Abstract:Objective: Most older adults in the United States (U.S.) prefer to remain in their current housing as they age, but difficulty with mobility (e.g., walking) may make this more challenging. We estimated the association between difficulty with mobility (mobility) and residential relocation in the following year. Methods: We included adults aged ≥65 participating in ≥2 rounds (years 2011-2019) of the National Health and Aging Trends Study. Mobility was self-reported and change in permanent address was captured by study staff. We used a prospective cohort design and logistic regression with a clustering correction to estimate adjusted associations. Results: We did not find evidence that mobility was associated with relocation versus no relocation in our sample of 26,444 observations from 5699 older adults. However, mobility was positively associated with moves to housing with fewer environmental barriers. Discussion: Mobility is associated with relocation to housing that is more accessible and may be an indicator of greater need for supports to age in place.

Abstract:Objectives: In this study, we examine how residents are affected by moving from a regular nursing home into an innovative living arrangement. In the past decade, a culture change has taken place, leading to rapid developments of innovative living arrangements that aim to change the physical, social, and organizational environment to better suit the needs of older adults needing 24-hour care. This has inevitably led to more group relocations in long-term care. Insight into the change in residents when relocating is lacking. Design: An observational longitudinal study. Setting and participants: Four Dutch care organizations in which 5 relocations took place from a regular to an innovative living arrangement. Residents (N = 97) requiring 24-hour care who were relocated from a regular nursing home to an innovative living arrangement were included. Methods: Data were collected 1 month before, 2 weeks after, and 6 months after relocating. Depressive signs and symptoms, cognitive functioning, and dependence in activities of daily living were measured using questionnaires. Furthermore, the daily lives of the residents were assessed using ecological momentary assessments. Results: Overall, no long-term change in depressive signs and symptoms, cognitive functioning, and dependence in activities of daily living was found when relocating. Furthermore, the daily life of residents was not different 6 months after moving. Relocating was accompanied by a significant short-term increase in depressive signs and symptoms in 2 out of 4 locations (P < .001). Conclusions and implications: This study shows that relocating to an innovative living arrangement does not lead to long-term changes in residents. There are indications that there might be a short-term change in depressive signs and symptoms that could be prevented by considering the approach and context. More research is needed into the changes in the physical, social, and organizational environment that are necessary for a positive impact on the daily lives of residents.

Abstract:Objectives: Elderly people who live in nursing homes can be faced with intramural relocations for various reasons, whether individual or per group. Because the impact of these intramural relocations on residents is unknown, our aim is to explore how relocations within nursing homes affect residents from different stakeholder-perspectives. Methods: We performed semi-structured individual interviews and a focus group with various stakeholders of intramural relocations to collect a broad outlook on its practice based on diverse perspectives and lived experiences. The interviews and focus group were audio-recorded, transcribed verbatim and analyzed using responsive and thematic analysis. Results: Seventeen interviews were held and one online focus group with six participants. In the interviews, participants mentioned various reasons for intramural relocations, such as outdated real estate for group relocations or changing healthcare demand for individual relocations. Participants distinguished various levels of impact on residents on different moments in time (before, during and after the relocation). The impact varied from very positive (e.g. looking forward and excited) to very negative (e.g. stressful, traumatic, hard to understand). Aspects that influenced the impact of relocation were related to 1) the mental resilience of residents, 2) how relocations were organized, 3) the presence and quality of social connections of residents and 4) if benefits of the new (care) environment were experienced. The focus group added insights on the importance of clear and timely communication with residents and recognizability of (personal) items and personnel from the former nursing home to reduce negative experiences of residents relocating within nursing homes. Conclusions: The impact of relocations within nursing homes differs per situation, moment in time and resident. Aspects found that influence the impact provide targets to reduce the negative impact on residents: practices should focus on good preparation, clear communication, preserving social connections of residents where possible and paying attention to the benefits of the new (care) environment for the residents. Further research may focus on the lived experiences and perceived impact of relocations within nursing homes of residents themselves to develop in depth insights into tailored (care) needs of residents during the relocation process.

Abstract:Dysregulation of epithelial‐to‐mesenchymal transition (EMT) may contribute to renal fibrogenesis. Our previous study indicated that bone morphogenetic protein‐2 (BMP‐2) significantly reversed transforming growth factor (TGF)‐β1‐induced renal interstitial fibrosis. In this study, we examined the underlying mechanism and elucidate the regulation of EMT process under BMP‐2 treatment. Cultured renal interstitial fibroblast (NRK‐49F) was treated with TGF‐β1 (10 ng/ml) with or without BMP‐2 (10–250 ng/ml) for 24 h. The expression of α‐smooth muscle actin (α‐SMA), E‐cadherin, fibronectin, or Snail transcriptional factors was analyzed by immunofluorescence staining or Western blotting. Cell migration was analyzed by wound‐healing assay. NRK‐49F treated with TGF‐β1 induced significant EMT including upregulatioin of α‐SMA, fibronectin, and snail proteins and down‐regulation of E‐cadherin. Interestingly, co‐treatment with BMP‐2 dose‐dependently reversed TGF‐β1‐induced cellular fibrosis, cell migration, and above EMT change. The above effect was closely correlated with Snail since BMP‐2 dose‐ and time‐course dependently induced a significant decrease in the level of Snail. Moreover, Snail siRNA significantly reversed TGF‐β1‐induced increases in the level of α‐SMA and fibronectin (intracellular and extracellular). We suppose that BMP‐2 have the potential to attenuate TGF‐β1‐induced renal interstitial fibrosis by attenuating Snail expression and reversing EMT process. J. Cell. The frequency and types of resident relocations in Dutch nursing homes: a nationwide cohort study of electronic health record data

Abstract:We examined the occurrence of relocations of nursing home residents, the distribution of individual and group relocation types, the distribution of destinations, and trends over time. One third of Dutch nursing home residents relocated at least once during length of stay. Roughly 75 percent were individual relocations and the other 25 percent concerned group relocations. The average yearly number of individual relocations was about 3 times as high in the first 4 months after admission compared with later periods. A considerable proportion of Dutch long-stay nursing home residents experienced one or more relocations. One third of Dutch nursing home residents relocated at least once during length of stay. Roughly 75 percent were individual relocations and the other 25 percent concerned group relocations. The average yearly number of individual relocations was about 3 times as high in the first 4 months after admission compared with later periods. We performed an historical cohort study of pseudonymized registration data from a Dutch electronic health record. We selected data from nursing home residents aged 65 years and older who stayed on a ward for physical impairment or dementia and passed away between 2015 and 2019. Our study sample consisted of 26,060 long-stay nursing home residents from 67 nursing homes in the Netherlands. We examined the number of relocations, trends over time, individual versus group relocations and relocation destinations. We found that approximately one-third of long-stay nursing home residents relocated at least once with an average of 36 relocations per 100 residents per year. Roughly, 75 percent of relocations were individual relocations and 25 percent concerned group relocations. In the first 4 months after admission, the average number of individual relocations per 100 resident per year was about 3 times as often compared to later periods after admission. Most individual relocations were within the same type of care. A considerable proportion of Dutch long-stay nursing home residents experienced one or more relocations. Relocations for individual reasons occurred mostly in the first months after admission. Further investigation is warranted to explore which factors lead to relocations.

Older People as Active Agents in Their Neighborhood Environments: Moving House Can Improve Quality of Life

Abstract:Background and objectives: Neighborhood environments are an important aspect of well-being for older people. Developments of the general ecological model recognize older people as active agents who adapt their environments to fit their changing needs. We provide empirical support for a model suggested by Wahl et al., by examining interactions between neighborhood environments, personal situations, relocation, and quality of life (QoL) among older people. Research design and methods: Two statistical models were tested with a sample of community-dwelling participants (aged 55-89) in the New Zealand Health, Work, and Retirement longitudinal surveys conducted in 2016 (T1) and 2018 (T2). Multiple linear regression assessed the association of perceptions of housing and neighborhood with QoL at T1 (n = 3682). Results: QoL was predicted by housing satisfaction, and neighborhood satisfaction, accessibility, and trust (controlling for age, gender, marital status, home ownership, socioeconomic status, physical health, and mental health). Mixed analysis of variance showed that those who moved house between T1 and T2 reported lower housing and neighborhood satisfaction than nonmovers at T1. Over time T1-T2, movers reported less decline in housing satisfaction, and more positive change on all neighborhood perceptions, with higher perceptions of neighborhood accessibility than nonmovers at T2. Discussion and implications: These findings support the theoretical model and provide the impetus for a more detailed study of the effects of the environment on well-being in older age. Housing and neighborhood environments are a very practical focus for social policy change at local and national levels.
Differentiating intentions and competence: Exploring living environments and health outcomes among older adults in diverse residential (im-)mobility scenarios

Abstract:The growing mobility and academic emphasis have challenged the traditional link between remaining in one place and successful aging, emphasizing dwelling conditions that fit older adults and their preferences for relocating or staying. Using 5034 older-participant data in the CFPS 2012/2014 waves, we categorized them into six categories based on their intended or unintended (im-)mobility. Descriptive statistics revealed diverse housing and community characteristics and their health consequences. Logistic regression and paired-sample Wilcoxon signed-rank tests examined health changes across categories, while panel-ordered logit models explored the influence of housing and community features on health. The results indicate no significant correlation between (im-)mobility types and health outcomes, although certain subgroups exhibit meaningful relationships. Intended immobility seems to provide more psychological benefits than substantive health advantages. Whether intentional or not, relocation often leads to improved housing conditions, better aligning them with physical needs. The study also outlines the (im-)mobility process among older adults, from intention and competence to action, and establishes an intention/competence model for their (im-)mobility. Enhanced comprehension in this field would be beneficial for reassessing whether older individuals should stay in place or relocate, and for emphasizing the significance of tailored strategies for the living arrangements of the older population.

Abstract:Tobacco consumption is a leading cause of various types of cancer and other tobacco-related diseases. In 2003, the World Health Assembly adopted the World Health Organization Framework Convention on Tobacco Control (WHO-FCTC), which aims to protect citizens from the health, social, environmental, and economic consequences of tobacco consumption and exposure to tobacco smoke. The Convention was to be ratified by the Member States of the WHO; in Brazil's case, ratification involved the National Congress, which held public hearings in the country's leading tobacco growing communities (municipalities). The current study analyzes this decision-making process according to the different interests, positions, and stakeholders. In methodological terms, this is a qualitative study based on document research, drawing primarily on the shorthand notes from the public hearings. We analyze the interests and arguments for and against ratification. The article shows that although preceded by intense debates, the final decision in favor of ratification was made by a limited group of government stakeholders, characterizing a decision-making process similar to a funnel.
Assessing older persons' readiness to move to independent congregate living

Abstract:Older adults are increasingly choosing to relocate to congregate-type independent living communities. Relocation to an independent living community is a late-life transition that is considered a stressful life event. Although relocation to an independent living community offers potential benefits, many older persons have difficulties during this transition, including poor adjustment, loneliness, and depression. All of these are associated with poorer health, higher healthcare costs, increased risk of institutionalization, and increased morbidity and mortality. This article provides guidelines for assessing the readiness of an older person to move to an independent living community and implications for advanced practice nurses whose role encompasses promoting the health and well-being of older adults. Using the assessment guidelines, the advanced practice nurse can identify older persons at risk for difficulty during relocation and intervene with guidance and strategies to promote positive relocation adjustment.
Impact of Social Frailty on Relocation of Older Adults

Abstract:Background: The relationship between frailty and variables such as housing are the least included in models of frailty and research on frailty or social frailty and relocation is negligible. The decision to relocate is complex and demanding for older adults with a loss of independence but little is known about what makes older adults relocate to congregated housing designated for older adults, let alone in combination with social frailty, and how they navigate this transition. Objectives: This mixed method descriptive study aims to understand the influence of social frailty for a population of French-speaking semi-independent older adults relocating to a housing continuum community. Design: Semi-structured individual interviews including sociodemographic data and the PRISMA-7 Frailty Scale were conducted with recently relocated older adults. Setting: A newly opened French-speaking housing continuum community in Eastern Canada that offers luxury apartments for independent older adults, two assisted living facilities for semi-independent older adults along with a long-term care facility. Participants: Twenty-nine older adults with a mean age of 85 years, mostly female, married or widowed and highly educated. Measurements: Content analysis of the transcribed recorded interviews and descriptive statistical analyses to examine relationships between the frailty PRISMA-7 scale, answers to additional questions and the sociodemographic data. Results: There was not a significant difference in the scores for socialization before and after relocation nor between prior help and current help; however, there was a significant negative correlation between help and socialization before and after relocation. Three main themes included: imposed influences, push and pull factors and post relocation. Conclusions: The results indicate that several social factors contributed to relocation and that participants were experiencing social frailty. Participants were at the crossover point of being vulnerable to experiencing additional deficits which would potentially have led to higher frailty had they not relocated.

Abstract:There are few existing studies from a spatial perspective that explore the mechanism of the living environment, relocated direction and relocated distance on the elderly's physical and mental health. Based on the 2019 household survey questionnaire data, this study utilizes a binomial logistic regression model and residents' in-depth interviews to study relocated behavior in Nanjing. We analyzed the spatial–temporal characteristics of relocated behaviors, the accumulation effect at different stages and the influence of relocated intention on physical and mental health among the elderly. The findings are as follows: (1) The mode of relocation among the elderly includes long-term cumulative effects and short-term effects. Frequent relocation has a cumulative negative impact on the physical and mental health of the elderly. (2) The elderly relocated in the short-term had a great negative impact on their mental health but had no significant impact on their physical health. (3) In the last relocation, active relocation had a significantly positive impact on physical health. The "centrifugal relocation" from the main urban area to the surrounding new cities has a significant positive impact on mental health. Moreover, long-distance relocations adversely affected mental health.

Community-Based Residential Relocation in Middle and Older Age: A United States–China Comparison Study on Its Predictors and Associated Mortality Risk

Abstract:Journal of Aging and Health, Ahead of Print. ObjectivesThis study compared the United States and China in examining the predictors of community-based residential relocation and its associated mortality risk.MethodsData from the 2010 to 2018 US Health and Retirement Study and from 2011 to 2018 China Health and Retirement Longitudinal Study were used (NUS = 20,292 and NChina = 11,694). Community-dwelling respondents (aged 50+) reported whether they had relocated and were followed up until 2018. Log-binomial regression and Cox survival analysis were used.ResultsIn both countries, younger age, higher education, urban residence, and being a renter were associated with higher likelihood of relocation. Community-based relocation was associated with a lower mortality risk (US: HR = 0.63, 95% CI = 0.57, 0.70; China: HR = 0.40, 95% CI = 0.31, 0.50), and this association was significantly stronger in China compared to the United States.DiscussionCommon predictors of community-based relocation were found in the United States and China. The relocation-related survival advantages may be attributed to a better post-move adaptation and living environment.

Hospital stays and probable dementia as predictors of relocation to long-term care facilities

Abstract:Objectives: This study aims to investigate the relocation of older adults in the US from community living to long-term care facilities (LTCFs). Specifically, it examines the predictive roles of possible and probable dementia and hospital stays in this complex health care transition. Study design: Utilizing data from the National Health and Aging Trends Study, a longitudinal cohort study (2011-2019), we employed a panel data approach, which consists of multiple observations over time for the same participants, allowing us to account for both cross-sectional variations (differences between participants) and time-series variations (changes in the same participant over time). Methods: The analysis involved longitudinal logistic regression models. Using the AD8 dementia screening interview, clock drawing test, immediate and delayed word recall test, orientation, and history of dementia diagnosis, we placed participants into categories of having no dementia, possible dementia, and probable dementia. A survey asked about hospital stays in the past year. Relocation to LTCFs was examined based on the changes to the living location. Results: The proportion of individuals transitioning to LTCFs tripled between 2011 and 2019, emphasizing the need to understand and manage this health care transition. Hospital stays significantly increased the probability of moving to LTCFs, especially nursing homes. Probable dementia demonstrated a 3-fold increase, aligning with the rising prevalence of Alzheimer disease. Difficulty walking and climbing stairs significantly increased relocation probabilities. Conclusions: The study findings emphasize complexity in late-life relocations influenced by dementia and hospital stays. Screening for cognitive function among community-dwelling older adults, particularly those with a history of hospital stays and mobility difficulties, can inform interventions and policies. Implications extend to health care policy, geriatric care, and the imperative for targeted interventions considering demographic variations. Future research should explore additional variables and address limitations to refine our understanding of the relocation process.

Abstract:As people age the home environment becomes increasingly important. Retirement commonly leads to spending more time in one's home, and relocating from your own home in older age could be associated with reduced health or wellbeing. The relationship between home and person is complex and perceived aspects of one's housing such as social, emotional and cognitive ties are considered important factors for health and wellbeing. However, little is known about how perceived aspects of the home change in relation to retirement and relocation. This paper used Situational Analysis to explore, via situational mapping, how community dwelling older adults (aged 60-75) perceived their housing situation in relation to retirement and relocation. The results suggest complex relations between relocation/retirement and perceived housing, and between different aspects of perceived housing. Furthermore, the results suggest that the relationship between life transitions and perceived housing can be seen as bi-directional, where different life transitions affect aspects of perceived housing, and that perceived housing affects (decisions for) relocation. The results suggest complex relations between retirement and relocation, as well as other life transitions, and perceived aspects of one's housing. It is important to consider these interactions to understand factors that affect health and wellbeing in older adults.

Abstract:Moving into a nursing home is a significant transition for older people which can be emotional and stressful. The literature on the decision to move and subsequent residential stress focuses on the time after admission; however, there is less understanding of older people's management of the process. To fully understand and possibly reduce negative consequences and health‐related risks, the aim of this article is to explore how older people manage the transition from home to nursing home. The study is based on 21 longitudinal interviews with seven older people during the process of moving. Following Dorothy Smith's theory of work, the analysis finds three types of work—practical, relational, and emotional—that older people engage in during the transition process. The results highlight the importance of considering the entire transition process, not only experiences before or after, if we are to reduce the negative, stress‐related consequences of relocations of this kind. The findings have practical implications for professionals in health and social care and point to the need to develop practical, relational, and emotional support for older people throughout the transition process.

Life events, social conditions and residential mobility among older adults

Abstract:This study explores whether and how social conditions—ties to kin and friends as well as loneliness—are related to older adults' residential mobility, in general, and in combination with a late‐life event, specifically. Drawing on panel data from the US Health and Retirement Study (N = 9107), logistic regression models examine whether older adult moves are triggered by life events (retirement, widowhood and health deterioration) and loneliness, and discouraged by the presence of nearby social ties (family and friends). The results indicate that becoming retired, becoming widowed, nearby family and nearby friends are indeed independently associated with moving. Loneliness is also associated with residential mobility, even when controlling for nearby family and friends. Social conditions do not, however, moderate the effect of late‐life events on older adults' residential mobility. The only exception is the moderating role of loneliness on the effect of retirement: contrary to our expectation, the likelihood of postretirement moves declines with increasing levels of loneliness.

Abstract:Background Residents of assisted living facilities (ALF) transfer to a nursing home when they require a higher level of care, but limited research has examined risk factors for transfer to a nursing home. The aims of this study were to identify (1) baseline factors associated with transfer to a nursing home and (2) time‐varying factors associated with transfer to a nursing home over 8 years, using a national dataset from the National Health Aging Trends Study (NHATS). Methods NHATS participants were included in this study if they: (1) resided in ALF from Round 1 (2011) through Round 8 (2018); (2) completed the sample person (SP) interview at baseline; (3) were admitted to ALF at age 65 years or older. We conducted Cox proportional hazards regression to examine candidate predictors (difficulty with basic activities of daily living (ADL), chronic conditions, hospitalization, sleep disturbances, mental health, physical performance, self‐reported health, participation in social and physical activity, and sociodemographic) associated with transfer to a nursing home. Employing backward elimination, we built parsimonious final models for analysis. Results The analytic sample included 970 participants of whom 143 transferred to nursing homes over 8 years. Those who had a better physical performance at baseline (HR = 0.83, 95% CI = 0.79–0.88) and were college educated (HR = 0.58, 95% CI = 0.36–0.92) demonstrated a significantly lower risk for transfer to a nursing home over 8 years. Residents who maintained physical activity (HR = 0.56, 95% CI = 0.37–0.86), better physical performance (HR = 0.87, 95% CI = 0.80–0.94), and difficulty with fewer basic ADLs (HR = 1.13, 95% CI = 1.02–1.26) were at lower risk for transfer to a nursing home over 8 years. Conclusions Our findings can be used to identify older adults in ALFs at risk of transfer to a nursing home. Strategies to promote physical function and physical activity could avoid/delay the need to transfer. Helping older residents to age in place will have important health and economic benefits.

Abstract:Moving in adolescence could be a stressful experience, potentially disturbing development into adulthood. This study investigated the relationship between residential mobility in adolescence and life satisfaction in young adulthood, focusing on moving timing, frequency, and distance, using (retrospective) longitudinal German survey data ( N = 2998, mean age = 20.18, 45.8% boy, 6.8% first-generation immigrant, 31.0% children of immigrants). Moving twice or more in adolescence was linked to lower life satisfaction in young adulthood, even after controlling for family background characteristics. Moving distance was unrelated to life satisfaction. Exploratory analyses showed that particularly for children of immigrants, frequent moving was related to lower life satisfaction. Since frequent moving in adolescence was more common in disadvantaged families, frequent moving could reinforce social inequalities into young adulthood.
