Abstract:Background: Low- and middle-income countries (LMICs) experienced increasing rates of colorectal cancer (CRC) incidence in the last decade and lower 5-year survival rates compared to high-income countries (HICs) where the implementation of screening and treatment services have advanced. This review scoped and mapped the literature regarding the content, implementation and uptake of CRC screening interventions as well as opportunities and challenges for the implementation of CRC screening interventions in LMICs. Methods: We systematically followed a five-step scoping review framework to identify and review relevant literature about CRC screening in LMICs, written in the English language before February 2020. We searched Medline, Embase, Web of Science and Google Scholar for studies targeting the general, asymptomatic, at-risk adult population. The TIDieR tool and an implementation checklist were used to extract data from empirical studies; and we extracted data-informed insights from policy reviews and commentaries. Results: CRC screening interventions (n = 24 studies) were implemented in nine middle-income countries. Population-based screening programmes (n = 11) as well as small-scale screening interventions (n = 13) utilised various recruitment strategies. Interventions that recruited participants face-to-face (alone or in combination with other recruitment strategies) (10/15), opportunistic clinic-based screening interventions (5/6) and educational interventions combined with screening (3/4), seemed to be the strategies that consistently achieved an uptake of > 65% in LMICs. FOBT/FIT and colonoscopy uptake ranged between 14 and 100%. The most commonly reported implementation indicator was 'uptake/reach'. There was an absence of detail regarding implementation indicators and there is a need to improve reporting practice in order to disseminate learning about how to implement programmes. Conclusion: Opportunities and challenges for the implementation of CRC screening programmes were related to the reporting of CRC cases and screening, cost-effective screening methods, knowledge about CRC and screening, staff resources and training, infrastructure of the health care system, financial resources, public health campaigns, policy commitment from governments, patient navigation, planning of screening programmes and quality assurance.
Optimising colorectal cancer screening acceptance: a review

Abstract:The study aims to review available evidence concerning effective interventions to increase colorectal cancer (CRC) screening acceptance. We performed a literature search of randomised trials designed to increase individuals' use of CRC screening on PubMed, Embase, Cochrane Database of Systematic Reviews and Database of Abstracts of Reviews of Effects. Small (≤ 100 subjects per arm) studies and those reporting results of interventions implemented before publication of the large faecal occult blood test trials were excluded. Interventions were categorised following the Continuum of Cancer Care and the PRECEDE-PROCEED models and studies were grouped by screening model (opportunistic vs organised). Multifactor interventions targeting multiple levels of care and considering factors outside the individual clinician control, represent the most effective strategy to enhance CRC screening acceptance. Removing financial barriers, implementing methods allowing a systematic contact of the whole target population, using personal invitation letters, preferably signed by the reference care provider, and reminders mailed to all non-attendees are highly effective in enhancing CRC screening acceptance. Physician reminders may support the diffusion of screening, but they can be effective only for individuals who have access to and make use of healthcare services. Educational interventions for patients and providers are effective, but the implementation of organisational measures may be necessary to favour their impact. Available evidence indicates that organised programmes allow to achieve an extensive coverage and to enhance equity of access, while maximising the health impact of screening. They provide at the same time an infrastructure allowing to achieve a more favourable cost-effectiveness profile of potentially effective strategies, which would not be sustainable in opportunistic settings.

Effectiveness of patient-targeted interventions to inform decision making and improve uptake of colorectal cancer genetic evaluation for at-risk individuals: A systematic review

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Background</h3><p>Inherited colorectal cancer syndromes increase the risk of contracting colorectal and other cancers. International guidelines recommend the identification of individuals with hereditary colorectal cancer and the supervision of asymptomatic individuals with a family history. However, detection of hereditary colorectal cancer is suboptimal. The prevalence of genetic counselling and testing for individuals with high genetic risk is low.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Objective</h3><p>To identify, characterize and summarize patient-targeted interventions on improving the uptake of colorectal cancer genetic evaluation for at-risk individuals and enhancing their informed decision making.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Design</h3><p>Systematic review.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Data Sources</h3><p>Six electronic databases (PubMed, CINAHL, Web of Science, Embase, PsycINFO, and Cochrane library) were searched to identify eligible clinical trials from each database's inception to March 25, 2020. The reference lists of the included studies and reviews were checked for additional articles.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Review Methods</h3><p>Studies were screened and independently appraised by two reviewers using the standardized critical appraisal checklist for randomized controlled trials and quasi-experimental studies from the Joanna Briggs Institute. The results were tabulated and reported in descriptive format.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>Based on the inclusion criteria, 8 articles satisfied the inclusion criteria and were included. The studies examined patient-targeted intervention strategies related to risk assessment, education, and decision aids. Outcomes included (1) informed decisions regarding microsatellite instability testing, (2) informed decisions concerning genetic testing, (3) genetic counselling and testing. Most of the included studies revealed that interventions had positive effects on the uptake of colorectal cancer genetic evaluation for at-risk individuals and their informed decision making.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusions</h3><p>There were few studies included in this review, and the results were inconsistent. Based on this review, the conclusion cannot be made that interventions for risk assessment, education, and decision aids have positive effects on the uptake of colorectal cancer genetic evaluation for at-risk individuals and their informed decision making. However, to our knowledge, this is the first systematic review to summarize the effectiveness of patient-targeted interventions to inform decision making and improve uptake of colorectal cancer genetic evaluation for at-risk individuals. This review provides important evidence for related topics. Future studies with rigorous designs are recommended. Nurses have a crucial role in personalized health care. The involvement of nurses in collaboration with all the stakeholders in the development, implementation and evaluation of cancer genetic screening programs to improve genetic referral of individual at risk.</p>

Abstract:Colorectal cancer (CRC) has the highest mortality rate among men and is the second highest among women under fifty, with incidence and mortality rates rising in younger populations. Studies indicate that up to one-third of patients diagnosed before fifty have a family history or genetic factors, highlighting the need for earlier screening. Contrariwise, diagnosis in healthy subjects through screening strategies enables early-stage detection of the tumor and better clinical outcomes. In recent years, mortality rates of CRC in Western countries have been on a steady decline, which is largely attributed to widespread screening programs and advancements in treatment modalities. Indeed, early detection through screening significantly improves prognosis, with stark differences in survival rates between localized and metastatic disease. This article aims to provide a comprehensive review of the existing literature, delving into the performance and efficacy of various CRC screening strategies. It navigates through available screening tools, evaluating their efficacy and cost-effectiveness. The discussion extends to delineating target populations for screening, emphasizing the importance of tailored approaches for individuals at heightened risk.
A Colorectal Cancer Screening Programs in Middle-Income Countries: A Scoping Review

Abstract:Background: Colorectal cancer (CRC) is the 3rd most common cancer worldwide, and a major contributor to cancer-related mortality. In contrast to declining CRC incidence and mortality rates across high-income countries, the CRC burden is increasing in low- and middle-income countries. Although CRC screening has been shown to be a cost-effective intervention that decreases CRC incidence and mortality, screening programs remain an unmet need in most low- and middle-income countries. This article reviews evidence on existing CRC screening efforts in middle income countries, where the majority of new CRC cases and deaths are projected to occur over the next decade. Aims: The aim of this study was to identify and describe opportunistic and organized CRC screening programs in middle income countries and to identify barriers and facilitators of such programs. Methods: We identified countries defined as middle income countries by the World Bank and conducted a scoping literature review using PubMed, Google Scholar, and ScienceDirect. For each country, we identified whether CRC screening guidelines or programs exist on the national, regional, or local levels, and summarized data on screening methods and uptake, when this information was available. We also summarized published literature describing barriers and facilitators to CRC screening in middle income countries. Results: Of the 108 countries defined as middle income countries by the World Bank, we identified CRC screening programs in six lower-middle income countries and 23 upper-middle income countries. Most countries have opportunistic CRC screening guidelines/programs. Countries with organized CRC screening programs had higher screening uptake rates, although very few have achieved CRC screening coverage rates of >50% of the eligible population. Most programs were initiated less than 10 years ago, limiting ability to evaluate effect on CRC incidence and mortality. Several barriers to CRC screening were identified, including lack of physician buy in, participant knowledge and resources, and participant fear of screening. Conclusions: While there has been growth of CRC screening programs in the last decade with the initiation of both opportunistic and organized screening programs in middle income countries, there remain significant barriers to the uptake and implementation of such programs.

Abstract:Purpose: One of the most common cancers in Asia is colorectal cancer (CRC). Early diagnosis and timely treatment are necessary for preventing complications and advanced stages of the disease. It is important to evaluate barriers and facilitators of screening in different countries. This systematic review aimed to identify the barriers and facilitators of CRC screening in Asia. Methods: In this systematic review, for identifying barriers and facilitators of CRC screening, a comprehensive search was conducted in PubMed, Web of Science and Scopus in 12 December 2020. Combination keywords such as colorectal cancer, screening, sigmoidoscopy, colonoscopy, faecal occult blood test, barriers, facilitators and the names of each Asian country were used for searching. Full text original studies in English language were accepted in the review. Results: In total, 36 articles were included in the review. Barriers and facilitators were evaluated. The most common reported barriers were lack of knowledge, fear of result, fear of procedure, fear of pain, lack of awareness, high cost and lack of gastrointestinal symptoms. The most frequent facilitators were having knowledge and awareness of CRC screening, perceived risk and severity, family history of cancer and physician recommendation. Conclusion: For promoting success in CRC screening programmes, knowing what the barriers and facilitators are is necessary. Awareness and various personal, professional and social factors have been shown to be the major barriers toward CRC screening in most Asian countries.

Abstract:BACKGROUND: Risk-tailored screening has emerged as a promising approach to optimise the balance of benefits and harms of existing population cancer screening programs. It tailors screening (e.g., eligibility, frequency, interval, test type) to individual risk rather than the current one-size-fits-all approach of most organised population screening programs. However, the implementation of risk-tailored cancer screening in the population is challenging as it requires a change of practice at multiple levels i.e., individual, provider, health system levels. This scoping review aims to synthesise current implementation considerations for risk-tailored cancer screening in the population, identifying barriers, facilitators, and associated implementation outcomes.METHODS: Relevant studies were identified via database searches up to February 2023. Results were synthesised using Tierney et al. (2020) guidance for evidence synthesis of implementation outcomes and a multilevel framework.RESULTS: Of 4138 titles identified, 74 studies met the inclusion criteria. Most studies in this review focused on the implementation outcomes of acceptability, feasibility, and appropriateness, reflecting the pre-implementation stage of most research to date. Only six studies included an implementation framework. The review identified consistent evidence that risk-tailored screening is largely acceptable across population groups, however reluctance to accept a reduction in screening frequency for low-risk informed by cultural norms, presents a major barrier. Limited studies were identified for cancer types other than breast cancer.CONCLUSIONS: Implementation strategies will need to address alternate models of delivery, education of health professionals, communication with the public, screening options for people at low risk of cancer, and inequity in outcomes across cancer types.

Colorectal Cancer in Pakistan: Is prevention possible?

Abstract:Dear Madam, Among malignancies, colorectal cancer (CRC) has the third- largest incidence Globally. (1) Even though Pakistan is considered as a low-risk area, in 2018, Idrees et a. l reported a 4% to 6.8% incidence. (2) This number is itself significant considering that underreporting remains a grave problem. The aetiology of CRC is vastly based on genetics, but is dietary intake is another major factor to consider. A 2017 meta-analysis shows that the inflammatory extent of diet, measured by the Dietary Inflammation Index, is associated with a 40% increased risk of CRC. (3) Evidence suggests that a diet rich in proinflammatory foods like proteins and fats affects gut microbiota, increases metabolite formation and enhances the risk of CRC. On the contrary, the increased fibre in the diet is shown to have anti-inflammatory effects and reduce this risk. (4). Due to our constraints in resources, healthcare facilities must use aforementioned knowledge to tackle the disease at all fronts. First degree relatives of CRC patients and high-risk individuals can be counselled about life style and dietary modifications at family healthcare facilities. They should be provided with a complete list of foods containing the above-mentioned components. This is especially important in the red-meat consuming areas of Northern Pakistan. This approach of prevention before treatment surpasses the major limitation of costly screening procedures and can be a great benchmark for mass campaigns. A good foundation for our campaigns can be established by taking notes from the CDC’s Screen for Life organization (SFL). In September 2018, through their search engine marketing and digital advertising efforts, SFL garnered more than 360 million impressions and 2 million clicks to web pages. (5) Similar avenues need to be explored in Pakistan to reach the general public. Raising awareness about CRC, the related lifestyle changes and pro-inflammatory foods can be achieved through pamphlets, advertisements, and social media platforms. Funding from such campaigns can also aid further research into CRC, specific to the Pakistani community. The aim should be to promote a more centred knowledge of risk factors prevalent in our society and target them. Considering the inaccessibility of screening methods, this approach of modifying dietary intake in first degree relatives leaves much room to be explored and has a potential to be a cost-effective solution. Further research on this approach, centred on the Pakistani population, is required.

Abstract:Objectives This review aimed to synthesize the available evidence on exploring various factors that affect knowledge, attitudes, and practices (KAP) in colorectal cancer (CRC) screening. Methods A systematic search across five databases was performed to identify factors influencing KAP scores towards CRC screening. The PRISMA guidelines were used to conduct the literature search, and the time spanned is from March to June 2023. The search included observational studies published between January 2000 and June 2023 that met the predetermined review criteria. Data were extracted following the Joanna Briggs Institute (JBI) appraisal checklist to evaluate the quality of the articles. Results Out of 16,904 records, 1174 articles were reviewed in full text, resulting in 43 high‐quality studies included based on the JBI checklist. These studies assessed knowledge (42), attitudes (26), and practices (11) related to CRC screening. Key factors to improving KAP towards CRC screening in the general public were sociodemographic, social media influence, and physician recommendations. For healthcare professionals, factors promoting KAP included screening methods, guidelines, qualifications, and understanding of CRC screening. Educators lacked awareness of CRC symptoms and needed training to teach CRC screening and prevention. Pharmacists showed positive attitudes towards early CRC detection but had varying knowledge levels. Conclusions KAP towards CRC screening is suboptimal among the general public, healthcare professionals, students, educators, and pharmacists worldwide. Routine CRC screening counselling is paramount to improving screening rates. Continuous medical education and training programmes are essential for healthcare professionals to enhance their KAP towards CRC screening. Students and university teachers should be educated and trained about CRC screening to improve their knowledge and foster positive behavioural changes. These comprehensive measures are critical for establishing an effective screening programme.

Abstract:Background In response to a 2011 cholera outbreak in Papua New Guinea, the Government of the Solomon Islands initiated a cholera prevention program which included cholera disease prevention and treatment messaging, community meetings, and a pre-emptive cholera vaccination campaign targeting 11,000 children aged 1–15 years in selected communities in Choiseul and Western Provinces. Methodology and Principal Findings We conducted a post-vaccination campaign, household-level survey about knowledge, attitudes, and practices regarding diarrhea and cholera in areas targeted and not targeted for cholera vaccination. Respondents in vaccinated areas were more likely to have received cholera education in the previous 6 months (33% v. 9%; p = 0.04), to know signs and symptoms (64% vs. 22%; p = 0.02) and treatment (96% vs. 50%; p = 0.02) of cholera, and to be aware of cholera vaccine (48% vs. 14%; p = 0.02). There were no differences in water, sanitation, and hygiene practices. Conclusions This pre-emptive OCV campaign in a cholera-naïve community provided a unique opportunity to assess household-level knowledge, attitudes, and practices regarding diarrhea, cholera, and water, sanitation, and hygiene (WASH). Our findings suggest that education provided during the vaccination campaign may have reinforced earlier mass messaging about cholera and diarrheal disease in vaccinated communities.
Interventions Designed to Increase the Uptake of Lung Cancer Screening: An Equity-Oriented Scoping Review

Abstract:Introduction: Participation in lung cancer screening (LCS) is lower in populations with the highest burden of lung cancer risk (through the social patterning of smoking behavior) and lowest levels of health care utilization (through structurally inaccessible care) leading to a widening of health inequities. Methods: We conducted a scoping review using the Arksey and O'Malley methodological framework to inform equitable access to LCS by illuminating knowledge and implementation gaps in interventions designed to increase the uptake of LCS. We comprehensively searched for LCS interventions (Ovid Medline, Excerpta Medica database, the Cochrane Library, Cumulative Index to Nursing and Allied Health Literature, and Scopus from 2000 to June 22, 2021) and included peer-reviewed articles and gray literature published in the English language that describe an intervention designed to increase the uptake of LCS, charted data using our previously published tool and conduced a health equity analysis to determine the intended-unintended and positive-negative outcomes of the interventions for populations experiencing the greatest inequities. Results: Our search yielded 3572 peer-reviewed articles and 54,292 pieces of gray literature. Ultimately, we included 35 peer-reviewed articles and one gray literature. The interventions occurred in the United States, United Kingdom, Japan, and Italy, focusing on shared decision-making, the use of electronic health records as reminders, patient navigation, community-based campaigns, and mobile computed tomography scanners. We developed an equity-oriented LCS framework and mapped the dimensions and outcomes of the interventions on access to LCS on the basis of approachability, acceptability, availability, affordability, and appropriateness of the intervention. No intervention was mapped across all five dimensions. Most notably, knowledge and implementation gaps were identified in dimensions of acceptability, availability, and affordability. Conclusions: Interventions that were most effective in improving access to LCS targeted priority populations, raised community-level awareness, tailored materials for sociocultural acceptability, did not depend on prior patient engagement/registration with the health care system, proactively considered costs related to participation, and enhanced utilization through informed decision-making.
Colorectal Cancer Screening: A Multicomponent Intervention to Increase Uptake in Patients Aged 45-49

Abstract:Purpose: Colorectal cancer (CRC) screening is recommended starting at age 45, but there has been little research on strategies to promote screening among patients younger than 50. This study assessed the effect of a multicomponent intervention on screening completion in this age group. Methods: The intervention consisted of outreach to patients aged 45 to 49 (n = 3,873) via mailed fecal immunochemical test (FIT) (sent to 46%), text (84%), e-mail (53%), and the extension to this age group of an existing standing order protocol allowing primary care nurses and medical assistants to order FIT at primary care clinics in an urban safety-net system. We used segmented linear regression to assess changes in CRC screening completion trends. Patients aged 51 to 55 were included as a comparison group (n = 3,943). Data were extracted from the EHR. Results: The percentage of patients aged 45 to 49 who were up-to-date with CRC screening (colonoscopy in 10 years or FIT in last year) increased an average of 0.4% (95% CI 0.3, 0.6)) every 30 days before intervention rollout and 2.8% (95% CI 2.5, 3.1) after (slope difference 2.3% [95% CI 2.0, 2.7]). This difference persisted after accounting for small changes in the outcome observed in the comparison group (slope difference 1.7% [95% CI 1.2, 2.2]). Conclusions: These results suggest that the intervention increased CRC screening completion among patients 45 to 49. Health care systems seeking to improve CRC screening participation among patients aged 45 to 49 should consider implementing similar interventions.
Selection of patients for large mailed fecal immunochemical test colorectal cancer screening outreach programs: A systematic review

Abstract:Objective Digital health care offers an opportunity to scale and personalize cancer screening programs, such as mailed outreach for colorectal cancer (CRC) screening. However, studies that describe the patient selection strategy and process for CRC screening are limited. Our objective was to evaluate implementation strategies for selecting patients for CRC screening programs in large health care systems. Methods We conducted a systematic review of 30 studies along with key informant surveys and interviews to describe programmatic implementation strategies for selecting patients for CRC screening. PubMed and Embase were searched since inception through December 2018, and hand searches were performed of the retrieved reference lists but none were incorporated ( n = 0). No language exclusions were applied. Results Common criteria for outreach exclusion included: being up-to-date with routine CRC screening ( n = 22), comorbidities ( n = 20), and personal history ( n = 22) or family history of cancer ( n = 9). Key informant surveys and interviews were performed ( n = 28) to understand data sources and practices for patient outreach selection, and found that 13 studies leveraged electronic medical care records, 10 studies leveraged a population registry (national, municipal, community, health), 4 studies required patient opt-in, and 1 study required primary care provider referral. Broad ranges in fecal immunochemical test completion were observed in community clinic ( n = 8, 31.0–59.6%), integrated health system ( n = 5, 21.2–82.7%), and national regional CRC screening programs ( n = 17, 23.0–64.7%). Six studies used technical codes, and four studies required patient self-reporting from a questionnaire to participate. Conclusion This systematic review provides health systems with the diverse outreach practices and technical tools to support efforts to automate patient selection for CRC screening outreach.

Abstract:Aim: To provide a systematic synthesis of primary care practice-based interventions and their effect on participation in population-based cancer screening programs. Background: Globally, population-based cancer screening programs (bowel, breast, and cervical) have sub-optimal participation rates. Primary healthcare workers (PHCWs) have an important role in facilitating a patient's decision to screen; however, barriers exist to their engagement. It remains unclear how to best optimize the role of PHCWs to increase screening participation. Methods: A comprehensive search was conducted from January 2010 until November 2023 in the following databases: Medline (OVID), EMBASE, and CINAHL. Data extraction, quality assessment, and synthesis were conducted. Studies were separated by whether they assessed the effect of a single-component or multi-component intervention and study type. Findings: Forty-nine studies were identified, of which 36 originated from the USA. Fifteen studies were investigations of single-component interventions, and 34 studies were of multi-component interventions. Interventions with a positive effect on screening participation were predominantly multi-component, and most included combinations of audit and feedback, provider reminders, practice-facilitated assessment and improvement, and patient education across all screening programs. Regarding bowel screening, provision of screening kits at point-of-care was an effective strategy to increase participation. Taking a 'whole-of-practice approach' and identifying a 'practice champion' were found to be contextual factors of effective interventions. The findings suggest that complex interventions comprised of practitioner-focused and patient-focused components are required to increase cancer screening participation in primary care settings. This study provides novel understanding as to what components and contextual factors should be included in primary care practice-based interventions.

Obstacles to Colorectal Cancer Screening in Chinese: A Study Based on the Health Belief Model

Abstract:BACKGROUND: Colorectal cancer (CRC) has emerged as the second most common cancer in Asia. This study uses the Health Belief Model (HBM) to examine the factors associated with the uptake of CRC testing. METHODS: A population-based telephone survey in which 1,004 randomly selected Chinese residents of Hong Kong aged between 30 and 65 yr were interviewed in 2006. Instruments were developed to measure the variables pertinent to the HBM, including perceived susceptibility, severity, benefit, barriers, and cues to action. The relationships of these predictors with CRC testing were examined using bivariate and multiple logistic regression analyses. RESULTS: Ten percent of the respondents had undertaken a CRC screening test. In the multiple logistic regression analyses, knowledge factors that were positively associated with CRC testing included knowledge of CRC symptoms (adjusted odds ratio [aOR] 3.33, 95% confidence interval [CI] 1.21-9.11) and knowledge of CRC risk factors (aOR 2.61, 95% CI 1.16-5.88). Five of the variables pertaining to the HBM were significant correlates of CRC testing-perceived severity of CRC (aOR 0.28, 95% CI 0.13-0.65), perceived health and psychological barriers to CRC testing (aOR 0.42, 95% CI 0.21-0.84, 95% CI 0.21-0.85), perceived access barriers to CRC testing (aOR 0.22, 95% CI 0.05-0.85), physician's recommendation (indicator of cues to action) (aOR 23.50, 95% CI 10.66-51.80), and having health insurance (indicator of cues to action) (aOR 2.06, 95% CI 1.01-4.19). CONCLUSIONS: CRC testing compliance among this Asian population is low. Knowledge of CRC symptoms and risk factors are low. Perceived health, psychological, and access barriers to CRC testing are high. The physician's recommendation and having health insurance coverage significantly increase testing, but physicians mainly recommend patients with a family history of CRC for testing, and only around one-third of the population has health insurance.
Effects of interventions on the screening behavior in female first-degree relatives of breast cancer patients: A systematic review

Abstract:Background: Women are more likely to develop breast cancer if their first-degree relatives (FDRs) have the disease, but they are often unaware of their individual risk and conduct screening behaviors. Objective: This study aimed to evaluate the effectiveness of interventions in increasing breast self-examination, clinical breast examination, and mammography rates in FDRs of breast cancer patients. Methods: We selected randomized clinical trials and quasi-experimental studies in eight databases. Interventions in each study were categorized as "promising", or "non-promising" according to whether they led to a positive change in screening behaviors. Interventions were also coded using the Behavioral Change Techniques (BCTs) Taxonomy and a promise ratio calculated for each. BCTs with a promise ratio ≥2 was classified as "promising". Results: Thirteen studies with 21 different BCTs were included. The most frequent BCTs were "Prompts/cues", "Credible source", and "Instructions on how to perform the behavior". Seven BCTs had a promise ratio of ≥2 and the four most promising were "Information about health consequences" (promise ratio = 6), "Problem solving" (promise ratio = 4), "Demonstration of the behavior" (promise ratio = 4), and "Adding objects to the environment" (promise ratio = 4). Conclusions: This review indicated an overall weak use of theory, and an insufficient description of several interventions to support the assessment of how specific BCTs were activated.

Abstract:A model-based description of the scaling and radial location of turbulent fluctuations in turbulent pipe flow is presented and used to illuminate the scaling behaviour of the very large scale motions. The model is derived by treating the nonlinearity in the perturbation equation (involving the Reynolds stress) as an unknown forcing, yielding a linear relationship between the velocity field response and this nonlinearity. We do not assume small perturbations. We examine propagating helical velocity response modes that are harmonic in the wall-parallel directions and in time, permitting comparison of our results to experimental data. The steady component of the velocity field that varies only in the wall-normal direction is identified as the turbulent mean profile. A singular value decomposition of the resolvent identifies the forcing shape that will lead to the largest velocity response at a given wavenumber–frequency combination. The hypothesis that these forcing shapes lead to response modes that will be dominant in turbulent pipe flow is tested by using physical arguments to constrain the range of wavenumbers and frequencies to those actually observed in experiments. An investigation of the most amplified velocity response at a given wavenumber–frequency combination reveals critical-layer-like behaviour reminiscent of the neutrally stable solutions of the Orr–Sommerfeld equation in linearly unstable flow. Two distinct regions in the flow where the influence of viscosity becomes important can be identified, namely wall layers that scale with R+1/2 and critical layers where the propagation velocity is equal to the local mean velocity, one of which scales with R+2/3 in pipe flow. This framework appears to be consistent with several scaling results in wall turbulence and reveals a mechanism by which the effects of viscosity can extend well beyond the immediate vicinity of the wall. The model reproduces inner scaling of the small scales near the wall and an approach to outer scaling in the flow interior. We use our analysis to make a first prediction that the appropriate scaling velocity for the very large scale motions is the centreline velocity, and show that this is in agreement with experimental results. Lastly, we interpret the wall modes as the motion required to meet the wall boundary condition, identifying the interaction between the critical and wall modes as a potential origin for an interaction between the large and small scales that has been observed in recent literature as an amplitude modulation of the near-wall turbulence by the very large scales.

Abstract:Background: Colorectal cancer (CRC) is the 3rd leading cause of cancer death in the US. Early detection through screening increases survival and decreases mortality. Approximately 59% of eligible adults in the US are up to date on CRC screening with lower rates in minoritized communities. We report on a tailored a CRC screening educational intervention focused on improving CRC screening rates in the Black, Latinx, and Chinese immigrant communities that we serve. Methods: An educational intervention utilizing NCI's Screen to Save CRC colorectal cancer educational materials was tailored through key contact interviews and focus groups with Black, Latinx, and Chinese immigrant communities and made available in Spanish/English/Mandarin. Invitations to participate were primarily disseminated through weekly email blasts to adults (persons age18+). A pre- and post-intervention survey was used to assess patient knowledge and intentions to screen. Persons completing the post-survey received a FIT test for CRC screening with a FIT. We used McNemar test to compare the results pre- and post-survey. Results: Of the 300 participants who completed the post-survey, 117 (39%) were male, and 183 were female (61%). 231 identified as white non-Latinx (77%), 48 (16%) Black, 3 (9%) Asians. 241 (80.3%) completed college. 186 (62%) had received CRC screening—176 (95%) by colonoscopy, 31 (17%) by fecal immunochemical test FIT prior to enrollment. 240 (80%) were eligible for FIT by age. 137 (83%) were interested in receiving FIT. Primary reasons for not pursuing FIT test included choosing to pursue a colonoscopy or having had a recent normal colonoscopy. 88 expressed interest pursuing FIT; 66 (75%) participants signed FIT consent form, and 41 (62%) completed FIT. Knowledge regarding CRC screening improved for the following questions: CRC starts in liver (p=0.0082), small intestine (p<0.0001) and colon and/or rectum (p=0.0027), "FIT checks the stool for fat (p<0.0001), tumors (p<0.0001) and polyps (p=0.0067)," "Risk factors for CRC are poor diet (p=0.0019), smoking/tobacco use (p=0.0148), lack of physical activity(p=0.0012)," "Lynch syndrome is a disorder that runs in families and increases my chances of developing CRC (p<0.0001)" and "Increasing my physical activity will not lower my chances of developing CRC (p=0.0019)." 153 (60.24%) said the education session was great and 91 (36.25%) said it was good. When asked if they would recommend this education session to family and friends, more than 80% of participants agreed. Conclusions: CRC education video helped increase patient knowledge regarding CRC, specifically about risk factors and improved knowledge regarding FIT as a CRC screening modality.
