Abstract:A 38-year-old man without a severe traumatic history reported to the outpatient department (OPD) with wrist pain at the dorsal aspect, mild swelling, stiffness, and restricted mobility at the left wrist joint. The patient had been experiencing these symptoms for a year. There was sharp tenderness, graded as 4 above the lunate bone, on examination of the left wrist joint. Advanced imaging, which is magnetic resonance imaging (MRI), and radiographs suggested that the patient had Kienbock's illness. Typically, the surgical approach for Kienbock's used is wrist fusion or proximal row carpectomy. However, in this case, a novel strategy of bone grafting, scaphoid-capitate fusion, and lunate excision was adopted. This case report explains the outcome of our scaphoid-capitate arthrodesis, which was done to maintain functional mobility and relieve discomfort by halting the progression of carpal collapse and carpal-ulnar translation.

Abstract:Kienbock's disease, or osteonecrosis of the lunate, can lead to chronic, debilitating wrist pain. Etiologic factors include vascular and skeletal variations combined with trauma or repetitive loading. In stage I Kienbock's disease, plain radiographs appear normal, and bone scintigraphy or magnetic resonance imaging is required for diagnosis. Initial treatment is nonoperative. In stage II, sclerosis of the lunate, compression fracture, and/or early collapse of the radial border of the lunate may appear. In stage IIIA, there is more severe lunate collapse. Because the remainder of the carpus is still uninvolved, treatment in stages II and IIIA involves attempts at revascularization of the lunate-either directly (with vascularized bone grafting) or indirectly (by unloading the lunate). Radial shortening in wrists with negative ulnar variance and capitate shortening or radial-wedge osteotomy in wrists with neutral or positive ulnar variance can be performed alone or with vascularized bone grafting. In stage IIIB, palmar rotation of the scaphoid and proximal migration of the capitate occur, and treatment addresses the carpal collapse. Surgical options include scaphotrapeziotrapezoid or scaphocapitate arthrodesis to correct scaphoid hyperflexion. In stage IV, degenerative changes are present at the midcarpal joint, the radiocarpal joint, or both. Treatment options include proximal-row carpectomy and wrist arthrodesis.

Examining the Efficacy of Arthroscopic Scaphocapitate Arthrodesis for Advanced Kienbock's Disease: Clinical and Radiological Outcomes

Abstract:Background: Altering wrist biomechanics, Kienbock's disease leads to progressive carpal collapse that results in early arthritis and degenerative changes. By shifting the loading axis toward the radioscaphoid joint, scaphocapitate arthrodesis (SCA) has been reported as a salvage procedure effective in treating symptomatic patients with advanced Kienbock's disease. In this study, we aimed to evaluate the clinical and radiological outcomes of arthroscopic SCA in symptomatic patients with advanced stages of Kienbock's disease. Methods: Between March 2010 and February 2021, we included 15 patients with symptomatic stage IIIA (n=2) and stage IIIB (n=13) Kienbock's disease who were followed up for a minimum of 24 months after arthroscopic SCA with or without lunate excision. The lunate was excised in 6 patients and retained in 9. Visual analog scale (VAS) pain score, grip strength, range of motion (ROM), active flexion-extension arc, and modified Mayo wrist score (MMWS) were measured preoperatively and at each follow-up examination after surgery. Operation-related complications and radiographic changes were also assessed. Results: There were 13 women and 2 men, with a mean age of 57.6 years (range, 21-74 years) at the time of undergoing arthroscopic SCA. Follow-up ranged from 24 to 116 months, with an average of 56.9 ± 32.3 months. Bony union was achieved in all patients. At preoperative examination, wrist ROM (67%) and grip strength (48%) significantly decreased, compared to the contralateral wrist. At the final follow-up, there were significant improvements in VAS, grip strength, and MMWS, whereas the active wrist ROM showed no significant change. Radioscaphoid angle recovered after surgery, while radiographic carpal collapse and ulnar translation of the carpus occurred. In subgroup analysis according to excision of the lunate, there were no significant differences in VAS, MMWS, grip strength, or total ROM. However, increased ulnar translation and decreased radial deviation were noted in the lunate excision group. Conclusions: Arthroscopic SCA achieved significant improvements in pain and wrist function in patients with advanced Kienbock's disease without any complications. Excision of the lunate when performing arthroscopic SCA seemed to induce progressive carpal ulnar translation, with no apparent clinical benefits over retaining it.

Abstract:Kienbock's disease refers is avascular necrosis of lunate bone also called lunato malacia. It is a rare disease with a prevalence of about 0.0066% Golay [1]. The disease was first described by Austrian radiologist Kienbock in 1910 Wagner and Chung [2]. The disorder usually limits wrist movements may cause pain-swelling in the wrist and is rarely seen in both wrists. It most commonly affects men between the ages of 20 to 40┬Āyears [3, 4]. The aetiology of the disease is not very clear but main factors which can be attributed are:- trauma, uneven bones in forearm, irregular lunate bone, underlying medical condition like lupus, sickle cell anaemia and cerebral palsy.

Abstract:Kienbock disease, or avascular necrosis of the lunate, is an uncommon cause of a painful and stiff wrist. Management options range from conservative treatment in the form of immobilization and corticosteroid injections to a wide variety of surgical treatments that depend on the structural integrity of the lunate, intercarpal relationships, and the condition of the articular cartilage of the wrist. A particularly difficult problem lies in the management of young patient in whom vascularized bone grafting of the lunate has failed but in whom arthritis has not yet developed. Pyrocarbon lunate implant arthroplasty is a newer treatment option for such a patient, and allows the preservation of the remainder of the proximal carpal row while directly addressing the degenerative lunate. This article describes the evidence and surgical technique for lunate implant arthroplasty and presents an illustrative case example. Dr. Chung receives funding from the National Institutes of Health and book royalties from Wolters Kluwer and Elsevier, and a research grant from Sonex to study carpal tunnel outcomes. Abstract:母乳で育てられた乳児は病気に罹りにくく最適であることが明らかになり,母乳哺育の重要性が再認識されてきた.平成2年度の母乳哺育状況を見ると,1ヶ月児で母乳のみの乳児は44.1 %,混合栄養児は42.8%,3ヶ月児で母乳のみが37.5%,混合栄養児は29.4%1)である. 母乳成分は同一人でも日内変動はもとより母親の栄養状態,泌乳期,年齢,在胎,授乳時のどの時点で採取したかなどによって影響されるが,母乳の脂肪酸組成は母親が摂取する食事内容に大きく影響される2,3).近年の食生活は, 西欧化とともに社会及び経済的変化による都市化や核家族化および外食産業等の発展により油脂類の消費を拡大した.この脂質摂取の増加は単に量だけでなくその内容の変化も伴なっている.さらに,ファミリーレストランの隆盛あるいはファーストフードや加工食品などの消費が増加した.これは低原価のマーガリンやショートニングおよび食用精製加工油脂の多量の供給より成り立っていて,使用されている食用加工油脂の多くは水素添加加工したものであり,このような食品にはトランス型不飽和脂肪酸(以下トラソス酸という)が含まれている4~6).そして人間の母乳にも本来天然の脂質に存在しないトランス酸が常に少量検出されている7~9). 母乳の脂質および脂肪酸組成については多くの報告があり10,11),日本人の母乳の脂肪酸組成についても詳細な報告が見られるが12),トランス酸についての報告は少なく,トランス酸摂取による生体への評価や乳児にとって重要な栄養源である母乳中のトランス酸の評価が未だ必ずしも明確にされていない. 本報は著者らの母乳脂質の実験研究を踏まえ近年の日本人の脂質摂取の現状,トランス型不飽和脂肪酸の構造,母乳に含まれるトランス酸, トランス酸が生体に与える影響について述べ, 乳児にとって重要な栄養源である母乳成分のトランス酸の評価を最近の文献と比較しながら検討する.これは臨床看護で現状の母乳哺育を維持したり,妊産褥婦に母乳哺育を推進する上で重要だと考えている.
Capitate Osteotomy and Transposition for Type III Kienböck's Disease.

Abstract:The aim of this study was to assess the results of capitate osteotomy and transposition for stage III Kienbock's disease. Capitate osteotomy and transposition combined with an autologous iliac bone graft was carried out in 17 patients. At the final follow-up for a mean of 68 months (range 16-127 months) after surgery, the mean visual analogue scale score was 0.6 (range 0-5). The mean Wrightington wrist function score was 8. The mean grip strength was 79% of the unaffected side. There were 16 satisfactory results. The one unsatisfactory result occurred in a woman who developed a nonunion of the osteotomy. There were no other complications of the surgery. Our results show that capitate osteotomy and transposition is a simple and reliable method for the management of stage III Kienbock's disease.

Abstract:Vascularized bone grafts have shown favorable outcomes in Kienbock's disease, preventing the progression of lunate collapse and avascular necrosis. Here we describe our experience using a 4 + 5 extensor compartmental artery (ECA) vascularized bone graft combined with K-wire fixation. Between September 2010 and June 2013, 9 patients with Lichtman stage II-IIIA disease underwent arthroscopy prior to 4 + 5 ECA graft placement combined with temporary fixation (scaphocapitate and triquetrum-capitate joints). The average follow-up was 69 months (range, 51-92 months). Changes in pain, range of motion, grip strength, and pinch strength were analyzed. All patients had satisfactory recovery, especially pain relief and grip strength improvement (both P < 0.01). Furthermore, magnetic resonance imaging follow-up was critical for monitoring lunate revascularization, especially in the early postoperative period. (C) 2020 SFCM. Published by Elsevier Masson SAS. Scaphocapitate arthrodesis with lunate preservation for Kienböck's disease: prospective outcomes study

Abstract:Purpose: The study evaluated the efficacy of SC arthrodesis with lunate preservation for treating patients diagnosed with stage IIIB or IIIC Kienböck's disease, who also exhibit neutral ulnar variance. The study further aimed to explore potential variations in outcomes between patients diagnosed with stage IIIB and IIIC Kienböck's disease. Methods: Thirty-two patients diagnosed with stage IIIB (n = 19) and stage IIIC (n = 13) Kienböck's disease underwent SC arthrodesis with distal radius bone grafting stabilised by Herbert compression screws. All participants underwent pre- and post-operative assessments including VAS score for pain, ROM, grip strength, MMWS, and the Quick DASH score. Additionally, RS angle, LHI ratio, and CHI ratio were assessed. Results: For all patients, the mean operative time was 73 min, follow-up was 45.6 months, time to union was 14 weeks, and time to full return to work was 24 weeks. The rate of union at the arthrodesis site was 91% (29 out of 32 patients) whilst the incidence of postoperative degenerative arthritis was 36% (8 out of 32 patients). Regarding changes in the means of outcomes from pre- to post-operatively, the VAS score decreased from 8.2 to 1.3 and grip strength improved from 36 to 79%. The RS angle was corrected from 59° to 50°. Significant improvements were noted in the mean MMWS from 45 to 75 and QuickDASH score from 78 to 21. However, no significant changes were observed in ROM, LHI, and CHI. There were no significant differences between patients with stage IIIB and stage IIIC in terms of these parameters, except for differences observed in the RS angle, LHI, and CHI preoperatively and in LHI and CHI postoperatively. Conclusion: Evidence level: II. Our research demonstrates that SC arthrodesis is a valuable approach for reducing pain, improving grip strength, and enhancing overall function in individuals with advanced Kienböck's disease. Importantly, our results indicate no notable differences in outcomes between patients diagnosed with stage IIIB or IIIC Kienböck's disease.
Medial Femoral Trochlea Osteochondral Flap Reconstruction of the Previously Unsalvageable Kienbock-Associated Lunate With a Coronal Split

Abstract:Purpose: Patients with Kienbock disease can present with coronal fracture and collapse of the proximal lunate (Unified B2/Bain grade 1, 2B). Traditionally, this was considered unsalvageable. However, medial femoral trochlea (MFT) osteochondral reconstruction, used to recreate the proximal lunate surface, can be paired with reduction and repair of the coronal plane fracture, thus restoring midcarpal congruity. The purpose of this study was to report radiographic and clinical outcomes following lunate coronal fracture fixation at the time of MFT osteochondral reconstruction. Methods: This was a retrospective study performed at a single institution. We identified patients with Kienbock disease who underwent MFT osteochondral reconstruction from 2014 to 2023. Patients were included if they had a coronal fracture of the lunate distal articular surface fixed at the time of surgery. Radiographic and clinical parameters were evaluated, including carpal height ratio, union rate, presence of heterotopic ossification, need for revision surgery, and patient-reported outcome measures. Results: Thirty-three patients were included, with a mean age of 27.5 years (range: 15-41); 19 (58%) were women. Mean radiographic follow-up time was 5.8 months, and mean clinical follow-up time was 22.6 months. Union was achieved in 30/33 patients (91%). Carpal height ratio improved from 1.32 to 1.4. Two patients (6%) required reoperation, one for removal of heterotopic ossification and another for conversion to proximal row carpectomy. Patients demonstrated meaningful improvement in brief Michigan Hand Questionnaire and Patient-Reported Outcomes Measurement Information Upper Extremity scores. Range of motion before and after surgery was similar. Conclusions: Lunate coronal fracture fixation with MFT osteochondral reconstruction represents an additional management option in select patients with Kienbock disease. This technique restores the midcarpal joint during lunate reconstruction and may allow patients to avoid salvage procedures. Early radiographic and clinical outcomes are promising. Abstract:Background: This study evaluated the radiological and clinical outcomes of bone grafts using fourth and fifth extensor compartmental arteries (4+5 ECAs) for the treatment of Kienböck's disease. Materials and Methods: In total, 21 patients (12 men and 9 women; mean age, 41 years; range, 19–59 years) were followed for a mean of 33 months. Radiological images were analyzed for the Lichtman stage, carpal height ratio, radioscaphoid angle, and Stahl's index. Clinical evaluation included range of motion, visual analog scale (VAS) score, grip strength, modified Mayo wrist score (MMWS), Lichtman outcome score, and Disabilities of the Arm, Shoulder and Hand (DASH) score. At the time of surgery, 6, 14, and 1 patients had Lichtman stages II, IIIA, and IIIB, respectively. Results: At the final follow-up visit, grip strength had improved from 65.4% to 79.7%, wrist extension had improved from 43° to 57°, and flexion had improved from 42° to 50°. There were no significant changes in the carpal height ratio, Stahl's index, or radioscaphoid angle. The mean VAS score was 1.7, and the mean DASH score was 6.9. The mean MMWS was 87.9, with excellent and good outcomes in 6 and 11 patients, respectively. Satisfactory Lichtman outcome scores were observed in 81%. Body mass index had a strong correlation and age had a weak correlation with MMWS (coefficient=−0.534, P =.013, and coefficient=−0.393, P =.078, respectively). Conclusion: The 4+5 ECA bone graft is effective for the treatment of Kienböck's disease in young patients with low body mass index. [ Orthopedics . 202x;4x(x)xx–xx.]

Abstract:Objective To investigate the outcomes of advanced Kienbock' s diseases treated by vascularized pedicled distal radius bone flap transfer.Method Two patients with advanced Kienbock's diseases ( type Ⅴ) underwent distal radius bone flap transfer pedicled by dorsal branches of the radial artery.The observations included pain evaluation,radiography,function assessment of the wrist ( range of motion and Krimmer scale),and quality of life ( DASH questionnaire).Results Pain disappeared after surgery in both patients at rest,and reduced in intensity to a mild level during wrist motion.In one patient,X-ray showed the high density image of the lunate had disappered and now bone had formed.In the other patient,there was no aggravation in X-ray or MRI.According to the assessment of hand function,the Krimmer score and DASH score were higher comparing with that assessed before the operation.Conclusion Distal radius bone flap transfer pedicled by dorsal branches of the radial artery is a feasible procedure to treat advanced Kienbock's diseases,with favorable preliminary results.
Simplifying the Decision-Making Process in the Treatment of Kienböck's Disease

Abstract:Background In recent years, the classification and treatment algorithm for adult Kienböck's disease (KD) has expanded. However, the priority of the investigations done in determining its management has not been discussed, as not every patient with KD requires magnetic resonance imaging (MRI) or wrist arthroscopy. Materials and Methods We discuss the role of these investigations and emphasize the importance of computed tomography (CT) imaging in evaluating the cortical integrity of the lunate and its role in the decision-making process and management of KD. Results We put forward an investigative algorithm that places into context the investigative roles of MRI, arthroscopy, and CT. Conclusion KD is a rare condition, and there is a lack of comparative studies to help us choose the preferred treatment. The decision on the management options in adult KD may be made by determining the integrity of the lunate cortex and deciding whether the lunate is salvageable or not by CT scan. MRI may provide useful information on the vascular status if the lunate cortex is intact, and the lunate is salvageable. If the lunate is fragmented, it is not salvageable, and MRI does not provide useful information. Arthroscopy has a role in selective cases.
Vascularized Capitate Transposition for the Treatment of Stage IIIB Kienböck Disease

Abstract:Purpose: The treatment of Kienböck disease (KD) continues to be controversial. In this study, we report the long-term follow-up outcomes of patients who were diagnosed with stage IIIB KD treated with vascularized capitate transposition. Methods: A total of 16 patients were retrospectively reviewed. Baseline clinical information was extracted from medical records, and wrist function was clinically evaluated, including x-ray images. Results: At the final follow-up, wrist pain was severe in 0 patients, moderate in 2 patients, mild in 5 patients, and absent in 9 patients. The mean postoperative active flexion and extension of the affected wrist was significantly improved after surgery compared with before surgery. The postoperative and preoperative mean grip strength was 35 kg and 27 kg, respectively. The Disabilities of the Arm, Shoulder, and Hand score was significantly improved after surgery compared with before surgery. Conclusions: Vascularized capitate transposition for the treatment of Lichtman stage IIIB KD is feasible and associated with improvements in wrist function and pain. Abstract:Kienböck disease, a rare disease that can cause chronic pain and motor dysfunction, occurs due to avascular necrosis of the lunate bone, which leads to dislocation of the carpal bone. Among various other etiologies, Kienböck disease can cause closed tendon rupture of the finger. In this report, we introduce a case of total rupture of the second extensor digitorum communis and the extensor indicis proprius tendons caused by undiagnosed Kienböck disease in an elderly female patient.
Unexpected Functional Diversity among FadR Fatty Acid Transcriptional Regulatory Proteins*

Abstract:The FadR protein of Escherichia coli has been shown to play a dual role in transcription of the genes of bacterial fatty acid metabolism. The protein acts as a repressor of β-oxidation and an activator of unsaturated fatty acid synthesis. FadR DNA binding is antagonized by long chain acyl-CoAs, and thus FadR acts as a sensor of fatty acid availability in the environment. When viewed from a genomic viewpoint, FadR proteins are unusual in that the DNA binding domain is very highly conserved among FadR-containing bacteria, whereas the C-terminal acyl-CoA binding domain shows only weak conservation. To further our understanding of the role of FadR in bacterial lipid metabolism we have examined the in vivo and in vitro properties of a diverse set of FadR proteins expressed in E. coli. In addition to E. coli FadR the proteins examined were those of Salmonella enterica, Vibrio cholerae, Pasteurella multocida, and Haemophilus influenzae. These FadR proteins were found to differ markedly in their effects on repression and induction of β-oxidation in E. coli and in their acyl-CoA binding abilities as measured by isothermal titration calorimetry. The E. coli and S. enterica proteins were the most similar, although they differed in their effects on utilization of oleic acid and acyl-CoA binding affinities, whereas the P. multocida and H. influenzae proteins showed only weak repression and poor acyl-CoA binding affinities. The V. cholerae FadR was strikingly superior to the other proteins in the amplitude of its regulatory response, and it bound long chain acyl-CoAs appreciably more strongly than the E. coli and S. enterica proteins. The significance of these findings is discussed in view of the protein sequences and the physiological niches occupied by these organisms.

Abstract:Cases of Kienböck's disease combined with gout are rare; to the best of our knowledge, the current report is the third such case reported in the literature. The current report presents the case of a 49-year-old male patient with Kienböck's disease combined with gout. The patient had experienced swelling and pain of their right wrist for 1 month. Through a combination of clinical history, physical examination, laboratory results, radiological and intraoperative findings, the patient was diagnosed with stage IIIB-IV Kienböck's disease and gout. The patient recovered well following a synovectomy and medication for gout. The rarity of the present case makes it difficult to examine the potential association between gout and Kienböck's disease. Kienböck's disease combined with gout tends to be ignored and is easily misdiagnosed. However, the surgical procedure selection for Kienböck's disease combined with gout is markedly different from the conventional surgery for Kienböck's disease or gout alone. The current study outlines a successful treatment strategy for gout combined with Kienbock's disease.

Abstract:Background: Scaphocapitate arthrodesis (SCA) is a described technique for chronic wrist pathology such as Kienbock disease and carpal instability. The technique aims to preserve motion, while maintaining carpal height, preserving the radioscaphoid articulation, and offloading the lunate. Limited case series evaluate the outcomes of this previously described technique. We aim to determine if the collective, updated literature on outcomes of SCA support its continued application by the wrist surgeon. Furthermore, we investigated if any technical variations offer an advantage in improving patient-reported outcomes after SCA. Methods: A literature search was performed using Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. Articles selected for critical review were examined for patient demographic data, functional outcomes, radiographic outcomes, and complications. A qualitative analysis was then performed to synthesize the available date. Results: A total of 18 unique articles published between 1991 and 2022 were identified, representing a total of 285 individual cases of SCA. Nonunion rate ranged 0%-22%. Preoperative visual analog scale pain scores ranged from 3.2 to 10, whereas postoperative visual analog scale pain scores ranged from 0 to 4. Patient-reported outcomes are reported by validated instruments, including the Disabilities of the Arm, Shoulder, and Hand, QuickDASH, Patient-rated Wrist Evaluation, and Mayo Wrist Score. Conclusions: SCA is a viable treatment option for patients with Kienbock disease. SCA offers reliable improvements in patient pain, grip strength, and patient-reported outcomes.
Radiography versus computed tomography for osseous staging of Kienböck's disease

Abstract:Journal of Hand Surgery (European Volume), Ahead of Print. This retrospective study compared the diagnostic accuracy of conventional radiography (CR) and computed tomography (CT) in classifying Kienböck's disease (KD) according to Lichtman staging. Besides age, cartilage condition and vascularity, bone morphology is important in choosing the most appropriate treatment in KD. In 278 patients, two musculoskeletal radiologists performed a consensus reading of 281 lunate necroses according to the Lichtman classification, based on CR and CT. Stage IIIc was most common in CR (43%) and CT (61%), followed by stage IIIa for CR (25%) and stage IV for CT (15%). In 67%, KD was classified as the same in both modalities. The KD stage had to be upgraded in CT in 34% of cases, notably with stage IIIa progressing to IIIc in 24 out of 69 patients and stage IIIb to IIIc in 23 out of 38 cases. Overall, the Lichtman score was significantly higher on CT. The findings suggest that CT is better than CR in accurately determining the stage of KD stage, indicating a potential shift towards more surgical salvage procedures.Level of evidence: III

Abstract:A carpal injury called a lunate fracture can cause severe carpal instability if treatment is not received. After the scaphoid, triquetrum, and trapezium, the lunate is the fourth most frequently fractured carpal bone. Due to lunate fracture, the functional prognosis is uncertain, and conservative treatment frequently results in surgery. Lunate fracture may be caused by anatomical features such as ulnar and radial variation, although necrosis is not commonly one of them. Vascularization in the lunate is unstable and dependent on the capsular arterioles. High shear stresses are frequently applied to the lunate, which rubs against the triangular fibrocartilage complex and the radius. This could lead to a fracture. Intraosseous compartment syndrome is most likely caused by the inflammatory condition Kienböck's disease. In order to protect the lunate against shear and compression loads, surgery entails decompressing the lunate. The consequences of current osteotomy techniques on biomechanics will be discussed. While some osteotomies may lessen the stresses conveyed to the lunate, they may also put too much strain on the ulnar side of the lunate. Techniques for treating wrist degeneration comprise extra-articular methods that keep the mid-carpal joint's architecture and vascularization intact. Bone grafting or repair may be indicated if lunate destruction is extensive. In the last phases, palliative methods are employed. For Kienböck's disease, there are currently no efficient biological therapies. The lunate is prevented from collapsing by decompression osteotomies, giving time for natural healing. The patient came to Acharya Vinoba Bhave Rural Hospital Outpatient Department of Orthopedics with a complaint of pain in her right wrist. At a private hospital in Amravati, the patient had gone through a proximal row carpectomy on the right side. After undergoing post-operative physiotherapy, the patient showed good improvement in her functional activities and quality of life. Physiotherapy helps in gaining back the functional activities for the post-operated lunate fracture patient.
