Abstract:The American Journal of Sports Medicine, Ahead of Print. Background:It is unclear whether the use of concomitant acromioplasty during rotator cuff repair (RCR) improves clinical outcomes and whether the outcomes are affected by acromial type.Purpose:To perform a systematic review of randomized controlled trials comparing clinical outcomes of RCR with and without acromioplasty, with a subanalysis of outcomes based on acromial type.Study Design:Systematic review; Level of evidence, 2.Methods:A systematic review was conducted according to the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines by searching PubMed, the Cochrane Library, and Embase to identify randomized controlled trials that directly compared outcomes between RCR with versus without acromioplasty. A subanalysis was performed on the studies that provided outcomes based on acromial type. The search phrase used was rotator cuff repair (acromioplasty OR subacromial decompression) randomized. Patients were evaluated based on retear rate, reoperation rate, and patient-reported outcomes (PROs).Results:Application of inclusion criteria yielded 5 studies (2 studies were level 1, and 3 studies were level 2) including a total of 409 patients, with 211 patients undergoing RCR alone (group A) and 198 patients undergoing RCR with acromioplasty (group B). The mean patient age was 58.5 and 58.3 years in groups A and B, respectively. The mean follow-up time was 52.9 months, and the overall percentage of male patients was 54.1%. The rotator cuff tear size was 20.7 mm and 19.8 mm for groups A and B, respectively. No significant differences were found between groups for any of the PROs at final follow-up. Overall retear rates did not significantly differ between groups based on acromial type. Between 2 studies that measured reoperation rate, a significantly higher reoperation rate was found for the nonacromioplasty group (15%) versus the acromioplasty group (4.1%) (P = .031). One of these studies found that 5 of 9 patients (56%) with a type III acromion in the nonacromioplasty group underwent reoperation compared with 0 of 4 patients with a type III acromion in the acromioplasty group.Conclusion:There is some evidence that acromioplasty during RCR reduces the risk for later reoperation. This may be particularly true for patients with type III acromions, although further studies with larger sample sizes are needed to corroborate these data.

Abstract:BACKGROUND: Rotator cuff tendon tears at the shoulder joint are one of the most common causes of shoulder discomfort and impairment. OBJECTIVE: This study aims to compare the long-term patient-reported outcomes of arthroscopic acromioplasty to arthroscopic rotator cuff repair. METHODS: Patients who underwent arthroscopic repair for irreparable rotator cuff tears between January 1 and December 1, 2022, and had any morphology of the acromial bone were randomly assigned to either the arthroscopic acromioplasty or arthroscopic rotator cuff repair groups. Excluded from the study were patients who had undergone revision surgery, had subscapularis involvement, had severe neurologic diseases, or had passed away. Baseline and long-term follow-up surveys collected scores from the American Shoulder and Elbow Surgeons (ASES), Simple Shoulder Test (SST), University of California-Los Angeles (UCLA), Visual Analog Scale (VAS) for Pain, and consistent scores. Averages were calculated after adjusting for standard deviation. RESULTS: In the first trial, 30 patients from Group A (arthroscopic acromioplasty) and 30 patients from Group B (arthroscopic rotator cuff repair) underwent long-term follow-up. The average age of patients in Group A was 58.62 ± 5.95 years, while in Group B it was 59.11 ± 7.98 years. At the final follow-up, there were no statistically significant differences between Group A and Group B patients for ASES (P= 0.233), VAS pain (P= 0.154), Constant (P= 0.584), or SST (P= 0.155). However, Group A showed statistically significant improvements compared to Group B at the 2-year postoperative follow-up for UCLA (P= 0.015) and SF-12 (P= 0.021). Additionally, in the short-term results, Group A outperformed Group B significantly in forward flexion (P< 0.05), abduction (P< 0.05), and external rotation in abduction (P< 0.05). CONCLUSION: Our study revealed that acromioplasty alone was superior to rotator cuff repair in terms of patient assessments of shoulder mobility and discomfort.

Acromioplasty combined with arthroscopic rotator cuff repair can reduce the risk of reoperation: a systematic review and meta-analysis

Abstract:Background: Whether to perform acromioplasty in arthroscopic rotator cuff repair (ARCR) is controversial, and the optimal surgical approach for rotator cuff tear repair is unknown. The purpose of this study was to compare the reoperation rate, retear rate and patient-reported outcomes (PROs) of ARCR with those of ARCR combined with acromioplasty (ARCR-A). Methods: PubMed, Embase and Cochrane Library were searched for relevant literature dated between database inception and 4 December 2023. The primary outcomes of this study were the reoperation rate and the retear rate. The secondary outcomes were PROs, including the visual analogue scale (VAS) pain score, the American Shoulder and Elbow Surgeons (ASES) score, the University of California-Los Angeles (UCLA) score, the Constant score and the Western Ontario Rotator Cuff (WORC) score. The quality of the included studies was evaluated by using the risk of bias assessment tool. RevMan 5.3 software was used for meta-analysis. Fixed (I2 <50%) or random (I2 ≥50%) effects models were applied to calculate the effect size. Results: Meta-analysis revealed that ARCR-A had a lower reoperation rate (OR = 0.35, 95%CI: 0.15-0.85, p = 0.02), but the difference in the retear rate between ARCR-A and ARCR was not significant (p = 0.25). In type 2 acromion patients, the reoperation rate was not significantly different between ARCR and ARCR-A (p = 0.12), but, for type 3 acromion patients, the retear rate was lower for ARCR-A than for ARCR (OR = 0.12, 95%CI: 0.01-0.94, p = 0.04). There were statistically significant differences in the 6-month postoperative Constant scores (p < 0.001), VAS pain scores (p = 0.003) 12-month postoperative ASES scores (p = 0.02) and 24-month postoperative WORC scores (p = 0.04), but these differences were not clinically significant. Conclusions: Combining ARCR with acromioplasty can reduce the rate of reoperation, especially in patients with type 3 acromion, but it provides no clinically important change in the retear rate and postoperative PRO compared with ARCR.
Clinical effect of modified anterolateral acromioarthroplasty during arthroscopic rotator cuff repair

Abstract:Background: This study aimed to compare the clinical effect of modified anterolateral and traditional acromioplasty in arthroscopic rotator cuff repair. Methods: The clinical data of 92 patients with total rotator cuff tears admitted to the Department of Joint Surgery of Jinhua Central Hospital from January 2016 to December 2019 were retrospectively analyzed. Among them, 42 patients underwent traditional acromioplasty during arthroscopic rotator cuff repair, and 50 underwent modified anterolateral acromioplasty. Patients were evaluated for preoperative and postoperative shoulder function, pain and critical shoulder angle, and incidence of rotator cuff re-tear at 12 months postoperatively. Results: The preoperative general data of patients in the classic and modified anterolateral acromioplasty groups did not differ significantly (P > 0.05) and were comparable. The UCLA, ASES, and Constant shoulder joint scores were significantly improved in both groups. The VAS score was significantly decreased at 12 months postoperative than preoperative, with a statistically significant difference (P ≤ 0.05). Shoulder function and pain scores did not differ significantly between the two groups at 12 months postoperatively (P > 0.05). The CSA did not differ significantly between preoperative and postoperative 12 months in the traditional acromioplasty group (P > 0.05). However, 12 months postoperative CSA in the modified anterolateral acromioplasty group was significantly smaller than the preoperative CSA, with a statistically significant difference (P ≤ 0.05). The rates of rotator cuff re-tears were 16.67% (7/42) and 4% (2/50) in the two groups at 12 months postoperatively, respectively, with statistically significant differences (P ≤ 0.05). Conclusions: Traditional and modified anterolateral acromioplasty while treating total rotator cuff tears using arthroscopic rotator cuff repair significantly improves shoulder joint function. However, modified anterolateral acromioplasty significantly reduced the CSA value and decreased the incidence of rotator cuff re-tears.

Abstract:Adsorption isoterms and capillary condensation in an open slit with walls decorated with arrays of pillars are examined using the density functional theory. Compared with the main substrate, the pillars can have the same or different parameters in the Lennard-Jones interaction potential between them and the fluid in the slit. The roughness of the solid surface, defined as the ratio between the area of the actual surface and the area of the surface free of pillars, is controlled by the height of the pillars. It is shown that the capillary condensation pressure first increases with increasing roughness, passes through a maximum, and then decreases. The amount of adsorbed fluid at constant volume of the slit has, in general, a nonmonotonic dependence on roughness. These features of adsorption and capillary condensation are results of increased surface area and changes in the fluid-solid potential energy due to changes in roughness.

Abstract:Abstract Background The shoulder joint has the greatest range of motion of any other joint. Pain of the shoulder joint is very common and accounts for about 16% of musculoskeletal disorders taking the third place after spine and knee. Rotator cuff disorders including degeneration or tear are most common causes of chronic shoulder pain. Causes of rotator cuff tear (RCT) still unclear and occur as a result of many factors. As degenerative changes of the rotator cuff that occur in old age increase the risk of rotator cuff tear also overhead workers and athletes considered a risk factor. Objective To evaluate the role of acromioplasty in arthroscopic repair of small to medium sized full thickness tear of rotator cuff according to the complication rate, the active range of motion and the function scores of the shoulder to add surgeons in deciding whether acromioplasty must be done routinely during arthroscopic repair of rotator cuff or not. Patients and Methods During the period between February 2019 and May 2022, A Prospective, Randomized Controlled Clinical trial was conducted in Ain Shams University hospital in Cairo. This study was designed to involve 36 cases with symptomatic small to medium sized RC tear, the patients were separated into two groups of 18. Group A comprised those applied with arthroscopic rotator cuff repair, bursectomy and antero- inferior acromioplasty. In Group B, arthroscopic rotator cuff repair was applied with bursectomy only and no acromioplasty. Each patient in this study was carefully assessed clinically by taking a detailed medical history and adequate clinical examination. Results There was a significant difference between the pre-operative and post-operative ASES score, WORC score and ROM but according to postoperative evaluation regarding ASES score, WORC score and ROM there was no statistically significance between the group of acromioplasty and the group of non-acromioplasty. Regarding complications, 3 cases of study population (36) cases were complicated, one case in group A (frozen shoulder) and 2 cases in group B (frozen and revision). With respect of the literature, the results were comparable to that of the literature as regard the functional outcome and complication rate. Conclusion The result of this study showed that there may be no need for routine application of acromioplasty during arthroscopic rotator cuff repair as it has concerns being less cost effective. We acknowledge that the cost was not assessed in our study. However, they are intuitive. Also, more long term follow up studies are required to assess the incidence of retear in rotator cuff repair without-acromioplasty as controversy is still present in the literature.

Abstract:This paper presents an investigation into the impact that plug-in hybrid electric vehicles (PHEVs) could have to mitigate the effects of fault-delayed voltage recovery. The energy storage and conversion system in PHEVs, given potentially high levels of future market penetration, represent a sizable dispersed energy storage resource or reactive power source that electric utilities could leverage to their benefit. In this investigation the impact of PHEVs utilized as reactive power resources on fault delayed voltage recovery on distribution systems is analyzed. It is known that fault delayed voltage recovery can cause serious problems on distribution circuits resulting in stalled air-conditioners, motor tripping and other load disruption events. The impact of these phenomena on the performance of distribution systems is documented with and without PHEVs present.

Abstract:The irreparable posterosuperior rotator cuff tear describes a tear of the supraspinatus and/or infraspinatus tendon that is massive, contracted, and immobile in both the anterior-posterior and medial-lateral directions. Patients with an intact subscapularis and preserved forward elevation are challenging to treat because there is not a consensus treatment algorithm. For low-demand, elderly patients, several subacromial surgical options are available that can provide pain relief without the risks or burden of rehabilitation posed by reverse total shoulder arthroplasty or a complex soft-tissue reconstruction (e.g., superior capsular reconstruction, tendon transfer, bridging grafts). Debridement, more specifically the "smooth-and-move" procedure, offers a reliable outcome with documented improvements in pain and function at long-term follow-up. Similarly, the biodegradable subacromial balloon spacer (InSpace; Stryker, Kalamazoo, MI) has been shown to significantly improve pain and function in patients who are not responsive to nonoperative treatment. Disease progression with these options is possible, with a small percentage of patients progressing to rotator cuff arthropathy. Biologic tuberoplasty and bursal acromial reconstruction are conceptually similar to the balloon spacer but instead use biologic grafts to prevent bone-to-bone contact between the humeral head and the acromion. Although there is no single gold standard treatment, the variety of surgical techniques allows patients and surgeons to effectively manage these challenging situations.
Clinical and Radiological Outcomes of Arthroscopic Superior Capsular Reconstruction Versus Primary Rotator Cuff Repair in Massive Rotator Cuff Tears: A Propensity Score-Matched Study

Abstract:Background: Arthroscopic superior capsular reconstruction (aSCR) has emerged as a treatment option for managing massive rotator cuff tears (MRCTs) given the unpredictable results after an arthroscopic rotator cuff repair (aRCR). Yet, few comparative studies of aSCR and aRCR have been conducted. Purpose: To compare the clinical and radiological outcomes between aRCR and aSCR in patients with MRCT. Study design: Cohort study; Level of evidence, 3. Methods: A total of 163 cases of MRCT from 2010 to 2020 with follow-up ≥2 years were retrospectively reviewed. Among them, 102 had aRCR and 61 had aSCR using fascia lata autograft. Propensity score matching was used to select controls matched for age, sex, diabetes mellitus, osteoporosis, preoperative American Shoulder and Elbow Surgeons score, Single Assessment Numeric Evaluation score, Constant score, pain visual analog scale (pVAS) score, range of motion (ROM), tear size, global fatty degeneration index, and acromiohumeral distance (AHD). Last, 33 cases in each group were selected after propensity score matching. Radiological assessment was conducted using serial postoperative magnetic resonance imaging. Pre- and postoperative findings-including American Shoulder and Elbow Surgeons, pVAS, Single Assessment Numeric Evaluation, and Constant scores and ROM-were assessed to compare clinical outcomes. For radiological outcomes, global fatty degeneration index, AHD, and healing rate were evaluated. Healing failure was defined as Sugaya classification IV or V in the aRCR group, as compared with a full-thickness tear of the graft in the aSCR group, which corresponded to Sugaya classification IV or V. Results: Postoperative clinical outcomes were significantly improved at the final follow-up in both groups. In the aSCR group, postoperative forward flexion, pVAS, and AHD were significantly improved as compared with the aRCR group (mean, 161° vs 148° [P = .02]; 1.03 vs 1.64 [P = .047]; 7.00 vs 5.23 mm [P < .001], respectively). The healing rate was 20 of 33 (60.6%) for aRCR and 29 of 33 (87.9%) for aSCR (P = .022). Conclusion: aSCR and aRCR are effective and reliable treatment options for MRCT. However, when compared with aRCR, aSCR showed improved clinical outcomes, including pVAS score, postoperative ROM, and favorable radiological findings, including AHD and a higher healing rate.

Abstract:Massive irreparable rotator cuff tears (MIRCTs) represent a vexing problem, particularly in the physiologically young patient with no glenohumeral arthritis and high functional expectations. Not even the last line of defense, the reverse shoulder arthroplasty can help all of our patients with MIRCTs and resultant pseudoparalysis, and in younger patients, joint preservation could be a preferable first line intervention. Superior capsular reconstruction (SCR) shares the joint preservation arena with an evolving multitude of options, including arthroscopic debridement, partial rotator cuff repair, biologic tuberoplasty, interposition balloon spacer, and lower trapezius tendon transfer. SCR shows promising biomechanical and clinical outcomes, including restoration of native glenohumeral biomechanics and contact pressures, achievement of humeral head depression, reduction of pain, and improved patient reported outcome measures (PROMs) and range of motion (ROM), despite a high rate of graft re-tear or discontinuity. Yet, comparable results may be achieved via simpler procedures including debridement, partial repair, tuberoplasty, and/or spacer. Ultimately, the true value of SCR, or any other technique, would be proven improvement in strength or survivorship from subsequent surgery, particularly reverse arthroplasty. Optimizing indications for these constantly evolving techniques is a moving target and essential for selecting the highest value procedures for patients based on their individual needs.

Abstract:BACKGROUND: Margin convergence (MC) and superior capsular reconstruction (SCR) are common treatment options for irreparable rotator cuff tears in younger patients, although they differ in associated costs and operative times. The purpose of this study was to compare range of motion, patient reported outcomes (PROs), and re-operation rates following MC and SCR. We hypothesized superior outcomes after SCR relative to MC regarding functional outcomes, subjective measures, and re-operation rates.METHODS: This was a multicenter retrospective review of 59 patients from three surgeons treating irreparable rotator cuff tears with either MC (n=28) or SCR (n=31) and minimum 1-year follow-up from 2014-2019. Visual Analogue Scale (VAS) for pain, Subjective Shoulder Value (SSV), active forward flexion (FF), external rotation (ER), re-tear rate, and conversion rate to reverse shoulder arthroplasty were evaluated. T-tests and Chi-Squared tests were used for continuous and categorical variables, respectively (*p<0.05).RESULTS: Baseline demographics, range of motion, and MRI findings were similar between groups. Average follow-up was 31.5 months and 17.8 months for MC and SCR groups (p<0.001). The MC and SCR groups had similar postoperative FF (151 ± 26 vs 142 ± 38 degrees; p=0.325) and ER (48 ± 12 vs 46 ± 11 degrees; p=0.284), with both groups not improving significantly from their preoperative baselines. However, both cohorts demonstrated significant improvements in VAS (MC: 7.3 to 2.5; SCR: 6.4 to 1.0) and SSV (MC: 54% to 82%; SCR: 38% to 87%). There were no significant differences in postoperative VAS, SSV, and rates of re-tear or rates of conversion to arthroplasty between groups. In patients with preoperative pseudoparesis (FF < 90°), SCR (n=9) resulted in greater postoperative FF than MC (n=5) (141 ± 38 vs 67 ± 24 degrees; p=0.002).CONCLUSION: Both MC and SCR demonstrated excellent postoperative outcomes in the setting of massive irreparable rotator cuff tear, with significant improvements in PROs and no significant differences in range of motion. Specifically for patients with preoperative pseudoparesis, SCR was more effective in restoring forward elevation. Further long-term studies are needed to compare outcomes and establish appropriate indications.

Abstract:Background: Older patients with shoulder instability have a higher prevalence of rotator cuff tears and anterior capsular lesions. Simultaneous rotator cuff repair and labral repair are commonly performed to improve shoulder stability and function. Purpose: To investigate the clinical outcomes of arthroscopic rotator cuff repair for older patients with shoulder dislocations combined with massive rotator cuff tears and intact labral tissue. Study Design: Case series; Level of evidence, 3. Methods: A cohort consisting of 11 patients older than 50 years with shoulder dislocations and massive rotator cuff tears undergoing arthroscopic rotator cuff repair was identified between December 2015 and January 2018. Rotator cuff repair was performed after Bankart, superior labral anterior-posterior, and humeral avulsion of the glenohumeral ligament lesions were excluded during arthroscopic surgery. Preoperative and 12-month postoperative outcomes including modified University of California Los Angeles (UCLA), American Shoulder and Elbow Surgeons (ASES), Western Ontario Shoulder Instability Index (WOSI), and visual analog scale for pain scores as well as range of motion (ROM) were recorded. Results: The supraspinatus tendon was torn in all patients. Also, 36.4% of the patients had 3 rotator cuff tendons torn. For shoulder function, the preoperative UCLA score (12.1 ± 2.5 [range, 9-16]) and ASES score (35.4 ± 12.7 [range, 24-44]) significantly improved to 29.4 ± 4.3 (range, 24-35; P < .001) and 79.4 ± 16.0 (range, 45-95; P < .001), respectively, at 12 months postoperatively. None of the patients experienced shoulder redislocations at 12 months after surgery. For shoulder stability, the postoperative WOSI score (156.8 ± 121.0 [range, 45-365]) was significantly better than was the preoperative score (713.0 ± 238.6 [range, 395-1090]) (P < .001). For comparisons between preoperative and postoperative ROM, forward flexion, abduction, and external and internal rotation at the side significantly improved. Conclusion: For patients older than 50 years with shoulder dislocations combined with massive rotator cuff tears and an intact labrum, arthroscopic rotator cuff repair alone achieved satisfactory functional outcomes and ROM without the recurrence of dislocations.

Abstract:Repair of rotator cuff tear is challenging because poor biologic factors can negatively impact tendon healing and risk complications. Sometimes, instead of attempting complex procedures with a guarded prognosis to reconstruct or replace rotator cuff like tissue, less is more. Minimally invasive, arthroscopic subacromial balloon spacer (SBS) for massive rotator cuff tears, demonstrates excellent long-term outcomes. Radiographically, SBS outcomes may be concerning, but clinically, patients do well. If pain is the primary complaint and function is maintained, the balloon spacer is an excellent option with a fast recovery. However, if function is deficient, or if the patient prioritizes strength recovery as their primary desired outcome, then more invasive procedures are often indicated.
Mid-term Results after Arthroscopic Repair of Full-Thickness Rotator Cuff Tears

Abstract:OBJECTIVE:To evaluate the mid-term results of arthroscopic repair of full-thickness rotator cuff tears.METHODS:From December 2002 to May 2007, 35 patients(35 shoulders) with full-thickness rotator cuff tears underwent arthroscopic treatment. Five patients were lost in the follow-up period, leaving 30 patients available for evaluation. There were 15 male and 15 female patients, the average age was 55.6 years(31-74 years). Three left shoulder and 27 right ones were involved. All the patients underwent subacromial bursectomy and acromioplasty, 19 cases were repaired by suture anchor. Eleven tears were repaired by suture anchor combined with side-to-side suture. Sixteen patients underwent single-row repair and 14 patients underwent dual-row repair. The follow-up was completed on June 2012. The University of California at Los Angeles (UCLA) scoring system was adopted before operation and at the final evaluation.RESULTS:Thirty patients were followed up for an average of 78.5 months(range 5-10 years). The average score increased from 14.2 ± 3.1 to 33.6 ± 2.1 (t = -37.154, P = 0.000) . The mean pain score was 2.5 ± 0.9 vs.9.5 ± 1.0(t = -24.466, P = 0.000) for preoperative vs. postoperative, the function score was 4.5 ± 1.5 vs. 9.4 ± 1.1 (t = -18.500, P = 0.000), the mean forward flexion score was 3.3 ± 1.6 vs. 4.9 ± 0.2(t = -5.614, P = 0.000), the mean forward flexion strength was 3.9 ± 0.5 vs. 4.7 ± 0.4 (t = -6.591, P = 0.000). The results were 19 excellent, 11 good. The average scores of single-row group and double-row group were 33.6 ± 1.7 and 33.6 ± 2.6 respectively. All patients were satisfied with the operation.CONCLUSIONS:This surgery has many advantages such as mini-invasion and rapid recovery. The clinical results of both single-row repair and dual-row repair are satisfactory. The key to the operation lies in accurate tear pattern recognition, enough tendon release and correct suturing method.

Abstract:The InSpace subacromial balloon spacer is indicated for the treatment of massive irreparable rotator cuff tears. The device is placed in the subacromial space with the aim of restoring shoulder function by limiting painful acromiohumeral contact and recentering the superiorly migrated humeral head. However, controversy exists because 2 randomized controlled trials have produced conflicting findings with regards to efficacy. These findings, along with narrow indications, have led to low use of the balloon among European and Latin American surgeons. The controversy generated by conflicting randomized controlled trials poses challenges in the interpretation of cost effectiveness, especially when the role of the balloon among a plethora of other options including superior capsular reconstruction (either with long head of biceps or dermal allograft), graft augmentation, tendon transfer, biologic tuberoplasty, acromial resurfacing, and reverse shoulder arthroplasty is not well defined. However, it is my opinion that the balloon has particular value in older, low-demand patients, in whom severe comorbidities make a shorter surgical duration and quicker recovery appealing.
