Abstract:Catheter-associated urinary tract infections (CAUTIs) are common and costly hospital-acquired infections, yet they are largely preventable. The greatest modifiable risk factor for developing a CAUTI is duration of catheterization, including initial indwelling catheter placement when it may not otherwise be necessary. Alternatives to indwelling urinary catheters, including intermittent straight catheterization and the use of external catheters, should be considered in applicable patients. If an indwelling urinary catheter is required, aseptic insertion technique and maintenance should be performed. Through the use of collaborative, multidisciplinary intervention efforts, CAUTI rates can be successfully reduced.

Abstract:Nosocomial urinary tract infections account for up to 40% of all hospital-acquired infections with 80% of these associated with the use of urinary catheters. The most important risk factor for infection is duration of catheterization. Clinical practices in catheter management vary widely and frequently are not evidence-based. It is vital that all indwelling catheters should be connected to a sterile closed urinary drainage system and that the connection between the catheter and the urinary drainage system is not broken except for good clinical reasons. Catheter care is a nursing procedure the importance of which is sometimes overlooked and cleansing of the perineum and meatus should be performed twice daily using soap and water.

Abstract:The elderly population is more likely to have urinary catheters used than younger populations. Most patients with short term indwelling catheters (those in place less than 30 days) will be residents of acute care institutions. The frequency of catheter-acquired bacteriuria is greater with longer durations of catheterisation, female gender, failure to maintain a closed drainage system, and in patients not receiving systemic antimicrobials.Systemic antimicrobials given within 48 hours of catheter removal decrease the incidence of urinary infection, but are not currently recommended because of concerns with antimicrobial resistance. Interventions such as topical meatal antimicrobials, disinfectants added to the urinary drainage bag, antimicrobial coatings for catheters, and antimicrobial irrigation have not been shown to decrease the incidence of infection. Asymptomatic bacteriuria should not be treated while the catheter remains in place. However, catheter-acquired bacteriuria should probably be treated following catheter removal.Long term indwelling catheters are most frequently used in elderly individuals resident in long term care institutions. These individuals are always bacteriuric, usually with a complex polymicrobial flora. There are no clinical benefits of treatment of asymptomatic bacteriuria in such persons. In fact, treatment will promote the emergence of resistant organisms. The optimal management of symptomatic infection has not been defined.

Abstract:From 5–10 % of elderly residents of long-term care facilities require chronic indwelling catheters for management of urine voiding. These residents are always bacteriuric, because of biofilm formation along the catheter, and experience increased morbidity associated with urinary tract infection. A wide variety of bacteria or yeast species are isolated. Occasional episodes of symptomatic infection may be accompanied by localizing genitourinary findings. However, when fever is present and there are no localizing findings, symptomatic infection is a diagnosis of exclusion. Many of these episodes are not from a urinary source, so critical clinical evaluation is always necessary. A urine specimen for culture should be obtained from patients with symptomatic infection prior to institution of antimicrobial therapy. When the catheter has been present for 2 weeks or longer, it should be replaced and the urine specimen collected through the new catheter. This provides a specimen of bladder urine without biofilm contamination, and catheter replacement also improves clinical outcomes. Treatment algorithms with a goal of limiting inappropriate treatment of asymptomatic bacteriuria have been developed. Empiric antimicrobial therapy should be avoided when possible. Guidelines for prevention of catheter-acquired urinary infection should be followed. The most important of these is to avoid use of a urinary catheter whenever possible and, when there is no longer an indication for the catheter, to remove it promptly.

Abstract:For more than 3500 years, urinary catheters have been used to drain the bladder when it fails to empty. For people with impaired bladder function and for whom the method is feasible, clean intermittent self-catheterization is the optimal procedure. For those who require an indwelling catheter, whether short- or long-term, the self-retaining Foley catheter is invariably used, as it has been since its introduction nearly 80 years ago, despite the fact that this catheter can cause bacterial colonization, recurrent and chronic infections, bladder stones and septicaemia, damage to the kidneys, the bladder and the urethra, and contribute to the development of antibiotic resistance. In terms of medical, social and economic resources, the burden of urinary retention and incontinence, aggravated by the use of the Foley catheter, is huge. In the UK, the harm resulting from the use of the Foley catheter costs the National Health Service between £1.0-2.5 billion and accounts for ∼2100 deaths per year. Therefore, there is an urgent need for the development of an alternative indwelling catheter system. The research agenda is for the new catheter to be easy and safe to insert, either urethrally or suprapubically, to be retained reliably in the bladder and to be withdrawn easily and safely when necessary, to mimic natural physiology by filling at low pressure and emptying completely without damage to the bladder, and to have control mechanisms appropriate for all users.

Abstract:Urinary catheterization has risks, and its use should be limited. Instrumentation of the urinary tract is responsible for up to 80% of nosocomial urinary tract infections. Other side effects are common including urinary retention when attempting to remove the catheter. Urinary catheterization is not indicated to obtain a urine sample for culture in hospitalized elderly patients with a suspected urinary tract infection who cannot provide a urine specimen on demand. The risks of catheterization are clear and urinary culture results can lead to inappropriate treatment, whereas the physician’s clinical assessment rather than the culture results are most important in directing antibiotic therapy.

Abstract:Catheter-associated urinary tract infection (CAUTI) is common, costly, and causes significant patient morbidity. CAUTIs are associated with hospital pathogens with a high propensity toward antimicrobial resistance. Treatment of asymptomatic patients with CAUTI accounts for excess antimicrobial use in hospitals and should be avoided. Duration of urinary catheterization is the predominant risk for CAUTI; preventive measures directed at limiting placement and early removal of urinary catheters have an impact on decreasing CAUTI rates. The use of bladder bundles and collaboratives, coupled with the support and active engagement from both hospital leaders and followers, seem to help prevent this common problem.

Expert Review of Anticancer Therapy

Abstract:There has been an explosion in the development of microscopic and miniaturized technology over the past decade and we have long awaited their arrival and integration into clinical practice. We have now reached the stage where promises are beginning to be delivered. This article reviews their place in modern medicine and looks toward the future. Miniature camera robots (microrobots) provide a mobile viewing platform, enhancing a surgeon’s view. Nanorobots have arisen from the fictional world of the ‘Fantastic Voyage’ and are finally approaching clinical application. As the targeting and drive forces are further developed, these vehicles could be realistically used for delivery of agents for diagnosis and therapies. These new robots have the potential to further evolve the robotic armamentarium for surgeons.

Abstract:CAUTIs (catheter-associated urinary tract infections) continue to be one of the most common health-care-related illnesses in the entire globe. CAUTIs are the cause of 40% of all hospital-acquired infections and 80% of all nosocomial urinary tract infections (UTIs). A urine catheter is implanted into a high percentage of inpatients at some point during their hospitalization, and indwelling urinary catheter adoption likely to be on the rise. Urinary catheters, made of plastic materials, inhibit the urinary tract's natural defence mechanisms and enhance the bacterial colonization or biofilm formation on the catheter surface, which may cause CAUTIs. It is associated with increased burden of disease, mortality, hospital bills and length of hospital stay. Therefore, to prevent these infections, technological innovations in catheter materials that limit biofilm formation will be required. Unfortunately, many health-care practitioners are unclear of the precise indications for bladder catheterization and accurate CAUTI criteria, which can lead to unnecessary catheterization, antibiotic overuse for asymptomatic bacteriuria and the spread of resistant organisms. As a result, we discuss CAUTIs in general, including definitions, pathophysiology, causation, indications for catheterization and a variety of effective CAUTI-fighting strategies.

Abstract:Study design: Literature review to evaluate the complications seen in patients on intermittent catheterization (IC) and intermittent self-catheterization (ISC).Objectives: To find the prevalence of most complications seen in patients on IC. To study the prevention and the treatment of these complications.Setting: An international literature review.Methods: Most relevant articles on the subject are reviewed.Conclusion: Urinary tract infection is the most frequent complication in patients performing IC. Catheterization frequency and the avoidance of bladder overfilling are amongst the most important prevention measures. Asymptomatic bacteriuria does not need to be treated with antibiotics. Long-term antibacterial prevention does seem to bear a risk of development of bacterial resistance. Previous treatment with indwelling catheters is a risk factor for chronic infection and urinary sepsis. Prostatitis is more frequently present than often thought. Epididymitis and urethritis are rare. Trauma from catheterization occurs regularly, but lasting effects are more limited. However, the prevalence of urethral strictures and false passages increases with longer use of IC. The use of hydrophilic catheters might be able to lower the urethral complication rate but additional proof through comparative studies is needed. The most important prevention measures are good education of all involved in IC, good patient compliance, the use of a proper material and the application of a good catheterization technique.

Abstract:In the long-term catheterized elderly, damage to the epithelial wall is primarily responsible for bladder infections, due to the presence of the catheter as a foreign body and its frequent manipulations. The presence of bacteriuria, a classic sign of infection in the non-catheterized patient, is of little diagnostic importance in the long-term catheterized patient, since it is both permanent and inevitable. Routine preventive measures--eg, frequent catheter changes, prophylactic antibiotics--are of no value. Other measures are worth considering and, if instituted, will reduce the frequency of severe complications from long-term catheterization and improve quality of life.

Abstract:Spinal cord injury can either be complete with no neural communication across the injury level or incomplete with limited communication. Similarly, motor neuron injuries above the sacral spinal cord are classified as upper motor neuron injuries, while those inside the sacral cord are classified as lower motor neuron injuries. Specifically, we provide recommendations regarding the urological management of complete upper motor neuron spinal cord injuries; however, we also make limited comments related to other injuries. The individual with a complete upper motor neuron injury may encounter five lower urinary tract conditions: first, neurogenic detrusor overactivity causing urinary incontinence; second, neurogenic detrusor underactivity resulting in high post-void residual volumes; third, detrusor sphincter dyssynergia, which is contraction of striated and/or smooth muscle urethral sphincters during detrusor contractions; fourth, urinary tract infection; and fifth, autonomic dysreflexia during detrusor contractions, which produces high blood pressure as well as smooth muscle detrusor sphincter dyssynergia. Intermittent catheterization is the recommended urinary management method because it addresses the five lower urinary tract conditions and has good long-term outcomes. This method uses periodic catheterizations to drain the bladder, but also needs bladder inhibitory interventions to prevent urinary incontinence between catheterizations. Primary limitations associated with this management method include difficulties with the multiple catheterizations, side effects of bladder inhibitory medications, and urinary tract infections. Three suggestions to address these concerns include the use of low-friction catheters, wireless, genital-nerve neuromodulation for bladder inhibition, and consideration of urine egress into the urethra as a risk factor for UTI as well as egress treatment. The second management method is reflex voiding. This program uses external condoms for urine collection in males and diapers for females. Suprapubic tapping is used to promote bladder contractions. This method is not recommended because it has high rates of medical complications. In particular, it is associated with high detrusor pressure, which can lead to ureteral reflux and kidney pathology. Botulinum toxin injection into the urethral striated sphincter can manage detrusor sphincter dyssynergia, reduce voiding pressures, and risks to the kidney. We suggest a modified method for botulinum toxin injections as well as five additional methods to improve reflex voiding outcomes. Finally, the use of intermittent catheterization and reflex voiding for individuals with incomplete spinal injuries, lower motor neuron injuries and multiple scleroses are briefly discussed.

Abstract:Previously published guidelines are available that provide comprehensive recommendations for detecting and preventing healthcare-associated infections. The intent of this document is to highlight practical recommendations in a concise format designed to assist acute care hospitals in implementing and prioritizing their catheter-associated urinary tract infection (CAUTI) prevention efforts. Refer to the Society for Healthcare Epidemiology of America/Infectious Diseases Society of America “Compendium of Strategies to Prevent Healthcare-Associated Infections” Executive Summary and Introduction and accompanying editorial for additional discussion. 1. Burden of CAUTIs a. Urinary tract infection is the most common hospital-acquired infection; 80% of these infections are attributable to an indwelling urethral catheter. b. Twelve to sixteen percent of hospital inpatients will have a urinary catheter at some time during their hospital stay. c. The daily risk of acquisition of urinary infection varies from 3% to 7% when an indwelling urethral catheter remains in situ. 2. Outcomes associated with CAUTI a. Urinary tract infection is the most important adverse outcome of urinary catheter use. Bacteremia and sepsis may occur in a small proportion of infected patients. b. Morbidity attributable to any single episode of catheterization is limited, but the high frequency of catheter use in hospitalized patients means that the cumulative burden of CAUTI is substantial. c. Catheter use is also associated with negative outcomes other than infection, including nonbacterial urethral inflammation, urethral strictures, and mechanical trauma.

Abstract:The indwelling urinary catheter is the leading cause of complicated urinary tract infections and Gram-negative bacteraemia in this age group. It accounts for about 40% of life-threatening septicaemia. There is a progressive increase in mortality independently associated with the duration of catheterization. Polymicrobial bacteriuria is common. Urease-producing bacteria lead to encrusted and blocked catheters. The current challenges are to develop effective methods to sensitize healthcare workers to avoid the routine use of indwelling catheters, remove them when no longer needed, develop alternative methods for care of incontinence, employ non-invasive methods to measure urine output, and improve urine drainage systems. The research paradigm needs to focus on prevention of catheter-associated infections rather than on futile attempts to treat irreversible sepsis.

Complications of Chronic Indwelling Urinary Catheters Among Male Nursing Home Patients: A Prospective Study

Abstract:We studied prospectively the incidence of symptomatic infections of presumed urinary tract origin requiring antimicrobial therapy among 54 male nursing home patients with chronic indwelling bladder catheters. During 514 patient-months at risk there were 106 episodes of symptomatic infection, for an incidence of 0.21 per patient-month at risk. Of the patients 80 per cent had at least 1 episode and 48 per cent had 2 or more. None of the clinical factors we examined, including age, nutritional status, stool incontinence, diabetes mellitus, episodes of catheter blockage and the use of chronic suppressant antimicrobial therapy, was associated with the development of symptomatic infection. Further research on host and pathogen-related factors that increase the risk for symptomatic infection, and improvements in infection control and catheter care protocols are necessary to decrease catheter-associated morbidity among male nursing home patients who must be managed by chronic indwelling catheterization.
