Abstract:Introduction and importance: Splenic epidermoid cysts are rare primary congenital cysts composed of an epithelial lining that represent a small proportion of nonparasitic splenic cysts. Despite their infrequency, there is a lack of uniform diagnostic and treatment guidelines for these cysts, emphasizing the need for further research and standardized reporting. Case presentation: A 45-year-old female presented with left upper quadrant abdominal pain, characterized by a pressing sensation. Physical examination revealed a palpable mass in the upper left quadrant. Laboratory test results were normal. Abdominal ultrasonography and CT tomography confirmed a large splenic unilocular cyst consistent with an epidermoid cyst. Surgical intervention involved aspiration of the cystic fluid, partial splenectomy, and pathological examination. The patient's postoperative recovery was uneventful. Clinical discussion: The origin of splenic epidermoid cysts remains unclear, with theories suggesting entrapment of mesothelial cells during embryonic development. The clinical presentation varies with cyst size, often causing abdominal pain and a palpable mass. Diagnostic modalities include ultrasonography and CT scans. Surgical intervention is recommended for symptomatic or suspicious cysts to prevent complications. The chosen approach depends on cyst characteristics and patient factors. This case highlights the challenges and considerations in managing splenic epidermoid the cysts and emphasizes the need for individualized treatment approaches. Conclusion: This case contributes to the understanding of splenic epidermoid cysts and demonstrates a successful subtotal splenectomy as a treatment approach. Further research and standardized guidelines are essential to improve the management of these rare lesions and to provide better insights into their etiology and optimal treatment strategies.

Although some spleen-preserving approaches have been reportedly used, splenic cyst recurrence usually occurs in true cyst cases, wherein the cyst is incompletely removed. In our case, partial splenectomy was performed and the giant cyst was completely removed.

Abstract:Introduction: An epidermoid cyst in an intrapancreatic accessory spleen (ECIPAS) is a rare non-neoplastic cyst, typically occurring in the pancreatic tail. It is difficult to preoperatively differentiate ECIPAS from other types of pancreatic neoplastic cysts. Case presentation: We herein report a case of a 32-year-old man with a cystic tumor in the tail of the pancreas. The patient underwent a laparoscopic spleen-preserving distal pancreatectomy, and histological examination revealed the presence of ECIPAS. In addition, we also performed a literature review of 42 case reports of ECIPAS. Conclusion: Although the preoperative diagnosis of ECIPAS is relatively difficult, familiarity with the imaging features, the clinical presentation and the location of the cyst could lead to a correct preoperative diagnosis of ECIPAS, which might thereby reduce the number of unnecessary resections.

Abstract:The splenic cyst is a rare disease with unknown etiology. The inner wall of the cyst has lining epithelium. The cyst can be unilocular or multilocular. According to pathology, it can be divided into four types: epidermoid cyst, dermoid cyst, cystic lymphangioma, and cystic hemangioma. Ultrasound examination is often the first choice for splenic cysts because of its nonradiation, low cost, and convenient examination. The images are mostly cystic masses with clear borders and dark areas without echoes, after the detection of splenic space-occupying lesions by ultrasonography, CT, and MRI. Here, we report robot-assisted partial splenectomy for a splenic cyst. Imaging diagnosis of abdominal CT enhancement: the cystic space-occupying of the spleen is considered. We should improve the preoperative examination and exclude operative contraindications. During the operation, there was about 8 cm of the upper pole of the spleen, and the boundary was clear. There was no obvious abnormality in the exploration of the abdominal viscera. The operation was successful. The operative time was 115 minutes, and the blood loss was 20 ml. On the first day after the operation, the patient took a liquid diet. The time of first anal exhaust was on the second day after operation. The patient was discharged at the fourth day. Postoperative pathology revealed epidermoid cyst. The therapy strategy of the splenic cyst is ambiguous. Better understanding of the splenic segmental anatomy and surgical skills has made minimally invasive partial splenectomy a preferred treatment for splenic cysts. In this paper, we report a case of splenic epidermoid cyst managed successfully by robot-assisted partial splenectomy.

Abstract:The experimentally induced cell‐mediated immune reaction has been examined ultrastructurally in rat oral mucosa, using DNCB/DNFB as haptens. In skin‐presensitized animals, an increased number of mononuclear cells was found in the epithelium at a very early phase (15‐60 min) after hapten exposure. In nonsensitized animals, an increased number of cells was evident in hapten‐exposed tissue also very early in the reaction. Mononuclear cells appeared in the connective tissue but very few in the epithelium. Interaction between mononuclear cells and Langerhans cells could be demonstrated in both experimental groups.

Abstract:Splenic cysts are a rare pathology, which can be classified as true (25%) or pseudocysts (75%). Total splenectomy has been the treatment of choice, particularly in recent times with the advent of the laparoscopic approach. However, as the spleen is an organ with multiple immunological functions, the laparoscopic partial splenectomy is an alternative, which is technically difficult but effective. We present a case of a 26-year-old woman with incidental evidence of a splenic cyst in an abdominal ultrasound scan. We performed a laparoscopic partial splenectomy to preserve the function of the spleen because of the patient's youth. Laparoscopic partial splenectomy allows the effective removal of lesions and preservation of splenic function. Although more research is needed to clarify the most effective approach, this case is further evidence that this surgical approach may be beneficial for selected patients.

Abstract:This paper studies the optimization of Mean-Shift (MS) image filtering scale parameters. A parameter sweep experiment representing 164 days of CPU is performed on the EGEE grid. The mathematical foundations of Mean-Shift and the grid environment used for the deployment are described in details. The experiments and results are then discussed highlighting the efficiency of gradient ascent algorithm for MS parameters optimization and a number of grid observations related to data transfers, reliability, task scheduling, CPU time and usability.

Abstract:New privacy regulations together with ever-increasing data availability and computational power have created a huge interest in data privacy research. One major research direction is built around k -anonymity property, which is required for the released data. Although k -anonymity protects against identity disclosure, it fails to provide an adequate level of protection with respect to attribute disclosure. We introduced a new privacy protection property called p-sensitive k -anonymity that avoids this shortcoming. We developed new algorithms (GreedyPKClustering and EnhancedPKClustering) and adapted an existing algorithm (Incognito) to generate masked microdata with p-sensitive k -anonymity property. All these algorithms try to reduce the amount of information lost while transforming data to conform to p-sensitive k -anonymity. They are different in the masking methods they use. The new algorithms are based on local recoding masking methods. Incognito, initially designed for k -anonymity, uses global recoding for masking. This paper’s goal is to compare the impact of the masking method on the quality of the masked microdata obtained. For this we compare the quality of the results (cost measures based on data utility) and the efficiency (running time) of these three algorithms for masking both real and synthetic data sets. 2000 Mathematics Subject Classification: 68P15, 68U35.

Abstract:Abstract This study is an effort to produce a generic and comprehensive solution to the simulation of mass diffusion through a multiphasic and heterogeneous material model. A Galerkin-type finite element formulation is developed to solve Fick's equation for steady-state and time-dependent analysis. The effect of the interface in modelling of a liquid—solid medium is presented in this work. To show the robustness of the proposed approach, the gas exchange (oxygen and carbon dioxide) process through the capillary network between the alveolar membrane and red blood cells has been analysed and then validated with experimental data. The current work is a significant asset to modelling the diffusion of oxygen between cells and scaffolds in tissue engineering or tissue regeneration/repair studies. It is one step towards the development of high-order elements for application of the simulation of mass transfer through a multiphasic and porous model with varying degrees of interconnectivity and pore size for tissue engineering applications.

Abstract:Objective To evaluate the safety and feasibility of laparoscopic partial splenectomy.Methods Medical records of 8 patients with splenic cyst were reviewed retrospectively.The patients were diagnosed with CT or MRI and received laparoscopic partial splenectomy in our hospital from April 2008 to November 2012.The maximum size of the tumors ranged from 5.3 to 17.2 cm in diameter(mean,8.9 cm).During the operation,pneumoperitoneum was established with a 10 mm trocar through the inferior umbilical fold.At the hilum of spleen,the upper pole or anus perineum blood vessels were isolated to cut and partially remove the spleen to follow ischemia line.Results All of the six operations were completed by laparoscopy within 175 to 325 minutes(mean,230.8 minutes).During the operation,a median of 150 ml blood loss was measured(50-700 ml).After the procedure,spleen fossa drainage was maintained for 3 to 6 days(mean,4 days).The mean hospital stay of the patients was 4.8 days(3-6 days).None of them had pancreatic injury,pancreatic fistula,hemorrhage,or spleen fossa infection.The 6 patients were followed up for 2 to 57 months(median,5.5 months),and none of them showed recurrence during the period.Conclusion Laparoscopic partial splenectomy is safe and less traumatic for benign splenic tumors.
Uncommon splenic cysts in paediatric patients: A case series

Abstract:Splenic cysts are uncommon and classified into parasitic and non-parasitic origins. Non-parasitic cysts are further categorised into primary and secondary forms; primary cysts develop congenitally and progress into adulthood and secondary cysts result from factors such as abdominal trauma, infection or ischemia. This case series presents three instances of splenic cysts in children. The first case involves a splenic epidermoid cyst, the second a pseudocyst and the third a splenic epithelial cyst. All patients exhibited an abdominal lump in the left quadrant that increased in size over time, without additional symptoms. The third patient had a history of abdominal blunt trauma a year prior to symptom onset. Treatment approaches varied: the first and third patients underwent total splenectomy, while the second patient underwent aspiration drainage with frozen section analysis and partial splenectomy. All patients, first, second and third, were discharged 6, 3 and 5 days postoperatively, respectively, without complications. Splenic epithelial cyst (SEC) emerged as the predominant primary non-parasitic splenic cyst type, with an unclear pathogenesis. Typically asymptomatic, splenic cysts are commonly detected incidentally during imaging or exploratory laparotomy. Histopathology stands as the gold standard diagnostic method for splenic cysts. Although rare, paediatric splenic cysts should be considered in cases of abdominal trauma. Imaging serves a vital role in diagnosis, guiding decisions between conservative or surgical interventions based on cyst size, symptoms and associated complications.

Abstract:Non-parasitic splenic cysts are rare and are seldom diagnosed outside the paediatric surgical practice. Giant true primary epithelial cysts greater than 14 cm in diameter are even rarer. Laparoscopic surgery is preferable; however, bleeding, splenectomy and recurrence are recognised risks. Here, we report a young female patient with a 21 cm symptomatic primary splenic cyst. The patient underwent a spleen-preserving laparoscopy and was followed up for 2 years when she had an MRI of the abdomen. Surgical, technical and perioperative treatment aspects are discussed here, in the context of the current literature.
Laparoscopic Partial Splenectomy Assisted by Fluorescence in a 13-Year-Old Girl

Abstract:Partial splenectomy allows preserving immune function in benign splenic lesions such as epidermoid cysts. Determining the plane of resection and perfusion of the spleen remnant can be difficult, especially in centrally located lesions. We present a 13-year-old girl with a symptomatic splenic cyst of 6 cm in diameter located next to the splenic hilum. Laparoscopic partial splenectomy was performed through a 10-mm umbilical approach and three accessory 5-mm ports. Intraoperative intravenous injection of indocyanine green (ICG) at 0.2 mg/kg guided the careful dissection of the splenic hilum and checked the spleen perfusion once the upper arterial branch was clamped. The subsequent wash-out of the ICG allowed inspection of the peripheral vascular return of the splenic remnant through polar veins. Surgery was uneventful with minimal blood loss. Follow-up ultrasound scan revealed a well-perfused small splenic remnant with no signs of recurrence. Laparoscopic partial splenectomy is feasible in benign splenic tumors, especially in those cases of peripheral location. Fluorescence facilitates the safe dissection of the splenic hilum, the visualization of the transection plane of the spleen and the perfusion of the remnant in cases of anatomically and technically complicated partial splenectomies.

Abstract:Aim: Splenic cysts are rare benign lesions of the spleen, usually asymptomatic and incidentally discovered at imaging. Material and methods: A case of huge epidermoid cyst of the spleen on a 16 years old female patient is presented. This case was symptomatic with fullness and palpable mass in the entire abdomen. Results: Initial evaluation with ultrasound and computed tomography established the diagnosis with the greater dimension of the cyst about 25cm. Because of the size and the relation with the hilum of the spleen, splenectomy was performed and any effort to rescue splenic parenchyma was impossible. Discussion-conclusions: Many techniques are reported as treatment options, but the standard of care for nonparasitic splenic cysts is splenectomy. Key word: Benign, Cyst, Epidermoid, Splenic.

Abstract:Splenic cysts are uncommon and are classified as primary cysts or secondary cysts based on the presence or absence of an intact epithelial lining, respectively. Epidermoid cysts have an epithelial lining of stratified squamous epithelium and have been reported in normal and intra-pancreatic accessory spleens. An 18-year-old girl presented with a symptomatic, uncomplicated splenic cyst causing massive splenomegaly and symptoms due to mass effect. Peripheral cytopaenias and lymphoproliferative neoplasm were excluded. Preoperative computed tomography was suggestive of a large splenic cyst causing massive splenomegaly and marked loco-regional pressure effects. Open splenectomy was performed and histology confirmed an epidermoid cyst with no features of malignancy. Splenic epidermoid cysts are a rare cause of massive splenomegaly. Surgical intervention is recommended due to symptoms and the risk of complications.

Abstract:Epidermoid splenic cysts are very rare. Symptoms emerge because of enlargement, infection, haemorrhage or rupture. Although splenectomy is indicated for large cysts, minimally invasive and preservation procedures, such as partial splenectomy or total cystectomy with splenorrhaphy, have been increasingly used during the last decade. We report herein the case of a 16-year old female presented with left upper abdominal quadrant pain, fever and abdominal distention treated in our department.

Abstract:Ninety-nine patients at Center for Disease Control (CDC) clinical stage IV were studied. Twelve (12.12%) of these patients turned out to be HIV seronegative. Ten out of the 12 HIV negative patients were immunocompetent whereas the other two had proportional decreases in both CD4+ and CD8+ T-lymphocytes. HIV-1, HIV-2, and dual infection, were detected in 51.5%, 2%, and 22.2% respectively of clinical AIDS patients. The other 12.12% of clinical AIDS patients were indeterminate for HIV antibodies. All HIV positive patients with the exception of two, were immunocompromised with respect to CD4+ and CD8+ T-lymphocyte counts. Two healthy spouses and three children of patients who died from the disease were seronegative for HIV antibodies. Herpes simplex virus type 2 (HSV-2) and cytomegalovirus (CMV) antibody titres were higher in HIV infected than uninfected blood. Patients with chronic diarrhoea, lymphadenopathy, pneumonia, and tuberculosis, either alone or in combination of two or more of such symptoms, were found to be more likely to be confirmed by serology and immunology as definitive AIDS patients in Ghana. In postmortem studies on 20 patients, pneumonia due to tuberculosis constituted the major cause of death. Toxoplasmosis, cytomegaloviral eosophagitis and enteritis, and cryptococcosis were the major opportunistic infections detected. Programmed cell death (apoptosis) was found by the DNA gel electrophoresis method to be an unlikely major mechanism of accelerated culture induced death of PBMCs from CDC stage IV AIDS patients.
