Abstract:Circulation Research, Ahead of Print. BACKGROUND:Salt sensitivity of blood pressure (SSBP), characterized by acute changes in blood pressure with changes in dietary sodium intake, is an independent risk factor for cardiovascular disease and mortality in people with and without hypertension. We previously found that elevated sodium concentration activates antigen-presenting cells (APCs), resulting in high blood pressure, but the mechanisms are unknown. Here, we hypothesized that APC-specific JAK2 (Janus kinase 2) through STAT3 (signal transducer and activator of transcription 3) and SMAD3 (small mothers against decapentaplegic homolog 3) contributes to SSBP.METHOD:We performed bulk or single-cell transcriptomic analyses following in vitro monocytes exposed to high salt and in vivo high sodium treatment in humans using a rigorous salt-loading/depletion protocol to phenotype SSBP. We also used a myeloid cell-specific CD11c+JAK2 knockout mouse model and measured blood pressure with radiotelemetry after N-omega-nitro-L-arginine-methyl ester and a high salt diet treatment. We used flow cytometry for immunophenotyping and measuring cytokine levels. Fluorescence in situ hybridization and immunohistochemistry were performed to spatially visualize the kidney's immune cells and cytokine levels. Echocardiography was performed to assess cardiac function.RESULTS:We found that high salt treatment upregulates gene expression of the JAK/STAT/SMAD pathway while downregulating inhibitors of this pathway, such as suppression of cytokine signaling and cytokine-inducible SH2, in human monocytes. Expression of the JAK2 pathway genes mirrored changes in blood pressure after salt loading and depletion in salt-sensitive but not salt-resistant humans. Ablation of JAK2, specifically in CD11c+APCs, attenuated salt-induced hypertension in mice with SSBP. Mechanistically, we found that SMAD3 acted downstream of JAK2 and STAT3, leading to increased production of highly reactive isolevuglandins and proinflammatory cytokine IL (interleukin)-6 in renal APCs, which activate T cells and increase production of IL-17A, IL-6, and TNF-α (tumor necrosis factor-alpha).CONCLUSIONS:Our findings reveal the APC JAK2 signaling pathway as a potential target for the diagnosis and treatment of SSBP in humans.

Abstract:Rationale: Chronic inflammation is a major contributor to the progressive pathology of hypertension, and T-cell activation is required for the genesis of hypertension. However, the precise role of myeloid cells in this process is unclear.Objective: To characterize and understand the role of peripheral myeloid cells in the development of hypertension.Methods and Results: We examined myeloid cells in the periphery of hypertensive mice and found that increased numbers of CD11b(+)Gr1(+) myeloid cells in blood and the spleen are a characteristic of 3 murine models of experimental hypertension (angiotensin II, L-NG-nitroarginine methyl ester, and high salt). These cells express surface markers and transcription factors associated with immaturity and immunosuppression. Also, they produce hydrogen peroxide to suppress T-cell activation. These are characteristics of myeloid-derived suppressor cells (MDSCs). Depletion of hypertensive MDSCs increased blood pressure and renal inflammation. In contrast, adoptive transfer of wild-type MDSCs to hypertensive mice reduced blood pressure, whereas the transfer of nicotinamide adenine dinucleotide phosphate oxidase 2-deficient MDSCs did not.Conclusion: The accumulation of MDSCs is a characteristic of experimental models of hypertension. MDSCs limit inflammation and the increase of blood pressure through the production of hydrogen peroxide.
Peripheral Blood Mononuclear Cells from Patients with Systemic Lupus Erythematosus Induce Salt Sensitivity of Blood Pressure in Humanized Mice

Abstract:Salt sensitivity of blood pressure (SSBP) is an independent risk factor for cardiovascular disease (CVD), even in normotensive people. Systemic lupus erythematosus (SLE) is also a risk factor for CVD and hypertension. SLE and CVD are on the rise, and it is not known if this is attributable to the increased dietary salt in the modern diet and SSBP. Isolevuglandins (IsoLGs) adduct self-proteins forming neoantigens in antigen presenting cells (APC) to activate T cells and produce cytokines that promote sodium retention, kidney damage, salt sensitivity of blood pressure, and SLE. We hypothesized that high salt intake increases both blood pressure and the proinflammatory milieu in mice humanized with the immune cells of patients with SLE. We humanized female NSG/MHCI/MHCII−/− immunodeficient mice with peripheral blood mononuclear cells (PBMCs) isolated from patients with SLE (n=5) and controls without autoimmune disease (n=4). The blood pressure was measured with radiotelemetry, immune phenotyping was performed with flow cytometry, and vascular reactivity was assessed in mesenteric arteries using wire myography after two weeks of a 4% high salt diet. High salt treatment significantly increased systolic blood pressure in lupus mice compared to the controls (135.5 vs. 123.8 mmHg, p=0.014). APC infiltration (macrophages, monocytes, dendritic cells), their activation (CD86) and proinflammatory markers (IL-23 p19, TNF-alpha), and oxidative stress (Isolevuglandins, Nox2) were significantly higher in the kidneys of lupus mice. Moreover, the signaling molecule p-STAT3 was significantly higher in myeloid cells in the kidneys of lupus mice. The classical monocyte and proinflammatory markers were significantly elevated in the spleen lymphocytes, and proinflammatory markers were increased in the kidney, spleen, and aorta in lupus mice. These mice exhibited vascular dysfunction (p<0.05), suggesting reduced vascular compliance. We measured urine albumin and found that lupus mice tended to excrete more albumin than the controls (17.24 vs. 25.89 μg/ml p=0.142), suggesting greater kidney injury. In conclusion, our findings suggest that immune cells from patients with SLE, in response to high salt intake, increase blood pressure and the proinflammatory milieu in the kidney, aorta, and spleen; impair vascular compliance; and worsen albuminuria in immunodeficient mice. 1R01HL144941-01A1 1R03HL155041 23CDA1053072. Sodium-Independent Dietary Effects on Renal Immune Cell Infiltration in Salt-Sensitive Hypertension.

Abstract:Recent studies have shown sodium-independent dietary effects to be important in the development of Dahl SS hypertension and renal disease. Dahl SS/JrHsdMcwiCrl (SS/Crl) rats fed a grain-based diet (Teklad 5L2F) were less susceptible to salt-induced blood pressure elevation (116.5±1.2 vs 141.9±14.4 mmHg) and albuminuria (21.7±3.5 vs 162.9±22.3 mg/day) compared to SS/JrHsdMcwi (SS/Mcw) rats fed a 4.0% high salt casein-based diet (AIN-76A) for 3 weeks (n=6-8/group). Given the role of immunity in hypertension, SS/Crl rats displayed significantly less total CD45+ leukocyte infiltration in the kidney than SS/Mcw (6.1±0.9 vs 11.6±2.6 x10 6 cells/kidney). Reductions were observed in all subsequent immune cell populations, including CD3+ T cells (6.8±0.8 vs 14.4±2.2 x10 5 ), CD45R+ B cells (1.5±0.4 vs 6.7±2.2 x10 5 ), and CD11b/c+ antigen presenting cells (5.0±0.7 vs 9.2±2.1 x10 6 cells/kidney) in SS/Crl versus SS/Mcw. The SS/Crl immune system appears to be less activated, as demonstrated by a respective 44.8% and 66.8% reduction in CD4+CD25+ and CD8+CD25+ T cells, as well as a 40.4% decrease in TNFα-producing CD4+ T cells. Upon taking a discovery approach, RNA sequencing data (n=4 pools of 3 rats each) revealed genes related to hematopoietic cell lineage, the complement and coagulation cascade, B cell signaling, and primary immunodeficiency. Specifically, RNA expression of components of the NLRP3 inflammasome pathway were upregulated, including TLR4 (44.0%), NLRP3 (33.8%) and caspase-1 (26.4%), with a similar trend for IL-1β (25.4%), in the renal medulla of SS/Mcw rats fed high salt. This coincided with increased protein expression of NLRP3 (1.5-fold) and ASC (3.2-fold), together demonstrating increased NLRP3 inflammasome mRNA and protein expression in SS/Mcw rats after high salt challenge. Interestingly, RNA expression of these NLRP3 inflammasome proteins were significantly reduced in SS/Crl versus SS/Mcw rats fed high salt, including TLR4 (39.2%), NLRP3 (31.2%), caspase-1 (32.8%) and IL-1β (39.2%). These data indicate that the NLRP3 inflammasome may be a key mechanistic step in determining how these non-sodium components of the diet affect immunity, and warrant further studies to elucidate its role on salt-sensitive hypertension and renal damage.

Abstract:The immune system plays a critical role in the development of hypertension. The immune response consists of pro-inflammatory cells, but also immunosuppressive cells that reduce T cell function. An important category of natural immunosuppressive cell is myeloid-derived suppressor cells (MDSC). We now show that blood and spleen CD11b+ Gr1+ myeloid cells are elevated 2-fold in both angiotensin II and L-NAME induced hypertension. These increased myeloid cells are MDSC in that they elevate IL-4R expression and suppress T cell proliferation. When hypertensive mice were depleted of MDSC, using either anti-Gr1 antibody or gemcitabine, there was a 15 mmHg rise in blood pressure and aggravation of T cells activation with increased production of IFN-γ, TNFα and IL-17 in both spleen and kidney. In contrast, adoptive transfer of MDSC reduced blood pressure in angiotensin-II induced hypertension by 25 mmHg (see Figure). These data suggest a new concept, that the accumulation of MDSC is a compensatory response to the inflammation induced by hypertension. They also indicate that MDSC play an important role in regulating blood pressure.

Abstract:The Dahl salt-sensitive (SS) rat is an established model of SS hypertension and renal damage. In addition to salt, other dietary components were shown to be important determinants of hypertension in SS rats. With previous work eliminating the involvement of genetic differences, grain-fed SS rats from Charles River Laboratories (SS/CRL; 5L2F/5L79) were less susceptible to salt-induced hypertension and renal damage compared with purified diet-fed SS rats bred at the Medical College of Wisconsin (SS/MCW; 0.4% NaCl, AIN-76A). With the known role of immunity in hypertension, the present study characterized the immune cells infiltrating SS/MCW and SS/CRL kidneys via flow cytometry and RNA sequencing in T-cells isolated from the blood and kidneys of rats maintained on their respective parental diet or on 3 weeks of high salt (4.0% NaCl, AIN-76A). SS/CRL rats were protected from salt-induced hypertension (116.5±1.2 versus 141.9±14.4 mm Hg), albuminuria (21.7±3.5 versus 162.9±22.2 mg/d), and renal immune cell infiltration compared with SS/MCW. RNA-seq revealed >50% of all annotated genes in the entire transcriptome to be significantly differentially expressed in T-cells isolated from blood versus kidney, regardless of colony or chow. Pathway analysis of significantly differentially expressed genes between low and high salt conditions demonstrated changes related to inflammation in SS/MCW renal T-cells compared with metabolism-related pathways in SS/CRL renal T-cells. These functional and transcriptomic T-cell differences between SS/MCW and SS/CRL show that dietary components in addition to salt may influence immunity and the infiltration of immune cells into the kidney, ultimately impacting susceptibility to salt-induced hypertension and renal damage.

Abstract:BACKGROUND: Salt sensitivity of blood pressure (SSBP) is an independent risk factor for cardiovascular morbidity and mortality, yet the etiology is poorly understood. We previously found that serum/glucocorticoid-regulated kinase 1 (SGK1) and epoxyeicosatrienoic acids (EETs) regulate epithelial sodium channel (ENaC)-dependent sodium entry into monocyte-derived antigen-presenting cells (APCs) and activation of NADPH oxidase, leading to the formation of isolevuglandins (IsoLGs) in SSBP. Whereas aldosterone via the mineralocorticoid receptor (MR) activates SGK1 leading to hypertension, our past findings indicate that levels of plasma aldosterone do not correlate with SSBP, and there is little to no MR expression in APCs. Thus, we hypothesized that cortisol acting via the glucocorticoid receptor (GR), not the MR in APCs mediates SGK1 actions to induce SSBP. METHODS: We performed cellular indexing of transcriptomes and epitopes by sequencing (CITE-Seq) analysis on peripheral blood mononuclear cells of humans rigorously phenotyped for SSBP using an inpatient salt loading/depletion protocol to determine expression of MR, GR, and SGK1 in immune cells. In additional experiments, we performed bulk transcriptomic analysis on isolated human monocytes following in vitro treatment with high salt from a separate cohort. We then measured urine and plasma cortisol, cortisone, renin, and aldosterone. Subsequently, we measured the association of these hormones with changes in systolic, diastolic, mean arterial pressure and pulse pressure as well as immune cell activation via IsoLG formation. RESULTS: We found that myeloid APCs predominantly express the GR and SGK1 with no expression of the MR. Expression of the GR in APCs increased after salt loading and decreased with salt depletion in salt-sensitive but not salt-resistant people and was associated with increased expression of SGK1. Moreover, we found that plasma and urine cortisol/cortisone but not aldosterone/renin correlated with SSBP and APCs activation via IsoLGs. We also found that cortisol negatively correlates with EETs. CONCLUSION: Our findings suggest that renal cortisol signaling via the GR but not the MR in APCs contributes to SSBP via cortisol. Urine and plasma cortisol may provide an important currently unavailable feasible diagnostic tool for SSBP. Moreover, cortisol-GR-SGK1-ENaC signaling pathway may provide treatment options for SSBP.

High Salt Challenge Augments Expression of Proinflammatory Cytokines and Contributes to Elevated Neuronal Activity in the Hypothalamic Paraventricular Nucleus

Abstract:Previous studies have demonstrated that brain proinflammatory cytokines (PIC) play an important role in neurogenic hypertension. However, little is known about the role of brain inflammation in salt sensitive hypertension (SSHTN). In this study, we tested the hypothesis that the PIC expression is increased in the hypothalamic paraventricular nucleus (PVN) and contributes to increased neuronal activity in SSHTN. To test this hypothesis, 8‐week‐old Dahl salt sensitive (DS) rats were divided into two groups (n=3~ 5/group) and fed with either a high salt (HS, 4% NaCI) or normal salt (NS, 0.4% NaCl) diet. Four weeks following diet treatment, rats were euthanized, the PVN was punched out, and real time PCR were performed to assay genes of interest. HS treatment resulted in respective 8 fold, 4 fold, and 7 fold increases in TNFα, IL1β and IL6 mRNA levels in the PVN. In addition, Fra1, a chronic neuronal activation marker, was also increased by 4 fold. Additionally, in order to examine the action of acute hypertonic NaCI on the expression of PIC and neuronal activation marker, adult male SD rats were divided into two groups (n=3~4 per group) and received intracerbroventricular infusion of either hypertonic (2M, 2μl), or isotonic (0.15 M, 2μl) NaCI under anesthetized condition. Three hours following the infusion, rats were euthanized and PVN mRNA levels of PIC and Fra1 assayed. Results indicate that hypertonic saline infusion induced significant increases in mRNA levels of PIC including TNFα (6‐fold), IL1β (13‐fold) and IL6 (21‐fold), as well as neuronal activation marker Fra1 (8‐fold). Finally, we tested whether or not the increase in PIC expression occurred in neurons. Incubation of hypertonic saline (10 mM NaCI plus full medium) for 3 hours resulted in a 12‐fold increase in IL6 and 2.8 fold increase in Fra1 mRNA levels in neuronal cultures from the hypothalamus containing the PVN of neonatal SD rats. Abstract:The SS (Dahl salt sensitive) rat is an established model of hypertension and renal damage that is accompanied with immune system activation in response to a high-salt diet. Investigations into the effects of sodium-independent and dependent components of the diet were shown to affect the disease phenotype with SS/MCW (JrHsdMcwi) rats maintained on a purified diet (AIN-76A) presenting with a more severe phenotype relative to grain-fed SS/CRL (JrHsdMcwiCrl) rats. Since contributions of the immune system, environment, and diet are documented to alter this phenotype, this present study examined the epigenetic profile of T cells isolated from the periphery and the kidney from these colonies. T cells isolated from kidneys of the 2 colonies revealed that transcriptomic and functional differences may contribute to the susceptibility of hypertension and renal damage. In response to high-salt challenge, the methylome of T cells isolated from the kidney of SS/MCW exhibit a significant increase in differentially methylated regions with a preference for hypermethylation compared with the SS/CRL kidney T cells. Circulating T cells exhibited similar methylation profiles between colonies. Utilizing transcriptomic data from T cells isolated from the same animals upon which the DNA methylation analysis was performed, a predominant negative correlation was observed between gene expression and DNA methylation in all groups. Lastly, inhibition of DNA methyltransferases blunted salt-induced hypertension and renal damage in the SS/MCW rats providing a functional role for methylation. This study demonstrated the influence of epigenetic modifications to immune cell function, highlighting the need for further investigations.

Abstract:Introduction: Salt sensitivity of blood pressure is a phenomenon in which blood pressure changes according to dietary sodium intake. Our previous studies found that high salt activates antigen presenting cells, resulting in the development of hypertension. The mechanisms by which salt-induced immune cell activation is regulated in salt sensitivity of blood pressure are unknown. In the current study, we investigated dietary salt-induced effects on the renin-angiotensin-aldosterone system (RAAS) gene expression in myeloid immune cells and their impact on salt sensitive hypertension in humans. Methods: We performed both bulk and single-cell sequencing analysis on immune cells with in vitro and in vivo high dietary salt treatment in humans using a rigorous salt-loading/depletion protocol to phenotype salt-sensitivity of blood pressure. We also measured plasma renin and aldosterone using radioimmunoassay. Results: We found that while in vitro high sodium exposure downregulated the expression of renin, renin binding protein and renin receptor, there were no significant changes in the genes of the renin-angiotensin system in response to dietary salt loading and depletion in vivo . Plasma renin in salt sensitive individuals tended to be lower with a blunted response to the salt loading/depletion challenge as previously reported. Discussion: These findings suggest that unlike systemic RAAS, acute changes in dietary salt intake do not regulate RAAS expression in myeloid immune cells.

Abstract:Introduction: Salt Sensitivity (SS) of blood pressure (BP) is an independent predictor of death due to cardiovascular disease, but its pathogenesis is poorly understood. Sodium (Na + ) is stored in the skin and muscle interstitium. This hyperosmolar Na + activates monocytes in vitro via oxidative stress with generation of isolevuglandin (isoLG) protein adducts that are immunogenic and activate the adaptive immune system. Methods: Five subjects with essential hypertension discontinued all anti-hypertensive therapy for two weeks before the study. SS was assessed by an inpatient protocol of salt loading (460 mmoL/24h) and salt depletion (10 mmoL/24h, plus furosemide 40 mg x 3). Muscle and skin Na + contents were measured at baseline (BA) by 23 Sodium magnetic resonance imaging ( 23 NaMRI). Urine and serum electrolytes, glomerular filtration rate and the % CD14 + monocytes containing isoLG adducts using flow cytometry were obtained at BA, after salt-loading (HI) and after salt-depletion (LO). All continuous data are displayed as median (interquartile range). Spearman’s correlation was used to test associations. Results: Median age was 54 years (44-55), 60% of subjects were female, screening systolic BP (SBP) was 140 mmHg (134-148), diastolic BP was 88 mmHg (84-99) and BMI was 35 kg/m 2 (30-39). SBP response to salt-depletion (salt-sensitivity index, SSI) varied from -13.8 to +1.8 mmHg. %isoLG + CD14 + cells were 48 (27-65) at BA, 55 (31-56) at HI, and 70 (33-72) at LO (p=0.594, by the Kruskal-Wallis test). The correlation between SSI and delta (Δ) %isoLG LO minus HI, was 0.86, [95% confidence interval (CI), -0.07-0.99] which may suggest conclusively as we gather more data that the greater the SSI, the larger the decrease in isoLGs by salt depletion. Muscle Na + content correlated with 24h urine Na + (BA) (r=0.90, 95% CI, 0.11-0.99), however, the correlation with BP, SSI or isoLGs was inconclusive, potentially due to the small sample size. Skin Na + content correlated with baseline %CD14IsoLG + (r=0.91; 95% CI, 0.12-0.99). Conclusions: Na + intake is a component of the determinants of muscle Na + . Skin Na + is associated with increased isoLGs in monocytes, a marker of immune cell activation. Variability in ΔCD14isoLG may serve as a biomarker for SS of BP in humans.

Abstract:Previous analysis of 13 overlapping subcongenic strains led to the identification of a 1.37 Mbp region on chromosome 13 (positions 80.92 to 82.29 Mbp in the Rn5 genome assembly) that influenced the mean arterial blood pressure of the Dahl salt-sensitive (SS) rat on a high-salt diet by more than 20 mmHg. The goal of the present study was to identify biological pathways that could mediate the blood pressure effect of this genomic region. RNA-seq analysis was performed for the renal outer medulla tissue in five selected subcongenic strains, the SS, and a congenic strain from which the subcongenic strains were derived. Rats were fed a 0.4% salt diet or switched to a high-salt diet for 7 days. Affymetrix GeneChip data for SS and three additional congenic or consomic strains were obtained from a previous study. The RNA-seq and microarray data were merged using a cross-platform normalization method to generate a transcriptome dataset containing 90 observations for each gene. A Bayesian model analysis was performed for 243 biological pathways to assess their likelihood to discriminate blood pressure levels across experimental groups. Seven pathways showed posterior probabilities greater than 0.4. These pathways involved neuroactive ligand-receptor interaction, phenylalanine, tyrosine and tryptophan biosynthesis, and protein degradation. A Bayesian approach was used to estimate undirected graphical models among the three known genes located in the 1.37 Mbp region (Astn1, Fam5b, and Rfwd2) and genes in each of the 7 pathways identified above and 11 additional pathways known to be involved in blood pressure regulation. The analysis identified several previously unknown relationships between the three candidate genes and genes in pathways that could regulate blood pressure. The study demonstrated a new, unbiased approach for identifying biological pathways mediating the effect of a candidate genomic region on hypertension.

Abstract:Hypertension, Ahead of Print. Hypertension is the primary modifiable risk factor for cardiovascular, renal, and cerebrovascular diseases and is considered the main contributing factor to morbidity and mortality worldwide. Approximately 50% of hypertensive and 25% of normotensive people exhibit salt sensitivity of blood pressure, which is an independent risk factor for cardiovascular disease. Human and animal studies demonstrate that the immune system plays an important role in the etiology and pathogenesis of salt sensitivity of blood pressure, kidney damage, and vascular diseases. Antigen-presenting and adaptive immune cells are implicated in salt-sensitive hypertension and salt-induced renal and vascular injury. Elevated sodium activates antigen-presenting cells to release proinflammatory cytokines including IL (interleukin) 6, tumor necrosis factor-α, IL-1β, and accumulate isolevuglandin-protein adducts. In turn, these activate T cells release prohypertensive cytokines including IL-17A. Moreover, high-salt intake is associated with gut dysbiosis, leading to inflammation, oxidative stress, and blood pressure elevation but the mechanistic contribution to salt-sensitivity of blood pressure is not clearly understood. Here, we discuss recent advances in research investigating the cause, potential biomarkers, and therapeutic targets for salt-sensitive hypertension as they pertain to the gut microbiome, immunity, and inflammation.

Abstract:The goal of the present study was to narrow a region of chromosome 13 to only several genes and then apply unbiased statistical approaches to identify molecular networks and biological pathways relevant to blood-pressure salt sensitivity in Dahl salt-sensitive (SS) rats. The analysis of 13 overlapping subcongenic strains identified a 1.37 Mbp region on chromosome 13 that influenced the mean arterial blood pressure by at least 25 mmHg in SS rats fed a high-salt diet. DNA sequencing and analysis filled genomic gaps and provided identification of five genes in this region, Rfwd2, Fam5b, Astn1, Pappa2, and Tnr. A cross-platform normalization of transcriptome data sets obtained from our previously published Affymetrix GeneChip dataset and newly acquired RNA-seq data from renal outer medullary tissue provided 90 observations for each gene. Two Bayesian methods were used to analyze the data: 1) a linear model analysis to assess 243 biological pathways for their likelihood to discriminate blood pressure levels across experimental groups and 2) a Bayesian graphical modeling of pathways to discover genes with potential relationships to the candidate genes in this region. As none of these five genes are known to be involved in hypertension, this unbiased approach has provided useful clues to be experimentally explored. Of these five genes, Rfwd2, the gene most strongly expressed in the renal outer medulla, was notably associated with pathways that can affect blood pressure via renal transcellular Na+and K+electrochemical gradients and tubular Na+transport, mitochondrial TCA cycle and cell energetics, and circadian rhythms.

Abstract:High salt consumption is a well established cause of hypertension [1] , yet blood pressure responses to changes in sodium and extracellular fluid balance vary widely in humans [2] . Salt sensitivity of blood pressure (SSBP) is defined as the heterogeneity in blood pressure responses to dietary salt intake. Approximately 25% of normotensive and 50% of hypertensive people have SSBP [3] , which is an independent risk factor for cardiovascular mortality [4] . Previous studies mainly focused on the vasculature, kidneys, and sympathetic nervous system when investigating the mechanisms of SSBP, whereas more recent data suggest that cells of the immune system can sense sodium, although the exact mechanisms remain unclear.

Abstract:The goal of the present study was to narrow a 1.37 Mbp region containing five genes on chromosome 13 (positions 80.92-82.29 Mbp in the Rn5 genome assembly) that we have previously found to influence the mean arterial pressure (MAP) by at least 25 mmHg in SS rats fed a high salt diet (8.0% NaCl for 14 days). The creation of 6 overlapping subcongenic strains that cover this region identified a 0.71 Mbp region (positions 81.01-81.72 Mbp) in which substitution of SS alleles with BN alleles reduced salt-induced hypertension in congenic SS rats by nearly 30 mmHg and significantly reduced urinary albumin excretion (UalbV) (see figure below; n=10-15/strain; * significant difference from SS; p<0.05). Analysis of the narrow candidate region revealed the presence of two protein-coding genes ( Pappa2 and Astn1 ) and a microRNA ( miR488 ), none of which are known to be mechanistically involved in hypertension. Pappa2 mRNA in these rat strains fed 0.4% NaCl diet was expressed at 6-10 fold higher levels in the renal cortex of the salt-resistant congenic strains with the BN allele (26-N, -O, and [[Unable to Display Character: –]]P) compared to strains with the SS allele (26-Q, -R, and [[Unable to Display Character: –]]S). A Pappa2 coding sequence variant of unknown functional importance was identified. Immunohistochemistry and fluorescence in situ hybridization studies localized Pappa2 to intercalated cells of the cortical and medullary collecting duct. Astn1 and miR-488 were not differentially expressed in renal tissue. Together, these findings point towards Pappa2 as a candidate gene for salt-induced hypertension in SS rats.

Abstract:Salt sensitivity of blood pressure is an independent cardiovascular risk factor as powerful as hypertension, yet it goes untreated due to the lack of a feasible diagnostic tool. We previously found that sodium enters antigen-presenting cells (APCs) via ENaC, leading to the formation of isolevuglandins (IsoLGs). IsoLGs are highly reactive oxidative products of fatty acids and act as neoantigens to trigger inflammation in hypertension. We found that activation of APCs via IsoLG is highly variable in humans. Thus, we hypothesized that this variability in IsoLG-mediated APC activation contributes to the variability observed in human salt sensitivity of blood pressure and that IsoLGs can be used as a diagnostic tool. We measured systolic blood pressure (SBP) and IsoLG-containing APCs (dendritic cells [DC], classical, intermediate and non-classical monocytes) by flow cytometry in 14 hypertensive subjects who were off therapy for 2 weeks, before (B) and after in-patient 24 hr salt loading (HI, 460 mmol Na + ) and salt depletion (LO, 10 mmol Na + and furosemide 40 mg x 3). Salt sensitivity was assessed by ΔSBP (LO minus HI). Median age was 49, with 5 females, median BMI (IQR) of 29.8 (25.2, 39.5) kg/m 2 and screening SBP of 135.1 (130, 144.9) mmHg. The median urinary Na + excretions (IQR) were 169.6 (125, 203.3), 360.9 (324.1, 421.2) and 54.8 (36.8, 76) mmol/24h at baseline, HI and LO days, respectively. Baseline percentages of classical and non-classical monocytes with IsoLGs were positively correlated with ΔSBP (r=0.64, p=0.01 and r=0.71, p<0.01 respectively). Changes in IsoLG-containing DCs and nonclassical monocytes positively correlated with ΔSBP (r=0.60, p=0.03 and r=0.69, p=0.01 respectively). Both the baseline levels and changes in IsoLG containing APCs associated with salt sensitivity. Greater decrease in IsoLG-containing APCs predicted greater decline in blood pressure in response to salt depletion, suggesting that oxidative stress in APCs is implicated in the pathogenesis of salt sensitivity. Our results further suggest that baseline percentage of IsoLG-containing APCs may be the first predictor of salt sensitivity that does not require a protocol of salt loading and depletion. This would enable the diagnosis of salt sensitivity in clinic practice.

Abstract:A 1.37Mbp region of the Brown Norway rat Chr13 confers protection from salt‐sensitivity to Dahl salt‐sensitive (SS) rats. Of five candidate genes identified in this region, only Rfwd2 was highly expressed in the renal medulla. A pathway‐based Bayesian graphical model analysis utilizing RNA‐seq and GeneChip data was used to identify relationships between Rfwd2 and genes in pathways related to blood pressure. GeneChip data was used to assess whether the genes with a high marginal posterior probability of interdependence with Rfwd2 were differentially expressed in the renal medulla of SS vs. salt‐resistant congenic SS.13BN26 rats fed 0.4% (LS) or 4% NaCl (HS) diet for 7‐days. Of the 9 genes with the highest absolute partial correlations with Rfwd2, 4 were quantifiable by GeneChip analysis and significantly different (P<0.05) between SS and SS.13BN26 rats: F8 (Factor VIII) (lower in SS on LS/HS); Csnk1d (Casein kinase 1 delta) (higher in SS rats on LS/HS); Pik3ca (Phosphatidylinositol‐4,5‐bisphosphate 3‐kinase) (lower in SS rats on LS, suppressed in both on HS diet); Acly (ATP citrate lyase) (increased with HS in both). These analyses have allowed the unbiased identification of genes associated with our candidate gene providing new insights into salt‐sensitivity.

Abstract:Epigenetic modifications of the genome play a key role in the regulation of gene expression. It has been reported that epigenetic modifications of several genes are associated with hypertension. To investigate the potential role of genome-wide changes in DNA methylation in salt-induced hypertension, experiments were performed on inbred Dahl SS rats obtained from two colonies maintained at the Medical College of Wisconsin (i.e. MCWSS) and Charles River Laboratory (CRLSS). The colonies are genetically identical, but CRLSS rats were maintained on a whole grain diet containing 1% NaCl (CRLSS_LS) while MCWSS rats were fed casein-based AIN-76A chow containing 0.4% NaCl (MCWSS_LS) until both colonies were switched to an AIN-76A chow containing 4% NaCl for 14 days starting at 6 weeks of age (CRLSS_HS and MCWSS_HS, respectively). Mean arterial pressure and albumin excretion rate in MCWSS_HS rats were significantly greater (142±14 mmHg and 100±16 mg/day, n=6) than in CRLSS_HS rats (118±2 mmHg and 20±2 mg/day, n=7). Reduced representation bisulfite genome sequencing (RRBS) measured 5-Methylcytosine levels at single-base resolution in the renal outer medulla in the above groups, each with four biological replicates. For genomic regions located within CpG islands (CGI’s) and exhibiting differential methylation between LS and HS in each colony, HS diet increased median methylation levels several-fold in both MCWSS (7.45% vs. 0.35% for MCWSS_HS vs. MCWSS_LS, respectively, p = 2.84E-31) and CRLSS rats (7.62% vs. 1.21% for CRLSS_HS vs. CRLSS_LS, respectively, p = 1.65E-32). For genomic regions exhibiting differential methylation between MCWSS and CRLSS, MCWSS_HS rats (which exhibited higher blood pressure) had higher median methylation levels than CRLSS_HS rats (7.56% vs. 2.75%, p = 2.12E-9). We observed 156 hypermethylated and 241 hypomethylated regions within CGI’s of MCWSS_LS compared to CRLSS_LS. Examples of differentially methylated genes include the serine protease Prss2 , the transcription factor E2f1 , and the matrix protein Spock2 . These results suggest that sodium-dependent and independent dietary components could induce changes in DNA methylation that may predispose and participate in the development of hypertension and renal damage.
