Abstract:Background As a common breast disease in lactating women, lactational mastitis is clinically manifested as painful breast lumps and systemic symptoms. To a certain extent, this disease may affect the physical and mental health of lactating women and even the breastfeeding of infants. Objective This study was conducted to evaluate the clinical efficacy of Lou's Wentong Method in the comprehensive treatment of lactational mastitis and its effects on relevant symptoms. Methods A total of 158 patients who were diagnosed with lactational mastitis in The First Affiliated Hospital of Zhejiang Chinese Medical University from January 2022 to January 2024 were included in this study to conduct a retrospective analysis. The treatment method comprised oral administration of traditional Chinese medicine prescriptions guided by the concept of "Wentong for Carbuncle", puncture and pus extraction for patients with abscess formation, and milk drainage for patients with milk stagnation. The clinical symptoms of these patients were collected according to the seven indexes in Lou's quantitative scorecard for evaluating the condition of canker sores before and after treatment. Additionally, the improvement of each symptom after treatment, the total effective rate of the clinical treatment, and the total symptom evaluation score before and after treatment were also analyzed. Results It was demonstrated that there were significant differences in the overall distribution of different severity symptoms (skin erythema, breast pain, number of lumps, size of lumps, breast abscesses, and body temperature) at three time points (P < 0.01). The evaluation of these symptoms at different time points revealed that all symptoms were effectively mitigated over time. Meanwhile, the efficacy was assessed according to the efficacy index to evaluate the efficacy of Lou's Wentong Method in the comprehensive treatment of lactational mastitis. The results corroborated the pronounced efficacy of Lou's Wentong Method in treating lactational mastitis, exhibiting statistical significance (P < 0.01). Conclusion Lou's Wentong Method has significant therapeutic effects in the comprehensive treatment of lactational mastitis. Furthermore, this method is conducive to alleviating the clinical symptoms of these patients as soon as possible and can reduce the pain of patients, thus promoting their recovery.
Treatment of Lactational Breast Abscesses with Cavity Diameter Larger Than 5 Cm Via Combined Ultrasonography-Guided Percutaneous Catheter Placement and Hydrostatic Pressure Irrigation.

Abstract:This study reports on our experience of treating lactational breast abscesses larger than 5 cm via ultrasonography (US)-guided percutaneous catheter placement and hydrostatic pressure irrigation. Twelve cases of puerperal single breast abscesses larger than 5 cm were collected. These patients were treated with US-guided percutaneous catheter placement and hydrostatic pressure irrigation combined with oral antibiotics. All 12 patients using US-guided treatment were completely successful without conversion to open surgical drainage. The range of recovery time was 5-16 days, and no major complications occurred. The patients were satisfied with the appearance of the scar, and there were no reports of recurrence during the follow-up period.Overall, US-guided percutaneous catheter placement and hydrostatic pressure irrigation are successful strategies for the treatment of lactational breast abscesses larger than 5 cm. These methods not only reduce the treatment time and improve the patients' clinical course but also provide cosmetic effects.IMPACT STATEMENTWhat is already known on this subject? The current consensus on breast abscess treatment is that lesions <3 cm can be effectively treated by aspiration alone, lesions >3 cm require catheter drainage, lesions <5 cm have proven to be safe and effectively treated by US-guided ultrasound therapy, and lesions >5 cm, whether multi-loculated or longstanding, require surgical incision and drainage.What do the results of this study add? We tried to use this method to increase the cure rate of US-guided minimally invasive treatment for large abscesses. The results showed that all patients were cured successfully, requiring no further surgical intervention. Moreover, no complications occurred, and no patients developed sequelae. During the three-month follow-up period, there was no evidence of recurrence in any case.What are the implications of these findings for clinical practice and/or further research? Questions remain regarding the treatment's generalisability, potentially lengthy hospitalisation, and technical limitations of the existing instrumentation. Long-term follow up and larger sample size Randomised clinical trials studies are still needed to rigorously and scientifically ensure the method's benefits over conventional open surgery in the future.

Abstract:<p>Lactational breast abscess is a serious complication of mastitis and commonly diagnosed in breast‐feeding women. The traditional drainage of breast abscess was often performed with incisive technique which may result in prolonged healing time, regular dressings, dressing pain, interfering with breastfeeding and unsatisfactory cosmetic outcome. As minimal invasive alternatives to incisive drainage, needle aspiration or percutaneous catheter placement cannot completely replace incisive drainage for the inability to treat large, multiloculated or chronic abscess. Vacuum‐assisted breast biopsy system (VABB) has been successfully applied in the treatment of benign breast diseases with satisfactory cosmetic outcomes. Among VABB devices, EnCor system has some distinctive features that make it an appropriate candidate for the treatment of lactational breast abscesses. In this study, for the first time, we investigated the feasibility, efficacy, and cosmetic results of surgical drainage of lactational breast abscess with US‐guided Encor VABB system. Our data suggests this procedure could serve as a promising alternative for women with lactational breast abscess who require incisive intervention with high cure rate, relatively short healing time, low recurrence rate, few complications, satisfactory cosmetics outcome and without interfering with breastfeeding.</p>

A Randomized Controlled Trial Comparing Ultrasonography-Guided Needle Aspiration and Surgical Drainage for the Management of Breast Abscess

Abstract:Background A breast abscess is a localized collection of inflammatory exudate in breast tissue. It is more common in lactating women. Treatment of the breast abscess is usually by incision and drainage, which is accompanied by extensive trauma, lengthy surgical incisions, psychological distress, surgical scar, and discomfort during dressing changes. Recent research has shown that ultrasonography-guided needle aspiration is an alternative to incision and drainage for breast abscess treatment, has superior cosmetic outcomes, and has fewer costs. However, there are no clear guidelines for the same. The primary objective of this study was to assess and contrast the outcomes and efficacy of two approaches in the treatment of breast abscesses: ultrasonography-guided needle aspiration and conventional incision and drainage. Methods This was a single-center, prospective, randomized, controlled, non-inferiority trial. Patients with breast abscesses were randomized either to receive needle aspiration or incision drainage. Patients with lactational breast abscesses were encouraged to breastfeed from either breast. The antibiotic was given for 10 days to all the patients. The study's main focus was on the primary outcome, which was the failure rate. Additionally, several secondary outcomes were examined, including postoperative pain, time required for healing, presence of residual abscess or recurrence, formation of fistulas, and the assessment of scar appearance. Results A total of 44 patients were randomized to 23 and 21 in each arm. There was no statistical difference in treatment failure (p-value=0.862), fistula formation (p-value=1.00), and recurrence (p-value=1.00). There was a significant statistical difference in healing period (p-value <0.001), scar formation (p-value <0.0001), continuation of breastfeeding (p-value=0.005), and clinical resolution (p-value=0.002). There was a significant reduction in post-intervention pain in the needle aspiration group than in the other group (p-value <0.001). Conclusion This study showed a significant difference in postoperative pain, healing time, the continuation of breastfeeding, and scar formation in the needle aspiration group, favouring minimally invasive treatment for breast abscess. However, due to the small sample size, we could not conclude that ultrasonography-guided needle aspiration of the breast abscess is non-inferior to the conventional incision and drainage.

Abstract:Background: Several Chinese mothers experience problems related to their breasts during breastfeeding and are referred to hospitals for treatment after a massage by a "cuirushi." Some of these patients develop large breast abscesses. Notably, the treatment of residual milk cavities is difficult after pus aspiration. Lactational residual milk cavities in the breast hinder effortless breastfeeding, but no solution has been reported thus far. This study aims to explore the use of a simple and minimally invasive method for treating lactational residual milk cavities and enabling mothers to breastfeed effortlessly. Materials and Methods: We retrospectively analyzed clinical data of 28 patients with lactational residual milk cavities treated using negative-pressure suction cannula and drainage at our department from December 2016 to May 2019. The success rate, safety, and feasibility of this technique are summarized. Results: Among the 28 patients, the first catheterization was successful in 7 (25%) patients, the second in 13 (46.4%) patients, and the third or subsequent catheterizations in 8 (28.6%) patients. The intubation time was <10 days in 3 patients, between 10 and 20 days in 12 patients, and >20 days in 13 patients. During and after treatment, 21 patients (75%) continued breastfeeding with the affected breast. Conclusions: Ultrasound-guided negative-pressure suction cannula is a new method to treat lactational residual milk cavities that may remain after initial therapy for lactational breast abscess. The treatment of lactational residual milk cavities offers significant advantages in supporting mothers during breastfeeding.

In order to solve this problem, a new high-frequency contactless conductivity detector for electrophoresis chip is designed. At the same time, theoretical study and analysis of this new detector are carried out, based on the construction of an equivalent circuit mode of high-frequency conductivity detector and the design of the structure. With the consideration of many factors including band broadening, the dimensions of the structure were designed and optimized. In summary, the detector has many advantages such as wide application range, simple structure and easy integration.

Bacterial culture results showed that 85.4% of cases were Methicillin-resistant Staphylococcus aureus . The number of aspirations was from 1 to 5. The cure rate was 91.5%, and 5.3% of these cases had a complication associated with galactocele after treatment. Conclusion: Ultrasound-guided needle aspiration is a minimally invasive treatment option for lactating breast abscesses with a high complete cure rate and good cosmetic results. Keywords: breast abscess, ultrasound-guided needle aspiration Breast abscess is a localized collection of pus within breast parenchyma and is mostly a complication of mastitis. 1 This disease is popular in women 18–50 years old and is divided into lactating and non-lactating abscesses, of which lactating breast abscess accounts for 0.4–11% of all puerperal women. 2 Nutritional problems, maternal hygiene, low living standards, and late and ineffective use of antibiotics are the main reasons for severe breast abscesses, especially in developing countries. 3 Treatment of breast abscess is a challenge, not only with the aim of cure but also preserving breast function. Incision and drainage abscess are the traditional treatment methods. However, this method has some disadvantages, such as general anesthesia, prolonged healing time, recurrent infection, and scars with poor cosmetic outcomes after treatment. 4 Incision and drainage combined with antibiotic therapy have a recurrence rate of about 10–38%. 5 Ultrasound guide-needle aspiration is a minimally invasive procedure that has been applied since the 90s, and currently, there have been a lot of studies demonstrating its effectiveness in treatment with some advantages, such as safety, good cosmetic outcome, and low-cost method. 6,7 Therefore, we conducted a study with the aim of assessing the effectiveness of ultrasound-guided needle aspiration of lactating breast abscess. The prospective and descriptive study with a convenience sample was conducted at Radiology Center in Bach Mai Hospital from June 2020 to July 2021. All lactating patients admitted to the hospital with suspected breast abscesses underwent breast ultrasound to detect abscesses. The exclusive criteria included recurrent or chronic abscess and necrosis skin overlying the abscess. The procedure was performed by a radiologist with more than five years of interventional experience, an assistant radiologist, and a nurse in the intervention room. Disinfect and local anesthetic the needle puncture site with Lidocaine 2%. Insert a needle through the skin into the abscess and suction the abscess under ultrasound guidance, absorbing the maximum amount of fluid possible. The size of the needle was 18 gauge to 14 gauge, depending on the feature of the pus of the abscess. If the abscess had a capsule, irrigating the abscess cavity with saline was performed several times until the suction fluid returned clear. Breast microecology improvement using probiotics following needle aspiration in patients with lactational breast abscess: a multi-center randomized double-blind controlled trial

Abstract:Although oral probiotics can improve breast microecology and alleviate the inflammatory response, there are no data regarding cases with existing abscesses. We aimed to investigate the effect of Lactobacillus fermentum CECT5716 during needle aspiration in patients with lactational breast abscesses. Patients (aged 20–41 years) with lactational single-cavity breast abscesses (diameter 3–6 cm) from 12 hospitals were randomly assigned to the experimental (n = 51) and control groups (n = 50). Outcome measures included the abscess cure rate on treatment day-5, delactation rate, relieving pain rate, and number of needle aspirations until day-28. The experimental group's 5-day cure rate (43.1%) was significantly higher ( p < 0.05). Breastfeeding continuation on day-5 did not differ significantly (experimental group: 88.2%, control group: 96.0%, p = 0.269). In the experimental and control groups, 19.6% and 14.0% of patients experienced moderate to severe pain on day-5, respectively, with no statistically significant differences ( p = 0.451). Four patients in each group developed diarrhea, with adverse reaction rates of 7.84% and 8.0%, respectively. No adverse reactions were reported in the infants. L. fermentum can shorten the healing time in patients with lactational breast abscesses.

Abstract:Background and Objective: We investigated whether (1) there are any differences in lactational breast abscesses between patients from whom methicillin-resistant Staphylococcus aureus (MRSA) and those from whom methicillin-sensitive S. aureus (MSSA) were isolated from pus samples and (2) there are differences in the effects of ultrasound-guided aspiration. Materials and Methods: The clinical data of 171 patients with lactational breast abscesses treated by ultrasound-guided aspiration in Beijing from January to July 2018 were retrospectively analyzed. The patients were divided into MSSA infection (N = 132) and MRSA infection (N = 39) groups according to their bacterial culture results. Abscess cavity location, abscess cavity number, maximum abscess cavity size, antibiotic utilization rate, and cure rate were compared between the groups. Cure rate refers to the proportion of the total number of cases remaining after the elimination of failed cases. The number of ultrasound-guided aspiration procedures performed for healing between the two groups was also compared. Results: There were no significant differences in abscess cavity location, abscess cavity amount, and abscess cavity size between both groups. The antibiotic utilization rate of the two groups were 58.3% (MSSA, 77/132) and 69.2% (MRSA, 27/39), respectively, and there were no significant differences between both groups. The cure rates of ultrasound-guided aspiration of the two groups were 97.7% (MSSA, 129/132) and 92.3% (MRSA, 36/39), and there were no significant differences between both groups. There were also no significant differences in the median number of aspiration performed for cure between the MRSA infection group (median = 3, range = 1-10) and the MSSA infection group (median = 3, range = 1-14). Conclusion: Lactational breast abscesses are the same irrespective of the type of S. aureus infection. Treatment by ultrasound-guided aspiration for patients with MRSA infection can achieve the same effect as that for those with MSSA infection.
Oral Lactobacillus Fermentum CECT5716 in the Patients with Lactational Abscess Treated by Needle Aspiration: the Late Follow-Up of a Randomized Controlled Trial.

Abstract:BACKGROUND:Lactational mastitis and breast abscess cause trouble for women. It has been shown that oral probiotics can improve breast microecology, thus alleviating inflammatory responses. Our study aims to understand the long-term effect of Lactobacillus fermentum CECT5716 on patients with lactational breast abscess after needle aspiration.METHODS:Data continued in a randomized controlled study of 101 subjects with lactational abscess from 12 hospitals were included. They were randomly divided into an experimental group and a control group. After needle aspiration treatment, the experimental group was orally administrated with L fermentum CECT5716 for 4 consecutive weeks, while the control group was treated with maltodextrin in the same way). In the third month after randomized controlled trial, the subjects were followed up by an online questionnaire investigation. The observation indexes included the relief of breast pain, recurrence of mastitis from the end of oral administration to the follow-up, and the effect on continuing breastfeeding.RESULTS:A total of 101 patients were enrolled and 83 valid questionnaires were received during follow-up, including 40 in the experimental group and 43 in the control group. The rate of stop breastfeeding due to recurrence of mastitis was 2.5% (1/40) in the experimental group and 18.6% (8/43) in the control group, with a statistically significant difference (odds ratio = 0.112, 95% confidence interval: 0.013-0.942, P < .05). The rate of stop breastfeeding was 10% (4/40) in the experimental group and 25.6% (11/43) in the control group, without significant difference. The pain relief rate in the experimental group was 80% (32/40), which showed no significant difference from that in the control group, that is, 72.1% (31/43). The recurrence rate of mastitis in the experimental group was 20% (8/40), which was not significantly different from that in the control group, that is, 16.3% (7/43).CONCLUSIONS:In lactating women with a history of breast abscess, oral L fermentum CECT5716 may reduce the risk of stop breastfeeding due to recurrence of mastitis.
The effectiveness of needle aspiration versus traditional incision and drainage in the treatment of breast abscess: a meta-analysis

Abstract:Breast abscess is a common and intractable clinical condition and the use of needle aspiration (NA) or incision and drainage (ID) in treatment is controversial. This meta-analysis aimed to systematically compare the clinical effectiveness of NA and ID in treating breast abscesses. The Web of Science, ScienceDirect, PubMed, Cochrane Library, EMBASE, China National Knowledge Infrastructure, and Wanfang Data were searched for randomized controlled trials (RCTs) published from inception to January 7, 2022. The ROB-2 tool assessed risk of bias; the GRADE methodology rated certainty in outcomes; and Stata 16.0 performed data analyses. Nine RCTs were included, including 703 patients. The results showed there was no significant difference in cure rate between the two groups (relative risk [RR] = 0.96, 95% confidence interval [CI] [0.86, 1.07]; p = .469), and after subgroup analysis, we found that it was not related to the use of ultrasound guidance or not. There was no significant difference in the recurrence rate (RR = 0.68, 95% CI [0.35, 1.30]; p = .241). Furthermore, the NA group was associated with shorter healing time (weighted mean differences = −11.02, 95% CI [−15.14, −6.90]; p < .001), lower incidence of breast fistula (RR = 0.21, 95% CI [0.06, 0.72]; p = .013), lower interrupted breastfeeding rate (RR = 0.28, 95% CI [0.20, 0.39]; p < .001), and higher satisfaction rate of appearance (RR = 1.51, 95% CI [1.03–2.21]; p = .035). NA has better advantages in terms of healing time, avoidance of breast fistula, continuous breastfeeding, and patient satisfaction. Although NA and ID have similar cure and recurrence rates, NA, with or without ultrasound guidance, could be used as a first-line treatment for breast abscesses. Patients with large volumes, multicompartmental abscesses, or those who have been ineffective against multiple NA, should be considered for ID. Breast abscess is a common and intractable clinical condition in general surgery. Compared with ID for breast abscesses, NA has better advantages in terms of healing time, avoidance of breast fistula, continuous breastfeeding, and patient satisfaction and could be used as a first-line treatment for breast abscesses. Exploring the Efficacy of Comprehensive Management of Breast Abscesses in Restoring Milk Volume

Abstract:Background Approximately 54.8% of patients with breast abscesses discontinue breastfeeding due to the lack of adequate breastfeeding support. Research aims We aimed to (1) examine the difference in milk volume produced by healthy breasts and breasts with abscesses; and (2) to explore the changes in milk volume before and after comprehensive management. Method A prospective, consecutive series study was designed. Lactating patients (N = 50) with breast abscesses were selected from March 2017 to November 2018. The volume and frequency of milk expression of the affected and the unaffected breast were recorded every 24 hr before and after comprehensive management. The difference in the milk volume produced by healthy breasts (control) and breasts with abscesses, as well as the milk volume produced by each breast before and after comprehensive management, was determined. Results There was a significant difference in the milk volume produced by healthy breasts and breasts with abscess before (t = 3.016; p = .004) and after (t = 4.336; p < .001) comprehensive management. The frequency of milk expression was significantly higher after comprehensive management than before it (z = -6.145; p < .001); the milk volume produced by each side significantly increased after comprehensive management (healthy breasts: t = -4.789; p < .001; breasts with abscess: t = 2.555; p = .014). Conclusion The total milk volume produced by breasts with abscesses can be less than that produced by healthy breasts. The management of abscesses by increasing the frequency of milk expression and degree of emptying can help mothers increase their milk volume.

Abstract:Abstract Introduction Breast massage and manual expression of breast milk are recommended for some breastfeeding problems. Aim This retrospective study evaluated the differences in the clinical characteristics and treatment of lactating mothers with breast abscess involving different breast massage participants. Methods Medical records of 654 women with breast abscess in Haidian Maternal and Child Health Hospital were obtained and analyzed. The women were categorized into the study and control groups based on the person who performed the massage (non-medical vs. medical personnel). The difference in the site of breast abscess, type of abscess cavity, necrotic organization inside the abscess cavity, treatment of abscess, and number of punctures treatment was compared. Results The incidences of breast abscess in the nipple areola area, multicavity, and necrotic tissue in the abscess cavity in the study group were higher than in the control group. The mean number of abscess cavities in the two groups was not significantly different. The puncture treatment rate was lower, while the mean puncture number was higher in the study group. Conclusion Breast massage by non-medical personnel resulted in more complex and difficult to treat abscesses, which underscores the need for mothers to be cautious in choosing breast massage providers.
A Novel Tubeless Urinary Catheter Protocol Enhanced Recovery after Minimally Invasive Lung Surgery

Abstract:Objectives: Although previous studies have shown the feasibility of non-intubated techniques, it is unknown whether avoiding urinary catheters can enhance recovery. This study aimed to determine whether the tubeless urinary catheter protocol is feasible and beneficial for minimally invasive lung surgery. Methods: Patients were randomized to the control group, completely tubeless group, and partially tubeless group. A propensity score-matched (PSM) analysis was performed to balance the non-random baseline characteristics. Complications and postoperative recovery were compared. Regression analysis was performed to identify the independent predictors of complications. A nomogram for predicting the risk of non-automatic micturition was constructed and internally validated. Results: One hundred fifty-nine patients were enrolled. The incidence rates of urinary irritation and urinary tract infection (UTI) were significantly lower in the tubeless groups (74.4 vs. 39.5%, p < 0.001; 28.2 vs. 8.6%, p = 0.001, respectively). The tubeless group had a higher proportion of 0-degree discomfort (81.5 vs. 30.8%, p = 0.001) and shorter duration of postoperative hospital stay than the control group (4.59 vs. 5.53 days, p < 0.001). No difference was observed in terms of urination retention and urinary incontinence between the tubeless group and the control group. After PSM, the advantages of the tubeless group still existed, and comparing to the partially tubeless group, the completely tubeless group was of even less UTI and more 0-degree discomfort (18.5 vs. 0.0%, p = 0.019; 96.3 vs. 59.3%, p = 0.002). The tubeless protocol was the only independent protective factor of urinary complications. A nomogram was constructed and showed good predictive ability. Conclusions: The tubeless catheterization protocol led to fewer complications, better compliance, and shorter hospital length of stay. The advantages were more significant with the completely tubeless protocol. The utility of our nomogram can assist clinicians in avoiding risks in performing the tubeless protocol.

Abstract:Background: Breastfeeding not only meets the nutritional needs of newborn growth and development but also promotes uterine contraction and discharge of lochia, which helps in maternal recovery. However, some mothers experience abnormal lactation and breast swelling due to a lack of breastfeeding knowledge, painful cesarean incisions, anesthesia, negative emotions, and other factors, resulting in a reduced breastfeeding rate, which adversely affects neonatal and maternal health. Aim: To explore the effects of care intervention with a health education form for breastfeeding on breastfeeding-related conditions. Methods: In this study, 207 mothers with postpartum breast pain and difficulty lactating were selected and divided into intervention and control groups using a random number table. Both groups of mothers were provided with basic nursing and related treatment measures after delivery. The intervention group additionally received care intervention with a health education form for breastfeeding. The scores of lactation volume, breast distension and pain, breastfeeding rate, breastfeeding self-efficacy, treatment effect, and complication rate of the two groups were compared. Results: After treatment, the breast pain score of the intervention group was significantly lower than that of the control group, while the lactation score, score of Breastfeeding Self-Efficacy Scale Short Form scale, parent-child communication score, maternal-infant interaction score, total score of maternal-infant communication, and breastfeeding rate of the intervention group were significantly higher than those of the control group. After intervention, the overall therapeutic effect of the intervention group was better than that of the control group, and the complication rate of the intervention group was lower than that of the control group. Conclusion: Breastfeeding health education and nursing intervention combined with basic clinical treatment have good clinical effects in managing postpartum breast distension and pain and increasing lactation yield.

Abstract:Background Idiopathic granulomatous mastitis (IGM) is a rare inflammatory disease of the breast with clinical features that are often confused with those of breast cancer leading to delayed diagnosis and treatment. This retrospective study aimed to evaluate the therapeutic effectiveness and cosmetic results of drainage surgery using ultrasound‐guided vacuum‐assisted excision (VAE) for the treatment of IGM at the abscess stage. Methods The time of recovery, cases of further surgical intervention, and cosmetic results were retrospectively collected and analyzed from patients who underwent drainage with VAE or conventional drainage between October 2017 and August 2021. Results A total of 65 patients diagnosed with IGM at the abscess stage who underwent drainage surgery with VAE or conventional drainage surgery were enrolled. Overall, 38 (58.5%) underwent conventional drainage surgery and 27 (41.5%) underwent drainage with VAE. We found that patients who underwent VAE recovered much faster than those who underwent traditional drainage surgery (30.1 vs. 48.0 days). Nine (33.3%) patients in the VAE group required further surgical intervention after drainage, whereas 33 (86.8%) patients in the control group underwent another surgery to resect residual lesions. Additionally, patients in the VAE group were more satisfied with the breast appearance, mainly due to less influence of the scars and better symmetry of their breasts. Conclusion Compared with conventional drainage surgery, drainage surgery using VAE for IGM patients at the abscess stage improved therapeutic and cosmetic outcomes. Furthermore, postoperative management of IGM is crucial.

Ultrasound-guided Percutaneous Catheter Drainage for the Treatment of Liver Abscess

Abstract:Objective To investigate ultrasound-guided percutaneous catheter drainage for the treatment of liver abscess results.Methods 28 cases with liver abscess using ultrasound-guided percutaneous catheter drainage from January 2009 to January 2011 in our hospital were considered as the observation group,and 28 cases liver abscess using abscess incision drainage from January 2004 to January 2006 in our hospital were considered as the control group.The clinical efficacy,hospital staying time and complications were compared between the two groups.Results The clinical efficacy in the observation group was 100%,better than 78.57% of the control group(P 0.05);temperature recovery time and hospital staying time in the observation group were(1.7±0.6) d and(10.7±3.3) d,shorter than(3.2±0.8) d and(17.5±3.6) d of the control group(P 0.05);there were no cases with complications in the observation group,while there were 7 cases in the control group(P 0.05).Conclusion Ultrasound-guided percutaneous catheter drainage for the treatment of liver abscess is not only superior to the efficacy of abscess incision drainage surgery,and it has fewer complications and trauma,can significantly shorten the patient's hospital staying time,it is worthy to be applied in clinic.

Abstract:To evaluate the initial efficacy of comprehensive treatment of external treatment of traditional Chinese medicine in the therapy of acute mastitis during lactation and its influence on sufferers' symptoms.From January 2021 to January 2023, a retrospective analysis was performed on 100 sufferers with acute mastitis during lactation who were received in our hospital as the study objects. Divided them into a control group (n = 50) and an observation group (n = 50). Among them, the control one was received in 50% magnesium sulfate for moist heat compress, and the observation one was treated with comprehensive external therapy of traditional Chinese medicine. After different treatment measures were carried out in the 2 groups, the total effective ratio of clinical therapy, excellent and good rates of lactation, quality of life measurements, pain scores before and after treatment, and TCM symptom points at pre-therapy and post-therapy, negative emotion scores at pre-therapy and post-therapy related to patients were analyzed.(1) In the data comparison of the total effective ratio of clinical therapy between the 2 groups of sufferers after therapy, the data in the observation one were greater than the control one, and the distinction was obvious, with P < .05. (2) In the data comparison of the excellent and good lactation rate between the 2 groups of sufferers at post-therapy, the data in observation one were greater than the control one, and the distinction was obvious, with P < .05. (3) In the data comparison of quality of life measurements between the 2 groups of sufferers at post-therapy, the data in the observation one were greater than the control one, and the distinction was obvious, with P < .05. (4) Before treatment, it had no obvious distinction in the pain scores between the 2 groups, with P > .05; in the data comparison of the pain points between the 2 groups after therapy, the data in observation one were less than the control one, and the distinction was obvious, with P < .05. (5) Before therapy, it had no obvious distinction in TCM symptom points between the 2 groups, with P > .05; in the data comparison of TCM symptom points between the 2 groups after therapy, the data in the observation one were less than the control one, and the distinction was obvious, with P < .05. (6) Before therapy, it had no obvious distinction in negative emotion points between the 2 groups, with P > .05; in the data comparison of negative emotion scores between the 2 groups of sufferers at post-therapy, the data in the observation one were less than those in the control one, and the distinction was obvious, with P < .05.The comprehensive therapy of TCM external treatment for acute mastitis during lactation has a significant therapeutic effect, which is conducive to improving the clinical symptoms of sufferers as soon as possible and at the same time, greatly improving the life quality of sufferers and improving their negative emotions, which can relieve the pain of sufferers and promote the recovery of the sufferers.

Abstract:Objective: To explore the effect of Wangbuliuxing (semen vaccariae) combined with massage at breast and acupoint on breastfeeding and lactation function in cesarean section women. Methods: A total of 120 cases of cesarean section women were randomly divided into an observation group and a control group, 60 cases in each group. The control group was treated with routine nursing. On the basis of the control group, the observation group was treated with oral administration of Wangbuliuxing decoction, twice a day for 7 days; in addition, massage at breast and acupoint (Zhongfu [LU 1], Yunmen [LU 2], Danzhong [CV 17], 2 min per acupoint per time) was given, twice a day for 7 days. The onset time of lactation, 48-hour postpartum lactation volume, breast swelling and pain, 42-day postpartum breastfeeding were compared between the two groups; the serum levels of prolactin at 48 and 72 h after delivery were measured, and the body mass of newborns at birth and 42 d after delivery were recorded. Results: The onset time of lactation in the observation group was earlier than that in the control group (P<0.05), the 48-hour postpartum lactation volume was higher than the control group (P<0.05), and the breast swelling pain 48 h after delivery was lighter than the control group (P<0.05). Forty-two days after delivery, the success rate of breastfeeding in the observation group was 91.7% (55/60), which was higher than 76.7% (46/60) in the control group (P<0.05). At 48 and 72 h after delivery, the level of serum prolactin in the observation group was higher than that in the control group (P<0.05). On 42 d after delivery, the body mass of newborns in the two groups was higher than that at birth (P<0.05), and that in the observation group was higher than the control group (P<0.05). Conclusion: Wangbuliuxing combined with massage at acupoint and breast could significantly shorten the onset time of lactation, improve the lactation volume, effectively improve breast swelling and pain, increase breastfeeding rate and promote the growth and development of newborns.
