Abstract:The proton spot scanning system requires extensive periodic quality assurance procedures to guarantee the conformality of planned dose distribution to the tumor. Detectors with stable, accurate, and fast data acquisition properties make the quality assurance procedures more efficient. For this purpose, a camera-based scintillation imaging system has been developed. A high scintillation efficiency screen coupled with a high-performance camera improved the dose sensitivity and precision required for the measurements. The dose distribution within the scintillating screen can be analyzed from the captured light distribution image in a relatively straightforward way. In this study, we investigated the performance of a 2D detection system by using a scintillating screen (silver-activated zinc sulfide powder deposited on the surface of a plastic scintillator HND-S2) for dose distribution measurement. The characteristics of the system in terms of response reproducibility, linear dose-response, and beam spot size dependence on proton dose were assessed in a series of proton irradiations. To evaluate the accuracy of lateral dose profiles measured by the ZnS: Ag based scintillating screen, we compared the results with plastic scintillator BC-408 and some commercial quality control devices. Several measurements were conducted with proton energies ranging from 70-235 MeV. We found that the detector showed stable response reproducibility (better than 1.6%) and good dose-response linearity (R-2>0.999). The beam spot size was independent of proton dose when the response of the pixel value was between 10% and 90% of the maximum. The in-air spot size measured by the ZnS: Ag based scintillating screen was found to be slightly smaller than the Lynx detector while the BC-408 showed a slightly bigger spot size. The inter-comparison results for scanned fields showed good agreements. The observed differences in beam profile measurement between the inorganic and organic scintillating screens were mainly due to their optical properties and structures. The developed scintillation imaging system with an effective resolution of 0.12 mm proved to be a reliable device for efficiently performing the quality checks for two-dimensional proton dosimetry.
Implementation of novel measurement-based patient-specific QA for pencil beam scanning proton FLASH radiotherapy

Abstract:Background: Several studies have shown pencil beam scanning (PBS) proton therapy is a feasible and safe modality to deliver conformal and ultra-high dose rate (UHDR) FLASH radiation therapy. However, it would be challenging and burdensome to conduct the quality assurance (QA) of the dose rate along with conventional patient-specific QA (psQA). Purpose: To demonstrate a novel measurement-based psQA program for UHDR PBS proton transmission FLASH radiotherapy (FLASH-RT) using a high spatiotemporal resolution 2D strip ionization chamber array (SICA). Methods: The SICA is a newly designed open-air strip-segmented parallel plate ionization chamber, which is capable of measuring spot position and profile through 2 mm-spacing-strip electrodes at a 20 kHz sampling rate (50 μs per event) and has been characterized to exhibit excellent dose and dose rate linearity under UHDR conditions. A SICA-based delivery log was collected for each irradiation containing the measured position, size, dwell time, and delivered MU for each planned spot. Such spot-level information was compared with the corresponding quantities in the treatment planning system (TPS). The dose and dose rate distributions were reconstructed on patient CT using the measured SICA log and compared to the planned values in volume histograms and 3D gamma analysis. Furthermore, the 2D dose and dose rate measurements were compared with the TPS calculations of the same depth. In addition, simulations using different machine-delivery uncertainties were performed, and QA tolerances were deduced. Results: A transmission proton plan of 250 MeV for a lung lesion was planned and measured in a dedicated ProBeam research beamline (Varian Medical System) with a nozzle beam current between 100 to 215 nA. The worst gamma passing rates for dose and dose rate of the 2D SICA measurements (four fields) compared to TPS prediction (3%/3 mm criterion) were 96.6% and 98.8%, respectively, whereas the SICA-log reconstructed 3D dose distribution achieved a gamma passing rate of 99.1% (2%/2 mm criterion) compared to TPS. The deviations between SICA measured log, and TPS were within 0.3 ms for spot dwell time with a mean difference of 0.069 ± 0.11 s, within 0.2 mm for spot position with a mean difference of -0.016 ± 0.03 mm in the x-direction, and -0.036 ± 0.059 mm in the y-direction, and within 3% for delivered spot MUs. Volume histogram metric of dose (D95) and dose rate (V40Gy/s ) showed minimal differences, within less than 1%. Conclusions: This work is the first to describe and validate an all-in-one measurement-based psQA framework that can fulfill the goals of validating the dose rate accuracy in addition to dosimetric accuracy for proton PBS transmission FLASH-RT. The successful implementation of this novel QA program can provide future clinical practice with more confidence in the FLASH application.

Abstract:Background A dosimeter with high spatial and temporal resolution would be of significant interest for pencil beam scanning (PBS) proton beams' characterization, especially when facing small fields and beams with high temporal dynamics. Optical imaging of scintillators has potential in providing sub‐millimeter spatial resolution with pulse‐by‐pulse basis temporal resolution when the imaging system is capable of operating in synchrony with the beam‐producing accelerator. Purpose We demonstrate the feasibility of imaging PBS proton beams as they pass through a plastic scintillator detector to simultaneously obtain multiple beam parameters, including proton range, pencil beam's widths at different depths, spot's size, and spot's position on a pulse‐by‐pulse basis with sub‐millimeter resolution. Materials and methods A PBS synchrocyclotron was used for proton irradiation. A BC‐408 plastic scintillator block with 30 × 30 × 5 cm3 size, and another block with 30 × 30 × 0.5 cm3 size, positioned in an optically sealed housing, were used sequentially to measure the proton range, and spot size/location, respectively. A high‐speed complementary metal‐oxide‐semiconductor (CMOS) camera system synchronized with the accelerator's pulses through a gating module was used for imaging. Scintillation images, captured with the camera directly facing the 5‐cm‐thick scintillator, were corrected for background (BG), and ionization quenching of the scintillator to obtain the proton range. Spots' position and size were obtained from scintillation images of the 0.5‐cm‐thick scintillator when a 45° mirror was used to reflect the scintillation light toward the camera. Results Scintillation images with 0.16 mm/pixel resolution corresponding to all proton pulses were captured. Pulse‐by‐pulse analysis showed that variations of the range, spots' position, and size were within ± 0.2% standard deviation of their average values. The absolute ranges were within ± 1 mm of their expected values. The average spot‐positions were mostly within ± 0.8 mm and spots' sigma agreed within 0.2 mm of the expected values. Conclusion Scintillation‐imaging PBS beams with high‐spatiotemporal resolution is feasible and may help in efficient and cost‐effective acceptance testing and commissioning of existing and even emerging technologies such as FLASH, grid, mini‐beams, and so forth.

Seven-probe scintillator dosimeter for treatment verification in HDR-brachytherapy

Abstract:Purpose: We introduce a novel concept of a compact multiprobe scintillator detector and demonstrate its applicability in HDR-brachytherapy. Our fabricated seven-probe system is sufficiently narrow to be inserted in a brachytherapy needle or in a catheter. Methods: Our multiprobe detection system results from the parallel implementation of a miniaturized scintillator detector at the end of a bundle of seven fibers. The resulting system, which is narrower than 320 microns, is tested with a MicroSelectron 9.1 Ci Ir-192 HDR afterloader, in a water phantom. The detection signals from all seven probes are simultaneously read with an sCMOS camera (at a rate of 0.06 s). The camera is coupled to a chromatic filter to cancel Cerenkov signal induced within the fibers upon exposure. By implementing an aperiodic array of six scintillating cells along the bundle axis (one probe is kept bare to assess the stem effect), we first determine the range of inter-probe spacings leading to optimal source tracking accuracy. Then, three different source tracking algorithms involving sequentially or simultaneously all the scintillating probes are tested and compared. In each case, dwell positions are assessed from dose measurements and compared to the treatment plan. Dwell time is also determined and compared to the treatment plan. Results: The optimum inter-probe spacing for an accurate source tracking ranges from 15 mm to 35 mm. The optimum detection algorithm consists of adding the readout signals from all detector probes. In that case, the error to the planned dwell positions is of 0.01+/-0.14 mm and 0.02+/-0.29 mm at spacings between the source and detector axes of 5.5 and 40 mm, respectively. Using this approach, the average deviations to the expected dwell time are of -0.006+/-0.009 s and -0.008+/-0.058 s, at spacings between source and probe axes of 5.5 mm and 20 mm, respectively.

Abstract:Objective: To demonstrates the ability of an ultra-fast imaging system to measure high resolution spatial and temporal beam characteristics of a synchrocyclotron proton pencil beam scanning (PBS) system. Approach: An ultra-fast (1kHz frame rate), intensified CMOS camera was triggered by a scintillation sheet coupled to a remote trigger unit (RTU) for beam on detection. The camera was calibrated using the linear (R2>0.9922) dose response of a single spot beam to varying currents. Film taken for the single spot beam was used to produce a scintillation intensity to absolute dose calibration. Main results: Spatial alignment was confirmed with the film, where the x and y-profiles of the single spot cumulative image agreed within 1mm. A sample brain patient plan was analyzed to demonstrate dose and temporal accuracy for a clinically-relevant plan, through agreement within 1mm to the planned and delivered spot locations. The cumulative dose agreed with the planned dose with a gamma passing rate of 97.5% (2mm/3%, 10% dose threshold). Significance: This is the first system able to capture single-pulse spatial and temporal information for the unique pulse structure of a synchrocyclotron PBS systems at conventional dose rates, enabled by the ultra-fast sampling frame rate of this camera. This study indicates that, with continued camera development and testing, target applications in clinical and FLASH proton beam characterization and validation are possible.

Design and Characterization of a Novel Scintillator Array for In Vivo Monitoring During UHDR PBS Proton Therapy

Abstract:Background: Ultra-high dose rate proton therapy shows promise in tissue sparing by enhancing therapeutic ratio through the FLASH effect. In radiotherapy, accurate in vivo dosimetry is crucial for quality assurance, but remains challenging for UHDR as existing systems lack spatial and temporal resolution to verify dose and dose rate in complex anatomical regions, especially for PBS proton therapy. Purpose: To develop and evaluate a novel 3D surface dosimetry method for UHDR PBS proton therapy using high-speed imaging of a scintillator array for real-time, high-resolution surface dose monitoring. The spatial, temporal, and dosimetric components are validated via imaging of a QA phantom and comparison against TPS predictions. Methods: A deformable multi-element scintillator array was designed with 7.5mm element pitch and 0.5mm inter-element gap. Scintillation linearity with dose was evaluated with variation in response to increasing imaging and irradiation angles. WED testing evaluated beam attenuation at two energy levels. Scintillation emission was imaged at 1kHz and mesh position was monitored via 2-camera stereovision. System setup was validated using a 3D QA phantom to assess spatial accuracy and guide setup correction. Stereovision properties of array elements guided angular correction and geometric transformation. Kernel-based residual spot fitting derived cumulative dose maps compared to TPS dose profile of 5x5cm UHDR PBS delivery using 3%/2mm gamma analysis. PBS and maximum dose rate maps were calculated. Results: Setup achieved average localization error of 0.62 mm, surpassing typical 1+ mm clinical threshold. Intensity correction based on angular information yielded cumulative spot dose uncertainty of ~1% (5.428mGy). Processed dose map compared to TPS via gamma analysis showed 99.9% passing rate at 3%/2mm. WED of the array measured 1.1mm, minimizing impact on dose distribution.

Abstract:Purpose: To demonstrate a proton imaging system based on well-established fast scintillator technology to achieve high performance with low cost and complexity, with the potential of a straightforward translation into clinical use. Methods: The system tracks individual protons through one (X, Y) scintillating fiber tracker plane upstream and downstream of the object and into a 13 cm-thick scintillating block residual energy detector. The fibers in the tracker planes are multiplexed into silicon photomultipliers (SiPMs) to reduce the number of electronics channels. The light signal from the residual energy detector is collected by 16 photomultiplier tubes (PMTs). Only four signals from the PMTs are output from each event, which allows for fast signal readout. A robust calibration method of the PMT signal to residual energy has been developed to obtain accurate proton images. The development of patient-specific scan patterns using multiple input energies allows for an image to be produced with minimal excess dose delivered to the patient. Results: The calibration of signals in the energy detector produces accurate residual range measurements limited by intrinsic range straggling. We measured the water-equivalent thickness (WET) of a block of solid water (physical thickness of 6.10 mm) with a proton radiograph. The mean WET from all pixels in the block was 6.13 cm (SD 0.02 cm). The use of patient-specific scan patterns using multiple input energies enables imaging with a compact range detector. Conclusions: We have developed a prototype clinical proton radiography system for pretreatment imaging in proton radiation therapy. We have optimized the system for use with pencil beam scanning systems and have achieved a reduction of size and complexity compared to previous designs.

Water-benzene interactions: an effective fragment potential and correlated quantum chemistry study.

Abstract:Structures and binding in small water-benzene complexes (1-8 water molecules and 1-2 benzene molecules) are studied using the general effective fragment potential (EFP) method. The lowest energy conformers of the clusters were found using a Monte Carlo technique. The binding energies in the smallest clusters (dimers, trimers, and tetramers) were also evaluated with second order perturbation theory (MP2) and coupled cluster theory (CCSD(T)). The EFP method accurately predicts structures and binding energies in the water-benzene complexes. Benzene is polarizable and consequently participates in hydrogen bond networking of water. Since the water-benzene interactions are only slightly weaker than water-water interactions, structures with different numbers of water-water, benzene-water, and benzene-benzene bonds often have very similar binding energies. This is a challenge for computational methods.

Abstract:Ion-beam radiotherapy is an advanced cancer treatment modality offering steep dose gradients and a high biological effectiveness. These gradients make the therapy vulnerable to patient-setup and anatomical changes between treatment fractions, which may go unnoticed. Charged fragments from nuclear interactions of the ion beam with the patient tissue may carry information about the treatment quality. Currently, the fragments escape the patient undetected. Inter-fractional in-vivo treatment monitoring based on these charged nuclear fragments could make ion-beam therapy safer and more efficient. We developed an ion-beam monitoring system based on 28 hybrid silicon pixel detectors (Timepix3) to measure the distribution of fragment origins in three dimensions. The system design choices as well as the ion-beam monitoring performance measurements are presented in this manuscript. A spatial resolution of 4 mm along the beam axis was achieved for the measurement of individual fragment origins. Beam-range shifts of 1.5 mm were identified in a clinically realistic treatment scenario with an anthropomorphic head phantom. The monitoring system is currently being used in a prospective clinical trial at the Heidelberg Ion Beam Therapy Centre for head-and-neck as well as central nervous system cancer patients.

Abstract:Objective: In current clinical practice for quality assurance (QA), intensity modulated proton therapy (IMPT) fields are verified by measuring planar dose distributions at one or a few selected depths in a phantom. A QA device that measures full 3D dose distributions at high spatiotemporal resolution would be highly beneficial for existing as well as emerging proton therapy techniques such as FLASH radiotherapy. Our objective is to demonstrate feasibility of 3D dose measurement for IMPT fields using a dedicated multi-layer strip ionization chamber (MLSIC) device. Approach: Our developed MLSIC comprises a total of 66 layers of strip ion chamber (IC) plates arranged, alternatively, in the x and y direction. The first two layers each has 128 channels in 2 mm spacing, and the following 64 layers each has 32 channels in 8 mm spacing which are interconnected every nine channels. A total of 768-channel IC signals are integrated and sampled at a speed of 6 kfps. The MLSIC has a total of 19.2 cm water equivalent thickness and is capable of measurement over a 25 × 25 cm2 field size. A reconstruction algorithm is developed to reconstruct 3D dose distribution for each spot at all depths by considering a double-Gaussian-Cauchy-Lorentz model. The 3D dose distribution of each beam is obtained by summing all spots. The performance of our MLSIC is evaluated for a clinical pencil beam scanning (PBS) plan. Main results: The dose distributions for each proton spot can be successfully reconstructed from the ionization current measurement of the strip ICs at different depths, which can be further summed up to a 3D dose distribution for the beam. 3D Gamma Index analysis indicates excellent agreement between the measured and calculated dose distributions. Significance: The dedicated MLSIC is the first pseudo-3D QA device that can measure 3D dose distribution in PBS proton fields spot-by-spot.

Prompt Gamma Imaging with a Multi-Knife-edge Slit Collimator for Large FOV Monitoring of Scanned Proton Pencil Beams

Abstract:Proton therapy is superior to conventional radiation therapy due to dose deposition sharply increasing at Bragg peak. However, an uncertainty in Bragg peak position as large as 15 mm could occur due to a variety of uncertainties in proton therapy treatment delivery. One solution is to monitor the treatment during delivery by prompt gamma imaging. Pencil beam scanning mode is one of the widely adopted modes in proton therapy treatment In this mode, two sets of magnets scan the proton beams in a 2D pattern, and therefore the emitted gamma photons are focused to a small region, and the position of this region are rapidly changing with time. This is different with conventional nuclear medicine imaging, when the gamma source are distributed over the entire patient body, and rapidly changing with time. To meet the requirement of 40 cm × 40 cm × 30 cm FOV for proton pencil beam scanning mode, we propose a prompt gamma imaging system design with a multi-knife-edge slit collimator based on the optimization of a single-knife-edge slit collimator design. Monte Carlo simulation was performed to evaluate the positioning accuracy of Bragg peak and the spatial resolution of the imaging system. The prompt gamma distribution was generated via Monte Carlo simulation of proton interaction with water in GATE. The generated prompt gamma distribution was subsequently used to evaluate the system performance. For the multi-knife-edge slits collimator-based imaging system, preliminary evaluation results show that it can provide a large FOV size of 40×40×30 cm 3 , ~0.17% detection efficiency at FOV center, <;2mm special resolution and <;3 mm positioning accuracy along Bragg peak direction. Based on the preliminary results, we conclude that the proposed multi-knife-edge slits collimator-based prompt gamma imaging system design is very promising and could potentially facilitate precise proton therapy.
Imaging of prompt gamma rays emitted during delivery of clinical proton beams with a Compton camera: feasibility studies for range verification

Abstract:The purpose of this paper is to evaluate the ability of a prototype Compton camera (CC) to measure prompt gamma rays (PG) emitted during delivery of clinical proton pencil beams for prompt gamma imaging (PGI) as a means of providing in vivo verification of the delivered proton radiotherapy beams. A water phantom was irradiated with clinical 114 MeV and 150 MeV proton pencil beams. Up to 500 cGy of dose was delivered per irradiation using clinical beam currents. The prototype CC was placed 15 cm from the beam central axis and PGs from 0.2 MeV up to 6.5 MeV were measured during irradiation. From the measured data (2D) images of the PG emission were reconstructed. (1D) profiles were extracted from the PG images and compared to measured depth dose curves of the delivered proton pencil beams. The CC was able to measure PG emission during delivery of both 114 MeV and 150 MeV proton beams at clinical beam currents. 2D images of the PG emission were reconstructed for single 150 MeV proton pencil beams as well as for a 5 × 5 cm mono-energetic layer of 114 MeV pencil beams. Shifts in the Bragg peak (BP) range were detectable on the 2D images. 1D profiles extracted from the PG images show that the distal falloff of the PG emission profile lined up well with the distal BP falloff. Shifts as small as 3 mm in the beam range could be detected from the 1D PG profiles with an accuracy of 1.5 mm or better. However, with the current CC prototype, a dose of 400 cGy was required to acquire adequate PG signal for 2D PG image reconstruction. It was possible to measure PG interactions with our prototype CC during delivery of proton pencil beams at clinical dose rates. Images of the PG emission could be reconstructed and shifts in the BP range were detectable. Therefore PGI with a CC for in vivo range verification during proton treatment delivery is feasible. However, improvements in the prototype CC detection efficiency and reconstruction algorithms are necessary to make it a clinically viable PGI system.

Abstract:The Bragg peak phenomenon empowers proton therapy in targeted dose delivery in principle and also critically demands high accuracy of proton beam’s dispatch track in practice. To ensure the accuracy, prompt gamma imaging (PGI) systems were studied for monitoring the delivery of proton beams in real time. In this work, a dual-head PGI system was proposed and optimized with multislit collimators and BGO scintillators. The dual-head system was then assessed through a contrast recovery coefficient (CRC)-variance approach to identify a preferred setup. The performance of the PGI system was evaluated in terms of detection efficiency, spatial resolution, and positioning accuracy through a series of Monte Carlo simulations. Simulation results show that the detection efficiency of the system is 0.53% at the center of the FOV for 4.44 MeV prompt gamma, and the spatial resolution is better than 2 mm across the FOV. The positioning accuracy defined as the most distal edge position (MDEP) error is less than 2 mm along the beam incident direction at a relatively low number of detected events ( $5 \times 10^{4}$ ) in the dual-head system corresponding to a beam spot of $10 ^{8}$ incident protons. The system design is feasible for the real-time proton therapy monitoring.
Development and Performance Evaluation of a Prompt Gamma Imaging System for Real-time Proton Therapy Monitoring

Abstract:The advantage of proton therapy over conventional photon therapy is compromised by proton range uncertainty and dose monitoring is of critical importance to improve the precision of proton therapy. In this work, we report the design and performance evaluation of a prompt gamma imaging (PGI) system being developed in our lab, with high spatial resolution, high sensitivity and 2D imaging capability for real-time proton therapy monitoring. Key components of the PGI system include a multi-slit tungsten collimator, a scintillation detector array based on BGO block and SiPM, a customized ASIC-based readout electronics and a White Rabbit (WR) switch-based data acquisition electronics. Initial experimental evaluation of the detector module with the system electronics demonstrated the feasibility of the detector module for prompt gamma detection. Spatial resolution, sensitivity and Bragg peak positioning accuracy of the system was investigated with GATE simulations. The sensitivity measured at FOV center was 0.251%. ~2 mm spatial resolution was achieved for different FOV regions in both X and Y directions with slightly better spatial resolution in X direction. Bragg peak positioning accuracy was quantified by positioning bias and root mean square error (RMSE) of the most distal edge position (MDEP) of the reconstructed prompt gamma images and evaluated under different count levels. With the count level increasing from 10 4 to 10 6 , a sharp decrease in MDEP error from 2.29 mm to 0.76 mm was observed, indicating improved Bragg peak positioning accuracy under higher count levels and excellent Bragg peak positioning capability of the PGI system. To conclude, the designed PGI system is viable for proton therapy monitoring. Further system assembling and experimental performance evaluation in real proton therapy facility are ongoing.
Brachytherapy treatment verification using gamma radiation from the internal treatment source combined with an imaging panel-a phantom study

Abstract:Brachytherapy has an excellent clinical outcome for different treatment sites. However,in vivotreatment verification is not performed in the majority of hospitals due to the lack of proper monitoring systems. This study investigates the use of an imaging panel (IP) and the photons emitted by a high dose rate (HDR)192Ir source to track source motion and obtain some information related to the patient anatomy. The feasibility of this approach was studied by monitoring the treatment delivery to a 3D printed phantom that mimicks a prostate patient. A 3D printed phantom was designed with a template for needle insertion, a cavity ('rectum') to insert an ultrasound probe, and lateral cavities used to place tissue-equivalent materials. CT images were acquired to create HDR192Ir treatment plans with a range of dwell times, interdwell distances and needle arrangements. Treatment delivery was verified with an IP placed at several positions around the phantom using radiopaque markers on the outer surface to register acquired IP images with the planning CT. All dwell positions were identified using acquisition times ≤0.11 s (frame rates ≥ 9 fps). Interdwell distances and dwell positions (in relation to the IP) were verified with accuracy better than 0.1 cm. Radiopaque markers were visible in the acquired images and could be used for registration with CT images. Uncertainties for image registration (IP and planning CT) between 0.1 and 0.4 cm. The IP is sensitive to tissue-mimicking insert composition and showed phantom boundaries that could be used to improve treatment verification. The IP provided sufficient time and spatial resolution for real-time source tracking and allows for the registration of the planning CT and IP images. The results obtained in this study indicate that several treatment errors could be detected including swapped catheters, incorrect dwell times and dwell positions.

Abstract:Abstract Objectives To evaluate patient-specific quality assurance (PSQA) of 3 targets in a single delivery using a novel film-based phantom. Methods The phantom was designed to rotate freely as a sphere and could measure 3 targets with film in a single delivery. After identifying the coordinates of 3 targets in the skull, the rotation angles about the equator and meridian were computed for optimal phantom setup, ensuring the film plane intersected the 3 targets. The plans were delivered on the CyberKnife system using fiducial tracking. The irradiated films were scanned and processed. All films were analysed using 3 gamma criteria. Results Fifteen CyberKnife test plans with 3 different modalities were delivered on the phantom. Both automatic and marker-based registration methods were applied when registering the irradiated film and dose plane. Gamma analysis was performed using a 3%/1 mm, 2%/1 mm, and 1%/1 mm criteria with a 10% threshold. For the automatic registration method, the passing rates were 98.2% ± 1.9%, 94.2% ± 3.7%, and 80.9% ± 6.3%, respectively. For the marker-based registration approach, the passing rates were 96.4% ± 2.7%, 91.7% ± 4.3%, and 78.4% ± 6.2%, respectively. Conclusions A novel spherical phantom was evaluated for the CyberKnife system and achieved acceptable PSQA passing rates using TG218 recommendations. The phantom can measure true-composite dose and offers high-resolution results for PSQA, making it a valuable device for robotic radiosurgery. Advances in knowledge This is the first study on PSQA of 3 targets concurrently on the CyberKnife system.

Value and pitfalls of the lateral lung scan.

Abstract:Two hundred eighty-one of 443 lung scans composed of anterior, posterior, and lateral projections (done in our hospital) demonstrated defects. In 3.9 percent of them (11 cases), the defects were delineated in the lateral views only, while in 29.2 percent (82 cases), the lateral views either outlined additional defects not appreciated on the straight views, or showed more extensive lung involvement. In the majority of instances, 56.6 percent (159 cases), the lateral views showed comparable findings and also tended to segmentally localize the defects better. However, in 10.3 percent (29 cases), defects present on the straight projections were not detected on the lateral views. Various causes that could give rise to artefactural abnormalities in the lateral lung scan and therefore inhibit its proper interpretation, are reviewed and discussed. Despite these problems, the lateral may be the only view to demonstrate abnormalities and, in fact, frequently provides additional useful information.

Abstract:The accurate measurement of three-dimensional (3D) dose distribution of carbon-ion pencil beams in water is crucial for regular quality assurance (QA) practice. Although ionization chambers scanned in a water tank is conventionally used for this purpose, these measuring methods are time-consuming and labor-intensive. In addition, the beam of Heavy Ion Medical Machine (HIMM) changes continuously in each extracted duration, these methods cannot obtain the accurate 3D dose distribution. To solve these problems, an integrated system (PBDOSE) is conceived and established, which employs multi-strip ionization chambers (MSICs) to measure 2D lateral profile dose distributions. The lateral profile dose distribution in the context of various depths is measured by a mobile MSIC and normalized by the measurement of a reference MSIC to reduce the effect of beam instability. On this basis, a 3D dose distribution is created by stacking those profiles in depth direction. By conducting a single depth scan within 3 min at HIMM, information such as the 3D dose distribution, lateral contour images, the Bragg curve, beam spot size, beam position, incident direction, and energy of the beam can be obtained. The Bragg curve acquired from PBDOSE with statistical uncertainties of 1.5% was almost identical to those measured by a commercial water phantom. Besides that, the lateral profiles revealed a remarkable agreement with Gafchromic EBT3 film measurements within differences less than 0.1 mm. PBDOSE shows great prospects in the accurate and efficient measurement of 3D dose distribution for carbon-ion therapy, and the clinical application of this system will improve the efficiency of regular QA procedures to a great extent.

Abstract:In proton-based radiotherapy, proton radiography could allow for direct measurement of the water-equivalent path length (WEPL) in tissue, which can then be used to determine relative stopping power (RSP). Additionally, proton radiographs allow for imaging in the beam's-eye-view. In this work, a proton radiography technique using a flat-panel imager and a pencil-beam scanning (PBS) system is demonstrated in phantom. Proton PBS plans were delivered on a Varian ProBeam system to a flat-panel imager. Each proton plan consisted of energy layers separated by 4.8 MeV, and a field size of 25 cm × 25 cm. All measured data is binned into a layer-by-layer delivery in post processing. To build a calibration curve correlating detector response to WEPL, the plans were delivered to slabs of solid water of increasing thickness. Pixel-by-pixel detector response in the time/energy domain is then fit as a function of WEPL. Tissue equivalent phantoms are imaged for evaluation of WEPL accuracy. A spatial resolution phantom and a head phantom are also imaged. For all experiments, the detector was run with an effective pixel size of 0.4 mm × 0.4 mm. The proposed method reconstructed RSP with mean errors of 2.65%, -0.14%, and 0.61% for lung, soft tissue, and bone, respectively. In a 40 mm thick spatial resolution phantom, a 2 mm deep pinhole with 1 mm diameter can be seen. The accuracy and spatial resolution of the method show that it could be implemented for patient position verification. Further development could lead to patient-specific verification of RSP to be used for treatment guidance.
