Abstract:Background: There is limited epidemiologic data on pediatric basketball injuries and the comparison of these injuries before and after adolescence and between male and female athletes. Purpose: To assess common sex- and age-based injury patterns in pediatric basketball players. Study Design: Descriptive epidemiology study. Methods: Injury data from the National Electronic Injury Surveillance System (NEISS) and participation data from the National Sporting Goods Association were used to complete this study. Data on pediatric basketball injuries between January 2012 and December 2018 in patients aged 7 to 11 years (childhood) and 12 to 17 years (adolescence) were extracted and used to calculate national injury incidence rates with 95% CIs. Sex and age group patterns were examined utilizing Pearson chi-square tests. Z tests were conducted for the comparison of injury rates between female and male athletes in each age group and overall injury rate across age groups. Results: An average of 9582 basketball injuries were reported annually in the NEISS, which calculated to an annual national estimate of 294,920 injuries. The most common diagnoses were ankle strain/sprain (17.7%), finger strain/sprain/fracture (12.1%), concussion/head injury (9.4%), knee strain/sprain (4.5%), and facial laceration (3.3%). There was a significant increase in injury prevalence in adolescents (12- to 17-year-old category: 238,678 injuries per year) when compared with childhood (7- to 11-year-old category: 56,242 injuries per year) ( P < .0001). Concussions/head injuries occurred at a high rate in childhood, second only to finger strain/sprain/fracture, and at a similar rate in females and males (injuries per 100,000 athlete-days: 4.9 [95% CI, 3.1-6.7] vs. 5.9 [4.3-7.5], respectively; P = .41). From childhood to adolescence, injury prevalence increased for all areas and across both sexes, except for female finger strain/sprain/fracture; however, the rate of increase for concussion/head injuries and knee injuries was significantly higher in female compared with male athletes ( P < .0001 for both). In adolescents, ankle injuries were the most common injury overall. Conclusion: Ankle injuries continue to be the most predominant pediatric basketball injury. However, disproportionate rates of both knee and concussion/head injuries in female athletes during adolescent basketball are of concern and have implications for injury prevention.

Abstract:BACKGROUND: Millions of youth participate in baseball and softball in the United States and these youth account for a large number of pediatric sports injuries. This study sought to characterize annual, seasonal, and age-related trends in pediatric softball and baseball injuries presenting to the Emergency Department (ED).METHODS: The National Electronic Injury Surveillance System (NEISS) database was retrospectively examined for all softball and baseball injuries from 2010 to 2019 involving pediatric patients aged 7-21 years. Patients were classified by age as children (ages 7-13), adolescents (ages 14-18), or young adults (ages 19-21). Case narratives were used to assign mechanisms of injury. National injury estimates were calculated using statistical weights provided by the Consumer Product Safety Commission.RESULTS: An estimated 1,372,573 pediatric softball and baseball ED visits occurred from 2010 to 2019. The mean age of the patient population was 13.6 years old (95% CI = 13.5-13.8 years), and baseball athletes were younger than softball athletes (13.2 years and 14.3 years) (p < 0.01). Moreover, baseball athletes hit by the bat were younger than their softball counterparts (11.8 years and 13.4 years). Most baseball and softball injuries were the result of being hit by the ball (52.8% and 54.2%) or sliding into a base (13.1% and 15.8%). The annual number of injuries decreased during the studied time period by 41.1% for baseball injuries and by 38.3% for softball injuries. The annual number of injuries related to all injury mechanisms decreased over the studied time period for both sports, with the exception of baseball throwing injuries, which increased by 8.6%. Baseball and softball injuries were both most likely to be present to the ED on Sunday (16.3% and 17.9%) and during the Spring (53.2% and 55.3%).CONCLUSION: Safety guidelines should focus on reducing the prevalence of injuries acquired by younger baseball and softball players during practice and educating coaches and players on existing pitch count guidelines.

Abstract:Orthopaedic Journal of Sports Medicine, Volume 12, Issue 5, May 2024. Background:Nearly 8 million high school students in the United States participate in sports each year. With the lack of recent population data, an update to previous studies on US high school athletes is needed.Purpose:To update the epidemiology of sports injuries in high school athletes in the United States.Study Design:Descriptive epidemiology study.Methods:The data from the National Health School Sports-Related Injury Surveillance Study, including 100 nationally representative high schools, were obtained for 9 high school sports (boys' football, soccer, basketball, wrestling, and baseball and girls' soccer, basketball, volleyball, and softball) between the 2015 and 2019 academic years. Injury rates were calculated as the ratio of injuries per 1000 athlete exposures (AEs), defined as 1 athlete participating in 1 practice or competition. Data on injured body area, injury type (sprains/strains, concussions, contusions, and fractures), time loss, and need for surgery were also obtained. Rate ratios (RRs) with 95% CIs and P values were calculated.Results:Athletic trainers reported 15,531 injuries during 6,778,209 AEs, with an overall rate of 2.29 injuries per 1000 AEs. Injury rates were highest in football (3.96), girls' soccer (2.65), and boys' wrestling (2.36). The overall injury rate was lower in girls' sports (1.86) compared with boys' sports (2.52) (RR, 0.74 [95% CI, 0.71-0.76]; P < .001) and was higher in competition compared with practice (RR, 3.39 [95% CI, 3.28-3.49]; P < .001). The most commonly injured body areas were the head/face (24.2%), ankle (17.6%), and knee (14.1%). Sprains/strains (36.8%) and concussions (21.6%) were the most common diagnoses. Overall, 39.2% and 34% of injuries resulted in a time loss of <1 week and 1 to 3 weeks, respectively. Surgery was required in 6.3% of injuries, with wrestling (9.6%), girls' basketball (7.6%), and boys' baseball (7.4%) being the sports with the highest proportion of injuries needing surgery.Conclusion:Study findings demonstrated that boys' football, girls' soccer, and boys' wrestling had the highest injury rates, with boys' sports overall having higher injury rates than girls' sports. Sprains/strains and concussions were the most common diagnoses. Few injuries required surgery.

Abstract:Background: Baseball and softball are popular sports in the United States and are responsible for a large number of youth sports injuries each year. Methods: The National Electronic Injury Surveillance System (NEISS) database was examined for softball and baseball injuries in pediatric patients (age, 7-21 years) from 2010 through 2019. Patients were classified as children (age, 7-13 years), adolescents (age, 14-18 years), or young adults (age, 19-21 years). Case narratives were used to categorize injuries as contact injuries (hit by bat or ball), field injuries (sliding into base, collision with another player, catching, or running), throwing injuries, or other. Results: An unweighted total of 24,717 baseball injuries and 13,162 softball injuries were recorded. A nationwide estimate of 861,456 baseball injuries and 511,117 softball injuries were sustained during the studied time period, with estimated respective injury rates of 86,146 and 51,112 per year. Injured softball players were most commonly adolescent (47%) and female (92%), while injured baseball players were most commonly children (54%) and male (90%). There was a greater proportion of baseball-related injuries involving the head/neck (41%) as compared with softball-related injuries (30%) ( P < .01). Conversely, a greater proportion of softball-related injuries involved the lower extremity (32%) as compared with baseball-related injuries (19%) ( P < .01). When comparing diagnosis, softball injuries were more often sprains/strains (28%) than baseball injuries (18%) ( P < .01). When comparing mechanisms of injury, baseball athletes were more likely to be evaluated with contact injuries than were softball athletes (49% vs 40%, P < .01). Conclusion: Youth baseball athletes were more likely to be injured through contact mechanisms and had a higher proportion of injuries related to the head/neck/face, whereas softball injuries more frequently involved the lower extremity and resulted in a sprain/strain. League guidelines should focus on reducing contact injuries within youth baseball, and injury-prevention programs should focus on reducing lower extremity injuries in youth softball.

Abstract:TOTHEEDITOR: Calcium-channel antagonists have been proposed as agents to prevent neuronal damage in ischemic stroke.' One study has shown that early treatment with nimodipine is effective in ischemic stroke.! However, follow-up studies have failed to substantiate these findings.' Nimodipineis effective in delayed cerebral ischemiafollowing subarachnoid hemorrhage when given prior to the ischemic event.' To see if calcium-channel antagonists might be effective when given prior to stroke, the followingretrospective study was done. Using InternationalClassification of Diseases, 9th Revision, Clinical Modification codes for ischemic stroke, a retrospective study of all patients with this diagnosis admitted in the first six months of 1991 was done. Patients were divided into two groups. Group I patients (n= 17) were receiving a calcium-channel antagonist for another medical indication when they were admitted for a stroke.Group II patients (n=29) were not receivinga calcium-channel antagonist on admission for stroke. The Barthel index and PULSE scores were evaluated on days 3 and 7 after stroke and at discharge.' Length of stay and discharge destination were also compared. Group I patients were receiving diltiazem (n=8), nifedipine (n=7), and verapamil (n=2). The mean ±SO length of stay was significantly shorter for group I patients (group I 8.5 ± 3.5 d; group II 20.7 ± 18.3 d; p=O.Ol). Significant differences in Barthel and PULSE scores were noted at days 3 and 7, with group I patients demonstrating better neurologic function. No difference was seen in discharge scores. There was a greater percentageof men among group I patients, but an analysis of covariance did not indicate gender to be a factor. In addition, significantly greater numbers in group I were discharged to home (group I 58 percent; group II 32 percent) and fewer group I patients required discharge to long-termcare (group I 6 percent;group II 24 percent). As a follow-up to this study, 89 patients with a diagnosis of stroke from North Dakota Medicaid records were reviewed.Twelve patientsreceived a calcium-channel antagonist prior to the ischemic stroke. Calcium-channel antagonist use patterns were similar to that of the previous study.The lengthof stay was different between the two groups (calciumchannel antagonist lOA ± 7.9 d; no calcium-channel antagonist 15.6 ± 16.2d; p=O.088), but did not reach statisticalsignificance. Although limited, these data appear to indicate that pretreatment with a calcium-channelantagonist may be associated with better outcomes in ischemic stroke. Since these studies are small and retrospective in design, controlled, prospective trials should be initiated to further evaluate this potentialeffect of calcium-channelantagonists.
Epidemiology of American Football-Related Fractures in the United States 2002-2021

Abstract:Background: American football is the most popular sport in the United States, with over 5.6 million people >6 years old playing the sport. Fractures in American football athletes are significant, as they compromise an athlete's performance and can also lead to prolonged recovery periods, affecting team dynamics and player careers. Analyzing these injuries is critical to evaluate preventive measures and tailor rehabilitation strategies to ensure the well-being and sustained peak performance of football athletes on and off the field. Purpose: To analyze the trend of American football fractures by body site, sex, and age in amateur athletes over a 20-year period between 2002 and 2021. Study design: Descriptive epidemiology study. Methods: The National Electronic Injury Surveillance System (NEISS) database was queried to characterize football-related fractures and injuries from 2002 to 2021 in patients aged 0-99 years old. National injury estimates were calculated using sample weights. Chi-square analysis and one-way ANOVA were performed to compare categorical variables. Results: Of 56,809 cases of American football-related fractures over 20 years, patients aged 10 to 14 years had the highest incidence of fractures, composing 41.2% of all fractures (n = 23,389), and patients aged <18 years represented 88.8% of all fractures (n = 50,457). The median age of players when fractures occurred was 13 years. Upper extremity fractures (n = 41,863 [73.7%]) were the most common fracture reported compared with lower extremity and head, neck, and trunk fractures. Among upper extremity fractures, finger fractures had the highest prevalence (35.6%), followed by lower arm fractures (19.9%) and wrist fractures (13%). While most cases resulted in release after treatment (93.1%), 5.9% resulted in treatment and hospitalization. When analyzing trends in American football-related fractures over time, we observed the largest number of fractures in 2006 (n = 3664), while the smallest number of fractures occurred in 2020 (n = 1313). Also, a downward trend was found in American football-related fractures since 2006, with a mean of 101 fewer fractures each year. Conclusion: Our analysis showed that American football-related fractures diagnosed in the emergency department in the United States were most likely to occur in pediatric patients (10-18 years old). In addition, patients experiencing American football-related fractures were most likely to have an upper extremity fracture and not require hospitalization. The trend in American football-related fractures has been decreasing since 2006.

Abstract:Purpose: The aim of this study was to determine frequency and distribution of sports- and recreation-related injuries (SRIs) affecting children and adolescents who visited a northern tier regional medical center emergency department during a 15-year period.Material and methods: A descriptive epidemiologic design was employed to retrospectively examine age, gender, month and year of injury, location of injury, sport/recreational activity, mechanism of injury, type and severity of injury, hospital admission and length of stay. Frequency of reported injuries were compared in categories of single factors using Chi-square tests of homogeneity. The impact of risk factors – gender, age class, and sport/activity – on incidence ratios were analyzed via Poisson regression. All statistical analyses were run in R.Results: Findings heretofore unreported or inconsistent with previous emergency department (ED) studies include a peak injury occurrence of SRIs during September; a preponderance of head/neck injuries and fractures and a higher percent of admitted patients; frequent occurrence of ice hockey injuries; increased severity of injury during ages 10-14; and a trend during 2000-2014 showing increased injury rate of various types of recreational injuries. Findings consistent with previous ED studies included variable distribution of injuries by age, gender and sport/activity; increased frequency of SRIs during ages 12-15 years and during the warmer months of the year; and trends of increased frequency of SRIs affecting females, increased injury rate of closed head injuries, and decreased occurrence of bicycle injuries during the study period. Further to these findings, several suggestions are made to inform and guide local injury prevention efforts and further research. Conclusions: Our study results provide information on a region-specific occurrence and distribution of SRIs in a northern tier hospital catchment area that can be valuable to guide regional injury prevention efforts and further research to evaluate specific patterns identified and success of prevention efforts.

Contact sport related head and neck injuries in pediatric athletes

Abstract:Objectives: To describe the type and frequency of sports related pediatric head and neck trauma. Methods: The National Electronic Injury Surveillance System (NEISS) was searched for football, basketball, soccer, lacrosse and ice hockey related facial injuries. Cross-sectional analysis of incidence, age, and sex and specific injury diagnoses, mechanisms, and facial locations were performed. We focused on craniofacial and soft tissue injuries and excluded patients with concussion or other neurological injuries. Results: A total of 24,905 cases were identified in the NEISS database, corresponding to an estimated, 764,293 emergency department visits. The most hazardous sports were basketball, accounting for 356,188 visits (46.6%), football with 249,633 visits (32.6%), and soccer with 128,113 (14.7%) visits. Lacrosse and ice hockey accounted for 16,869 (1.9%) and 13,490 (1.5%) visits, respectively. There has been a decrease in injuries over the past 10 years, particularly for football (53%), ice hockey (46%), and lacrosse (41%). Soccer (13%) and basketball (9%) noticed the smallest decreases. Conclusion: Children who play basketball, football and soccer are especially prone to emergency department visits related to the head and neck. Overall, there has been an improvement in number of injuries across the five sports investigated. Sports amenable to head and neck protective equipment saw the largest improvement (football, ice hockey, lacrosse), whereas only small decreases were noted in basketball and soccer. Changes in regulations for sports not amenable to more protective equipment may help decrease adverse events.
Epidemiology of Sports-Related Tendon Ruptures Presenting to Emergency Departments in the United States

Abstract:The American Journal of Sports Medicine, Ahead of Print. Background:Participation in recreational and competitive sports can predispose people to musculoskeletal injuries. Chronic overuse with insufficient recovery, overloading, and direct contact can result in acute primary tendon ruptures. There is scarce literature regarding the epidemiology of sports-related acute primary tendon ruptures in the United States (US).Purpose:To identify the incidence rate (IR) of sports-related acute primary tendon ruptures presenting to US emergency departments using the National Electronic Injury Surveillance System (NEISS) database and describe the trends in the IR from 2001 to 2020.Study Design:Descriptive epidemiology study.Methods:The NEISS database was queried using injury case narratives; cases with a clear diagnosis of an acute primary tendon rupture were included in the final analysis. National estimates, estimated IRs (reported as per 1,000,000 person-years at risk), and temporal trends in the annual IR (reported as average annual percent change) of acute primary tendon ruptures were calculated using NEISS sample estimates and US Census Bureau population estimates.Results:An estimated total of 141,382 patients (95% confidence interval [CI], 107,478-175,286) presented to US emergency departments with a sports-related tendon rupture over the study period (IR, 22.9 person-years at risk [95% CI, 17.4-28.3]). The mean age was 37.7 years (95% CI, 37.0-38.5). Of all cases of a tendon rupture identified from 2001 to 2020, 60.1% were secondary to exercise/sports-related injury mechanisms. An Achilles tendon rupture was the most common injury, representing 55.9% of cases. Basketball was the most common sports-related injury mechanism, accounting for 36.6% of cases. The overall injury rate in male patients was 7.7 times that of female patients (IR ratio, 7.7 [95% CI, 4.4-13.5]; P < .05). The annual incidence of all sports-related tendon ruptures in the US increased significantly from 2001 to 2020 (average annual percent change, 1.9 [95% CI, 1.0-2.8]; P < .01).Conclusion:Recreational/competitive sports participation accounted for a large proportion of acute primary tendon ruptures in the US during the study period.

Abstract:Determine national estimates of injuries, mechanisms of injury (MOI), and injury severity among men and women engaging in track and field activities in the United States (U.S.), aged 18 years and older, who present to emergency departments (ED).Retrospective analyses of injury narratives were conducted using data from the National Electronic Injury Surveillance System (NEISS) of the Consumer Product Safety Commission (CPSC), comprising individuals 18 and older presenting to U.S. EDs from 2004 to 2015, with injuries associated with track and field, applying the NEISS product code 5030 and patient narratives. National injury estimates were calculated using sample weights. National injury incidence rates were determined using U.S. census estimate data (denominator), and comparisons of categorical variables by gender were made using a chi-squared test, and associated p-values.Estimated 42,947 ED visits among individuals 18 and older presented for track and field-related injuries in the U.S. from 2004 to 2015, consisting of 23,509 incidents among men, and 19,438 among women. The highest rates of injury occurred in 2010 among men, and 2011 among women, with 3.47, and 2.70 injuries per 100,000 U.S. population, respectively. No statistically significant differences (α = 0.05) were found between genders for injury severity (p = 0.32), injury diagnosis (p = 0.30), and body region (p = 0.13), but there was a significant difference overall between genders for mechanism of injury (p = 0.01).To develop appropriate injury preventive interventions for track and field athletes, additional studies exploring associations between injury characteristics, namely the mechanisms of injury, and gender, are necessary.

Comparison of pediatric performance athlete injuries presenting to emergency departments

Abstract:Objective: Pediatric injuries in performance sports represent a significant healthcare burden and account for over 50,000 annual Emergency Department (ED) visits in the United States. The objective of this study was to characterize and compare pediatric injury presentation across the most common performance sports. Methods: The National Electronic Injury Surveillance System (NEISS) database was retrospectively analyzed for pediatric injuries (3-18 years) related to gymnastics, dance, or cheerleading from 2015-2019. Cases were categorized as children (˂11 years) or adolescent (≥11 years). Injuries were categorized as orthopedic (fractures, dislocations, sprain, strains), non-orthopedic (contusion, internal injury, laceration), concussion, or other/unknown. Case narratives were used to categorize mechanism of injury as contact or non-contact. Appropriate sample NEISS weights estimation was applied for statistical analysis and Confidence Intervals (CI). Results: A total of 393,110 injuries were observed over the five-year study period, with a mean of 78,622 annual injuries. Gymnastics, dance, and cheerleading accounted for 136,422 injuries, 96,416 injuries, and 160,272 injuries, respectively. Most cases were adolescent (71%; 95% CI: 68-74%) and occurred in a sports-related setting (65%, 95% CI: 57-72%). Gymnastics had the highest proportion of injuries among children (50%) as compared to dancers (25%) or cheerleaders (12%) (p < 0.01). Non-contact injuries most affected the lower extremity (43-68%) and resulted in an orthopedic diagnosis (63-71%), and contact injuries had a higher proportion of injuries affecting the head, neck, and face (29-51%) and non-orthopedic diagnoses (29-38%). Gymnastics had the most upper extremity non-contact injuries (42%) and dance the most lower extremity non-contact injuries (68%) (p < 0.01). Cheerleading had the highest incidence of overall concussions (8%), contact injuries (47%), and concussions due to contact injury (15%). Conclusion: Pediatric gymnasts, cheerleaders, and dancers have important similarities and differences in injury pattern which may lead to the development of sport-specific injury prevention programs for pediatric performance athletes.

Abstract:Abstract Introduction Severe sports-related injuries are a common affliction treated in Level I trauma departments. Detailed knowledge on injury characteristics from different medical settings is essential to improve the development of injury prevention strategies in different team sports. Methods Team sport injuries were retrospectively analysed in a Level I trauma department registry over 15 years. Injury and treatment data were compared with regard to competition and training exposure. Injury data such as “time of visitation”, “type of injury”, “multiple injured body regions” and “immediate hospitalisation” helped to define the severity level of each team sports injury. Results At the Level I trauma department, 11.361 sports-related injuries were seen over 15 years, of which 34.0 % were sustained during team sports. Soccer injuries were the most common injuries of all team sports (71.4 %). The lower extremity was the most affected body region overall, followed by the upper extremity. Head injuries were mainly seen in Ice hockey and American football and concussion additionally frequently in team handball. Slight injuries like sprains or contusions occurred most frequently in all team sports. In soccer and team handball, injuries sustained in competition were significantly more severe (p < 0.001) than those sustained in practice. Volleyball and basketball had a trend to higher rate of severe injuries sustained during practice sessions. Conclusion Depending on the specific injury profile of each team sports, injury prevention strategies should address competitive as well as training situations, which may need different strategies.

Abstract:Sir, We were presented with a baby born to a mother taking sodium valproate who wished to breast feed. There are no data on breast milk levels of sodium valproate; it is presumed from collateral evidence that some of the drug would pass across from maternal serum into the milk. Studies in animals given modest doses of sodium valproate during pregnancy (30-90 mg/kg) showed no significant teratogenic effects. When a much higher dose was used (about 600 mg/kg) there was a dose-related increase in the rate of intrauterine death and major fetal abnormalities, most of which involved the kidneys or the lower lumbar spine (Whittle, 1976). Information on the outcome of the pregnancies where sodium valproate has been taken as the main anticonvulsant is very scanty; in most reports two or more drugs have been used or, in the case of an abnormality, the family history has suggested a strong genetic element (W. Reckitt-Labaz, 1978, personal communication). Case 1, a 22-year-old primipara, was epileptic and changed to sodium valproate in November 1975. She was well controlled taking 1600 mg/day in divided doses. She had had irregular periods and did not realise that she was pregnant until she was about 14 weeks. She consequently booked at the hospital clinic at about 18 weeks. Her anticonvulsant drug was not changed. She was admitted in labour at term in July 1978 and delivered a healthy boy weighing 3010 g; Apgar score was 9 at one minute, and no abnormalities were observed. The baby breast fed well and was discharged home on the 6th day, slightly under his birthweight. Mother and baby were seen at 29 days after delivery. The baby was still breast feeding well, gaining weight, and beginning to smile. The sodium valproate level was measured in the mother's serum, as shown in the Figure. The level fell slightly at term without any change in the oral dose and then rose to the previous level by 29 days. The valproate level in the serum of the neonate was of the same order as that of the mother's at delivery, but fell to insignificant levels by 5 days and was undetectable at 29 days (Figure). The mean level of valproate in breast milk at 5 days was 50 mmol/l; it had fallen to 21 mmol/l by 29 days. The neonatal valproate level indicates that valproate crosses the placenta freely and maternal serum levels will reflect fetal serum levels. Sodium valproate crosses through from serum into breast milk and will be found at a level between 5 and 10% ofthat in the mother's serum. The absorption of valproate from breast milk by the neonate appears to be insignificant. In this instance there was no evidence of any abnormality associated with the use of this drug. It may be concluded that breast feeding while on theraneutic doses of sodium valproate is safe and will not Valproate

Abstract:Background Injury characteristics of high school track and field throwing athletes in the United States are not well studied. Understanding epidemiology of injuries is important to identify treatment and prevention strategies. Objective To describe injury rates and patterns in U.S. high school track and field throwing events from a longitudinal national sports injury surveillance system. Design Descriptive epidemiology study. Setting Data were provided by the National High School Sports Related Injury Surveillance System, High School RIO (Reporting Information Online). Methods Athletic trainers reported injury and exposure data through the High School RIO website on a weekly basis. An athlete exposure (AE) was defined as one athlete participating in one school‐sanctioned practice or competition. Throwing events of discus, shot put, and javelin were analyzed in this study. Main Outcome Measures Injury rate, rate ratios (RR), injury proportion ratios (IPR). Participants U.S. high school athletes. Results A total of 267 track and field throwing injuries occurred during 5,486,279 AEs. Overall, the rate of injuries in competition was higher than in practice (RR 1.35, 95% confidence interval [CI] 1.01–1.80). In practice, the rate of injuries was higher for girls than boys (RR 1.53, 95% CI 1.12–2.08). The most frequently injured body part was the shoulder (21.7%), followed by the ankle (16.5%) and knee (12.0%). The most common types of injury were muscle strains (26.14%) and ligament sprains (25%). Recurrent injuries accounted for a higher proportion of chronic injuries compared to new injuries (IPR 1.85, 95% CI 1.16–2.97). Conclusion This study described injury characteristics of high school track and field throwing athletes from 2008 to 2019. Based on our results, injury prevention may be particularly important for female throwers with prior injury.

Abstract:Background: Ongoing monitoring of concussion rates and distributions is important in assessing temporal patterns. Examinations of high school sport-related concussions need to be updated. This study describes the epidemiology of concussions in 20 high school sports during the 2013-2014 to 2017-2018 school years. Methods: In this descriptive epidemiology study, a convenience sample of high school athletic trainers provided injury and athlete exposure (AE) data to the National High School Sports-Related Injury Surveillance Study (High School Reporting Information Online). Concussion rates per 10 000 AEs with 95% confidence intervals (CIs) and distributions were calculated. Injury rate ratios and injury proportion ratios examined sex differences in sex-comparable sports (soccer, basketball, baseball and softball, cross country, track, and swimming). We also assessed temporal trends across the study period. Results: Overall, 9542 concussions were reported for an overall rate of 4.17 per 10 000 AEs (95% CI: 4.09 to 4.26). Football had the highest concussion rate (10.40 per 10 000 AEs). Across the study period, football competition-related concussion rates increased (33.19 to 39.07 per 10 000 AEs); practice-related concussion rates decreased (5.47 to 4.44 per 10 000 AEs). In all sports, recurrent concussion rates decreased (0.47 to 0.28 per 10 000 AEs). Among sex-comparable sports, concussion rates were higher in girls than in boys (3.35 vs 1.51 per 10 000 AEs; injury rate ratio = 2.22; 95% CI: 2.07 to 2.39). Also, among sex-comparable sports, girls had larger proportions of concussions that were recurrent than boys did (9.3% vs 6.4%; injury proportion ratio = 1.44; 95% CI: 1.11 to 1.88). Conclusions: Rates of football practice-related concussions and recurrent concussions across all sports decreased. Changes in concussion rates may be associated with changes in concussion incidence, diagnosis, and management. Future research should continue to monitor trends and examine the effect of prevention strategies.

Abstract:A large proportion of United States (U.S.) youth play basketball, baseball, softball, or T-ball. Each of the activities poses a documented risk of craniofacial and neck injuries. However, few studies have assessed the national prevalence of pediatric craniofacial and neck injuries in this population, particularly following the COVID-19 pandemic. The National Electronic Injury Surveillance System (NEISS) dataset was used to identify pediatric craniofacial and neck injuries associated with basketball, baseball, softball, or T-ball from 2003–2022 in a cross-sectional study. The annual number of injuries before and after the onset of the COVID-19 pandemic with 95% confidence intervals were calculated. Interrupted time series analysis (ITSA) was used to estimate the pandemic's impact on the monthly number of injuries incurred nationally. Both overall and stratified by sport involvement, the annual number and rate of injuries identified in NEISS decreased significantly after the COVID-19 pandemic. ITSA demonstrated that the monthly number of injuries decreased -4094.4 (95% CI = -5100, -3088.7) immediately after the beginning of the pandemic. The number of injuries began increasing towards pre-pandemic levels at a rate of 110.6 (95% CI = 64, 157.2) injuries per month after the initial plunge. Prior to the-pandemic, there was a steady decline in craniofacial and neck injuries due to basketball, baseball, softball and T-ball among children, aged <18 years. The shutdown during the initial months of the COVID-19 pandemic resulted in a precipitous drop in such injuries. Current rates are approaching pre-pandemic levels and may exceed them. Continued efforts are needed to keep the pre-pandemic progress.

Abstract:Context: The advent of Web-based sports injury surveillance via programs such as the High School Reporting Information Online system and the National Collegiate Athletic Association Injury Surveillance Program has aided the acquisition of girls' and women's basketball injury data. Objective: To describe the epidemiology of injuries sustained in high school girls' basketball in the 2005-2006 through 2013-2014 academic years and collegiate women's basketball in the 2004-2005 through 2013-2014 academic years using Web-based sports injury surveillance. Design: Descriptive epidemiology study. Setting: Online injury surveillance from basketball teams in high school girls (annual average = 100) and collegiate women (annual average = 57). Patients or other participants: Girls' and women's basketball players who participated in practices and competitions during the 2005-2006 through 2013-2014 academic years in high school or the 2004-2005 through 2013-2014 academic years in college. Main outcome measure(s): Certified athletic trainers collected time-loss (≥24 hours) injury and exposure data. Injury rates per 1000 athlete-exposures (AEs) were calculated. Injury rate ratios (IRRs) with 95% confidence intervals (CIs) were used to compare injury rates by school size or division, time in season, event type, and competition level. Results: The High School Reporting Information Online system documented 2930 time-loss injuries during 1 609 733 AEs; the National Collegiate Athletic Association Injury Surveillance Program documented 3887 time-loss injuries during 783 600 AEs. The injury rate was higher in college than in high school (4.96 versus 1.82/1000 AEs; IRR = 2.73; 95% CI = 2.60, 2.86). The injury rate was higher in competitions than in practices for both high school (IRR = 3.03; 95% CI = 2.82, 3.26) and collegiate (IRR = 1.99; 95% CI = 1.86, 2.12) players. The most common injuries at both levels were ligament sprains, concussions, and muscle/tendon strains; the majority of injuries affected the ankle, knee, and head/face. These injuries were often caused by contact with another player or a noncontact mechanism. Conclusions: Injury rates were higher in collegiate than in high school athletes and in competitions than in practices. Similarities in distributions of injuries by body parts, specific diagnoses, and mechanisms of injury suggest that both levels may benefit from similar injury-prevention strategies.

Abstract:Background: The current coronavirus 2019 (COVID-19) pandemic has prompted a multitude of public health response measures including social distancing, school cancellations, and cessation of organized sports. Purpose: To examine the impact of COVID-19 and corresponding public health measures on the characteristics of common pediatric musculoskeletal injuries associated with sports. Study Design: Cohort study; Level of evidence, 3. Methods: This was a multicenter retrospective cohort study comparing patients with sports injuries presenting to 3 geographically diverse level I pediatric trauma hospitals and outpatient orthopaedic surgery clinics in the United States during the COVID-19 pandemic and a prepandemic period at the same institutions. Patients were included if they presented for care between February 15 and July 15 in 2020 (pandemic cohort) or between March 15 and April 15 in 2018 and 2019 (prepandemic cohort). Results: Included were 1455 patients with an average age of 12.1 ± 4.5 years. When comparing patients presenting in 2018 and 2019 with those presenting in 2020, we observed a decrease in mean age during the pandemic (12.6 ± 4.0 vs 11.0 ± 5.2 years; P = .048). Additionally, a decrease in the proportion of injuries attributed to sports (48.8% vs 33.3%; P < .001) and those occurring at school (11.9% vs 4.0%; P = .001) was observed. The proportion of injuries attributable to clavicle fractures increased during the early stages of the pandemic (13.2% vs 34.7%; P < .001). There was no statistically significant delay to care in injuries presenting during the pandemic (41.5 ± 141.2 vs 19.23 ± 79.1 days; P = .175). Conclusion: Across 3 tertiary care institutions, patients were seen without significant delay during the pandemic. We observed a significant decline in pediatric musculoskeletal injuries associated with sports during the COVID-19 pandemic. This decrease has been accompanied by a shift in both injury type and mechanism.

Abstract:Objective: Epidemiology provides fundamental opportunities to protect student-athlete health. The goal of this study was to describe the epidemiology of sport-related concussion (SRC) across 8 years (2015/2016-2022/2023) and compare boys' and girls' sports for SRC incidence and SRC mechanisms. Methods: This was a retrospective cohort study performed using a statewide high school head injury surveillance system of high school student-athletes (n = 2,182,128; boys, n = 1,267,389; girls, n = 914,739). Exposures of interest included study year and boys and girls in comparable sports. Clinical incidence was calculated by dividing SRC counts in each sport by the number of participants per 100 player-seasons and presented with 95% CIs. The 2019/2020 and 2020/2021 data were included in the analysis, however caution is warranted due to the COVID-19 pandemic. Clinical incidence ratios (CIRs) were estimated for sex-comparable sports, and significance was determined if 95% CIs excluded 1.00. The authors compared mechanism of injury in boys' and girls' comparable sports with chi-square analyses (p < 0.05). Results: Among 25,482 total SRCs, the overall clinical incidence of SRC for all boys and girls was 1.17 (95% CI 1.15-1.18) per 100 player-seasons across all years. Across all years, the overall clinical incidence in boys' sports was 1.34 (95% CI 1.32-1.36) per 100 player-seasons, and 0.93 (95% CI 0.91-0.95) per 100 player-seasons in girls' sports. Boys' sports with the highest clinical incidence included football, ice hockey, and wrestling. Girls' sports with the highest clinical incidence included basketball, soccer, lacrosse, competitive cheer, and gymnastics. Girls consistently had higher SRC rates relative to boys for baseball/softball, basketball, and soccer (CIR range 1.65 [95% CI 1.41-1.93] to 3.32 [95% CI 2.67-4.16]). Girls had lower SRC in lacrosse in 2015/2016 (CIR 0.63, 95% CI 0.40-0.97); no difference in 2016/2017-2020/2021, but had higher clinical incidence in 2021/2022 (CIR 1.69, 95% CI 1.18-2.44) relative to boys. In boys the most common mechanism of SRC occurred from person-to-person contact (n = 8752, 62.8%), whereas girls commonly sustained SRC from person-to-object contact (n = 2369, 33.4%) and from person-to-person contact (n = 2368, 33.4%). There were significant associations between boys' versus girls' sports and mechanism of injury within baseball/softball (χ2 = 12.71, p = 0.005); basketball (χ2 = 36.47, p < 0.001); lacrosse (χ2 = 185.15, p < 0.001); and soccer (χ2 = 122.70, p < 0.001). Conclusions: These findings can help understand the potential impact of interventions aimed at preventing or reducing SRC. Including girls' sports within this study extends research for a largely underrepresented group.

Abstract:IN a previous paper on the influence of certain salts upon the act of clotting' a fluid capable of spontaneous coagulation was selected, and to this fluid salts in varying proportions were added. In this way it was sought to demonstrate the important part played by salts as agents favouring or retarding coagulation, and also that more or less antagonism or interference resulted from the simultaneous admixture of certain of these salts. The cooperative influence of salts was suggested by some of the experimental results obtained, according to which a maximum effect (as measured by the rapidity of the act of clotting and the degree of shrinkage of the clot) was witnessed when sodium, potassium and calcium chloride were added in certain proportions. This question was not, however, followed up. The experiments about to be detailed form a natural sequence to those of the above investigation. The coagulation of certain proteids by heat was the process now examined into, the question being:-how the presence of salts might affect this process. Into the differenlce, if any, between the spontaneous act of clotting or setting, e.g. of blood recently shed, of milk to which rennet has been added, and the separation by heat of proteid bodies, we do not propose to enter at all fully; and whether Gerinnung be one thing and Abscheidung another, this we must leave undecided. Neither have we in the first instance attempted to determine what might be the essentials, if one may use the expression, of the act of heat coagulation, we have simply taken that act and noted its modification by the presence of certain salts in certain proportions, and in respect of this same act we have examined the mutual reaction of these salts upon one another when present together. Secondly, we have touched upon the question of the essential factors of heat coagulation, but this really represents a side issue.
