Abstract:BACKGROUND: Dexmedetomidine, an alpha-2 agonist, has been used for attenuation of he- modynamic response to laryngoscopy but not through the nebulized route. We evaluated the effects of preoperative dexmedetomidine nebulization on the hemodynamic response to laryngoscopy and intubation. To evaluate the AIM: effects of nebulised dexmedetomidine as a premedication in Blunting the haemodynamic response to laryngoscopy and tracheal intubation. After MATERIALS AND METHODS: written informed consent and ethics committee approval, 50 patientsBelonging to ASA status I or II were randomly allocated to two groups.Patients not Giving consent,predicted airway difculty and uncontrolled hypertension were Excluded. All the patients were monitored by ASA standard monitors.They were Randomized into two groups- C and D Group C received 5 ml of normal saline and Group D received 5 ml of dexmedetomidine at a dose of 1 μg/kg as nebulisation 10 mins prior to induction.Then the patients were premedicated and induced with Propofol and intubated after giving a non depolarizing muscle relaxant. Vital monitoring like HR,SBP,DBP,MAP,SPO2,was noted at baseline, post nebulization ,post intubation at 1,5,10 minutes. Demographic details and surgery cha RESULTS: racteristics were comparable between the Groups.Heart rate between the two groups were comparable.The MAP,SBP and DBP values after intubation were lower in group D, which was statistically Signicant(P<0.05).The mean dose of propofol for induction was also lower in Group D. To conclude,nebulised CONCLUSION: dexmedetomidine effectively blunts the pressor response To laryngoscopy and intubation with no serious adverse effects

Abstract:Introduction: Pressor response to laryngoscopy and endotracheal intubation has been associated with sympathetic and parasympathetic responses which can be attenuated by alpha 2 receptor agonists. Aims: The present study aimed to compare and evaluate the effectiveness of dexmedetomidine in attenuating hemodynamic response to endotracheal intubation and also on the induction dose of propofol. Materials and Methods: It was a prospective, randomized, double-blinded, parallel group, placebo-controlled study in tertiary care hospital. A total of 50 patients aged 18–60 years, American Society of Anaesthesiology 1 and 2, scheduled for elective surgeries under general anesthesia (GA). Each patient in Group 1 received a loading dose of dexmedetomidine at 0.001 mg/kg over 10 min before induction of anesthesia and Group 2 received 20 ml normal saline over 10 min. Hemodynamic parameters prior to laryngoscopy and up to 10 min after intubation were measured. The requirement of induction dose of propofol was also calculated. Results: Dexmedetomidine showed significant efficacy in suppressing pressor response during endotracheal intubation. Furthermore, lesser dose of propofol was required for induction of GA. Conclusion: Dexmedetomidine can be used as an adjuvant in GA to prevent pressor response during intubation and decrease the requirement of intravenous induction agent propofol.

Abstract:Background: Our study was conceptualised to evaluate and compare the effects of two different doses of dexmedetomidine infusion after single loading dose not only for attenuation of hemodynamic response to laryngoscopy and intubation but also to evaluate the efficacy to reduce post-operative analgesic requirement and its effect on sedation score post-operatively. Methods: This prospective randomized study was conducted in the postgraduate Department of Anaesthesiology and Intensive care, Government Medical College, Jammu after getting approval from Institutional ethical committee. Around 120 patients of either sex ranging in age from 18 to 60 years, belonging to ASA physical status grade 1 and 2, scheduled for elective laparoscopic cholecystectomy under general anaesthesia with endotracheal intubation were enrolled for the study. Results: Dexmedetomidine infusion of 0.5 m g/kg /h after loading dose of 1 mg/kg resulted in stable, steady and smooth reduction in heart rate, and arterial blood pressure with no episode of severe hypotension or other adverse event. The total analgesic requirement in 24 hours in group D1 was 123.83±6.08 mg. The total analgesic requirement in 24 hours in group D2 was 83.72±4.28 mg. The difference between the two groups was statistically significant. Conclusion: Dexmedetomidine infusion of 0.5 mg/kg /h after loading dose of 1 mg/kg successfully maintained hemodynamic stability both intraoperatively and postoperatively. This dose increased the pain free period postoperatively, thus reducing the total analgesic requirement. Dexmedetomidine can, therefore, be considered an anaesthetic adjuvant in laparoscopic cholecystectomy and infusion dose of 0.5mg/kg/h is better than 0.2 mg/kg/ h after loading dose of 1 mg/kg. At T3, there still was a trend toward greater improvements in the small-vessel density, proportion of perfused small-vessels, perfused small-vessel density, and De Backer score from baseline in the dexmedetomidine group than in the propofol group.This trial demonstrated that dexmedetomidine sedation may be better able to improve microcirculation in cardiac surgery patients during the early postoperative period compared with propofol.

Abstract:Objective To investigate the effect of dexmedetomidine on minimal induced dose of propofol and cardiovascular responses to tracheal intubation.Methods A double-blind randomized controlled trial was conducted.Sixty patients who underwent elective laparoscopic surgery were randomly divided into the dexmedetomidine group (treatment group) and control group.Patients in treatment group were given dexmedetomidine at dose of 0.1 μg/kg · min for 10 min before anesthesia induction,and then infused with 0.4 μg/kg · h until the end.Sodium chloride injection was infused at the same rate in control group.After 10 minutes from the start of dexmedetomidine or sodium chloride injection,propofol was infused by 0.4 mg/kg · min.The dosage of propofol used was recorded when eyelash reflex disappearing,and BIS value in 40 to 60.Propofol was infusion at the same rate,and laryngoscope was incubated at 2 minutes after fentanyl and rocuronium were given.The alertness/sedation (OAA/S scores),mean arterial pressure (MAP),heart rate,pluse oxygen saturation (SpO2) and BIS values were recorded at baseline (before dexmedetomidine or sodium chloride injection infusion),5 and 10 minutes after dexmedetomidine or sodium chloride injection infusion,at the time of eyelash reflex disappearing,before endotracheal intubation,placing the laryngoscope,1,3 and 5 min after intubation.Results OAA/S scores and BIS values in the treatment group were significantly lower than that of control group at 5 minutes and 10 minutes after the dexmedetomidine or sodium chloride injection infusion(P ＜0.05).MAP in treatment group was higher than that of control group at the time of eyelash reflex disappearing,before endotracheal intubation,placing the laryngoscope,1,3 and 5 minutes after intubation (P ＜0.05).Heart rate in treatment group was lower than that of control group at 5 and 10 minutes after dexmedetomidine or sodium chloride injection infusion and before the endotracheal intubation(P ＜ 0.05).There was no significant difference in terms of SpO2 between two groups (P ＞ 0.05).When patients consciousness disappeared and BIS values were in 40-60,the minimum induced dose of propofol was (88.00 ± 25.91) mg in treatment group and (117.33 ± 25.45) mg in control group.The dosage of propofol treatment group was obviously less than control group (t =4.423,P ＜ 0.05).Conclusion Dexmedetomidine reduces the minimum induced dose of propofol while maintaining more stable hemodynamic changes during anesthesia induction.However,there has no obvious inhibition effect on cardiovascular response to tracheal intubation.
Evaluation of effects of intravenous infusion of dexmedetomidine or lignocaine on stress response and postoperative pain in patients undergoing craniotomy for intracranial tumors: A randomized controlled exploratory study

Abstract:Background: Goals of anesthesia in neurosurgery include stable cerebral hemodynamics and provide relaxed brain to surgeon. Dexmedetomidine and lignocaine as an adjuvant can fulfill these criteria but literature comparing the two are sparse. We compared the effects of intravenous infusion of dexmedetomidine or lignocaine on stress response, postoperative pain, and recovery in patients undergoing craniotomy for intracranial tumors. Methods: Approval was obtained from IEC, and the study was prospectively registered (CTRI/2022/11/047434). Written and informed consent was obtained from 105 patients fulfilling inclusion criteria, and they were divided into three groups. Group D received intravenous infusion of dexmedetomidine 1 mcg/kg over 15 minutes followed by infusion at rate of 0.5 mcg/kg/h, Group L received intravenous infusion of lignocaine 2 mg/kg over 15 minutes followed by infusion at rate of 1.5 mg/kg/h, and Group N received intravenous infusion of normal saline at the rate of 4-8 ml/h till skin suturing. SPSS v23 (IBM Corp.) was used for data analysis. Results: There was a significant difference between groups in terms of intraoperative hemodynamic variations, brain relaxation score, extubation criteria, postoperative pain, stress indicator response, and quality of recovery. Conclusions: Dexmedetomidine as an adjuvant to anesthetic drugs has a better profile than lignocaine in suppressing stress response and preventing hemodynamic variations at intubation, skull pin application, and surgical incision. Dexmedetomidine increases the duration of effective analgesia more than lignocaine, in postoperative period in patients undergoing craniotomy.
Effects of Dexmedetomidine on Cardiovascular Responses During Anesthetic Induction in Hypertensive Patients

Abstract:Objective To evaluate the effects of dexmedetomidine on cardiovascular responses during anesthetic induction and endotracheal intubation in hypertensive patients. Methods This was a prospective,double-blind,and placebo controlled study. Patients with preoperative diagnosed and well controlled hypertension were enrolled and randomly divided into dexmedetomidine group (group D) and control group (group C).Dexmedetomidine (an initial dose of 1.0 μg/kg within 10 min,followed by a continuous infusion of 0.7 μg·kg-1 ·h-1 for 15 min) or normal saline was administered 25 minutes before anesthetic induction.Anesthesia was induced with sufentanil,propofol,and rocuronium.Bispectral index was maintained between 40 to 60.Hemodynamic parameters including systolic (SBP) and diastolic blood pressures (DBP),and heart rate (HR) were recorded and the changes were calculated during both anesthetic induction and intubation. Results After administration of dexmedetomidine,SBP and HR were significantly lower in the treatment group than that in the control group [ (139.1 ±23.9) mm Hg vs.(165.2±1 8.3)mm Hg,P＜0.001 and (63.8±11.3) bpm vs. (77.2±13.0) bpm,P=0.001,respectively].Before insertion of the laryngoscope,the percentage decreased in SBP and DBP were significantly less in the treatment group than that in the control group[(-14.6±9.6)％vs.(-34.5±10.7)％,P＜0.001 and (-7.2±9.6)％ vs.(-28.7±12.0)％,P＜0.001,respectively].One minute after endotracheal intubation,SBP,DBP,and HR were significantly lower in the treatment group than that in the control group [SBP:(134.0±25.8) mm Hg vs.(171.6 ± 30.0) mm Hg,P＜0.001; DBP:(78.1±17.7) mm Hg vs.(93.3±17.5) mm Hg,P=0.009; and HR:(71.1±11.0) bpm vs.(91.1± 14.0) bpm,P＜0.001,respectively]. Conclusions Dexmedetomidine combined with propofol and sufentanil suppressed the decrease in blood pressure due to anesthetic induction and blunted the cardiovascular responses to endotracheal intubation in hypertensive patients.

Abstract:Objectives: To investigate whether a single dose of dexmedetomidine (DEX) improves surgical field visibility. Study Design: Randomized, prospective, double-blind study. Setting: Tertiary teaching hospital. Interventions: ASA I or II patients undergoing tympanoplasty with ossiculoplasty were randomly assigned to receive either 0.8 mu g/kg DEX (Group D) or the same volume of saline (Group N) 10 min before anesthesia induction. Main Outcomes: The primary outcome was the visibility of the surgical field rated by surgeons by Boezaart score. The secondary outcomes were consumption of anesthesia, hemodynamic profiles, and subsequent recovery. Results: Boezaart scores for surgical visibility were lower in Group D than in Group N (1.3 +/- 0.8 versus 1.8 +/- 0.9, P = 0.014). Minimum alveolar concentrations of sevoflurane (Group D 1 [0.9/1.1] versus Group N 1 [1/1.2], P = 0.018) and remifentanil consumption (Group D 370 [218/504] mu g versus Group N 583 [300/1028] mu g, P = 0.002) were less in Group D. Except for a transient increase in blood pressure and a decrease in heart rate during DEX infusion, hemodynamic profiles were more stable in Group D than in Group N. More patients needed morphine rescue and presented with postoperative nausea and vomiting in Group N than in Group D (Group D 1 versus Group N 8, P = 0.029). Recovery time was comparable between the two groups (Group D 19 min versus Group N 18 min, P = 0.569). Conclusion: Use of a single dose of DEX resulted in improved surgical visibility, less consumption of anesthesia, and more favorable hemodynamic profile while not delaying recovery time.

Abstract:BACKGROUND: Dexmedetomidine is an alpha-2 adrenergic agonist with sedative, anxiolytic, and analgesic properties. This study was designed to evaluate the inhibitory effects of preoperative administration of 0.5 µg/kg dexmedetomidine on hemodynamic responses caused by endotracheal intubation in elderly patients undergoing treatment for hypertension.METHODS: Forty elderly (≥ 65 years old) patients who had been receiving hypertension treatment, had American Society of Anesthesiologists physical status II, and were scheduled to undergo elective noncardiac surgery were randomly selected and assigned to 2 groups. Group C received normal saline and group D received 0.5 µg/kg dexmedetomidine intravenously over 10 min just before endotracheal intubation. Systolic blood pressure (SBP), diastolic blood pressure (DBP), mean arterial pressure (MAP), and heart rate (HR) were recorded preoperatively in the ward, immediately after study drug administration, and at 1, 3, and 5 min after endotracheal intubation.RESULTS: Compared to group C, group D showed significantly lower SBP and MAP at 1, 3, and 5 min as well as significantly lower DBP and HR at 3 and 5 min after endotracheal intubation.CONCLUSIONS: In elderly patients receiving hypertension treatment, a single preanesthetic dose of dexmedetomidine (0.5 µg/kg) effectively suppressed the hemodynamic responses to endotracheal intubation.
Effects of Single-dose Intravenous Dexmedetomidine on Combined Spinal Epidural Anesthesia: a Prospective Randomized Controlled Trial

Abstract:Objective To investigate the effect of single-dose intravenous dexmedetomidine on hemodynamics,sedation and post anesthesia shivering in knee arthroscopic operation.Methods A total of 200 patients undergoing the treatment of meniscus under knee arthroscope were randomly divided into 2 groups.Group D were intravenously infused dexmedetomidine 0.8 μg / kg in 15 minutes,and Group C were infused saline 0.2 ml / kg in 15 minutes.Blood pressure,heart rate,SpO 2,Ramsay score,post anesthesia shivering rate and whether or not comfortable after operation were compared at time points of T0(before administration of medicine),T1(10 minutes after administration of medicine),T2(20 minutes after administration of medicine),T3(30 minutes after administration of medicine),and T4(60 minutes after administration of medicine).Results Ramsay scores were significantly higher in Group D than in Group C since T1(P < 0.05);rate of post anesthesia shivering was significantly lower in Group D than in Group C(10% vs.48%,χ2= 35.066,P = 0.000).More patients felt comfortable in Group D than in Group C(90% vs.40%,χ2= 54.945,P = 0.000).There was no significant difference between group D and C in blood pressure,heart rate and SpO 2(P > 0.05) at T0(before administration of medicine).From T1,patients' heart rate decreased,blood pressure lowered,SpO 2 raised,but no clinical significance was shown.Conclusions The infusion of 0.8 μg / kg dexmedetomidine in 15 minutes could be safely used in knee arthroscopic surgery under combined spinal epidural anesthesia.It can lead to proper sedation,increased comfortable level and decreased post anesthesia shivering rate,with good hemodynamics and oxygenation states.

Abstract:Objective To observe the effects of dexmedetomidine ( DEX ) administrated in different modes on the stress reaction in patients undergoing endotracheal intubation .Methods In this prospective , randomized , double-blind study , 90 ASAⅠand Ⅱ patients undergoing selective general anesthesia were included .The patients were randomly allocated to three groups ( n =30 ): DEX intranasal group ( group N ) , DEX intravenous group ( group V ) and control group ( group C ) .DEX were given 1 μg/kg in 1 ml intranasally and at the same time 20 ml of normal saline ( NS) was infused intravenously within 10 min just before induction of anesthesia in group N; in group V, 1 ml NS was given intranasally and dexmedetomidine 1 μg/kg in 20 ml was infused intravenously in the same way;and NS were given intranasally and intravenously respectively in the same way in group C .General anesthesia induction and endotracheal intubation were taken when the infusion were finished .MAP, HR and BIS were recorded at OR(T0), 5 min and 10 min after intranasal administration (T1,T2), 1 min (T3), 3 min (T4) and 5 min (T5) after endotracheal intubation (P<0.05);blood samples were taken at T0 , T3 , T4 , T5 to detect plasma cortisol concentration and blood glucose;the occurrence of hypoxemia (SpO2 <90%), hypertension or hypotension were also recorded .Results MAP, HR, plasma cortisol concentration and blood glucose in group C were significantly higher after endotracheal intubation in group C ( P<0.05);HR was significantly lower at T 2 and MAP, HR, plasma cortisol concentration and blood glucose fluctuated slightly and were significantly lower after endotracheal intubation in both group N and group V (P <0.05).There was no discomfort and hypoxemia during intranasal and intravenous administration, and the hemodynamics were stable in all groups . Conclusion Both modes of dexmedetomidine administration can inhibit the stress reaction effectively in patients undergoing endotracheal intubation .Furthermore, the administration of intranasal is simple , safe and well tolerant .

Abstract:Background: Preoperative anxiety leads to adverse clinical outcomes and long-term maladaptive behavioural changes. The role of intranasal atomised dexmedetomidine and atomised ketamine as premedication to produce sedation and anxiolysis in paediatric neurosurgical patients has not been extensively studied. Objective: To study the efficacy of intranasal atomised dexmedetomidine and intranasal atomised ketamine as premedication in producing sedation and facilitating smooth induction in children undergoing spinal dysraphism surgery. Design: A prospective randomised double-blind trial. Setting: A tertiary teaching hospital. Patients: Sixty-four children aged 1 to 10 years undergoing spinal dysraphism surgery. Methods: Children were randomised to receive intranasal atomised dexmedetomidine 2.5 μg kg -1 (Group D, n = 32) and intranasal atomised ketamine 5 mg kg -1 (Group K, n = 32) 30 min before surgery. Outcomes measured: The primary outcome was to compare the level of sedation in both groups using the University of Michigan Sedation Score (UMSS). The secondary outcomes included an assessment of the ease of parental separation, intravenous cannulation and satisfactory mask acceptance along with perioperative vitals (heart rate, blood pressure and oxygen saturation). The incidence of emergence agitation and time to discharge were also noted. Results: The degree of sedation was significantly better in Group D as compared to Group K at 20 min (UMSS, 1.55 ± 0.51 versus 1.13 ± 0.34, difference, -0.406; 95% CI, -0.621 to -0.191; P = 0.0001) and 30 min (2.32 ± 0.6 versus 1.94 ± 0.50, difference, -0.374; 95% CI, -0.650 to -0.100; P = 0.007). The ease of parental separation, venous cannulation and mask acceptance ( P = 0.83, 0.418 and 0.100 respectively) were comparable in both groups. The heart rate was lower in group D at 10, 20 and 30 min post-drug administration but was clinically insignificant. The incidence of emergence agitation and time to discharge was also similar with no adverse events reported. Conclusion: Intranasal atomised dexmedetomidine produces greater sedation as compared to intranasal atomised ketamine with comparable ease of parental separation, venous cannulation and mask acceptance with no adverse effects.
Addition of Dexmedetomidine to Propofol Anesthesia for Middle-Ear Surgeries: A Prospective Randomized Double-Blind Study

Abstract:Background Middle-ear surgery commonly performed under a microscope requires a bloodless field provided by hypotensive anesthesia. Our objective was to study the effects of dexmedetomidine on propofol consumption and intraoperative hemodynamic stability. Methods One hundred adults undergoing elective middle-ear surgery were randomized into two groups. The propofol+dexmedetomidine group (Group PD) received a loading dose of dexmedetomidine 1μg/kg in 10ml normal saline over 10min followed by infusion of the same at 0.5μg/kg/h. Propofol-only group (Group P) received 10ml normal saline over 10min followed by an infusion of the same. General anesthesia was induced with intravenous morphine, propofol, and vecuronium, and maintained with propofol, oxygen, and N2O. During microscope use, we aimed to maintain mean arterial pressure (MAP) within 60-69mmHg. Results There was no significant difference in the mean (SD) consumption of propofol [Group P 8.6 (2.1)mg/kg/h vs Group PD 8.1 (1.5)mg/kg/h, P=0.172]. The induction dose of propofol was significantly less in Group PD [1.8 (0.3) vs 2 (0.4)mg/kg, P<0.001]. Except for the baseline value, the heart rate was significantly lower in Group PD, P<0.001. The time duration during which MAP was within 60-69mmHg was higher in Group P [37.5 (36.8) vs 30.9 (38.3)min] though the difference was not statistically significant. The recovery was delayed in Group PD [25.4 (8.6) vs 17.6 (4.9)min, P<0.001]. Group PD had a significantly better operative field, P=0.0003. Conclusion The addition of dexmedetomidine did not reduce propofol consumption but reduced the induction dose of propofol. Propofol and dexmedetomidine combination provided comparable mean arterial pressure and better operative field but caused delayed recovery.
Lidocaine inhalation attenuates the circulatory response to laryngoscopy and endotracheal intubation

Abstract:STUDY OBJECTIVE: To evaluate the effect of lidocaine inhalation on the circulatory response to direct laryngoscopy and endotracheal intubation.DESIGN: Prospective, randomized study.SETTING: Operating theater at a public hospital.PATIENTS: Eighty patients (ASA physical status I and II ages 25 to 45 years) scheduled for major abdominal surgery.INTERVENTIONS: In the first stage, 40 patients were randomly assigned to receive inhalation of either lidocaine 40 mg or a 0.9% solution of sodium chloride (placebo). In the second stage, the next 20 consecutive patients received inhalation of lidocaine 120 mg, and another 20 consecutive patients received intravenous (IV) lidocaine 1 mg/kg.MEASUREMENTS AND MAIN RESULTS: Mean arterial pressure rose significantly in the i.v. lidocaine group (21.2 mmHg; p < 0.05), the saline inhalation group (29.2 mmHg; p < 0.05), and the lidocaine 40 mg inhalation group (22.9 mmHg; p < 0.05), but not in the lidocaine 120 mg inhalation group (10.1 mmHg). The heart rate (HR) response to intubation with lidocaine inhalation was dose dependent. In the saline inhalation group, HR increased by 15.6 beats per minute (bpm) (p < 0.05); in the lidocaine 40 mg inhalation group, HR increased by 9.1 bpm (p < 0.05); and in the lidocaine 120 mg inhalation group, HR increased by only 3.1 bpm.CONCLUSION: Inhalation of lidocaine 120 mg prior to induction of anesthesia is an effective, safe, and convenient method to attenuate the circulatory response to laryngoscopy and endotracheal intubation.

Abstract:Background : Postoperative nausea and vomiting (PONV) are common side-effects following strabismus surgery. The present study aimed to compare the effects of different doses of dexmedetomidine (DEX) on PONV incidence in pediatric patients undergoing strabismus surgery. Methods : In this prospective randomized double-blinded study, 126 pediatric patients undergoing strabismus surgery were randomized into one of three groups: Placebo group, normal saline; DEX1 group, 0.3 ug/kg dexmedetomidine, and DEX2 group, 0.5 ug/kg dexmedetomidine. Oculocardiac reflex (OCR) events were recorded during surgery. PONV or postoperative vomiting (POV) was recorded for 24 hours in the ward. Pediatric anesthesia emergence delirium (PAED) scale and emergence agitation (EA) scale were recorded in the recovery room. Results : Intraoperative OCR was significantly reduced in DEX2 group (42%) as compared to that of Placebo group (68%) (p=0.0146). During the first 24 hours post-op, the overall incidence of PONV was significantly lower in DEX2 group (10%) than that of Placebo group (32%) (p=0.0142). There was no significant difference in POV among the three groups. PAED or EA scores among the three groups were similar during recovery time. Conclusion : Dexmedetomidine (0.5 ug/kg) reduced OCR and PONV without lengthening extubation time or recovery time in pediatric patients undergoing strabismus surgery.

Abstract:Objective To explore the influence of dexmedetomidine for nasal administration by three different concentrations on the stress response in patients with general anaesthesia during the peri-extubation period of gynecological surgery and to explore the optimized dosage.Methods Eighty patients undergoing general anaesthesia were randomly divided into four groups:control group (group C),and three different doses of intranasal dexmedetomidine,groups D1,D2,D3 receiving intranasal dexmedetomidine 0.6,1.2 and 1.8 μg/kg 30 min before the end of the operation,respectively.Group C was given the same volume of saline.Heart rate (HR),mean arterial pressure (MAP) and ratepressure-product (RPP) were recorded immediately after intranasal administration (T1),the end of the surgery (T2),recovery time (T3),extubation time (T4),1 min after extubation (T5),5 min after extubation (T6) and 10 min after extubation (T7).Blood samples were taken at T1,T2,T4 and T6 to detect the serum concentrations of norepinephrine (NE) and cortisol (Cor).Postoperative recovery outcomes were recorded.Results Compared with group C,MAP,HR and RPP at T3-T5 in group D1 were reduced significantly (P＜0.05).In addition,those were all decreased in groups D2 and D3 at T2-T7 (P＜0.05).Compared with group C,the serum concentrations of NE and Cor at T4 and T6 in group D1,at T2,T4 and T6 in groups D2,D3 were decreased obviously (P＜0.05).Awaken time,extubation time and PACU stay time were shortened significantly in groups C,D1 and D2 compared with group D3 (P＜0.05).Conclusion Intranasal administration of dexmedetomidine 30 min before the end of the operation could attenuate the extubation reaction and the adverse reaction of cardiovascular system during the peri-extubation period.

Abstract:BACKGROUND: Dexmedetomidine has been shown to blunt the stress response to surgery. Hence a study was designed to evaluate the effect of intravenous (IV) Dexmedetomidine infusion during general anesthesia for abdominal surgeries on blood glucose levels and on Sevoflurane requirements during anesthesia.MATERIALS AND METHODS: Forty patients scheduled for abdominal surgery under general anesthesia were divided into Dexmedetomidine (D) group and Placebo (P) group of 20 each. Group D received a loading dose of Inj. Dexmedetomidine at 1 μg/kg/10 min diluted to 20 mL, followed by maintenance with 0.5 μg/kg/h., till the end of surgery. Group P received similar volume of IV normal saline. Anesthesia was maintained with nitrous oxide in oxygen and Sevoflurane keeping entropy between 40 and 60. Data were analyzed using students t test, chi square test and Fisher Exact test as applicable.RESULTS: During the first postoperative hour, Dexmedetomidine group showed blood glucose levels of 118.2 ± 16.24 mg/dL, compared to placebo group which was 136.95 ± 19.76 mg/dL and it was statistically significant (P < 0.01). Mean hourly Sevoflurane requirement in Group D was 11.10 ± 2.17 mL, compared to 15.45 ± 3.97 mL in placebo group. In peri-operative period, the heart rate and MAP were significantly lower in Group D, when compared to placebo. Patients in Group D were better sedated and post-operative pain score was better in Group D compared to Group P.CONCLUSION: IV Dexmedetomidine was effective in blunting stress response to surgical trauma as indicated by lower blood glucose levels, and reduces Sevoflurane requirements during entropy guided general anesthesia without affecting time for extubation.

Abstract:Background: Postoperative delirium (POD) is a serious complication in elderly patients undergoing cardiac surgery. This study was aimed at investigating the effect of perioperative administration of dexmedetomidine for general anesthesia maintenance on occurrence and duration of POD in elderly patients after cardiac surgery. Methods: This randomized control trial was done in central India after the institution's ethical committee gave their approval. One hundred and sixty-four patients were enrolled after cardiac surgery. Each patient gave their written consent after being fully informed. Patients over 60 years’ old who were going to have heart surgery were included in the study. Results: The difference in POD incidence between the dexmedetomidine and propofol groups was not statistically significant (p=0.0758). When compared to patients receiving propofol, patients receiving dexmedetomidine had a median delirium onset time that was delayed (second day vs. first day) and a shorter median delirium duration (2 days versus 3 days). Additionally, the patients receiving dexmedetomidine showed a lower VAS score and less use of opiate analgesics. Regarding other postoperative results, no difference was seen. Conclusions: Dexmedetomidine administration during cardiac surgery decreased the incidence, postponed the onset, and shorten the duration of POD in elderly patients.
