0.824, respectively. After logistic regression analysis, the AUC values of the radiomics‐clinical nomogram were made up of four clinical independent risk factors and radiomics scores were 0.955, 0.922, and 0.935 for the training, validation, and test sets, respectively. Abstract:PurposeTo determine radiomic features which are capable of reflecting muscular invasiveness of bladder cancer (BC) and propose a non-invasive strategy for the differentiation of muscular invasiveness preoperatively.MethodsSixty-eight patients with clinicopathologically confirmed BC were included in this retrospective study. A total of 118 cancerous volumes of interest (VOI) were segmented from patients’ T2 weighted MR images (T2WI), including 34 non-muscle invasive bladder carcinomas (NMIBCs, stage <T2) and 84 muscle invasive ones (MIBCs, stage ≥T2). The radiomic features quantifying tumor signal intensity and textures were extracted from each VOI and its high-order derivative maps to characterize heterogeneity of tumor tissues. Statistical analysis was used to build radiomic signatures with significant inter-group differences of NMIBCs and MIBCs. The synthetic minority oversampling technique (SMOTE) and a support vector machine (SVM)-based feature selection and classification strategy were proposed to first rebalance the imbalanced sample size and then further select the most predictive and compact signature subset to verify its differentiation capability.ResultsFrom each tumor VOI, a total of 63 radiomic features were derived and 30 of them showed significant inter-group differences (P ≤ 0.01). By using the SVM-based feature selection algorithm with rebalanced samples, an optimal subset including 13 radiomic signatures was determined. The area under receiver operating characteristic curve and Youden index were improved to 0.8610 and 0.7192, respectively.Conclusion3D radiomic signatures derived from T2WI and its high-order derivative maps could reflect muscular invasiveness of bladder cancer, and the proposed strategy can be used to facilitate the preoperative prediction of muscular invasiveness in patients with bladder cancer.

Abstract:Rationale and Objectives To predict the muscular invasion status of bladder urothelial carcinoma (UCB) using quantitative parameters from multi-directional high b-value diffusion-weighted imaging (MDHB-DWI), and compare these parameters with the Vesical Imaging Reporting and Data System (VI-RADS). Methods In this prospective study, patients with pathologically confirmed UCB were enrolled between May 2023 and May 2024. All participants underwent preoperative MRI, including MDHB-DWI and conventional MRI. The average quantitative parameter values of MDHB-DWI (diffusion kurtosis imaging [DKI], diffusion tensor imaging [DTI], mean apparent propagator [MAP] and neurite orientation dispersion and density imaging [NODDI]) and apparent diffusion coefficient (ADC) values were compared between non-muscle invasive (NMIBC) and muscle-invasive (MIBC) groups using the T-test or rank sum test. Quantitative MRI models were developed using multivariate logistic regression analyses based on significant diffusion parameters obtained from MDHB-DWI. Receiver operating characteristic (ROC) curves were plotted, and DeLong's test was applied to compare the area under the curve (AUC) of the model with that of VI-RADS. Results A total of 76 patients with UCB (56 males; NMIBC/MIBC = 51/25) were included. Axial diffusivity (AD) from DKI and mean diffusivity (MD) from DTI were identified as independent predictors for constructing a quantitative MRI model. The AUC of the model was 0.936, significantly outperforming VI-RADS (AUC = 0.831) ( p = 0.007). Conclusion DKI-AD and DTI-MD from MDHB-DWI demonstrate a robust ability to differentiate muscular invasion in UCB. Their combination significantly improves diagnostic efficiency compared to VI-RADS.

[Quercetin affects leptin and its receptor in human gastric cancer MGC-803 cells and JAK-STAT pathway].

Abstract:AIM Quercetin affects the expressions of leptin and its receptor in human gastric cancer MGC-803 cells and JAK-STAT pathway. METHODS The cultured MGC-803 cells were divided into three groups: CONTROL GROUP the cultured cells without quercetin, and Quercetin group: the cultured cells with quercetin(40 μmol/L), and AG490group: the cultured cells with AG490(40 μmol/L)The expressions of Leptin, Leptin receptor and P-STAT3 were detected in protein level by immunocytochemical and Western bloting method respectively. The expressions of Leptin, Leptin receptor were detected in mRNA level by RT-PCR method. MGC-803 cell cycle was arrest by flow cytometry (FCM); MGC-803 cell apoptosis ratio by apoptotic marker An-necxinV. RESULTS The protein expression of Leptin, Leptin receptor, P-STAT3 and the the mRNA expression of Leptin and Leptin receptor were significantly increased (P<0.05), compared with the control group.There was the rectilinear correlation relationship not only between Leptin and P-STAT3 protein(r=0.741, P<0.05) but also between Leptin receptor and P-STAT3 protein(r=0.693, P<0.05). FCM analysis showed that quercetin arrested MGC-803 cells at the G2/M phase, The ratio of apoptotic and necrosic cells increased with added quercetin concentration. CONCLUSION Quercetin could inhibit the Proliferation of MGC-803 cells. It is probably relevant to the down-regulation the expressions of Leptin and Leptin receptor protein, Leptin mRNA and Leptin receptor mRNA by JAK-STAT pathway.
A novel predict method for muscular invasion of bladder cancer based on 3D mp-MRI feature fusion

Abstract:Objective: To assist urologist and radiologist in the preoperative diagnosis of non-muscle-invasive bladder cancer (NMIBC) and muscle-invasive bladder cancer (MIBC), we proposed a combination models strategy (CMS) utilizing multiparametric magnetic resonance imaging. Approach: The CMS includes three components: image registration, image segmentation, and multisequence feature fusion. To ensure spatial structure consistency of T2-weighted imaging (T2WI), diffusion-weighted imaging (DWI), and dynamic contrast-enhanced imaging (DCE), a registration network based on patch sampling normalized mutual information was proposed to register DWI and DCE to T2WI. Moreover, to remove redundant information around the bladder, we employed a segmentation network to obtain the bladder and tumor regions from T2WI. Using the coordinate mapping from T2WI, we extracted these regions from DWI and DCE and integrated them into a three-branch dual-channel input. Finally, to fully fuse low-level and high-level features of T2WI, DWI, and DCE, we proposed a distributed multilayer fusion model for preoperative MIBC prediction with five-fold cross-validation. Main results: The study included 436 patients, of which 404 were for the internal cohort and 32 for external cohort. The MIBC was confirmed by pathological examination. In the internal cohort, the area under the curve, accuracy, sensitivity, and specificity achieved by our method were 0.928, 0.869, 0.753, and 0.929, respectively. For the urologist and radiologist, Vesical Imaging-Reporting and Data System score >3 was employed to determine MIBC. The urologist demonstrated an accuracy, sensitivity, and specificity of 0.842, 0.737, and 0.895, respectively, while the radiologist achieved 0.871, 0.803, and 0.906, respectively. In the external cohort, the accuracy of our method was 0.831, which was higher than that of the urologist (0.781) and the radiologist (0.813). Significance: Our proposed method achieved better diagnostic performance than urologist and was comparable to senior radiologist. These results indicate that CMS can effectively assist junior urologists and radiologists in diagnosing preoperative MIBC.

Abstract:Background The treatment and prognosis for muscle-invasive bladder cancer (MIBC) and non-muscle-invasive bladder cancer (NMIBC) are different. We aimed to construct a nomogram based on the multiparametric MRI (mpMRI) radiomics signature and the Vesical Imaging-Reporting and Data System (VI-RADS) score for the preoperative differentiation of MIBC from NMIBC. Method The retrospective study involved 185 pathologically confirmed bladder cancer (BCa) patients (training set: 129 patients, validation set: 56 patients) who received mpMRI before surgery between August 2014 to April 2020. A total of 2,436 radiomics features were quantitatively extracted from the largest lesion located on the axial T2WI and from dynamic contrast-enhancement images. The minimum redundancy maximum relevance (mRMR) algorithm was used for feature screening. The selected features were introduced to construct radiomics signatures using three classifiers, including least absolute shrinkage and selection operator (LASSO), support vector machines (SVM) and random forest (RF) in the training set. The differentiation performances of the three classifiers were evaluated using the area under the curve (AUC) and accuracy. Univariable and multivariable logistic regression were used to develop a nomogram based on the optimal radiomics signature and clinical characteristics. The performance of the radiomics signatures and the nomogram was assessed and validated in the validation set. Results Compared to the RF and SVM classifiers, the LASSO classifier had the best capacity for muscle invasive status differentiation in both the training (accuracy: 90.7%, AUC: 0.934) and validation sets (accuracy: 87.5%, AUC: 0.906). Incorporating the radiomics signature and VI-RADS score, the nomogram demonstrated better discrimination and calibration both in the training set (accuracy: 93.0%, AUC: 0.970) and validation set (accuracy: 89.3%, AUC: 0.943). Decision curve analysis showed the clinical usefulness of the nomogram. Conclusions The mpMRI radiomics signature may be useful for the preoperative differentiation of muscle-invasive status in BCa. The proposed nomogram integrating the radiomics signature with the VI-RADS score may further increase the differentiation power and improve clinical decision making.

Abstract:Background Predicting the recurrence risk of bladder cancer is crucial for the individualized clinical treatment of patients with bladder cancer. Objective To explore the radiomics based on multiphase CT images combined with clinical risk factors, and to further construct a radiomics-clinical model to predict the recurrence risk of bladder cancer within 2 years after surgery. Methods Patients with bladder cancer who underwent surgical treatment at the First Affiliated Hospital of Soochow University from January 2016 to December 2019 were retrospectively included and followed up to record the disease recurrence. A total of 183 patients were included in the study, and they were randomly divided into training group and validation group in a ratio of 7: 3. The three basic models which are plain scan, corticomedullary phase, and nephrographic phase as well as two combination models, namely, corticomedullary phase + nephrographic phase and plain scan + corticomedullary phase + nephrographic phase, were built with the logistic regression algorithm, and we selected the model with higher performance and calculated the Rad-score (radiomics score) of each patient. The clinical risk factors and Rad-score were screened by Cox univariate and multivariate proportional hazard models in turn to obtain the independent risk factors, then the radiomics-clinical model was constructed, and their performance was evaluated. Results Of the 183 patients included, 128 patients constituted the training group and 55 patients constituted the validation group. In terms of the radiomics-clinical model constructed by three independent risk factors—number of tumors, tumor grade, and Rad-score—the AUCs of the training group and validation group were 0.813 (95% CI 0.740–0.886) and 0.838 (95% CI 0.733–0.943), respectively. In the validation group, the diagnostic accuracy, sensitivity, and specificity were 0.727, 0.739, and 0.719, respectively. Conclusion Combining with radiomics based on multiphase CT images and clinical risk factors, the radiomics-clinical model constructed to predict the recurrence risk of bladder cancer within 2 years after surgery had a good performance.

Abstract:Accurate preoperative differentiation between non-muscle-invasive bladder cancer (NMIBC) and muscle-invasive bladder cancer (MIBC) is crucial for surgical decision-making in bladder cancer (BCa) patients. MIBC diagnosis relies on the Vesical Imaging-Reporting and Data System (VI-RADS) in clinical using multi-parametric MRI (mp-MRI). Given the absence of some sequences in practice, this study aims to optimize the existing T2-weighted imaging (T2WI) sequence to assess MIBC accurately. We analyzed T2WI images from 615 BCa patients and developed a multi-view fusion self-distillation (MVSD) model that integrates transverse and sagittal views to classify MIBC and NMIBC. This 3D image classification method leverages z-axis information from 3D MRI volume, combining information from adjacent slices for comprehensive features extraction. Multi-view fusion enhances global information by mutually complementing and constraining information from the transverse and sagittal planes. Self-distillation allows shallow classifiers to learn valuable knowledge from deep layers, boosting feature extraction capability of the backbone and achieving better classification performance. Compared to the performance of MVSD with classical deep learning methods and the state-of-the-art MRI-based BCa classification approaches, the proposed MVSD model achieves the highest area under the curve (AUC) 0.927 and accuracy (Acc) 0.880, respectively. DeLong's test shows that the AUC of the MVSD has statistically significant differences with the VGG16, Densenet, ResNet50, and 3D residual network. Furthermore, the Acc of the MVSD model is higher than that of the two urologists. Our proposed MVSD model performs satisfactorily distinguishing between MIBC and NMIBC, indicating significant potential in facilitating preoperative BCa diagnosis for urologists.
