Abstract:The function of the large bowel is to absorb water from the remaining indigestible food matter and subsequently pass useless waste material from the body, but there has been only a small amount of data in the literature on its biomechanical characteristics that would facilitate our understanding of its transport function. Our study aims to fill this gap by affording comprehensive inflation/extension data of intestinal segments from distinct areas, spanning a physiologically relevant deformation range (100-130% axial stretches and 0-15 mmHg lumen pressures). These data were characterized by the Fung-type exponential model in the thick-walled setting, showing reasonable agreement, i.e. root-mean-square error ~30%. Based on optimized material parameters, i.e. a(1)<a(2) and the anisotropy degree (a(1) + a(4))/(a(2) + a(4))<1, with a(1) and a(2) representing circumferential and axial stiffness, and a(4) the stiffness interaction, the tissue was stiffer axially. The transverse colon was the stiffest of all regions both circumferentially (a(1) = 5.304 ± 0.952 versus proximal colon: 4.043 ± 0.643, distal colon: 1.505 ± 0.222 and rectum: 2.339 ± 0.285) and axially (a(2) = 16.639 ± 0.792 versus proximal colon: 16.580 ± 1.042, distal colon: 13.209 ± 1.185 and rectum: 12.553 ± 0.689). Our biomechanical testing and material characterization results for the large intestine of healthy young animals are expected to aid in comprehending the adaptation/remodeling that occurs with ageing, pathological conditions and surgical procedures, as well as for the development of suitable biomaterials for replacement.

Abstract:The dielectric properties of pancreatic tissues from human healthy and tumour-bearing tissues have been extracted from impedance measurement on ex vivo , freshly excised samples. They are compared to pig pancreas samples, measured following the same protocol. The purpose is to add data to the scarce literature on the properties of the human pancreas and pancreatic tumours, for treatment planning, tissue identification and numerical simulations. The conductivity measured at 500 kHz for human healthy pancreas is 0.26 S/m, while the conductivity of tumour-bearing tissues is 0.44 S/m. Those values differ significantly from that listed in the IT IS database at 0.57 S/m, suggesting an update might be to consider. However, measures of relative permittivity are in accordance with the database with a value of approximately 2.3x10 3 . Ex vivo porcine model, while being less conductive than human pancreas with 0.16 S/m at the same frequency, is deemed a relevant model when studying pancreatic applications of electromagnetic fields-based treatments, such as radiofrequency ablation.

Abstract:The large intestine is an important part of the human digestive system and the target of many diseases. The biomechanical properties of the large intestine are closely related to the proper functioning of its mechanical behavior. Therefore, the study of its biomechanical properties can help to better understand the key factors of lesions and provide a theoretical basis for the research and application of disease treatment, artificial anal sphincter, and other related medical devices. Physiologic structure of the large intestine is the basis for the study of its biomechanical properties. Constitutive models are commonly used to describe the biomechanical properties of soft tissues and to provide a mathematical characterization of the effects of biological components on the behavior of materials. The studies and results of biomechanical experiments provide a basis for determining the material parameters and the causes of functional damage to the material. In this paper, the biomechanical properties of the large intestine are investigated from two aspects: active properties and passive properties, and three perspectives: physiological structure, constitutive models, and biomechanical experiments. The paper concludes that their effective combination is an important comprehensive method to study the biomechanical properties of the large intestine. It provides a new perspective for the study of biomechanical properties of the large intestine, and provides a theoretical basis for future related biomechanical studies and the development of related medical devices.

Automated measurement of intestinal mucosa electrical parameters using a new digital clamp

Abstract:Electrophysiological studies that include measurements of the electrical parameters of the epithelium offer insight into the cell's ability to react to different biological effectors and their functional viability. These parameters are commonly measured using a Ussing permeation chamber; however, most Ussing permeation chambers currently available must follow a strict operational protocol, and the type of electrodes used has to be taken into special consideration. The purpose of this study was to develop a new Ussing permeation chamber device with an automatic digital clamp which uses a microcontroller. Conventional electrodes, such as platinum or Ag/AgCl electrodes, are replaced by stainless steel 316L working electrodes. The electrode-electrolyte interface (inox-Ringer's) study was performed by impedance spectroscopy in the range of 1-10 kHz. The determination of Warburg's model electrical parameters was inferred from the Nyquist diagram. The model validation of the new digital clamp was performed experimentally on isolated segments of mouse jejunum. Two main study results should be mentioned. One is that impedance spectroscopy on stainless steel electrodes has provided Warburg's parameters, allowing the development of a transfer function model. The other is that the new digital clamp can simultaneously measure or calculate conductance, potential difference and short-circuit current. These results have also confirmed the great importance of Warburg's model for determining the electrical parameters of the electrode-electrolyte interface, and have shown that the measurement of intestinal mucosa electrical parameters can be achieved with a digital correction. Finally, the results suggest that stainless steel electrodes can be used successfully in a Ussing permeation chamber as working electrodes.
Dielectric behavior of beef meat in the 1-1500kHz range: Simulation with the Fricke/Cole-Cole model

Abstract:The electrical properties of biological tissues have been researched for many years. Impedance measurements observed with increasing frequencies are mainly attributed to changes in membrane conductivity and ion and charged-molecule mobility (mainly Na(+), K(+), CL(-) ions). Equivalent circuits with passive electrical components are frequently used as a support model for presentation and analyses of the behavior of tissues submitted to electrical fields. Fricke proposed an electrical model where the elements are resistive and capacitive. The model is composed of a resistive element (Rp) representing extracellular fluids (ECF) placed in parallel with a capacitive element (Cs) representing insulating membranes in series and a resistive element (Rs) representing intracellular fluids (ICF). This model is able to describe impedance measurements: at lower frequencies, most of the current flows around the cells without being able to penetrate them, while at higher frequencies the membranes lose their insulating properties and the current flows through both the extracellular and intracellular compartments. Since meat ageing induces structural change, particularly in membrane integrity, the insulating properties of membranes decrease, and intracellular and extracellular electrolytes mix, thus driving changes in their electrical properties. We report a method combining the Fricke and Cole-Cole models that was developed to monitor and explain tissues conductivity changes in preferential directions during beef meat ageing.
A computational model of the human colon for use in medical robotics

Abstract:This paper reports on the results of modeling the human colon and analyzes its effectiveness as a finite element analysis (FEA) tool for testing conventional and robotic colonoscope technologies. FEA has been used to model colon tissue before, but these analyses have been carried out on smaller tissue samples, such as to fit strain energy functions to mechanical testing data or to explore the inflation-extension response of a section of the colon. The full colon has not yet been modeled in this way, and this study aims to show the usefulness of such a tool for testing endoscopic devices for diagnostic and therapeutic purposes in the colon. Data from the literature and databases have been used to approximate the stress response of the tissues, and a critical analysis of the limitations of the approximations has been carried out. Characteristic colonoscope loops were created to analyze the mechanical response of the colon and provide comparable results to conventional colonoscopy. The results showed how stress would be propagated along the length of the colon and how neighboring structures can affect the stresses and strains experienced by the colon wall, demonstrating the need for and capabilities of a full FEA model of the colon to test endoscopic devices.

Abstract:Several morbid conditions may necessitate extensive intestinal resection, leading to short-bowel syndrome. When clinical treatment becomes inefficient, a surgical approach is necessary. Distal colon interposition is one of the viable techniques. The interposition of colon segments between remnants of the small bowel improved lifestyle, increased transit time, and diminished diarrhea. The aim of this study is to observe the longitudinal muscular contractions after distal colon interposition. Sixteen male Wistar rats (EPM-1) were submitted to an 80% small bowel resection associated with a partial colectomy of the distal colon immediately after the bifurcation of the middle colic artery followed by a 3-cm isoperistaltic distal colon interposition. After 70 days, the animals were submitted to euthanasia and segments of the jejunum, ileum, remnant colon, and interposed colon were prepared for pharmacological tests. The isometric contractions were measured by a polygraph. After 30 minutes, the dose/effect curves were obtained for both metacholine and barium chloride stimulation through the extraluminal surface (serosa). After this period, we observed a significant increase in the length, diameter, and thickness of the intestinal wall. Regarding the sensibility (pD(2)), no difference was found (interposed colon = 7.21 +/- 0.2; remnant colon = 7.65 +/- 0.1; remnant jejunum 7.46 +/- 0.1; and remnant ileum 7.57 +/- 0.1), even though the animals were submitted to different procedures. In relation to the maximal effect (E(max)), the longitudinal muscle contraction responses (interposed colon = 11.79 +/- 0.1; remnant jejunum = 15.42 +/- 0.2; and remnant ileum = 11.48 +/- 0.2) were lower than those of the remnant colon (E(max) = 22.42 +/- 0.1). This means that there was a possible adaptation of colonic segments to their new location.
A novel multiple electrode direct current technique for characterisation of tissue resistance during surgery

Abstract:Electrochemical and electrical characteristics have the potential to help differentiate between, and assess the health state of, different biological tissues. However, measurement and interpretation of these characteristics is non-trivial. We propose a new DC galvanostatic sensing method for application to laparoscopic cancer surgery. This presents a simple and cost-effective measurement coupled with straightforward data interpretation. This paper describes the electrochemical and electrical theory underpinning the technique. Additionally, we describe a measurement system employing this technique and present an investigation into the feasibility of using it for measuring the resistance of different tissue types. Measurements were performed on ex vivo porcine liver, colon and rectum tissues. Outputs were consistent with theory and showed a significant difference between the resistance of the different tissue types, (one-way ANOVA, F(2, 28) = 1369, p <; 0.01). These findings indicate that this novel technique may be viable as a low cost method for the discrimination and health assessment of tissues in clinical scenarios.

Abstract:Background: The large intestine provides a compensatory role in energy recovery when surgical interventions such as extensive small intestinal resections or bypass operations lower the efficiency of nutrient absorption in the upper gastrointestinal (GI) tract. While microorganisms in the colon are known to play vital roles in recovering energy, their contributions remain to be qualified and quantified in the small intestine resection. Objective: We develop a mathematical model that links nutrient absorption in the upper and lower GI tract in two steps. Methods: First, we describe the effects of small intestine resection on the ileocecal output (ICO), which enters the colon and provides food for microbes. Second, we describe energy recovered by the colon's microorganisms via short-chain fatty acid (SCFA) production. We obtain model parameters by performing a least-squares regression analysis on clinical data for subjects with normal physiology and those who had undergone small intestine resection. Results: For subjects with their intestines intact, our model provided a metabolizable energy value that aligns well with the traditional Atwater coefficients. With removal of the small intestine, physiological absorption became less efficient, and the metabolizable energy decreased. In parallel, the inefficiencies in physiological absorption by the small intestine are partly compensated by production of short-chain fatty acids (SCFA) from proteins and carbohydrates by microorganisms in the colon. The colon recovered more than half of the gross energy intake when the entire small intestine was removed. Meanwhile, the quality of energy absorbed changed, because microbe-derived SCFAs, not the original components of food, become the dominant form of absorbed energy. Conclusion: The mathematical model developed here provides an important framework for describing the effect of clinical interventions on the colon's microorganisms.

Abstract:The purpose of this research was to evaluate the potential toxic effects of Plak-Lite disclosing solution, which contains 0.75% sodium fluorescein U.S.P. in buffered glycerol in hamsters under defined and exaggerated conditions. In oral dosing experiments, volumes calculated at least 50, 200 or 1000 times the human daily use level were administered. For intraoral application the 1000 X human use level was selected. The control group was dosed orally with the 1000 X dose volume of water. All animals were treated once a day for 31 or 92 days. Daily examination of animals revealed no overt drug or dosing-related effects. Weights recorded at 31 or 92 days were similar for all groups. Histological evaluation performed on tongue, oral mucosa, cheek pouch, stomach (3 sections), liver, kidney, spleen, small intestine (3 sections), large intestine and bladder revealed no pathological changes. This study demonstrates the safety of Plak-Lite disclosing solution when administered at exaggerated dose levels via routes simulating human use.
Development and Characterization of a 2D Porcine Colonic Organoid Model for Studying Intestinal Physiology and Barrier Function

Abstract:The porcine colon epithelium plays a crucial role in nutrient absorption, ion transport, and barrier function. However ethical concerns necessitate the development of alternatives to animal models for its study. The objective of this study was to develop and characterise a two?dimensional (2D) in vitro model of porcine colonic organoids that closely mimics native colon tissue, thereby supporting in vitro research in gastrointestinal physiology, pathology, and pharmacology. Porcine colonic crypts were isolated and cultured in three-dimensional (3D) organoid systems, which were subsequently disaggregated to form 2D monolayers on transwell inserts. The integrity of the monolayers was evaluated through the measurement of transepithelial electrical resistance (TEER) and electron microscopy. The functional prerequisites of the model were evaluated through the measurement of the mRNA expression of key ion channels and transporters, using quantitative RT-PCR. Ussing chamber experiments were performed to verify physiological activity. The 2D monolayer displayed robust TEER values and retained structural characteristics, including microvilli and mucus-secreting goblet cells, comparable to those observed in native colon tissue. Gene expression analysis revealed no significant differences between the 2D organoid model and native tissue with regard to critical transporters. Ussing chamber experiments demonstrated physiological responses that were consistent with those observed in native colonic tissue. In conclusion, 2D porcine colonic organoid model can be recommended as an accurate representation of the physiological and functional attributes of the native colon epithelium. This model offers a valuable tool for investigating intestinal barrier properties, ion transport, and the pathophysiology of gastrointestinal diseases, while adhering to the 3R principles.

Electrical impedance characterization of normal and cancerous human hepatic tissue

Abstract:The four-electrode method was used to measure the ex vivo complex electrical impedance of tissues from 14 hepatic tumors and the surrounding normal liver from six patients. Measurements were done in the frequency range 1-400 kHz. It was found that the conductivity of the tumor tissue was much higher than that of the normal liver tissue in this frequency range (from 0.14 +/- 0.06 S m(-1) versus 0.03 +/- 0.01 S m(-1) at 1 kHz to 0.25 +/- 0.06 S m(-1) versus 0.15 +/- 0.03 S m(-1) at 400 kHz). The Cole-Cole models were estimated from the experimental data and the four parameters (rho(0), rho(infinity), alpha, f(c)) were obtained using a least-squares fit algorithm. The Cole-Cole parameters for the cancerous and normal liver are 9 +/- 4 Omega m(-1), 2.2 +/- 0.7 Omega m(-1), 0.5 +/- 0.2, 140 +/- 103 kHz and 50 +/- 28 Omega m(-1), 3.2 +/- 0.6 Omega m(-1), 0.64 +/- 0.04, 10 +/- 7 kHz, respectively. These data can contribute to developing bioelectric applications for tissue diagnostics and in tissue treatment planning with electrical fields such as radiofrequency tissue ablation, electrochemotherapy and gene therapy with reversible electroporation, nanoscale pulsing and irreversible electroporation.
[Comparison of dielectric properties of normal human esophagus and esophageal cancer using an open-ended coaxial probe]

Abstract:Objective: To study the difference in dielectric properties (relative permittivity and electrical conductivity) between normal human esophageal tissues and esophageal cancer tissues resected in esophageal cancer surgery for rapid identification of esophageal cancer during surgery. Methods: An open- end coaxial probe in the frequency range of 50 MHz to 4 GHz was used for dielectric measurement of the normal tissues and tumor tissues immediately after resection in 51 esophageal cancer cases. The difference in dielectric characteristics of the tissues was analyzed in the full frequency range, and the measurement data at 6 specific frequencies (64, 128, 298, 433, 915, and 2450 MHz) were analyzed statistically. The Cole-Cole model was used for data fitting in the entire frequency band and the measured values were compared with reported values. Results: In the frequency range of 50 MHz- 4 GHz, the dielectric measurement values of esophageal cancer tissues were significantly higher than those of normal esophageal tissues. At the 6 specific frequencies, the dielectric properties also differed significantly between normal esophageal tissues and esophageal cancer tissues (P < 0.01). The Cole-Cole model achieved a good fitting result of the measured data. The measured values in this study were lower than the reported values. Conclusion: There are significant differences in the dielectric properties between normal esophageal tissue and tumor tissue, which may provide a new theoretical basis for rapid identification of esophageal cancer during surgery.
The Ussing chamber system for measuring intestinal permeability in health and disease

Abstract:Background: The relationship between intestinal epithelial integrity and the development of intestinal disease is of increasing interest. A reduction in mucosal integrity has been associated with ulcerative colitis, Crohn's disease and potentially could have links with colorectal cancer development. The Ussing chamber system can be utilised as a valuable tool for measuring gut integrity. Here we describe step-by-step methodology required to measure intestinal permeability of both mouse and human colonic tissue samples ex vivo, using the latest equipment and software. This system can be modified to accommodate other tissues. Methods: An Ussing chamber was constructed and adapted to support both mouse and human tissue to measure intestinal permeability, using paracellular flux and electrical measurements. Two mouse models of intestinal inflammation (dextran sodium sulphate treatment and T regulatory cell depletion using C57BL/6-FoxP3DTR mice) were used to validate the system along with human colonic biopsy samples. Results: Distinct regional differences in permeability were consistently identified within mouse and healthy human colon. In particular, mice showed increased permeability in the mid colonic region. In humans the left colon is more permeable than the right. Furthermore, inflammatory conditions induced chemically or due to autoimmunity reduced intestinal integrity, validating the use of the system. Conclusions: The Ussing chamber has been used for many years to measure barrier function. However, a clear and informative methods paper describing the setup of modern equipment and step-by-step procedure to measure mouse and human intestinal permeability isn't available. The Ussing chamber system methodology we describe provides such detail to guide investigation of gut integrity.

Abstract:Much of our understanding of gut-microbial interactions has come from mouse models. Intestinal immunity is complex and a combination of host genetics and environmental factors play a significant role in regulating intestinal immunity. Due to this complexity, no mouse model to date gives a complete and accurate representation of human intestinal diseases, such as inflammatory bowel diseases. However, intestinal tissue from patients undergoing bowel resection reflects a condition of severe disease that has failed treatment; hence a more dynamic perspective of varying inflammatory states in IBD could be obtained through the analyses of pinch biopsy material. Here we describe our protocol for analyzing mucosal pinch biopsies collected predominantly during colonoscopies. We have optimized flow cytometry panels to analyze up to 8 cytokines produced by CD4+ and CD8+ cells, as well as for characterizing nuclear proteins and transcription factors such as Ki67 and Foxp3. Furthermore, we have optimized approaches to analyze the production of cytokines, including TGF-beta from direct ex vivo cultures of pinch biopsies and LPMCs isolated from biopsies. These approaches are part of our workflow to try and understand the role of the gut microbiota in complex and dynamic human intestinal diseases.

Abstract:Introduction: The electrical impedance spectroscopy technique can be used to measure the electrical impedance of the human bladder tissue, for differentiating pathological changes in the urothelium. Methods: In this study, the electrical impedance spectroscopy technique and then, a numerical technique, finite element analysis (FEA) were used to model the electrical properties of this tissue to predict the impedance spectrum of the normal and malignant areas of this organ. Results: After determining and comparing the modeled data with the experimental results, it is believed that there are some factors that may affect the measurement results. Thus, the effect of inflammation, edema, changes in the applied pressure over the probe and the distensible property of the bladder tissue were considered. Furthermore, the current distribution inside the human bladder tissue was modeled in normal and malignant cases using the FEA. This model results showed that very little of the current actually flows through the urothelium and much of the injected current flows through the connective tissue beneath the urothelium. Conclusion: The results of the models do not explain the measurements results. In conclusion, there are many factors, which may account for discrepancies between the measured and modeled data.
Ex vivo normothermic perfusion of isolated segmental porcine bowel: a novel functional model of the small intestine

Abstract:Abstract Background There is an unmet need for suitable ex vivo large animal models in experimental gastroenterology and intestinal transplantation. This study details a reliable and effective technique for ex vivo normothermic perfusion (EVNP) of segmental porcine small intestine. Methods Segments of small intestine, 1.5–3.0 m in length, were retrieved from terminally anaesthetized pigs. After a period of cold ischaemia, EVNP was performed for 2 h at 37°C with a mean pressure of 80 mmHg using oxygenated autologous blood diluted with Ringer’s solution. The duration of EVNP was extended to 4 h for a second set of experiments in which two segments of proximal to mid-ileum (1.5–3.0 m) were retrieved from each animal and reperfused with whole blood (control) or leucocyte-depleted blood to examine the impact of leucocyte depletion on reperfusion injury. Results After a mean cold ischaemia time of 5 h and 20 min, EVNP was performed in an initial group of four pigs. In the second set of experiments, five pigs were used in each group. In all experiments bowel segments were well perfused and exhibited peristalsis during EVNP. Venous glucose levels significantly increased following luminal glucose stimulation (mean(s.e.m.) basal level 1.8(0.6) mmol/l versus peak 15.5(5.8) mmol/l; P < 0.001) and glucagon-like peptide 1 (GLP-1) levels increased in all experiments, demonstrating intact absorptive and secretory intestinal functions. There were no significant differences between control and leucocyte-depleted animals regarding blood flow, venous glucose, GLP-1 levels or histopathology at the end of 4 h of EVNP. Conclusions This novel model is suitable for the investigation of gastrointestinal physiology, pathology and ischaemia reperfusion injury, along with evaluation of potential therapeutic interventions.
