Abstract:Background: Preoperative portal vein embolization (PVE) is frequently used to improve future liver remnant volume (FLRV) and to reduce the risk of liver failure after major liver resection. Objective: This paper aimed to assess postoperative outcomes after PVE and resection for suspected perihilar cholangiocarcinoma (PHC) in an international, multicentric cohort. Methods: Patients undergoing resection for suspected PHC across 20 centers worldwide, from the year 2000, were included. Liver failure, biliary leakage, and hemorrhage were classified according to the respective International Study Group of Liver Surgery criteria. Using propensity scoring, two equal cohorts were generated using matching parameters, i.e. age, sex, American Society of Anesthesiologists classification, jaundice, type of biliary drainage, baseline FLRV, resection type, and portal vein resection. Results: A total of 1667 patients were treated for suspected PHC during the study period. In 298 patients who underwent preoperative PVE, the overall incidence of liver failure and 90-day mortality was 27% and 18%, respectively, as opposed to 14% and 12%, respectively, in patients without PVE (p < 0.001 and p = 0.005). After propensity score matching, 98 patients were enrolled in each cohort, resulting in similar baseline and operative characteristics. Liver failure was lower in the PVE group (8% vs. 36%, p < 0.001), as was biliary leakage (10% vs. 35%, p < 0.01), intra-abdominal abscesses (19% vs. 34%, p = 0.01), and 90-day mortality (7% vs. 18%, p = 0.03). Conclusion: PVE before major liver resection for PHC is associated with a lower incidence of liver failure, biliary leakage, abscess formation, and mortality. These results demonstrate the importance of PVE as an integral component in the surgical treatment of PHC.

Abstract:Objective To evaluatge percutaneous microwave ablation liver partition and portal vein embolization for planned hepatectomy (PAPEP) in treatment of hilar cholangiocarcinoma with insufficient future liver remnant.Methods The clinical data of an advanced hilar cholangiocarcinoma case with small FLR treated by PAPEP was retrospective analyzed.Preoperative assessment showed a type Ⅳ (Bismuth) and staged Ⅳa (AJCC) carcinoma patient with fight hepatic artery and right portal vein branch involvement.T-bil level decreased after PTCD,B ultrasound guided percutaneous microwave ablation liver partition (PMA) was performed,portal vein embolization (PVE) was done 1 day after PMA.Results The FLR increased by 37.55％ to 527.44 ml 13 days after PMA + PVE.A right trisegmentectomy combined with caudate lobectomy was performed,left portal vein was resected and reconstructed.Postoperative subphrenic abscess and upper gastrointestinal bleeding caused by duodenal ulcer was successfully managed.No recurrence was found during 7 months follow-up.Conclusions PAPEP is effective for the radical resection of advanced hilar cholangiocarcinoma with insufficient future liver remnant.

Abstract:Objective To evaluate the safety and effectiveness of percutaneous microwave/radiofrequency ablation liver partition and portal vein embolization for planned hepatectomy (PAPEP) for hepatocelluar carcinoma(HCC) and perihilar cholangiocarcinoma with insufficient future liver remnant. Methods B ultrasound guided percutaneous microwave ablation liver partition (PMA) was applied on the future transection plane, the tumor in the left lobe was synchronal treated by dehydrated alcohol injection. one to three days after PMA, portal vein embolization (PVE) was performed. Ten to thirteen days after PVE, right trisectionectomy extended right hepatectomy surgery procedure was performe. Results The standard liver volume (SLV)was 1231.2mL、1202.9mL and 1217.1mL respectively, and the remnant liver volume was 355.6mL、383.4mL and 385.0mL, which was 28.9%、31.9% and 31.6% of the SLV before PMA. The time for PMA and PVE was 118~132min and 158~180min respectively. The FLR was increased by 41.2%, 37.6%, 23.7%to 502.1mL, 527.4mL, 476.3mL after PMA+ PVE. The operation time was 230~440min, the intraoperative blood loss was 120-1800mL. One of HCC patients complicated ascites and jaundice was cured by expectant treatment;the PHCC patient suffered subphrenic abscess which was cured by percutaneous drainage. The patients were discharged 15~40d postoperatively. Conclusion PAPEP maybe a revolutionary strategy in place of ALPPS to treat HCC and PHCC with insufficient FLR.
Systematic sequential therapy for ex vivo liver resection and autotransplantation: A case report and review of literature

Abstract:BACKGROUND Perihilar cholangiocarcinoma (pCCA) is a highly malignant tumor arising from the biliary tree. Radical surgery is the only treatment offering a chance of long-term survival. However, limited by the tumor's anatomic location and peri-vascular invasion, most patients lose the chance for curative treatment. Therefore, more methods to increase the resectability of tumors as well as to improve outcomes are needed.CASE SUMMARY A 68-year-old female patient had a hepatic hilar mass without obvious symptoms. Laboratory results showed hepatitis B positivity. Magnetic resonance imaging indicated that the mass (maximum diameter: 41 mm) invaded the left and right branches of the main portal vein, as well as the middle, left and right hepatic veins; enlarged lymph nodes were also detected in the hilum. The patient was diagnosed with pCCA, and the clinical stage was determined to be T4N1M0 (stage IIIC). Considering the tumor's anatomic location and vascular invasion, systematic conversion therapy followed by ex vivo liver resection and autotransplantation (ELRA) was determined as personalized treatment for this patient. Our original systemic sequential therapeutic strategy (lenvatinib and tislelizumab in combination with gemcitabine and cisplatin) was successfully adopted as conversion therapy because she achieved partial response after three cycles of treatment, without severe toxicity. ELRA, anastomotic reconstruction of the middle hepatic vein, right hepatic vein, root of portal vein, inferior vena cava and right hepatic artery, and lymph node dissection were performed at one month after systemic therapy. Pathological and immunohistochemical examination confirmed the diagnosis of pCCA with lymph node metastasis. Although the middle hepatic vein was partially obstructed four months later, hepatic vein stent implantation successfully addressed this problem. The patient has survived for 22 mo after the diagnosis, with no evidence of recurrence or metastasis.CONCLUSION An effective therapeutic strategy for conversion therapy greatly increases the feasibility and efficiency of ELRA.

Abstract:Objective To explore the clinical outcome of percutaneous microwave/radiofrequency ablation liver partition and portal vein embolization for planned hepatectomy (PAPEP) for the treatment of hepatocelullar carcinoma (HCC) with insufficient future liver remnant (FLR).Methods The retrospective descriptive research method was taken.The clinical data of 2 patients of giant primary HCC (FLR complicated with cirrhosis) who were admitted to the Zhejiang Province People's Hospital from November 2015 to January 2016 were collected.Treatment plan:ultrasound-guided percutaneous microwave ablation (PMA) or percutaneous radio-frequency ablation (PRA) was performed on patients under intravenous anesthesia,at the same time portal vein embolization (PVE) was conducted.FLR was measured by rescan of computed tomography (CT) at 7-9 days postoperatively,according to FLR hyperplasia and general situation of the patient,right trisegmentectomy and expanded right hemihepatectomy were implemented respectively.Observation indicatiors indicators included (1)standard liver volume (SLV) and FLR before PRA/PVE treatment,(2) liver function,body temperature,FLR and time of FLR hyperplasia after PRA/PVE treatment,(3)liver function,body temperature,complications,FLR and duration of hospital stay after hepatectomy.Follow-up using outpatient examination and telephone interview was performed to detect carcinoma recurrence and other follow-up contents up to March 2016.Results (1) Before PRA + PVE,SLVs of 2 patients were 1 103 mL and 1 132mL,FLRs of 2 patients were respectively 417 mL and 297 mL which occupied the SLV at the percentage of 37.8％ and 26.2％ respectively.(2) After PRA + PVE treatment,there were no changes in liver function and fever.After PRA + PVE treatment,FLR of 2 patients increased to 652 mLand 522 mL,respectively,which occupied the SLV at the percentage of 59.1％ and 46.1％ respectively,with increased rates of 56.4％ and 75.6％ and time of FLR hyperplasia of 10 days and 17 days.The two patients received successful hepatectomy,without intraoperative red blood cell infusion.(3) At postoperative day 3,albumin (Alb),alanine transaminase (ALT),aspartate transaminase (AST) and total bilirubin (TBil) of liver function were 28.0 g/L,63 U/L,39 U/L and 19.0 μmol/L in patient 1 with a temperature of 38 degree,and 24.3 g/L,78 U/L,11 U/L and 38.1 μmol/L in patient 2 with a normal temperature.Liver function of 2 patients was improved after conservative treatment.No severe complications were occurred.Rescan of CT showed the FLR of patients continued increasing.The duration of hospital stay of 2 patients was 10 days and 14 days,respectively.Conclusion PAPEP for HCC with insufficient FLR is safe and effective.

Focus groups were conducted using a semi-structured interview schedule with 13 family members of people with dementia and service providers from 3 divisional offices of a homecare programme, 7 nursing homes and 4 hospitals. Rural areas of Saskatchewan, Canada. Barriers to service use identified by family members and service providers included stigma associated with having a family member with dementia; need for privacy; family members’ attitudes; need for self reliance; lack of awareness about services; financial constraints; acceptability of services; accessibility of services, and ability of services to offer useful support. Family members focused on a variety of different issues, including problems in assessment, waiting for nursing home placement and stress. Some family members suggested that accepting help from formal support services was an admission that they could not cope alone. Abstract:Nanostructured porous silicon (nanoPS) can be described as a network of silicon crystals with sizes in the range of a few nanometers. The typical large specific surface area and high reactivity of nanoPS make this material very suitable for many different applications in the field of sensing. Moreover, its biocompatibility and biodegradability opens the way to the development of biosensors. Within this context, in the present work the use of nanoPS in the field of electrical biosensing is explored. More specifically, nanoPS-based devices with the structure Al/nanoPS/silicon/Al and AuNiCr/nanoPS/silicon/Al were fabricated for the electrical detection of glucose and the bacteria Escherichia Coli. The experimental results show that the current-voltage characteristics of the metal/nanoPS/silicon/metal structures show a strong dependence on the presence/absence and surface concentration of glucose and the bacteria Escherichia Coli. The present work describes our findings in the correlation between surface concentration of glucose and bacteria E. Coli and current for a given voltage.

Abstract:Preoperative portal vein embolization (PVE) is increasingly used to optimize the volume and function of the future liver remnant (FLR) and to reduce the risk for complications of major hepatectomy for hepatocellular carcinoma (HCC) or hilar cholangiocarcinoma (CCA). In patients with HCC who are candidates for extended hepatectomy and in patients with HCC and well-compensated cirrhosis who are being considered for major hepatectomy, FLR volumetry is routinely performed, and PVE is employed in selected cases to optimize the volume and function of the FLR prior to surgery. Similarly, in patients with hilar CCA who are candidates for extended hepatectomy, careful preoperative preparation using biliary drainage, FLR volumetry, and PVE optimizes the volume and function of the FLR prior to surgery. Appropriate use of PVE has led to improved postoperative outcomes after major hepatectomy for these diseases and oncological outcomes similar to those in patients who undergo resection without PVE. Specific indications for PVE are being clarified. FLR volumetry is necessary for proper selection of patients for PVE. Analysis of the degree of hypertrophy of the FLR after PVE (a dynamic test of liver regeneration) complements analysis of the pre-PVE FLR volume (a static test). Together, FLR degree of hypertrophy and FLR volume are the best predictors of outcome after major hepatectomy in an individual patient, regardless of the degree of underlying liver disease. This article synthesizes the literature on the approach to patients with HCC and CCA who are candidates for major hepatectomy. The rationale and indications for FLR volumetry and PVE and outcomes following PVE and major hepatectomy for HCC and CCA are discussed.

Abstract:Background Recurrent or locally advanced peri-hilar cholangiocarcinoma (PHCC) usually involves the portal vein (PV) leading to significant stenosis. With disease progression, clinical symptoms such as ascites, bleeding, and hepatic insufficiency are usually observed. Little is know about the benefit of PV stenting in relieving the symptoms associated to portal hypertension and allowing anticancer therapies. The aim of this study is to review our experience in PV stenting for PHCC patients. Methods From 2014 to 2022, data from PHCC patients underwent PV stenting at Verona University Hospital, Italy, were reviewed. The indications were: gastrointestinal bleeding from esophagus-gastric varices, ascites not responsive to medical therapy, severe thrombocytopenia, liver insufficiency (hepatic jaundice, coagulopathy, and/or hyperammoniemia), or asymptomatic high-grade PV stenosis. Cavernous transformation and intrahepatic thrombosis in both sides of the liver were considered contraindication. Systematic anticoagulation therapy was not administered. Results Technical success was achieved in all 16 (100 %) patients. The improvement of clinical symptoms were observed in 12 (75 %) patients. Anticancer therapy was administrated in 11 (69 %) patients. 2 (13 %) complications were observed: 1 biliary injury and 1 recurrent cholangitis that required a percutaneous trans -hepatic biliary drainage placement. Stent occlusion for tumor progression occurred in 1 patient and a re-stenting procedure was successfully performed. No case of thrombotic stent occlusion was observed during follow up. The 1-year stent patency was 86 % and the median patency period was 8 months (IQR, 4–12). Conclusion PV stenting is a feasible and safe palliative treatment that improves clinical condition, allow anticancer therapies, and provide a better quality of life.

Abstract:Introduction: The majority of patients with perihilar cholangiocarcinoma (PHC) has locally advanced disease or distant lymph node metastases on presentation or exploratory laparotomy, which makes them not eligible for resection. As the prognosis of patients with locally advanced PHC or lymph node metastases in the palliative setting is significantly better compared with patients with organ metastases, ablative therapies may be beneficial. Unfortunately, current ablative options are limited. Photodynamic therapy causes skin phototoxicity and thermal ablative methods, such as stereotactic body radiation therapy and radiofrequency ablation, which are affected by a heat/cold-sink effect when tumours are located close to vascular structures, such as the liver hilum. These limitations may be overcome by irreversible electroporation (IRE), a relatively new ablative method that is currently being studied in several other soft tissue tumours, such as hepatic and pancreatic tumours. Methods and analysis: In this multicentre phase I/II safety and feasibility study, 20 patients with unresectable PHC due to vascular or distant lymph node involvement will undergo IRE. Ten patients who present with unresectable PHC will undergo CT-guided percutaneous IRE, whereas ultrasound-guided IRE will be performed in 10 patients with unresectable tumours detected at exploratory laparotomy. The primary outcome is the total number of clinically relevant complications (Common Terminology Criteria for Adverse Events, score of≥3) within 90 days. Secondary outcomes include quality of life, tumour response, metal stent patency and survival. Follow-up will be 2 years. Ethics and dissemination: The protocol has been approved by the local ethics committees. Data and results will be submitted to a peer-reviewed journal. Conclusion: The Ablation with irreversible eLectroportation in Patients with Advanced perihilar CholangiocarcinomA (ALPACA) study is designed to assess the feasibility of IRE for advanced PHC. The main purpose is to inform whether a follow-up trial to evaluate safety and effectiveness in a larger cohort would be feasible.

Abstract:Purpose To compare the safety and effectiveness of liver vein deprivation (LVD) and portal vein embolization (PVE) in patients scheduled to undergo liver resection. Materials and Methods This retrospective cohort study included 59 patients who underwent either PVE (n = 28) or LVD (n = 31) in preparation for liver resection. The primary outcome was percent change in future liver remnant volume (FLRV). Secondary endpoints were degree of hypertrophy (DH) and kinetic growth rate (KGR). Results Low baseline FLRV, time interval in days between the procedure and follow-up imaging (Ti) positively impacted the primary and secondary endpoints in both groups. Percent change in FLRV was higher in the LVD group (52.8% ± 5.3%) than in the PVE group (22.3% ± 3.0%; P <.001). DH was also higher in the LVD group (15.4% ± 1.7%) than in the PVE group (6.4% ± 0.9%; P 35% were excluded from the analysis, LVD group demonstrated higher values than the PVE group in KGR (0.57%/d ± 0.06%/d vs 0.29%/d ± 0.05%/d; P <.001), percent change in FLRV (64.2% ± 6.0% vs 25.9% ± 4.3%; P <.001), and DH (15.4% ± 1.4% vs 6.6% ± 1.0%; P <.001). No adverse events were noted in either group. Conclusion LVD appears to be safe and may be superior to PVE in inducing hypertrophy of FLR in patients scheduled to undergo surgical resection.

Abstract:Background: In China, both percutaneous microwave/radiofrequency ablation liver partition plus portal vein embolization (PALPP) and transarterial chemoembolization (TACE) plus portal vein embolization (PVE) have been utilized in planned hepatectomy. However, there is a lack of comparative studies on the effectiveness of these two techniques for cases with insufficient future liver remnant (FLR). Methods: Patients were categorized into either the PALPP group or the TACE + PVE group. Clinical data, including FLR growth rate, complications, secondary resection rate, and overall survival rate, were compared and analyzed for both groups retrospectively. Results: Between December 2014 and October 2021, a total of 29 patients underwent TACE + PVE (n = 12) and PALPP (n = 17). In the TACE + PVE group, 7 patients successfully underwent two-stage hepatectomy, while in the PALPP group, 13 patients underwent the procedure (two-stage resection rate: 58.3% vs. 76.5%, P = 0.42). There were no significant differences in postoperative complications of one-stage procedures (11.8% vs. 8.3%, P > 0.05) and second-stage resection complication (0% vs. 46.2%, P = 0.05) between the TACE + PVE and PALPP groups. However, the PALPP group demonstrated a shorter time to FLR volume growth for second-stage resection (18.5 days vs. 66 days, P = 0.001) and KGR (58.5 ml/week vs. 7.7 ml/week, P = 0.001). Conclusions: Compared with TACE + PVE, PALPP results in a more significant increase in FLR volume and a higher rate of two-stage resection without increasing postoperative complications.

Abstract:Background: One of the main limitations to achieving a complete tumor resection in patients with technically resectable liver tumors is the presence of a small future liver remnant (FLR). Portal vein embolization (PVE) allows hypertrophy of the non-embolized lobe, reducing the risk of postoperative liver failure. Aim: To describe the experience of portal embolization prior to hepatectomy and its effectiveness in converting advanced unresectable liver tumors into resectable tumors. Methods: Non-concurrent cohort study. All patients who underwent PVE before hepatectomy between 2016 and 2020 in our center were included. Demographic and diagnostic variables, pre and post-PVE volumes, perioperative variables, and global and disease-free survival were analyzed. Results: Nineteen patients were included. Median age 66 (54-72) years and 57.9% (n= 11) were women. Bilateral metastases were present in 78.9% (n= 15). Sixteen patients (84.2%) received neoadjuvant chemotherapy. One patient (5.3%) had a complication after PVE. The median time between embolization and volumetry was 5.3 weeks (4.7-7.1). Median FLR before and after PVE were 19.8% (16.2-27.7) and 30% (25.2-40.5), respectively. The median percentage of hypertrophy was 48% (40.4-76.5). Fifteen patients (78.9%) underwent hepatectomy. Significant complications occurred in 26.6% (n= 4); among them, three patients (20%) presented postoperative liver failure. Conclusions: PVE is safe and effective in promoting FLR hypertrophy in the presence of chemotherapy, allowing patients with advanced liver tumors to undergo surgery with curative intent.

Abstract:Portal vein embolization with stem cell augmentation (PVESA) is an emerging approach for enhancing the growth of the liver segment that will remain after surgery (i.e., future liver remnant, FLR) in patients with liver cancer. Conventional portal vein embolization (PVE) aims to induce preoperative FLR growth, but it has a risk of failure in patients with underlying liver dysfunction and comorbid illnesses. PVESA combines PVE with stem cell therapy to potentially improve FLR size and function more effectively and efficiently. Various types of stem cells can help improve liver growth by secreting paracrine signals for hepatocyte growth or by transforming into hepatocytes. Mesenchymal stem cells (MSCs), unrestricted somatic stem cells, and small hepatocyte-like progenitor cells have been used to augment liver growth in preclinical animal models, while clinical studies have demonstrated the benefit of CD133 + bone marrow–derived MSCs and hematopoietic stem cells. These investigations have shown that PVESA is generally safe and enhances liver growth after PVE. However, optimizing the selection, collection, and application of stem cells remains crucial to maximize benefits and minimize risks. Additionally, advanced stem cell technologies, such as priming, genetic modification, and extracellular vesicle-based therapy, that could further enhance efficacy outcomes should be evaluated. Despite its potential, PVESA requires more investigations, particularly mechanistic studies that involve orthotopic animal models of liver cancer with concomitant liver injury as well as larger human trials.

Abstract:Background: While resection remains the only curative option for perihilar cholangiocarcinoma, it is well known that such surgery is associated with a high risk of morbidity and mortality. Nevertheless, beyond facing life-threatening complications, patients may also develop early disease recurrence, defining a "futile" outcome in perihilar cholangiocarcinoma surgery. The aim of this study is to predict the high-risk category (futile group) where surgical benefits are reversed and alternative treatments may be considered. Methods: The study cohort included prospectively maintained data from 27 Western tertiary referral centers: the population was divided into a development and a validation cohort. The Framingham Heart Study methodology was used to develop a preoperative scoring system predicting the "futile" outcome. Results: A total of 2271 cases were analyzed: among them, 309 were classified within the "futile group" (13.6%). American Society of Anesthesiology (ASA) score ≥ 3 (OR 1.60; p = 0.005), bilirubin at diagnosis ≥50 mmol/L (OR 1.50; p = 0.025), Ca 19-9 ≥ 100 U/mL (OR 1.73; p = 0.013), preoperative cholangitis (OR 1.75; p = 0.002), portal vein involvement (OR 1.61; p = 0.020), tumor diameter ≥3 cm (OR 1.76; p < 0.001), and left-sided resection (OR 2.00; p < 0.001) were identified as independent predictors of futility. The point system developed, defined three (ie, low, intermediate, and high) risk classes, which showed good accuracy (AUC 0.755) when tested on the validation cohort. Conclusions: The possibility to accurately estimate, through a point system, the risk of severe postoperative morbidity and early recurrence, could be helpful in defining the best management strategy (surgery vs. nonsurgical treatments) according to preoperative features.

Abstract:Purpose: The purpose of this retrospective study was to evaluate the efficacy of right portal vein embolisation (PVE) in inducing contralateral liver hypertrophy before extended hepatectomy. Materials and methods: Twenty-six consecutive patients, 14 with liver metastases (ten from colorectal cancer; four from carcinoid tumours) and 12 with biliary cancers (ten Klatskin tumours; one gallbladder tumour; one intrahepatic cholangiocarcinoma) with insufficient predicted future remnant liver (FRL) underwent right PVE to induce hypertrophy of the contralateral hemiliver prior to surgical resection. Total liver volume, tumour volume and FRL volume were calculated on a 3D workstation. The ratio of the FRL to the total functional liver volume was <30% in all patients. Results: The FRL volume increased by 5%-25% (15% on average) after right PVE in patients with liver metastases and by 9%-19% (14% on average) in patients with biliary cancers. In all patients, the ratio of FRL to functional liver volume was >or=30% after right PVE. No postoperative deaths due to severe liver failure occurred in the 20 patients who underwent extended hepatectomy. Conclusions: Right PVE extends the indications for hepatectomy in patients with liver metastases and those with biliary cancers who have an insufficient potential hepatic functional reserve.
Oncological Outcomes of Major Liver Resection Following Portal Vein Embolization: A Systematic Review and Meta-analysis

Abstract:Background: Preoperative portal vein occlusion with either percutaneous portal vein embolization (PVE) or portal vein ligation is routinely used to induce liver hypertrophy prior to major liver resection in patients with hepatic malignancy. While this increases the future liver remnant, and hence the number of patients suitable for resection, recent evidence suggests that induction of liver hypertrophy preoperatively may promote tumor growth and increase recurrence rates. The aims of this current study were to evaluate the impact of PVE on hepatic recurrence rate and survival in patients with colorectal liver metastases (CRLM). Methods: The MEDLINE, EMBASE and Web of Science databases were searched to identify studies assessing the oncological outcomes of patients undergoing major liver resection for CRLM following PVE. Studies comparing patients undergoing one-stage liver resection with or without preoperative PVE were included. The primary outcome was postoperative hepatic recurrence (PHR), while secondary outcomes were 3- and 5-year overall survival (OS). Results: Of the 2131 studies identified, six non-randomized studies (n = 668) met the eligibility criteria, comparing outcomes of patients undergoing major liver resection with or without PVE (n = 182 and n = 486, respectively). No significant difference was observed in PHR (odds ratio [OR] 0.78; 95 % confidence interval [CI] 0.42-1.44), 3-year OS (OR 0.80; 95 % CI 0.56-1.14) or 5-year OS (OR 1.12; 95 % CI 0.40-3.11). Conclusions: PVE does not have any adverse effect on PHR or OS in patients undergoing major liver resection for CRLM. Further studies based on individual patient data are needed to provide definitive answers.

Abstract:OBJECTIVE:To evaluate the clinical value of PVE (portal vein embolism) prior to surgery in primary liver cancer (PLC) patients and the effect of liver cirrhosis on liver lobe hyperplasia after PVE.METHODS:13 patients with primary liver cancer non-suitable for curative hepatectomy underwent k sequential transcatheter arterial chemoembolization (TACE) (1-3 times) and percutaneous selective portal vein embolization (PVE) when the remnant liver volumes were predicted to be insufficient. All patients were divided into non-cirrhosis (n = 7) and cirrhosis group (n = 6). Left liver remnant volumes were assessed by computed tomography (CT) before and after PVE. Right liver resection was performed when the remnant liver volume was sufficient.RESULTS:All patients underwent TACE treatment was successful. The frequency of TACE was 1-3. PVE was all successfully performed at weeks 2-4 after final TACE, all PVE patients subsequently underwent hepatic lobectomy. Left liver volume increased from (457.0 ± 121.0) cm(3) pre-PVE to (633.6 ± 120.2)cm(3) post-PVE. Hepatic lobe volume increased (44.4 ± 39.7)%. Statistical difference existed in left hepatic lobe volume before and weeks 4-6 after PVE (P = 0.000). The mean volume of left liver, calculated before and 4-6 weeks after PVE, increased from (442.0 ± 96.8) to (652.3 ± 115.8) cm(3) in non-cirrhotic group and from (474.5 ± 152.4) to (611.7 ± 132.3) cm(3) in cirrhotic group. Hepatic lobe volume increased (54.5 ± 50.7)% and (32.7 ± 19.9)% respectively. Statistical differences were both detected in left hepatic lobe volume before and 4-6 weeks after PVE (P = 0.011, P = 0.003). However, no significant inter-group difference existed at Weeks 4-6 weeks (P = 0.295) . Liver function damage was minimal after PVE and no serious complications occurred.CONCLUSION:Sequential transcatheter arterial chemoembolization, TACE and percutaneous selective PVE before surgery may cause remnant liver compensatory hypertrophy and increase the resection rate of primary liver cancer. Liver cirrhosis has no significant effect on liver lobe hyperplasia after PVE.
