Abstract:Backgrounds: Although worldwide evidence tends to prove that diabetes adversely affects periodontal health, there are insufficient clues indicating whether periodontitis may aggravate metabolic control and systemic inflammation. This study, as a preliminary part of an ongoing research project, aims to clarify the relationship of periodontal parameters with metabolic levels and systemic inflammatory markers in patients with diabetes.Methods: A total of 140 qualified, adult patients with type 2 diabetes and periodontitis were recruited into this study. Periodontal examinations, including a full-mouth assessment of probing depths (PDs), bleeding on probing, gingival recession, and clinical attachment level, were determined. Blood analyses were carried out for glycated hemoglobin (HbA1c), fasting glucose, high-sensitivity C-reactive protein (hsCRP), tumor necrosis factor-alpha (TNF-alpha), and lipid profiles. Subjects were divided into three groups according to tertiles of the mean PD and compared.Results: Upon an analysis of covariance, subjects with an increased mean PD had significantly higher levels of HbA1c and hsCRP (P <0.05). No significant difference was found among different groups in the levels of serum TNF-alpha, fasting glucose, and lipid profiles (P >0.05). After controlling for age, gender, body mass index, duration of diabetes mellitus, smoking, regular physical exercise, and alcohol consumption, positive correlations were found between mean PD and HbA1c (r = 0.2272; P= 0.009) and between mean PD and hsCRP (r = 0.2336; P= 0.007). After adjustment for possible confounders, the mean PD emerged as a significant predictor variable for elevated levels of HbA1c and hsCRP (P<0.05).Conclusion: Chronic periodontitis was associated with glycemic metabolic and serum hsCRP levels in patients with type 2 diabetes. Abstract:BackgroundPeriodontitis has been associated with type 2 diabetes mellitus. We investigated the effects of local aberrant secretion of adipokines in diabetic rats on systemic metabolism.Material and MethodsOtsuka Long-Evans Tokushima Fatty (OLETF) and non-diabetic Long-Evans Tokushima Otsuka rats were used as a diabetic model and associated control, respectively. Periodontitis was induced using a silk ligature for 36 wk. Rats were grouped into OLETF with (OP+) or without (OP-) periodontitis and Long-Evans Tokushima Otsuka with (LP+) or without (LP-) periodontitis. Alveolar bone resorption and destruction were evaluated by micro-computed tomography and hematoxylin and eosin staining. After 20 wk of periodontitis induction, lipids, insulin, interleukin-1, leptin and plasminogen activator inhibitor-1 were analyzed, and mRNA expressions of NF-B, Mark8, TLR2 and -4, IKBKB and Nampt were evaluated by quantitative polymerase chain reaction in adipose tissue.ResultsAfter ligation, OLETF rats exhibited typical periodontitis lesions with the clinical features of type 2 diabetes mellitus. When compared with the OP(-) group, the area under curve of the oral glucose tolerance test and homeostatic model assessment-insulin resistance values were significantly higher in the OP(+) group. Micro-computed tomography showed that the OP(+) group had more bone resorption than the OP(-) group. When compared with the OP(-) group, the OP(+) group also exhibited higher total cholesterol (p < 0.05), leptin (p < 0.05), insulin (p > 0.05) and higher expression of Nampt (p < 0.05) and IKBKB (p < 0.05). The LP(+) group had a trend towards higher bone resorption when compared with the LP(-) group. Additionally, no difference in white adipose tissues or adipokines was found between LP(+) and LP(-) rats.ConclusionsChronic periodontitis can alter lipid profiles in affected rats, elevate adipose tissue expression of Nampt and affect the metabolism of adipose tissue through the NF-B pathway to inflame diabetes.

Abstract:OBJECTIVE:To evaluate the effects of periodontal treatment on the clinical response, systemic inflammatory parameters, and metabolic control of type 2 diabetes patients with moderate to severe periodontitis.METHODS:A total of 56 patients with mean clinical attachment level (CAL)>3 mm were included in the subgroup analysis. A repeated-measures ANOVA (group factor: treatment group and control group; time factor: initial visit, 1.5, 3, and 6 months) was used to analyze the probing depth (PD), CAL, bleeding on probing (BOP), high-sensitivity C-reactive protein (hsCRP), glycated hemoglobin (HbA1c), and fasting plasma glucose.RESULTS:Significantly lower PD (F=62.898, P-0.000), CAL (F=51.263, P-0.000), BOP (F=75.164, P=0.000), hsCRP (F=6.391, P=0.010), HbA1c(F=4.536, P=0.011), and fasting plasma glucose level (F= 3.073, P=0.031) were observed after therapeutic periodontal improvement. The inter-group differences for PD (t=-2.050, P=0.045), BOP (t=-4.538, P=0.000), and hsCRP (t=-2.261, P=0.028) were statistically significant after therapy.CONCLUSION:Non-surgical periodontal treatment can effectively improve periodontal status, circulating inflammatory status, and metabolic control of diabetic patients with moderate to severe periodontitis.

Abstract:Background: Scientific evidence on the effects of chronic periodontitis on diabetes mellitus remains inadequate and inconclusive. This intervention study is designed to evaluate the effects of periodontal treatment on clinical response, systemic inflammatory parameters, and metabolic control in patients with Type 2 diabetes. Methods: A total of 134 patients were randomly allocated into two treatment groups and one control group. Treatment group 1 underwent non-surgical periodontal treatment at baseline and additional subgingival debridement at the 3-month follow-up. Patients in treatment group 2 received non-surgical periodontal treatment and supragingival prophylaxis at the 3-month follow-up, and those in the control group received no intervention throughout the study. All participants were reexamined at 1.5, 3, and 6 months after initial treatment. At each visit, clinical periodontal examinations were conducted and blood samples were taken to evaluate high-sensitivity C-reactive protein (hsCRP), tumor necrosis factor-a (TNF-alpha), glycated hemoglobin (HbA1c), fasting plasma glucose (FPG), and lipid profiles. Results: Both treatment groups had a significantly lower hsCRP level after periodontal therapy (P < 0.05). Although HbA1c declined significantly in treatment group 2 (P <0.05), the intergroup difference for HbA1c, FPG, TNF-alpha, and lipid profiles was not statistically significant after therapy (P > 0.05). Conclusions: Non-surgical periodontal treatment can effectively improve periodontal and circulating inflammatory status. Abstract:Metabolic syndrome is a cluster of metabolic abnormalities including abdominal obesity, insulin resistance, hypertension and atherogenic dyslipidemia and is associated with increased risk of cardiovascular disease and diabetes. Periodontitis is a chronic inflammatory disease caused by plaque biofilm, which shares risk factors with the major noncommunicable diseases such as heart disease, diabetes and chronic respiratory disease. Many cross-sectional studies have investigated the link between periodontal disease and metabolic syndrome showing that the odds of periodontitis increase with the number of metabolic syndrome components present in an individual although the causal relationship between them has not been determined yet. In this paper, the research progress on the relationship and mechanism of periodontitis with the main characteristics of metabolic syndrome are presented in order to provide new evidences for the prevention and intervention of metabolic syndrome and periodontitis.
Effect of periodontal treatment on glycated haemoglobin and metabolic syndrome parameters: A randomized clinical trial

Abstract:Aim: To assess the effect of periodontal treatment on HbA1c and diagnostic parameters of patients with metabolic syndrome (MetS). Materials and methods: One hundred and fifty-eight patients with MetS and moderate and severe periodontitis were included. They were randomized into a test group (n = 79), which received non-surgical periodontal treatment, and a control group (n = 79), which received no treatment. Medical treatment was delivered to both groups. Clinical periodontal, anthropometric and serological parameters were assessed at baseline, 3 and 6 months. The main outcome was glycated haemoglobin (HbA1c) levels, and the secondary outcomes were changes in the MetS parameters, C-reactive protein (CRP) and HOMA indexes. Results: Significant reductions in all periodontal parameters were observed in the test group, compared with the control group, at 3 and 6 months (p < .001). HbA1c levels, MetS parameters, CRP and HOMA indexes showed no significant differences between the test group and the control group at 3 and 6 months. Conclusions: Periodontal treatment led to a substantial reduction in periodontal inflammation, although there was no significant effect on the parameters used for MetS diagnosis in patients with early diagnosed and well-controlled MetS.

Abstract:The LHC collimation system will require simultaneous management by the LHC control system of more than 500 jaw positioning mechanisms in order to ensure the required beam cleaning and machine protection performance in all machine phases, from injection at 450 GeV to collision at 7 TeV. Each jaw positions a critical parameter for the machine safety. In this paper, the architecture of the LHC collimator controls is presented. The basic design to face the accurate control of the LHC collimators and the interfaces to the other components of LHC Software Application and control infrastructures are described. The full controls system has been tested in a real accelerator environment in the CERN SPS during beam tests with a full scale collimator prototype. The results and the lessons learned are presented.
The impact of frame semantic annotation levels, frame-alignment techniques, and fusion methods on factoid answer processing

Abstract:The impact of frame semantic enrichment of texts on the task of factoid question answering (QA) is studied in this paper. In particular, we consider different techniques for answer processing with frame semantics: the level of semantic class identiﬁcation and role assignment to texts, and the fusion of frame semantic-based answer-processing approaches with other methods used in the Text REtrieval Conference (TREC). The impact of each of these aspects on the overall performance of a QA sys-tem is analyzed in this paper. The TREC 2004 and TREC 2006 factoid question sets were used for the experiments. These demonstrate that the exploitation of encapsulated frame semantics in FrameNet in a shallow semantic parsing process can enhance answer-processing performance in factoid QA systems. This improvement is dependent on the level of semantic annotation, the frame semantic alignment method, and the method of fusing frame semantic-based answer-processing models with other existing models. A more comprehensively annotated environment with all different part-of-speech target predicates provides a higher chance of correct factoid answer retrieval where semantic alignment is based on both semantic classes and a relaxed set of semantic roles for answer span identiﬁcation. Our experiments on fusion techniques of frame semantic-based and entity-based answer-processing models show that merging answer lists with respect to their scores and redundancy by exploiting a fusion function leads to a more effective overall factoid QA system compared to the use of individual models.
Relationship of metabolic syndrome to chronic periodontitis.

Abstract:Background: Many studies have suggested that periodontal disease is related to metabolic disorders. Few studies have investigated metabolic syndrome (MS), as defined by the International Diabetes Federation in 2005, in the Chinese population. The aim of this study was to assess the association of periodontitis with MS. Methods: In this case-control study, 152 patients with MS and 56 generally healthy adults received a comprehensive clinical dental examination. Attachment loss, probing depth, bleeding index, and plaque index were examined at two sites per tooth (mesio-buccal and disto-lingual). The number of missing teeth was noted in each subject. Results: The male patients with MS were significantly younger and their periodontal parameters were significantly higher than those of the healthy group. The female patients with MS were significantly older, and all periodontal parameters were higher than those in the healthy group; only plaque index was statistically significantly different (P = 0.001). After adjustment for gender, age, and smoking, the corresponding adjusted odds ratios for MS were 6.9 (95% confidence interval [CI]: 1.07 to 44.77), 9.9 (95% Cl: 1.50 to 65.24), and 15.6 (95% Cl: 2.20 to 110.43) for subjects with attachment loss 3 mm in >0% to 33% of sites, >33% to 67% of sites, and >67% of sites, respectively. Most periodontal parameters were highest in patients with four components of MS, but these differences were not statistically significant. Conclusion: Patients with MS had poor periodontal conditions, and periodontal disease was associated with MS, independent of other risk factors. J Periodontol 2009;80:541-549.

Abstract:Background/Aims: Eptifibatide is a parenteral glycoprotein IIb-IIIa inhibitor that prevents platelet aggregation. Although contraindicated in dialysis patients due to limited safety and dialysis data, eptifibatide is prescribed in this population and is associated with bleeding complications. This study was done to determine dialysis clearance (CLD) of eptifibatide using an in vitro system. Methods:Three common dialyzers were tested. In vitro dialysis was performed at a dialysate flow rate of 500 ml/min, ‘blood’ flow rate (QB) of 200 and 400 ml/min, and the minimal ultrafiltration rate. Eptifibatide CLD and fraction removed were calculated for each condition. Results: CLD ranged from 122 to 225 ml/min and was not significantly different among the dialyzers tested. CLD was flow dependent with higher clearances observed at higher QB. The estimated fraction of eptifibatide removed was 73–83%. Conclusions: These data suggest that hemodialysis is an effective method to decrease the effects of eptifibatide in patients with impaired kidney function.
Severe periodontitis is associated with systemic inflammation and a dysmetabolic status: a case-control study

Abstract:Background and aim: A cluster of metabolic factors defines a syndrome that predisposes to diabetes and cardiovascular disease. Chronic infections such as periodontitis might alter these individual metabolic factors and the systemic inflammatory burden. The aim of this study was to investigate the association between severe periodontitis and increase in inflammatory and metabolic risk factors for cardiovascular disease. Materials and methods: We examined 302 patients with severe periodontitis and 183 healthy controls, and we collected a blood sample from each subject in order to investigate differences in inflammatory (leukocyte numbers and differential counts) and metabolic markers (lipids and glucose). Results: After correcting for differences in age, gender, smoking and ethnicity, periodontitis subjects exhibited a low-grade systemic inflammation (increased white cell counts, 1.10+/-1.02 x 10(9)/l, 95%CI 1.05-1.15, p=0.0001), dyslipidemia [lower high-density lipoprotein cholesterol, 1.14+/-1.03 mmol/l, 95%CI 1.08-1.20, p<0.0001 and higher low-density lipoprotein cholesterol, 1.12+/-1.03, 95%CI 1.05-1.19, p<0.0001) and increased non-fasting serum glucose levels (1.04+/-1.01 mmol/l, 95%CI 1.02-1.06, p=0.01) when compared with controls. The associations were confirmed in a subpopulation of Caucasian non-smokers. A trend for a dose dependent effect of the number of periodontal pockets on the tested inflammatory and metabolic markers was observed. Conclusions: These data suggest a possible link between severe generalized periodontitis, systemic inflammation and a dysmetabolic state in otherwise healthy individuals.
The Associations of Cardiometabolic and Dietary Variables with Clinical Periodontitis in Adults with and without Type 2 Diabetes: A Cross-Sectional Study

Abstract:Periodontitis is a commonly occurring inflammatory oral disease affecting a large proportion of global and US adults and is characterized by the destruction of the tooth-supporting apparatus. Its etiology is multifactorial, and type 2 diabetes and diet play critical roles in its remission and progression. However, few studies have addressed nutritional and serum vitamin D status in adults with periodontitis in the presence of diabetes. A cross-sectional study (n = 78), and a sub-set of age- and BMI-matched case-control studies (n = 50), were conducted to examine differences in dietary and cardiometabolic variables, and serum vitamin D in adults with periodontitis with or without diabetes. Participants provided fasting blood samples and 24-h diet recalls on at least two different days. Data on health history, body weight, height, nutritional habits, and clinical features of periodontitis were also collected. The Mann-Whitney U Test (with exact p-value estimation by Monte Carlo simulation) was used to examine differences by diabetes status in continuous and ordinal variables. Results revealed significantly lower serum vitamin D, and dietary intake of fruits, vegetables, dairy, vitamins A and C in adults with periodontitis with vs. without diabetes in the sub-study (all p < 0.05). In the overall sample, adults with diabetes presented with higher caries risk measures and lower numbers of teeth than those without diabetes; plaque and bleeding scores did not differ by diabetes status. Finally, a significant associations of food habits was observed, especially consuming protein-rich foods twice a day with a lower bleeding score, and daily consumption of fried or fast foods with a fewer number of teeth present (all p < 0.05). The present findings show significant dietary and serum vitamin D inadequacies among adults with periodontitis, and diabetes further aggravates the observed malnourishment and oral health.

Abstract:Periodontal disease is a complex and progressive chronic inflammatory condition that leads to the loss of alveolar bone and teeth. It has been associated with various systemic diseases, including diabetes mellitus and obesity, among others. Some of these conditions are part of the metabolic syndrome cluster, a group of interconnected systemic diseases that significantly raise the risk of cardiovascular diseases, diabetes mellitus, and stroke. The metabolic syndrome cluster encompasses central obesity, dyslipidemia, insulin resistance, and hypertension. In this review, our objective is to investigate the correlation between periodontal disease and the components and outcomes of the metabolic syndrome cluster. By doing so, we aim to gain insights into the fundamental mechanisms that link each systemic condition with the metabolic syndrome. This deeper understanding of the interplay between these conditions and periodontal disease can pave the way for more effective treatments that take into account the broader impact of managing periodontal disease on the comprehensive treatment of systemic diseases, and vice versa.

Abstract:<p>This review investigated the association of periodontal disease with diabetes mellitus (DM) and diabetic complications. PubMed/MEDLINE was searched including search terms "periodontal" OR "periodontitis" AND "diabetic complications" OR "diabetic retinopathy" OR "diabetic nephropathy" OR "diabetic neuropathy" OR "cardiovascular disease diabetes" OR "myocardial infarction diabetes" OR "cerebrovascular disease diabetes" OR "stroke diabetes" OR "peripheral vascular disease diabetes". Fourteen studies included in this review consistently reported an increased risk for diabetic complications including microvascular, macrovascular and death in the presence of periodontal disease. Higher risks for diabetic retinopathy (odds ratios: 2.8 - 8.7), neuropathy (3.2 - 6.6), nephropathy (1.9 - 8.5), cardiovascular complications (1.28 - 17.7) and mortality (2.3 - 8.5) were reported for people with diabetes with periodontitis compared to those with diabetes who have no periodontitis. This novel review summarizes current data providing further evidence of a link between poor oral health and DM and its complications. It has also drawn attention to major limitations of the available data linking periodontal disease and diabetic complications.</p>

Abstract:Dans cette these nous nous interessons a des problemes d’apprentissage automatique dans le cadre de sorties structurees avec une structure sequentielle. D’une part, nous considerons le probleme de l’apprentissage de mesure de similarite pour deux tâches : (i) la detection de rupture dans des signaux multivaries et (ii) le probleme de deformation temporelle entre paires de signaux. Les methodes generalement utilisees pour resoudre ces deux problemes dependent fortement d’une mesure de similarite. Nous apprenons une mesure de similarite a partir de donnees totalement etiquetees. Nous presentons des algorithmes usuels de prediction structure, efficaces pour effectuer l’apprentissage. Nous validons notre approche sur des donnees reelles venant de divers domaines. D’autre part, nous nous interessons au probleme de la faible supervision pour la tâche d’alignement d’un enregistrement audio sur la partition jouee. Nous considerons la partition comme une representation symbolique donnant (i) une information complete sur l’ordre des symboles et (ii) une information approximative sur la forme de l’alignement attendu. Nous apprenons un classifieur pour chaque symbole avec ces informations. Nous developpons une methode d’apprentissage fondee sur l’optimisation d’une fonction convexe. Nous demontrons la validite de l’approche sur des donnees musicales.
[Correlation Between Periodontitis and Metabolic Syndrome of the Middle-Aged and Aged Population in Shijingshan Community of Beijing].

Abstract:OBJECTIVE:To survey the metabolic status of the middle-aged and aged population with periodontitis in Shijingshan community of Beijing, and investigate the relationship between periodontitis and metabolic syndrome (MS).METHODS:The middle-aged and aged population in the community were investigated by questionnaires, periodontal clinical examinations and blood biochemical tests in 2005. A total of 903 subjects were enrolled, who were divided into two groups by severity of periodontitis. Their waist circumferences, values of high-density lipoprotein cholesterol (HDL-C), systolic blood pressure, fasting glucose, MS and its individual components (central obesity, insulin resistance, hypertriglyceridemia, low HDL-C and hypertension) were compared between the two groups. The Logistic regression model was set to analyze the relationship between periodontitis and MS.RESULTS:There was a significantly higher mean of systolic blood pressure, fasting glucose in the subjects with moderate-severe periodontitis than that with no-mild periodontitis. With severity of periodontitis increasing, the prevalence of MS, high blood glucose and low HDL-C increased significantly. After adjustment for gender, age, and smoking, the subjects with moderate-severe periodontitis were 1.524, 1.527 and 2.349 times more likely to suffer from MS, high blood glucose and low HDL-C than those with no-mild periodontitis, respectively.CONCLUSION:With severity of periodontitis increasing, the prevalence of MS, high blood glucose and low HDL-C increased significantly in the middle-aged and aged population of the community in Beijing. Severity of periodontitis is associated with MS, high blood glucose and low HDL-C.
All‐cause and cause‐specific mortality in US adults with periodontal diseases: A prospective cohort study

Abstract:Aim This prospective cohort study investigated the association between periodontal diseases (PDs) and all‐cause and cause‐specific mortality. Materials and Methods We utilized adult participants recruited from six National Health and Nutrition Examination Survey cycles (1999–2014) and linked mortality data from the National Death Index up to December 2019. Baseline clinical periodontal examinations were performed by trained and calibrated examiners. All‐cause and cause‐specific mortality was modelled through multivariable Cox proportional hazards and Fine–Gray models to account for competing risks. All models were adjusted for demographic and lifestyle variables, clinical measurements and comorbidities. Results Overall, 15,030 participants were included, with a median length of follow‐up of 9 years. Risk of all‐cause mortality was 22% greater in people with PD than the control group (adjusted hazard ratio [HR]: 1.22, 95% confidence interval [CI]: 1.12–1.31). Risks of mortality by cardiovascular diseases (CVD), respiratory disease and diabetes were highest in participants with severe PD (CVD—sub‐distribution HR [SHR]: 1.38, 95% CI: 1.16–1.64; respiratory—SHR: 1.62, 95% CI: 1.07–2.45; diabetes—SHR: 1.68, 95% CI: 1.12–2.53). Conclusions Severe PD is associated with all‐cause and cause‐specific mortality among US adults after multivariable adjustment.

Abstract:Aim To evaluate the association between periodontitis, all‐cause and cause‐specific mortality, and its prognostic utility among adults with diabetes. Materials and Methods Periodontal health records were retrieved from the NHANES database for 4297 participants with diabetes aged >30 years at baseline during 1988–1994, 1999–2004 and 2009–2014. Multivariable Cox proportional hazards regression model was applied to calculate the hazard ratios (HRs) and 95% confidence intervals (CIs) for moderate/severe periodontitis with all‐cause and cause‐specific mortality in participants with diabetes. Area under the curve (AUC) was used to assess predictive value. Results During a median follow‐up of 15.41 years, 1701 deaths occurred. After multivariate adjustments, moderate/severe periodontitis was significantly associated with increased risk of all‐cause (HR: 1.27; 95% CI: 1.07–1.50; p = .005) and cardiovascular disease (CVD)‐related (HR: 1.35, 95% CI: 1.03–1.76, p = .031) mortality in participants with diabetes. The absolute risk difference based on the cumulative incidence information was 0.022 (5‐year, 95% CI: 0.021–0.023) and 0.044 (10‐year, 95% CI: 0.041–0.048). Periodontitis improved the prediction of all‐cause (AUC: 0.652; 95% CI: 0.627–0.676) and CVD‐related (AUC: 0.649; 95% CI: 0.624–0.676) mortality over standard risk factors (all‐cause: AUC: 0.631; 95% CI: 0.606–0.656; CVD‐related: AUC: 0.629; 95% CI: 0.604–0.655). Conclusions Moderate/severe periodontitis is associated with an increased risk of all‐cause and CVD‐related mortality in adults with diabetes. Periodontitis might represent a marker for residual risk.
