Abstract:The aim of this study was to determine clinical efficacy of ulinastatin combined with octreotide in treatment of acute pancreatitis, and its effect on serum endothelin, endotoxin level and immune function. It was an analytical observational study carried out from September 2016 to March 2018. A total of 108 patients with acute pancreatitis were randomly divided into observation group and control group, 54 cases in each group. Control group was additionally treated with octreotide. Observation group was treated with ulinastatin. Therapeutic effects of two groups were compared. The total effective rate in observation group was higher than that in control group (p=0.046). After seven days of treatment, serum endothelin and endotoxin levels in observation group were lower than those in control group (both p<0.001); IgA, IgM and IgG levels in observation group were higher than those in control group (p=0.031, 0.007, and 0.001, respectively). Ulinastatin combined with octreotide can reduce level of endothelin and endotoxin and improve immune function.
Ulinastatin combined with somatostatin enhances disease control and modulates serum inflammatory factors in patients with severe pancreatitis

Abstract:Objective: This study was designed to explore the effects of ulinastatin combined with somatostatin on disease control and serum inflammatory factors in patients with severe pancreatitis. Methods: The data of 80 patients with severe pancreatitis treated in the First Affiliated Hospital of Jiangxi Medical College from May 2020 to April 2022 were analyzed retrospectively. Among them, 36 patients treated with somatostatin alone (3 mg somatostatin added in 50 mL normal saline) on the basis of standard treatment were assigned to a control group, and the other 44 patients treated with both ulinastatin (100,000 U of ulinastatin injection added in 250 mL 5% glucose solution) and somatostatin (3 mg somatostatin added in 50 mL normal saline) were enrolled into a study group. The levels of serum inflammatory factors (interleukin-1β (IL-1β), interleukin-6 (IL-6), and soluble intercellular adhesion molecule-1 (sICAM-1)), biochemical indexes (C-reactive protein, white blood cell count, and serum amylase) and gastrointestinal function indexes (motilin and gastrin) in the two groups were analyzed and compared before and after treatment. Additionally, the alleviation of clinical symptoms, treatment response and occurrence of adverse reactions were compared between the two groups. The mortality rate of patients within 1 month after the treatment was evaluated, and the risk factors affecting the prognosis were analyzed through logistics regression. Results: Before treatment, there was no significant difference between the two groups in the levels of IL-1β, IL-6 and sICAM-1 (P>0.05), while after treatment, the levels of all three factors decreased significantly in both groups (P<0.0001), with more notable decreases in the study group than those in the control group (P<0.0001). Before treatment, the two groups were not significantly different in the levels of C-reactive protein, white blood cell count, and serum amylase (P>0.05), while after treatment, all the three levels decreased notably in both groups (P<0.0001), with notably lower levels in the study group than those in the control group (P<0.0001). Before treatment, the levels of motilin and gastrin in the two groups were not significantly different (P>0.05), while after treatment, motilin increased significantly and gastrin decreased significantly in both groups (P<0.0001), and the study group showed a notably higher motilin level and a notably lower gastrin level than the control group (P<0.0001). The study group experienced a significantly earlier disappearance time of abdominal distension and abdominal pain and a significantly shorter hospitalization time than the control group (P<0.0001). Moreover, the study group showed a notably higher overall response rate than the control group (P=0.029), and presented a notably lower incidence of adverse reactions than the control group (P=0.036). According to univariate analysis, age, onset time, Acute Physiology and Chronic Health Evaluation II score and therapeutic regimen were the factors impacting the patients' prognosis. According to logistics regression analysis, therapeutic regimen was an independent risk factor affecting the prognosis. Conclusion: Compared with somatostatin alone, ulinastatin combined with somatostatin is more effective in the treatment of severe pancreatitis. The combination can substantially alleviate the inflammatory response and improve the gastrointestinal function and clinical symptoms of patients, without increasing adverse reactions. Therefore, ulinastatin combined with somatostatin is worthy of clinical promotion.
Effect of Ulinastatin Combined with Somatostatin on Inflammatory Markers, Hemodynamics and Immune Cells in the Treatment of Severe Pancreatitis

Abstract:Objective: Severe pancreatitis presents a formidable clinical challenge, often associated with high mortality rates and compromised quality of life. This study aimed to assess the efficacy of combining ulinastatin with somatostatin in treating severe pancreatitis, with a focus on improving patient outcomes. Methods: We conducted a study on 98 severe pancreatitis patients at our hospital from January 2022 to March 2023. These patients were randomly divided into two groups: a control group (n=49) treated with somatostatin and an experimental group (n=49) treated with ulinastatin plus somatostatin. The control group received 250 micrograms per hour of somatostatin intravenously for 72 hours. The experimental group received 200 000 units of ulinastatin every 8 hours intravenously, along with the same somatostatin regimen. We compared clinical efficacy, inflammatory markers (TNF-α, CRP, IL-6), hemodynamic parameters (MAP, CVP, HR, SVR), and immune cell function between the groups. Results: Post-treatment, the experimental group showed significant improvements compared to the control group (P < .05) in various parameters. Decreases in AMS, TNF-α, CRP, IL-6, MAP, CVP, and CD8+ T-cells were more pronounced in the experimental group. Notably, AMS levels dropped from 450 U/L to 150 U/L, and TNF-α levels from 55 pg/mL to 20 pg/mL in the experimental group. Conversely, increases in HR, SVR, CD4+ T-cells, CD4+/CD8+ ratio, and NK cell counts were observed. For instance, CD4+ T-cells rose from 300 cells/μL to 500 cells/μL. The experimental group exhibited a higher clinical efficacy rate of 97.96%, compared to 85.71% in the control group. The combined treatment of ulinastatin with somatostatin demonstrated significant effectiveness in improving clinical outcomes compared to the control group. Statistical analysis robustly supported these findings, providing confidence in their reliability. Importantly, the combined therapy showed promise in reducing mortality rates and enhancing the quality of life for patients with severe pancreatitis. Conclusion: The findings of this study hold substantial clinical implications, potentially influencing treatment protocols and patient management strategies for severe pancreatitis. The integration of ulinastatin combined with somatostatin into standard care protocols could significantly improve treatment outcomes and patient prognosis.

Abstract:OBJECTIVE To investigate the curative effect of Xuan Bai Cheng Qi Tang and mirabilite combined with pulse high‐volume hemofiltration (PHVHF) in treatment of patients with severe acute pancreatitis (SAP) ,in or‐der to provide references for clinical treatment of SAP .METHODS A total of 13 SAP cases were selected from Jan . 2015 to Oct .2015 in hospital intensive medicine department .On the basis of conventional western medicine treat‐ment ,the patients were given Xuan Bai Cheng Qi Tang and mirabilite ,enema and external ,and then combined with PHVHF therapy .The changes of life signs ,abdominal symptoms ,blood biochemical indexes ,blood gas anal‐ysis ,acute physiology and chronic health conditions and APACHEII score and Ranson score before and after the treatment of patients were observed .RESULTS The body temperature ,heart rate and respiratory rate of patients were significantly relieved than those before treatment (P<0 .05) .The hemodynamics of the survival of 12 pa‐tients were stable ,and the average arterial pressures were (42 ± 3) and (74 ± 7)mmHg ,which were significantly increased after treatment(P<0 .05) .The serum levels of alanine aminotransferase ,serum amylase and total bili‐rubin were significantly decreased after treatment (P<0 .05) .The arterial oxygen partial pressure ,arterialcarbon dioxide partial pressure and oxygenation index were significantly higher than those before treatment (P< 0 .05) . CONCLUSION The clinical effects of Xuan Bai Cheng Qi Tang and mirabilite combined with PHVHF in treatment of patients with severe acute pancreatitis is significant ,which can be used as one of the methods in treatment of SAP .

Abstract:The intravenous administration of d‐lysergic acid diethylamide (LSD) to pregnant female rabbits induced hyperthermia as well as disaggregation of polysomes in fetal organs and maternal brain. The LSD‐induced polysome shift in maternal brain and fetal organs was not due to an activation of ribonuclease or associated with alterations in the levels of free amino acids. Pretreatment with the receptor blocking agents haloperidol and pizotyline blocked both LSD‐induced polysome shift in maternal brain and fetal organs and LSD‐induced hyperthermia. Fine dissection of adult rabbit brain showed that the extent of LSD‐induced disaggregation of polysomes does not exhibit any marked regional variation.
Therapeutic Effect of Bifidobacterium Combined with Early Enteral Nutrition in the Treatment of Severe Acute Pancreatitis: a Pilot Study

Abstract:OBJECTIVE: To explore the therapeutic effect of Bifidobacterium combined with early enteral nutrition in severe acute pancreatitis. PATIENTS AND METHODS: A total of 60 patients with severe acute pancreatitis admitted from November 2012 to November 2016 were retrospectively analyzed. According to the different treatment methods, the patients were divided into Bifidobacterium combined with early enteral nutrition group (experiment group) and early enteral nutrition group (control group). Serum ALB (albumin), CRP (C-reactive protein), WBC (white blood cell count) and PCT (procalcitonin) levels in both groups were observed. The pain relief time, diet recovery time, length of stay, and hospitalization costs between the two groups, were compared. The APACHE (acute physiology and chronic health evaluation scoring system) II score and SOFA (sequential organ failure assessment) score before and after nutritional support were compared between the two groups. Adverse events and complications were observed as well. RESULTS: 58 patients recovered and 2 died after treatment. Improvements in laboratory indicators such as ALB, CRP, WBC and PCT were much better in the experiment group than the control group (p<0.05). Both the length of days and hospitalization cost were lower in the experiment group than those of the control group (p=0.0029, p=0.0435). In the comparison of hospitalization symptoms, shorter pain relief time and diet recovery time were found in the experiment group than those in the control group (p=0.0003, p=0.0218). After the treatment, APACHE II score and SOFA score of the experiment group were also higher than the control group. No significant differences in adverse events and complications between the two groups were exerted (p>0.05). CONCLUSIONS: Bifidobacterium combined with early enteral nutrition can improve the nutritional status of patients with severe acute pancreatitis in the acute stage, which also enhances the patient's immune capacity and the body's resistance to disease.
Clinical effect of Ulinastatin combined with early continuous renal replacement therapy in the treatment of severe acute pancreatitis and its influence on CRP and IL-6

Abstract:Objective To investigate the clinical curative effect of Ulinastatin combined with early continuous renal replacement therapy in the treatment of severe acute pancreatitis.Methods 60 cases of patients with severe acute pancreatitis treated in Peking University Shougang Hospital from January 2015 to January 2016 were selected and divided into the combined group and the control group by random number table,each group had 30 cases.Patients of the two groups were treated with fasting,continuous gastrointestinal decompression,fluid infusion,somatostatin,anti-inflammatory,improving minicirculation;according to the condition,giving early enteral nutrition;noninvasive or invasive mechanical ventilation respiratory;giving Ulinastatin injection.Patients in the combined group were treated with continuous renal replacement therapy at the same time,continuous treatment of 14 days.The clinical efficacy of the two groups was compared,and the changes of CRP and IL-6 in the two groups before treatment and after treatment for 1,3,7 days were compared and analyzed.Results The total effective rate of the combined group was 93.3％,the total effective rate of the control group was 70.0％,there was statistically significant difference between the two groups (P ＜ 0.05).There were no statistically significant differences in CRP and IL-6 levels before treatment between the two groups (P ＞ 0.05).After treatment for 1,3,7 days,the CRP and IL-6 levels of the two groups were significantly lower than before treatment,and the CRP and IL-6 levels of the combined group were significantly lower than those of the control group,the differences were statistically significant (P ＜ 0.05).Conclusion Ulinastatin combined with continuous renal replacement therapy in the treatment of severe acute pancreatitis can significantly improve the clinical efficacy,improvepatients' inflammatory state by decreasing CRP and IL-6 levels,which is conducive to promote the recovery of the disease.
Effect of Baicalin and Octreotide on the Expression Levels of P‐selectin Protein in Multiple Organs of Rats with Severe Acute Pancreatitis

Abstract:Aim:To investigate the effect of baicalin and octreotide on the expression levels of P-selectin protein in multiple organs of rats with severe acute pancreatitis and explore the underlying mechanism.Methods:Rats were randomly divided into sham-operated, model control, baicalin-treated and octreotide-treated groups. At 3, 6 and 12 h after operation, the mortality rates of rats, the contents of plasma endotoxin as well as serum NO and ET-1, the pathological changes in multiple organs, and the expression levels of P-selectin protein in each group were observed.Results:At 12 h after operation, the mortality rates of rats in treated groups were significantly lower than that in the model control group (P < 0.05), and the pathological severity scores in multiple organs in treated groups were also significantly lower than those in the model control group (P < 0.05). The contents of plasma endotoxin, serum PLA(2) (at 6 and 12 h after operation), ET-1 and NO (at 3 and 12 h after operation) in treated groups were significantly lower than those in the model control group (P < 0.05, P < 0.01 or P < 0.001). In the baicalin-treated group, the expression levels of P-selectin protein in liver (at 3 h after operation), kidney (at 3 and 6 h after operation), pancreas, lung and spleen were significantly lower than those in the model control group (P < 0.01). In the octreotide-treated group, the expression levels of this protein in lung, intestinal mucosa (at 6 and 12 h after operation), lymph nodes (at 3 and 6 h after operation), spleen and thymus were significantly lower than those in the model control group (P < 0.05). Additionally, the products of the staining intensity and positive rate of P-selectin protein in pancreas, spleen (at 3 h after operation), intestinal mucosa (at 6 h after operation), thymus (at 6 h after operation) and lung (at 6 h after operation) in treated groups were significantly lower than those in the model control group (P < 0.05).Conclusion:Both baicalin and octreotide can exert some protective effects on multiple organs and the former is superior to the latter in protecting pancreas. Furthermore, decreasing the expression levels of P-selectin protein in these organs is one of the possible mechanisms.
Ulinastatin Combined with Shenmai Injection for Treating Acute Pancreatitis in 57 Cases

Abstract:Objective To explore the efficacy of ulinastatin combined with Shenmai Injection in treating acute pancreatitis, and to observe the improvement of urine amylase, blood routine and biochemical indexes. Methods 150 patients from June 2014 to June 2015 were selected and divided into the control group and the treatment group according to the sequence of patient records and the wishes of the patients, 75 cases in each group. The control group received ulinastatin treatment (20 U/d, once daily for 5 d;10 U/d, once daily for 5 d), the treatment group received ulinastatin combined with Shenmai Injection treatment ( 50 mL/d, once daily for 10 d ) . After continuous medication for 10 d, the improvement of urine amylase, blood and biochemical indexes case were detected, analyzed and compared. Results Before treatment, the urine amylase, blood biochemical indexes of the two groups showed no statistically significant difference ( P ﹥ 0. 05 ); after treatment, the indexes in both groups significantly decreased ( P ﹤ 0. 05 ) , and the treatment group was significantly lower than the control group ( P ﹤ 0. 05 ) . The total effective rate in the control group was 85. 33%, which was significantly lower than 93. 33% of the treatment group ( P ﹤ 0. 05 ) . The incidence rate of adverse reaction in the treatment group was 9. 33%, which had no statistically significant difference with 8. 00% in the control group ( P ﹥ 0. 05 ) . Conclusion Ulinastatin combined with Shenmai Injec-tion in treating acute pancreatitiscan significantly reduce the patient ’ s urine amylase, blood and biochemical indexes, has better clinical efficacy, which is worthy of clinical promotion.
Effect of Somatostatin and Growth Hormone on Neutrophil Apoptosis in Severe Acute Pancreatitis

Abstract:Objective To investigate the role of neutrophil apoptosis in the mechanism of severe acute pancreatitis,and to observe the effect of somatostatin(SS)and growth hormone(GH)treatment on neutrophil apoptosis.Methods 60 patients with severe acute pancreatitis were randomly divided into three treatment groups:SS group,SS+GH group,and the conventional treatment group. Neutrophil apoptosis was assessed by Annexin-V FITC staining using flow cytometry. Results Neutrophil apoptosis was delayed significantly in severe acute pancreatitis compared with control group. Treatment with SS(P0.05)or SS+GH(P=0.05)could improve neutrophil apoptosis. Significant difference existed among the three treatment groups. Conclusions Delayed neutrophil apoptosis may play a significant role in the development of severe acute pancreatitis,and it is also related with disease severity. Somatostatin and growth hormone can improve neutrophil apoptosis in severe acute pancreatitis.
Effectiveness and safety of probiotics on patients with severe acute pancreatitis: A systematic review and meta-analysis

Abstract:Background: This meta-analysis aimed to assess the efficacy and safety of probiotics in conjunction with early enteral nutrition for the treatment of severe acute pancreatitis (SAP). This study focused on multiple clinical endpoints, including mortality rate, risk of organ failure, and duration of hospital stay. Methods: In accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. The study adhered to the Patient, Intervention, Comparison, Outcome framework and utilized randomized controlled trials to examine the impact of probiotics on patients with SAP. Data extraction and quality assessment were conducted independently by 2 evaluators, with discrepancies resolved collaboratively, or by a third adjudicator. Statistical analyses were performed using chi-square statistics, I2 metrics, and both fixed- and random-effects models, as dictated by heterogeneity levels. Results: The meta-analysis covered 6 randomized controlled trials. Compared to control groups (placebo or standard care without probiotics), probiotics did not significantly reduce mortality rates or organ failure risk. However, they notably shortened hospital stays by a weighted mean difference of -5.49 days (95% confidence interval: -10.40 to -0.58; P = .010). The overall bias risk was low to moderate. Conclusions: Probiotics combined with early enteral nutrition did not significantly improve mortality rates or reduce the risk of organ failure in patients with SAP, but shortened hospital stays. Further studies are required to corroborate these findings.
THE EFFECT OF PERCUTANEOUS CATHETER DRAINAGE COMBINED WITH SOMATOSTATIN ON INFLAMMATION AND PLASMA THROMBOXANE 2, PROSTACYCLIN I2 LEVELS IN PATIENTS WITH SEVERE PANCREATITIS

Abstract:Objective: Explore the effectiveness of ultrasound-guided percutaneous catheter drainage (PCD) combined with somatostatin in the treatment of severe pancreatitis (SAP) patients. Methods: A retrospective study method was adopted to select 95 patients with SAP who were treated in our hospital from January 2018 to June 2022 for clinical research. Among them, 48 patients received routine treatment+somatostatin (control group), and other 47 patients received ultrasound guided PCD treatment on the basis of the control group (research group). The differences in the peripheral white blood cells (WBC), procalcitonin (PCT), interleukin-6 (IL-6), and Tumor Necrosis Factor- α(TNF- α), prostacyclin I2 (PGI2), plasma thromboxane 2 (TXA2), blood amylase, serum albumin (ALB), acute physiological function and chronic health score (APACHE II), sequential organ failure score (SOFA), and clinical efficacy were compared. The ICU treatment time, hospital stay, and incidence of complications were also recorded for the two groups of patients. Results: Before treatment, there was no statistically significant difference in APACHE II score and SOFA score between the research group and the control group (P>0.05); The APACHE II scores of the research group after 14 days of treatment and 28 days of treatment were lower than those of the control group. The SOFA scores of the research group after 28 days of treatment were lower than those of the control group, and the differences were statistically significant (P<0.05); Before treatment, there was no statistically significant difference in WBC, PCT, IL-6, TNF-α, PGI2, TXA2, blood amylase, and ALB levels between the research group and control group (P>0.05); PCT, IL-6, TNF-α, TXA2 and blood amylase levels in the research group after 28 days of treatment were lower than that in the control group, and the differences were statistically significant (P<0.05); The ICU treatment time and hospitalization time of the research group were lower than those of the control group, and the differences were statistically significant (P<0.05); After 28 days of treatment, clinical efficacy evaluation was conducted, and the overall efficacy of the research group patients was better than that of the control group, with statistically significant differences (P<0.05); The complication rate of the research group was 27.66%, and that of the control group was 47.92%, which was significantly lower in the research group than in the control group (P<0.05). Conclusion: Ultrasound guided PCD combined with somatostatin treatment for SAP patients can more effectively alleviate the degree of inflammatory response, effectively alleviate the severity of the patient's condition, reduce the occurrence of related complications, and improve clinical treatment effectiveness.
Ulinastatin in the treatment of severe acute pancreatitis: A single-center randomized controlled trial

Abstract:Background: Severe acute pancreatitis (AP) is one of the most common diseases of the gastrointestinal tract and carries a significant financial burden with high disability and mortality. There are no effective drugs in the clinical management of severe AP, and there is an absence of evidence-based medicine concerning the treatment of severe AP. Aim: To explore whether ulinastatin (UTI) can improve the outcome of severe AP. Methods: The present research included patients who were hospitalized in intensive critical care units (ICUs) after being diagnosed with severe AP. Patients received UTI (400000 IU) or placebos utilizing computer-based random sequencing (in a 1:1 ratio). The primary outcome measures were 7-d mortality, clinical efficacy, inflammatory response, coagulation function, infection, liver function, renal function, and drug-related adverse effects were evaluated. Results: A total of 181 individuals were classified into two groups, namely, the placebo group (n = 90) and the UTI group (n = 91). There were no statistically significant differences in baseline clinical data between the two groups. The 7-d mortality and clinical efficacy in the UTI group were remarkably improved compared with those in the placebo group. UTI can protect against hyperinflammation and improve coagulation dysfunction, infection, liver function, and renal function. UTI patients had markedly decreased hospital stays and hospitalization expenditures compared with the placebo group. Conclusion: The findings from the present research indicated that UTI can improve the clinical outcomes of patients with severe AP and has fewer adverse reactions.

Abstract:Purpose: To investigate the effect of low molecular weight heparins (LMWHs) and ulinastatin on soluble myeloid cells and intestinal mucosal function (IMF) in patients with severe pancreatitis. Methods: A total of 107 patients with severe pancreatitis were divided into two groups: control group (CG, n = 53) and study group (SG, n = 54). The CG was treated with LMWH while SG was similarly treated but in addition received ulinastatin simultaneously. The following parameters were evaluated in the two groups: treatment effects, IMF, time for various indicators to normalize, vascular endothelial function, complication symptoms, T lymphoid subgroup indicators, inflammatory factors, anti-inflammatory factors, soluble B7-H2, and soluble myeloid cell receptor-1 level changes. Results: After treatment, SG showed lower levels of L/M value, DAO and D-lactic acid than in CG (p < 0.05). Gastrointestinal function, leukocytes, amylase, and body temperature in SG had a shorter time to return to normal than in CG (p < 0.05). The levels of IL-10 in SG were higher than in CG, while sB7-H2, TNF-α, sTREM-1 and IL-1 levels were lower than those in the CG (p < 0.05). After treatment, NO levels in SG were higher, but TXB2, vWF and ET levels were lower than in CG (p < 0.05). In addition, CD4+, CD4+/CD8+ indicators were higher and CD8+ lower in SG than in CG (p < 0.05). Conclusion: Ulinastatin + LMWHs improves IMF in patients suffering from severe pancreatitis, shortens the time for various indicators to normalize, and reduces incidence of complications. However, further clinical trials are required to ascertain this therapeutic strategy for the management of severe pancreatitis. An Updated Systematic Review With Meta-analysis Efficacy of Prebiotic, Probiotic, and Synbiotic Treatment of Patients With Severe Acute Pancreatitis

Abstract:Objectives: The aim of this study was to systematically review the clinical outcomes of all randomized controlled trials of patients with severe acute pancreatitis (SAP) and treated with pre/pro/synbiotics. Methods: A systematic literature search of the MEDLINE, Embase, , and the Cochrane Central Register of Controlled Trials was conducted. Eligible studies were randomized controlled trials that evaluated the clinical outcomes of patients with SAP treated with pre/pro/synbiotics. Results: Eleven trials comprising 930 patients were included. Patients treated with pre/pro/synbiotics had a significantly shorter hospital stay [weighted mean difference, -4.33 days; 95% confidence interval (CI), -7.71 to -0.95; P = 0.010; I-2 = 66.9%] compared with control. In a subgroup analysis where only patients classified as SAP were included, those treated with pre/pro/synbiotics had lower risk of single- or multiple-organ failure (relative risk, 0.62; 95% CI, 0.44-0.88; P = 0.995; I-2 = 0.0%) and decreased hospital stay (weighted mean difference, -0.65 days; 95% CI, -0.90 to -0.41; P = 0.121; I-2 = 45.3%) compared with control. Conclusions: Patients with SAP treated with pre/pro/synbiotics did not have a worse clinical outcome and had lower risk of organ failure and duration of hospital stay. Further studies should examine the optimal timing, type, and dosages of these promising treatments.
Effects Of Baicalin On Inflammatory Mediators And Pancreatic Acinar Cell Apoptosis In Rats With Sever Acute Pancreatitis

Abstract:BACKGROUND: To investigate the effects of Baicalin and Octreotide on inflammatory mediators and pancreatic acinar cells apoptosis of rats with severe acute pancreatitis (SAP).METHODS: SD rats were randomly divided into sham operated group (I group), model control group (II group), Baicalin treated group (III group) and Octreotide treated group (IV group). Each group was also divided into subgroup of 3, 6 and 12 h (n = 15). The mortality rate, ascites/body weight ratio as well as the level of endotoxin, NO and ET-I in blood were measured. The pathological severity score of pancreas, apoptotic indexes, and expression levels of Bax and Bcl-2 proteins in each group were investigated.RESULTS: The survival rate of III and IV group has a significant difference compared with II group (P-12 h < 0.05). The ascites volume, contents of inflammatory mediators in blood and pathological severity score of pancreas of III and TV group declined at different degrees compared to II group (P < 0.055 P < 0.01 or P < 0.001). Apoptotic index in III group was significantly higher than that in II group at 3 and 6 h (P-3,P- 6 h < 0.05). Apoptotic index in TV group was significantly higher than that in II group at pancreatic tail at 6 h (P-6 h < 0.05). Expression level of Bax in III group was significantly higher than that in II group (pancreatic head P-3 h,P-6 h < 0.01, pancreatic tail P-3 h < 0.0001).CONCLUSIONS: Compared with Octreotide in the treatment of SAP, the protective mechanisms of Baicalin include reducing the excessive inflammatory mediators' release, inducing the pancreatic acinar cells apoptosis.

Abstract:Objectives: To study the therapeutic effect of early peripancreatic lavage of ulinastatin on severe acute pancreatitis(SAP). Methods: Sixteen pigs were divided into 4 groups: model(SAP), saline lavage(SL), ulinastatin lavage(UL), intravenous ulinastatin(IU). UL and SL group were given peripancreatic lavage of ulinastatin by ultrasound-guided perirenal catheterization and IU group was intravenously instilled with ulinastatin. The multi-organ functions and the inflammatory factors were observed. Results: UL group has the best therapeutic effect. The changes of multi-organ functions and the inflammatory factors were compared with SAP group as follows. In time window of treatment: amylase (p < 0.01), lipase (p < 0.01), ALT (p > 0.05), AST (p < 0.05), CR (p < 0.01), UR (p < 0.01), IL-6 (p < 0.01), IL-10 (p < 0.01). In post-treatment phase: amylase (p < 0.01), lipase (p < 0.01), ALT (p < 0.01), AST (p < 0.01), CR (p <0.05), UR (p > 0.05), IL-6 (p <0.01), IL-10 (p <0.01). Conclusions: Early peripancreatic lavage of ulinastatin in SAP could effectively improve the multi-organ functions and inflammatory response. (C) 2018 IAP and EPC. Published by Elsevier B.V.

Abstract:The aim of the present study was to investigate the effect of peritoneal lavage with ulinastatin on the expression levels of nuclear factor κB (NF-κB) and tumor necrosis factor (TNF)-α in multiple organs of rats with severe acute pancreatitis (SAP). Male Wistar rats were randomly divided into the following groups: Sham-operated (C), SAP model (SAP), saline lavage (SL), intravenous ulinastatin (IU) and peritoneal lavage with ulinastatin (UL). The SAP model was induced by the retrograde infusion of 5% sodium taurocholate into the biliopancreatic ducts of the rats. Intraperitoneal lavage or injection was performed immediately following the establishment of the SAP model in groups SL, IU and UL. Intraperitoneal lavage with or without ulinastatin was performed for 3 h. The survival time of half of the rats in each group was recorded over a 12-h period. At 3 h after the induction of SAP, inflammatory mediators and the expression levels of NF-κB and TNF-α in multiple organs of the rats in each group were also detected. The survival rates of the rats in group UL at 6 h and 9 h were increased compared with those in group SAP, and were also higher than that in groups SL and IU. The levels of serum inflammatory mediators were effectively reduced in groups SL, IU and UL, the greatest effects were observed in group UL. The expression levels of NF-κB and TNF-α in multiple organs were significantly lower in group UL compared with other groups. Intraperitoneal lavage with ulinastatin significantly ameliorated the inflammatory reaction and inhibited NF-κB and TNF-α expression in multiple organs of SAP model rats.

Abstract:OBJECTIVE To investigate the changes of levels of WBC ,CRP ,and endotoxin of severe acute pancrea‐titis patients complicated with intestinal infections and observe the clinical effect of enteral nutrition support so as to provide guidance for early control of the intestinal infections .METHODS A total of 64 severe acute pancreatitis patients complicated with intestinal infections who were treated in hospitals from Apr 2011 to May 2016 were re‐cruited as the study objects and were given enteral nutrition support therapy after admission to hospitals .The lev‐els of WBC ,CRP ,and endotoxin were compared before and after the treatment ,and the cellular immune function indexes and nutrition‐related indexes were observed and compared before and after the treatment .RESULTS The levels of WBC ,CRP ,and endotoxin of the patients complicated with intestinal infection were respectively (15 .05 ± 1 .58)× 109/L ,(101 .27 ± 18 .29)mg/L ,and (0 .45 ± 0 .14)EU/ml after the enteral nutrition support therapy and were respectively (18 .22 ± 2 .36) × 109/L ,(165 .24 ± 31 .52)mg/L ,and (0 .97 ± 0 .23)EU/ml before the treatment ,and there was significant difference (P<0 .05) .The levels of CD3 ,CD4 ,and CD4/CD8 of the patients complicated with intestinal infection were respectively (71 .03 ± 4 .24)% ,(41 .57 ± 4 .46)% ,and 1 .68 ± 0 .33 after the treatment ,significantly higher than (61 .25 ± 5 .48)% ,(30 .14 ± 3 .18)% ,and 1 .12 ± 0 .24 before the treat‐ment ;the CD8 level was (25 .73 ± 2 .58)% after the treatment ,significantly lower than (28 .16 ± 3 .25)% before the treatment(P<0 .05) .The levels of ALB ,PA ,and TF of the patients with intestinal infection were respective‐ly (36 .28 ± 5 .49)g/L ,(331 .58 ± 41 .28)mg/L ,and (3 .24 ± 0 .76)g/L after the treatment ,higher than (31 .47 ± 4 .21)g/L ,(221 .45 ± 28 .16)mg/L ,and (2 .13 ± 0 .51)g/L before the treatment ,and the nutrition‐related indexes were remarkably elevated (P<0 .05) .CONCLUSION The levels of WBC ,CRP ,and endotoxin of the patients with severe acute pancreatitis are remarkably elevated when they are complicated with intestinal infection .The enteral nutrition support therapy may reduce the levels of WBC ,CRP ,and endotoxin and remarkably improve the cellular immune function and nutrition status ,and it plays a positive role in alleviating the intestinal infection symptom and relieving the patients′suffering .
Ulinastatin Enhances the Therapeutic Effect of Intraperitoneal Lavage on Severe Acute Pancreatitis in Rats

Abstract:The present study investigated the therapeutic effect of peritoneal lavage with ulinastatin on the outcome of rats with severe acute pancreatitis (SAP). A total of 110 male Wistar rats were randomly divided into the following groups: Control (C), SAP model (SAP), saline lavage (SL), intravenous ulinastatin (IU) and low-dose (LUL), medium-dose (MUL), high-dose (HUL) and ultrahigh-dose (UHUL) ulinastatin lavage. The treatments were performed immediately subsequent to the establishment of the SAP model. Intraperitoneal lavage with or without ulinastatin was performed for 3 h. Survival time was recorded for 12 h and the median survival time was calculated. Histopathological analyses, and measurements of plasma amylase and lipase levels were performed. Blood pH, lactic acid and base excess were also detected. The LUL, MUL, HUL, UHUL and IU groups showed an increase in the median survival time compared with the SAP group, with the maximal effect observed in the MUL group (P<0.01). The SL, MUL and IU groups showed a reduced activity of amylase and lipase compared with the SAP group. The SL (P<0.01) and the MUL groups (P<0.01) additionally showed a reduction in the lactic acid in arterial blood relative to the SAP group but the IU group did not. The MUL group showed greater improvements in pH (P<0.01) and base excess (P<0.01) versus the SAP group than the SL and IU groups. Furthermore the MUL group demonstrated a more marked reduction in the histological changes in necrosis, edema and inflammation compared with the SL and IU groups. Intraperitoneal lavage with ulinastatin significantly improves the prognosis of SAP in rats.
